RESUMEN

INTRODUCTION: The role of the clinical nurse specialist is complex but is defined differently across the world. The role of clinical nurse specialist stoma care is undefined and it is uncertain what aspects of the role are included in the general day-to-day working role. AIMS: The aim was to gain consensus opinion to answer the research question: 'What is the role of the clinical nurse specialist in stoma care?' DESIGN: Delphi consensus. METHODS: Previous data gained from a scoping review and expert consultation was utilized to form role statements. At a UK conference the 13 statements and 173 sub-categories were voted upon. Consensus was agreed if 75% of voters voted agree or strongly agree. Two stages of voting occurred with results from the first vote being shared in the second voting session. RESULTS: All 13 statement and most (150/193) statement sub-categories reached consensus, with 20 sub-categories added during voting session one. CONCLUSIONS: The four pillars of advanced practice were met by the 13 statements with clinical and education reaching higher consensus and agreement than leadership/management and research. The results of the consensus study provide a clearer articulation of the clinical nurse specialist stoma care role, which is complex and multifaceted which has not been described previously. IMPLICATIONS FOR PRACTICE: Consideration of role evolution is made possible, to gain a greater expertise in the scope of practice it is necessary to include prescribing, management and research which could improve service delivery and optimize patient outcomes. There was no patient or public contribution, which in hindsight would have potentially improved the process but it was considered that patients might not recognize the full role of the nurse, understanding only aspects of the role that were patient-centred. PATIENT OR PUBLIC CONTRIBUTION: No patients or public were involved in any aspect of this paper-in hindsight this might have been useful.

RESUMEN

There are many benefits of standardised care but often this is not possible due to variations in the availability of healthcare resources in the UK. Within stoma care, there is evidence to guide care but with limited standardisation, particularly for community nurses. Stoma care is provided inconsistently, possibly due to variations in training and experience of the nurse. It is known that patients have greater needs in the first few months after stoma formation and this need for support is compounded if there are stoma complications. Community nurses can provide practical support and information, as well as psychological support and encouragement to help people become independent with stoma care and adapt to life with a stoma.

INTRODUCTION: Nurses frequently care for patients who have stomas. A common complication is sore peristomal skin (skin around the stoma). AIMS AND OBJECTIVES: The study aim was to answer the research question: what is the current nursing practice for peristomal skin problems among UK stoma specialist nurses? The question was explored through investigation of descriptions, treatments and opinions of peristomal skin problems. Results were examined to ascertain if practice reflects the literature and if care was evidence-based. METHODS: A questionnaire was posted in September 2009 to the stoma care nurses in all UK NHS hospitals (n=596). RESULTS: The proportion of completed or partially completed questionnaires was 15% (89 of 596). Most of the responding nurses held a stoma-related qualification (86%), a degree (55%) and had specialised in stoma care for over 5 years (67%). Respondents used erythema to describe sore skin (80%). Stoma powder (98%) and convex appliances (98%) were the most commonly used treatments. The most common cause of sore skin was appliance leakage (61%). CONCLUSIONS: The study population was deemed suitably qualified and experienced to answer the research question. Many responses were reflected in the literature (predominantly opinion articles), reflecting a degree of reliability and validity. It could be concluded that stoma specialist nurses can accurately assess and use stoma accessories to treat sore skin, but due to the paucity of research, the care cannot be defined as evidence-based. More research is needed to determine universally accepted definitions and treatments for sore peristomal skin.

ABSTRACT Objective: to construct and validate a high fidelity clinical simulation scenario on nursing care to colostomy patients. Method: descriptive study of construction and validation of the appearance and content of a high fidelity clinical simulation scenario referring to nursing care for colostomy patients. To guide the elaboration of the scenario, a study was carried out in the literature on nursing care for the colostomy patient in the hospital environment. The scenario was constructed according to the items proposed by Fabri, based on the Bloom's Taxonomy. For the selection of the nine experts, the criteria proposed by Fehring were used. The scenario, after being structured, was tested by a group of students from the 3rd and 4th years of graduation in nursing in a clinical simulation laboratory of a public university. It was considered 80% level of agreement. Results: in the validation of the scenario, all the experts agreed with the proposed items, suggesting the inclusion of some references, book chapters, skills training, preparation of reading material and checklist follow-up of participants. This scenario test allowed the identification of relevant contributions for adjustments of the simulated activity and allowed to test the debriefing with the support of the checklist. It was also evidenced the need to include information in the patient medical record and to increase the time of development of the scenario to solve the proposed objectives. Conclusion: the results show that for practical simulated well defined and successful are required to careful preparation, validation and prior testing of the planned activities.


RESUMEN Objetivo: construir y validar un escenario de simulacro clínico de alta fidelidad sobre atención de enfermería a pacientes con colostomía. Método: estudio descriptivo de construccióny validación de aparienciay contenido de unescenario de simulacro clínico de alta fidelidad, referente a la atención de enfermería a pacientes con colostomía. Para orientar la elaboración del escenario, se realizó una investigación en la literatura sobre los cuidados de enfermería al paciente con colostomía en el ambiente hospitalario. El escenario se construyó según los ítems propuestos por Fabri, con base en la Taxonomía de Bloom. Para seleccionarlos nueve especialistas, se utilizaron los criterios propuestos por Fehring. El escenario, luego de ser estructurado, ha sido testeado por un grupo de estudiantes de 3º y 4º año de la carrera de grado en enfermería en un laboratorio de simulacro clínico de una universidad pública. Se consideró un nivel de concordancia del 80%. Resultados: en la validación delescenario, todos los especialistas concordaron conlos ítems propuestos, sugiriendola inclusión de algunas referencias, capítulos de libros, capacitación de habilidades, confección de material de lectura y checklist de acompañamiento a los participantes. Este test delescenario permitió identificar las contribuciones relevantes para los ajustes de la actividad simulada y testear el debriefing conel apoyo del checklist. También se puso en evidencia la necesidad de incluir informacionesen elinforme del paciente y de aumentar el tiempo de desarrollo delescenario para resolverlos objetivos propuestos. Conclusión: los resultados demuestran que para prácticas simuladas bien delineadas y exitosas son necesarias una elaboración criteriosa, una validación y untest previo de las actividades planificadas.

Para direcionar a elaboração do cenário, foi realizada uma pesquisa na literatura sobre cuidados de enfermagem ao paciente com colostomia no ambiente hospitalar. O cenário foi construído segundo os itens propostos por Fabri, com base na Taxonomia de Bloom. Para seleção dos nove peritos foram utilizados os critérios propostos por Fehring. O cenário, após estruturado, foi testado por um grupo de estudantes do 3o e 4o anos de graduação em enfermagem em um laboratório de simulação clínica de uma universidade pública. Conclusão: os resultados mostram que para práticas simuladas bem delineadas e exitosas são necessárias a elaboração criteriosa, a validação e a testagem prévia das atividades planejadas.

RESUMEN

Aim: To describe how nurses are working in their daily practices with caregivers/families of children with stomata with respect to self-care and procedures for hospital discharge; to identify when nurses begin the guidance for hospital discharge to the caregiver/family of the child with a stoma; to discuss how the family involvement is constituted in the care of children with stoma from admission until discharge, from the perspective of the nurse. Methods: This is a descriptive case study in which we used a qualitative approach. The study was conducted with 31 nurses of the Neonatal Intensive Care Unit of a University Hospital in the city of Rio de Janeiro. Results: nurses guide families regarding the hospital discharge of stomized children (58%) and start with the guidance as soon as they have an indication of the development of the stoma (58%).
