ABSTRACT

Objective To analyze the clinical efficacy of the laparoscopic breast-conserving surgery combined with radiofrequency ablation to treat the early-stage breast cancer. Methods We collected 55 patients diagnosed early-stage breast cancer in retrospect, which started from January 2014 to December 2016.Twenty-seven of them were performed the laparoscopic breast-conserving surgery combined with radiofrequency ablation while others went through laparoscopic breast-conserving surgery without radiofrequency ablation.Meanwhile,we adopted the student t-test and the chi-square test to compare results of two groups.More specific, the main indexes of this study are including the post-operative local recurrence, the incidence of fat liquefaction or the incision-infection,operation time,post-operative hospital stay and the hospitalization expense. Results The laparoscopic breast-conserving surgery combined with radiofrequency ablation group had low local-recurrence than the laparoscopic breastconserving surgery group (0 and 7.69%).Additionally,there were no statistical differences between two groups in the incidence of fat liquefaction. However,The laparoscopic breast-conserving surgery combined with radiofrequency ablation group had more hospitalization expense than the laparoscopic breast-conserving surgery group [(4.1 ± 0.7) ten thousand yuan and (2.3 ± 0.6) ten thousand yuan,P < 0.05].Conclusions Although the laparoscopic breast-conserving surgery combined with radiofrequency ablation group remarkably increased the hospitalization expense because of the utility of the radiofrequency ablation related apparatus,it may provide the probability of shaving more residual tumor cell and may low down the recurrence, especially not rising up the incidence of the post-operative fat liquefaction. Therefore, this surgery method might be one of the potential developments in the minimal-invasive of early stage breast cancer.

ABSTRACT

Objective To explore methods for performance appraisal of directors of municipal hospitals in Shanghai. Methods To assess the management performance of directors of 24 tertiary hospitals each year, measured by five dimensions of social satisfaction, management efficiency, capital operation, development sustainability and staff satisfaction, and 23 indexes. Results Implementation of the performance appraisal has witnessed constant improvement of patient satisfaction and quality of medical care,as well as rational control of medical expenses and cost.In 2014 for example, the cost of emergency/outpatients per visit at such hospitals were 312 yuan and 1 5 600 yuan per hospitalization respectively, with 7.6 days of stay in average. Also improved were service efficiency, clinical research output and hospital business performance in general. Conclusion Performance appraisal of directors of public hospitals proves an important means for guiding the hospitals to adhere to public welfare, consistent with the government medical institution operation target, and to strengthen internal management of such hospitals.

ABSTRACT

Objective Studies of the positioning, framework and weight of the performance of directors of third-grade public hospitals will pave the way for launching a study of the performance of these directors, and for the performance appraisal of the directors of third grade public hospitals. Methods The following methods are called into play:Meta-analysis of literature to summarize the research outcomes of the appraisal thinking and modeling for managers of public hospitals;the balance score card to design the theoretical base of public hospital performance framework;the Delphi Method to identify the functionality expected of public hospitals, and the thinking and framework for performance appraisal;in addition to experts consultancy to demonstrate via questionnaires. Results Positioning of directors performance appraisal of third-grade public hospitals comprises both desirable social effect outcomes and desirable operating status. This performance appraisal consists of such five dimensions as social satisfaction, effective management, asset operation, development sustainability, and employee recognition, totaling 12 performance objectives. Conclusion Scientific and reasonable design of the framework and weight of the perfofinance appraisal for managers of public hospitals is conducive to assuring the public interest nature for public hospitals.

ABSTRACT

Performance appraisal is an important means for keeping hospitals on correct tracks for their medical services and their public interest nature,in addition to its importance in enhancing internal performance management within public hospitals.Since 2006,Shanghai Shenkang Hospital Development conducted performance appraisal for directors of 23 public hospitals in Shanghai. This paper highlights the importance of performance appraisal for these directors, and its significance in keeping the correct director of hospital operation.

ABSTRACT

Objective To separate and cultivate homo umbilical cord blood (UCB) hematopoietic stem cell (HSC) in vitro, and u-tilize bone marrow desmohemoblast stem cell as trophoblastic layer combined with cytokine to amplify umbilical cord blood hematopoietic stem/progenitor cell. Methods Ficoll lymph-cell separating medium density gradient centrifugalization was used to segregate UCBHSC.Bone marrow desmohemoblast stem ceil and cytokine were added, and the sum of NC cells and CD34 + cells was counted. Results The sum of NC cells amplified 75.2±15.0 times, and the sum of CD 34 + cells amplified 18.7±12.3 times. Conclusions It has significant effect on amplification of hematopoietic stem cell with bone marrow desmohemoblast stem cell and eytokine when HSC are cultured in vitro.

ABSTRACT

Objective To find out about the intention of medical workers with regard to reward for personal services and explore the value of their labor. Methods An intention survey through questionnaires was conducted by means of random sampling among 1 424 medical workers from 6 hospitals in Shanghai with regard to reward for personal services and a model of expected income was obtained via multivariate stepwise regression analysis. Results Only 2.9% of the medical workers felt satisfied with their current total income. Such factors as professional titles, educational backgrounds, working years, and specialties all exerted some impact on the medical workers expected income. The expected income was several times that of the actual income, and there was a large difference between the expected incomes of people at different levels. Conclusion Reward to medical workers for personal services in China needs to be enhanced and reform of the distribution system should keep step with the reform of the personnel system.

ABSTRACT

Objective To determine and compare the complete sequence of COII gene from Anopheles sinensis, Aedes albopictus and Culex quinquefasciatus and to identify the molecular homology among these mosquitoes. Methods The recombinant plasmids including COII gene from the three species of mosquitoes were obtained by the T A cloning technique, and the COII genes were sequenced and analyzed. Results The homology of the COII gene nucleotide sequence and the deduced amino acid sequence of COII among the three species of mosquitoes varied from 84.1% to 87.9% and from 85.1% to 89.5%, respectively. The C+G content of COII gene was 23.2%-24.9%, the frequency of transversion higher than that of transition. Conclusion Aedes albopictus and Culex quinquefasciatus are closely related species at their COII molecular level according to COII gene.
