ABSTRACT

OBJECTIVE To explore the construction of a new scientific management model for temporary drug purchase, and to provide a reference for hospitals to improve the level of rational drug use. METHODS Guided by clinical diagnosis and treatment needs and patient medication safety, our hospital carried out the whole process management practice of temporarily purchased drugs by optimizing the review process, creating a review team, formulating pre-audit and post follow-up evaluation standards based on comprehensive drug evaluation, and evaluated the practice effect through the number of temporary purchase applications, implementation rate, drug structure optimization and other indicators. RESULTS Since January 2021, our hospital had implemented a new mode of temporary drug purchase management. By December 2022, clinical pharmacists had reviewed 111 temporary drug procurement applications, effectively intercepted 13 irrational drug use applications (11.71%), reduced the overall implementation rate of temporary drug procurement by 8.36%,and proposed five batches of drug structure optimization suggestions; 24 drugs were successively introduced such as camrelizumab, sorafenib, busulfan. After optimizing the management mode, the number of temporary drug procurement applications decreased by more than half from 133 in 2019 and 138 in 2020 to 66 in 2021 and 45 in 2022. CONCLUSIONS The model is helpful to optimize the hospital drug catalog, strengthen rational drug use, ensure the safety of patients’ drug use, and fully reflects the professional value of clinical pharmacists in hospital pharmacy management and rational drug use.

ABSTRACT

Objective:To investigate the effect and mechanism of NACK knockdown on the proliferation and apoptosis of T-cell acute lymphoblastic leukemia (T-ALL) Jurkat cells. Methods:Lentivirus transfection technology was used to transfect Jurkat cells and knock down NACK gene.Real time fluorescent quantitative PCR and Western blot were used to detect the silencing efficiency of NACK gene.CCK-8 method and flow cytometry were used to detect the effects of NACK knockdown on the proliferation and apoptosis of Jurkat cells. The expressions of protein related with Notch1 pathway, such as Hes1 and c-Myc, were detected by Western blot. Results:After NACK-shRNA was successfully transfected into Jurkat cells by lentiviral vector, the expression of NACK mRNA and protein was reduced signi-ficantly ( P<0.05). Compared with the negative control group and the blank control group, the CCK-8 method showed that the cell proliferation in the experimental group was significantly inhibited [The inhibition rates of cell proliferation in the experimental group, negative control group and blank control group were (37.27±4.48)%, (4.25±2.10)% and (2.43±1.40)%, respectively]( F=132.640, P<0.05), and the flow cytometry test showed that the apoptosis in the experimental group increased significantly [The apoptosis rates of experimental group, negative control group and blank control group were (26.38±3.03)%, (6.07±2.61)% and (3.40±1.98)%, respectively]( F=90.534, P<0.05). Western blot results confirmed that the expression of Notch1 pathway-related proteins Hes1 and c-Myc was down-regulated compared with the negative control group and the blank control group, and the difference was statistically significant ( P<0.05). Conclusions:Targeting silent NACK can down-regulate the expression of Notch1 pathway-related proteins, which leads to the inhibition of Jurkat cell proliferation and increased apoptosis, thereby exerting its anti-T-ALL effect.

ABSTRACT

Objective To establish a two-dimensional high-performance liquid chromatography method for the determination of tigecycline in human cerebrospinal fluid, which can be used for the drug monitoring in patients with intracranial infection. Methods The quantification was carried out by an external standard method. The first-dimension column was a Aston SNX5 phenyl chromatographic column (50 mm×4.6 mm, 5 μm) with ammonium phosphate (pH was adjusted with ammonium hydroxide to 7.5)-methanol (45∶55, V/V) as the mobile phase and the flow rate was 1.2 ml/min. The second-dimension chromatographic column was Aston SC5 C18 (275 mm×4.6 mm, 5 μm), with ammonium phosphate (pH was adjusted with ammonium hydroxide to 7.4)-ammonium phosphate (pH was adjusted with ammonium hydroxide to 3.0)- acetonitrile (30∶50∶20, V/V/V) as the mobile phase and the flow rate was 1.0 ml/min. The detection wavelength was 340 nm. The temperature was 40 ℃ and the injection volume was 200 μl. Results The calibration curve of tigecycline showed good linearity from 64.5 to 1 290.0 ng/ml in human cerebrospinal fluid (r=0.999 8). The RSD of intra and inter-day precision were less than 5.0% with the detection accuracy of 98.80%−106.51%. Conclusion This method is simple, quick, accurate, specific and sensitive. It meets the requirements of tigecycline determination in clinical human cerebrospinal fluid, which offers the individualized therapeutic assurance for patients with intracranial infection.

ABSTRACT

OBJECTIVE: To investigate the best compatibility proportion of Mongolian medicine “Terminalia chebula decomposing the poison of Aconitum kusnezoffii”. METHODS: Totally 40 rats were randomly divided into blank control group (0.05% CMC-Na), raw A. kusnezoffii group (0.12 g/kg by crude drug) and raw A. kusnezoffii-T. chebula (1 ∶ 3,1 ∶ 1,3 ∶ 1, mass ratio) group (0.12 g/kg raw A. kusnezoffii by raw material), 8 rats in each group. They were given relevant medicine intragastrically once a day, for consecutive 28 d. 0.5 h after last medication, serum contents of cTn-I and MB were determined, and the changes of cardiological structure were observed; the detoxification effects of T. chebula on cardiotoxicity induced by A. kusnezoffii were investigated. Binding rates of 3 kinds of aconitine (concentrations of aconitine, mesaconitine and hypaconitine were 5, 10, 20, 40, 80, 160, 400 ng/mL) to binding rate of plasma protein with normal human plasma were determined by equilibrium dialysis combined with liquid chromtography-mass spectrometry. The concentrations of 3 kinds of aconitine were fixed as 100 ng/mL. After adding main detoxification component tannic acid in different proportions of T. chebula (1 ∶ 0.025, 1 ∶ 0.075, 1 ∶ 0.1, 1 ∶ 0.5), the effects of them on plasma protein binding rate of 3 kinds of aconitine were investigated; the possible mechanism of T. chebula decomposing the poison of A. kusnezoffii inducing cardiotoxicity were investigated. RESULTS: In detoxification experiment, compared with blank control group, serum contents of cTn-I and MB were increased significantly in raw A. kusnezoffii group (P<0.05). There were obvious pathological changes in myocardial tissue, such as disorder of cell arrangement, cell shrinkage and cytoplasm staining. Compared with raw A. kusnezoffii group, serum contents of cTn-I and MB in raw A. kusnezoffii-T. chebula (1 ∶ 3, 1 ∶ 1, 3 ∶ 1) groups were decreased significantly (P<0.05), and there was no significant difference between the raw A. kusnezoffii-T. chebula (3 ∶ 1, 1 ∶ 1) groups and blank control group (P>0.05); myocardial pathological changes were improved to varying degrees. The structure of myocardial tissue in raw A. kusnezoffii-T. chebula (3 ∶ 1) groups were similar to blank control group. In the mechanism investigation experiment under the condition of different concentrations, plasma protein binding rates of 3 kinds of aconitine with normal human plasma were about 30%, without statistical significance (P>0.05) and significant concentration-dependent manner. After adding tannic acid, plasma protein binding rate of 3 kinds of aconitine in A. kusnezoffii was decreased to different extent; when 3 kinds of aconitine were combined with tannic acid in the ratio of 1 ∶ 0.1, 1 ∶ 0.075 and 1 ∶ 0.5, the plasma protein binding rate decreased significantly (P<0.05), in proportion-dependent manner. CONCLUSIONS: Compatible use of raw A. kusnezoffii-T. chebula (3 ∶ 1) show that best detoxification effect on A. kusnezoffii-induced cardiotoxicity. Under this compatibility ratio, the plasma protein binding rate of 3 kinds of aconitine in A. kusnezoffii with normal human plasma is relatively high and the free drug concentration is relatively low.

ABSTRACT

OBJECTIVE:To study the relationship between cisplatin-resistant of human ovarian cancer cells (SKOV3) and Toll-like receptor 9 (TLR9) under hypoxia. METHODS:Immunofluorescence method was used to detect the TLR9 expression in rupture and intact membranes SKOV3. SKOV3 was taken, adding cisplatin, then CCK-8 was used to detect cell inhibition rate af-ter 6,12,24 h of added into TLR9 specific agonists CpG-ODN 2006 (group A) and its isotype control CpG-ODN 2006 control (group B). SKOV3 or SKOV3 pretreated by 0(not added),1,10,102,103 μmol/L TLR9 specific antagonist chloroquine for 3 h were taken, adding cisplatin, cell inhibition rate was detected after 24 h of added into SKOV3 normoxia(21% O2),hypoxia(1%O2)incubating 6,12,24 h,HICR under hypoxia in pretreatment was calculated. Western blot method was used to detect the multi-drug resistance associated protein(MRP)expression in SKOV3 under the conditions of normoxia 24 h cell supernatant(group C), hypoxia 24 h cell supernatant(group D),hypoxia 24 h cell supernatant+10 μmol/L chloroquine(group E). RESULTS:TLR9 was expressed in both cell membrane and cytoplasm of SKOV3. Compared with group A,cell inhibition rate in group B was decreased (P<0.01). Compared with normoxia cell supernatant, cell inhibition rate was decreased after hypoxia cell supernatant (P<0.05). Compared with not added chloroquine, adding 10,102 μmol/L chloroquine can obviously decrease HICR(P<0.01),and the most significant decrease was showed when using hypoxia 24 h cell supernatant+10 μmol/L chloroquine. Compared with group C,MRP expression in group D was strengthened (P<0.01);compared with group D,MRP expression in group E was weakened (P<0.01). CONCLUSIONS:TLR9 receptors can be activated under hypoxia, and cause SKOV3 to cisplatin-resistance,the effect may be related with up-regulating MRP expression.

ABSTRACT

The World Health Organization Quality of Life-BREF [WHOQOL-BREF] has generally been used for patients, few studies in migrants who move from rural to urban within one country. Many studies asserted that social isolation presents a risk to individual health. Poor social networks are associated with worse QOL. This study examined health-related quality of life [HRQOL] and social support in new-generation migrant workers and compared it with urban workers. Nine hundred thirty new-generation migrant workers and 939 urban controls completed the WHOQOL-BREF questionnaire and Social Support Rating Scale [SSRS] by stratified sampling in 2011. Spearman's correlation was performed to clarify the relationship between social support and HRQOL in migrants. Multiple linear regression analyses were used to identify the variables that were associated with HRQOL. The general health, psychological health, and environmental scores of QOL in new-generation migrant workers were lower than in urban workers. New-generation migrants had poorer social support compared with urban controls with regard to general support, objective support, and support utilization. A positive correlation was found between social support and HRQOL. Workers with a higher level of education achieved better psychological, environmental, and general scores than workers with a primary education. Physical, social, environmental, and general health was also closely connected with the age factor. Physical health scores were higher in males than in females. These data suggest that new-generation migrant workers have significant impairment in HRQOL and receive less social support. HRQOL may be affected by social support, education, age, and gender

ABSTRACT

Objective To detect the proteins levels of RhoA and CDC42 in bone marrow mononucleated cells (BMMC) of patients with primary acute leukemia, and further determine the role of abnormal interactions between hematopoietic progenitor and bone marrow microenvironment on abnormal behaviors of leukemia cells. Methods BMMC samples were separated from 54 primary acute leukemia patients and 22 normal donors and the cell lysis samples were prepared. RhoA and CDC42 proteins were determined by Western blotting. Independent pair T test was conducted to evaluate whether the differences in RhoA and CDC42 expression were statistically significant between leukemia patients and normal donors. Spearman was applied in analyzing the correlation between expression of RhoA and CDC42 proteins and clinical characters of patients. Results RhoA and CDC42 proteins level of primary acute leukemia patients was significantly higher than that of normal samples. Especially, patients with M2,M3 and M5 subtypes exhibited significant higher RhoA proteins levels and M3 subtype exhibited significant higher CDC42 protein levels. Conclusion RhoA and CDC42 protein levels of primary acute leukemia patients are significantly higher than that of normal donors. This result suggests that RhoA and CDC42 associated efficient migration of leukemia cells could be implicated in abnormal interaction of leukemic cell with bone marrow microenvironment.

ABSTRACT

Various questionnaires for screening diabetes in Chinese population were evaluated. Two questionnaires (Finnish Diabetes Risk Score and Danish Diabetes Risk Score) were applied. Sensitivity, specificity and the area under the receiver operating characteristic (ROC) curve (AUC) for both questionnaires were calculated. The AUCs were 0. 760 (95% CI: 0.713-0.806) for the Finnish questionnaire (P<0.01) and 0.706 (95% CI: 0.647-0.764) for the Danish one (P<0.01). The sensitivities, specificities and predictive values of both questionnaires were not so good in the present survey as in the published reports. It suggests that these screening questionnaires must be modified to suitable for Chinese population.

ABSTRACT

OBJECTIVETo determine the ETO-interaction domain of nuclear receptor co-repressor (N-CoR) for abolishing the biological function of AML1-ETO.

METHODSTen different regions of N-CoR (N-CoRYs) were generated by means of polymerase chain reaction (PCR), and cloned into yeast expression plasmid pGADGL to construct pGADGL/N-CoRYs. The yeast two-hybrid technique and X-gal staining were used to determine the binding between the 10 different regions of N-CoR and ETO.

RESULTSIt was shown that the co-existence of 988-1,126 and 1,551-1,803 amino acid residues of N-CoRY was the ETO-interaction domains required for the binding with ETO.

CONCLUSIONTwo domains of N-CoR that interact with two zinc fingers of ETO, and keep stable binding between the two proteins were identified.

ABSTRACT

Objective To investigate the relations between intrauterine asphyxia and peroxidation and newborn hypoxic-ischemic encephalopathy(HIE). Methods The levels of superoxide dismutase (SOD) and malondialdehyde (MDA) in cord blood of 60 newborns with intrauterine asphyxia during labor(which was divided into two groups,39 cases with asphyxia in groupⅠ, and 21 cases with asphyxia in groupⅡ),and in 30 newborns without intrauterine asphyxia(control group) were determined. The levels of SOD and MDA in cord blood of newborns with HIE were compared with those in newborns without HIE. The incidence of HIE was estimated simultaneously. Results (1) The levels of SOD were (12 896?247) U/g Hb in groupⅠ, (9846?268) U/g Hb in groupⅡ, (17 282?134) U/g Hb in control group, significantly lower in the former two groups compared with control group, while the level of SOD in group Ⅰ was higher than that in group Ⅱ(P121 min group, and the levels of SOD was (9786?249) U/g Hb.(2)The levels of MDA were (6.3?0.4) ?mol/L in group Ⅰ, (8.6?1.5) ?mol/L in group Ⅱ, and (4.1?0.5) ?mol/L in control group, significantly higher in the former two groups compared with control group (P

ABSTRACT

AIM: To investigate homeodomain-interacting protein kinase 2(HIPK2) gene expression level and its clinical significance in acute leukemia(AL) patients.METHODS: HIPK2 mRNA was determined by semi-quantitative reverse transcriptase polymerase chain reaction(semi-quantitive RT-PCR) in patients with acute leukemia and healthy donors. The relative transcription level was compared between the study group and the control group.RESULTS: ① The relative expression level of HIPK2 mRNA in AL patients was 0.364?0.286,significantly lower than that in control(1.160?0.272,P0.05).CONCLUSION: AL has significantly lower HIPK2 mRNA expression as compared to normal control, which may be associated with leukemogenesis and/or disease progression of AL.
