Introducción: El trauma craneoencefálico (TCE) se define como una patología caracterizada por la alteración cerebral secundaria a una lesión traumática en la región de la cabeza, con la presencia de alteración de la consciencia y/o amnesia debido al trauma, cambios neurológicos, neurofisiológicos, con posibles fracturas de cráneo o lesiones intra craneanas atribuibles al trauma. Metodología: Es un estudio observacional, descriptivo, prospectivo en pacientes que sufrieron TCE internados en el servicio de neurocirugía en el Complejo Hospitalario Dr. Arnulfo Arias Madrid durante el periodo de marzo de 2022 a febrero de 2023. Resultados: Los resultados de este estudio mostraron que predomina el sexo masculino (78%) sobre el femenino (22%) en pacientes con TCE. Las causas de lesión más comunes fueron caídas de sus pies (27%), caídas de altura (25%) y colisión de moto/automóvil (20%). Las presentaciones clínicas más comunes fueron pérdida de consciencia (49%), amnesia/desorientación (19%) y cefalea (10%). Conclusión: El seguimiento promedio de los pacientes con TCE fue de 16 días. El género masculino fue el más frecuente, con mayor proporción en la edad media y adulta mayor. La etiología principalmente fue por caídas, manifestándose comúnmente por perdida del estado de alerta, y teniendo hallazgos múltiples en la tomografía cerebral. Introduction: Cranioencephalic trauma (TBI) is defined as a pathology characterized by cerebral alteration secondary to traumatic injury in the head region, with the presence of altered consciousness and/or amnesia due to trauma, neurological, neurophysiological changes, with possible skull fractures or intracranial lesions attributable to trauma. Methodology: This is an observational, descriptive, prospective study in patients who suffered TBI hospitalized in the neurosurgery service at the Complejo Hospitalario Dr. Arnulfo Arias Madrid during the period from March 2022 to February 2023. Results: The results of this study showed a predominance of male (78%) over female (22%) patients with TBI. The most common causes of injury were falls from their feet (27%), falls from height (25%) and motorcycle/automobile collision (20%). The most common clinical presentations were loss of consciousness (49%), amnesia/disorientation (19%) and headache (10%). Conclusion: The average follow-up of patients with TBI was 16 days. The male gender was the most frequent, with a higher proportion in middle age and older adults. The etiology was mainly due to falls, commonly manifested by loss of alertness, and multiple findings in brain tomography. Most patients were managed conservatively. Objective:To investigate the efficacy and safety of high semi-decubitus position in the second stage of labor in epidural analgesic delivery of primipara.Methods:A total of 120 women who gave birth in Hang-zhou women's Hospital from May 1,2021 to July 30,2021 were randomly divided into experimental group(n=60)and control group(n=60)by random number table method.The control group adopted supine position in the sec-ond stage of labor,and the experimental group transferred to high semi-supine position after the supine position was used applied for 30 minutes when the fetal head was not exposed.To compare the effects of different postur-al positions on the duration of the second stage of labor,mode of delivery,postpartum injury,perineal tearing,la-bor force experience and labor control,and neonatal asphyxia.Results:The duration of uterine opening to fetal head exposure,the duration of uterine opening to fetal head crown and the duration of second stage of labor in experimental group were shorter than those in control group.The rate of natural delivery of experimental group was higher than that of control group.The episiotomy rate and the second degree perineal laceration rate were lower than that of control group.The amount of blood loss during delivery and 2h postpartum of experimental group was less than that of control group.The scores of birth experience and birth control of experimental group were higher than those of control group,and the differences were statistically significant(P<0.05).There was no significant difference in 1-minute Agpar score and 5-minute Agpar score between the two groups(P>0.05).Conclusions:The application of high semi-decubitus position under epidural labor analgesia promoted vaginal natural labor,shortened the time of second stage of labor,reduced episiotomy rate and perineal laceration de-gree,reduced postpartum hemorrhage,had positive labor force experience,and increased the sense of labor con-trol.

Objective:To investigate the effects of epidural labor analgesia on neonatal sucking ability and breastfeeding.Methods:This was a prospective cohort study. Convenience sampling was used to enroll women who delivered with or without epidural analgesia (epidural analgesia and non-analgesia groups) in the Affiliated Maternity and Child Health Care Hospital of Nantong University from January 2022 to December 2022. Intra-oral negative pressure values during breastfeeding were measured by a pressure measuring instrument within 3 d after birth (0-24 h, >24-48 h, and >48-72 h), and the mean, maximum, and minimum pressure values were recorded. The Infant Breastfeeding Assessment Tool (IBAT) (Chinese version) was used to evaluate sucking posture. Breastfeeding status during hospitalization and within 1, 4, and 6 months after discharge were followed up. Two independent samples t-test, Chi-square (Fisher's exact) test, and generalized estimating equations were used for data analysis. Results:A total of 156 mother-infant pairs were analyzed in this study, with 78 pairs in the epidural analgesia group and 78 pairs in the non-analgesia group. The whole, first, and second stages of labor were longer in the analgesia group than those in the non-analgesia group [ M( P25- P75), total: 430 min (353-541 min) vs. 300 min (235-422 min), Z=-5.65; first stage: 385 min (310-490 min) vs. 260 min (205-347 min), Z=-5.81; second stage: 40 min (26-67 min) vs. 33 min (18-45 min), Z=-2.33; all P<0.05], and more women in the analgesia group accepted oxytocin and doula before delivery [94.8% (74/78) vs. 43.6% (34/78), χ2=48.15; 42.3% (33/78) vs. 19.2% (15/78), χ2=9.75; both P<0.05]. There was no significant difference in scores on the Fatigue Scale-14, Edinburgh Postnatal Depression Scale, Pittsburgh Sleep Quality Index, and Life Events Scale for Pregnant Women between the two groups. After adjusting for confounding factors by generalized estimating equations, the mean intra-oral negative pressure values in the epidural analgesia group were lower than those in the non-analgesia group at 0-24 h, >24-48 h, and >48-72 h after birth [5.14 (3.39-6.52) vs. 6.39 (4.95-9.15) kPa, 5.07 (3.94-7.38) vs. 6.20 (5.08-8.54) kPa, and 6.19 (4.64-8.36) vs. 6.81 (5.88-8.05) kPa, Wald χ2=5.59, P=0.018]; the peak value in the epidural analgesia group was also lower than that in the non-analgesia group [15.81 (9.74-21.68) vs. 16.21 (13.58- 24.88) kPa, 13.50 (9.83-16.50) vs. 17.62 (14.01-22.40) kPa, and 14.66 (10.47-18.71) vs. 18.04 (15.33- 19.85) kPa, Wald χ2=7.25, P=0.007]; there were no significant differences in the trough value between the two groups. The daily IBAT scores for the sucking position of neonates in the epidural analgesia group were all lower than those in the non-analgesia group in the first three days after delivery [10 (9-10) vs. 10 (10- 11) scores, 10 (9-11) vs. 11 (10-11) scores, and 10 (10-11) vs. 11 (11-12) scores, Wald χ2=17.05, P=0.001]. The rates of early exclusive breastfeeding within postpartum 3 d were also lower in the epidural analgesia group than those in the non-analgesia group [67.9% (53/78) vs. 75.6% (59/78), 41.0% (32/78) vs. 44.9% (35/78), and 38.5% (30/78) vs. 55.1% (43/78), Wald χ2=4.21, P=0.040]. No significant differences were found in the exclusive breastfeeding rates between the two groups at 1, 4, and 6 months after delivery. Conclusion:Epidural labor analgesia can lead to decreased early intra-oral negative pressure in neonates, affect neonatal sucking posture, and reduce the rate of exclusive breastfeeding during hospitalization, but has no significant correlation with the exclusive breastfeeding rate within six months after delivery.

AIM: To evaluate the effect of remimazolam on early postoperative cognitive function in elderly patients with hip fracture based on a randomized controlled trial. METHODS: A total of 106 elderly patients, aged 65-90 years, ASA grade Ⅱ or III, who underwent hip fracture surgery under combined spinal-epidural anesthesia in the Sixth Affiliated Hospital of Wenzhou Medical University from December 2022 to June 2023 and met the inclusion criteria, were selected and randomized into remimazolam group (group R) and propofol group (group P) according to the random number table, with 53 cases in each group. Patients in group P received a slow intravenous injection of propofol at a dose of 0.3-0.5 mg / kg (injection time of 1min), followed by a pump infusion at 0.5-3 mg · kg

In recent years, the incidence of spinal cord injury (SCI) has shown an increasing trend, and the resulting motor impairment cause significant harm to patients themselves and public health. Although the current clinical treatments for motor disorders after SCI are diverse, mainly focusing on improving dysfunction and improving patients' daily living ability through external stimulation, there is still a lack of radical solutions. Epidural electrical stimulation (EES) as an emerging treatment technology, has been shown in a number of international clinical studies on the SCI significant potential of motor function recovery, has significant curative effect, convenient, widely applicable, adjustable advantages, the rehabilitation of movement disorders after SCI has a lot of big benefits, in the spinal cord injury rehabilitation has great prospect. However, most of the domestic research are limited to the neurogenic bladder caused by abnormal autonomic nerve function after SCI, and mostly combine traditional Chinese and western medicine rehabilitation treatment, the emerging therapy of EES, this paper aims to review the basic principle of EES, potential mechanism of action and its c linical application progress in the treatment of movement disorders after SCI, in order to provide reference for the clinical application and scientific research of EES treatment of motor disorders after SCI.

Objective:To investigate the clinical efficacy of different administration methods for labor analgesia.Methods:This randomized controlled study was conducted on 152 parturient women who underwent vaginal delivery at the Xinxiang Central Hospital from February to June 2023. These women were divided into a control group and an observation group, with 76 women per group. Women in the control group received 3 mL of 0.1% ropivacaine hydrochloride after routine epidural catheterization, followed by 3 mL of analgesic drugs after 3 minutes. For those without any special reactions, an analgesic pump containing 9 mL of medication was used after 3 minutes until the end of labor. In the observation group, 3 mL of 0.1% ropivacaine hydrochloride was administered as a trial dose before the epidural catheter was inserted into the epidural space. After 3 minutes, the postpartum woman lay flat and was given another 3 mL of analgesic pump medication. After 3 additional minutes, an additional 9 mL of the medication was administered. The analgesic pump was connected and turned on until the catheter was removed after delivery. The epidural catheter failure rate was assessed in each group. Before epidural puncture (T 1), within 30 minutes after the onset of anesthesia (T 2), and at the time of full dilation of the cervix (T 3), Visual Analogue Scale scores, the number of patient attempts to additionally use an analgesia pump, maternal satisfaction, the number of cases requiring cesarean section conversion, the incidence of adverse reactions (such as nausea, vomiting, and fever), and Apgar scores for newborns were recorded in each group. Results:The epidural catheter failure rate in the observation group was 1.3% (1/76), which was significantly lower than 10.5% (8/76) in the control group ( χ2 = 5.79, P < 0.05). At T 2 and T 3, the Visual Analogue Scale score in the observation group was (0.89 ± 0.08) points and (2.1 ± 0.07) points, respectively, which were significantly lower than (2.55 ± 0.07) points and (3.35 ± 0.07) points in the control group ( t = 238.54, 133.17, both P < 0.001). The number of patient attempts to additionally use an analgesia pump in the observation group was (1.00 ± 0.84) counts, which was significantly less than (4.00 ± 0.65) counts in the control group ( t = 29.47, P < 0.001). Maternal satisfaction in the observation group was 100% (76/76), which was significantly higher than 92.1% (70/76) in the control group ( χ2 = 6.25, P < 0.05). The number of cases requiring cesarean section conversion in the observation group was 0 (0/76), which was significantly lower than 52.6% (4/76) in the control group ( χ2 = 4.10, P < 0.05). Conclusion:Administering a trial dose (3 mL) of 0.1% ropivacaine hydrochloride before placing the epidural catheter during labor analgesia can greatly increase the placement success rate of the catheter and enhance pain relief effectiveness. This has a certain clinical application value.

Objective:To investigate the effect of dexmedetomidine combined with chloroprocaine on epidural anesthesia for cesarean section.Methods:A total of 133 women who underwent epidural anesthesia for cesarean section at the Department of Anesthesiology, Jinhua People's Hospital between January 2020 and December 2022 were included in this prospective case-control study. These women were divided into a ropivacaine group ( n = 66) and a chloroprocaine group ( n = 67) using a random number table method. The ropivacaine group received epidural anesthesia with dexmedetomidine combined with ropivacaine, while the chloroprocaine group received epidural anesthesia with dexmedetomidine combined with chloroprocaine. The anesthesia effect (anesthesia onset time, anesthesia duration, peak effect time), changes in blood pressure (systolic blood pressure, diastolic blood pressure), adverse reactions (shivering, nausea and vomiting, urinary retention, skin itching, numbness of lower limbs), and satisfaction with muscle relaxation were observed in the two groups. Results:The anesthesia onset time, anesthesia duration, and peak effect time in the chloroprocaine group were (6.91 ± 1.54) minutes, (61.54 ± 5.31) minutes, and (11.79 ± 4.12) minutes, respectively, which were significantly shorter than those in the ropivacaine group [(9.65 ± 1.92) minutes, (83.57 ± 6.69) minutes, (18.32 ± 4.81) minutes, t = 9.08, 21.05, 8.41, all P < 0.001). The systolic and diastolic blood pressure increased in both groups after 10 minutes of anesthesia and at the end of the procedure; however, the increments observed in the chloroprocaine group were relatively smaller. There were significant differences in systolic and diastolic blood pressure, measured at 10 minutes of anesthesia and at the end of the procedure, between the two groups ( F = 7.36, P < 0.001; F = 5.12, P = 0.001). There were significant differences in systolic and diastolic blood pressure between different time points ( F = 10.03, P < 0.001; F = 6.72, P < 0.001). The group-by-time interaction effect on systolic and diastolic blood pressure was also highly significant ( F = 9.83, P < 0.001; F = 8.01, P < 0.001). The chloroprocaine group exhibited a significantly lower incidence of adverse reactions compared with the ropivacaine group [4.48% (3/67) vs. 15.15% (10/66), χ2 = 4.29, P < 0.05). Additionally, the chloroprocaine group had a significantly higher satisfaction rate with muscle relaxation compared with the ropivacaine group [94.03% (63/66) vs. 81.82% (54/66), Z = 5.73, P < 0.05]. Conclusion:The combination of dexmedetomidine and chloroprocaine offers remarkable benefits in epidural anesthesia for cesarean sections. This combined approach not only enhances epidural anesthesia but also stabilizes the blood pressure of puerperants, reduces adverse reactions, and provides exceptional muscle relaxation. It deserves consideration for clinical application.

Objective:To explore application value of combined spinal-epidural anesthesia under the assistant of ultrasound location on lower limb fractures.Methods:A total of 80 patients with lower limb fractures who underwent surgical treatment at the 901th Hospital of the PLA Joint Service Support Force from July 2020 to July 2022 were selected,and they were divided into a control group and an observation group according to the random number table method,with 40 cases in each group.The control group received epidural anesthesia under ultrasound,and the observation group received the combined lumbar-epidural anesthesia under ultrasound.The lidocaine dosage,anesthetic block time,anesthesia onset time and dosage of additional analgesic,as well as the changes in vital signs such as diastolic blood pressure(DBP),systolic blood pressure(SBP)and heart rate(HR),between the two groups of patients were compared.In addition,the thrombelastogram,anesthetic effects and occurrence of adverse reactions also were compared between two groups.Results:The lidocaine dosage,anesthetic block time,anesthesia onset time and dosage of additional analgesic of the observation group were significantly higher than those in the control group,and the differences were statistically significant(t=8.657,23.483,12.785,16.600,P<0.05),respectively.There were no statistically significant differences in DBP,SBP,HR,comprehensive coagulation index(CI),reaction time(R),formation time of blood cell agglutination(K)and incidence of adverse reactions between two groups before anesthesia,at the 30th minute after anesthesia and after the end of anesthesia(P>0.05).Compared with α angle before anesthesia and at the 30th minute after anesthesia,the α angles of the two groups increased significantly after anesthesia(t=6.564,5.783,P<0.05),respectively.The mechanical strength(G)of the blood clot and the maximum amplitude(MA)of chromatogram of the observation group were significantly reduced after completed anesthesia,and the differences of them were statistically significant(t=8.480,4.236,P<0.05),respectively.There were no significant differences between G and MA in the control group(P>0.05).The total effective rate of the observation group was 97.50%,which was significantly higher than 80.00% of the control group,and the difference was statistically significant(x2=4.507,P<0.05).Conclusion:The combined spinal-epidural anesthesia under the assistant of ultrasound location can effectively reduce the amount of anesthetics in patients with lower limb fracture,which has fast block and effect.In addition,it has little influence on circulatory function,and it is less likely to form a hypercoagulable state.It has better clinical anesthesia effect.

OBJECTIVE To investigate the effects of 0.2% chloroprocaine combined with ropivacaine on epidural labor analgesia and median effective concentration (EC50) of ropivacaine. METHODS Totally 67 parturients who scheduled for vaginal delivery and required epidural labor analgesia were collected from our hospital from July to October 2023 and randomly divided into RL group (33 cases) and R group (34 cases). The concentration of ropivacaine was determined by modified Dixon sequential method. RL group was given 0.2% Chloroprocaine hydrochloride injection+Ropivacaine hydrochloride injection+0.4 μg/mL Sufentanil citrate injection; R group was given Ropivacaine hydrochloride injection+0.4 μg/mL Sufentanil citrate injection. EC50 of ropivacaine, analgesic effect during delivery, total dosage of analgesic drugs, analgesic satisfaction score, the incidence of adverse reactions, delivery status, and Apgar score of newborns were observed in two groups. RESULTS EC50 of ropivacaine, onset time, remedial analgesia rate, the incidence of perineal distension and breakthrough pain and total dosage of analgesic drugs of RL group were significantly lower than R group, and analgesic satisfaction score was significantly higher than R group (P<0.05). There was no statistical significance in the incidence of adverse reactions such as numbness, weakness, and chills in the lower limbs, or the duration of labor, amount of bleeding, mode of delivery, and Apgar score of newborns between 2 groups (P>0.05). CONCLUSIONS For epidural labor analgesia, 0.2% chloroprocaine combined with ropivacaine can reduce EC50 of ropivacaine, improve analgesia effect and have good safety.

Introducción: La hipotensión intracraneal espontánea es un síndrome causado por la disminución del volumen de líquido cefalorraquídeo consecuencia de su fuga al espacio extradural. Aunque la ICHD-3 proporciona un alto nivel de especificidad diagnóstica, esta enfermedad puede manifestarse de forma atípica. Hasta en un 30% no es posible establecer el punto de escape, pero con el refinamiento de los exámenes de imágenes este porcentaje se ha reducido a un 15%-20%. Actualmente, su manejo no se encuentra estandarizado y las recomendaciones se basan en evidencia de limitada calidad metodológica, además de la variabilidad de protocolos entre distintos centros. Desarrollo En esta revisión actualizamos los procedimientos diagnósticos y terapéuticos. Por un lado, analizamos el rol de la resonancia nuclear magnética de encéfalo y médula espinal completa como primer paso diagnóstico y, por otro lado, señalamos los exámenes destinados a determinar la fuga de líquido cefalorraquídeo. Tal es el caso de la mielo-resonancia, la mielo-tomografía computarizada, tanto estándar, dinámica y por sustracción digital, además de la cisternografía con 111-Indium-DPTA. Sin embargo, determinar cuál de estos exámenes es el óptimo es objeto de debate. Lo mismo ocurre con el tratamiento: reposo; parche sanguíneo epidural a ciegas, parche guiado por fluoroscopia o tomografía computarizada, parche de fibrina; o cirugía. Conclusiones Se requiere de una mayor investigación, especialmente con trabajos multicéntricos controlados, para una mejor comprensión de la fisiopatología, el diagnóstico por imágenes, los enfoques terapéuticos y evaluación objetiva de los resultados clínicos. Solo así se establecerán pautas diagnósticas y de tratamiento validadas.


Introducction: Spontaneous intracranial hypotension is a syndrome caused by decreased CSF volume secondary to its leakage into the extradural space Although ICHD-3 provides a high level of diagnostic specificity, manifestations may be atypical, making diagnosis challenging. The site of leakage may be undetermined in point Up to 30% of cases, although with recent refinement of imaging, this percentage has been reduced to 15-20%. Currently, management is not standardized and recommendations are based on inconclusive evidence, with variability of protocols between centres. Development. In this review, we update diagnostic and therapeutic procedures. We analyse the role of whole brain and spinal cord MRI as a first investigation and review tests aimed at determining cerebrospinal fluid leakage, such as MRI myelography, conventional CT myelography, dynamic CT myelography, and digital subtraction CT myelography, as well as 111-Indium-DPTA cisternography. Determining optimal use of these investigations remains a matter of debate. The same is true for treatment: rest, blind epidural blood patch, fluoroscopy or CT-guided epidural blood patch, fibrin patch and surgery are discussed. Conclusión: Further research, especially multicentre controlled studies, is required to improve understanding of pathophysiology, diagnostic imaging, therapeutic approaches and to objectively assess clinical outcomes. Only then will diagnostic and treatment guidelines be evidence-based.

Introducción: La utilidad de las técnicas de anestesia regional se ha mostrado por muchos años, estas son las de elección por los beneficios que aporta, como es el caso de la cesárea, aunque no están exentas de complicaciones, y las neurológicas que son las más temidas por la gravedad del cuadro clínico presentado y los posibles desenlaces. Objetivo: Describir la evolución clínica de la puérpera con diagnóstico de neumoencéfalo después de anestesia neuroaxial peridural. Presentación de caso: Se trata de una paciente puérpera con antecedente de cefalea migrañosa que después de ser sometida a una anestesia peridural, comienza con dolor de cabeza ligero a moderado que en un principio aliviaba con analgésicos, después hace una convulsión tónica clónico generalizada que fue necesaria intubar y trasladar a terapia intensiva. Mediante la tomografía axial computarizada se detecta neumoencéfalo de pequeñas proporciones en región parietal, además, de signos de edema cerebral, el cuadro evoluciona satisfactoriamente, la paciente a las horas es trasladad a sala donde se reencuentra con su bebe y días después es dada de alta sin secuelas. Conclusiones: El neumoencéfalo como complicación de la anestesia peridural en la paciente gestante, generalmente no tienen gran repercusión clínica y desaparece solo en un período no mayor de 72 horas, pero en ocasiones puede presentarse como complicación grave que puede acarrear daño neurológico permanente e incluso la muerte.

Objective: To describe the evolution of a postpartum patient who was diagnosed with Pneumocephalus after epidural anesthesia. Clinical case: Postpartum patient with a history of migraine headache, which after undergoing epidural anesthesia, began with light to moderate headache that was initially relieved with analgesics, then had a generalized tonic-clonic seizure that required intubation and transfer to the intensive therapy. Using computed axial tomography, small Pneumocephalus was detected in the parietal region in addition to signs of cerebral edema. The condition progressed satisfactorily. Within hours, the patient was transferred to the ward where she was reunited with her baby and day slater she was discharged without squeals. Conclusions: Pneumocephalus as a complication of epidural anesthesia in pregnant patients generally does not have great clinical repercussions and disappears only in a period of no more than 72 hours, but sometime sit can occur as a serious complication that can lead to permanent neurological damage and even death, death.

Introducción: El neumoencéfalo (sinonimia: aerocele o neumatocele intracerebral), se define como la presencia de gas dentro de cualquiera de los compartimentos intracraneales (intraventricular, intraparenquimatosa, subaracnoidea, subdural y epidural). Objetivo: Describir los hallazgos clínicos, estudios complementarios, conducta terapéutica y evolución de un caso con neumoencéfalo como complicación de bloqueo regional epidural por radiculopatía lumbosacra. Presentación de caso: Se presentó un paciente masculino de 57 años de edad que comenzó con un cuadro súbito de desorientación, excitabilidad psicomotriz y convulsiones tónico-clónicas, a partir de una inyección epidural de metilprednisolona como método analgésico. Conclusiones: El caso presentado exhibió manifestaciones neurológicas inespecíficas, la aparición súbita posterior al proceder invasivo hizo sospechar en un evento neurológico agudo o fenómeno tromboembólico. Los estudios complementarios como la tomografía axial computarizada craneal simple, permitió su diagnóstico para tener una conducta consecuente. El manejo conservador del neumoencéfalo como complicación del uso de anestesia epidural, constituyó una conducta terapéutica eficaz y repercutió en la satisfactoria evolución del paciente(AU)


Introduction: Pneumocephalus (synonym: aerocele or intracerebral pneumatocele), is defined as the presence of gas within any of the intracranial compartments (intraventricular, intraparenchymal, subarachnoid, subdural and epidural). Objective: To describe the clinical findings, complementary studies, therapeutic conduct and evolution of a case with pneumocephalus as a complication of regional epidural block due to lumbosacral radiculopathy Case presentation: A 57-year-old male patient was presented who began with a sudden episode of disorientation, psychomotor excitability and tonic-clonic seizures, following an epidural injection of methylprednisolone as an analgesic method. Conclusions: The case presented exhibited non-specific neurological manifestations, the sudden appearance after the invasive procedure raised suspicion of an acute neurological event or thromboembolic phenomenon. Complementary studies such as simple cranial computed axial tomography, allowed its diagnosis to have a consistent conduct. The conservative management of pneumocephalus as a complication of the use of epidural anesthesia constituted an effective therapeutic approach and had an impact on the patient's satisfactory evolution(AU)

Introducción: La cefalea postpunción meníngea (CPPM) posterior a la anestesia raquídea es una de las complicaciones más frecuentes asociadas a factores intrínsecos del paciente y de la técnica anestésica. Objetivo: Describir la frecuencia y los factores asociados con el desarrollo de la cefalea postpunción meníngea. Materiales y métodos: Serie retrospectiva de pacientes que ingresaron a un hospital de segundo nivel y se les confirmó el diagnóstico de cefalea secundaria a la anestesia raquídea. Resultados: Serie de 49 casos, 88 % de sexo femenino y 12 % de sexo masculino, con una edad media de 27,7 años. Los procedimientos quirúrgicos con desenlace de CPPM fueron: cirugías de ginecología y obstetricia (63 %), cirugías de urgencias de otras especialidades (28 %) y cirugías electivas (8 %). La técnica anestésica se realizó con agujas biseladas tipo Quincke calibre 25 gauge (G) en 14%, calibre 26 G 33 % y 27 G 53 %. El 51 % se realizó en posición de sedestación y el 49 % en decúbito lateral izquierdo. El 10% de los casos se manejó con parche hemático, en tanto que el antecedente de migraña se presentó en el 8 %. Discusión: En la actualidad, el uso de agujas con diseño de punta cónica es el estándar de oro, ya que permite obtener resultados confiables y disminuye complicaciones como la CPPM. Conclusión: La CPPM luego de una anestesia espinal se relacionó con factores como la edad (joven), el sexo (femenino) y el uso de agujas biseladas. Los otros factores de riesgo identificados fueron poco concluyentes, aunque no se pueden descartar, debido a la naturaleza de este estudio.


Introduction: Post dural puncture headache (PDPH) following spinal anesthesia is one of the most frequent complications associated with intrinsic patient and anesthetic technique factors. Objective: To describe the frequency and associated factors related to the development of PDPH. Materials and methods: Retrospective series of patients admitted to a second level hospital with a confirmed diagnosis of headache secondary to spinal anesthesia. Results: Series of 49 cases, 88 % female and 12 % male, mean age 27.7 years. The surgical procedures resulting in CPPM were gynecology and obstetrics surgeries 63 %, emergency surgeries of other specialties 28 % and elective surgeries 8 %. The anesthetic technique was performed with beveled needles Quincke type 25 gauge (G) in 14 %, 26 G gauge 33% and 27 G 53 %. In the seated position 51 % and in the left lateral decubitus position 49% were performed. A blood patch was used in 10 % of the cases and a history of migraine was present in 8 %. Discussion: The use of needles with conical tip design is currently the gold standard, they give reliable results and reduce complications such as PDPH. Conclusion: PDPH after spinal anesthesia was related to factors such as age (young), sex (female) and the use of traumatic needles. The other risk factors identified were inconclusive, although they cannot be ruled out due to the nature of this study.

RESUMEN El traumatismo craneoencefálico es un evento ocasionado por la fuerza sobre el continente que guarda al contenido cerebral, que hace que la inercia ocasione lesiones por aceleración y desaceleración produciendo hemorragias y/o fracturas Estas pueden generar una serie de complicaciones como el absceso epidural que al estar muy cerca al cerebro constituye una emergencia, siendo complejo su manejo y seguimiento. En esta oportunidad mostramos el caso de un paciente con absceso epidural ocasionado por traumatismo craneoencefálico y manejo neuroquirúrgico, con buena evolución clínica.


ABSTRACT Cranioencephalic trauma is an event caused by force on the container that holds the brain content, which causes inertia to cause acceleration and deceleration injuries, producing hemorrhages and/or fractures; These can generate a series of complications such as epidural abscess which, being very close to the brain, constitutes an emergency for its management and follow-up15. On this occasion we show the case of a patient with epidural abscess caused by cranioencephalic trauma and neurosurgical management.

Resumen: Actualmente, la anestesia neuroaxial es preferida por sus múltiples ventajas con respecto al manejo del dolor. El procedimiento involucra la colocación de un catéter epidural en el espacio peridural, una complicación muy rara es la ruptura de éste. Se estima una incidencia de 0.002 a 0.004%. El presente trabajo reporta el caso de un paciente masculino de 77 años sometido a una resección transuretral de próstata, que presenta ruptura del catéter durante el retiro. La tomografía reportó la presencia del catéter a nivel subaracnoideo, sin presencia de síntomas en el paciente. Se egresó y a los seis meses de seguimiento no presentaba síntomas. Es de importancia comprender que siempre se debe notificar al paciente en caso de que esto suceda y después realizar estudios de imagen para su localización. Posteriormente se debe valorar la presencia de síntomas y, si existen, considerar una laminectomía para su retiro.


Abstract: Nowadays, neuraxial anesthesia is preferred due to its multiple advantages related to pain management. This procedure involves placing an epidural catheter within the epidural space, with rupture of the catheter being an uncommon complication. Its incidence is around 0.002 to 0.004%. The present work reports the case of a 77-year-old male who underwent a transurethral prostate resection, where the catheter broke while being removed. CT scan reported the presence of the catheter within subarachnoid space, without the patient presenting symptoms. It is important to understand that notifying the patient is a priority in case this complication happens, and then order imaging studies to locate the catheter. Finally, if symptoms are present, a laminectomy should be assessed to remove the remaining piece.

El trabajo de parto lleva con este la experiencia que causa uno de los dolores más intensos que se han descrito. En el Instituto de Seguridad Social de El Salvador (ISSS) se registran aproximadamente 600 a 700 partos vaginales por mes y de estos aproximadamente 10-20% solicitan analgesia para trabajo de parto, la cual el servicio de anestesia provee a través del abordaje epidural. Es conocido que se considera que el medicamento ideal para la analgesia de trabajo de parto es la bupivacaína o lidocaína 3, sin embargo, es de especial importancia describir el puntaje de APGAR de los recién nacidos de madres que recibieron cualquiera de estos dos fármacos por la vía ya mencionada. Objetivo: Determinar cómo influye el uso de lidocaína y bupivacaína como anestésico epidural sobre el puntaje APGAR posterior al nacimiento en mujeres embarazadas en fase activa del trabajo de parto con necesidad de analgesia epidural. Métodos: El presente es un estudio observacional descriptivo retrospectivo de fuentes primarias. El servicio de anestesiología del Hospital Materno Infantil Primero de Mayo cuenta con un registro sobre cuántos y a cuáles pacientes se les administra analgesia epidural por lo que el equipo investigador realizó una revisión de la técnica anestésica y se describe el Apgar al final del parto por vía vaginal en aquellas pacientes que recibieron analgesia epidural con lidocaína o bupivacaína. Se realizó el filtrado con la base de datos de Excel utilizando criterios de inclusión y exclusión, logrando incluir finalmente 81 pacientes en el análisis de los datos. Se encontró durante el escrutinio de estos que a 25 de estas se les realizó cesárea de emergencia y en 6 de estos mismos expedientes no se consignó de manera completa todos los datos necesarios para el análisis de ello, por lo que se utilizaron finalmente 51 expedientes clínicos para la realización de la presente investigación. Resultados: El APGAR promedio de los pacientes que recibieron analgesia de trabajo de parto con bupivacaína es de 8.75 y 8.9 al minuto y cinco minutos respectivamente y el promedio de APGAR alcanzado por los recién nacidos de pacientes que recibieron analgesia de trabajo de parto con lidocaína es de 9 puntos al minuto y cinco minutos. Conclusión: El promedio de APGAR en pacientes que recibieron bupivacaína es de 8.75 y el de lidocaína es de 9. (provisto por Infomedic International)


Labor brings with it the experience that causes one of the most intense pains ever described. At the Social Security Institute of El Salvador (ISSS) approximately 600 to 700 vaginal deliveries are registered per month and of these approximately 10-20% request labor analgesia, which the anesthesia service provides through the epidural approach. It is known that the ideal drug for labor analgesia is considered to be bupivacaine or lidocaine 3, however, it is of special importance to describe the APGAR score of newborns born to mothers who received either of these two drugs by the aforementioned route. Objective: To determine how the use of lidocaine and bupivacaine as epidural anesthetic influences the post-birth APGAR score in pregnant women in the active phase of labor requiring epidural analgesia. Methods: The present is a retrospective descriptive observational observational study from primary sources. The anesthesiology service of the Hospital Materno Infantil Primero de Mayo has a record of how many and to which patients epidural analgesia is administered, so the research team conducted a review of the anesthetic technique and described the Apgar at the end of vaginal delivery in those patients who received epidural analgesia with lidocaine or bupivacaine. Filtering was performed with the Excel database using inclusion and exclusion criteria, finally including 81 patients in the data analysis. It was found during the scrutiny of these that 25 of them had undergone emergency cesarean section and in 6 of these same records not all the data necessary for the analysis were completely recorded, so that 51 clinical records were finally used for the present investigation. Results: The average APGAR of the patients who received labor analgesia with bupivacaine was 8.75 and 8.9 at one minute and five minutes respectively, and the average APGAR achieved by the newborns of patients who received labor analgesia with lidocaine was 9 points at one minute and five minutes. Abstract Emery-Dreifuss Muscular Dystrophy is a very rare type of muscular dystrophy, associated with contractures, atrophy, and muscle weakness, besides cardiomyopathy with severe arrhythmias. Published studies focusing on this disorder are scarce. We describe the anesthetic management of a male patient with Emery-Dreifuss Muscular Dystrophy, to be submitted to umbilical and inguinal hernioplasty and hydrocele repair under epidural anesthesia. The anesthesia approach enabled us to circumvent the patient's susceptibility to malignant hyperthermia and his potentially difficult airway, in addition to maintaining hemodynamic stability. The day after surgery the patient resumed walking, and two days later he was discharged from the hospital.

Resumen: Pocas han sido las Figuras españolas que han aportado alguna interesante novedad al mundo de la anestesia, pero ninguna tan injustamente tratada y olvidada como la de Pagés, verdadero pionero de la anestesia epidural, que él denominaría «anestesia metamérica¼ en su artículo publicado en el mes de marzo de 1921 en la Revista Española de Cirugía, fundada por él mismo. Años después, en 1931, Dogliotti, profesor de Cirugía de Módena publicó sus experiencias sobre la anestesia epidural, a la que llamó «anestesia peridural segmentaria¼, ignorando el trabajo de Pagés publicado 10 años antes. El trabajo de Dogliotti fue rápidamente reconocido, asumiendo todos los méritos de la paternidad de la técnica epidural, quedando Pagés relegado a un olvido injusto que con este trabajo tratamos de reparar. La idea central de este texto versará sobre el reconocimiento de Pagés como verdadero promotor del abordaje epidural con fines quirúrgicos. Destacar sus ideas innovadoras sobre el bienestar del paciente y la minimización de los efectos adversos de las técnicas anestésicas y quirúrgicas, así como valorar una obra que, aunque corta, bien podría incluirse entre los mejores cirujanos españoles de principio del siglo XX, un cirujano con alma de anestesiólogo.


Abstract: Few Spanish Figures have contributed any interesting novelty to the world of anesthesia. But none so unjustly treated and forgotten such as Pagés, a true pioneer of epidural anesthesia, which he would call «metameric anesthesia¼ in his article published in March 1921 in the Spanish Journal of Surgery, founded by himself. Later, in 1931, Dogliotti, Modena Professor of Surgery published his experiences on epidural anesthesia, which he called «segmental peridural anesthesia¼, ignoring Pagés work published 10 years earlier. Dogliotti's work was quickly recognized, assuming all the merits of the paternity of the epidural technique, leaving Pagés relegated to an unjust oblivion that with this work we try to repair. The central idea of this text will focus on the recognition of Pagés as a true promoter of the epidural approach for surgical purposes. Highlight his innovative ideas about patient well-being and minimizing the adverse effects of anesthetic and surgical techniques. As well as assessing a work that, although short, could well be included among the best Spanish surgeons of the early twentieth century, a surgeon with the soul of an anesthesiologist.

Abstract Dural puncture is either diagnosed by unexpectedly profound response to medication test dose or development of a postpartum postural headache. Epidural blood patch is the gold standard for treatment of PDPH when conservative management fails. However, postpartum headaches can be resistant to multiple epidural blood patches. In such cases, preexisting intracranial processes should be considered and ruled out. We report here the unique case of a pregnant patient who developed a resistant headache in the postpartum period related to an incidental intracranial aneurysm. Subsequent treatment with endovascular embolization adequately relieved her symptoms. Early surgical consultation and a multidisciplinary team approach involving neurology and neuroimaging is required for successful management of patients such as the one described here.

OBJECTIVE To investigate the effect of ropivacaine combined with dexmedetomidine on postoperative analgesia in women undergoing cesarean section, and to explore the feasibility of the opioid-free analgesia mode after cesarean section under spinal-epidural anesthesia. METHODS Totally 80 women undergoing cesarean section were randomly divided into observation group (ropivacaine combined with dexmedetomidine for analgesia) and control group (ropivacaine combined with opioid drug sufentanil for analgesia) , with 40 cases in each group. The exercise and rest score in visual analogue scale (VAS) within 48 hours after operation, the use of analgesia pump (the time of first analgesia pump pressing, the times of analgesia pump pressing within 24 hours and 48 hours after operation), the time of block (the onset time of spinal anesthesia sensory block, the time to the highest level of spinal anesthesia sensory block, the time of sensory recovery and the time of movement recovery) , the time of prognosis (the time of gastrointestinal ventilation recovery, the time of getting out of bed and the hospitalization time), and the incidence of adverse events were compared in 2 groups. RESULTS Finally, 64 parturients (32 in the observation group and 32 in the control group) were involved in the analysis. Compared with the control group, the recovery time of sensation and movement were significantly prolonged, the ventilation time was significantly shortened, and the incidence of nausea, vomiting and abdominal distension was significantly decreased in the observation group (P<0.05) . There was no significant difference in the other indexes between the two groups (P>0.05). CONCLUSIONS Ropivacaine combined with dexmedetomidine under spinal-epidural anesthesia could provide similar analgesic effect as combined with opioids drug sufentanil, shorten the time of gastrointestinal ventilation recovery, and reduce the incidence of nausea,vomiting and abdominal distension, with no increased risk of low blood pressure or urinary retention.
