RESUMO

OBJECTIVES: To compare the delineation of mandibular cancer by 3D T1 turbo field echo with compressed SENSE (CS-3D-T1TFE) images and MDCT images, and to compare both sets of images with histopathological findings, as the gold standard, to validate the accuracy and clinical usefulness of CS-3D-T1TFE reconstruction. METHODS: Twenty-four patients with mandibular squamous cell carcinoma (SCC) who underwent MRI including CS-3D-T1TFE and MDCT examinations before surgery were retrospectively included. For both examinations, 0.5-mm-thick coronal plane images and 0.5-mm-thick plane images perpendicular and parallel to the dentition were constructed. Two radiologists rated bone invasion in three categories indexed by cortical bone, cancellous bone, and mandibular canal (MC), and inter-rater agreement was assessed by weighted kappa statistics. In 20 of the 24 patients who underwent surgery, the correlation of bone invasion with the histopathological evaluation by pathologists was assessed using Pearson's correlation coefficient. Soft-tissue invasion was assessed by diagnosing the presence of invasion into the mylohyoid muscle, gingivobuccal fold, and masticator space, and inter-rater agreement was assessed by kappa statistics. RESULTS: The interobserver agreement for bone invasion assessment was almost perfect with CS-3D-T1TFE and substantial with MDCT. The image evaluations by both observers agreed with the pathological evaluations in 15 of the 20 cases, showing high correlation (r > 0.8). CS-3D-T1TFE also showed higher inter-rater agreement than MDCT for all measures of soft-tissue invasion. CONCLUSIONS: CS-3D-T1TFE reconstructed images were clinically useful in accurately depicting the extent of mandibular cancer invasion and potentially solving the problem of lesion overestimation associated with conventional MRI. KEY POINTS: • Reconstructed CS-3D-T1TFE images were useful for the diagnosis of mandibular cancer. • CS-3D-T1TFE images showed higher inter-rater agreement than MDCT and high correlation with pathological findings. • CS-3D-T1TFE images may solve the problem of overestimation of the tumor extent, which has been associated with MRI in the past.

OBJECTIVE: Though the motions of structures outside the mouth in trumpet performance have been reported, the dynamics of intraoral structures remain unelucidated. This study explored the tongue's movement in trumpet playing using cine magnetic resonance imaging (cine MRI) and demonstrated the effects of intraoral anatomical structures on changes in pitch and dynamics. METHODS: Cine MRI was applied to 18 trumpet players, who were divided into two groups (7 beginner, 11 advanced) based on their ability to play a certain high note. They were instructed to play a custom-made MRI-compatible simulated trumpet. Pitch-change tasks and dynamics-change tasks were assigned. The positions of the anatomical points and intraoral areas were identified on outlined images, and the changes associated with each task were evaluated. RESULTS: A forward and upward projection of the tongue was observed in the production of higher pitches, and there were no significant differences in all areas. In louder dynamics, a backward and downward bending of the tongue occurred, the tongue area became smaller (p<0.001), and the oral cavity area became larger (p<0.001). No significant differences between beginner and advanced trumpet players were seen in the changes in pitch and dynamics. CONCLUSION: It was demonstrated using cine MRI that certain tongue movements were associated with each task. Tongue protrusion in the production of higher pitch and bending in louder dynamics can be rationalized using acoustics theory and the movements of anatomical structures. These findings seem to be consistent regardless of the player's proficiency.

Background: Diffusion-weighted imaging (DWI) and dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) are widely used in the orofacial region. Furthermore, quantitative analyses have proven useful. However, a few reports have described the correlation between DWI-derived parameters and DCE-MRI-derived parameters, and the results have been controversial. Purpose: To evaluate the correlation among parameters obtained by DWI and DCE-MRI and to compare them between benign and malignant lesions. Material and Methods: Fifty orofacial lesions were analysed. The apparent diffusion coefficient (ADC), true diffusion coefficient (D), pseudodiffusion coefficient (D*) and perfusion fraction (f) were estimated by DWI. For DCE-MRI, TK model analysis was performed to estimate physiological parameters, for example, the influx forward volume transfer constant into the extracellular-extravascular space (EES) (Ktrans) and fractional volumes of EES and plasma components (ve and vp). Results: Both ADC and D showed a moderate positive correlation with ve (ρ = 0.640 and 0.645, respectively). Ktrans showed a marginally weak correlation with f (ρ = 0.296), while vp was not correlated with f or D*; therefore, IVIM perfusion-related parameters and TK model perfusion-related parameters were not straightforward. Both D and ve yielded high diagnostic power between benign lesions and malignant tumours with areas under the curve (AUCs) of 0.830 and 0.782, respectively. Conclusion: Both D and ve were reliable parameters that were useful for the differential diagnosis. In addition, the true diffusion coefficient (D) was affected by the fractional volume of EES.

Odontogenic tumors (OGTs), which originate from cells of odontogenic apparatus and their remnants, are rare entities. Primary intraosseous carcinoma NOS (PIOC), is one of the OGTs, but it is even rarer and has a worse prognosis. The precise characteristics of PIOC, especially in immunohistochemical features and its pathogenesis, remain unclear. We characterized a case of PIOC arising from the left mandible, in which histopathological findings showed a transition from the odontogenic keratocyst to the carcinoma. Remarkably, the tumor lesion of this PIOC prominently exhibits malignant attributes, including invasive growth of carcinoma cell infiltration into the bone tissue, an elevated Ki-67 index, and lower signal for CK13 and higher signal for CK17 compared with the non-tumor region, histopathologically and immunohistopathologically. Further immunohistochemical analyses demonstrated increased expression of ADP-ribosylation factor (ARF)-like 4c (ARL4C) (accompanying expression of ß-catenin in the nucleus) and yes-associated protein (YAP) in the tumor lesion. On the other hand, YAP was expressed and the expression of ARL4C was hardly detected in the non-tumor region. In addition, quantitative RT-PCR analysis using RNAs and dot blot analysis using genomic DNA showed the activation of Wnt/ß-catenin signaling and epigenetic alterations, such as an increase of 5mC levels and a decrease of 5hmC levels, in the tumor lesion. A DNA microarray and a gene set enrichment analysis demonstrated that various types of intracellular signaling would be activated and several kinds of cellular functions would be altered in the pathogenesis of PIOC. Experiments with the GSK-3 inhibitor revealed that ß-catenin pathway increased not only mRNA levels of ankyrin repeat domain1 (ANKRD1) but also protein levels of YAP and transcriptional co-activator with PDZ-binding motif (TAZ) in oral squamous cell carcinoma cell lines. These results suggested that further activation of YAP signaling by Wnt/ß-catenin signaling may be associated with the pathogenesis of PIOC deriving from odontogenic keratocyst in which YAP signaling is activated.

The primary taste cortex is located in the insula. However, exactly where in the insula the human primary taste cortex is located remains a controversial issue. Human neuroimaging studies have shown prominent variation concerning the location of taste-responsive activation within the insula. A standard protocol for gustatory testing in neuroimaging studies has not been developed, which might underlie such variations. In order to localize the primary taste cortex in an fMRI experiment, we used a taste delivery system to suppress non-taste stimuli and psychological effects. Then, we compared brain response to taste solution during a passive tasting task condition and a taste quality identification task condition to verify whether this cognitive task affected the location of taste-responsive activation within the insula. To examine which part of insula is the primary taste area, we performed dynamic causal modeling (DCM) to verify the neural network of the taste coding-related region and random-effects Bayesian model selection (BMS) at the family level to reveal the optimal input region. Passive tasting resulted in activation of the right middle insula (MI), and the most favorable model selected by DCM analysis showed that taste effect directly influenced the MI. Additionally, BMS results at the family level suggested that the taste inputs entered into the MI. Taken together, our results suggest that the human primary taste cortex is located in the MI.

OBJECTIVES: This study conducted a receiver operating characteristic (ROC) analysis by applying improved cluster signal-to-noise (CSN) analysis to digital intraoral radiographs and develop an observer-free method of analyzing image quality related to the observer performance in the detection task. METHODS: Two aluminum step phantoms with a thickness interval of 1.0 mm were used for this study. One phantom had holes of increasing depth (from 0.05 to 0.35 mm) and the other had no holes. Phantom images were obtained under various exposure dose and image capture modes using a dental X-ray unit, a photostimulable phosphor imaging plate, and scanner system. These phantom images were analyzed using the FindFoci plugin in ImageJ software. Subsequently, true positive rates (TPRs) and false positive rates (FPRs) were calculated by analyzing phantom images with and without holes. We constructed ROC curves by plotting the TPRs against the FPRs and calculated the area under the ROC curve (AUC). Using the same phantom images with holes, eight observers assessed the number of detectable holes. Correlations between observer detection performance and AUC values were evaluated. RESULTS: AUC values increased as the exposure dose increased and showed different tendencies depending on the image capture mode. The AUC values showed a high correlation with observer detection performance (r = 0.76). CONCLUSIONS: AUC values obtained from CSN analysis reflect image quality and replace the observer detection performance test of image quality.

OBJECTIVES: The objective of this study was to analyze the relationship between maxillomandibular characteristics and the severity of temporomandibular disc displacement in female patients with a skeletal class III (SKIII) pattern. METHODS: Fifty-seven samples were included in the study. The evaluation of articular disc conditions was conducted using magnetic resonance imaging, while 25 cephalometric variables from lateral and postero-anterior (P-A) cephalograms were measured to determine their maxillomandibular characteristics. The samples were categorized into three groups based on the articular disc conditions: (1) normal disc position (NDP), (2) disc displacement with reduction (DDwR), and (3) disc displacement without reduction (DDwoR). The relationship between the maxillomandibular characteristics and disc conditions was examined through both basic statistical analysis and multivariate analysis using principal component analysis (PCA). RESULTS: The Kruskal-Wallis and Dunn-Bonferroni tests revealed a significant difference between the groups in terms of the deviation of mandibular characteristics observed on the P-A cephalogram. The DDwoR group exhibited significantly larger menton deviation, ramal height asymmetry index, and total mandibular length asymmetry index compared to the NDP and DDwR groups. Moreover, the PCA successfully extracted all cephalometric variables into eight principal components. Among them, only the principal component related to mandibular asymmetry was able to differentiate the SKIII samples with DDwoR from the other groups. CONCLUSIONS: The findings of this study highlight a significant relationship between mandibular asymmetry and the severity of disc displacement, particularly DDwoR, in female patients with a SKIII pattern.

PURPOSE: To evaluate whether a pharmacokinetic analysis is useful for monitoring the response of oral cancer to chemoradiotherapy (CRT). MATERIALS AND METHODS: Twenty-nine patients were included. They underwent dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) before and after CRT. The DCE-MRI data were analyzed using a Tofts and Kermode (TK) model. The histological evaluation of the effects of CRT was performed according to Ohboshi and Shimosato's classification. RESULTS: None of the pre-CRT parameters were significantly different between the responders and nonresponders. The post-CRT volume of the extravascular extracellular space (EES) per unit volume of tissue (v(e) ) of responders (0.397 ± 0.080) was higher than that of nonresponders (0.281 ± 0.076) (P = 0.01). The change of the v(e) between the pre- and post-CRT of the responders (0.154 ± 0.093) was larger than that of the nonresponders (0.033 ± 0.073) (P = 0.001). Therefore, the increase in the v(e) strongly suggested a good tumor response to CRT, which reflected an increase of the EES secondary to the destruction of the cancer nest. The changes in the volume transfer constant (K(trans) ) were significantly different between the responders and nonresponders (P = 0.018). CONCLUSION: Both the increase of the v(e) and the elevation of permeability (K(trans) ) were indicative of a good tumor response to CRT. The pharmacokinetic analysis had potential for monitoring the histopathological response to CRT.

BACKGROUND: Diffusion-weighted imaging (DWI) and diffusion-tensor imaging (DTI) can be used to evaluate changes that accompany skeletal muscle contraction. PURPOSE: To investigate whether jaw opening or closure affect the diffusion parameters of the masseter muscles (MMs). MATERIAL AND METHODS: Eleven healthy volunteers were evaluated. Diffusion-tensor images were acquired to obtain the primary (λ(1)), secondary (λ(2)), and tertiary eigenvalues (λ(3)). We estimated these parameters at three different locations: at the level of the mandibular notch for the superior site, the level of the mandibular foramen for the middle site, and the root apex of the mandibular molars for the inferior site. RESULTS: Both λ(2) and λ(3) during jaw opening were significantly lower than that at rest at the superior (P = 0.006, P < 0.0001, respectively) and middle site (P = 0.004, P = 0.0001, respectively); however, the change in λ(1) was not significant. At the lower site, no parameter was significantly different at rest and during jaw opening. There was no significant difference in T2 between at rest (40.3 ± 4.4 ms) and during jaw opening (39.2 ± 2.7 ms; P = 0.12). The changes induced by jaw closure were marked at the inferior site. In the middle and inferior sites, the three eigenvalues were increased by jaw closure, and the changes in λ(1) (P = 0.0145, P = 0.0107, respectively) and λ(2) (P = 0.0003, P = 0.0001) were significant (especially λ(2)). CONCLUSION: The eigenvalues for diffusion of the MM were sensitive to jaw position. The recruitment of muscle fibers, specific to jaw position, reflects the differences in changes in muscle diffusion parameters.

OBJECTIVES: This study aimed to quantitatively examine the effect of digital image processing of digital intraoral radiographic images on the resolution characteristics of the output image using a task transfer function (TTF). METHODS: A photostimulable phosphor system with three types of image processing filters, including periodontal, endodontic, and dentine-to-enamel junction filters, was used. Each filter can be used in conjunction with the sharpness filter (+ S). Images were obtained from the original phantom, which combined aluminum disk and plate. The TTF, which indicates the resolution characteristics, was calculated. A one-dimensional profile curve was also measured, and the fluctuation in the pixel value was evaluated in detail. The results were compared to investigate the effects of digital image processing on digital intraoral radiographic images. RESULTS: The TTF values were specific to each filter. The change in the TTF strongly reflected the characteristics of the one-dimensional profile curve. The TTF was compared with a one-dimensional profile curve and was able to quantitatively express the resolution characteristics of all directions in the image. CONCLUSIONS: We attempted to evaluate the resolution characteristics of digital intraoral radiographic images with image processing filters using the TTF. The effect of each image processing filter and the + S filter on the resolution can be simply expressed using the TTF. Our results show that the TTF is useful for characterizing the resolution characteristics of image processing filters for image quality.

OBJECTIVES: To compare the gamma distribution (GD), intravoxel incoherent motion (IVIM), and monoexponential (ME) models in terms of their goodness-of-fit, correlations among the parameters, and the effectiveness in the differential diagnosis of various orofacial lesions. METHODS: A total of 85 patients underwent turbo spin-echo diffusion-weighted imaging with six b-values. The goodness-of-fit of three models was assessed using Akaike Information Criterion. We analysed the correlations and compared the effectiveness in the differential diagnosis among the parameters of GD model (κ, shape parameter; θ, scale parameter; fractions of diffusion: ƒ1, cellular component; ƒ2, extracellular diffusion; ƒ3, perfusion component), IVIM model (D, true diffusion coefficient; D*, pseudodiffusion coefficient; f, perfusion fraction), and ME model (apparent diffusion coefficient, ADC). RESULTS: The GD and IVIM models showed a better goodness-of-fit than the ME model (p < 0.05). ƒ1 had strong negative correlations with D and ADC (ρ = -0.901 and -0.937, respectively), while ƒ3 had a moderate positive correlation with f (ρ = 0.661). Malignant entity presented significantly higher ƒ1 and lower D and ADC than benign entity (p < 0.0001). Malignant lymphoma had significantly higher ƒ1 in comparison to squamous cell carcinoma (p = 0.0007) and granulation (p = 0.0075). The trend in ƒ1 was opposite to the trend in D. Malignant lymphoma had significant lower ƒ3 than squamous cell carcinoma (p = 0.005) or granulation (p = 0.0075). CONCLUSIONS: The strong correlations were found between the GD- and IVIM-derived parameters. Furthermore, the GD model's parameters were useful for characterising the pathological structure in orofacial lesions.

Mixed tumor of the skin (MTS) is a rare neoplasm derived from the sweat glands with a reported frequency of 0.01-0.098% among all primary skin tumors. MTS often occurs in the head and neck region and is characterized by a mixture of epithelial, myoepithelial and stromal components. MTS also shows various morphological patterns, thus the presence of variants with rare components and its rarity make the clinical diagnosis even more difficult. A 47-year-old man was referred due to a painless, slowly growing, exophytic swelling intracutaneous mass of the upper lip. Magnetic resonance imaging revealed that the mass was a solid tumor with a fatty component in the proximal portion, while the distal portion was cystic and possibly contained highly viscous fluid. The mass was located between the skin and the orbicularis oris muscle in the upper lip. Excisional biopsy was performed and the lesion showed two intriguing features: A tumor with extensive lipomatous stroma and some large cysts. It was histopathologically diagnosed as lipomatous MTS with cystic formation in the upper lip. No evident signs of recurrence were observed during follow-up. The present report describes this case and includes a brief literature review of reported cases in the lip, since MTS can be confused with various skin lesions in clinical settings due to this rarity. Recognition by clinicians of different variants of MTSs, including the present case, is important for preventing erroneous diagnosis and treatment.

OBJECTIVES: This study aimed to investigate the impact of a deep learning-based reconstruction (DLR) technique on image quality and reduction of radiation exposure, and to propose a low-dose multidetector-row computed tomography (MDCT) scan protocol for preoperative imaging for dental implant surgery. METHODS: The PB-1 phantom and a Catphan phantom 600 were scanned using volumetric scanning with a 320-row MDCT scanner. All scans were performed with a tube voltage of 120 kV, and the tube current varied from 120 to 60 to 40 to 30 mA. Images of the mandible were reconstructed using DLR. Additionally, images acquired with the 120-mA protocol were reconstructed using filtered back projection as a reference. Two observers independently graded the image quality of the mandible images using a 4-point scale (4, superior to reference; 1, unacceptable). The system performance function (SPF) was calculated to comprehensively evaluate image quality. The Wilcoxon signed-rank test was employed for statistical analysis, with statistical significance set at p value < 0.05. RESULTS: There was no significant difference between the image quality acquired with the 40-mA tube current and reconstructed with the DLR technique (40DLR), and that acquired with the reference protocol (3.00, 3.00, p = 1.00). The SPF at 1.0 cycles/mm acquired with 40DLR was improved by 156.7% compared to that acquired with the reference protocol. CONCLUSIONS: Our proposed protocol, which achieves a two-thirds reduction in radiation dose, can provide a minimally invasive MDCT scan of acceptable image quality for dental implant surgery.

OBJECTIVES: To evaluate whether a pharmacokinetic analysis is useful for both predicting and monitoring the response to chemoradiotherapy (CRT) in oral cancer. METHODS: Patients with oral squamous cell carcinoma treated with preoperative CRT and surgery were enrolled. They underwent dynamic contrast-enhanced MRI before (n = 23), and after CRT (n = 20). We estimated four parameters: arrival time of contrast medium (TA), exchange rate constant from the extracellular extravascular space (EES) to plasma (k(ep)), elimination of contrast medium from the central compartment (k(el)) and an amplitude scaling constant (AH) using the Brix model. The histological evaluation of the effects of CRT was performed according to Ohboshi and Shimosato's classification. We analysed the correlation between the parameters and the histological evaluation. RESULTS: The pre-CRT AH between the responders and non-responders was significantly different (P = 0.046), however, the three parameters (TA, K(ep), K(el)) were not significantly different among the groups (P = 0.76, P = 0.60, P = 0.09). As AH decreased, the tumour response improved. The change in the AH between the pre- and post-CRT of responders was significantly higher than that of non-responders (P = 0.043). CONCLUSION: The AH, which is affected by the ratio of the EES, was an important parameter for predicting and monitoring the tumour response to CRT.

PURPOSE: The directional dependence of accuracy with cone beam computed tomography (CBCT) has not been investigated thoroughly. The purpose of the present study was to clarify the effects of measurement direction and of phantom locations in the fields of view (FOVs) on the accuracy of linear measurement and on the limits of measuring thin objects with CBCT. MATERIALS AND METHODS: An aluminum phantom was scanned by CBCT. The thickness in both the longitudinal and horizontal directions (LD and HD) was measured at both the center and periphery in the three FOVs. The length was determined by a 50% relative threshold method to eliminate observer-dependent measurement errors. The measurement limits of a thin object were assessed in thin parts of the phantom (thickness, 0.3 to 1.0 mm). RESULTS: The measurement accuracy in the LD was excellent (within 1 pixel), while that in the HD was fair (0 to 2.35 pixels difference); a slight overestimation was found near the center of the phantom (1.06 to 2.35 pixels difference) in comparison to that of the edges (within 1 pixel difference). The overestimation was more marked at the longitudinal periphery than at the center of each FOV. A thickness of at least three to four pixels was necessary to keep errors within the one-pixel range along both directions. CONCLUSIONS: The accuracy of linear measurement with CBCT is excellent, especially when measuring in the LD. Distances are slightly overestimated in the HD near the center of the phantom. Close attention is therefore necessary when planning the placement of implants adjacent to horizontal vital structures. Second, a thickness of at least 3 to 4 pixels was necessary to maintain high accuracy for linear measurement. This must be taken into consideration when measuring a minute structure, such as thin cortical bone.

This study aims to quantitatively evaluate the effect of additional copper-filters (Cu-filters) on the radiation dose and contrast-to-noise ratio (CNR) in a dental cone beam computed tomography (CBCT). The Cu-filter thickness and tube voltage of the CBCT unit were varied in the range of 0.00-0.20 mm and 70-90 kV, respectively. The CBCT images of a phantom with homogeneous materials of aluminum, air, and bone equivalent material (BEM) were acquired. The CNRs were calculated from the voxel values of each homogeneous material. The CTDIvol was measured using standard polymethyl methacrylate CTDI test objects. We evaluated and analyzed the effects of tube current and various radiation qualities on the CNRs and CTDIvol. We observed a tendency for higher CNR at increasing tube voltage and tube current in all the homogeneous materials. On the other hand, the CNR reduced at increasing Cu-filter thickness. The tube voltage of 90 kV showed a clear advantage in the tube current-CNR curves in all the homogeneous materials. The CTDIvol increased as the tube voltage and tube current increased and decreased with the increase in the Cu-filter thickness. When the CNR was fixed at 9.23 of BEM at an exposure setting of 90 kV/5 mA without a Cu-filter, the CTDIvol at 90 kV with Cu-filters was 8.7% lower compared with that at 90 kV without a Cu-filter. The results from this study demonstrate the potential of adding a Cu-filter for patient dose reduction while ensuring the image quality.

PURPOSE: This study investigated the effects of 1 year of training on imaging diagnosis, using static ultrasonography (US) salivary gland images of Sjögren syndrome patients. MATERIALS AND METHODS: This study involved 3 inexperienced radiologists with different levels of experience, who received training 1 or 2 days a week under the supervision of experienced radiologists. The training program included collecting patient histories and performing physical and imaging examinations for various maxillofacial diseases. The 3 radiologists (observers A, B, and C) evaluated 400 static US images of salivary glands twice at a 1-year interval. To compare their performance, 2 experienced radiologists evaluated the same images. Diagnostic performance was compared between the 2 evaluations using the area under the receiver operating characteristic curve (AUC). RESULTS: Observer A, who was participating in the training program for the second year, exhibited no significant difference in AUC between the first and second evaluations, with results consistently comparable to those of experienced radiologists. After 1 year of training, observer B showed significantly higher AUCs than before training. The diagnostic performance of observer B reached the level of experienced radiologists for parotid gland assessment, but differed for submandibular gland assessment. For observer C, who did not complete the training, there was no significant difference in the AUC between the first and second evaluations, both of which showed significant differences from those of the experienced radiologists. CONCLUSION: These preliminary results suggest that the training program effectively helped inexperienced radiologists reach the level of experienced radiologists for US examinations.

Purpose: The objective of this study was to investigate the effects of (i) the difference in the beam qualities on the presampled modulation transfer function (MTF) using the edge method and (ii) the readout direction of the edge profile for the photostimulable phosphor (PSP) system. Approach: The International Electrotechnical Commission (IEC) defined a technique using the "radiation qualities based on a phantom made up of an aluminum added filter" (RQA). A general radiographic system with a tube voltage of 50 kV and 9.7 mm of additional aluminum filtration was used to conform the x-ray to the IEC-specified beam quality definition RQA3. Additionally, we employed two different beams with tube voltages of 60 and 70 kV using a dental x-ray unit. The MTF was measured in the readout direction from low-to-high exposure regions and vice versa with respect to the scanning and subscanning directions of the PSP system. Results: The difference in the 50%MTF value for all directions between 60 and 70 kV averaged less than 0.05 . The 50%MTF of RQA3 was on average 0.2 lower than the value for 60 kV for each direction. For all beam qualities, no difference was observed between the MTFs measured in the readout direction from the low-to-high exposure regions and vice versa. Conclusions: The MTFs, measured using the dental x-ray unit, were unaffected by the tube voltage, and they were slightly higher than those measured using the RQA3. Furthermore, the MTF was unaffected by the differences in the readout directions of the edge profile.

OBJECTIVES: This study aimed to improve the impact of the metal artefact reduction (MAR) algorithm for the oral cavity by assessing the effect of acquisition and reconstruction parameters on an ultra-high-resolution CT (UHRCT) scanner. METHODS: The mandible tooth phantom with and without the lesion was scanned using super-high-resolution, high-resolution (HR), and normal-resolution (NR) modes. Images were reconstructed with deep learning-based reconstruction (DLR) and hybrid iterative reconstruction (HIR) using the MAR algorithm. Two dental radiologists independently graded the degree of metal artefact (1, very severe; 5, minimum) and lesion shape reproducibility (1, slight; 5, almost perfect). The signal-to-artefact ratio (SAR), accuracy of the CT number of the lesion, and image noise were calculated quantitatively. The Tukey-Kramer method with a p-value of less than 0.05 was used to determine statistical significance. RESULTS: The HRDLR visual score was better than the NRHIR score in terms of degree of metal artefact (4.6 ± 0.5 and 2.6 ± 0.5, p < 0.0001) and lesion shape reproducibility (4.5 ± 0.5 and 2.9 ± 1.1, p = 0.0005). The SAR of HRDLR was significantly better than that of NRHIR (4.9 ± 0.4 and 2.1 ± 0.2, p < 0.0001), and the absolute percentage error of the CT number in HRDLR was lower than that in NRHIR (0.8% in HRDLR and 23.8% in NRIR). The image noise of HRDLR was lower than that of NRHIR (15.7 ± 1.4 and 51.6 ± 15.3, p < 0.0001). CONCLUSIONS: Our study demonstrated that the combination of HR mode and DLR in UHRCT scanner improved the impact of the MAR algorithm in the oral cavity.

OBJECTIVES: This study evaluated the correlation among the diffusion-derived parameters obtained by monoexponential (ME), intravoxel incoherent motion (IVIM) and γ distribution (GD) models and compared these parameters among representative orofacial tumours. METHODS: Ninety-two patients who underwent 1.5 T MRI including diffusion-weighted imaging were included. The shape parameter (κ), scale parameter (θ), ratio of the intracellular diffusion (ƒ1), extracellular diffusion (ƒ2) and perfusion (ƒ3) were obtained by the GD model; the true diffusion coefficient (D) and perfusion fraction (f) were obtained by the IVIM model; and the apparent diffusion coefficient (ADC) was obtained by the ME model. RESULTS: ƒ1 had a strongly negative correlation with the ADC (ρ = -0.993) and D (ρ = -0.926). A strong positive correlation between f and ƒ3 (ρ = 0.709) was found. Malignant lymphoma (ML) had the highest ƒ1, followed by squamous cell carcinoma (SCC), malignant salivary gland tumours, pleomorphic adenoma (Pleo) and angioma. Both the IVIM and GD models suggested the highest perfusion in angioma and the lowest perfusion in ML. The GD model demonstrated a high extracellular component in Pleo and revealed that the T4a+T4b SCC group had a lower ƒ2 than the T2+T3 SCC group, and poor to moderately differentiated SCC had a higher ƒ1 than highly differentiated SCC. CONCLUSIONS: Given the correlation among the diffusion-derived parameters, the GD model might be a good alternative to the IVIM model. Furthermore, the GD model's parameters were useful for characterizing the pathological structure.
