ABSTRACT

Objective:To determine the clinical effect of sequential therapy by local injection of triamcinolone acetonide and lattice CO 2laser for hypertrophic scar. Methods:A total of 80 hypertrophic scar patients, including 45 male and 35 female, in our clinic were randomly divided into test ( n=40) and control ( n=40) groups from March 2019 to May 2020. Patients aged from 18-42 years with average age 28.1. Patients in test groups were treated with triamcinolone acetonide and lattice CO 2laser sequentially. After final treatments, third-party blind evaluation, Vancouver scar scale, visual analog scale and dermatology life quality index were performed. Results:Test group acquired more satisfied result in third-party blind evaluation (82.5% vs. 52.5%, χ2=8.216, P<0.05). Vancouver scar scale, visual analog scale and dermatology life quality index were not significantly different before treatment for both groups while test group acquired better improvement after treatment ( P<0.05). Conclusions:Sequential therapy by local injection of triamcinolone acetonide and lattice CO 2laser is effective for hypertrophic scar and worths wide application in the clinic.

ABSTRACT

Objective:To investigate the relationship of HRAS gene mutation and lymph node metastasis and ultrasonographic characteristics in differentiated thyroid carcinoma (DTC).Methods:The clinical data of 162 patients with DTCs who underwent thyroidectomy and confirmed by postoperative pathological examination in Beijing Friendship Hospital from January 2014 to February 2019 were retrospectively analyzed. There were 139 patients with papillary thyroid carcinoma (PTC group) and 23 patients with follicular thyroid carcinoma (FTC group); the PTC were further classified as classic variant of papillary thyroid carcinoma (CVPTC, n=34), follicular variant of papillary thyroid carcinoma (FVPTC, n=36) and tall cell variant (TCV, n=69). Tissue HRAS mutation frequency was detected in 162 DTC patients and 19 patients with follicular adenoma (FA); blood HRAS mutation frequency was detected in 195 healthy subjects. The correlation between HRAS mutation (IVS1-82del gctgggcctggg) and cervical lymph node metastasis was analyzed, and the ultrasonographic characteristics of DTC patients were also analyzed. Results:The frequency of HRAS mutation in DTC patients was higher than that in healthy controls[37.0%(60/162) vs. 26.2%(51/195), χ2=4.538, P=0.03], while there was no significant difference between FTC and FA [39.1%(9/23) vs. 5/19, χ2=0.769, P=0.38]. In DTC patients there was no significant difference in cervical lymph node metastasis between HRAS mutation group and wild type group [57.3% (43/75) vs. 42.6% (32/75), χ2=1.898, P=0.16]. Among CVPTC, FVPTC and TCV patients, the rates of cervical lymph node metastasis were 7/12, 8/14 and 48.0% (12/25) in HRAS mutation group, while those were 50.0% (11/22), 40.9% (9/22) and 43.1% (19/44) in wild-type group, respectively (χ2=1.009, P=0.98).There were 9 patients with HRAS mutation in FTC group, and the cervical lymph node metastasis in mutation group and wild-type group was 5/9 and 4/14, respectively ( P=0.38). The ultrasonographic characteristics of PTC patients with HRAS mutation were more likely to have clear boundaries [66.7%(34/51) vs. 42.0%(37/88); χ 2=7.833, P<0.01] and not close to the membrane[84.3%(43/51) vs. 65.9%(58/88);χ2=5.506, P=0.02]. Conclusion:DTC patients are more likely to have HRAS (IVS1-82del gctgggcctggg) mutation, and the ultrasonic characteristics of DTC patients with HRAS mutation are likely to have clear boundaries and be not close to the membrane.

ABSTRACT

CX3CL1, also known as Fractalkine, is the only member of chemokines CX3C subclass. It plays an important role in a variety of central nervous system diseases and ischemic cerebrovascular diseases by binding to its specific receptor CX3CR1. In recent years, a large number of studies have investigated the specific role and related molecular mechanism of CX3CL1/CX3CR1. This article reviews the effect and molecular mechanism of CX3CL1/CX3CR1 in ischemic cerebrovascular disease, aiming to expand the understanding of the mechanism of CX3CL1/CX3CR1, and provide new ideas and intervention targets for the prevention, diagnosis and treatment of ischemic cerebrovascular disease.

ABSTRACT

With the improvement in people's awareness of diseases and the level of diagnosis and treatment, the incidence rates of immunoglobulin G4-related disease (IgG4-RD) and autoimmune liver disease (AILD) are constantly increasing, and IgG4-related sclerosing cholangitis is the overlapping part of the disease spectrum of IgG4-RD and AILD, while the association between IgG4-related autoimmune hepatitis (IgG4-AIH) and these two diseases remains unclear. This article reviews the hepatic manifestation of IgG4-RD, summarizes the clinical features of IgG4-AIH, and analyzes whether IgG4-AIH is a subtype of AIH or a hepatic involvement of IgG4-RD. It is believed that IgG4-AIH has similar but different clinical manifestations and histopathological features from classical AIH, and IgG4-AIH may be classified as two types, i.e., a subtype of AIH and the liver manifestation of IgG4-RD. The research on the pathogenesis, clinical features, and clinical diagnosis and treatment of IgG4-AIH should be taken seriously in future.

ABSTRACT

Rickettsia felis is a Gram-negative prokaryotic organism that is obligatorically parasitic in cells. Cat fleas are the main vector of Rickettsia felis. Clinical symptoms of human infection with Rickettsia felis include fever, fatigue, headache, macular papules, and eschar. There are few reports of serious complications or deaths due to infection of Rickettsia felis. A confirmed case of severe encephalitis caused by Rickettsia felis infection was admitted to neurology department of Affiliated Hospital of Jining Medical University on January 29, 2020. After comprehensive treatment of antiviral by acyclovir, mannitol dehydration to reduce intracranial pressure, human immunoglobulin to regulateimmunity, minocycline hydrochloride capsule, levofloxacin mesylate and sodium chloride against Rickettsia felis infection, control of seizure and pulmonary infection, the patient's condition was improved and then discharged. By summarizing the experience and lessons in the treatment of this patient, we hope to remind everyone to strengthen the understanding of Rickettsia felis infection.

ABSTRACT

Objective:To explore whether the professional commitment of nursing undergraduates was affected and changed during the COVID-19 epidemic.Methods:Three surveys were conducted on all nursing undergraduates in the school of nursing of a comprehensive university at different stages before and during the epidemic. Three surveys all adopted "College Students' Professional Commitment Survey Scale". SPSS 23.0 was used for descriptive statistics and ANOVA analysis.Results:The overall scores of professional commitment in the three surveys were (3.54±0.55), (3.42± 0.57) and (3.57±0.68), all of which were above the average level, with little change. There was no statistically significant difference in the mean scores of professional commitment among the three surveys ( F = 2.803, P = 0.062). In all dimensions, except ideal commitment ( F = 0.966, P = 0.381), affective commitment ( F = 3.502, P = 0.031), normative commitment ( F = 34.791, P < 0.001) and continuance commitment ( F = 7.632, P = 0.001) all had significant statistical significance. Conclusion:The change of professional commitment of nursing undergraduates during the epidemic was not significant, but it was affected positively. During the epidemic, the value of nursing was highlighted and the professional identity of nursing undergraduates was enhanced. We should vigorously carry forward the great anti-epidemic spirit, and make good use of anti-epidemic materials to carry out students' professional ideological education. At the same time, vocational spirit, knowledge of professional prevention and protection and specialized nursing education should be strengthened.

ABSTRACT

Objective:To explore the effects of habit reversal training on the use of inhaled corticosteroids (ICS) in patients with bronchial asthma.Methods:Select 89 patients with bronchial asthma using ICS admitted to the respiratory department of the Anhui No.2 Provincial People′s Hospital from 2016 January - 2019 January, and they were randomly divided into control group and study group, 45 cases in control group and 44 cases in study group. Patients in the control group received usual care and propaganda measures, the patients in the study group were intervened by the behavioral habit reversal training consisting of consciousness practice, behavior evaluation, antagonistic action training, anxiety management, follow-up and social support.. Asthma control test questionnaire (ACT), self-developed evaluation method of ICS inhalation technique, bronchial asthma quality of life questionnaire (AQLQ), were used to evaluate the difference of inhaled glucocorticoid corticosteroid therapy before and after the intervention.Results:The ICS inhalation technique scores (6.24±0.82) were higher than those of the control group (4.13±0.67), the difference was statistically significant ( t=-13.307, P<0.05). ACT scores of the study group were better than those of the control group ( Z=-2.780, P=0.005), the difference was statistically significant ( P<0.05). After intervention, AQLQ total scores in the study group, and the asthma symptoms, psychological status, and scores of the self-health dimension in AQLQ in the study group were (211.74±29.84), (51.09±6.49), (39.57±5.24), (24.36±3.41), higher than those in the control group(191.18±27.91), and (43.18±5.73), (31.21±4.62), (19.94±2.84), the difference was statistically significant ( t value was -7.988 - -3.358, P<0.05). Conclusion:Habit reversal training can effectively enhance the technical level of bronchial asthma patients operating ICS device, further improve the status of asthma control, improve the quality of life of patients, and improve the quality of nursing services for such patients.

ABSTRACT

Fetal omphalocele is a surgical disease with severe defect of fetal abdominal wall which requires emergency treatment after birth. The incidence rate is 2.1/10000, and the mortality rate is 52% to 67%. The improper handling of omphalocele can cause capsular rupture and intestinal exposure, leading to neonatal fluid loss, hypothermia, intestinal necrosis and other complications. It is the major cause of death in newborns. In this paper, we reviewed the characteristics, delivery mode, delivery time, umbilical cord care, and transferring of omphalocele newborns to provide reference for clinical nursing.

ABSTRACT

Objective@#To investigate the relationship between current-exposure time products (mAs) and image quality in digital mammography.@*Methods@#The CDMAM phantom of 2 to 7 cm thickness were exposed by manual exposure modes using Mo-Rh anode-filer combinations. The exposure parameters were set as follows: (2 cm, 27 kVp, 10 to 90 mAs); (3 cm, 29 kVp, 20 to 120 mAs); (4 cm, 29 kVp, 20 to 200 mAs); (5 cm, 30 kVp, 40 to 220 mAs); (6 cm, 31 kVp, 40 to 260 mAs); (7 cm, 32 kVp, 80 to 280 mAs). The image quality figure (IQF), contrast to noise ratio (CNR), figure of merit (FOM) and the average glandular dose (AGD) were obtained from images. The optimum filtration and kVp for each breast thickness was found from the calculated FOMs. The optimum mAs for each breast thickness were found from the calculated FOMs and IQFs. And the Pearson correlation analysis was used to analyze the correlations among AGD, CNR and IQF.@*Results@#With the increase of mAs, the average glandular dose were linearly increased, and the FOM first increased and then decreased or changed slowly. The AGD, CNR and IQF were significantly correlated when the compression thickness was less than 7 cm (r all>0.87, P<0.05). The optimized exposure parameters were list as follows (2 cm,27 kVp,20 to 30 mAs);(3 cm,29 kVp,30 to 50 mAs); (4 cm,29 kVp,80 to 100 mAs); (5 cm, 30 kVp, 80 to 120 mAs); (6 cm, 31 kVp, 100 to 140 mAs); (7 cm, 32 kVp, 80 to 120 mAs).@*Conclusion@#The optimum range of mAs can be confirmed with different breast compression thickness and the setting of automatic exposure parameters should be chosen in clinical practice.

ABSTRACT

Objective@#To investigate the relationship between exposure to famine in fetus and infant period and the risks for hypertension in adulthood.@*Methods@#A total of 5 960 participants born between 1956 and 1965 were included in the study and were divided into unexposed group (1963-1965), fetal exposed group (1959-1961), early- childhood exposed group (1956-1958) and transitional group (1962). Logistic regression model was used to explore the association between famine exposure in early life and the risk for hypertension in adulthood.@*Results@#Both the fetal exposure and the early-childhood exposure were the risk factors for hypertension in adulthood (OR=1.249, 95%CI: 1.049-1.486 and OR=1.360, 95%CI: 1.102-1.679). Meanwhile, in rural area, compared with unexposed group, the fetal exposure (OR=1.401, 95%CI: 1.091-1.798) and the early-childhood exposure (OR=1.460, 95%CI: 1.145-1.862) were also associated with a greater risk of hypertension in adulthood. In addition, fetal exposure and early-childhood exposure to famine in women were associated with 36.0% and 31.9% increased risks for hypertension (95%CI: 7.8%-71.7% and 95%CI: 4.8%-66.0%) according to the stratified analysis.@*Conclusion@#Fetal exposure to famine might increase the risk for hypertension in adulthood.

ABSTRACT

Objective@#To explore the effect of pre- and post-adaptive statistical iterative reconstruction-Veo (ASiR-V) on image quality and radiation dose in paranasal sinus CT, and to find the best combinations.@*Methods@#One head specimen was scanned with the routine spiral CT scanning parameters [noise index (NI)=8] and different levels of pre-ASiR-V (0—100%, with an interval of 10%). The images were reconstructed with different post-ASiR-V (0—100%, with an interval of 10%) for the bone algorithm and standard algorithm. All of 242 thin-layer images of paranasal sinuses were obtained. The region of interest (ROI) was selected to measure the CT value to calculate the contrast to noise ratio (CNR) and figure of merit (FOM). The volume CT dose index (CTDIvol) and Smart mA were recorded. The linear regression was conducted to analyze the relationship between CTDIvol, SmartmA, CNR and FOM. And with the same pose-ASiR-V level, the CNR of images which reconstructed by bone and soft algorithms were compared with pair-wise t test. The image quality was subjectively evaluated by three independent experienced radiologists using a 4-point scale (4 was the best).@*Results@#As the pre-ASiR-V levels (0—100%) increased, Smart mA and CTDIvol were reduced with a linear negative correlation (r=-0.981, -0.976, both P<0.001). The Smart mA decreased by 72.05% and CTDIvol by 71.22%. Keeping the same pre-ASiR-V level,the CNR increased with the increase of post-ASiR-V level (for the bone algorithm images:R2=0.976, 0.992, 0.982, 0.982, 0.975, 0.975, 0.979, 0.996, 0.952, 0.978, 0.965;for the standard algorithm images: R2=0.944, 0.990, 0.988, 0.993, 0.996, 0.987, 0.984, 0.996, 0.996, 0.990, 0.965).Under the same level of post-ASiR-V, the CNR and FOM fluctuated with the pre-ASiR-V level (for the bone algorithm images:R2=0.335, 0.341, 0.344, 0.364, 0.385, 0.405, 0.418, 0.429, 0.455, 0.474, 0.516; for the standard algorithm images: R2=0.223, 0.278, 0.327, 0.285, 0.309, 0.329, 0.325, 0.346, 0.360, 0.390, 0.380). All subjective image quality could meet the diagnostic requirements (the score≥3).@*Conclusion@#At NI=8, for the bone algorithm, the best combination is 80% pre-ASiR-V and 100% post-ASiR-V; for the standard algorithm, the best iteration combination is 100% and 100%. The appropriate choice of pre- and post-ASiR-V levels in paranasal sinus CT scan can effectively reduce the radiation dose under the premise of maintaining the image quality that meets the diagnostic needs.

ABSTRACT

Objective:To explore the effects of tube voltage on the image quality and radiation dose of paranasal sinus CT on a 16 cm wide-detector CT.Methods:The sinus area of head specimen was scanned with sequential scanning mode and tube voltages of 70, 80, 100, 120 and 140 kV, respectively. The parameters were set as follows: NI=9, automatic tube current modulation, Smart mA with the maximum range at the corresponding tube voltage, 120 mm detector width. The images were reconstructed with bone and soft tissue algorithms separately.The slice thickness was 0.625 mm for all reconstructed images. The axial, coronal and sagittal images were reformatted as 2 mm thickness with clinical baseline. The CT value and standard deviation (SD) were measured in the region of interest(ROI) of central axial image and the contrast-to-noise ratio (CNR) was calculated. The volume CT dose index(CTDI vol) and dose length product (DLP) were recorded, and the figure of merit (FOM) of images was computed. The images obtained in the study were subjectively evaluated by two experienced radiologists and one technician using a five-point scoring system. Results:The subjective evaluation of images obtained in the study met the diagnostic requirements. With the reconstruction of bone algorithm, the CNR at the tube voltages of 100 and 80 kV were 66.98 and 64.75, respectively, which was significantly higher than those at the tube voltages of 70, 120 and 140 kV (51.61, 61.56 and 57.76, respectively). The maximum and minimum CTDI vol were 34.11 mGy at 140 kV and 17.45 mGy at 70 kV. The FOM value was 152.26 at 100 kV. With the reconstruction of soft tissue algorithm, the maximum CNR was 195.62 at 80 kV, which was markedly greater than those at 70, 100, 120 kV and 140 kV (139.46, 154.49, 148.06 and 155.58 respectively). The maximum FOM value was 1273.56 at 80 kV, which was significantly higher than those at the tube voltages of 70, 100, 120 kV and 140 kV (1114.56, 809.98, 735.63 and 709.62, respectively). Conclusions:For the sinus CT of head specimen, the scan protocols with 100 kV combined with bone reconstruction algorithm and 80 kV combined with soft tissue reconstruction algorithm can provide relatively lower radiation dose and good image quality.

ABSTRACT

Objective:To investigate the effect of organ dose modulation (ODM) technique on reducing the breast radiation dose in chest CT scanning.Methods:In the phantom test, the PBU-2 adult chest module was used. The clinical chest scan protocol was used and three sets of scans performed on the chest module: (1) ODM off group, ODM was not used; (2) ODM part group, ODM was applied only in the breast region; (3) ODM all group, ODM was applied in the whole scan scope. Other scan parameters were same for the three groups, with smart mA applied. The volume CT dose index (CTDI vol) was recorded for all three groups. A long rod ionization chamber was placed in a fixed position in front of the right breast area to measure the breast skin dose (D). The contrast noise ratio (CNR) and the figure of merit (FOM) were measured respectively. In clinical research, 72 female patients who underwent chest CT scanning in Beijing Tongren Hospital Capital Medical University from August to November 2018 were retrospectively recruited. According to the application of ODM, the patients were divided into ODM off group (without ODM, 36 cases) and ODM part group (ODM applied in the breast region, 36 cases). The CTDI vol and the dose length product (DLP) were recorded. CNR, noise of images were measured and calculated, respectively. The image quality was evaluated by subjective evaluation scores. The one way ANOVA analysis was used in comparing the difference of CNR among the 3 groups in module test. As for clinical cases, the independent samples t test was used to compare the difference in CTDI vol, DLP, CNR and the noise between two groups; and the rank-sum test was used for comparison in image quality subjective evaluation. Results:In module test, the radiation dose was highest in ODM off group, and lowest in ODM all group. The CTDI vol were (6.90±0.02), (6.26±0.02) and (5.99±0.02) mGy, and the D values were (9.17±1.01), (8.01±0.92) and (7.58±0.87) mGy for ODM off group, ODM part group and ODM all group respectively. The CNR values of images with soft tissue algorithm reconstruction were highest in ODM off group and lowest in ODM all group, while no statistically significant difference was displayed ( P>0.05). The CNR values of the images with lung algorithm reconstruction showed the same trend, with statistically significant difference among the three groups ( F=154.732, P=0.006). The FOM of the lung and soft tissue algorithm images was maximized when the ODM was partially applied. As for clinical cases, compared with ODM off group, the dose of ODM part group showed significantly decreased, with CTDI vol decreased by 16.12% ( t=2.604, P=0.011), and the DLP decreased by 16.85% ( t=3.293, P=0.002). No significant difference was found in CNR, noise and subjective score by two doctors between two groups ( P>0.05). Conclusion:The application of ODM in chest CT imaging can reduce the radiation dose of breast with simultaneously maintaining the image quality.

Keeping the same pre-ASiR-V level,the CNR increased with the increase of post-ASiR-V level (for the bone algorithm images: R2=0.976, 0.992, 0.982, 0.982, 0.975, 0.975, 0.979, 0.996, 0.952, 0.978, 0.965;for the standard algorithm images: R2=0.944, 0.990, 0.988, 0.993, 0.996, 0.987, 0.984, 0.996, 0.996, 0.990, 0.965).Under the same level of post-ASiR-V, the CNR and FOM fluctuated with the pre-ASiR-V level (for the bone algorithm images: R2=0.335, 0.341, 0.344, 0.364, 0.385, 0.405, 0.418, 0.429, 0.455, 0.474, 0.516; for the standard algorithm images: R2=0.223, 0.278, 0.327, 0.285, 0.309, 0.329, 0.325, 0.346, 0.360, 0.390, 0.380). All subjective image quality could meet the diagnostic requirements (the score≥3). Conclusion:At NI=8, for the bone algorithm, the best combination is 80% pre-ASiR-V and 100% post-ASiR-V; for the standard algorithm, the best iteration combination is 100% and 100%. The appropriate choice of pre- and post-ASiR-V levels in paranasal sinus CT scan can effectively reduce the radiation dose under the premise of maintaining the image quality that meets the diagnostic needs.

The central venous access device (CVAD) is widely used in infusion of intravenous high nutrient solution, chemotherapy drugs, rapid infusion, blood transfusion, etc. CVAD is left in the patient′s blood vessels for a long time, although it has certain advantages in intravenous therapy, but also risks associated with it. In the past 20 years, many studies have focused on the mechanism of CVAD on vascular injury, trying to explore its mechanism from a different perspective at the microscopic level of pathophysiology. This article will review the three aspects of acute damage to the blood vessel caused by CVAD catheterization and chronic changes caused by indwelling catheters, analysis of the causes of vascular injury caused by CVAD catheterization, and research on preventing vascular injury caused by CVAD catheterization, aiming at providing reference for clinical intravenous therapy.

ABSTRACT

Objective:To evaluate the efficacy of self-made Wenyang-Huoxue Decoction combined with gestrinone capsule in the treatment of adenomyosis dysmenorrhea. Methods:A total of 88 patients with adenomyosis dysmenorrhea who met the inclusion criteria in Datong Traditional Chinese Medicine Hospital from January 2016 to December 2018 were enrolled and divided into two groups according to random number table method, 44 cases in each group. The control group was treated with conventional western medicine and the study group was treated with self-made Wenyang-Huoxue Decoction on the basis of the control group. Two groups were treated continuously for 6 months. The TCM syndrome score and dysmenorrhea score were performed before and after treatment. The vaginal ultrasound was used to detect and calculate uterine volume, Nottingham Health Poofite (NHP) was used to evaluate the quality of life. The ELISA was used to detect serum carbohydrate antigen 125 (CA125), TNF-α and IL-8 levels. The clinical efficacy was evaluated. Results:The total effective rate of the study group was 93.2% (41/44) and that of the control group was 72.7% (32/44), where the difference between the two groups was statistically significant ( χ2=6.510, P<0.01). After treatment, the TCM syndrome score, dysmenorrhea score and uterine volume of the study group were significantly lower than those in the control group ( t values were 7.956, 4.959 and 4.235, respectively, all Ps<0.01); the scores of pain, energy, emotion, sleep, social activity and physical activity were significantly lower than those in the control group ( t values were 8.592, 9.341, 6.652, 13.597, 4.085, 7.858, respectively, all Ps<0.01). After treatment, serum CA125 (33.02 ± 5.11 IU/ml vs. 46.22 ± 6.51 IU/ml, t=10.580), TNF-α (41.13 ± 5.03 pg/ml vs. 55.50 ± 6.15 pg/ml, t=11.997) and IL-8 (38.02 ± 5.50 pg/ml vs. 49.42 ± 6.51 pg/ml, t=8.873) in the study group were significantly lower than those in the control group ( P<0.01). Conclusions:Self-made Wenyang-Huoxue Decoction combined with gestrinone capsule can reduce the levels of serum CA125, TNF-α and IL-8 in patients with adenomyosis dysmenorrhea, and improve the clinical symptoms, and the quality of life.

Compared with surgery and radiofrequency ablation, stereotactic body radiation therapy (SBRT) has similar efficacy for local treatment of colorectal cancer with pulmonary oligometastasis, but it applies to a wide range of people. Oligometastasis of colorectal cancer may have radiation resistance, and good local control can be achieved by increasing the dose. The prescription dose and prognostic factors of SBRT in the treatment of lung oligometastasis of colorectal cancer have not been determined. There are no serious toxicities in most studies, but there are still radiation-related adverse events.

Under the premise of comprehensively implementing various emergency treatment tasks and ensuring the normal operation of other wards, the Nursing Department has put in place emergency plans and deployed due manpower for rapid response, timely personnel deployment, and reasonable reserve echelon structure. These measures have been taken as required by the patients′ numbers, critical conditions, disease diagnosis, and the guidelines of treatment and protection. While ensuring the completion of treatment work, we managed to leverage nursing human resources in a scientific, standardized and maximized efficiency manner, to ensure the quality of nursing, and the physical and mental health of nursing staff.
