RESUMEN

Social media represents a powerful tool for disseminating verified health information on topics such as colostomy, and the roles of healthcare professionals and institutions to ensure the veracity of the information conveyed is increasingly relevant. The main objectives of this study were to analyze the roles of these healthcare professionals and institutions in the conversation about colostomy, without being framed in a specific health communication campaign, and to know the use of reliable information in the conversation. The study was carried out by analyzing Twitter messages containing the hashtag "colostomy" and "Chron" between the 1 January and the 30 April 2022. It was conducted using the NodeXL software, focusing on content analysis of tweets and users' accounts. The results show that accounts with healthcare activity influence the impressions generated on the network (p = 0.018), finding that nurses are the most active healthcare professionals (22.24%) also having a significant effect on the overall network interactions (p = 0.022). In contrast, we found that institutions do not actively participate on the network. We emphasize the responsibility of institutions for health education and the need for professionals to improve communication skills on social networks, but also the need to improve communication skills on social media to support public health campaigns through these increasingly important channels.

RESUMEN

A comprehensive, interdisciplinary wound care team is of great importance to the management of acute, chronic, and recurrent wounds. This management functions best for the patient when all members of the team are in regular discussion regarding the wound care plan, providing more efficient and timelier patient-centered care. This article reviews the roles of different disciplines in the management of wounds. These disciplines include rehabilitation physicians, wound care nurses, registered nurses and certified nursing assistants, surgical teams, specialists of infectious diseases and mental health, dieticians, and patients and caregivers. A case study is also provided.

RESUMEN Antecedentes: la cirugía de restitución del tránsito intestinal presenta complicaciones posoperatorias tales como infección de sitio quirúrgico, asociado a percepción de pobre resultado cosmético por parte de los pacientes. Objetivo: describir la técnica quirúrgica de incisión y cierre de piel en "punto de mira" para la reversión de estoma y los resultados posoperatorios. Material y métodos: entre noviembre de 2020 y mayo de 2021 se realizó esta técnica a 15 pacientes con estatus de colostomía e ileostomía. Se analizaron los resultados transoperatorios y posoperatorios. Resultados: edad promedio: 38±2,5 años, índice de masa corporal: 28±1,5 kg/m2, tiempo de estadía hospitalaria: 4±2 días. La técnica presentó buena exposición de tejidos y ningún paciente presentó infección de sitio quirúrgico. A los 30 días del alta, el 100% de los pacientes expresaron alta satisfacción por el resultado estético evidenciado mediante la escala Likert 3. Conclusión: la utilización de este procedimiento ofrece ventajas técnicas, y alta satisfacción de los pacientes.


ABSTRACT Background: Stoma reversal has postoperative complications such as surgical site infection associated with patients' perception of a poor cosmetic outcome. Objective: The aim of this study is to describe the gunsight skin incision and closure technique for stoma reversal and the postoperative results. Material and methods: Between November 2020 and May 2021, this technique was performed on 15 patients with colostomy and ileostomy. The intraoperative and postoperative results were analyzed. Results: Mean age was 38 ± 2.5 years, body mass index was 28±1.5 kg/m2, and length of hospital stay was 4 ± 2 days. The technique provided good tissue exposure and no patients presented surgical site infection. Thirty days after discharge 100% of patients reported high satisfaction with the cosmetic result as evidenced by the 3-point Likert scale. Conclusion: This procedure offers technical advantages and high patient satisfaction.

Rationale and key points This article discusses post-discharge care for patients following stoma formation. While the patient is taught the practicalities of managing the stoma appliance in hospital, this learning should be integrated into their daily life following discharge from hospital. Nurses are required to consider and address any issues that might affect the patient in the long term. These include changes in body image, adapting to having a stoma, preventing or treating complications associated with a stoma, and long-term follow-up. ¼ Most patients manage successfully with their stoma following a period of adaptation. ¼ Follow-up for patients with a stoma can occur over the telephone and/or face-to-face in the patient's home or in the clinic. ¼ There are several support groups in the UK that can be accessed by patients with a stoma. ¼ Patients should seek advice and assistance from a stoma specialist nurse if any issues related to the stoma arise, such as leakage or sore peristomal skin.

Rationale and key points This article provides an overview of the various types of abdominal stoma and surgical procedures that result in stoma formation. It outlines the enhanced recovery pathway that is used to guide postoperative care, and some of the areas that should be incorporated in postoperative stoma care. These areas include training in the practical care of the stoma and its appliance, dietary advice and reintegrating lifestyle activities. In addition, the article briefly examines stoma reversal, and explores the role of the stoma specialist nurse. ¼ Nurses require an understanding of the various surgical procedures that might result in the formation of a stoma. ¼ An understanding of the stoma appliances that are available is essential to enable nurses to provide effective care for patients with a newly formed stoma. ¼ Effective postoperative care that incorporates support and education enables patients to adapt quickly to having a stoma. ¼ Enabling patients to understand changes that occur as a result of stoma formation will increase their confidence in managing their stoma alongside the effect on their quality of life.

Rationale and key points This article addresses preoperative care of patients for whom surgery to form a stoma is planned. It discusses conditions that may result in the formation of a stoma, explores preoperative investigations that may be necessary before surgery, outlines various types of stoma and stoma appliances, and offers guidance on preparing patients to manage this lifestyle transition. The stoma specialist nurse has the principal role in providing patient education. However, nurses in all practice settings may encounter patients who are due to undergo surgery to form a stoma, and can provide preoperative support and education. ¼ Nurses require an understanding of the enhanced recovery pathway to support patients who are due to undergo surgery to form a stoma. ¼ Optimal siting of the stoma is essential to enable patient self-care. ¼ Effective preoperative care of patients undergoing planned surgery to form a stoma enables patients to gain independence quickly following stoma formation. ¼ Enabling patients to self-care for their stoma is associated with improved quality of life and enhanced body image.

RESUMEN La narrativa de las situaciones de enfermería resalta las vivencias y significados de la persona objeto de cuidado con un proceso de enfermedad y da cuenta de la relación de cuidado enfermero-paciente como aspecto clave para la recuperación de la salud física, mental y espiritual, teniendo como base el proceso de atención de enfermería. Se presenta la situación de enfermería de una persona mayor de 74 años de edad, posquirúrgica de laparotomía exploratoria más resección de masa en colon sigmoide, que derivó en una colostomía. Se resaltan situaciones relacionadas con la ansiedad y preocupación de parte de una de sus hijas por el estado de salud de su madre. Con base en la narrativa se analizaron los patrones de conocimiento personal, empírico, ético y estético, la visión del mundo de enfermería integrativa-interactiva, así como la aplicación de la teoría de rango medio de las transiciones; los supuestos de las propiedades de la transición de Afaf Meleis.


ABSTRACT The narrative of nursing situations highlights the experiences and meanings of the person object of care with a disease process, and accounts for the caring relationship between the nurse and the patient in need of nursing care as a key aspect for the recovery of physical, mental, and spiritual health, based on the nursing care process. The nursing situation of a 74-year-old woman after a laparotomy surgery plus resection of a mass in the sigmoid colon, that led to a colostomy, is presented. Situations related to anxiety and concern of one of her daughters about the state of health of her mother are highlighted. Based on the narrative, we analyzed the fundamental patterns of personal, empirical, ethical, and aesthetic knowledge; and also the Interactive-Integrative Nursing Worldview, as well as the application of Afaf Meleis' Transitions mid-range Theory and its assumptions of the transition properties.

BACKGROUND: A correlational survey to assess the quality of life (QoL) of ostomates with selected factors in a selected hospital of Delhi and to develop guidelines for the health professionals to improve QoL of ostomates was undertaken during the year 2005-2007. The objectives of the study were as follows: (1) To assess QoL of ostomates (2) To identify the factors associated with QoL of ostomates (3) To seek relationship between selected factors and QoL of ostomates (4) To develop guidelines for the health professionals to improve QoL of ostomates. MATERIALS AND METHODS: The corelational survey was conducted on 50 ostomates from Surgical Oncology OPD of B.R.A. Institute Rotary Cancer Hospital, AIIMS, Delhi. Purposive sampling technique was employed to select the sample subjects. Structured interview schedule was prepared. Guidelines were prepared for health professionals to improve the quality of life of ostomates. RESULT: Majority of the ostomates possessed best quality of life. There was a significant association between QoL score of ostomates with age, sex, duration of surgery, education, income, and occupation. There was no significant association between QoL scores of ostomates and marital status and type of ostomy. CONCLUSION: This study conclude that nurses have a great role to play in the physical, psychological, economical, social, familial, and sexual aspects in the care of ostomates and to offer psychological support and empathy, to reinforce coping skills to promote an optimal QoL.also she has a great role to influence and educate all the aspects of care to the patients and their relatives. Their is a need to develop staff development program for nursing personnel in the clinical area in healthcare system.

BACKGROUND: Ethical issues in medical practice are increasing in number, diversity and complexity and posing professional diemmas for physicians. It is the duty of Muslim physicians in collaboration with jurists, to resolve these issues. OBJECTIVES: These guidelines aim at answering the following two questions: 1. On religious grounds to what extent is a Muslim patient bound ground to accept surgical treatment requiring permanent diversion of stool and urine? 2. What should be the role of medical staff in convincing the patient aril his relatives to accept diversion stomas. METHODS: 1. Identification of the consequences of diversion stomas. 2. Verification of the religious rule in relation to seeking treatment. 3. Identify the effect of carrying a stoma bag on the patient's purity during worship 4. Outline the role of the medical staff in convincing the patient vial his relatives to accept a needed stoma RESULTS #ENTITYSTARTX00026; DISCUSSION: A Muslim patient needing a permanent diversion stoma has a religiously proven duty to listen to the instructions of a proficient physician in order to save himself if safety is most probable. Carrying a stoma bag does not interfere totally with patients purity. The exact role of medical staff in convincing the patient and his relatives to accept the procedure is discussed.
