الملخص

【Objective】 Virtual reality (VR) technology is closely related to eye vision. With the development and progress of hardware and software equipment, VR has been applied widely in the field of ophthalmology. This article describes the application of VR technology in the clinical research and ophthalmology education, reviews the current research results and advantages of this new technology, including the new curative effect in amblyopia/strabismus, myopia and glaucoma, as well as research on the technology’s application in cataract surgery training and ophthalmology education. The article also discusses the dangers and difficulties of VR application and predicts its future application trend. In view of the shortcomings of VR in current research applications, the paper discusses and looks forward to provide powerful strategies for amblyopia, myopia and other ophthalmic diseases and clinical research.

AIM: To explore the neuro-ophthalmological characteristics of acute macular neuroretinopathy(AMN)after SARS-CoV-2 infection.METHODS: A total of 8 patients(14 eyes), including 6 females and 2 males, who were diagnosed with AMN in the neuro-ophthalmology department of Xi'an No.1 Hospital(The First Affiliated Hospital of Northwest University)from December 27, 2022 to February 1, 2023 were included in the study. All patients had a history of SARS-CoV-2 infection before the disease, and the results of best corrected visual acuity(BCVA), non-contact indirect intraocular pressure measurement, fundus color photography, near infrared(IR), spectral-domain optical coherence tomography(SD-OCT), OCT angiography(OCTA), fundus fluorescein angiography(FFA), indocyanine green angiography(ICGA), visual field, visual evoked potential(VEP), and electroretinogram(ERG)were collected. Furthermore, the neuro-opthalmology characteristics of the included patients were analyzed and summarized.RESULTS: The included 8 patients aged from 20 to 43, with an average age of(30±6.63)years old. The patients had a history of SARS-CoV-2 infection 3 to 11(mean 5±3.51)d before the disease, and 6 out of 8 patients developed visual symptoms within 5 d of infection with SARS-CoV-2, with manifestated with decreased vision or visual scotoma. The visual acuity varied from 0.08 to 1.0, with visual field defect characterized by central, paracentral or peripheral scotoma. VEP showed prolongation latency of P100 or P2, and ERG revealed impaired function of retinal photoreceptor cell. In the early stage of the disease, the size and shape of early visual acuity, visual field, and extraretinal lesions in patients with AMN associated with SARS-CoV-2 infection may not match, and the lower the visual acuity, the later the VEP peaks.CONCLUSION: The neuro-ophthalmic features of SARS-CoV-2 infection-associated AMN require the attention of clinicians. In addition to multi-mode fundus imaging, clinicians should use a variety of methods to comprehensively evaluate visual function and prognosis of patients.

ChatGPT is a large language models(LLMs)that uses deep learning techniques to produce human-like responses to natural language inputs. It belongs to the family of generative pre-training transformer(GPT)models currently publicly available developed by OpenAI in November 2022. ChatGPT is capable of capturing the nuances and intricacies of human language, generating appropriate and contextually relevant responses. It can assist medical professionals in various tasks, such as research, diagnosis, patient monitoring, and medical education, from identifying research programs to assisting in clinical and laboratory diagnosis, to know new developments in their fields and scientific writing. ChatGPT has also attracted increasing attention and widely used in ophthalmology. However, the use of ChatGPT and other artificial intelligence tools in such tasks comes now with several limitations, ethical and legal concerns, such as credibility, plagiarism, copyright infringement, and biases. Future research can focus on developing new methods to mitigate these limitations while harnessing the benefits of ChatGPT in medicine and related aspects.

This paper provides a comprehensive analysis of the current state of intelligent ophthalmology in China, including technological advancements, academic exchange platforms, policy support, future challenges, and potential solutions. Technologically, remarkable progress have been made in various areas of intelligent ophthalmology in China, including diabetic retinopathy, fundus image analysis, and crucial aspects such as quality assessment of medical artificial intelligence products, clinical research methods, technological evaluation, and industrial standards. Researchers are constantly improving the safety and standardization of intelligent ophthalmology technology by formulating clinical application guidelines and standards. Academic exchange platforms have been established to provide extensive collaboration opportunities for professionals across diverse fields, and various academic journals serve as publication platforms for intelligent ophthalmology research. Regarding public policy, the Chinese government has not only established a supportive policy environment for the advancement of intelligent ophthalmology through various documents and regulations, but provided a legal basis and management framework. However, there are still challenges to overcome, such as technological innovation, data privacy and security, outdated regulations, and talent shortages. To tackle these issues, there is a requirement for increased technological research and development, the establishment of regulatory frameworks, talent cultivation, and greater awareness and acceptance of new technologies among patients. By comprehensively addressing these challenges, intelligent ophthalmology in China is expected to continue leading the industry's global development, bringing more innovation and convenience to the field of ophthalmic healthcare.

AIM: To evaluate the performance of three distinct large language models(LLM), including GPT-3.5, GPT-4, and PaLM2, in responding to queries within the field of ophthalmology, and to compare their performance with three different levels of medical professionals: medical undergraduates, master of medicine, and attending physicians.METHODS: A total of 100 ophthalmic multiple-choice tests, which covered ophthalmic basic knowledge, clinical knowledge, ophthalmic examination and diagnostic methods, and treatment for ocular disease, were conducted on three different kinds of LLM and three different levels of medical professionals(9 undergraduates, 6 postgraduates and 3 attending physicians), respectively. The performance of LLM was comprehensively evaluated from the aspects of mean scores, consistency and confidence of response, and it was compared with human.RESULTS: Notably, each LLM surpassed the average performance of undergraduate medical students(GPT-4:56, GPT-3.5:42, PaLM2:47, undergraduate students:40). Specifically, performance of GPT-3.5 and PaLM2 was slightly lower than those of master's students(51), while GPT-4 exhibited a performance comparable to attending physicians(62). Furthermore, GPT-4 showed significantly higher response consistency and self-confidence compared with GPT-3.5 and PaLM2.CONCLUSION: LLM represented by GPT-4 performs well in the field of ophthalmology, and the LLM model can provide clinical decision-making and teaching aids for clinicians and medical education.

With the reform of the national medical and health system entering a new stage of high-quality development of public hospitals,the large-scale implementation of day surgery in hospitals is imminent in the face of increasing patient demand.In this paper,the medical administration management and medical insurance policies related to day surgery in China and their im-pacts were sorted out,and the example of large-scale implementation of day surgery by a specialized ophthalmic medical institu-tion through pre-hospitalization mode was used to illustrate how to use management tools to break through the bottleneck in the promotion process of day surgery,and the positive effect of large-scale development of day surgery on both doctors and patients was expounded.

Multimodal data fusion(MDF),a data processing technology in the field of artificial intelligence,refers to a comprehensive processing technology of multimodal data such as pictures,texts,audio and videos by computer.Compared with traditional single-mode data processing,MDF has higher accuracy and reliability in the analysis of described objects.In recent years,the application of MDF in ophthalmology has made great progress,and studies have shown that this tech-nology can improve the accuracy of artificial intelligence-assisted diagnosis.This paper review the principles and advanta-ges,brief history of development,architecture and technical difficulties of MDF,as well as its application in ophthalmologic clinics and problems to be solved.

Currently, the dominating treatment of corneal blindness is corneal transplantation, but the shortage of corneal donors has been a major problem in corneal transplantation.Compared with the traditional preparation method of artificial cornea, with the emergence of new materials, new technologies and new commercial activities, hydrogel provides a new possibility for the preparation of artificial cornea.Hydrogel is similar to the equivalent of extracellular matrix, with high biocompatibility, low immunogenicity and other characteristics, so it has been popularized and applied in many industries.As an important material of regenerative medicine, hydrogel has shown a strong application prospect in pharmaceutical research, 3D cell culture, stem cell research, 3D printing bio-ink and other fields.The main materials used for generating hydrogels are hyaluronic acid, gelatin, sodium alginate, etc.A lot of researches on using hydrogels as materials combined with different 3D printing technologies to prepare artificial corneas have been carried out and certain technical and theoretical breakthroughs have been made.This article briefly reviewed the research progress of hydrogel combined with 3D printing in the preparation of artificial cornea.

Microfluidic chip or lab-on-a-chip is a multidisciplinary cross-technology. Among them, organ-on-a-chip technology enables precise regulation of cells and microenvironment at micron level.This tecnology is expected to simulate in vivo human physiology and overcome the shortcomings of traditional animal models and cell culture techniques.In ophthalmology, organ-on-a-chip models are primarily focused on creating biomimetic models of the cornea, retina and posterior chamber to study diseases such as dry eye, glaucoma, age-related macular degeneration and diabetic retinopathy.In addition, continuous monitoring and real-time diagnosis of tear and intraocular fluid biomarkers using microfluidic chips have become a current research hot topic.The microfluidic chips also have a wide range of applications in drug analysis, drug development, and drug screening.This article reviews the recent progress and shortcomings of microfluidic chip in in vitro model construction, point-of-care testing and drug development, and discusses its future development in ophthalmology.

In September 2022, the American Academy of Ophthalmology released the latest version of Refractive Errors Preferred Practice Pattern? (PPP). It provides authoritative guidance for the diagnosis and treatment of refractive errors based on the best evidence-based medical updates.The new version of Refractive Errors PPP continues the rigorous, comprehensive and detailed features of the 2017 version.Updates have been made to the definition, epidemiology, classification, and diagnosis and correction of refractive errors.For example, the new version of Refractive Errors PPP defines high refractive errors as myopia≥-6.0 D, hyperopia≥+ 3.0 D, astigmatism≥-3.0 D, adds content on myopia prevention and control, recommends outdoor activities for at least 2 hours per day, selects appropriate optical prevention and control methods, and considers the use of low-concentration atropine eye drops to delay the onset and development of myopia.It enriched the evidence-based medical evidence that myopia is an interactive influence of genetic and environmental factors, and emphasized the future pandemic trend of high myopia, and the global public health significance of delaying the progression of myopia.In addition, the new version of Refractive Errors PPP emphasizes for the first time that myopia can cause irreversible visual impairment, suggesting that it is necessary to delay the onset time of myopia in children and slow the speed of myopia progression.This article introduces and interprets the main contents and updates of the new edition of the Refractive Errors PPP.

ABSTRACT Purpose: The number of medical schools in Brazil has increased in recent years; however, vacancies for specialization in ophthalmology probably have not kept up with the growing demand. This study wants to estimate the increase in medical schools, the demand for ophthalmology specialization, and evaluate learning opportunities in ophthalmology. Methods: This was a retrospective study with research from the Ministry of Education and Brazilian Council of Ophthalmology database from 2002 to 2021. These data were checked through 120 notices published by the institutions in 2021. Results: The number of medical school vacancies increased by 370%, whereas the number of certified ophthalmology vacancies increased by 64%. There was an 11.4% misalignment between the Brazilian Council of Ophthalmology data in the Ministry of Education. Conclusion: The proportion of medical graduates has increased much more than opportunities for ophthalmology specialization. The effect on the search for unaccredited specialization positions is unknown, and policies for monitoring the specialization of ophthalmology vacancies should be established.


RESUMO Objetivo: Nos últimos 20 anos, o número de escolas médicas no Brasil aumentou, mas as vagas para especialização em Oftalmologia não acompanharam a demanda crescente. Este estudo quer estimar a demanda por especialização e avaliar a oferta de oportunidades de aprendizado em Oftalmologia. Métodos: Estudo epidemiológico com pesquisa em banco de dados provenientes do Ministério da Educação e Conselho Brasileiro de Oftalmologia. Estes dados foram checados através de 120 editais publicados pelos serviços de Residência em 2021. Resultados: De 2002 a 2021, o número de vagas em faculdades de Medicina aumentou 370%, enquanto o número de vagas certificadas de especialização em Oftalmologia aumentou 64%. Houve um desalinhamento de 11.4% entre os dados do Conselho Brasileiro de Oftalmologia e do Ministério da Educação. Conclusão: A proporção de graduados em Medicina aumentou muito mais do que a oferta de oportunidades de especialização em Oftalmologia, o impacto disto na busca por vagas de especialização não acreditadas é desconhecido, políticas de monitoramento das vagas de especialização em Oftalmologia devem ser estabelecidas.

ABSTRACT Medical specialties have recognized that breaking bad news assists clinical practice by mitigating the impact of difficult conversations. This scenario also encourages various studies on breaking bad news in ophthalmology since certain ocular diagnoses can be considered bad news. Thus, the objective is to review the scientific literature on breaking bad news in ophthalmology. The literature databases like MEDLINE/PUBMED, EMBASE, LILACS, SCOPUS, COCHRANE, and SCIELO, were screened for related research publications. Two independent reviewers read all the articles and short-listed the most relevant ones. Seven articles, in the formats of original article, review, editorial, oral communication, and correspondence, were reviewed. Conclusively it reveals that ophthalmologists are concerned with communicating bad news effectively but lack related studies. Nevertheless, there is a growing realization that training in breaking bad news can increase physicians' confidence during communication, thus, benefiting the therapeutic relationship with the patient and his family. Therefore, it would be valuable to include breaking bad news training in the curriculum of residencies.


RESUMO O reconhecimento sobre a comunicação de más notícias como mitigadora de conversas difíceis por outras especialidades médicas, incentiva o estudo desta temática na oftalmologia. Sendo assim, o objetivo deste estudo é revisar a produção de pesquisas científicas sobre a comunicação de más notícias em oftalmologia. Para isso, foi realizada uma revisão de literatura. As bases de dados utilizadas foram MEDLINE/PUBMED, EMBASE, LILACS, SCOPUS, COCHRANE e SCIELO. Dois revisores independentes leram todos os artigos e selecionaram a amostra final. Sete artigos foram escolhidos nos formatos de artigo original, revisão, editorial, comunicação oral e correspondência. Os oftalmologistas estão preocupados em comunicar as más notícias de forma eficaz, mas faltam estudos sobre o tema. No entanto, há uma crescente percepção de que o treinamento de comunicação de más notícias aumenta a confiança dos médicos na comunicação, beneficiando a relação terapêutica. Portanto, seria valioso incluir este treinamento no currículo das residências.

ABSTRACT Purpose: Diabetes mellitus is a leading cause of impaired vision. The objective of this study was to evaluate the feasibility of use of portable retinograph and remote analysis of images along with a virtual questionnaire for screening for diabetic retinopathy in basic health units in the city of Ribeirão Preto/SP during the Covid-19 pandemic. Methods: Standard Covid-19 protocol was followed during the screening. Blood pressure and capillary blood glucose were measured. Demographic and social data were collected through a standardized online questionnaire via smartphone. After pupillary dilation, fundal images were obtained with portable retinographs by trained ophthalmology residents. Two standardized 45° images were acquired: one posterior segment and another nasal to the optic nerve. Diabetic retinopathy was classified according to the Early Treatment Diabetic Retinopathy Study. Results: A total of 350 patients (64% female; 45% aged 55-70 years; 55% Caucasian) were evaluated. For 40.5% of patients, the campaign was the first opportunity for retinal evaluation; 47.56% had diabetes mellitus for >10 years. On repeat analysis of images stored in a cloud-based repository by retinal specialist, a 7.8% difference was observed in the Early Treatment Diabetic Retinopathy Study diabetic retinopathy classification, compared to the screening findings. Mild diabetic retinopathy was observed in 12.23%, moderate diabetic retinopathy in 6.31%, and proliferative diabetic retinopathy in 2.58% patients. Macular edema was present in 4.58% patients. Diabetic retinopathy was not detected in 72.78% patients. Conclusion: Use of portable retinographs together with telemedicine can provide efficient alternative to traditional methods for screening and diagnosis of diabetic retinopathy.


RESUMO Objetivo: A diabetes mellitus é considerada uma epidemia global e causa de baixa visual em países em desenvolvimento. Este estudo foi realizado com o objetivo de avaliar a viabilidade do retinógrafo portátil e análise remota de imagens associada a questionário virtual para o rastreio de retinopatia diabética em Unidades Básicas de Saúde da cidade de Ribeirão Preto/SP durante a pandemia de Covid-19. Métodos: Trezentos e sessenta pacientes compareceram a campanha. O acolhimento foi realizado na Unidade Básica de Saúde pela equipe de enfermagem, respeitando medidas de prevenção do Covid-19 Os realizou-se aferição da pressão arterial e glicemia capilar seguida de dilatação. Dados demográficos e sociais foram coletados através de questionário on-line padronizado via smartphone e realizou-se a triagem da retinopatia diabética através da obtenção de imagens com retinógrafos portáteis realizados por residentes de oftalmologia previamente treinados, com a aquisição de 2 imagens padronizadas de 45º: uma do segmento posterior e outra nasal ao nervo óptico. Resultados: Trezentos e sessenta pacientes foram atendidos durante a campanha. Dez pacientes (1,02%) foram excluídos devido à opacidade de meios e impossibilidade de obtenção de imagens de fundo de olho. Foram avaliados 350 pacientes, 64% do sexo feminino, 45% entre 55 e 70 anos e 55% brancos. A Campanha foi a primeira avaliação de retina para 40,5% dos pacientes e 47,56% apresentavam diabetes mellitus há mais de 10 anos. Na análise comparativa da classificação da retinopatia diabética segundo Early Treatment Diabetic Retinopathy Study (triagem X Nuvem) observou-se uma diferença de 7,8% nos resultados. Retinopatia diabética leve foi observada em 12,23%, moderada em 6,31%, proliferativa em 2,58%; edema macular presente em 4,58% e ausência de retinopatia diabética em 72,78% dos pacientes. Conclusão: A utilização de retinógrafos portáteis juntamente a telemedicina, para o rastreamento da retinopatia diabética pode ser considerada uma alternativa eficiente para triagem e diagnóstico da retinopatia diabética dentro ou fora do cenário pandêmico, auxiliando na prevenção de perda visual pelo diabetes.

La lidocaína es el anestésico local más utilizado a nivel mundial para la cirugía de catarata. También se usa de manera común en otras intervenciones quirúrgicas oftalmológicas del segmento anterior, la superficie del globo ocular, los párpados y vías lagrimales, así como en el segmento posterior del ojo. Esta revisión pretende brindar una actualización sobre las principales características, los efectos y vías de administración de la lidocaína usada en la oftalmología. Se realizó una búsqueda sistemática sobre el tema en publicaciones científicas indexadas en bases de datos, cuya información recopilada se resumió en este trabajo. La lidocaína se presenta en múltiples formas farmacéuticas, con variedad en concentración y formulación. Por lo general, para inyección se usan las concentraciones al 0,5 por ciento, 1 por ciento y 2 por ciento, para anestesia tópica en gel al 2 por ciento y en solución oftálmica al 4 por ciento. Su efecto como anestésico local es bien conocido, a nivel ocular se puede conseguir a través de inyecciones perioculares e intraoculares o mediante su aplicación tópica. Este efecto anestésico de la lidocaína ofrece cierta capacidad de dilatación pupilar, el cual se ha estudiado y aprovechado con frecuencia en los últimos años. Hoy día se encuentran en estudio otros efectos de la lidocaína a nivel local y sistémico. Los usos de la lidocaína en la oftalmología actual, están respaldados fundamentalmente por su eficacia y seguridad comprobadas en el tiempo(AU)


Lidocaine is the most widely used local anesthetic worldwide for cataract surgery. It is also commonly used in other ophthalmic surgical procedures of the anterior segment, surface of the eyeball, eyelids and lacrimal ducts, as well as in the posterior segment of the eye. This review aims to provide an update on the main characteristics, effects and ways of administering lidocaine used in ophthalmology. A systematic search on the subject was carried out in scientific publications indexed in databases, the information collected was summarized in this work. Lidocaine comes in multiple pharmaceutical forms, with a variety of concentrations and formulations. Generally, 0.5 percent, 1 percent and 2 percent concentrations are used for injection, 2 percent for topical anesthesia in gel and 4% in ophthalmic solution. Its effect as a local anesthetic is well known, at ocular level it can be achieved through periocular and intraocular injections or by topical application. This anesthetic effect of lidocaine offers some pupillary dilation capacity, which has been frequently studied and exploited in recent years. Other local and systemic effects of lidocaine are currently under study. The uses of lidocaine in ophthalmology today are supported primarily by its time-tested efficacy and safety(AU)

ABSTRACT Purpose: To determine and analyze the usability metrics of a free mobile learning app for ophthalmology in Brazil. Methods: Metric data from the management dashboard of the CBOQUIZ app were used. All users registered on the platform between March 2019 and June 30, 2021 were included. The number of questions answered, number of correct answers, number of questions answered and correct answers by subject area, and user performance by geographic region were analyzed. Results: There were 458 active users during the research period and 107,245 questions answered (average, 234.16 questions per user). Of the questions answered, 81,600 (75.5%) were correct and 2,645 were incorrect. The states in Brazil with the best performance were Espírito Santo, Paraiba, and Paraná. The subject area with the lowest hit rate was basic sciences (69.1%), within which embryology demonstrated the lowest hit rate (58.28%). The posterior segment had the highest number of questions answered, followed by miscellaneous topics and the anterior segment. Questions on strabismus were the least answered. Conclusion: The app was used consistently throughout the period studied, and participants adhered to this teaching modality. Performance asymmetry was observed across the Brazil states. The CBOQUIZ app can be used to homogenize ophthalmology teaching in the country.

ABSTRACT Purpose: To assess the effect of the coronavirus disease 2019 (COVID-19) pandemic on cataract surgery by residents who had mandatory surgical simulator training during residency. Methods: In this retrospective, observational analytical study, the total number of cataract surgeries and surgical complications by all senior residents of 2019 (2019 class; prepandemic) and 2020 (2020 class; affected by the reduced number of elective surgeries due to the COVID-19 pandemic) were collected and compared. All residents had routine mandatory cataract surgery training on a virtual surgical simulator during residency. The total score obtained by these residents on cataract challenges of the surgical simulator was also evaluated. Results: The 2020 and 2019 classes performed 1275 and 2561 cataract surgeries, respectively. This revealed a reduction of 50.2% in the total number of procedures performed by the 2020 class because of the pandemic. The incidence of surgical complications was not statistically different between the two groups (4.2% in the 2019 class and 4.9% in the 2020 class; p=0.314). Both groups also did not differ in their mean scores on the simulator's cataract challenges (p<0.696). Conclusion: Despite the reduction of 50.2% in the total number of cataract surgeries performed by senior residents of 2020 during the COVID-19 pandemic, the incidence of surgical complications did not increase. This suggests that surgical simulator training during residency mitigated the negative effects of the reduced surgical volume during the pandemic.

Introducción: La oftalmología es la ciencia de la medicina que se encarga del estudio del sistema visual y el órgano de la visión, así como de sus anexos. Para ello es dotada de un conjunto de tecnologías de avanzada que permite describir las múltiples patologías que tienen los pacientes. En la actualidad se nutre del uso de los dispositivos móviles, fundamentalmente en la gestión del conocimiento, pero no se explotan sus otras dimensiones. Objetivo: Identificar los referentes que fundamentan el uso de la telefonía móvil en el proceso de enseñanza aprendizaje del residente de oftalmología. Métodos: Se realizó una investigación de desarrollo tecnológico en el campo de la educación médica superior. Se utilizaron como métodos teóricos el histórico-lógico, el analítico-sintético y el método sistémico estructural funcional. Resultados: La telefonía móvil constituye una nueva tendencia en la educación superior cubana y en la educación médica como alternativa para desarrollar los procesos de enseñanza aprendizaje, a través de su uso para la socialización, la comunicación entre los estudiantes, y la gestión de la información, como medio de enseñanza aprendizaje y herramienta de trabajo. Conclusiones: Existe una contradicción entre la formación que deben tener los residentes en oftalmología en cuanto al uso de las tecnologías, y la preparación que poseen para su utilización en función de su especialidad. A pesar de que existen varios estudios que demuestran la importancia del uso de la telefonía móvil en la formación de los residentes de oftalmología, como objeto de estudio y herramienta de trabajo, no se ha establecido la vía para incluirla como una de las tecnologías dentro de la residencia de oftalmología en Cuba(AU)


Introduction: Ophthalmology is the medical science dealing with the study of the visual system and the organ of vision, as well as its annexes. For this purpose, it is equipped with a set of advanced technologies that allow describing the multiple pathologies presented by patients. Nowadays, it is nourished by the use of mobile devices, mainly in knowledge management, but its other dimensions are not exploited. Objective: To identify the references that support the use of mobile telephony in the teaching-learning process of ophthalmology residents. Methods: A technological development research was carried out in the field of higher medical education. As theoretical methods, the historical-logical, the analytical-synthetic, and the systemic-structural-functional methods were used. Results: Mobile telephony is a new trend in Cuban higher education, as well as in medical education, as an alternative to develop teaching-learning processes, through its use for socialization, communication among students, and information management, as a teaching-learning means and a work tool. Conclusions: There is some contradiction between the training that ophthalmology residents should receive with respect to the use of technologies and the preparation that they have for their usage in accordance with their specialty. Although there are several studies that demonstrate the importance of using mobile telephony in the training of ophthalmology residents, as an object for studying and a work tool, there has not been a way to include it as one of the technologies within the ophthalmology residency in Cuba(AU)

Background: In 1978, the advent of magnetic resonance imaging (MRI) was a remarkable volte-face in the world of diagnostics. Employing the phenomenon of nuclear resonance enables us to exploit the properties of differential protons in living tissues. The ability of providing higher and variable contrast and the absence of ionizing radiations make it superior to computed tomography. Being the diagnostic tool of choice, it is an indispensable part of assessment of the location and characteristics of different ocular and orbital pathologies (vascular, inflammatory, and neoplastic). Purpose: The intrinsic and extrinsic properties of MRI provide multi-parametric imaging, making it of paramount importance in ophthalmological evaluation. Also, MRI-dynamic color mapping provides non-invasive and quantitative assessment of soft tissues in motion. An in-depth knowledge of the basic principle and technique of MRI aids in diagnosing as well optimal planning of surgical interventions. Synopsis: In this video, we shall be displaying the anatomical, clinical, and radiological aspects of MRI with an overlap to make it easier to understand the implications of this miraculous invention. Highlights: A good understanding of MRI analysis makes the ophthalmologists independent and helps in ruling out the differential diagnoses, exact extent and invasion, precise surgical planning and therefore, avoiding tragic outcomes. This video is an attempt to simplify and emphasize on the importance of MRI interpretation for an ophthalmologist.

Purpose: During the coronavirus disease 2019 (COVID?19) pandemic, private practice, inpatient consult services, and academic residency programs in ophthalmology saw a decrease in patient encounters. This study elucidates how community hospital ophthalmology consult (OC) services were affected during the pandemic. We aim to determine whether there was a change in resident OC volume in a community?based ophthalmology program consult service during the COVID?19 pandemic. Secondary objectives included analyzing the change in the types of diagnoses and the number of patients seen for diabetic retinopathy over the same time. Methods: A retrospective cross?sectional study was conducted reviewing the electronic health record (EHR) charts from OCs for the period 2017–2021. Records were categorized by referral source and the nature of OCs (trauma, acute, or chronic); OCs were further grouped by year and weak of referral. An intermonth analysis of weekly OC counts in each category was performed for the average number of consults in February–April 2017–2019 and for February–April 2020. A one?tailed t?test was performed. All t?tests assumed equal variances. Results: Weekly OCs in 2020 revealed no statistically significant differences in overall cases or in acute or chronic cases when the volume before the COVID?19 pandemic was compared to the volume after the onset of the pandemic. However, a statistically significant increase in the average weekly trauma cases was noted when 2020 (an average of 2.7 cases per week) was compared to the weekly average for the same weeks of years 2017– 2019 (0.4; P = 0.016). This statistically significant increase in trauma in 2020 disappeared when comparing weeks 11–17 in 2020 (2.2 cases per week) and the average of 2017–2019 (1.1).Conclusion: This report outlines no significant change in OCs before and after the onset of the pandemic compared to three previous years. There was, however, an increase in trauma consults during the pandemic and an increase in the number (though not the proportion) of diabetic retinopathy (DR+) patients seen by residents. This report uniquely describes no significant changes in the resident volume of patients seen during the COVID?19 global pandemic

Introducción: Las tecnologías de la información y las comunicaciones (ICT por sus siglas en inglés) han originado la enseñanza mediante los llamados entornos virtuales de enseñanza-aprendizaje. La Universidad Virtual de la salud (UVS) de Pinar del Río, es una de las instituciones que se destaca en este quehacer, no obstante, son insuficientes las publicaciones relacionadas con la virtualización de contenidos en la especialidad de Oftalmología. Objetivo: desarrollar una aplicación web para la virtualización de los procesos docentes en la especialidad de Oftalmología en la Universidad de Ciencias Médicas de Pinar del Río. Métodos: La investigación se basa en el enfoque dialéctico. Desde este enfoque se utilizaron métodos de investigación teóricos: análisis histórico lógico, análisis y síntesis, inducción y deducción, y empíricos: modelación y análisis documental. Para la modelación y diseño de artefactos de ingeniería se utilizó el Enterprise Architect y para la elaboración de la web el WordPress. Resultados: Se podrán visualizar los contenidos de la especialidad en cada uno de los años de rotación, tanto en pregrado como en el postgrado, a través de interfaz gráfica amigable, sencilla y amena. Conclusiones: Con la elaboración de esta aplicación web se resuelven problemas de masividad, espacio y tiempo, así como de llevar el conocimiento más actualizado y pertinente hasta donde están quienes más lo necesitan, sin que tengan que ausentarse de su entorno laboral y familiar, de forma masiva.


Introduction: ICT have originated training in the so-called virtual teaching-learning environments. The UVS of Pinar del Río is one of the institutions that stands out in this task; however, the publications related to the virtualization of contents in the specialty of Ophthalmology are insufficient. Objective: to develop a web application for the virtualization of teaching processes in the specialty of Ophthalmology at the University of Medical Sciences of Pinar Del Río. Methods: the dialectical approach is the basis of the research. Theoretical methods used were: logical-historical analysis, analysis and synthesis, induction and deduction; empirical methods included modeling and documentary analysis. The Enterprise Architecture was used for the modeling and design of engineering artifacts, and WordPress was used for the development of the Web. Results: the contents of the specialty might be visualized in both undergraduate and postgraduate years of rotation through a friendly, simple and enjoyable graphical interface. Conclusions: the use of this web application can solve problems of massiveness, space and time, bringing the most updated and relevant knowledge to where people need it, without having to be absent from their work and family environment.
