ABSTRACT

Objective To analyze the reasons of misdiagnosis of primary syphilitic chancre and strengthen the understanding of atypical features of this disease. Methods A case series of twenty-seven challenging primary syphilis patients who were not immediately recognized as chancre was included in our study. The clinical data including the patients' age, sex, skin lesions, HIV status, syphilis serologic test results, treatment, and follow-up results were collected. Hematoxylin-eosin and immunohistochemistry staining of skin biopsy sections were reviewed. Results Four female cases with extragenital chancres presenting as erythema or erosive skin lesions on the nipple were misdiagnosed as Paget's disease or eczema. The disorder of missed or misdiagnosed male cases manifested as syphilitic balanitis or multiple chancres on the penis root and adjacent pubis rather than coronal sulcus or frenum. Patients with nonreactive nontreponemal tests at initial presentation were also easily missed or misdiagnosed. Conclusion Primary syphilis presenting as multiple lesions rather than a single chancre, at atypical locations, or with a nonreactive nontreponemal test result, tends to be missed or misdiagnosed.

ABSTRACT

Objective: To evaluate the prevention and treatment effects of acupuncture-moxibustion for Alzheimer disease (AD) based on various AD mouse models. Methods: Several representative types of mouse models were selected according to the pathophysiological causes of AD, including senescence accelerated mouse/prone (SAMP) mice, soluble amyloid-β protein (Aβ) injection mice/rats, amyloid precursor protein (APP) transgenic mice, and APP/PS1 double transgenic mice. Through the observation of behavioral changes and analysis of core items, the possible mechanisms of acupuncture-moxibustion in preventing and treating AD were explored. Results: Acupuncture-moxibusiton therapy can improve AD mice's cognitive dysfunction; the major action mechanisms including increasing cerebral blood flow, improving the expressions of vital proteins in the hippocampus, preventing neuron cell apoptosis, promoting the clearance of Aβ deposition, activating autophagy pathway to reduce memory deficits and regulating the metabolisms of brain-derived neurotrophic factor, tyrosine kinase receptor B, N- acetylaspartate and glutamic acid. Conclusion: Although the optimal mouse model is not determined, it is sure that acupuncture-moxibustion therapy can improve cognitive function. A more suitable AD animal model should be duplicated in order to better explore the inherent action mechanism of acupuncture-moxibustion in preventing and treating AD.

ABSTRACT

Objective:Nivolumab is one of the most common programmed death 1 (PD-1) inhibitors used as an immune checkpoint inhibitor (ICI). It brings significant therapeutic effects but often accompanied by serious drug toxicity. The pulmonary toxicities of nivolumab are not clear. This study aims to systematically explore the nivolumab-associated pulmonary toxicities and provide reference for clinical treatment. Methods:Data were extracted from US Food and Drug Administration (FDA) Adverse Event Reporting System (FAERS) database from January 1, 2016 to September 30, 2019. Two types of disproportionality analysis, information component (IC) and reporting odds ratio (ROR), were applied in nivolumab-associated pulmonary adverse events (AEs) signal detection. Results:A total of 28 489 309 records were extracted from FAERS database and 8 181 records were associated with nivolumab. Analysis was conducted in 179 AEs and 86 signals were detected. Notably, potent signals were detected in radiation pneumonitis (IC025: 3.99, ROR025: 17.25), pneumonitis (IC025: 3.34, ROR025: 10.64) and bronchial fistula (IC025: 2.94, ROR025: 8.78). Nivolumab-associated pulmonary toxicities were more frequently reported in dyspnoea (IC025: 0.50, ROR025: 1.44), pneumonia (IC025: 0.08, ROR025: 1.07) and pneumonitis (IC025: 3.34, ROR025: 10.64). Results of IC and ROR methods were similar to each other. Most pulmonary toxicities were observed in patients with non-small cell lung cancer (N=3 711, 32.13%), malignant melanoma (N=1 658, 14.36%) and renal cell carcinoma (N=731, 6.33%). Conclusion:Significant pulmonary toxicities were detected in patients treated with nivolumab. Thus, it is highly important for clinicians to be vigilant about nivolumab-associated pulmonary AEs and be prepared to take immediate action for patient safety.

ABSTRACT

Objective:To apply the ultrasound elastography in quantitative evaluation for spasticity of forearm flexor muscles after stroke, and observe its characteristics. Methods:From January to October, 2019, 30 inpatients with stroke were recruited, followning spasticity of wrist and finger flexor muscles. They were measured the shear wave velocity (SWV) of flexor carpi ulnaris (FCU), flexor digitorum superficialis (FDS), flexor digitorum profundus (FDP) and flexor carpi radialis (FCR) with ultrasound elastography, and assessed the tension of wrist and finger flexors with modified Ashworth Scale (MAS). Results:The SWV of all the muscles were more in stretching position than in relaxation position in both sides (|Z| > 3.844, P < 0.001). The SWV of all the muscles were more in the affected side than in the unaffected side in stretching position (|Z| > 3.593, P < 0.001). The differences of SWV between stretching and relaxation were more in all the muscles in the affected side than in the unaffected side (t > 3.199, P < 0.01). The mean SWV of all the muscles significantly correlated with the MAS score of wrist (r = 0.605, P < 0.001), while the mean SWV of FDS and FDP correlated with the MAS score of finger (r = 0.540, P < 0.01). Conclusion:Ultrasound elastography is useful to quantitatively evaluate the spasticity of each muscle of forearm flexors after stroke.

ABSTRACT

Objective:To apply the ultrasonic shear wave elastography to evaluate triceps surae and Achilles tendon for patients with lower extremity dyskinesia after stroke. Methods:Thirty-two inpatients during 2018 and 2019 with unilateral lower extremity dyskinesia after stroke were studied with 2-D ultrasound and shear wave elastography on the bilateral triceps surae and Achilles tendons before and after rehabilitation. Shear wave velocity (SWV) of the triceps surae and the Achilles tendons, the length and thickness of Achilles tendon (soleus tendon) were measured. Results:Before rehabilitation, the SWV of the Achilles tendons and the triceps surae increased more in the affected side than in the unaffected side (t > 2.426, P < 0.05), as well as the length of the Achilles tendons (t = 11.801, P < 0.001). After rehabilitation, the SWV of the triceps surae decreased (t > 2.447, P < 0.05), as well as the length of the Achilles tendons (t = 8.577, P < 0.001). Conclusion:Ultrasound shear wave elastography can be used to evaluate the elastic characteristics of the Achilles tendon and the triceps surae, to guide the rehabilitation for stroke patients.

ABSTRACT

Objective:To investigate the cytotoxicity of a recombinant Vibrio vulnificus cytolysin membrane pore-forming peptide (rMpf) to J774A.1 cells as well as its influences on reactive oxygen species (ROS) and lysosomes, and to analyze the cellular localization of the rMpf. Methods:The rMpf was expressed using an Escherichia coli expression system and then used to treat murine J774A.1 cells in vitro. The viability of rMpf- and PBS-treated J774A.1 cells and the levels of ROS and lysosomes in these cells were analyzed by flow cytometry. Confocal microscopy was used to observe the cellular localization of rMpf in the J774A.1 cells. Results:No significant differences in cell viability, ROS production or lysosome formation in J774A.1 cells were found between low-dose (4 μg/ml) rMpf-treated and untreated groups. However, the cell viability, ROS production and lysosome formation were significantly decreased after treating J774A.1 cells with higher doses of rMpf (20 and 60 μg/ml). Moreover, the rMpf was found to localize in both the cytoplasm and nuclei of J774A.1 cells in a dose-dependent manner. Conclusions:The rMpf could localize in both the cytoplasm and nuclei of J774A.1 cells. It would inhibit the cellular ROS production and lysosome formation to damage macrophage function. This study provided a novel sight for understanding the cytotoxic domain and pathogenesis of Vibrio vulnificus cytolysin and other membrane pore-forming cytolysins.

ABSTRACT

In recent years, the clinical experts consensuses or guidelines of ankylosing spondylitis (AS)/spondyloarthritis (SpA) have been constantly updated, but to better understand and practice, patient self-participation management is one of the key points to improve the level of diagnosis and treatment. Through questionnaire survey of these patients, we screened out the most concerned issues, and established the AS/SpA patient practice guideline working group with multidisciplinary physicians and patients. Fifteen opinions, as the AS/SpA patient practice guidelines, are proposed in accordance with the relevant principles of the "WHO guidelines development manual" , and with the international normative process.

ABSTRACT

Objective@#To evaluate the long-term healing rate of transsphincteric anal fistula treated with anal fistula plug procedure and the risk factors affecting the healing of anal fistula.@*Methods@#A retrospective case-control study was conducted to analyze the clinical data of 207 patients with transsphincteric anal fistulas who received anal fistula plug procedure at the Department of General Surgery, Beijing Chaoyang Hospital of Capital Medical University from August 2008 to September 2012. Inclusion criteria: (1) consistent with the diagnosis of transsphincteric anal fistula: the anal fistula passed through the internal and external sphincter; (2) complete data; (3) initial treatment with anal fistula plug procedure. Exclusion criteria: (1) acute rectal or perianal infection or poorly controlled focal infection; (2) recent incision and drainage of perianal abscess or spontaneous rupture of abscess; (3) patients with malignant tumor; (4) patients with Crohn′s disease or ulcerative colitis; (5) patients with heart, liver, brain, lung or renal insufficiency; (6) cachexia due to various chronic wasting diseases; (7) patients could not tolerate surgery. Patients were followed up for anal fistula healing. The cumulative healing rate of patients with transsphincteric anal fistula was plotted using the Kaplan-Meier method, and the factors affecting anal fistula healing were explored by univariate and multivariate logistic regression analysis.@*Results@#There were 186 males and 21 females with age of 15 to 69 (mean 38) years. The duration of anal fistula was 3-60 (mean 15) months. Three patients had a history of previous episodes of perianal abscess and underwent incision and drainage of perianal abscess (all more than 3 months). During follow-up ending on October 31, 2018, 72 patients (34.8%) were lost to follow-up. Among 135 patients who were successfully followed up, the average follow-up period was 96 (75-124) months. Seventy-five patients had anal fistula healing, with healing rate of 55.6%. Kaplan-Meier survival curve showed that the healing time of anal fistula was prolonged and finally stabilized at 55.6%. In the patients who failed initial treatment with anal fistula plug packing, there were 6 cases whose anal fistula healed spontaneously without other treatment. Among them, 3 cases healed spontaneously 2 years and 3 cases 3 years after operation without recurrence. From 2008 to 2012, the annual healing rates of anal fistula plug treatment were 3/6, 61.5% (24/39), 42.1% (24/57), 12/15 and 12/18, respectively. Multivariate logistic regression analysis showed that the duration of anal fistula≥6 months (OR=3.187, 95% CI: 1.361-7.466, P=0.008) was an independent risk factor for anal fistula healing after treatment with anal fistula plug.@*Conclusion@#The long-term efficacy of anal fistula plug procedure in the treatment of transsphincteric anal fistula is positive, and this procedure should be implemented as soon as possible.

ABSTRACT

This paper reviewed the mechanism and clinical application of Dahuang-Zhezhong Pill in Jin Gui Yao Lue. It was found that the anti-tumor effect of Dahuang-Zhezhong Pill was mainly achieved by inhibiting cell proliferation, inducing cell apoptosis, regulating immune function, inhibiting tumor angiogenesis, affecting tumor cell metastasis, and reversing cell resistance. It was used in the treatment of liver cancer, gastric cancer, esophageal cancer, lung cancer and other malignant tumors. Therefore, we should clarify the anti-tumor mechanism, the dominant dosage, the dominant tumor species of Dahuang-Zhezhong Pill, and standardize the dosage of treatment and adverse reactions.

ABSTRACT

Objective:By the method of network pharmacology, the mechanism of Pinelliae Rhizoma and Citri Exocarpium Rubrum in the treatment of metabolic syndrome was explored. Method:Effective components of Pinelliae Rhizoma and Citri Exocarpium Rubrum were retrieved by TCMSP database, and then selected by ADME parameters. TCMID,BATMAN-TCM,SymMap,TCM-MESH database were used to supplement effective components of TCMSP. TCMSP target prediction model was used to predict potential targets of drugs. DRUGBANK,DisGeNET,CTD,GeneCards,OMIM,PharmGkb,KEGG,DiGSeE databases were retrieved to obtain the targets of metabolic syndrome, and the chips were downloaded and analyzed through GEO database No.GSE98895 to screen out the differential genes of normal people and patients with metabolic syndrome, and supplement the target databases of metabolic syndrome. The intersections of Pinelliae Rhizoma-Citri Exocarpium Rubrum and metabolic syndrome disease targets were obtained by Rstudio 3.6.2. The above intersection targets were imported into the Metascape database for module analysis and overall GO(Biological Process),KEGG and Reactome pathway analysis. The core targets were selected from the intersection targets by using the cytohubba plug-in,the core genes were input into the database of BioGPS,Genecards to analyze the tissue distribution and subcellular distribution, and the core targets were assigned by using the database of DisGeNET. Result:A total of 34 active components and 120 targets of Pinelliae Rhizoma and Citri Exocarpium Rubrum were screened out,and 115 targets were obtained after the intersection of Rstudio 3.6.2. The results of Metascape module analysis and whole analysis were mostly the same, mainly involving the biological processes, such as ligand receptor interaction, calcium ion,cGMP-PKG,cholinergic synapse, thyroid hormone, insulin. The cytohubba plug-in was used to screen out 17 targets, involving 17 key genes, such as VEGFA and NOS3. The tissue and subcellular distribution of the core targets mainly included lymphoblasts,CD33+_myeloid cells, amygdala, pineal and cytoplasmic matrix, mitochondria. The main proteins were signal molecules, kinases and nucleic acids. Conclusion:Pinelliae Rhizoma and Citri Exocarpium Rubrum could treat metabolic syndrome through complex biological processes and pathways, such as blood circulation, ligand receptor interaction of nerve activity,cGMP-PKG,interleukin-related action, calcium ion. This indicates that traditional Chinese medicine(TCM) treated diseases through multi-component,multi-target,multi biological processes,multi-channel and other ways(which is also proved by the distribution of core genes in the tissue, subcellular and protein ascription information),indicating the superiority of the holism concept of TCM. Erchetang and its similar prescriptions are suitable for treating metabolic syndrome, which also indicates that the principle of "treating different diseases with the same therapy" of TCM is not only reflected at the theoretical level; and network pharmacology needs to be further proved in the combination with experimental verification.
