ABSTRACT CONTEXT: Various skin manifestations have been reported in coronavirus disease. It may be difficult to determine the etiology of these lesions in view of the increased frequency of handwashing during the pandemic, along with occurrences of irritant contact dermatitis and allergic contact dermatitis due to disinfectant use; usage of herbal medicine and supplements to strengthen the immune system; and urticarial or maculopapular drug eruptions due to COVID-19 treatment. The variety of associated skin manifestations seen with COVID-19 makes it challenging to identify virus-specific skin manifestations. Petechiae, purpura, acrocyanosis and necrotic and non-necrotic purpura, which can be considered as manifestations of vascular involvement on the skin, have been reported. CASE REPORT: Here, we report a case of eruptive cherry angiomas, which was thought to have developed due to COVID-19, with a papulovesicular rash on distal extremities that progressed over time to reticular purpura. CONCLUSION: The case presented had a papulovesicular rash at the onset, which evolved to retiform purpura, and eruptive cherry angiomas were observed. It should be kept in mind that dermatological signs may vary in patients with COVID-19.

Abstract: Epstein-Barr virus is a DNA virus infecting human beings and could affect 90% of human population. It is crucial to take in account that in Latin America, unlike what happens in developed countries, the exposure to the virus is very early and therefore people have a much longer interaction with the virus. The virus is related to many diseases, mainly the oncological ones, and when the onset is in cutaneous tissue, it can present many clinical variants, as well acute as chronic ones. Among the acute ones are infectious mononucleosis rash and Lipschutz ulcers; the chronic presentations are hypersensivity to mosquito bites, hydroa vacciniforme, hydroa vacciniforme-like lymphoma, its atypical variants and finally nasal and extra-nasal NK/T-cell lymphoma. Although they are not frequent conditions, it is crucial for the dermatologist to know them in order to achieve a correct diagnosis.

Abstract: Infective dermatitis associated with HTLV-1 (IDH) is the main cutaneous marker of HTLV-1 infection. This disease occurs primarily in children and should be differentiated from other eczemas, especially from atopic dermatitis. The largest series of IDH are from Jamaica and Brazil. There are an estimated 15 to 20 million infected people in the world, and Brazil is one of the endemic regions. Studies suggest that IDH in children may be a marker for the development of T-cell leukemia/lymphoma (ATL) or myelopathy associated with HTLV-1/tropical spastic paraparesis (HAM / TSP) in adulthood.

الملخص

Cytomegalovirus (CMV) infection is a common opportunistic systemic infection in immunocompromised patients, but skin involvement is rare. Herein, we report a 10 year-old girl from consanguineous parents who was referred to our center because of disseminated maculopapular rash. She had history of upper and lower respiratory tract infections. In immunological studies, increased serum IgE level and decreased responses to tetanus and diphtheria were detected. Polymerase chain reaction (PCR) examination of bronchoalveolar lavage and serum sample revealed the presence of CMV. Early diagnosis of cutaneous CMV and appropriate treatment are the key actions in management of patients with underlying immunodeficiencies to avoid further complications.

Se reporta el caso de un hombre de raza blanca, 37 años de edad, hijo de madre con diabetes mellitus tipo 1, con antecedentes patológicos personales de padecer de obesidad exógena desde aproximadamente cinco años. Comenzó a presentar de forma diseminada una erupción papulosa de color amarillento, que se diagnosticó clínicamente e histológicamente como xantomas eruptivos asociados a diabetes mellitus tipo 2 de debut e hipertrigliceridemia. Se describen las generalidades de los xantomas eruptivos, sus formas de presentación, así como las características etiológicas e histopatológicas de la enfermedad.


The case of a white, 37 years old man is reported, his mother with diabetes mellitus type 1, with personal pathological antecedents of suffering exogenous obesity from approximately five years. He began to present in a disseminated form a papular eruption of yellowish color that was clinically and histological diagnosed like eruptive xantomas associated to diabetes mellitus type 2 appearing for the first time and hypertriglyceridemia. The generalities of the eruptive xantomas are described, their presentation forms, as well as the etiologic and histopathologic characteristic of the disease.

In early 2006, a lumpy skin disease [LSD] outbreak has invaded cattle in different localities of Egypt, exerting severe economic losses to livestock industry. Representative specimens [skin biopsies] were collected form nodular skin lesions of infected foreign [imported from Ethiopia, at Ismailia private quarantine] and local cattle [at Fayoum, Menofia and Sharquia governorates]. A polymerase chain reaction [PCR] assay was used, as a basic step, for rapid diagnosis of the causative agent in clinical specimens to control spread of infection in the rest of Egypt. The PCR assay, utilizing a LSDV P32 based primer set, could identify LSDV in all outbreak clinical specimens. The specific PCR amplification products [amplicons] were purified and subjected to direct nucleotide sequencing. Blast search, multiple alignments and phylogenetic analyses of the nucleotide sequence data revealed that outbreak LSDV is closely related to other capripoxviruses of LSD, sheep pox and goat pox. Selection and processing of clinical specimens, methods of DNA isolation, and PCR assay applied in this endeavor, presented a reliable laboratory diagnostic tool for LSDV

BACKGROUND: Infections of the skin by herpes viruses do not always present themselves in typical fashion. Early diagnosis, however, is crucial for appropriate treatment. Polymerase chain reaction (PCR) allows diagnosis and differential diagnosis of herpes virus infections, but the method is not yet available in large parts of the world, where diagnosis is made based on morphology alone. AIM: To refine criteria for the diagnosis of herpes virus infections of the skin by way of correlation of clinical and histopathologic findings with results of PCR studies. METHODS: We studied 75 clinically diagnosed patients of "zoster," "varicella," and "herpes simplex", to correlate clinical and histopathological findings with results of PCR studies on paraffin embedded biopsy specimens. RESULTS: Clinical suspicion of infection by herpes viruses was confirmed by histopathology in 37% of the cases and by PCR studies in 65% of the cases. Zoster was frequently misdiagnosed as infection with herpes simplex viruses (30%). When diagnostic signs of herpes virus infection were encountered histopathologically, PCR confirmed the diagnosis in 94%. By way of correlation with results of PCR studies, initial lesions of herpes virus infections could be identified to have a distinctive histopathological pattern. Herpetic folliculitis appeared to be a rather common finding in zoster, it occurring in 28% of the cases. CONCLUSION: We conclude that correlation of clinical and histopathological features with results of PCR studies on one and the same paraffin embedded specimen permits identification of characteristic morphologic patterns and helps to refine criteria for diagnosis both clinically and histopathologically.

الملخص

O vírus linfotrópico humano de células T do tipo 1 (HTLV-1) é o primeiro retrovírus isolado do ser humano. Descreveu-se, em pouco tempo, o seu papel etiológico em algumas doenças, com destaque para a leucemia/linfoma de células T do adulto (ATLL), a mielopatia associada ao HTLV-1/paraparesia espástica tropical (HAM/TSP) e a uveíte associada ao HTLV-1 (HAU). Na década de 90, o HTLV-1 foi associado a eczema grave da infância, conhecido como dermatite infecciosa (DI). Desde então, diversos outros tipos de lesões cutâneas têm sido observados em pacientes infectados pelo HTLV-1, em especial, nos doentes de HAM/TSP ou de ATLL. Porém, mesmo portadores assintomáticos do vírus apresentam doenças dermatológicas. Excetuando-se a dermatite infecciosa, não há lesão da pele específica da infecção pelo HTLV-1. Aqui, os autores apresentam as principais lesões dermatológicas descritas em pacientes infectados pelo HTLV-1, destacando o valor epidemiológico e clínico desses achados.

El herpes zoster es una infección aguda, causada por el virus de la varicela zoster (VZV), que se manifiesta clínicamente por la aparición de vesículas umbilicadas, limitadas a un dermatoma, acompañándose de dolor y en ocasiones de malestar general. الملخص

Cytomegalovirus infection is common disease but most of the healthy patients are asymptomatic. Cytomegalovirus infection is a serious problem in immunocompromised patients particularly in acquired immunodeficiency syndrome. About 90 per cent of patients with AIDS develop active cytomegalovirus infection and up to 25 per cent may experience life threatening infection due to the virus. However, cytomegalovirus cutaneous lesion have rarely been reported. A 40-year-old Thai man presented with multiple prurigo nodularis-like lesions on both legs and feet. The investigation revealed consistent with acquired immunodeficiency syndrome associated with cytomegalovirus cutaneous infection.
