RESUMEN

Local services can provide better and more joined-up care for patients when different organisations work collaboratively in an integrated system. Population health management (PHM) provides the shared data about local people's current and future health and wellbeing needs. Joint care planning and support addresses both the psychological and physical needs of an individual recognising the huge overlap between mental and physical wellbeing. Joint posts and joint organisational development are likely to become more commonplace and community nurses will have a vital contribution to planning and delivery of integrated care to improve health and care outcomes for their local populations.

This historical reprint from The Public Health Nurse, Volume 22, Number 11, published in November 1930, came from a speech presented at the National Organization for Public Health Nursing Round Table for Supervisors at the Biennial NOPHN convention in Milwaukee, Wisconsin, June 30, 1930. Ruth Telind was a supervisor with the Henry Street Visiting Nurse Service of New York. Her message about the orientation and development of staff nurses has relevance today. The work of visiting nurses has changed substantially, of course. The methods by which the aims of community education, recreation, health promotion, child welfare, and care for those who are chronically and acutely ill are accomplished today may seem far removed from the makeshift approaches of this Depression-era supervisor's experience. Nevertheless, the message that nurses have unique talents and interests that can be fostered to improve outcomes for the organization and to increase the satisfaction and retention of those nurses reminds supervisors, nurse educators, and executive directors of any nursing organization that the same principles apply 80 years later.

Reading the minutes of the annual meetings of the Brooklyn visiting nurses over the years, one is reminded of the old French proverb Plus ça change, plus c'est la même chose (The more things change, the more things stay the same). The questions of 1988 are often the same questions the board members struggled with 50, 80, and 100 years ago. Human nature changes very little. Science, it is true, gives us new instruments with which to work, and additional education is at our disposal. But the well-being of the client and the family, and our advocacy role present the same problems for solution. questions about caseloads to be carried, types of care to be given, and cooperation for community betterment appear in the minutes of the Association from the earliest days. The reactions to these problems have been crucial and vital in guiding the direction the service has taken. Recurrent problems have also demonstrated that no solution is permanent. The same questions will appear and reappear for careful thought according to the demands of the period in which we live and the way in which society has shaped the nature of care required. Over the last 100 years, there has been steady growth, with constant enlargement of vision and constant application of newer methods of advancement in client care. The use of leeches and blood letting of 100 years ago may seem absurd by today's standards. One can only imagine the developments in home nursing over the second century of caring.

Healthcare practitioners are aware that elder abuse exists in our society, and in light of present economic conditions, its prevalence may be increasing. This problem often goes undetected because abuse frequently is not acknowledged by the abused or the abuser. Home care nurses are in a prime position to recognize elder abuse and to intervene accordingly. This article addresses the characteristics of abused older adults, their abusers, strategies to detect elder abuse, and interventions that home care nurses can use to manage or to prevent the problem.

Objetivo: explorar as publicações na área da saúde nos últimos seis anos sobre consulta de enfermagem na Estratégia Saúde da Família (ESF), de modo a compor evidências bibliográficas sobre os processos de trabalho que se configuram hoje nesse cenário. Método: Trata-se de um estudo exploratório de abordagem qualitativa. O método escolhido foi a revisão integrativa. A seleção dos artigos foi realizada no portal Biblioteca Virtual em Saúde (BVS), sem filtro por bases de abril-julho 2014. Os dados foram analisados segundo a análise temática de Bardin. Resultados: Foram gerados 61 artigos nas bases Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), Banco de Dados em Enfermagem (BDENF) e Coleção Nacional das Fontes de Informações do Sistema Único de Saúde (Coleciona SUS). Após exclusão dos semelhantes e avaliação da pertinência do conteúdo para este trabalho, foram analisados 30 artigos. Conclusão: A consulta de enfermagem, muitas vezes, configura um cenário de atenção à saúde de forma verticalizada e medicalizada.


Objective: to explore the publications in the field of health in the last six years concerning nursing consultation in Family Health Strategy in order to gather bibliographical evidences about the process of the works that are today in this setting. Method: It is an exploratory research of qualitative approach. The chosen method was integrative review. The articles selection was made on the BVS portal with no filter based between April and July 2014. The data were analyzed according to the thematic analysis of Bardin. Results: 61 articles were found in the LILACS, BDENF and ColecionaSUS data bases. After excluding the similar ones and evaluating the relevance of the content to this work there were 30 articles. Conclusion: The nursing consultation often sets a scenario of attention to health in a verticalized and medicalized way.
