ABSTRACT

OBJECTIVE@#To investigate the mechanism of cAMP-PKA signaling pathway mediated by Chinese medicine formula Shaoyao Gancao Decoction (, SGD) on the regulation of aquaporin 5 (AQP5) and muscarinic receptor 3 (M3R) levels in Sjögren's syndrome (SS).@*METHODS@#Of the 30 mice, 5 were randomly selected as control, and others were used for creating SS model. After successful modeling, mice were randomly divided into 5 groups (n=5 per group) and intragastrically administered with saline (8 mL/kg), pilocarpine (1.4 mg/kg), or low, medium and high doses SGD (0.14, 0.21, 0.35 g/kg Radix paeoniae with 0.01 g/kg Radix glycyrrhizae, respectively) for 6 weeks. Human labial gland acinar cells were treated with pilocarpine or varying doses of SGD with saline as the placebo. Hematoxylin and eosin staining was used to observe the histopathological changes of the submandibular glands of mice. The serum levels of anti-SS antigen A (SS-A), anti-SS antigen B (SS-B), M3R, and α-fodrin in submandibular glands of mice were measured by enzyme-linked immunosorbent assay. Immunofluorescence staining was used to observe the spatial localization of AQP5 and M3R in acinar cells. Reverse transcriptase polymerase chain reaction and Western blot were used to detect the expressions of PKA, cAMP, Epac1, AQP5, M3R, nuclear factor kappa-B (NF-κB), and tumor necrosis factor (TNF)-α in submandibular gland tissues and cells of each group.@*RESULTS@#Compared to normal mice, body weight, 5-min salivary secretion, 30-min secretion of tears and breakup time of tear film of model mice decreased at 1-6 weeks after immunization (all P<0.05), whereas water intake increased (all P<0.05). In the model group, glands of the submandibular glands showed atrophy, accompanied by acini of different sizes, decreased numbers and loose arrangement, with catheter dilatation and different degrees of lymphocyte infiltration. Conditions of mice in SGD groups were improved. The positive expression of AQP5 and M3R were higher in the acinar cells treated with all doses SGD compared to the normal group; serum levels of SS-A, SS-B, and α-fodrin were lower, and that of M3R was higher in all doses SGD treated animals than the model or pilocarpine treated ones (all P<0.05). Compared to the model and pilocarpine groups, the mRNA and protein levels of NF-κB and TNF-α were lower in mice or cells treated with medium or high-dose SGD (all P<0.05), while those of PKA, Epac1, AQP5 and M3R were higher (all P<0.05).@*CONCLUSION@#SGD can improve symptoms of SS by regulating the cAMP-PKA signaling pathway and increasing AQP5 and M3R levels.

ABSTRACT

OBJECTIVES@#To evaluate the response to cardiac resynchronization therapy (CRT) and the correlation between CRT and pulmonary artery hemodynamic parameters.@*METHODS@#The patients with chronic heart failure indicator for CRT were enrolled. The left ventricular end-systolic volume (LVESV) was measured by echocardiography and New York Heart Association (NYHA) classification was evaluated between one week before and six months after CRT. Mean pulmonary artery pressure (mPAP), pulmonary artery systolic pressure (PASP) and pulmonary vascular resistance (PVR) were measured by right heart catheterization. Left ventricular reverse remodeling (LVRR) is defined as a decrease of 15% or more in LVESV at the 6th month after CRT; Clinical response is defined as a decrease of NYHA classification at or above grade 1 at the 6th month after CRT. Pulmonary hypertension (PH) was defined as mPAP≥25 mmHg. According to the response, patients were divided into 3 groups: group A (LVRR+clinical response), group B (no LVRR+clinical response) and group C (no LVRR+no clinical response). The changes of NYHA classification, echocardiographic and pulmonary hemodynamic parameters were observed in the 3 groups. The Kaplan-Meier survival curve was used to analyze the differences in all-cause mortality, combined end-point events of death or re-hospitalization due to heart failure among different groups.@*RESULTS@#A total of 45 patients with CRT implantation [aged (63.27±9.55) years, 36 males] were included. The average follow-up period was (33.76±11.50) months. Thirty-one patients (68.89%) were in group A, 9 of whom with PH. Eight patients (17.78%) were in group B, 7 of whom with PH. Six patients were in group C, all with PH. Cardiac function including NYHA classification, echocardiographic and pulmonary hemodynamic parameters had been significantly improved in group A after CRT implantation (0.05). There were no significant changes in NYHA classification, echocardiographic and pulmonary hemodynamic parameters in group C (>0.05). Compared with group C, group A and group B had lower all-cause mortality (=0.005) and lower incidence of composite endpoint events (=0.001).@*CONCLUSIONS@#Patients with LVRR and clinical response after CRT have a good prognosis. Patients with clinical response but without LVRR have a better prognosis than those without clinical response and LVRR, which may be related to the decrease of pulmonary hemodynamic parameters such as mPAP and TPG.

ABSTRACT

With the better diagnosis of early gastric cancer, increasing diversity and complexity of its treatment has united a multidisciplinary team led by physicians and surgeons. Japanese Gastric Cancer Treatment Guidelines are compiled and reviewed by a committee composed of physicians, surgeons and pathologists, which are based on the epidemiological trend of gastric cancer in Japan over the past two decades and clinical evidence from Japanese domestic studies and other promi-nent international clinical trials. Updated every three to six years, the guidelines now have five versions. They are one of the authoritative documents for the diagnosis and treatment for gastric cancer in the world. As far as the treatment strategies for early gastric cancer are concerned, they include the changes of endoscopic resection, the evolution of minimally invasive surgery, the widespread application of laparoscopic surgery and the promotion of the concept of functional preservation. The evolutionary trends are perfect reflections of a philosophical shift of Japanese physicians towards precision medicine:from radical resection to functional preservation, and from long-term survival only to better quality of life.

ABSTRACT

Standardized surgical residency training is an important part of graduate medical education, a necessary path towards becoming a surgeon with the capability to deal with common clinical surgical problems in an independent, correct and evidence-based manner. It is also the fundamental policy for homogeneous clinician training, medical human resources construction and improvement of health care quality. Since its nationwide implementation at the end of 2013, there have been great achievements while problems and confusions were also exposed. Surgical residency training is now facing new challenges and opportunities. With the rapid development of minimally invasive surgery, young surgeons have been actively promoted to learning laparoscopic technique; however, traditional surgical basic skills as well as the essence of handson apprenticeship shall not be neglected. In the meantime, the Internet, and the series of revolutions and breakthroughs it brings, need to be utilized in residency training and construction of a system for standardized training management, which can be a creative model for future medical education.

ABSTRACT

There are many kinds of processed products of Aconiti Lateralis Radix Praeparata (ALRP), but their differences in toxicity and efficacy have not been identified. The minimum premature ventricular contraction (PVC) method was used to evaluate the biological toxicity of eight processed products. The results showed that the minimal toxic dose (MTD) of an ethanol extract of Shengfupian (SFP) was 0.16 g·kg-1, which was much lower than that of Heishunpian (HSP), Baifupian (BFP), Baofupian (BAP), Paofuzi (PFZ) or Zhengfupian (ZFP), with MTDs ranging from 2.64 to 5.75 g·kg-1. No cardiotoxicity was detected with Chaofupian (CFP) and Paotianxiong (PTX). A novel +dp/dtmax assay for acute heart failure in rats was developed to evaluate the cardiac activity. It was found that all eight processed products had cardiac effects, with Shengfupian showing the strongest cardiotonic effect and the ability to restore damaged cardiac function to normal within 15 minutes of injection. Heishunpian, Baifupian and the three other products displayed moderate activity, while Paofuzi and Paotianxiong were the weakest. An LC-MS/MS method was utilized to determine the content of 13 alkaloids in water extracts. The results demonstrated that hypertoxic aconitine, mesaconitine, and hypaconitine could not be detected, higenamine was only present in Shengfupian, and salsolinol was about 4-56 times higher in Shengfupian than in other products. A correlation analysis showed that salsolinol had the best correlation with the cardiotonic index, with a correlation coefficient as high as 0.817, while the three monoester alkaloids failed to correlate with the cardiotonic effect. Higenamine and salsolinol were cardiotonic, while the 11 other components had no cardiotonic activity. This study establishes methods for precise evaluation of cardiotoxicity and cardiac activity, reveals the toxicity and efficacy of common processed products, and identifies the key quality markers for cardiac activity, providing scientific support for the quality evaluation and clinical application of processed products of aconite.

ABSTRACT

Objective To understand the spatial and temporal distribution characteristics of dengue fever in China from 2011 to 2018, and predict the incidence of dengue fever in China in 2019. Methods Based on the case data of dengue fever in China from 2011 to 2018 in the Chinese Disease Prevention and Control Information System, the trend of dengue fever was described and predicted by using the autoregressive integrated moving average model (ARIMA) with R 3.6.0 software. Based on the data of the incidence of dengue fever in the country, provinces and cities from 2011 to 2016 provided by the national scientific data sharing platform for population and health, global and local spatial autocorrelation analysis was performed using GeoDa 1.12 software to determine the dengue fever hotspots. Results The incidence of dengue fever was 14 302 in 2019, showing no disease outbreaks. The incidence of dengue fever in 2012(Moran’s I=-0.088, P=0.037), 2013(Moran’s I=-0.121, P=0.040) and 2014(Moran’s I=-0.076, P=0.045) showed a global spatial negatively correlaton. In 2016(Moran’s I=0.078, P=0.048), the incidence of dengue fever was positively correlated with global space. The results of local autocorrelation analysis showed that the high incidence of dengue fever was mainly in the southeast coastal areas of China. Conclusions In 2019, the epidemic of dengue fever in China showed no obvious fluctuation trend, and the epidemic situation showed spatial clustering distribution.

ABSTRACT

Line scanning quantitative analysis method on silicate with small laser beam ( < 15 μm) was developed using laser ablation sector field inductively coupled plasma mass spectrometry (LA-SF-ICP-MS). Differences on signal intensity and elemental fractionation induced by different laser sampling patterns were compared. While spot ablation with small laser beam, the elemental signal intensity decreased with time significantly, and the elemental fractionation was obvious. In contrast, the elemental signal intensity by line scanning was higher and more stable and line scanning was free of elemental fractionation. Therefore, identical ablation pattern and condition should be used for the standard and the unknown sample in LA-ICP-MS quantitative analysis. A single pulse experiment was carried out to investigate the washout time when coupled to two-volume ablation cell. The result indicated that the elemental intensity decayed to the background value needed 2-3 s. The optimal parameters on SF-ICP-MS were set to reduce the effect of signal overlapping. Homogeneous sample KL2-G and titanite grains with composition zoning were analyzed by this method. Accurate element contents and element ratios indicated that fast washout time and optimal instrument parameters made it feasible to perform line scanning quantitative analysis accurately. Comparing to traditional microanalysis, line scanning quantitative analysis could reduce the laser beam size (<15 μm) and improve the spatial resolution efficiently. The potential of the technique to unveil compositional complexities in greater detail would help to improve our understanding of geochemical processes in mineral scale.

ABSTRACT

PURPOSE: Previous western studies have found Caucasians with skin cancer, either melanoma or nonmelanoma skin cancer (NMSC), have an elevated risk of second primary cancer. Our objective was to assess the risk of second primary cancer in Taiwanese with NMSC. MATERIALS AND METHODS: By using data from Taiwan's National Health Insurance Research Database, we conducted a population-based cohort study to assess the risk of incident second primary cancer in Taiwanese affected by NMSC. RESULTS: We identified 505 subjects with NMSC and 2,020 matched controls. After adjustment for potential confounders including age, sex, urbanization, and Charlson Comorbidity Index, people who had NMSC had a 1.43-fold (95% confidence interval [CI], 1.05 to 1.96) risk for the development of second primary cancer as comparedwith control group. Menwith NMSC had a 2.99-fold (95% CI, 1.00 to 9.10) risk for second primary cancer involving the lip, oral cavity, and pharynx and a 3.51-fold (95% CI, 1.21 to 10.17) risk for second primary cancer involving the genitourinary organs when compared to the control group. By contrast, women with NMSC did not have an increased risk of second primary cancer. CONCLUSION: This study revealed Asians with NMSC have an increased risk of second primary cancer. Our findings can be a useful reference for health care for people diagnosed with NMSC.

ABSTRACT

Objective To compare the effect of impulse electroacupuncture (impulse-EA) and music electroacupuncture (music-EA) for Tongdu Qishen on spatial learning and memory, and expression of cytokine in the hippocampus of APP/PS1 mice. Methods A total of 32 APP/PS1 double transgenic male mice were randomly divided into model group (n=8), drug group (n=8), impulse-EA group (n=8) and music-EA group (n=8), the same background and age male C57BL/6 mice were observed as normal group (n=8). The impulse-EA group and music-EA group accepted EA at Baihui (GV20) and Yintang (GV29), connected with their own electroacupuncture stimulators, for 20 minuts, then, they were pricked Renzhong (GV26) for a while. The drug group accepted donepezil hydrochloride 0.92 mg/kg intra-gastrically. The normal group, model group and drug group were grabbed and bounded in the same way. After 15 days of treatment, they were assessed with Morris water maze. The expression of high mobility group box pro-tein-1 (HMGB1) and interleukin-10 (IL-10) in hippocampus were measured with immunohistochemistry and Western blotting. Results The escape latency shortened inEA groups compared with that of the model group since the fourth day of Morris water maze training (P<0.05), and it was the least in the music-EA group; and the ratio of swimming across the target quadrant increased (P<0.05), but there was no significant difference between EA groups (P>0.05). The ex-pression of HMGB1 decreased (P<0.05) and the expression of IL-10 increased (P<0.05) in EA groups com-pared with that of the model group, and HMGB1 decreased more and IL-10 increased more in the music-EA group than in the impulse-EA group (P<0.05), according to the measurement in immunohistochemistry, not in Western blotting. Conclusion Both impulse-EA and music-EA based on Tongdu Qishen can promote the recovery of the learning and memory in APP/PS1 mice, and music-EA may do more effectively in inhibition of pro-inflammatory factors and promotion of the anti-inflammatory factors.

Objective To design a virtual reality(VR) system for fracture reduction training using manikin technology. Methods The real-time posture information of the manikin skeleton was acquired by the inertial measurement unit(IMU).3DMAX was used to construct a 3-dimensional human skeleton model based on the human skeleton CT.The model was driven by the received sensor data and displayed the instant attitude. The skeletal attitudes and operation information were displayed in real time through the Visual Studio platform, which was displayed on the screen facing the trainee. Thus,a VA combination interactive training system was constructed. Results and Conclusion A set of simulated fracture reduction training system with high accuracy, low cost and VR combination was designed.Compared with the high-cost,precision optical instrument, the root mean square error (RMSE) of the fraction angle calculation was 0.595° in the static state, and was 1.609° in the dynamic state. This system can provide effective information for training operations and provide a new interactive platform for fracture reduction training.
