RESUMEN

The training program for the Family and Community Medicine specialty (MFyC), which has been in effect since 2005, is currently undergoing a review and update process. This article proposes contributions to deepen the specific contents of the specialty in order to guide towards a more significant competency development. To carry out values-oriented training, it is suggested to deploy the values function and promote the creation of care spaces where the daily experience of those values is possible. It is proposed to establish a scale of values where the two essential values of the family physician are, in this order, a commitment to the individual person, and a commitment to the group of people under their care. Additionally, it is proposed to reorganize the competency map around five competency integrators or meta-competencies: patient-centered clinical method, population-based clinical governance, primary care oriented to the community, health promotion or community health based on assets, and research in the family and community field.

Family medicine is the first gateway to primary health care in health systems. This feature makes the definition of family medicine complex due to its inclusive, holistic and continuous approach. The definition of family medicine should include the professional individual aspect of the family physician as well as its duties, authorities and responsibilities.1 Since the development of family medicine, definitions of the discipline have been frequently modified and updated, continuing to the present day. Family medicine represents the most basic aspect of the health-care system, which makes developing such definitions quite complex, and definitions must be revised and updated as conditions change. The first definition of family medicine, presented in 1974, was followed by a definition by Olesen et al. in 2000, and later by the European Society of General Practice/Family Medicine (WONCA Europe) in 2002. The WONCA Europe definition was then updated in 2011. Although this last definition explains family medicine in the most detailed way, today it needs updating especially in defining the individual characteristics of family medicine and its relations with the environment. There is a need to define the highly intensive role of family physicians while considering, the physicians' spiritual and personal agenda as human beings. The Göktas definition of family medicine/general practice, which was suggested at the WONCA Europe 2018 conference in Krakow, Poland, represents a suitable means of completing the 2011 WONCA Europe definition in this regard.

BACKGROUND AND OBJECTIVES: Osteoporosis is considered a generalised skeletal disorder in which there is impaired bone resistance, which predisposes the individual to a greater risk of fracture. The aim of this cross-sectional study was to collect and present data on the main clinical characteristics of patients who consult medical internists in Spain. Understanding these characteristics can help in implementing action plans to improve these patients' care more effectively and efficiently. MATERIAL AND METHODS: Through an analysis of the Osteoporosis in Internal Medicine (OSTEOMED) registry, this study presents the main clinical characteristics of patients with osteoporosis who attended internal medicine consultations in 23 Spanish hospital centres between 2012 and 2017. We analysed the reasons for the consultations, the densitometric values, the presence of comorbidities, the prescribed treatment and other lifestyle-related factors. RESULTS: In total, 2024 patients with osteoporosis were assessed (89.87% women, 10.13% men). The patients' mean age was 64.1±12.1 years (women, 64.7±11.5 years; men, 61.2±14.2 years). There was no significant difference between the sexes in their history of recent falls (9.1% and 6.7%); however, there were significant differences in the daily intake of calcium from milk products (553.8±332.6mg for women vs. 450.2±303.3mg for men; P<.001) and in the secondary causes of osteoporosis (13% of men vs. 6.5% of women; P<.001). In the sample, there were 404 fractures (20%), with a notable number of confirmed vertebral fractures (17.2%, 35.6% in men vs. 15.2% in women; P<.001). A large portion of the patients did not undergo the indicated treatment and presented low levels of physical activity and sun exposure. A significant percentage of the patients presented associated comorbidities, the most common of which were hypertension (32%) and dyslipidaemia (28%). CONCLUSIONS: These results define the profile of patients with osteoporosis who attend internal medicine consultations in Spain. The results also show the multisystemic character of this condition, which, along with its high prevalence, determine that the specific internal medicine consultations dedicated to managing the condition are the appropriate place for caring for these patients.

In 1964, medical residents and interns of the Mexican Republic decided to carry out a work stoppage that stretched over for several months. Several prominent doctors decided to join the causes that motivated the movement, among which Dr. Ismael Cosío Villegas, who at that moment was the director general of the Huipulco hospital for tuberculosis patients; his participation and dedication are historical, and his determination caused for his resignation to be demanded. His trajectory remains practically unknown by the medical community.


En 1964, los médicos residentes e internos de la República Mexicana decidieron realizar un paro de labores que se extendió por meses; varios médicos notables decidieron unirse a las causas que motivaron dicho movimiento, entre ellos el director general en ese momento del Sanatorio para Tuberculosos de Huipulco, el doctor Ismael Cosío Villegas, cuya participación y entrega es histórica. La determinación del doctor Cosío Villegas provocó que se le exigiera su renuncia. La comunidad médica aún se desconoce su trayectoria.

OBJECTIVES: Postgraduate courses can contribute to better-qualified personnel in resource-limited settings. We aimed to identify how entry characteristics of applicants predict performance in order to provide support measures early. METHODS: We describe demographic data and end-of-module examination marks of medical doctors who enrolled in a first semester module of two one-year MSc programmes between 2010 and 2014. We used t-tests and one-way anova to compare, and post hoc tests to locate differences of mean marks between categories of entry characteristics in univariate analysis. After exclusion of collinear variables, multiple regression examined the effect of several characteristics in multivariable analysis. RESULTS: Eighty-nine students (47% male) with a mean age of 32 (SD 6.4) years who received their medical degree in the UK (19%), other European (22%), African (35%) or other countries (24%) attended the 3-months module. Their mean mark was 69.1% (SD 10.9). Medical graduates from UK universities achieved significantly higher mean marks than graduates from other countries. Students' age was significantly negatively correlated with the module mark. In multiple linear regression, place of medical degree (ß = -0.44, P < 0.001) and time since graduation (ß = -0.28, P = 0.007) were strongest predictors of performance, explaining 32% of the variation of mean marks. CONCLUSION: Students' performance substantially differs based on their entry criteria in this 1st semester module. Non-UK graduates and mature students might benefit from early support.

RESUMEN

An efficient primary care is of particular importance for any countries' health care system. Many differences exist on how distinctive countries try to obtain the goal of an efficient, cost-effective primary care for its population. In this article we conducted a selective literature review, which includes both scientific and socio-political publications. The findings are complemented with the experience of a Spanish physician from Seville in her last year of training in family medicine, who completed a four months long rotation in the German health care system. We highlighted different features by comparing both countries, including their health care expenditure, the relation between primary and secondary care, the organization in the academic field and the training of future primary care physicians. It is clear that primary care in both countries plays a central role, have to deal with shortcomings, and in some points one system can learn from the other.

INTRODUCTION: On the centenary of the foundation of the Paediatrics Society of Madrid, a tribute is presented to rural medical practice of that time, although there are few documents on the history of rural medicine. The main objective is to describe the context of the rural medical practice in the late 19th and beginning 20th century, while presenting a historical biographical review of Manuel Martínez Saldise, who was medical specialist from Cazalegas (Toledo). He was appointed an Honorary Member by the Paediatrics Society of Madrid in 1927. MATERIAL AND METHODS: A search was carried out in repositories of digitized media, web portals of history of medicine, PubMed, IME files of local councils and medical colleges. The family archives were reviewed with the collaboration of his descendants. RESULTS: The hiring of rural doctors was carried out by the municipalities, and the salary largely depended on private practice as well as "retainers". Specialist physicians took part in epidemics, legal medicine, and in hygiene measures. They also had disputes with mayors, chiefs, with colleagues and with protectionism. A summary of the biography and occupational activity of Manuel Martínez Saldise is presented. COMMENTS: Rural doctors were subjected to the society of their time, with the issues that arose, denouncing the shortcomings of the local administrations, dedicated efforts to their family and the most disadvantaged.

The Working Group responsible for the Italian translation of the third edition of the International Code of Ethics, appointed by the President of the International Commission on Occupational Health (ICOH), Dr. Jukka Takala, completed last April the revision work. The final text, already available on the ICOH website, has been printed and distributed by the Italian National Institute for Insurance against Accidents at Work (INAIL) at the 79th National Congress of the Italian Society of Occupational Medicine and Industrial Hygiene (SIMLII), in Rome. The curators of this third Italian edition have accomplished the delicate task of adaptation in Italian, taking into account the specificities of the practice of medicine in the Italian work environment. It involves many professionals with diverse roles and responsibilities in the public and private sectors for safety, hygiene, health and environment in relation to work. More than twenty years after the first Italian edition, we trace the evolution of the ICOH International Code of Ethics, in order to focus its birth, national and international distribution, and continuous improvement as well as its ability to direct the stakeholders towards a participatory prevention model, in a legislative framework that has seen over the past two decades a radical change in the Italian world of work.

INTRODUCTION: The method for selecting medical graduates for residency positions has a strong influence on teaching and learning strategies in medical schools. The methodology currently used in Spain does not seem appropriate for ranking the candidates or improving curriculum development. Thus, and taking into account the most consistent methodologies used in the United Kingdom and USA, we have designed a new method to be used in our country. OBJECTIVES: To analyze the limitations of the methodology used in Spain, and propose a new one aimed to improve the accuracy of selection itself and avoiding the negative influence of the current method on curricular development. In addition, we emphasize the necessity of improving teaching and learning in the clinical context to assure that graduating students reach an adequate level of clinical competence. CONCLUSIONS: The method for selecting candidates to residency post currently used in Spain, which relies mainly on testing theoretical knowledge, should be changed for an alternative methodology taking into account student,s performance through the course and assessing his/her ability for clínical contextualization of knowledge and level of clinical competence.

INTRODUCTION: The design of an appropriate method for the selection of medical graduates for residency posts is extremely important, not only for the efficiency of the method itself (accurate identification of most competent candidates), but also for its influence on the study and teaching methodologies operating in medical schools. Currently, there is a great variation in the criteria used in different countries and there is no definitively appropriate method. The use of isolated or combined criteria, such as the marks obtained by students in medical schools, their performance in tests of theoretical knowledge and evaluations of clinical competence, or personal interviews, have a limited value for identifying those candidates who will perform better during the residency and later on during independent practice. OBJECTIVES: To analyse the variability in the methodologies used for the selection of residents employed in different countries, in particular those used in the United Kingdom and USA, where external agencies and medical schools make systematic analyses of curriculum development. The advantages and disadvantages of national or transnational licensing examinations on the process of convergence and harmonization of medical degrees and residency programmes through Europe are discussed. The present analysis is used to design a new and more efficient multi-criteria methodology for resident selection in Spain, which will be published in the next issue of this journal. CONCLUSIONS: Since the multi-criteria methods used in UK and USA appear to be most consistent, these have been employed for designing the new methodology that could be applied in Spain. Although many experts in medical education reject national examinations for awarding medical degrees or ranking candidates for residency posts, it seems that, when appropriately designed, they can be used to verify the level of competence of graduating students without necessarily distorting curriculum implementation or improvement.

OBJECTIVE: To examine the professional profile of the family doctor in different countries and the social welfare context where their work is carried out. DESIGN: Qualitative Methodology of production of field diaries of a normal day. LOCATION: Primary Heath Care of Toledo and Tenerife in Spain, and Paraguay, Mexico, and Peru. PARTICIPANTS AND CONTEXTS: Non-random sampling, intentional, followed by snowball sample until data saturation. METHOD: Participants wrote a diary of a typical day's work, their circumstances and socio-health context, and were studied by content analysis. Techniques to control the biases were used the check the participants and the triangulation between the obtained results and the existing bibliography, and data found on the Internet daily. We performed a mental map to transcribe the results graphically and in a comprehensive form. RESULTS: A total of 24 diaries of a normal day were obtained (9 doctors in Spain, 7 in Mexico, 4 in Paraguay, and 4 in Peru). We found some similarities, but many differences between countries. In contexts of humble but spirited, rural, with traditional roots and undemanding, there was a wider range of tasks of the family doctor, the coexistence of public and private work, and modern and traditional medicine, with greater presence of family and community care, more physician satisfaction and better patient-physician relationship. CONCLUSIONS: The professional profile of the family doctor is diverse and a context-dependent variable, and is not derived directly from external theory of family medicine.

INTRODUCTION AND OBJECTIVES: Digital health (DH) is a broad concept, bringing together technology and healthcare, that is playing an increasingly important role in the daily routine of healthcare professionals (HCPs) and promises to contribute to the prevention and treatment of cardiovascular disease. There are no solid data on the position of Portuguese HCPs toward the implementation of DH in cardiovascular medicine. This national cross-sectional study aims to provide a snapshot of DH implementation in Portuguese cardiovascular HCP routines and to identify both expectations and barriers to its adoption. METHODS: An 18-question survey was created specifically for this study and distributed to 1174 individuals on the Portuguese Society of Cardiology mailing list. RESULTS: We collected 117 valid responses (response rate 10%). Almost all participants had smartphones and laptops, and two-thirds had tablets. Electronic medical information systems were the most used DH tool (84% of respondents) and were considered the most important for improving cardiovascular care. Implantable technologies (sensors and devices), telemedicine and social media were used by more than two out of three respondents and considered «very important¼ or «extremely important¼ by most of them. Most participants showed positive expectations regarding the impact of DH in cardiovascular medicine: 78% agreed that DH could improve health outcomes, 64% that it promotes health literacy and 63% that it could decrease healthcare costs. The top-rated barriers were patients' inability to use smartphones, limited access to electronic devices, and lack of legal regulation of DH. CONCLUSION: Most Portuguese cardiovascular HCPs had at least three electronic devices (mainly smartphones, laptops and tablets) and showed positive expectations regarding DH's current and future impact on cardiovascular medicine. Patient DH literacy, technology adoption, and DH regulation were identified as the most important barriers to increasing the adoption of DH tools in cardiovascular medicine.

Physicians have long needed and sought out the support and advice of experienced colleagues. This practice, endorse by Hippocrates and Galen, remaining unchanged until the Enlightenment. During that age, cross-consultations boomed. Monographic works were written, the characteristics and qualities that consulting physician had to possess were studied the problems that it could cause were examined, and rules and guidelines to follow during a cross-consultation were established. It remained unchanged until the end of the 19th century, when the emergence of various medical specialties offered the possibility of seeking specialized assistance. This specialization gave rise to a fragmentation of medical care which favored the emergence of the internist as a "universal consultant." In the last quarter of the 20th century, in light of the importance of and problems arising from cross-consultation, it began to be studied on its own, specialized services were created to attend to them, and, finally, comanagement appeared.

Resumo Introdução: Pesquisas científicas indicam que a espiritualidade desempenha um papel importante na vida da maioria dos pacientes. Além disso, atividades e crenças religiosas podem, de acordo com algumas pesquisa, estar relacionadas à melhor saúde e qualidade de vida1. Objetivo: Este estudo teve como objetivos avaliar o nível de espiritualidade de estudantes de Medicina e de médicos já formados, e analisar o ensino da interface "medicina e espiritualidade" na escola médica. Método: Realizamos um estudo transversal descritivo por meio da aplicação de questionários a estudantes de Medicina e médicos de uma escola médica pública brasileira. Resultado: Avaliaram-se 234 participantes. A maioria acredita em uma força superior. A espiritualidade foi maior entre médicos já formados e entre pessoas do sexo feminino. A maioria acredita que a formação universitária não prepara o médico para abordar o tema com os pacientes. Apesar dessa limitação, a maioria já abordou a espiritualidade com seus pacientes. Conclusão: Médicos e estudantes de Medicina consideram importante contemplar, de maneira ecumênica e respeitosa, aspectos espirituais dos pacientes. Apesar disso, consideram que não receberam preparo suficiente na escola médica para essa abordagem.


Abstract Introduction: Scientific research indicates that spirituality plays an important role in the daily life of most patients. Moreover, there are studies indicating that religious activities and beliefs may be related to better health and quality of life1. Objective: to evaluate the level of spirituality of medical students and graduated physicians, in addition to analyzing the teaching of the "Medicine and Spirituality" interface in medical school. Methodology: A descriptive cross-sectional study was carried out by applying questionnaires to medical students and doctors at a Brazilian public medical school. Results: a total of 234 participants were evaluated. Most believe in a higher power. Spirituality was higher among doctors who had already graduated and among females. Most believe that university education does not prepare doctors to address the topic with the patients. Despite this limitation, most have already discussed spirituality with their patients. Conclusion: Doctors and medical students consider it important to contemplate spiritual aspects of patients using an ecumenical and respectful approach. Nevertheless, they consider they were not sufficiently prepared in medical school for this approach.

RESUMEN

The significant and progressive reduction in the number of permanent teachers in medical schools (professor, associate professor and assistant professor) is a reason for concern for the National Conference of Deans. This reduction will intensify in the coming decade (2017-2026). Forty-three percent of the permanent faculty will retire, as will 55% of the faculty linked to clinical areas, 34% of the faculty not linked to clinical areas and 32% of the faculty of basic areas. This deficit is significant now, and, in a few years, the situation will be unsustainable, especially in the clinical areas. This report reveals the pressing need to adopt urgent measures to alleviate the present situation and prevent a greater problem. The training of future physicians, immediately responsible for the health of our society, depends largely on the theoretical and practical training taught in medical schools, with the essential collaboration of healthcare institutions. Paradoxically, while the number of teachers decreases substantially, there is an exponential increase in the number of medical schools and students who are admitted every year without academic or healthcare justification.

Introducción: Perfeccionar las habilidades de comunicación y comprensión del inglés en los momentos actuales es una necesidad indispensable para aquellos que realizan estudios de maestría y doctorado, pues les permite divulgar los resultados de sus investigaciones con mayor precisión, así como asimilar claramente la literatura especializada que consulten. Objetivo: Analizar algunas de las principales estrategias de traducción empleadas por los participantes en el curso de comprensión y comunicación de los resultados de investigación. Métodos: Se realizó una investigación educativa en el periodo 2021-2022 en la Universidad de Ciencias Médicas de Cienfuegos, para lo cual se seleccionó una muestra de 42 profesionales, matriculados en los programas de formación doctoral en ciencias biomédicas, estomatológicas y psicológicas. A tal efecto, se empleó la observación como método para describir el proceso de entrenamiento en traducción de textos médicos del inglés al español. Resultados: La estrategia más empleada por los doctorandos fue la traducción literal, que se halló en los 107 fragmentos analizados y en 86,9 % de estos fue la única aplicada; a esta le siguió la transposición (70,0 %). Los calcos léxicos constituyeron la estrategia menos usada, pues solo figuraron en 11,2 % de los textos médicos traducidos del inglés al español. Conclusiones: Las estrategias más utilizadas fueron la traducción literal y la transposición, lo cual se debe a que las estructuras léxicas y sintácticas sirven de base para establecer analogías entre los idiomas español e inglés y, por tanto, facilitan la reexpresión que conlleva el proceso de traducción.


Introduction: Improving communication and understanding skills in English at present is an indispensable necessity for those who conduct masters and doctorate studies, since they allow them to disclose the results of research with more precision, as well as to clearly assimilate the specialized literature. Objective: To analyze some of the main translation strategies used by the participants in the course about understanding and communication research results. Methods: An educational study was carried out in the period 2021-2022 at the University of Medical Sciences in Cienfuegos, with a sample of 42 professionals enrolled in doctoral training programs of biomedical, stomatological and psychological sciences. For this purpose, observation was used as a method to describe the training process in translation of medical texts from English to Spanish. Results: The most used strategy by doctorate students was literal translation, which was found in the 107 fragments analyzed, and it was the only one applied in 86.9% of the cases; followed by transposition (70.0%). Lexical calque was the least used strategy, as it only appeared in 11.2% of the medical texts translated from English to Spanish. Conclusions: The most used strategies were literal translation and transposition, which is due to the fact that lexical and syntactic structures serve as a basis for establishing analogies between Spanish and English languages and, therefore, facilitate the re-expression involved in translation process.

RESUMEN Recientes investigaciones destacan la importancia de la ciberseguridad en la creciente era digital. El 83% de las organizaciones han experimentado brechas de seguridad en el 2022, costándoles en promedio 4,35 millones de dólares americanos por incidente. En Perú, la ciberseguridad está regulada por diversas normas, estableciendo medidas de protección de datos y la seguridad informática. Objetivo Describir la situación de la ciberseguridad en los servicios de apoyo al médico ocupacional (SAMO). Material y métodos Se incluyeron 11 SAMO. Se elaboró un cuestionario no validado para recolectar la información consentida sobre la gestión de la ciberseguridad de los SAMO que brindaban servicio a un importante proyecto de construcción en Lima Metropolitana. Resultados La mayoría de los establecimientos de salud (más del 80%) tuvo planes de respuesta a incidentes de seguridad cibernética para garantizar una respuesta rápida ante un ataque cibernético; realizaban copias de seguridad de los datos críticos con regularidad y los almacenaban en un lugar diferente a establecimiento; y actualizaban regularmente los sistemas operativos y programas de softwares buscando asegurar que se utilizan versiones seguras. Conclusión Existe una gestión de la seguridad informática predominantemente reactiva. El reporte discute la importancia de la ciberseguridad, resaltando la exposición de la información médica y los servicios a riesgos cibernéticos y planteamos retos futuros para la ciberseguridad, subrayando la importancia de la preparación ante amenazas futuras en un entorno en constante transformación digital.


SUMMARY Recent investigations emphasize the importance of cybersecurity in the growing digital era; 83% of organizations experienced breaks in cyber security in 2022, spending a mean of 4,35 million dollars per incident. Cybersecurity in Peru is regulated by legal norms aimed at protecting data and providing informatic security. Objective To describe cybersecurity in the support services to occupational physicians (SSOP) in the city of Lima. Methods A non-validated survey was created and distributed to 11 SSOPs in Lima that provide a service to an important building project in Lima. Results More than 80% of health care establishments had a response plan against cybernetic attacks; security copies of the data were done regularly storing them in a different place than the establishment and regularly updated security software's. Conclusion The cybersecurity program is reactive. We discuss the importance of cybersecurity and analyze future challenges as well as emphasize the importance of preemptive preparedness in an environment of constant digital transformation.

O conhecimento e o desenvolvimento de habilidades relacionadas à abordagem de gênero na formação médica são fundamentais para o exercício do cuidado integral. Enfocamos o ensino dos temas gênero e sexualidade na graduação de um curso de Medicina sob a perspectiva docente. A pesquisa de natureza qualitativa utilizou a entrevista semiestruturada para construção dos dados empíricos. Em 2022, foram realizadas 16 entrevistas com responsáveis por disciplinas/unidades curriculares de diversas especialidades médicas de um curso de graduação em Medicina no estado de São Paulo. Apesar do reconhecimento da importância na formação, vigora ainda uma abordagem biomédica e/ou patológica das temáticas de gênero e sexualidade no currículo. Como dificuldades, destaca-se o reconhecimento da questão geracional pela falta de habilidade para lidar com os temas e a pouca interação entre especialidades médicas.(AU)


Including the development of knowledge and skills related to the gender approach in medical training is essential for the delivery of comprehensive care. This study focuses on the teaching of gender and sexuality on a medical degree program from the perspective of teachers. We conducted a qualitative study using semi-structured interviews to collect empirical data. Sixteen interviews were conducted in 2022 with teachers responsible for subjects/modules in medical specialties on a medical degree program in the state of São Paulo. Despite the recognition given to the importance of teaching gender and sexuality in medical training, a biomedical and/or disease-based approach to these topics prevails in the curriculum. Challenges include generational issues, including the lack of skills needed to deal with these topics, and limited interaction between medical specialties.(AU)


El conocimiento y el desarrollo de habilidades relacionadas al abordaje de género en la formación médica son fundamentales para el ejercicio del cuidado integral. Enfocamos la enseñanza de los temas género y sexualidad en la graduación de un curso de medicina bajo la perspectiva docente. La investigación de naturaleza cualitativa utilizó la entrevista semiestructurada para la construcción de los datos empíricos. En 2022, se realizaron dieciséis (16) entrevistas con responsables por disciplinas/unidades curriculares de diversas especialidades médicas de un curso de graduación en medicina en el Estado de São Paulo. A pesar del reconocimiento de la importancia en la formación, vigora todavía un abordaje biomédico y/o patológico de las temáticas de género y sexualidad en el currículo. Como dificultad se destaca el reconocimiento de la cuestión generacional por la falta de habilidad para enfrentar los temas y la poca interacción entre especialidades médicas.(AU)

Introdução: Os acidentes ocupacionais com material biológico representam um problema de saúde pública. A exposição ocupacional dos profissionais da saúde configura-se como um risco de transmissão de diversos patógenos. Na literatura, há carência de estudos que analisem o perfil dos acidentes com material biológico nos médicos da atenção primária. Objetivo: Buscou-se compreender o perfil epidemiológico dos acidentes com material biológico em médicos da atenção primária em Minas Gerais. Métodos: Estudo epidemiológico descritivo com análise do perfil dos acidentes com material biológico em médicos da atenção primária em Minas Gerais, utilizando dados secundários. Resultados: No período analisado, foram registrados 111 acidentes com material biológico, dos quais 54% ocorreram somente em 2020 e 2021. A maioria dos casos deu-se em mulheres (59%), e os tipos mais frequentes de exposição foram mucosa (38%) e percutânea (33%). Dos médicos, 23% não possuíam esquema vacinal contra a hepatite B completo. Em média, em 36% dos acidentes os testes sorológicos foram negativos e em 61% não foram realizados ou o campo foi ignorado/deixado em branco. Em apenas 7,2% dos casos a quimioprofilaxia foi indicada, mas ressaltam-se os registros ignorados ou em branco. Mais da metade dos acidentados não emitiu a Comunicação de Acidente de Trabalho (CAT). Conclusões: Os acidentes com material biológico predominam em médicas e nas formas de exposição mucosa e percutânea. Investimentos em medidas de biossegurança e educação permanente são necessários para prevenir casos e estimular sua notificação.


Introduction: Occupational accidents with biological material represent a public health problem. The occupational exposure of health professionals represents a risk of transmission of various pathogens. In the literature, there is a lack of studies that analyze the profile of accidents with biological material among primary health care physicians. Objective: We aimed to understand the epidemiological profile of accidents involving biological material among primary health care physicians in Minas Gerais, Brazil. Methods: Descriptive epidemiological study that analyzed the profile of accidents with biological material among primary health care doctors in Minas Gerais, using secondary data. Results: In the period analyzed, 111 accidents with biological material were recorded, of which 54% occurred only in 2020 and 2021. Most cases occurred in women (59%) and the most frequent types of exposure were mucosal (38%) and percutaneous (33%). About a quarter (23%) of physicians did not have a complete immunization record for hepatitis B. On average, in 36% of accidents serological tests were negative and in 61% they were not performed or the field was ignored/left blank. In only 7.2% of cases, chemoprophylaxis was indicated, but ignored or blank records stood out. More than half of the victims did not fill out a work accident report. Conclusions: Accidents with biological material predominate in female doctors and in forms of mucosal and percutaneous exposure. Investments in biosafety measures and permanent education are necessary to prevent cases and encourage their notification.


Introducción: Los accidentes de trabajo con material biológico representan un problema de salud pública. La exposición ocupacional de los profesionales de la salud representa un riesgo de transmisión de varios patógenos. En la literatura faltan estudios que analicen el perfil de accidentes con material biológico en médicos de atención primaria. Objetivo: Buscamos comprender el perfil epidemiológico de los accidentes con material biológico en médicos de atención primaria en Minas Gerais. Métodos: Estudio epidemiológico descriptivo con análisis del perfil de accidentes con material biológico en médicos de atención primaria en Minas Gerais, utilizando datos secundarios. Resultados: En el período analizado se registraron 111 accidentes con material biológico, de los cuales el 54% ocurrió solo en 2020 y 2021. La mayoría de los casos ocurrieron en mujeres (59%) y los tipos de exposición más frecuentes fueron mucosa (38%) y percutánea (33%). El 23% de los médicos no disponía de un calendario completo de vacunación frente a la hepatitis B. En promedio, en el 36% de los accidentes, las pruebas serológicas fueron negativas y en el 61% no se realizó o se ignoró/dejó el campo en blanco. Solo en el 7,2% de los casos se indicó quimioprofilaxis, pero destacan los registros ignorados o en blanco. Más de la mitad de las víctimas no emitieron el CAT. Conclusiones: Predominan los accidentes con material biológico en médicas y en formas de exposición mucosa y percutánea. Son necesarias inversiones en medidas de bioseguridad y educación permanente para prevenir casos e incentivar su notificación.

Fundamento: el estudio de las enfermedades debe integrar perspectivas e incluir variables relacionadas con los profesionales sanitarios. La aparición de nuevas enfermedades como la COVID-19 ha generado un grupo de investigaciones dirigidas a los profesionales y estudiantes inmersos en su prevención y control. Objetivo: caracterizar las actitudes de los estudiantes de Enfermería vinculados al pesquisaje activo de casos sintomáticos de COVID-19. Métodos: en la universidad médica camagüeyana se realizó un estudio descriptivo, transversal, en el período de marzo a mayo de 2021, con la combinación de metodologías cualitativas. Se aplicaron métodos teóricos para la fundamentación de la investigación y empíricos como la observación externa directa, entrevista semiestructurada y composición, a una muestra de 30 estudiantes de licenciatura en Enfermería. Resultados: las actitudes se caracterizaron por tener una dirección que no indicó rechazo abierto, pero se percibió la dificultad de abordar el tema de la posibilidad del propio contagio en el 49,30 %. Hubo un 62,10 % de inconsistencia interna en las actitudes, al constatarse una relación antagónica entre sus componentes. Conclusiones: la vinculación de los estudiantes de Enfermería a escenarios clínicos y comunitarios permitió la identificación de características en sus componentes, consistencia o funciones que implican exposición a situaciones de riesgo o limitan el desempeño; es imprescindible modificarlas en aras de garantizar las acciones de sostenibilidad y la formación profesional.


Background: the study of diseases must integrate perspectives and include variables related to health professionals. The emergence of new diseases such as COVID-19 has generated a group of research aimed at professionals and students immersed in its prevention and control. Objective: to characterize the attitudes of nursing students linked to active screening in cases of COVID-19. Methods: a descriptive, cross-sectional study was carried out at Camagüey Medical University from March to May 2021, with a combination of qualitative methodologies. Theoretical methods were applied to support the research and empirical methods, such as direct external observation, semi-structured interviews and composition, to a sample of 30 undergraduate Nursing students. Results: the attitudes were characterized by having a direction that did not indicate open rejection, but the difficulty of addressing the issue of the possibility of contagion itself was perceived in 49.30%. There was 62.10% of internal inconsistency in attitudes, as an antagonistic relationship was found among its components. Conclusions: the linking of Nursing students to clinical and community settings allowed the identification of characteristics in their components, consistency or functions that imply exposure to risk situations or limit performance; it is essential to modify them in order to guarantee sustainability actions and professional training.
