ABSTRACT

BACKGROUND:Synovitis plays a leading role in the early progression of knee osteoarthritis and is a potential early therapeutic target. However, the mechanism of synovitis remains unclear. In animal models, increased systemic iron and intracellular iron uptake can induce and exacerbate osteoarthritis, but the association with ferroptosis in synovitis remains unclear.Further studies are needed to investigate the role of ferroptosis in the development and progression of synovitis in osteoarthritis. OBJECTIVE:To investigate the role of ferroptosis mediated by glutathione peroxidase 4(GPX4)in the development of synovitis in knee osteoarthritis. METHODS:The synovial tissues of 43 patients with osteoarthritis who underwent arthroscopic or joint replacement surgery and 10 patients undergoing arthroscopic treatment of meniscal injury or ligament tear were collected and divided into three groups according to the Kellgren-Lawrence grading of X-ray images:normal control group(KLG 0,n=10),early knee osteoarthritis group(KLG 1,2,n=20)and late knee osteoarthritis group(KLG 3,4,n=23).Hematoxylin-eosin staining was used to observe the severity of synovitis in each group, and iron deposition in the synovium in each group was evaluated by Prussian blue staining. The expressions of Acyl-CoA Synthetase Long Chain Family Member 4(ACSL4),GPX4,cyclooxygenase 2 and tumor necrosis factor α in the synovium were detected by immunohistochemical staining.Western blot and immunofluorescence were used to detect the expression of ferroptosis-related proteins ACSL4 and GPX4. RESULTS AND CONCLUSION:Compared with the normal control group, iron content in synovial tissue was increased in the knee osteoarthritis groups, and iron deposition in the late knee osteoarthritis group was higher than that in the early osteoarthritis group.ACSL4 was highly expressed in the synovial tissue of knee osteoarthritis compared with the normal control group(P<0.01),and GPX4 was lowly expressed in the synovial tissue of knee osteoarthritis(P<0.01).The expression level of ACSL4 increased with the progression of osteoarthritis, while the expression level of GPX4 decreased with the progression of osteoarthritis. The expression of cyclooxygenase 2 in the synovium of osteoarthritis was significantly higher than that in the normal synovium, and the expression was the highest in the early stage of osteoarthritis, which was significantly different from that in the advanced stage of osteoarthritis(P<0.01).The expression of tumor necrosis factor α in the synovium of osteoarthritis was significantly higher than that in the normal synovium, but there was no significant difference between early and late osteoarthritis groups(P>0.05).To conclude, the deposition of iron exists in the synovial tissue of osteoarthritis and ferroptosis is involved in the occurrence and progression of synovitis in knee osteoarthritis.

ABSTRACT

Purpose@#Radiation-induced dermatitis (RD) is a common side-effect of therapeutic ionizing radiation that can severely affect patient quality of life. This study aimed to develop a risk prediction model for the occurrence of RD in patients with cervical carcinoma undergoing chemoradiotherapy using electronic medical records (EMRs). @*Methods@#Using EMRs, the clinical data of patients who underwent simultaneous radiotherapy and chemotherapy at a tertiary cancer hospital between 2017 and 2022 were retrospectively collected, and the patients were divided into two groups: a training group and a validation group. A predictive model was constructed to predict the development of RD in patients who underwent concurrent radiotherapy and chemotherapy for cervical cancer. Finally, the model's efficacy was validated using a receiver operating characteristic curve. @*Results@#The incidence of radiation dermatitis was 89.5% (560/626) in the entire cohort, 88.6% (388/438) in the training group, and 91.5% (172/188) in the experimental group. The nomogram was established based on the following factors: age, the days between the beginning and conclusion of radiotherapy, the serum albumin after chemoradiotherapy, the use of single or multiple drugs for concurrent chemotherapy, and the total dose of afterloading radiotherapy. Internal and external verification indicated that the model had good discriminatory ability. Overall, the model achieved an area under the receiver operating characteristic curve of .66. @*Conclusions@#The risk of RD in patients with cervical carcinoma undergoing chemoradiotherapy is high. A risk prediction model can be developed for RD in cervical carcinoma patients undergoing chemoradiotherapy, based on over 5 years of EMR data from a tertiary cancer hospital.

ABSTRACT

Bronchopulmonary dysplasia(BPD)is a chronic respiratory system disease that causes respiratory failure and death in premature infants, and hyperoxic exposure is the main risk factor for its occurrence.Cellular senescence describes a state of cell cycle blockade, and in recent years studies have confirmed that exposure to hyperoxia can cause cellular senescence.Cellular senescence plays a crucial role in the development of the lung epithelium, lung interstitium, pulmonary vasculature, and airways, and abnormal development of these tissues is associated with the development of BPD.Therefore, this paper takes cellular senescence and BPD as the starting point to review the mechanism of hyperoxia-induced cellular senescence in the occurrence and development of BPD and the anti-aging drugs currently applied in clinical practice, in order to provide a new direction for the prevention and treatment of BPD.

ABSTRACT

Oxygen therapy is a common therapeutic method to improve oxygenation of premature infants, but long-term exposure to high oxygen can cause damage to immature organs and abnormal development. In addition to bronchopulmonary dysplasia and retinopathy, high oxygen levels will increase the risk of chronic kidney disease and hypertension in adulthood.High oxygen exposure can lead to kidney damage and developmental abnormalities in premature infants, including reduced number and increased volume of glomeruli, renal cell apoptosis, and abnormal development of renal tubules. The mechanism may be related to abnormal signaling pathways related to renal development. This article reviews the relationship between hyperoxia and kidney development and the possible mechanism of kidney disease, in an attempt to provide theoretical reference for early clinical intervention.

ABSTRACT

Objective:To explore the eye movement characteristics of gaze following in children with autism spectrum disorder (ASD) guided by three cues: non-sociality (arrow), high level sociality (eye) and low level sociality (head turning). Methods:Total of 23 ASD children with a developmental age of 2-3 years old and 23 typical development (TD) children with a physiological age of 2-3 years old were included from April to October 2022. The children were guided to follow gaze with three types of cues, namely arrow, eye and head turning, and the eye tracking features were collected with the Tobbi Pro X3-120 eye tracker. SPSS 17.0 was used to analyze the data. A generalized linear model was used to analyze the differences of different score (DS), time to first fixation( TFF), first fixation duration(FFD) and total fixation duration (TFD) between the two groups.Results:(1) The interaction of DS between the groups and cue types was significant (χ 2=11.22, P<0.01), the main effect of groups was significant (χ 2=3.95, P<0.05), and the main effect of cue types was significant (χ 2=31.43, P<0.001). Further simple effect analysis showed that under eye contact conditions, the DS of the ASD group was lower than that of the TD group ( P<0.001). There was no statistically significant difference between the two groups under arrow conditions and head turning conditions (both P>0.05). The DS of the ASD group under eye contact conditions was lower than those under arrow and head turning conditions (both P<0.05), and the DS under arrow conditions was lower than that under head turning conditions ( P<0.001). (2)The interaction of TFF between the groups and the cue type was not significant (χ 2=5.53, P>0.05), and the main effect of groups was not significant (χ 2=3.04, P>0.05). The main effect of cue type was not significant (χ 2=3.19, P>0.05). (3)The interaction of FFD between the groups and cue types was not significant (χ 2=0.29, P>0.05), and the main effect of groups was not significant (χ 2=0.66, P>0.05). The main effect of cue types was not significant (χ 2=3.93, P>0.05). (4)The interaction of TFD between the groups and cue types was not significant (χ 2=2.33, P>0.05). The main effect of groups was significant (χ 2=18.35, P<0.001), and the TFD of the ASD group (1.30 (0.91, 1.58)s) was lower than that of the TD group (1.59(1.31, 1.72)s). The main effect of cue types was significant (χ 2=32.22, P<0.001), and the TFD under arrow condition (1.18 (0.85, 1.40)s) was lower than that under eye contact condition (1.61 (1.29, 1.73)s) and head turning condition (1.62 (1.18, 1.74)s) ( P<0.05). There was no statistically significant difference in TFD under eye contact and head turning conditions (both P>0.05). Conclusion:Under the guidance of non-social and low-level social stimuli, children with ASD exhibit gaze following abilities comparable to TD children when communication development is age matched. Under the guidance of high-level social stimuli, children with ASD have poorer gaze following ability, manifested as lower accuracy in responding to cues. The level of social information in cues is an important factor affecting the gaze following ability of children with ASD.

ABSTRACT

Spinocerebellar ataxia (SCA) is a group of highly heterogeneous autosomal dominant genetic disease, including many subtypes. SCA11 is a rare subtype of SCA, and is caused by mutant TTBK2 gene. A case of SCA11 was reported in this article. Whole exome sequencing showed that there was a c.1284dupA frameshift mutation in TTBK2 gene. Literature review found that only 6 pedigrees of SCA11 have been reported, but the mutation site of this case is a novel identified mutation that has not been reported in the Human Gene Mutation Database.

ABSTRACT

Objective To investigate the regulatory effect of Jiedu Huayu Tongfu prescription on intestinal homeostasis in patients with hepatitis B cirrhosis with liver-gallbladder damp-heat syndrome, as well as its effect on endotoxin, inflammatory factors, and cellular immune function. Methods A total of 72 patients who attended The First Affiliated Hospital of Henan University of Chinese Medicine from June 2019 to January 2021 and met the diagnostic and inclusion criteria were enrolled as subjects and then randomly divided into observation group and control group, with 36 patients in each group. In the treatment group, 2 patients were lost to follow-up, 2 patients were excluded, and 32 patients completed the study; in the control group, 2 patients were lost to follow-up, 1 patient was excluded, and 33 patients completed the study. In addition to the basic treatment including antiviral therapy and liver-protecting treatment, the patients in the observation group were given Jiedu Huayu Tongfu granules, and those in the control group were given oral administration of Bifidobacterium tetravaccine tablets; the course of treatment was 4 weeks for both groups. The 16S rDNA sequencing technique was used for sequencing of fecal flora, and the two groups were measured in terms of the changes in liver function [alanine aminotransferase (ALT), aspartate aminotransferase (AST), total bilirubin (TBil), and albumin (Alb)], endotoxin (ET), levels of tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6), and T lymphocyte subsets (CD3 + T, CD4 + T, CD8 + T, and CD4 + /CD8 + ) after treatment. For normally distributed continuous data with homogeneity of variance, the paired t -test was used for comparison within each group, and the independent samples t -test was used for comparison between two groups; the Wilcoxon rank-sum test was used for comparison of non-normally distributed continuous data. The chi-square test was used for comparison of categorical data. Results The observation group had a significantly higher overall response rate than the control group (87.5% vs 60.6%, χ 2 =-2.299, P =0.022). After treatment, both groups had significant reductions in ALT, AST, and TBil and a significant increase in Alb (all P 0.05). Conclusion Jiedu Huayu Tongfu prescription, in combination with etiological and basic treatments, can alleviate clinical symptoms, reduce liver injury, and improve cellular immune function in patients with hepatitis B cirrhosis with liver-gallbladder damp-heat syndrome. Jiedu Huayu Tongfu prescription can improve the imbalance of intestinal flora by increasing the abundance of the probiotic bacteria such as Firmicutes, Lactobacillus, and Bifidobacterium and the pathogenic bacteria such as Bacteroidetes and Cercozoa, and its effect in further improving liver and immune function may be associated with the regulation of intestinal microecology.

ABSTRACT

At present, the implementation of Internet hospitals is still hampered by the double digital divide between doctors and patients. In order to develop Internet healthcare better, a Grade-A tertiary hospital has implemented digital governance for traditional healthcare system. The healthcare service chain has been reshaped by integrating online and offline health services and using multi-subject synergy and government-driven logic. Information barriers were broken down by strengthening hospital′s top-level design and implementing personalized follow-up visits for specialized diseases and live broadcast of health education. By establishing cross-regional specialist alliances and medical consortia oriented to patient needs, the time and space limitations of medical resources have been broken through. Digital governance practices have realized the two-way bridging of digital divide between doctors and patients, expanded the new mode of digital healthcare, and provided reference for the sustainable development of Internet hospitals.

ABSTRACT

Objective:To observe the changes of nutrition impact symptoms (NIS) and fat-free mass and analyze the relationship between them in patients with head and neck cancer (HNC) during radiotherapy. Methods:A convenient sampling method was adopted to select HNC patients who received radiotherapy in the outpatient clinic of department of radiotherapy for head and neck cancer in a cancer hospital in Beijing from March 2017 to January 2020. The nutrition impact symptoms (NIS) were assessed by a nutrition impact symptoms checklist and the fat free mass was assessed by bioelectrical impedance analysis at three time points: before radiotherapy (T1), during radiotherapy (T2) and at the end of radiotherapy (T3). The Generalized Estimating Equations (GEE) was used to analyze the relationship between them. Results:A total of 542 HNC patients were included in the analysis. During radiotherapy, the patients' NIS number and score presented an increasing trend, reaching the highest level and peaked at the end of radiotherapy. The change of fat-free mass showed a decreasing trend and reached the minimum at the end of radiotherapy. Patients with higher NIS scores had more percentage loss of fat-free mass. Conclusions:During radiotherapy, patients with HNC showed an increase in NIS score and lost fat-free mass. Patients with higher NIS score lost more percentage of fat-free mass, which suggested that clinical medical staff should pay attention to NIS management and take comprehensive intervention measures in time to reduce the loss of fat-free mass.

ABSTRACT

Objective:To discuss the executive function (EF) characteristics of preschool aged male children diagnosed as high-functioning autism (HFA), and to compare the EF differences between HFA and typically developing (TD) children.Methods:Twenty-six preschool male HFA children aged 4-5 years old and chronological age and gender matched TD children were recruited respectively. Parent questionnaire behavior rating scale of executive function-preschool version (BRIEF-P) and executive function battery EF-TOUCH were adopted to explore the EF characteristics and the differences between the two groups. SPSS 22.0 software was used for statistical analysis, including descriptive statistics, χ2 test, t-test and Mann-Whitney U test. Results:EF-TOUCH results demonstrated significant differences with correct proportions of inhibition tasks (Arrow, Pig, silly sounds game (SSG)), working memory task (pick the picture (PTP)) and cognitive flexibility task (something′s the same (STS)) between groups (Arrow: Z=-2.278, P=0.023; Pig: Z=-2.599, P=0.009; SSG: Z=-1.985, P=0.047; PTP: t=2.635, P=0.011; STS: Z=-3.556, P=0.000). The accuracy rate in HFA group were significantly lower than TD group (HFA group: Arrow: 0.76 (0.57, 0.92), Pig: 0.95 (0.90, 1.00), SSG: 0.85 (0.57, 0.94), PTP: 0.74±0.11, STS: 0.77 (0.62, 0.90); TD group: Arrow: 0.92 (0.78, 0.95), Pig: 1.00 (0.98, 1.00), SSG: 0.94 (0.82, 1.00), PTP: 0.81±0.09, STS: 0.93 (0.82, 0.97)). Reaction time(RT) of Pig task was longer in HFA group than TD group (HFA: (1 624.29±234.33)ms; TD: (1 481.39±220.78)ms, t=-2.263, P=0.028). RTs of Arrow and SSG for both groups were not significantly different ( P>0.05). Children of HFA group exhibited worse inhibition and working memory(WM) than TD children in BRIEF-P(inhibition: HFA group (60.73±10.47), TD group (54.73±6.87); WM: HFA group (68.04±11.51), TD group (61.69±8.44))( t=-2.443, P=0.018; t=-2.267, P=0.028 respectively). No significant differences were found in the rest factors and domains for both groups (all P>0.05). Conclusions:Apparent inhibition control, working memory and cognitive flexibility deficiencies were found in preschool aged male HFA children. There are significant EF differences between preschool aged male HFA and typically developing children.
