ABSTRACT

OBJECTIVE To establish a quality standard for rice-fired Glehnia littoralis . METHODS Appearance observation , powder microscopic identification and thin-layer chromatography (TLC)identification were performed for the samples of rice-fired G. littoralis decoction piece. According to the relevant methods stated in 2020 edition of Chinese Pharmacopoeia (part Ⅳ),the contents of moisture ,total ash ,acid-insoluble ash ,water-soluble extract and acid-soluble extract were determined. The contents of psoralen, zanthoxylin, bergapten, imperatorin and isoimperatorin were determined by high performance liquid chromatography (HPLC). RESULTS The rice-fired G. littoralis decoction pieces were round-like small segments ,slightly rough ,yellow(peeled) or dark yellowish brown (with peel ),special gas and slightly sweet taste. The powder was yellowish white. Under the microscope , secretions and secretory cells ,ducts, gelatinized starch granules ,ray cells ,parenchyma cells ,etc. could be seen. TLC showed the spots developed clearly. In the chromatogram of the test sample ,there was the same blue fluorescent spot at the corresponding position of the chromatogram of isoimperatorin control sample. The moisture ,total ash ,acid-insoluble ash ,water-soluble extract and ethanol-soluble extract from 9 batches of samples were 5.82%-6.27%,3.19%-3.59%,0.21%-0.27%,24.91%-30.30% and 20.66% -25.83% ,respectively. The linear range of psoralen ,zanthotoxin, bergapten, imperatorin and isoimperatorin were 0.240-2.400,0.320-3.200,0.224-2.240,0.292-2.920,0.208-2.080 µg/mL(all r>0.999 0). Limits of quantitation were 0.032 0, 0.030 0,0325 0,0.032 0,0.045 0 µg,respectively. Limits of detection were 0.100 8,0.089 6,0.071 5,0.090 0,0.132 0 µg, respectively. RSDs of prescision ,stability(24 h)and reprodu- cibility tests were less than 3%. Average recoveries were 100.56% (RSD=1.36% ,n=6),100.73%(RSD=2.25% ,n=6), 100.36%(RSD=0.98%,n=6),98.24%(RSD=0.40%,n=6) E-mail:853063968@qq.com and 99.40%(RSD=0.35%,n=6),respectively. The contents of above five components were 5.85-13.31,8.63-33.38,6.23- E-mail:shixiaofeng2005@sina.com 15.25,6.12-12.98,5.52-10.77 µg/g,respectively. The total contents were 34.20-83.47 µg/g. CONCLUSIONS It is preliminarily proposed that the moisture ,total ash and acid-insoluble ash should not exceed 7.30%,4.10%,0.30%. The water-soluble extract and ethanol-soluble extract are no less than 21.00% and 18.00%,respectively. The total content of coumarin should not be less than 52.03 µg/g(with peel )and 26.34 μg/g(peeled). Established quality standard can be used for the quality control of rice-fired G. littoralis .

ABSTRACT

Phospholysine phosphohistidine inorganic pyrophosphate phosphatase (LHPP) is a highly conserved histidine phosphatase from worms to humans in evolution. Recent studies have shown that LHPP, as a tumor suppressor, can inhibit the proliferation, growth and migration of the cells and promote apoptosis by participating in the regulation of various cell signaling pathways and protein phosphorylation. Therefore, the deeply study on the mechanism of LHPP is helpful to explore the best diagnostic markers and therapeutic targets for related tumors, which is essential for improving the prognosis of related tumors.

ABSTRACT

Objective:To explore the risk factors of paralytic ileus (PI) after simultaneous pancreas-kidney (SPK) transplantation. Methods:From January 2017 to December 2019, clinical data were reviewed retrospectively for 115 cases of SPK transplantation. The risk factors of PI after SPK were analyzed. According to the occurrence of PI, they were divided into two groups of occurrence and non-occurrence. One-way analysis of variance was utilized for analyzing such influencing factors as gender, age, body mass index (BMI), diabetic type, duration of diabetes, mode of dialysis, duration of dialysis, diabetic gastroenterology, history of open surgery, bowel preparation, operative duration, hemorrhagic volume, immunosuppressant and hypoproteinemia. Multivariate Logistic regression analysis was performed for screening the suspected risk factors. Results:Among them, 19 patients (16.5%) had PI. Univariate analysis showed that PI was associated with diabetic gastroenterology, operative duration, history of open surgery, no bowel preparation and hypoproteinemia ( P<0.05). Multivariate Logistic regression analysis revealed that the risk factors of PI after SPK included diabetic gastroenterology, operative duration time, history of open surgery and no bowel preparation ( P<0.05). Conclusions:Diabetic gastroenterology, operative duration, history of open surgery and no bowel preparation are risk factors for PI after SPK. Clinical interventions for the above factors are necessary.

ABSTRACT

Splenic lymphoma with villous lymphocytes (SLVL) or splenic marginal zone lymphoma with circulating villous lymphocytes is rare, and prolymphocytic transformation of SLVL is rarer. At present, only one case of SLVL with t(8;14)(q24;q32) translocation has been reported. In this study, we report a case of B-lymphoproliferative disorder with villous lymphocytes harboring t(8;14)(q24;q32) chromosome translocation that we inclined to SLVL with a prolymphocytic transformation. A 73-year-old female showed marked hepatosplenomegaly and high lymphocytosis (lymphocytes > 200 × 10/L). The abnormal lymphocytes had short coarse villi and round nuclei with prominent nucleoli. The immunophenotypes showed CD19, CD20, HLA-DR, CD22, CD5, Kappa, CD25, CD71, Lambda, CD7, CD10, CD23, CD34, CD33, CD13, CD14, CD117, CD64, CD103, and CD11c. The karyotype showed complex abnormality: 46XX,+ 3,-10, t(8;14)(q24; q32)[11]/46XX[9]. The cytoplasmic projection, immunological characteristics, and trisomy 3 chromosome abnormality supported the diagnosis of SLVL. However, the presence of prominent nucleoli and high lymphocytosis suggested prolymphocytic transformation, probably as a result of t(8,14) chromosome translocation. In this report, we described an unusual case of B-lymphoproliferative disorder with villous lymphocytes harboring t(8;14)(q24;q32) translocation, which could provide help in the diagnosis and differential diagnosis of B-lymphocytic proliferative diseases.

ABSTRACT

Objective@#To compare the efficacy and safety of plasma exchange (PE) combined with double plasma absorption and simple PE in the treatment of acute-on-chronic liver failure.@*Methods@#We retrospectively analyzed 251 cases of acute-on-chronic liver failure treated with artificial liver treatment since January 2015. Changes in clinical manifestations, laboratory tests, and complications of the patients before and after different modes of treatment were compared and short-term efficacy was tracked. In accordance with different data, t-test, Pearson's chi-squared test and Fisher's exact test were used for statistical analysis.@*Results@#The effectiveness of low-volume PE combined with double plasma molecular adsorption system (DPMAS) and equal amount of PE combined with DPMAS was significantly better than simple PE (83.7%, 84.05% and 82.15 vs 55.6%, P < 0.05) in early stage of liver failure. In late-stage of liver failure, there was no significant difference in the treatment efficiency of each group (P > 0.05). Bilirubin and bile acid levels were significantly decreased in combined treatment groups than that to simple PE group (P < 0.05). PTA and albumin improvement rate of DPMAS PE groups were significantly lower than that of simple PE group (P < 0.05). There was no statistical difference in adverse reactions between each group.@*Conclusion@#PE combined with DPMAS improves the treatment efficiency of early hepatic failure and decrease dosage of plasma when compared with simple PE. A beforehand DPMAS treatment after PE treatment can improve the adverse effects of DPMAS on blood coagulation function and albumin levels.

ABSTRACT

Hepatitis B remains a worldwide health problem with a high mortality rate associated with end-stage liver diseases. The Model for End-Stage Liver Disease and Child-Pugh score is still widely used as the prognostic model for liver cirrhosis and inflammatory reaction, B-type natriuretic peptide and portal vein thrombosis are independent risk factors. Many new models have been proposed for liver failure, including a new model HINAT ACLF. HINAT ACLF is better than its preceding models and increased neutrophil-to-lymphocyte ratio is a single factor to promote thyroid hormone, cholinesterase, and antiviral treatment response in this model. HBV DNA is frequently involved in the predictive model of liver cancer, but with widespread use of antiviral drugs, the value of its long-term prognostication has gradually reduced. Therefore, liver stiffness value instead of HBV DNA shows apparent advantages in mREACH-B. ALBI has good median survival prediction value for hepatocellular carcinoma patients. Alpha-fetoprotein and staging method, which is based on tumor number and size, are still independent risk factors for liver cancer.

ABSTRACT

Objective@#To investigate the value of neutrophil-lymphocyte ratio (NLR) in predicting hepatitis B-related liver failure.@*Methods@#A total of 349 subjects were enrolled, among whom 60 were healthy persons who underwent physical examination (group A), 111 had severe chronic hepatitis B (group B), 92 had decompensated hepatitis B cirrhosis (group C), and 86 had acute-on-chronic liver failure (ACLF) (group D). Routine blood test results, liver function parameters, and coagulation parameters were collected, and NLR was calculated. According to disease progression, group B was further divided into groups B1 (with progression to ACLF) and B2 (without progression to ACLF). NLR was compared between groups, and its prognostic value was evaluated.@*Results@#NLR was 2.22(1.76-3.05) in group A, 2.54(1.78-3.49) in group B, 3.07(1.95-5.04)in group C, and 3.41(2.01-5.15) in group D, and NLR gradually increased with the aggravation of disease condition. The univariate and multivariate regression analyses of groups B1 and B2 showed that NLR and prothrombin activity were prognostic factors for disease progression. There was a significant difference in baseline NLR between groups B1 and B2 (3.87 ± 1.54 vs 2.71 ± 1.54, P = 0.004). There was a significant increase in NLR when severe hepatitis B in 16 patients progressed to ACLF (P = 0.042). The receiver operating characteristic (ROC) curve analysis showed that the cut-off value of NLR for predicting the progression of severe hepatitis B to ACLF was 2.79, with an area under the ROC curve (AUC) of 0.739 (P = 0.002). NLR was also a reference index for judging end-stage liver disease with a cut-off value of 3.94 (AUC = 0.612, P = 0.001).@*Conclusion@#Peripheral NLR can reflect disease progression and predict the development of liver failure.

ABSTRACT

Objective@#To investigate the impact of transforming growth factor-β1 (TGF-β1) silencing by small interfering RNA (siRNA) on the expression of platelet-derived growth factor-β (PDGF-BB) and its receptor (PDGF-βR) in rats with hepatic fibrosis.@*Methods@#A total of 40 male Sprague-Dawley rats were randomly divided into normal control group, model group, TGF-β1 siRNA treatment group, and negative control group. All rats except those in the normal control group were given subcutaneous injection of 40% carbon tetrachloride to establish a rat model of hepatic fibrosis. The rats in the negative control group and the TGF-β1 siRNA treatment group were given tail vein injection of negative control plasmid or TGF-β1 siRNA plasmid twice a week at a dose of 0.25 mg/kg, and those in the normal control group and the model group were given the injection of sterile isotonic saline twice a week. The rats were sacrificed after 12 weeks and liver tissue samples were collected. Real-time PCR, immunohistochemistry, and Western blot were used to measure the expression of PDGF-BB, PDGF-βR, and phosphorylated extracellular signal-regulated kinase 1/2 (p-ERK1/2) in liver tissue. A one-way analysis of variance, the q test, and the Kruskal-Wallis test were used for statistical analysis based on data type.@*Results@#Compared with the model group and the negative control group, the TGF-β1 siRNA treatment group had significantly inhibited mRNA and protein expression of PDGF-BB and PDGF-βR (F = 24.785 and 22.92, P < 0.01), as well as significantly inhibited expression of p-ERK1/2 (P < 0.05).@*Conclusion@#Targeted TGF-β1 siRNA can effectively downregulate the expression of PDGF-BB, PDGF-βR, and p-ERK1/2 in liver tissue and thus help to improve hepatic fibrosis.

ABSTRACT

Objective To improve the quality standard ofZushima Plaster. Methods TLC was used to do qualitative identification of daphnetin and 7-hydroxycoumarin inZushima Plaster. The contents of daphnetin and 7-hydroxycoumarin inZushima Plaster were determined by HPLC. The chromatographic column was Agilent Eclipse Plus C18 (4.6 mm×250 mm, 5μm), and the mobile phase consisted of acetonitrile-0.1% phosphoric asid (15∶85, V/V) at a flow rate of 1.0 mL/min. The column temperature was maintained at 30℃, and the detection wavelength was set at 327 nm. Results Characteristic spots of daphnetin and 7-hydroxycoumarin could be detected by TLC from the samples, and the spots were clear and specific without interference from negative samples. The calibration curve of daphnetin and 7-hydroxycoumarin were linear in the ranges of 0.180-1.800μg and 0.100-1.00μg (r=0.999 1,r=0.999 2), and the average recovery rates were 98.99% and 101.48%, RSDwere1.43% and 1.32%, respectively (n=9).Conclusion The established method appears easy to use, accurate and specific, and therefore can be used for the quality control ofZushima Plaster.
