الملخص

La pandemia de COVID-19 ha destacado la importancia de la rehabilitación en pacientes con COVID prolongado. Objetivo: describir los efectos de tres tratamientos en un programa de rehabilitación respiratoria en pacientes post COVID-19 en un hospital militar peruano. Materiales y métodos: se llevó a cabo un estudio descriptivo y observacional. La muestra se dividió en tres grupos con diferentes tratamientos: RR+VNI+O2, Oxigenoterapia convencional y RR+CNAF+O2. Se evaluaron 348, 151 y 113 pacientes respectivamente en cada grupo. Se utilizó la Escala de Borg, mMRC, el cuestionario específico de Saint George y el genérico SF-12 para medir la percepción de falta de aire, fatiga y calidad de vida post pandemia. Resultados: tras los tratamientos, se observó un aumento significativo en la saturación de oxígeno, disminución en la frecuencia cardíaca, disnea y fatiga percibida. Conclusión: esto indica una mejora notable en la intensidad del cansancio y una significativa recuperación en la calidad de vida de los pacientes evaluados.


The COVID-19 pandemic has highlighted the importance of rehabilitation in patients with long COVID. objective: Describe the effects of three treatments in a respiratory rehabilitation program in post-COVID-19 patients in a Peruvian military hospital. Materials and methods: a descriptive and observational study was carried out. The sample was divided into three groups with different treatments: RR+NIV+O2, conventional oxygen therapy and RR+CNAF+O2. 348, 151 and 113 patients were evaluated respectively in each group. The Borg Scale, mMRC, the specific Saint George questionnaire and the generic SF-12 were used to measure the perception of shortness of breath, fatigue and post-pandemic quality of life. Results: after the treatments, a significant increase in oxygen saturation, decrease in heart rate, dyspnea and perceived fatigue was observed. Conclusion: this indicates a notable improvement in the intensity of fatigue and a significant recovery in the quality of life of the patients evaluated.


A pandemia de COVID-19 destacou a importância da reabilitação em pacientes com COVID longa. Objetivo: descrever os efeitos de três tratamentos em um programa de reabilitação respiratória em pacientes pós-COVID-19 em um hospital militar peruano. Materiais e métodos: foi realizado um estudo descritivo e observacional. A amostra foi dividida em três grupos com diferentes tratamentos: FR+VNI+O2, oxigenoterapia convencional e FR+CNAF+O2. Foram avaliados 348, 151 e 113 pacientes respectivamente em cada grupo. A Escala de Borg, mMRC, o questionário específico de Saint George e o SF-12 genérico foram utilizados para mensurar a percepção de falta de ar, fadiga e qualidade de vida pós-pandemia. Resultados: após os tratamentos foi observado aumento significativo da saturação de oxigênio, diminuição da frequência cardíaca, dispneia e fadiga percebida. Conclusão: isto indica uma melhora notável na intensidade da fadiga e uma recuperação significativa na qualidade de vida dos pacientes avaliados.

الملخص

La atrofia muscular espinal (AME) 5q es una de las enfermedades neuromusculares de mayor incidencia en la infancia. Sin embargo, la prevalencia de AME tipo 1, su forma más severa de presentación, es menor debido a muertes prematuras evitables antes de los dos años por insuficiencia ventilatoria subtratada. La irrupción de nuevos tratamientos modificadores de la enfermedad pueden cambiar dramáticamente este pronóstico y es una oportunidad para actualizar el manejo respiratorio, a través de cuidados estandarizados básicos, preferentemente no invasivos, abordando la debilidad de los músculos respiratorios, la insuficiencia tusígena y ventilatoria, con un enfoque preventivo. La siguiente revisión literaria entrega estrategias para evitar la intubación y la traqueostomía usando soporte ventilatorio no invasivo (SVN), reclutamiento de volumen pulmonar (RVP) y facilitación de la tos. Se analizan en detalle los protocolos de extubación en niños con AME tipo 1.


Spinal muscular atrophy (SMA) 5q is one of the neuromuscular diseases with the highest incidence in childhood. Nevertheless, the prevalence of its most severe form SMA1 is lower due to premature preventable deaths before two years of age related to ventilatory insufficiency undertreated. The emergence of new disease-modifying treatments can dramatically change this prognosis and is an opportunity to update respiratory management, through basic standardized care, mostly non-invasive, addressing respiratory muscles pump weakness, cough and ventilatory insufficiency with a preventive approach. This literature review provides consensus recommendations for strategies to avoid intubation and tracheostomy using noninvasive ventilatory support (NVS), lung volume recruitment (LVR), and cough facilitation. Extubation protocols in children with SMA type 1 are analyzed in detail.

Objetivo: Descrever os parâmetros validados por enfermeiros relacionando com a sua prática assistencial na monitorização de pacientes críticos sob ventilação mecânica. Métodos: Estudo descritivo-exploratório, de abordagem quantitativa, cujos dados foram discutidos à luz da Resolução Conselho Federal de Enfermagem n. 639/2020 e de parâmetros clínicos obtidos validados por enfermeiros intensivistas. Resultados: Os parâmetros validados foram discutidos em termos de sua aplicabilidade clínica para o alcance da competência monitorização respiratória, um dos itens presentes na referida resolução como privativa do enfermeiro. Criou-se um quadro com os parâmetros gerais para a monitorização respiratória/ventilatória por enfermeiros, com alvo clínico e local de obtenção do parâmetro no ventilador mecânica, para colaborar com a sua usabilidade. Conclusão: Apresentou-se a aplicabilidade de parâmetros validados por enfermeiros na assistência aos pacientes sob ventilação mecânica. Pondera-se sua contribuição para maior qualidade na assistência, melhores desfechos clínicos, assim como, evitar complicações associadas à ventilação mecânica. (AU)


Objective: To describe the respiratory/ventilatory parameters validated by nurses relating to their care practice in critical patients on mechanical ventilation. Methods: Descriptive-exploratory study with quantitative approach, whose data were discussed based on COFEN Resolution 639/2020 and clinical parameters obtained validated by critical care nurses. Results: The validated parameters were discussed in terms of clinical applicability for achieving the competence "respiratory monitoring", one of the items present in the aforementioned resolution as private nurse care. A framework was created with the general parameters for respiratory/ventilatory monitoring by nurses, with clinical target and location of obtaining the parameter on the mechanical ventilator, to collaborate with its usability. Conclusion: The applicability of parameters validated by nurses in care of mechanically ventilated patients was presented. Its contribution to better quality care and clinical outcomes, as well as avoiding complications associated with mechanical ventilation. (AU)


Objetivo: describir los parámetros respiratorios/ventilatorios validados por enfermeros relacionando con su práctica asistencial en pacientes críticos con ventilación mecánica. Métodos: investigación descriptiva-exploratoria con abordaje cuantitativo, cuyos datos fueron discutidos en la Resolución COFEN 639/2020 y de parámetros clínicos obtenidos y validados por enfermeros intensivistas. Resultados: los parámetros validados fueron discutidos en términos de su aplicabilidad clínica para el alcance de la competencia "monitoreo respiratorio", uno de los ítems presentes en la referida resolución como exclusiva del enfermero. Se creó un cuadro con los parámetros generales para el monitoreo respiratorio/ventilatorio por enfermeros, con objetivo clínico y lugar de obtención del parámetro en el ventilador mecánico, para colaborar con su usabilidad. Conclusión: se presentó la aplicabilidad de parámetros validados por enfermeros en la asistencia a pacientes con ventilación mecánica. Abstract Objective: To evaluate the success rate of high-flow nasal cannula (HFNC) therapy using an adapted obsolete mechanical ventilator (MV), Optiflow™ and Vapotherm™ in newborns (NBs). Method: This was a retrospective observational study conducted in the neonatal intensive care unit (NICU). The sample comprised NBs who underwent HFNC therapy due to ventilatory dysfunction, for weaning from non-invasive ventilation (NIV), or post-extubation. The three groups, stratified according to gestational age (GA) and birth weight, and corrected GA and weight at the beginning of HFNC use, were as follows: Optiflow ™, Vapotherm ™, and obsolete Mechanical Ventilator (MV) adapted for high flow therapy. Subsequently, the NBs were divided into a success group (SG) and a failure group (FG). HFNC success was defined as a therapy duration exceeding 72 h. Results: A total of 209 NBs were evaluated, with 31.1 % using HFNC due to ventilatory dysfunction, 2.4 % after extubation, and 66.5 % after NIV weaning. HFNC success rate was observed in 90.9 % of the NBs, with no difference between equipment types (Vapotherm ™, Optiflow ™, and adapted VM). Conclusion: Different types of HFNC equipment are equally effective when used in neonatology for respiratory dysfunction, as a method of weaning from NIV and post-extubation. Adapted obsolete MV can be an alternative for HFCN therapy in resource-constrained settings.

Abstract Objective: To correlate the nasal anatomical characteristics of newborns with the dimensions of short binasal prongs. Method: Observational, cross-sectional study carried out in two hospitals in southern Brazil. The authors evaluated 1620 newborns with neonatal data and nasal dimensions. To measure the dimensions of the nasal region, the authors considered the width of the medial columella, the right nostril diameter, and the left nostril diameter. These data were correlated with the dimensions of two models of short binasal prongs. Results: Of the total newborns evaluated, 807 were female (49.8%), and 813 were male (50.2%). The majority were white (96.2%). The mean gestational age was 37.4 ± 2.9 weeks, ranging from 22 to 42 weeks. The birth weight was 2946.8 ± 699.3 g, ranging from 490.0 to 4740.0 g. Most of the nasal measures were significantly larger than both prong model measurements. Conclusion: The sizes of short binasal prongs available on the Brazilian market do not match the nasal anatomical characteristics of newborns.

Introducción: El edema agudo del pulmón es una enfermedad frecuente en los pacientes que se presentan en los servicios de urgencias. Objetivo: Caracterizar a los pacientes con edema agudo del pulmón en el servicio de urgencias. Método: Se realizó un estudio descriptivo y transversal en 37 pacientes con edema agudo del pulmón, que asistieron al cuerpo de guardia del Hospital Clinicoquirúrgico Docente Dr. Joaquín Castillo Duany de Santiago de Cuba, desde enero a noviembre de 2019. Las variables estudiadas fueron edad, sexo, estado al egreso y uso de la ventilación no invasiva. Resultados: No hubo diferencias entre ambos sexos, predominaron los mayores de 60 años, que padecían de hipertensión arterial. Se comprobó el poco uso de la ventilación no invasiva o invasiva. Conclusiones: El edema agudo del pulmón es más frecuente en pacientes mayores de 60 años, sin distinción de sexo. La mayoría lo presenta relacionado con la hipertensión arterial y existe un escaso uso de la ventilación no invasiva en estos casos.


Introduction: Acute pulmonary edema is a frequent disease among patients in emergency services. Objective: To characterize patients with acute pulmonary edema who attended the emergency services. Methods: A descriptive and cross-sectional study of 37 patients with acute pulmonary edema who attended the emergency services of Dr. Joaquín Castillo Duany Teaching Clinical Surgical Hospital in Santiago de Cuba was carried out, from January to November 2019. The studied variables were age, sex, alive or dead when discharged, noninvasive ventilation usage. Results: There were no differences between the sexes; there was a prevalence of patients older than 60 years of age, who suffered from hypertension. It was demonstrated low noninvasive or invasive ventilation usage. Conclusions: Acute pulmonary edema is more frequent among patients older than 60 years of age in both sexes. Most of the patients suffered from hypertension and there was a deficit in the implementation of noninvasive ventilation in these cases.

Introducción. Las cardiopatías congénitas (CC) en Chile corresponden a la segunda causa de muerte en menores de 1 año, requiriendo cirugías paliativas y/o correctivas el 65% de estas. En el post operatorio frecuentemente se utiliza ventilación mecánica invasiva (VM) y succión endotraqueal (SET) para remover secreciones. Sin embargo, la kinesiología respiratoria (KTR) ha mostrado mejoras significativas en la distensibilidad toracopulmonar (Cest) y resistencia de vía aérea (Rva) en otros grupos de usuarios pediátricos y adultos en VM. Objetivo. Comparar los cambios en la Cest y Rva en usuarios pediátricos en VM post cirugía de cardiopatía congénita (CCC) sometidos a KTR versus SET exclusiva. Métodos. Revisión sistemática de estudios publicados en bases de datos PUBMED, PeDro, Scielo y Google Scholar que comparan el uso de KTR ó SET sobre los cambios en mecánica ventilatoria en usuarios pediátricos en VM post cirugía de cardiopatía congénita, limitados a inglés, español y portugués, excluyendo a sujetos con traqueostomía o con oxigenación por membrana extracorpórea. Se utilizó guía PRISMA para la selección de artículos. Se revisaron 397 artículos y se seleccionó 1 artículo extra de los artículos sugeridos. Se eliminó 1 artículo por duplicidad. Por títulos y resúmenes se seleccionaron 2 artículos, los cuales al leer el texto completo fueron retirados debido a que la población no correspondía a cardiópatas. Resultados. El final de artículos seleccionados fue de 0 artículos, debido a lo cual se removió el operador Booleano "NOT", y se removió la población de cardiopatías. De este modo quedaron 2 artículos seleccionados para la revisión cualitativa final donde se compara KTR versus SET, y KTR en kinesiólogos especialistas y no especialistas, mostrando ambos aumento en la Cest y disminución de la Rva a favor de la KTR, hasta los 30 minutos post intervención. Conclusiones. No se encontraron artículos que demuestren cambios en Cest y Rva con el uso de KTR + SET versus SET exclusiva, en usuarios pediátricos ventilados posterior a CCC. Con la remoción de filtros seleccionamos 2 artículos que demuestran aumento de Cest y disminución de Rva en sujetos pediátricos en VM, uno comparando con SET, y por grupos de especialistas y no especialistas en respiratorio. Se sugieren estudios primarios para evaluar los efectos de esta intervención en esta población.


Introduction. Congenital heart diseases (CHD) are the second general cause for children death under 1 year. In Chile, approximately 65% CHD need surgery, could was palliative or corrective. In the postoperative period, invasive mechanical ventilation (MV) is frequently used as a life support method, but it is associated with complications. Tracheal suction (SET) is regularly used to remove secretions; however, respiratory chest physiotherapy (KTR) has shown significant improvements in thoraco-pulmonary compliance and airway resistance in other groups of pediatrics and adult's users in MV. Objetive. to compare changes in thoraco-pulmonary compliance and airway resistance in pediatric subjects under mechanical ventilation after congenital heart disease surgery comparing chest physiotherapy and exclusive tracheal suction. Methods. systematic review of studies published in PUBMED, PeDro, Scielo and Google Scholar databases who compares KTR or SET use on changes in ventilatory mechanics in pediatric users under MV after congenital heart disease surgery, limited to English, Spanish and Portuguese languages, excluding user with tracheostomy or extracorporeal membrane of oxygenation. It was use the PRISMA guide to articles selection. A search was carried out, with a total of 397 articles reviewed (English: PubMed = 3, PeDro = 8, Scholar = 383; Spanish: Scholar = 3, Scielo = 0; and Portuguese: Scielo = 0). One extra article was selected from the suggested articles, and 1 article was eliminated due to duplication. By titles and abstracts, 2 articles were selected, but the population did not correspond to heart disease. Results. the final selected articles were 0 articles. By this reason, it were removed: Boolean operator "NOT", and congenital heart disease population. Thus, 2 articles were selected for the final qualitative review where it was compares KTR versus SET, and KTR by specialist and non-specialist. Both articles shown improvement in compliance and resistance until 30 minutes post intervention. The CC population was in a 40 to 60% range in both studies. Conclusions. it was no found articles that demonstrate changes in compliance and resistance in the airway with the use of KTR + SET versus exclusive SET in pediatric users after CCC connected to MV. After filter remotion, we found 2 studies shown improves in increase compliance and reduce resistance in pediatric user in MV, ones comparing with SET, and the other one comparing between specialists in respiratory pediatric physiotherapy and not specialists. It suggests to made primary clinical studies about this intervention in CC population.

OBJECTIVE To systematically evaluate the efficacy and safety of midazolam and dexmedetomidine/propofol for the sedation of critically ill patients undergoing mechanical ventilation, and to provide evidence-based reference for clinical treatment. METHODS Retrieved from PubMed, Embase, Web of Science, Cochrane Library, Clinical trials. gov, China Journal Full Text Database, Chinese Science and Technology Journal Database, Wanfang database and China Biomedical Literature Database, the data on the efficacy and safety of midazolam and dexmetomidine/propofol for the sedation of critically ill patients undergoing mechanical ventilation were collected from the establishment of the database to March 31, 2023. After extracting data from clinical studies that met the inclusion criteria, the meta-analysis was conducted by using the RevMan 5.3 statistical software. RESULTS A total of 31 literature were included, with a total of 2 765 patients. Results of meta-analysis showed that the mechanical ventilation time [MD=14.13, 95%CI (13.75, 14.52), P<0.000 01] and the length of hospitalization in the intensive care unit [MD=0.92, 95%CI (0.54, 1.30), P<0.000 01] of patients in the midazolam group was longer than dexmedetomidine/ propofol group. The incidence of bradycardia in midazolam group was lower dexmedetomidine/propofol group [OR=0.60, 95%CI (0.41, 0.90), P=0.01], but there was no statistically significant difference in the incidence of hypotension between the two groups [OR=0.69, 95%CI (0.47, 1.01), P=0.06]. The incidence of delirium [OR=3.88, 95%CI (2.74, 5.49), P<0.000 01], ventilator- associated pneumonia [OR=2.32, 95%CI (1.19, 4.51), P=0.01], and respiratory depression [OR=5.70, 95%CI (3.09, 10.52), P<0.000 01] in midazolam group were higher than dexmedetomidine/propofol group. CONCLUSIONS Compared with dexmedetomidine/propofol, midazolam increases patients’ mechanical ventilation time and the length of hospitalization in the intensive care unit in terms of efficacy, and increases the risk of delirium and pulmonary complications in terms of safety, but has a smaller cardiovascular impact.

In November 2023, the American Heart Association and the American Academy of Pediatrics jointly released key updates to the neonatal resuscitation guidelines based on new clinical evidence. This update serves as an important supplement to the "Neonatal resuscitation: 2020 American Heart Association guidelines for cardiopulmonary resuscitation and emergency cardiovascular care". The aim of this paper is to outline the key updates and provide guidance on umbilical cord management and the selection of positive pressure ventilation equipment and its additional interfaces in neonatal resuscitation.

Objective To observe the effects of inverse ratio ventilation on respiratory mechanics and postoperative pulmonary com-plications in morbidly obese patients during laparoscopic sleeve gastrectomy Methods A total of 62 morbidly obese patients for a laparo-scopic sleeve gastrectomy were scheduled(18-65 years old,BMI≥35kg/m2 in combination with diabetes,hypertension,hyperlipemia,and other metabolic diseases,or BMI≥40kg/m2,ASA Ⅱ or Ⅲ).Patients were randomly assigned to two groups:inverse ratio ventilation group(IRV)and control group(PVG).Two groups were adjusted with an actual tidal volume(VT)of 7-8ml/kg,and respiratory rates of 12 breaths/min.In the IRV group,the ratio of I∶E was 2∶1;in the control group,the ratio of I∶E was 1∶2.We recorded the indexes of respiratory mechanics and the blood gas at the baseline(T0),5min after anesthesia(T,),15min after pneumoperitoneum(T2),30min after pneumoperitoneum(T3),60min after pneumoperitoneum(T4)and the end of surgery(T5),and the occurrence of pulmonary com-plications on day 1,day 2 after operation was observed,and the cumulative incidence of PPCs at 7 days was counted.Results Compared to the control group,the dynamic lung compliance(Cdyn),mean airway pressure(Pmean),positive end expiratory pressure(PEEP),and the PaO2 at T,-T5 of the inverse ventilation group were increased significantly(P<0.05),the Ppeak、Pplat at T2-T5 and dynamic lung compliance(VD/VT)at T3-T5 were decreased significantly(P<0.05),and no difference in the PaCO2 and PETCO2 in the two groups.Grade 1 pulmonary complications occurred in both groups,and there was no difference in the total occurrence of pulmonary com-plications at 7 days.Conclusion Inverse ventilation effectively may improve respiratory mechanics and oxygenation in morbidly obese pa-tients undergoing laparoscopic sleeve gastrectomy.

Objective To compare the anesthetic effect and safety of Ciprofol and Propofol combined with Fentanyl in painless bronchoscopy for conscious patients in intensive care unit(ICU).Methods 60 conscious patients who underwent painless bronchoscopy from October 2022 to January 2023 were selected.According to the random number table method,30 cases were divided into the control group(Propofol 1.5 mg/kg + Fentanyl 1 μg/kg)and 30 cases were divided into the experimental group(Ciprofol 0.4 mg/kg + Fentanyl 1 μg/kg).Systolic blood pressure,diastolic blood pressure,heart rate,respiratory frequency,and percutaneous arterial oxygen saturation(SpO2)were recorded before anesthesia(T1),during the examination(T2),and at the completion of the examination(T3)in the two groups,and the success rate of induction of general anesthesia,the time of induction of anesthesia,the time of bronchoscopy,the time of eye-opening,the total dosage of medications used,the incidence of injection pain,and the incidence of intraoperative adverse reactions(hypotension,respiratory depression,bradycardia,and bucking)in the two groups were compared.Results Analysis of the data revealed significant group,time,and interaction effects for systolic blood pressure,diastolic blood pressure,heart rate,and respiratory frequency at different time points(P<0.05).The experimental group had a shorter induction time of anesthesia[1.40(1.10,1.62)min]in comparison to the control group[1.60(1.30,2.10)min](P<0.05),and the experimental group required a less amount of drug to achieve the desired outcome[(21.40±1.82)mg]compared to the control group[(78.75±6.71)mg](P<0.05).Furthermore,the incidence of injection pain(3.33%)and respiratory depression(6.67%)was significantly reduced in the experimental group compared to the control group(36.67%)and(30.00%)(P<0.05).Conclusion The use of Ciprofol in combination with Fentanyl has been found to provide effective anesthesia during bronchoscopy,while also increasing safety and decreasing the incidence of adverse events.It is worthy of clinical application.

Objective To analyze the effects of nasal endoscopic continuous penetrating suture and nasal packing on discomfort and complications in patients undergoing deflection of nasal septum(DNS)plasty.Methods 116 patients undergoing DNS plasty were enrolled between March 2018 and March 2023,including 69 cases in packing group score(nasal packing)and 47 cases in suture group(continuous penetrating suture).The scores of pain[evaluated by visual analogue scale(VAS)]and epistaxis caused by nasal dressing changes at 3 and 7 d after surgery,subjective discomfort score(headache,dry mouth,epiphora,sleep difficulties,nasal obstruction and nasal pain)at 3 d after surgery,total inspiratory and expiratory resistance before and at 14 d after surgery,and the occurrence of complications at 1 month after surgery were compared between the two groups.Results At 3 d after surgery,VAS of pain and epistaxis score caused by nasal dressing changes were lower in suture group than packing group(P<0.05),and score of subjective discomfort was lower in suture group(P<0.05).At 14 d after surgery,total inspiratory and expiratory resistance were lower in suture group than packing group(P<0.05).The total incidence of postoperative complications was lower in suture group than packing group(P<0.05).Conclusion Compared with nasal packing,nasal endoscopic continuous penetrating suture can reduce pain and bleeding,improve postoperative subjective discomfort and nasal ventilation function,and reduce complications after DNS plasty.

Objective:To clarify the application effect of information-guided enteral nutrition-associated diarrhea (ENAD) management process in patients with chronic obstructive pulmonary disease (COPD) undergoing non-invasive assisted ventilation.Methods:A mixed cohort study of pre- and post-control was conducted. Thirty-nine patients with COPD who were admitted to the emergency intensive care unit (ICU) of Huzhou First People's Hospital from July 1, 2021 to July 31, 2022 were enrolled. Taking the completion of the software development of ENAD management software for critically ill patients on January 28, 2022 as the time node, 20 patients admitted from July 1, 2021 to January 28, 2022 were set as the control group, and 19 patients admitted from January 29 to July 31, 2022 were set as the observation group. The two groups of patients received the same enteral nutrition support treatment, and the control group implemented the conventional ENAD treatment process with enteral nutrition intolerance disposal process as the core. On the basis of the control group, the observation group implemented the information-guided ENAD treatment process, and the system software actively captured the information of ENAD patients and reminded the medical team to improve the patient's diarrhea-related examination and provide alternative treatment plans. The duration of antidiarrhea, feeding interruption rate, and energy and protein intake, blood biochemical indexes, incidence of abnormal blood electrolyte metabolism, daily continuous non-invasive assisted ventilation and endotracheal intubation after 7 days of targeted diarrhea intervention were compared between the two groups.Results:Except for the basal pulse rate, there were no significant differences in gender distribution, age, and vital signs, basic nutritional status, arterial blood gas analysis and blood biochemistry at admission between the two groups, indicating comparability between the two groups. When ENAD occurred, the patients in the observation group obtained earlier cessation of diarrhea than those in the control group [days: 3.00 (2.00, 3.25) vs. 4.00 (3.00, 5.00), P < 0.01], and the feeding interruption rate was significantly lower than that in the control group [10.53% (2/19) vs. 65.00% (13/20), P < 0.01]. After 7 days of diarrhea intervention, the energy intake of the observation group was significantly higher than that of the control group [kJ·kg -1·d -1: 66.28 (43.34, 70.36) vs. 47.88 (34.60, 52.32), P < 0.01], the levels of hemoglobin (Hb), albumin (Alb) and serum prealbumin (PAB) were significantly higher than those in the control group [Hb (g/L): 119.79±10.04 vs. 110.20±7.75, Alb (g/L): 36.00 (33.75, 37.25) vs. 31.00 (30.00, 33.00), PAB (mg/L): 155.79±25.78 vs. 140.95±14.97, all P < 0.05], the daily continuous non-invasive assisted ventilation duration was significantly shorter than that of the control group [hours: 14 (12, 16) vs. 16 (14, 18), P < 0.01], and the arterial partial pressure of carbon dioxide (PaCO 2) was significantly lower than that of the control group [mmHg (1 mmHg ≈ 0.133 kPa): 66.00 (62.00, 70.00) vs. 68.00 (67.50, 70.05), P < 0.05]. However, there were no significant differences in protein intake, incidence of abnormal electrolyte metabolism, and incidence of endotracheal intubation due to acute respiratory failure between the two groups. Conclusion:The information-guided ENAD treatment process can enable the COPD patients undergoing continuous non-invasive assisted ventilation who experience ENAD to receive earlier cessation of diarrhea, and improve the protein energy metabolism and respiratory function of the patients.

Objective:To explore a simple method for measuring the dynamic intrinsic positive end-expiratory pressure (PEEPi) during invasive mechanical ventilation.Methods:A 60-year-old male patient was admitted to the critical care medicine department of Dongying People's Hospital in September 2020. He underwent invasive mechanical ventilation treatment for respiratory failure due to head and chest trauma, and incomplete expiratory flow occurred during the treatment. The expiratory flow-time curve of this patient was served as the research object. The expiratory flow-time curve of the patient was observed, the start time of exhalation was taken as T 0, the time before the initiation of inspiratory action (inspiratory force) was taken as T 1, and the time when expiratory flow was reduced to zero by inspiratory drive (inspiratory force continued) was taken as T 2. Taking T 1 as the starting point, the follow-up tracing line was drawn according to the evolution trending of the natural expiratory curve before the T 1 point, until the expiratory flow reached to 0, which was called T 3 point. According to the time phase, the intrapulmonary pressure at the time just from expiratory to inspiratory (T 1 point) was called PEEPi 1. When the expiratory flow was reduced to 0 (T 2 point), the intrapulmonary pressure with the inhaling power being removed hypothetically was called PEEPi 2. And it was equal to positive end-expiratory pressure (PEEP) set in the ventilator at T 3 point. The area under the expiratory flow-time curve (expiratory volume) between T 0 and T 1 was called S 1. And it was S 2 between T 0 and T 2, S 3 between T 0 and T 3. After sedation, in the volume controlled ventilation mode, approximately one-third of the tidal volume was selected, and the static compliance of patient's respiratory system called "C" was measured using the inspiratory pause method. PEEPi 1 and PEEP 2 were calculated according to the formula "C =ΔV/ΔP". Here, ΔV was the change in alveolar volume during a certain period of time, and ΔP represented the change in intrapulmonary pressure during the same time period. This estimation method had obtained a National Invention Patent of China (ZL 2020 1 0391736.1). Results:① PEEPi 1: according to the formula "C =ΔV/ΔP", the expiratory volume span from T 1 to T 3 was "S 3-S 1", and the intrapulmonary pressure decreased span was "PEEPi 1-PEEP". So, C = (S 3-S 1)/(PEEPi 1-PEEP), PEEPi 1 = PEEP+(S 3-S 1)/C. ②PEEPi 2: the expiratory volume span from T 2 to T 3 was "S 3-S 2", and the intrapulmonary pressure decreased span was "PEEPi 2-PEEP". So, C = (S 3-S 2)/(PEEPi 2-PEEP), PEEPi 2 = PEEP+(S 3-S 2)/C. Conclusion:For patients with incomplete expiratory during invasive mechanical ventilation, the expiratory flow-time curve extension method can theoretically be used to estimate the dynamic PEEPi in real time.

The main clinical research advances of critical care in 2023 includes: new trials of Chinese herbal medicine, hydroxocobalamin (vitamin B12), methylene blue as well glucocorticoids have shown the potential to improve outcomes of patients with sepsis and septic shock; international committees launched new global definition and managing recommendations for acute respiratory distress syndrome (ARDS). Besides, a cluster of new evidences has emerged in many aspects as following: fluid control strategy in sepsis (restrictive/liberative), antibiotic infusion strategy (continuous/intermittent), oxygen-saturation targets for mechanical ventilation (conservative/liberative), blood pressure targets after resuscitation from out-of-hospital cardiac arrest (hypotension/hypertension), blood pressure targets after successful stroke thrombectomy (intensive/conventional), and nutritional support strategies (low protein-calories/conventional protein-calories, fasting/persistent feeding before extubation). Thus, given above progress, carrying out high -quality domestic multi-center clinical registration researches, constructing shareable standardized databases, as well raising public awareness of sepsis, should be the essential steps to improve our level of intensive care medicine.

Objective:To investigate the effects of extracorporeal carbon dioxide removal (ECCO 2R) combined with continuous renal replacement therapy (CRRT) on respiratory efficiency and diaphragm function in patients with acute respiratory distress syndrome (ARDS) received mechanical ventilation. Methods:A prospective randomized controlled study was conducted. Sixty patients with mild to moderate ARDS admitted to the department of respiratory and critical care medicine of Henan Provincial People's Hospital from January 2019 to January 2021 were enrolled, and they were divided into observation group and control group according to the random number table method, with 30 cases in each group. All patients received antibiotics, anti-inflammatory, and mechanical ventilation therapy. On this basis, the observation group received ECCO 2R and CRRT, while the control group received bedside CRRT. Baseline data including gender, age, etiology, acute physiology and chronic health evaluationⅡ(APACHEⅡ), etc., were recorded. Arterial blood gas analysis [including arterial partial pressure of oxygen (PaO 2), arterial partial pressure of carbon dioxide (PaCO 2), and oxygenation index (PaO 2/FiO 2)] was performed at 12 hours and 24 hours during the treatment, and respiratory mechanics parameters [including tidal volume, respiratory rate, maximum expiratory pressure (MEP), and maximum inspiratory pressure (MIP)] were recorded, and rapid shallow breathing index (RSBI) was calculated. The levels of glutathione peroxidase (GSH-Px), malondialdehyde (MDA), and superoxide dismutase (SOD) in serum were detected by enzyme-linked immunosorbent assay (ELISA). Diaphragm thickness and diaphragm activity were measured by ultrasonography at 24 hours during the treatment. Results:There were no significantly differences in age, gender, etiology, and APACHEⅡ score between the two groups, indicating that the baseline data of the two groups were balanced and comparable. Compared with the 12 hours after treatment, the PaO 2 and PaO 2/FiO 2 in the observation group significantly increased, PaCO 2 significantly decreased, RSBI significantly decreased, MEP and MIP significantly increased, and serum GSH-Px and MDA significantly decreased, while SOD significantly increased at 24 hours during the treatment. In the control group, only PaCO 2 significantly decreased. Compared with the control group, the PaCO 2 significantly decreased in the observation group at 12 hours and 24 hours [mmHg (1 mmHg≈0.133 kPa): 55.05±7.57 vs. 59.49±6.95, 52.77±7.88 vs. 58.25±6.92, both P < 0.05], but no significantly differences in PaO 2 and PaO 2/FiO 2. Compared with the control group, the observation group showed significant decreases in RSBI at 12 hours and 24 hours (times·min -1·L -1: 85.92±8.83 vs. 90.38±3.78, 75.73±3.86 vs. 90.05±3.66, both P < 0.05), significant increases in MEP and MIP [MEP (mmH 2O, 1 mmH 2O≈0.01 kPa): 86.64±5.99 vs. 83.88±4.18, 93.70±5.59 vs. 85.04±3.73; MIP (mmH 2O): 44.19±6.66 vs. 41.17±3.13, 57.52±5.28 vs. 42.34±5.39, all P < 0.05], and significant decreases in serum GSH-Px and MDA [GSH-Px (mg/L): 78.52±8.72 vs. 82.10±3.37, 57.11±4.67 vs. 81.17±5.13; MDA (μmol/L): 7.84±1.97 vs. 8.71±0.83, 3.67±0.78 vs. 8.41±1.09, all P < 0.05], as well as a significant increase in SOD (U/L: 681.85±49.24 vs. 659.40±26.47, 782.32±40.56 vs. 676.65±51.97, both P < 0.05). Compared with the control group, the observation group showed significant increases in diaphragm thickness and diaphragm activity at 24 hours of treatment [diaphragm thickness (cm): 1.93±0.28 vs. 1.40±0.24, diaphragmatic thickening fraction: (0.22±0.04)% vs. (0.19±0.02)%, quiet breathing diaphragm displacement (cm): 1.42±0.13 vs. 1.36±0.06, deep breathing diaphragm displacement (cm): 5.11±0.75 vs. 2.64±0.59, all P < 0.05]. Conclusion:ECCO 2R combined with CRRT can reduce work of breathing and oxidative stress levels in ARDS patients receiving non-invasive ventilation, and protect diaphragm function.

Objective:To investigate the correlation between diaphragmatic-rapid shallow breathing index (D-RSBI) and lung ultrasound score (LUS) in elderly patients with mechanical ventilation and its predictive value for weaning results.Methods:A retrospective study was conducted. The clinical data of elderly patients (age > 60 years old) with invasive positive pressure ventilation (IPPV) admitted to the department of intensive care unit (ICU) of the First Affiliated Hospital of Jinzhou Medical University from January 2021 to June 2022 were enrolled. According to the outcome of withdrawal, the patients were divided into successful and failed groups. The differences in gender, age, acute physiology and chronic health evaluation Ⅱ (APACHEⅡ), D-RSBI and LUS before weaning and extubation were compared between the two groups. Pearson correlation was used to analyze the correlation between D-RSBI and LUS. The predictive value of D-RSBI and LUS on weaning results of elderly patients with IPPV was analyzed by receiver operator characteristic curve (ROC curve).Results:A total of 398 elderly patients with IPPV were enrolled, including 300 successful weaning patients and 98 failed weaning patients. There were no significant differences in gender and age between the failed group and successful group [male: 55.1% (54/98) vs. 59.0% (177/300), age (years old): 67.02±5.03 vs. 66.96±4.99, both P > 0.05]. APACHEⅡ score in the failed group was significantly higher than that in the successful group (17.09±3.30 vs. 16.06±3.81, P < 0.05), and the D-RSBI and LUS score before extubation were significantly higher than those in the successful group [D-RSBI (time·min -1·mm -1): 2.19±0.33 vs. 1.60±0.22, LUS: 17.30±3.04 vs. 11.97±3.20, both P < 0.01]. All patients showed a significant positive correlation between D-RSBI and LUS score ( r = 0.406, P = 0.000). ROC curve analysis showed that the area under the curve (AUC) of D-RSBI for predicting weaning outcomes in elderly IPPV patients was 0.920, with a 95% confidence interval (95% CI) of 0.881-0.958 and P = 0.000. When the cut-off value was 1.85 times·min -1·mm -1, the sensitivity was 88.7% and the specificity was 86.7%. The AUC of LUS score for predicting weaning outcome in elderly IPPV patients was 0.875, with a 95% CI of 0.839-0.912 and P = 0.000. When the cut-off value was 14.50, the sensitivity was 75.7% and the specificity was 84.7%. Conclusion:There is a significant correlation between D-RSBI and LUS score in elderly mechanically ventilated patients, both of them can predict weaning outcome in elderly patients with mechanical ventilation.

Objective:To explore the application value of dynamic monitoring of gastric residual volume (GRV) in achieving different target energy in severe mechanical ventilation patients.Methods:A prospective randomized controlled study was conducted. Forty-two patients with mechanical ventilation admitted to the department of critical care medicine of General Hospital of Ningxia Medical University from July to December 2022 were enrolled. According to the random number table method, patients were divided into GRV guided enteral nutrition by traditional gastric juice pumpback method (control group, 22 patients) and GRV guided enteral nutrition by bedside ultrasound (test group, 20 patients). General data were collected from both groups, and clinical indicators such as hypersensitive C-reactive protein (hs-CRP), interleukin-6 (IL-6), neutrophil percentage (Neut%), procalcitonin (PCT), absolute lymphocytes (LYM), prealbumin (PA), and retinol-binding protein (RBP) were dynamically observed. Inflammation, infection, immunity, nutritional indicators, and the incidence of reflux/aspiration, ventilator-associated pneumonia (VAP) were compared between the two groups, and further compared the proportion of patients with respectively to reach the target energy 25%, 50%, and 70% on days 1, 3, and 5 of initiated enteral nutrition.Results:① There were no significant differences in gender, age, body mass index (BMI), duration of mechanical ventilation, and acute physiology and chronic health evaluationⅡ(APACHEⅡ), sequential organ failure assessment (SOFA), severe nutritional risk score (NUTRIC) at admission between the two groups, indicating comparability. ② On day 1 of initiated enteral nutrition, there were no significant differences in infection, inflammation, immunity and nutrition indicators between the two groups. On day 3 of initiated enteral nutrition, the hs-CRP in the test group was lower than that control group, LYM and PA were higher than those control group [hs-CRP (mg/L): 129.60±75.18 vs. 185.20±63.74, LYM: 1.00±0.84 vs. 0.60±0.41, PA (mg/L): 27.30±3.66 vs. 22.30±2.55, all P < 0.05]. On day 5 of initiated enteral nutrition, the hs-CRP, Neut%, PCT in the test group were lower than those control group, LYM and PA were higher than those control group [hs-CRP (mg/L): 101.70±54.32 vs. 148.40±36.35, Neut%: (85.50±7.66)% vs. (92.90±6.01)%, PCT (μg/L): 0.7 (0.3, 2.7) vs. 3.6 (1.2, 7.5), LYM: 1.00±0.68 vs. 0.50±0.38, PA (mg/L): 27.10±4.57 vs. 20.80 ± 3.51, all P < 0.05]. There were no significantly differences in IL-6 and RBP between the two groups at different time points. ③ The proportion of 50% and 70% of achieved target energy in the test group on day 3, day 5 of initiated enteral nutrition were higher than those of the control group (70.0% vs. 36.4%, 70.0% vs. 36.4%, both P < 0.05). ④ The incidence of reflux/aspiration and VAP in the test group on day 5 of initiated enteral nutrition were significantly lower than those control group (incidence of reflux/aspiration: 5.0% vs. 28.6%, incidence of VAP: 10.0% vs. 36.4%, both P < 0.05). Conclusion:Dynamic monitoring of GRV by bedside ultrasound can accurately improve the proportion of 50% of achieved target energy on day 3 and 75% on day 5 in severe mechanical ventilation patients, improve the patient's inflammation, immune and nutritional status, and can prevent the occurrence of reflux/aspiration and VAP.

Objective:To investigate the therapeutic effect of high-flow nasal cannula oxygen therapy (HFNC) and non-invasive positive pressure ventilation (NPPV) on patients with pulmonary edema caused by seawater drowning.Methods:A retrospective analysis method was used. Based on the Utstein database of emergency drowning in the First Hospital of Qinhuangdao, the clinical data of patients with seawater drowning pulmonary edema admitted to the emergency medicine department of the First Hospital of Qinhuangdao from January 1, 2019 to December 31, 2022 were collected. The patients were divided into NPPV group and HFNC group according to different ventilation methods. The general data, endotracheal intubation rate in 7 days, arterial blood gas analysis indexes [arterial partial pressure of oxygen (PaO 2), arterial partial pressure of carbon dioxide (PaCO 2), arterial oxygen saturation (SaO 2)] and hemodynamic indexes (systolic blood pressure, diastolic blood pressure, mean arterial pressure, heart rate, blood lactic acid) before and after treatment, length of stay in intensive care unit (ICU), oxygen therapy comfort of the two groups were compared. Results:A total of 54 patients were enrolled, including 21 patients in the NPPV group and 33 patients in the HFNC group. There were no significant differences in gender, age, state of consciousness and other general information between the two groups. Compared with NPPV group, the rate of endotracheal intubation in HFNC group within 7 days was significantly lower [24.2% (8/33) vs. 33.3% (7/21), P < 0.05]. Before treatment, there were no significant differences in arterial blood gas analysis and hemodynamics between the two groups. After treatment, the above indexes in both groups were significantly improved compared with those before treatment, and PaO 2, SaO 2, systolic blood pressure, diastolic blood pressure and mean arterial pressure in HFNC group were significantly higher than those in NPPV group [PaO 2 (mmHg, 1 mmHg≈0.133kPa): 93.56±6.37 vs. 82.14±6.25, SaO 2: 1.02±0.09 vs. 0.95±0.11, systolic blood pressure (mmHg): 117.37±8.43 vs. 110.42±8.38, diastolic blood pressure (mmHg): 79.43±7.61 vs. 72.21±4.32, mean arterial pressure (mmHg): 92.34±6.32 vs. 85.12±5.38], PaCO 2, heart rate and blood lactic acid were significantly lower than those in NPPV group [PaCO 2 (mmHg) : 34.26±5.63 vs. 37.24±6.22, heart rate (times/min): 73.38±7.56 vs. 86.25±5.41, blood lactic acid (mmol/L): 1.38±0.36 vs. 2.25±1.14], and the differences were statistically significant (all P < 0.05). In addition, the length of ICU stay in HFNC group was significantly shorter than that in NPPV group (days: 13.30±2.38 vs. 16.27±4.26), and the comfort rate of oxygen therapy was significantly higher than that in NPPV group [66.7% (22/33) vs. 42.8% (9/21)], with statistical significance (all P < 0.05). Conclusion:HFNC can improve the oxygenation of patients with pulmonary edema caused by seawater drowning, improve hemodynamics, reduce the rate of tracheal intubation, shorten the length of ICU stay, and improve the comfort of oxygen therapy, which has certain clinical application value.

Objective:To investigate the clinical effect of helium-oxygen mechanical ventilation on inflammation of the diseased lung segment and diaphragm function in patients with acute respiratory distress syndrome (ARDS) caused by pneumonia who suffered difficulty weaning from mechanical ventilation.Methods:A prospective controlled study was conducted. A total of 40 patients with ARDS caused by pneumonia and requiring tracheal intubation with difficulty weaning from mechanical ventilation, admitted to the department of critical care medicine in Pingtan Branch of Fujian Medical University Union Hospital from October 2020 to December 2021 were enrolled. Patients were divided into nitrogen oxygen ventilation group and helium-oxygen ventilation group according to random number table, with 20 cases in each group. The nitrogen oxygen ventilation group was given 60% nitrogen and 40% oxygen ventilation treatment, and the helium-oxygen ventilation group was given 60% helium and 40% oxygen ventilation treatment. Peak airway pressure (Ppeak), plateau airway pressure (Pplat), tidal volume (V T), minute ventilation volume (MV) and pulse oxygen saturation (SpO 2) were collected at 0, 1, 2, 3 hours after ventilation treatment. At the same time, the concentrations of inflammatory factors interleukin-6 (IL-6) and C-reactive protein (CRP) in epithelial lining fluid in patients with diseased lung segments were measured before and after ventilation treatment for 3 hours, and the diaphragmatic excursion and the diaphragmatic thickening fraction were measured before and after ventilation treatment for 3 hours. Results:There were no significant differences in gender, age, oxygenation index, serum CRP, serum procalcitonin (PCT), body temperature, serum creatinine (SCr), alanine aminotransferase (ALT), fasting blood glucose (FPG), hemoglobin (Hb), and basic heart and lung diseases between the two groups. Under the condition that V T and SpO 2 are relatively unchanged, the airway pressure in helium-oxygen ventilation group decreased significantly after 1 hour of ventilation [Ppeak (cmH 2O, 1 cmH 2O≈0.098 kPa): 22.80±4.47 vs. 28.00±5.07, Pplat (cmH 2O): 19.15±3.90 vs. 23.20±3.81, both P < 0.05], and the airway pressure in the nitrogen oxygen ventilation group increased significantly after 1 hour [Ppeak (cmH 2O): 22.35±2.13 vs. 19.75±1.94, Pplat (cmH 2O): 18.50±1.70 vs. 16.50±1.88, both P < 0.05]. There were no significant differences in CRP and IL-6 levels in epithelial lining fluid in the diseased lung segment before and after ventilation in the nitrogen oxygen ventilation group, while the levels of these indexes in the helium-oxygen ventilation group after ventilation were significantly lower than those before ventilation, and significantly lower than those in the nitrogen oxygen ventilation group [CRP (mg/L): 10.15 (6.39, 15.84) vs. 16.10 (11.63, 18.66), IL-6 (μg/L): 1.15 (0.78, 1.86) vs. 2.67 (1.67, 4.85), both P < 0.05]. There were no statistically significant differences in the diaphragmatic excursion and the diaphragmatic thickening fraction before and after ventilation in the nitrogen oxygen ventilation group, while the above indexes in the helium-oxygen ventilation group were significantly higher than those before ventilation, and were significantly higher than those in the nitrogen oxygen ventilation group [diaphragmatic excursion (cm): 1.93 (1.69, 2.20) vs. 1.34 (1.22, 1.83), diaphragmatic thickening fraction: (48.22±8.61)% vs. (33.29±11.04)%, both P < 0.05]. Conclusion:Helium-oxygen ventilation can reduce the airway pressure of patients with mechanical ventilation, alleviate the inflammatory response of lung segment, improve the function of respiratory muscle, and is expected to be an important treatment for severe lung rehabilitation.
