ABSTRACT

Objective:To investigate the efficacy of robot-assisted femoral tunnel localization in reconstruction of the medial patellofemoral ligament (MPFL).Methods:A retrospective study was conducted to analyze the 36 patients who had been admitted to Department of Sports Medicine, The Fourth Hospital of Wuhan between January 2019 and January 2022 due to recurrent patellar dislocation. There were 15 males and 21 females; age: 23.5 (18.3, 29.0) years; number of dislocations: 2.5 (2.0, 3.0). They were stratified into 2 cohorts based on utilization of robot-assistance. In the observation group (17 cases), the femoral tunnel localization was robot-assisted in MPFL reconstruction; in the control group (19 cases), the femoral tunnel localization was guided by C-arm fluoroscopy in MPFL reconstruction. The 2 groups were compared in terms of operation time, frequency of guide wire placement, visual analogue scale (VAS) at postoperative 1 d, patellar tilt angle (PTA) and the disparity between actual femoral tunnel insertion and ideal tunnel insertion point (Sch?ttle point) at postoperative 1 to 3 d, and Lysholm knee score and International Knee Documentation Committee (IKDC) score at the last follow-up.Results:There was no significant difference in the preoperative general data between the 2 groups, showing comparability ( P>0.05). All patients were followed up for 12.0 (10.3, 13.0) months. In the observation group, the operation time [(64.1±16.7) min], frequency of guide wire placement [1.0 (1.0, 2.0) times], VAS [2.5 (2.0, 3.0) points], and disparity between actual femoral tunnel insertion and ideal tunnel insertion point [(4.7±1.2) mm] were significantly better than those in the control group [(84.2±19.7) min, 3.0 (2.0, 4.0) times, 3.5 (3.0, 4.0) points, and (6.1±1.2) mm] ( P<0.05). There was no statistical difference between the 2 groups in PTA, Lysholm knee score or IKDC score ( P>0.05). Conclusions:The short-term clinical efficacy of robot-assisted femoral tunnel localization is satisfactory in MPFL reconstruction. Compared with the intraoperative C-arm fluoroscopy, robot-assisted localization can decrease the frequency of guide wire placement, enhance femoral tunnel accuracy and efficiency, and alleviate more postoperative pain for the patients.

ABSTRACT

BACKGROUND@#Endocrine therapy (ET) and ET-based regimens are the preferred first-line treatment options for hormone receptor (HR)-positive and human epidermal growth factor receptor 2 (HER2)-negative metastatic breast cancer (HR+/HER2- MBC), while chemotherapy (CT) is commonly used in clinical practice. The aim of this study was to investigate the efficacy and clinical outcome of ET and CT as first-line treatment in Chinese patients with HR+/HER2- MBC.@*METHODS@#Patients diagnosed with HR+/HER2-MBC between January 1st, 1996 and September 30th, 2018 were screened from the Chinese Society of Clinical Oncology Breast Cancer database. The initial and maintenance first-line treatment, progression-free survival (PFS), and overall survival (OS) were analyzed.@*RESULTS@#Among the 1877 included patients, 1215 (64.7%) received CT and 662 (35.3%) received ET as initial first-line treatment. There were no statistically significant differences in PFS and OS between patients receiving ET and CT as initial first-line treatment in the total population (PFS: 12.0 vs. 11.0 months, P = 0.22; OS: 54.0 vs . 49.0 months, P =0.09) and propensity score matched population. For patients without disease progression after at least 3 months of initial therapy, maintenance ET following initial CT (CT-ET cohort, n = 449) and continuous schedule of ET (ET cohort, n = 527) had longer PFS than continuous schedule of CT (CT cohort, n = 406) in the total population (CT-ET cohort vs. CT cohort: 17.0 vs . 8.5 months; P <0.01; ET cohort vs . CT cohort: 14.0 vs . 8.5 months; P <0.01) and propensity score matched population. OS in the three cohorts yielded the same results as PFS.@*CONCLUSIONS@#ET was associated with similar clinical outcome to CT as initial first-line treatment. For patients without disease progression after initial CT, switching to maintenance ET showed superiority in clinical outcome over continuous schedule of CT.

ABSTRACT

Cognitive impairment is a common dysfunction after central nervous system disease or injury, which seriously affects the daily life of patients and brings heavy economic burdens to the family and society. Neurofeedback training (NFT) based on electroencephalography (EEG) is a non-invasive method of neuroregulation, which can improve cognitive function and behavior by autonomously adjusting brain function through feedback. This paper reviews the application of EEG signal-based neurofeedback training in cognitive rehabilitation, and discusses the current problems and future development trends in this field, so as to provide new ideas for clinical research and treatment of cognitive disorders.

ABSTRACT

Objective:To investigate the early clinical effect of fascia lata autograft bridging combined with the long head of biceps tendon transposition for treatment of irreparable massive rotator cuff tear.Methods:All of 31 cases of massive irreparable rotator cuff tear treated in our hospital from March 2016 to March 2020 were analyzed retrospectively. Among them, 17 cases (10 males, 7 females) were repaired with fascia lata autograft bridging under arthroscopy (patch group), the average age was 61.47±6.63 (ranging from 51 to 72) and 14 cases (4 males, 10 females) were repaired with fascia lata autograft bridging combined with the long head of biceps tendon transposition (combined group), the average age was 62.57±6.11 (ranging from 53 to 71). The operation time, intraoperative blood loss, postoperative complications, visual analogue scale (VAS) of pain before operation, at 1 week and 12 months after operation, Constant-Murley score of shoulder joint and American Association of shoulder and elbow Surgeons (ASES) score before operation, at 6 months and 12 months after operation were compared between the two groups. The outcome of rotator cuff healing was evaluated by MRI 1 year after operation.Results:All patients were followed up for 12-27 months (mean 18.33 ±6.8 months). There was no perioperative complication, and there was no significant difference in operation time between the two groups ( P>0.05) . The VAS score in the patch group was significantly higher than the combined group 1 week after operation ( t=2.09, P=0.048) , and there was no significant difference in VAS score 12 months after operation between the two groups. Constant-Murley score and ASES score in the combined group were significantly higher than the patch group at 6 months after operation ( t=5.23, P<0.001; t=4.45, P<0.001) , and there was no significant difference in Constant score and ASES score between the two groups at 12 months after operation. Constant score and ASES score in the two groups were significantly higher than those before operation. One year after operation, the MRI of the affected shoulder showed that the incidence of autograft patch thinning (Sugaya grade III) was 52.94%, the autograft patch structure failure rate (Sugaya grade IV and V) was 17.65% in the patch group, the autograft patch thinning rate (Sugaya grade III) was 35.71%, and the structural failure rate (Sugaya grade IV and V) was 7.14% in the combined group. The difference was statistically significant (χ 2=7.12, P=0.028) . Conclusion:Fascia lata autograft patch bridging combined with long head of biceps tendon transposition technique for treatment of irreparable massive rotator cuff tear has less pain 1 week after operation and better recovery of shoulder function half a year after operation. MRI showed better patch healing 1 year after operation.

OBJECTIVES@#Chlorogenic acid has various physiological activities such as antibacterial, anti-inflammatory, and antiviral activities. Studies have shown that chlorogenic acid can alleviate the inflammatory response of mice with acute lung injury (ALI), but the specific mechanism is still unclear. This study aims to investigate whether chlorogenic acid attenuates lipopolysaccharide (LPS)-induced ALI in mice by regulating the microRNA-223 (miR-223)/nucleotide-binding oligomerization domain-like receptor protein 3 (NLRP3) axis.@*METHODS@#SPF grade BALBc male mice were randomly divided into a control group, a model group, a chlorogenic acid group, a chlorogenic acid+miR-223 negative control (miR-223 NC) group, and a chlorogenic acid+miR-223 inhibitor (miR-223 antagomir) group, 10 mice in each group. Except the control group, the other groups were instilled with 4 mg/kg LPS through the airway to establish the ALI mouse model. After the modeling, the mice in the chlorogenic acid group were continuously given chlorogenic acid (100 mg/kg) by gavage for 7 d. The chlorogenic acid+miR-223 NC group and the chlorogenic acid+miR-223 antagomir group were given 100 mg/kg chlorogenic acid by gavage every day, and then were injected with 10 μL of miR-223 NC (0.5 nmol/μL) and miR-223 antagomir (0.5 nmol/μL) respectively for 7 consecutive days.The control group and the model group were replaced with normal saline. The lung tissues of mice were taken to measure the ratios of lung wet to dry weight (W/D). The bronchoalveolar lavage fluid of mice was collected to measure the levels of TNF-α, IL-6, and IL-1β by ELISA kit and to count the number of eosinophils (EOS), lymphocytes, neutrophils under light microscope. After HE staining, the pathological changes of lung tissues were observed and lung injury was scored. qRT-PCR method were used to determine the expression levels of miR-223 in lung tissues. Western blotting was used to determine the expression levels of NLRP3 protein in mouse lung tissues. Luciferase reporter assay was used to analyze the targeting relationship of miR-223 to NLRP3.@*RESULTS@#Compared with the control group, the lung W/D value, the lung injury score and the level of inflammatory factors in the bronchoalveolar lavage fluid were significantly increased in the model group (all P<0.05); the infiltration of inflammatory cells in the lung tissue was severe; the alveolar space was significantly increased; the alveolar wall was significantly thickened; the number of EOS, lymphocytes, and neutrophils in the bronchoalveolar lavage fluid was significantly increased (all P<0.05); the expression levels of miR-223 in lung tissue were significantly decreased (P<0.05); and the protein expression levels of NLRP3 were significantly increased (P<0.05). Compared with the model group, the W/D value of lungs, lung injury score, and levels of inflammatory factors in bronchoalveolar lavage fluid were significantly decreased in the chlorogenic acid group, the chlorogenic acid+miR-223 NC group, and the chlorogenic acid+miR-223 antagomir group (all P<0.05); lung tissues damage was alleviated; the numbers of EOS, lymphocytes, and neutrophils in bronchoalveolar lavage fluid were significantly decreased (all P<0.05); the expression levels of miR-223 in lung tissues were significantly increased (P<0.05); and the expression levels of NLRP3 protein were significantly decreased (P<0.05). Compared with the chlorogenic acid group, the lung W/D value, lung injury score, and inflammatory factor levels in the bronchoalveolar lavage fluid were significantly increased in the chlorogenic acid+miR-223 antagomir group (all P<0.05); lung tissue damage was aggravated; the number of EOS, lymphocytes and neutrophils in bronchoalveolar lavage fluid significantly increased (all P<0.05); the expression levels of miR-223 in lung tissues were significantly decreased (P<0.05); and the expression levels of NLRP3 protein were significantly increased (P<0.05). The results of luciferase reporter assay showed that miR-223 had a targeting relationship with NLRP3.@*CONCLUSIONS@#Chlorogenic acid may increase the level of miR-223, target the inhibition of NLRP3 expression, reduce LPS-induced inflammatory response in ALI mice, and alleviate pathological damage of lung tissues.

ABSTRACT

Objective:To determine the spiritual needs of shrinking man syndrome (SMS) in young and middle-aged patients, and to provide reference for determining targeted intervention plans.Methods:From May to July, 2021, 14 young and middle-aged patients with SMS who received hyperparathyroidism thermal ablation in the Interventional Medicine Department of China-Japan Friendship Hospital were selected by purpose sampling method. Qualitative research phenomenology was used to conduct semi-structured in-depth interviews, and the interview data were analyzed by Colaizzi 7-step analysis method.Results:There were 4 themes and 14 sub-themes: "understanding and self-acceptance of illness" (attribution to illness; disease uncertainty; acceptance and illness), "need for social support" (love and respect; connection and companionship; family support; medical support; patient support), "life value and sense of meaning" (life meaning and goal; self-esteem; religious belief), "self-actualization and transcendence" (hope for improvement; eager for miracles; realize your value).Conclusions:Medical staff should pay attention to the spiritual needs of young and middle-aged patients with SMS, and specifically meet their spiritual needs during diagnosis, treatment, nursing and follow-up, so as to improve the patients′ sense of life meaning and value, and help them to better survive with disease and return to society.

ABSTRACT

Objective:To investigate the mediating effect of hope on resilience and post traumatic growth (PTG) in patients with refractory secondary hyperparathyroidism (rSHPT).Methods:It was convenient to select rSHPT patients and non-refractory SHPT patients who received maintenance hemodialysis treatment in China-Japan Friendship Hospital from September 2018 to September 2021. Totally 495 patients with rSHPT and 1 295 patients with mild secondary hyperparathyroidism (SHPT) were surveyed by the Chinese Version of The Herth Hope Scale, the Chinese version of the Connor-Davidson Resilience Scale and the Post Traumatic Growth Rating Scale (PTGI). After matching according to the 1∶1 Propensity Score Matching (PSM), 436 cases were set in each of the two groups. The differences of hope, resilience and PTG scores between the two groups were compared. Pearson correlation analysis was used to analyze the correlation between hope, resilience and total score of PTG in rSHPT group. Regression analysis and SPSS Process mediation Model 4 were used to test the mediating effect of hope on resilience and PTG.Results:The hope score (32.16 ± 4.15), psychological resilience score (61.22 ± 14.38), and the PTG score (52.34 ± 18.92) of rSHPT patients was significantly lower than 33.41 ± 2.97 ( t=-5.72, P<0.05), 63.19 ± 7.25 ( t=-2.77, P<0.05), 57.95 ± 10.07 ( t=-6.34, P<0.05) of SHPT patients. There was a positive correlation between hope, resilience and PTG score ( r=0.671, 0.488, 0.523, all P<0.01). Regression analysis showed that resilience could positively predict PTG ( β=0.518, P<0.01). Psychological resilience positively predicted hope ( β=0.188, P<0.01). Resilience ( β=0.204, P =0.002) and hope ( β=1.442, P<0.01) could positively predict PTG. Hope played a partial mediating role in the relationship between resilience and PTG, and the mediating effect accounted for 60.23%. Conclusions:rSHPT patients generally had lower levels of hope, resilience and PTG. Resilience can affect PTG directly or indirectly through hope. Medical staff should improve the resilience and hope level of patients with rSHPT through positive psychological intervention measures, so as to enhance the positive promoting effect of psychological resilience on PTG.

ABSTRACT

Objective:To assess whether air-charged urodynamic(ACC)can replace water-filled urodynamic(WFC)and the value of ACC for the diagnosis of benign prostatic hyperplasia(BPH)in these patients.Methods:In this prospective study, 27 male patients with BPH(aged 60-90 years)were randomly selected for WFC and ACC examinations during the same period to compare the values of urodynamic pressure parameters recorded for the two groups.Pressure values were compared using Bland-Altman plots and the paired sample t-test, and differences in abdominal pressure changes in the two groups were compared using the chi-square test. Results:Changes in WFC and ACC pressure values showed the same trends, but at the three points where data were recorded and compared, the mean abdominal pressure(Pabd), end of bladder filling pressure(Pves.fill)and maximum detrusor pressure(Pdet.void)were(25.3±8.5)cmH 2O(1 cmH 2O=0.098 kPa), (26.0±8.08.4)cmH 2O and(98.8±32.8)cmH 2O for WFC and(32.5, 5.5±5.3)cmH 2O, (32.6±5.0)cmH 2O and(95.3±36.3)cmH 2O for ACC, respectively, The pressure values of the two methods were significantly different( P<0.05). WFC pressure values were unstable, with 74.07%(20/27)showing a marked decrease during bladder filling, significantly different from those measured by ACC( P<0.001). Band-Altman plots illustrated high consistency between pressure values measured via WFC and ACC. Conclusions:The pressure measured by ACC does not represent that measured by WFC, suggesting a clinical need to establish standard reference data for ACC.The abdominal pressure instability measured by WFC may be one of the factors leading to different detrusor pressures from the two methods.The high consistency between pressure values from ACC and WFC indicates that the former can also be used to assess changes in bladder function in elderly BPH patients.

ABSTRACT

Objective:To observe the effect of combining scalp acupuncture with feedback training on muscle tone, motor ability, plantar pressure distribution and joint range of motion of stroke survivors with lower limb spastic paralysis.Methods:A total of 212 stroke survivors were randomly divided into a combination group ( n=70), an exercise group ( n=70) and a scalp acupuncture group ( n=72) according to a random number table. All of the patients received routine medication and 120 minutes of rehabilitation training six days a week for 8 consecutive weeks. The rehabilitation training included guidance on good posture, posture change training, balance training and gait training. The scalp acupuncture and exercise groups were additionally provided with 40 minutes of scalp acupuncture and lower limb intelligent feedback training, while the combination group was given both. Muscle tension, balance and lower extremity movement were quantified before and after the training, and a gait evaluation system was used to detect the plantar pressure distribution on the affected side and the range of motion range of the lower limb joints during walking. Results:After the treatment, significant differences were observed in the average modified Ashworth scores, modified Berg scale scores, Fugl-Meyer lower extremity ratings and modified Barthel Index scores compared with before the treatment. The average scores of the combination group were then significantly better than those of the other two groups. The average load bearing ratios of the affected toes, metatarsal bones Ⅰ-V, medial arch, external arch, medial heel and external heel had improved significantly in all of the groups, with the improvement of the combination group again significantly greater than those of the other groups. The same pattern of improvement was observed in the groups′ average pelvic rotation angles, hip flexion angles, knee flexion angles and ankle dorsiflexion angles.Conclusion:Combining scalp acupuncture with intelligent feedback can significantly improve lower extremity functioning after a stroke. That should improve performance in the activities of daily living of stroke survivors.

ABSTRACT

Objective:To explore the effect of suspension exercise training (SET) on the lumbar function and balance ability of patients with thoracolumbar fracture and spinal cord injury.Methods:Totally 64 patients with thoracolumbar fracture and spinalcord injury were randomly divided into a control group and an observation group, each of 32. Both groups received pedicle screw reduction with internal fixation and spinal dome decompression, followed by routine post-operative rehabilitation. In addition, the observation group performed double bridge and single leg suspension training and pelvic lift training. Everyone′s walking ability, spinal nerve functionality and balance were evaluated 3 days after the operation and 2 months after the training. Stride frequency, stride length and walking speed were observed. American Spinal Injury Association (ASIA) nerve function classifications were assigned. The trajectory length, trajectory included area and movement deviation of each subject′s center of gravity (CG) was recorded.Results:The average stride frequency, step length and walking speed of the observation group had all improved significantly after the intervention, and were significantly better than the control group′s averages at that point. This was also true of the proportion in ASIA neurological function grade D (65.63%), CG trajectory length and area, and the Romberg rate of deviation along the X-axis and Y-axis of the CG.Conclusion:SET training after pedicle screw reduction and internal fixation and spinal dome decompression can significantly improve the spinal nerve functioning of persons after a thoracolumbar fracture with incomplete spinal cord injury. It enhances their balance and walking ability. Such therapy is worthy of clinical promotion and application.

Health management is the process of comprehensive management for health risk factors of individuals or groups. The general practice concept when used in health management of the medical examination centers, can optimize the medical examination process and facilitate health guidance. The Health Management Center of the Fourth Affiliated Hospital of Zhejiang University School of Medicine applied such concept in its management, and found increased number of persons taking medical examinations, decreased rate of complaints, and decreases in both waiting days and registration fees by outpatients with positive medical examination outcomes. In addition, general practitioners guided the majority of people undergoing medical examinations with health prescriptions, and improved the quality of health management.

ABSTRACT

Objective:To present our strategies and suggestions for management of acute hand injury during the epidemic of COVID-19.Methods:From January 1 to February 20, 2020, 78 patients with acute hand injury were admitted to the Department of Hand Surgery, Wuhan Fourth Hospital. They were 45 males and 33 females, aged from 21 to 69 years (mean, 43.2 years). Of them, 60 were treated from January 1 to 20, 2020, by medical staff under protection of class 1, and 18 from January 21 February 20, 2020, by medical staff under protection of class 2. The COVID-19 infection was recorded in the patients and medical staff as well. The measures taken and experience in control and prevention of 2019-nCoV infection during the epidemic were reviewed.Results:From January 1 to 20, 2020, 5 patients were diagnosed of COVID-19 infection, including 3 mild and 2 moderate cases, and 5 medical staff members were also diagnosed of COVID-19 infection, including 3 mild, one moderate and one severe cases. From January 21 to February 20, 2020, 2 patients were diagnosed of COVID-19 infection, including one mild and one moderate cases, but none of the medical staff was infected by COVID-19.Conclusions:During the epidemic of COVID-19, the front line medical staff may face a high risk of COVID-19 infection in the whole process of management of acute hand trauma. The risk of nosocomial COVID-19 infection can be reduced by complete pre-hospital examination, choice of proper anesthesia and surgical procedures, perioperative prevention measures for medical staff, improved postoperative ward management, and psychological counseling for the patients.

Objective To evaluate the efficacy of arthroscopic Brostr(o)m technique in the treatment of chronic ankle instability.Methods Seventeen patients with chronic ankle instability were treated at Department of Foot and Ankle Surgery,Wuhan Fourth Hospital from March to December 2016.They were 5 males and 12 females,aged from 18 to 52 years (mean,28 years).The ankle instability confirmed preoperatively involved the left side in 9 cases and the right side in 8 ones.Arthroscopic Brostr(o)m technique was used to repair the anterior talofibular ligament.All the patients were evaluated preoperatively and at the last follow-up using American Orthopaedic Foot and Ankle Society (AOFAS) score and visual analogue scale (VAS).The talar tilt angle and anterior translation were also assessed radiographically in pre-and postoperative ankle stress views.Results They obtained a mean follow-up of 12 months (range,from 10 to 18 months).Wound infection occurred in none of the patients;paresthesia appeared in the superficial fibular nerve area in one case which was recovered spontaneously.At the last follow-up,their AOFAS scores were improved from preoperative 47.5 ± 3.4 to 95.7 ± 2.1,VAS pain scores were decreased from preoperative 5.7 ± 1.8 to 1.6 ± 1.4,anterior talar translation was reduced from preoperative 10.12 ± 3.23 mm to 4.02 ± 1.68 mm,and talar tilt angle decreased from 15.20° ± 3.43° to 6.02° ± 2.64°.All the above differences were statistically significant (P < 0.05).Conclusion Arthroscopic Brostr(o)m technique may be considered as a valid option for treatment of chronic ankle instability,because it can well restore the stability of ankle joint and lead to satisfactory short-term results.

ABSTRACT

Abjective To observe the influence of moxibustion on the bone metabolism of persistent vegetative state patients. Methods A total of 60 patients with persistent vegetative state were divided randomly into the treatment group and control group, 30 in each. Both groups were treated with routine therapy, and the patients used moxibustion on the acupoints of zhongwan, xiawan , qihai and guanyuan. The treatment lasted 12 weeks. The changes of T-lymphoctyte subgroups, serum immunoglobulin were tested and compared before and 12th week after treatment. Result After 12th week treatment, the BMD of distal radius (0.563 ± 0.205 g/cm2 vs. 0.451 ± 0.114 g/cm2, t=2.615), the second lumbar vertebra (0.596 ± 0.108 g/cm2 vs. 0.533 ± 0.127 g/cm2, t=2.069) and the femoral neck (0.567 ± 0.214g/cm2 to 0.463 ± 0.163g/cm2, t=2.117) in the treatment group were significantly higher than those in the control group (Ps<0.05). The PTH (6.41 ± 0.56 pmol/L vs. 6.72 ± 0.42 pmol/L, t=0.018), CT (217.48 ± 57.35 ng/L vs. 228.46 ± 52.44ng/L, t=0.012) and BGP (9.16 ± 1.57 g/L vs. 10.37 ± 2.37 g/L, t=0.023) in the treatment group were significantly higher than those in the control group (Ps<0.05). Conclusions Moxibustion can help to treat the bone metabolism of persistent vegetative state patients and prevent the osteoporosis.

ABSTRACT

Objective To explore the effects of application of self-made assessment card for spinal nerve motor function in clinical nursing.Methods An assessment card for spinal nerve motor function was designed and produced according to "spinal cord injury grading standards" by American Spinal Injury Association(ASIA).A total of 40 nurses working in orthopedic ward of our hospital were selected as study subjects,and divided into the experimental group and the control group with 20 nurses in each group.Theoretical and practical training regarding spinal nerve motor function was provided to nurses in both groups.Then totally 398 patieuts with spine or spinal cord diseases hospitalized in our orthopedic ward were recruited from June to September 2016 as subjects for assessment.The experimental group used self-made assessment card for spinal nerve motor function as guidance;the control group performed routine assessment for spinal nerve motor function.Results There were no significant differences between the experimental group and the control group in scores of theoretical and practical examinations (P>0.05);compared with the control group,the error of assessment in the experimental group was reduced by 9.5%,missed assessment rate decreased by 16.2%;and the time for assessnent in the experimental group was significantly shorter than that in the control group(P<0.05).Conclusion The assessment card for spinal nerve motor function can reduce the error of assessment,save the time for assessment,enhance the capacity of specialist nurses and improve work efficiency.

ABSTRACT

Objective To observe the serum homocysteine (Hcy) levels in retinal branch vein occlusion (BRVO) patients with with hypertension or non-hypertension.Methods A total of 120 patients (120 eyes) with BRVO were divided into hypertension group [72 eyes,blood pressure 140-175/90-105 mmHg (1 mmHg=0.133 kPa)] and non-hypertension group (48 eyes,blood pressure 100-139/70-88 mmHg).According to the sex and age,78 patients with hypertensive non-retinal vascular diseases and 48 patients with non-hypertensive and non-retinal vascular diseases were collected by a way of same-size ratio as hypertension control group and non-hypertension control group,respectively.Fasting venous blood was collected from all patients in the moming and serum Hcy levels were measured by rate method.The total Hcy concentration over 15.0 μ mol/L was defined as high level Hcy.Fasting serum glucose and fasting serum lipid were also measured.Measurement data among groups were compared with t test.Results The serum Hcy levels were (26.82 ± 28.0),(8.39± 3.11),(21.37 ± 4.24),(9.25 ± 3.31) μmol/L in the hypertension group,hypertension control group,non-hypertension group and non-hypertension control group,respectively.The serum Hcy levels of patients in the hypertension group was significantly higher than that in the hypertension control group (t=3.324,P=0.004).The serum Hcy levels of patients in the non-hypertension group was significantly higher than that in the non-hypertension control group (t=2.216,P=0.049).The serum Hcy levels of patients in the hypertension group was significantly higher than that in the non-hypertension group,but the difference had not statistical significance (t=0.581,P=0.566).Among 120 patients,there were 68 patients (56.67%) with high level of Hcy (40 patients in the hypertension group and 28 patients in the non-hypertension group).Among the 40 patients with high levels of Hcy in the hypertension group,36 patients were older than 50 years old (90.00%) and 4 patients were less or equal than 50 years old (10.00%).Among the 28 patients with high levels of Hcy in the non-hypertension group,16 patients were older than 50 years old (57.14%);12 patients were less or equal than 50 years old (42.86%),whose indexes of serum glucose and serum lipid were not abnormal.There was significant difference in age distribution of patients with high level of Hcy between the hypertension group and the non-hypertension group (x2=9.882,P=0.002),but there was no significant difference in sex distribution (x2=2.052,P=0.216).Conclusions The level of serum Hcy increased both in BRVO patients with hypertension and non-hypertension.The indexes of serum glucose and serum lipid were not abnormal in BRVO patients aged less or equal than 50 years old with non-hypertensive except for the increase of serum Hcy level.

ABSTRACT

Objective To explore the effects of application of standard evaluation process of spinal cord motor function in patients with thoracic or lumbar diseases. Methods Selected 60 patients with thoracic and lumbar disease treated in the orthopedic ward of Henan Provincial People's Hospital from April 2016 to June 2016 as the experimental group. We retrospectively analyzed 60 cases of patients with thoracic or lumbar diseases treated with routine nursing evaluation process admitted between January to March 2016. We compared before and after the standardization evaluation process of spinal cord motor function, the theory and operation achievements of nurses, satisfaction of doctors with nurse specialist ability, mean hospitalization days and average cost of two groups. Results The development and implementation of the standardized evaluation process of spinal motor neurotically function, after the training, the theoretical assessment score of the nurse was (94.70±1.18) points, and the operation assessment score was (93.50±2.34) points; the average length of stay was (6.10±1.98) days. The average cost of hospitalization was (29 088±341) yuan; the overall satisfaction of doctors to nurses increased to 14/15; compared with 42.30±12.95, 46.80±15.31, 7.80±2.37, 37 566±592 of the control group, the difference was statistically significant (t=4.76-8.31, P=0.00). Conclusion Application of the standard evaluation process for Spinal cord motor function in patients with thoracic or lumbar diseases can standardize nurses' working procedure, making nursing work more comprehensive, timely and professional, and can better guide clinical practice.

ABSTRACT

Malignant tumor is the most heterogeneous human disease, and it has been gradually reached the consensus in clinical oncology that the personalized treatment of tumor patients should be directed by molecular diagnostics. However, the ever-changing characteristic of tumor in heterogeneity and the fact that tissue samples are usually unavailable highlight the function and significant of liquid biopsy. The material that could be used in liquid biopsy are mainly the circulating tumor DNA, circulating tumor cells and extracellular vesicles, which though have their own advantages and disadvantages, but could complement to each other, thus accurately and comprehensively reflecting the characteristic of the tumor and gave the best direction for patient′s individualized therapy. This article reviewed the recent advances of liquid biopsy and explained its importance in tumor precision medicine.

ABSTRACT

We report here a human case of Taenia asiatica infection which was confirmed by genetic analyses in Dali, China. A patient was found to have symptoms of taeniasis with discharge of tapeworm proglottids. By sequencing of the mitochondrial cytochrome c oxidase subunit 1 (cox1) gene, we observed nucleotide sequence identity of 99% with T. asiatica and 96% with T. saginata. Using the cytochrome b (cytb) gene, 99% identity with T. asiatica and 96% identity with T. saginata were found. Our findings suggest that taeniasis of people in Dali, China may be mainly caused by T. asiatica.

ABSTRACT

The phosphatidylinositide-3-kinase(PI3K)/ protein kinase B(Akt)/ mammalian target of rapamycin(mTOR)pathway is recognized to have an important role in the development and progression of colo-rectal cancer(CRC). The most extensively characterized downstream targets of mTORC1 are ribosomal protein S6 kinase 1(p70S6K1)and eukaryotic translation initiation factor(eIF)4E-binding protein 1(4EBP1),both of which are crucial to the regulation of protein synthesis. The abnormal expression of p70S6K1 and 4EBP1 in CRC has become the focus of attention.
