ABSTRACT

Since 2015 when the transmission of schistosomiasis was controlled in China, the country has been moving towards elimination of schistosomiasis, with the surveillance-response as the main interventions for schistosomiasis control. During the period of the 13th Five-Year Plan, the transmission of schistosomiasis had been interrupted in four provinces of Sichuan, Jiangsu, Yunnan and Hubei and the prevalence of schistosomiasis has been at the historically lowest level in China. As a consequence, the goal set in The 13th Five-Year National Schistosomiasis Control Program in China is almost achieved. However, there are multiple challenges during the stage moving towards elimination of schistosomiasis in China, including the widespread distribution of intermediate host snails and complicated snail habitats, many types of sources of Schistosoma japonicum infections and difficulty in management of bovines and sheep, unmet requirements for the current schistosomiasis control program with the currently available tools, and vulnerable control achievements. During the 14th Five-Year period, it is crucial to consolidate the schistosomiasis control achievements and gradually solve the above difficulties, and critical to provide the basis for achieving the ultimate goal of elimination of schistosomiasis in China. Based on the past experiences from the national schistosomiasis control program and the challenges for schistosomiasis elimination in China, an expert consensus has been reached pertaining to the objectives, control strategy and measures for The 14th Five-Year National Schistosomiasis Control Program in China, so as to provide insights in to the development of The 14th Five-Year National Schistosomiasis Control Program in China.

ABSTRACT

Objective To investigate the prevalence of Schistosoma japonicum infections in Elaphurus davidianus released to Poyang Lake areas and evaluate the impact of the project of “E. davidianus released to wild environments for natural reproduction and growth” “(E. davidianus return home project”) on the transmission of schistosomiasis in Poyang Lake areas. Methods During the period from April 2018 to December 2019, the population distribution, inhabiting activity and natural reproduction of E. davidianus released to Poyang Lake areas were investigated by means of GPS and artificial observations. The S. japonicum infection was identified in animal feces in E. davidianus inhabitats using a hatching test, and snail distribution was surveyed in E. davidianus inhabiting grass islands using a systematic sampling method. Results A total of 51 E. davidianus were released to the Poyang Lake areas in 2018, which subsequently produced 5 E. davidianus habitats in Yinlong Lake, Longkou, Nanchi Lake, Lianzi Lake and Zhu Lake. E. davidianus was found to predominantly inhibit in grass islands, farmlands and forest lands in hilly regions around the Poyang Lake areas. The natural reproduction rate of E. davidianus was 25% in the habitats in 2019, and the mean density of snails was 0.009 to 0.039 snails/0.1 m2 in E. davidianus inhabitats; however, no S. japonicum infection was identified in snails. In addition, the mean densities of wild E. davidianus and bovine feces were 4.6 samples/hm2 and 2.1 samples/hm2, Conclusions The E. davidianus released to Poyang Lake areas may get infections with S. japonicum, and cause schistosomiasis transmission through fecal contamination in grass islands. The impact on and the prevalence of S. japonicum infection was 4.35% and 13.16% (P = 0.236), respectively; however, the intensities of S. japonicum infections were “+++” and “+”, respectively. Conclusions The E. davidianus released to Poyang Lake areas may get infections with S. japonicum, and cause schistosomiasis transmission through fecal contamination in grass islands. The impact on local schistosomiasis transmission and the response strategy requires to be investigated following the release of E. davidianus to lake regions, to ensure the achievement of the goal of schistosomiasis elimination and the successful implementation of the “E. davidianus return home project” in Poyang Lake areas.

ABSTRACT

Objective@#Despite the remarkable progress in efforts to control disease spread, the nationwide elimination of hepatitis B in China is still hindered by the persistently high rate of hepatitis B virus (HBV) infection in Western China. This study aimed to evaluate the strategy of hepatitis B prevention and control in Western China and identify potential areas and strategies for improvement.@*Methods@#Susceptible population vaccination, health education, professional training of doctors, and other prevention and control measures have been implemented in Wuwei city since 2010. Data were obtained from three representative cross-sectional serosurveys conducted in 2010, 2013, and 2015. The serum samples were subjected to enzyme-linked immunosorbent assays to detect the following seromarkers: HBV surface antigen (HBsAg), antibody against hepatitis B surface antigen (anti-HBs), and antibody against hepatitis B core antigen (anti-HBc). Estimates of variance were determined using Taylor series linearization methods.@*Results@#The three serosurveys revealed decreases in the prevalence of HBsAg (7.19% in 2010 @*Conclusion@#Although vaccine-based prevention and control measures reduced the rate of HBV infection in Wuwei City over time, the hepatitis B infection rate in children younger than 10 years was still higher than the national average level. Therefore, the prevention and control of mother-to-child transmission and the management of the infected should be the focus of future prevention and control work.

ABSTRACT

Jiangxi Province was once one of schistosomiasis heavy endemic provinces in China. Thanks for more than 60 years’ unremitting effort, great achievements have been obtained in schistosomiasis control in this province. This paper reviews the control history of schistosomiasis, and demonstrates the current epidemic situation of the disease in Jiangxi Province, and its contribution to national schistosomiasis control achievements. The difficulties and challenges of schistosomiasis elimination in this province are analyzed and the countermeasures are also put forward accordingly.

Schistosomiasis is one of the major infectious diseases that seriously endanger the health of people, and social and economic development. Before the year of 1949, Yujiang County of Jiangxi Province was one of hyper-endemic regions for schistosomiasis. After the founding of the People’s Republic of China, under the leadership and high attention of the Chinese Communist Party and the government, after the arduous struggle with schistosomiasis, Yujiang County became the first county that reached the standard of eradication of schistosomiasis in 1958. Moreover, Chairman Mao Zedong gladly wrote the famous poem of "Farewell to the God of Plague". Since then, the work of schistosomiasis control has been greatly inspired by the poem. In 2016, Yujiang County became the first county of schistosomiasis elimination in Jiangxi Province. In this paper, the experience and process of eradication and elimination of schistosomiasis in Yujiang County are systematically reviewed and summarized by analyzing the endemic situation of schistosomiasis before and after the eradication and elimination in Yujiang County, to expound the significance and enlightenment of the schistosomiasis control in Yujiang County. This paper commemorates the 60th anniversary of the publication of Mao Zedong’s poem.

This article analyzes the influencing factors of rural public service supply of schistosomiasis prevention and control in China and the successful experiences of schistosomiasis prevention and pattern of Yujiang from an angle of view of rural public goods.This article points out the short supply of rural public goods in the service of schistosomiasis prevention and control in the traditional small-scale peasant economy,and the possibility of promoting the rural public goods in the cooperation economic model.The essential features of the schistosomiasis prevention and control pattern of Yujiang include strengthening the leadship of the Communist Party of China,improving health education,adjusting the measures to local condition,and so on.

ABSTRACT

[Objective]To explore the spatial distribution pattern of liver cancer in Sihui city ,Guangdong Province from 2004 to 2011,and to provide theoretical evidence for prevention and control of liver cancer.[Methods]Spatial scan statistic was applied,and analysis were conducted by using software SaTScan 9.4.4 to detect the clusters in Sihui from 2004 to 2011.Map making and results visualizing were carried out with software ArcGIS 10.2.[Results]A total of 1252 cases was reported in Sihui City,Guangdong Province from 2004 to 2011,with the incidence of liver cancer increasing from 32.67/100000 to 41.14/100000. Spatial scans results showed that highly clustered areas are mainly located in the south and southeast,which include Dasha,Longfu and Dongcheng countries in all people (LLR=6.34,P=0.019,RR=1.31) and the male (LLR=5.83,P=0.025,RR=1.33),respectively;highly clustered areas in female were include Dasha,Dongcheng,Chengzhong and other two countries(LLR=5.18,P=0.043,RR=1.41). Lowly clustered areas in all people are mainly located in the north,which include Luoyuan,Didou and Jiangfu countries(LLR=8.66, P=0.001,RR=0.67).[Conclusion]The incidence of primary liver cancer in Sihui was increasing over time,and spatial clustering of the cases were observed. Further study should be focused on the risk factors of liver cancer in related areas to strengthen the prevention and control of liver cancer.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the use of a 32P application device (AD) in the treatment of condyloma acuminatum (CA) in the rectum, and to compare its clinical effect with that of the microwave therapy.</p><p><b>METHODS</b>This study included 107 cases of CA in the rectum, 99 males and 8 females, aged 21-58 (33.6 +/- 9.4) years. Forty-six of the patients (the AD group) were treated with a self-made 32P application device, which, as a tube-shaped carrier of radionuclide 32P colloid, was fixed in the rectum at the diseased part for medication at 4.9-8.2 Gy for 3-5 hours once and 1-2 times a week. The other 61 (the microwave group) were treated by microwave burning under local anesthesia. Both groups of patients were followed up for over 3 months for comparison of the therapeutic results and observation of the stability and reliability of the 32P application device.</p><p><b>RESULTS</b>The rates of cure, reoccurrence and adverse reaction were 84, 8%, 13.0% and 8.7% in the AD group, compared with 40.3%, 55.7% and 75.4% in the microwave group, with statistically significant differences between the two groups (P < 0.01).</p><p><b>CONCLUSION</b>The 32P application device, with its advantages of low cost, easy operation, good effect, high safety and reliability, low recurrence, fewer adverse events and good acceptability, is highly valuable for the treatment of CA in the rectum.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate whether the polymorphisms of beta2-adrenergic receptor (beta2-AR) at position 16, 27, 164 are associated with asthma in Northern Chinese subjects.</p><p><b>METHODS</b>Genomic DNA was collected from unrelated Northern Chinese population of Han ethnicity, including 125 unrelated asthmatic individuals and 96 healthy controls. Beta2-AR genes at position 16, 27, 164 were amplified by using restriction fragment length polymorphism (RFLP) and allelic specific polymerase chain reaction methods. All asthmatics had their serum IgE (total and specific) antibody or skin-prick test measured, bronchial reactivity to methacholine (Mch) and bronchial reversibility by beta2-agonist evaluated.</p><p><b>RESULTS</b>(1) The frequency of Gly 16 homozygous was significantly higher in the asthmatic group than that in healthy controls (22.4% vs. 8.3%, P < 0.05), OR was 2.9 with 95% CI 1.26-6.78. The proportion of Gly 16 allele was also higher in asthmatics than that in control (0.46 vs. 0.36, P < 0.05); Gly16 homozygous was not independently associated with asthma pathogenesis (P = 0.21, OR 0.42 with 95% CI 0.11-1.61). (2) Of 51 night attack patients, 18 carrying Gly16 homozygosity, if compared with 10 of 74 nonnocturnal asthmatics carrying this genotype, there was significant difference between these two groups (35.3% vs. 13.5%, P < 0.01). (3) The average dose of PD20-Mch was significantly lower in patients carrying Gln 27 homozygous than those carrying homozygous Glu 27 and Gln/Glu 27 heterozygous (0.2 +/- 0.3, 1.6 +/- 0.8, and 2.1 +/- 3.0 micromol/L, P < 0.05).</p><p><b>CONCLUSION</b>Beta2AR gene polymorphisms might confer susceptibility to asthma in Chinese Northern patients. Beta2-AR gene, coordinated with other candidate loci, plays a role in the development of asthma.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate whether beta 2-adrenergic receptor gene (beta 2AR) polymorphism at position 16, 27, 164 is in association with asthma susceptibility or asthmatic phenotype (including nocturnal asthma, serum IgE level, bronchial responsiveness, the status of asthmatics).</p><p><b>METHODS</b>By using PCR-RFLP and allelic-specific PCR (ASP), the polymorphism of beta 2AR gene at position 16, 27, 164 in 125 Han origin asthmatics and 96 normal healthy controls with the same ethnic nearby Beijing region were genotyped. All patients had their serum total IgE (TIgE) measured by RAST, pulmonary ventilatory function assessed by FEV1% and FEV1/FVC, bronchial responsiveness challenged by methacholine (if FEV1% > 70%), and brocho-reversibity by inhaling beta 2-agonist.</p><p><b>RESULTS</b>There was higher prevalence of Gly16 homozygous of beta 2AR in asthmatics than that in normal healthy controls (22.4% vs 8.3%, P < 0.05), with odd ratio (OR) 2.918 (95% CI: 1.256-6.781); Also there was higher frequency of Gly16 homozygous of beta 2AR in nocturnal asthmatics than that in nonnocturnal asthmatics (35.3% vs 13.5%, P < 0.01), but Gly16 homozygous of beta 2AR was low an independent risk factor for the pathogenesis of asthma. The dose of methacholine was low in asthmatics carrying Gln27 homozygous beta 2AR than Glu27 homozygous beta 2AR and Gln/Glu27 heterozygous beta 2AR in brocho-challenge test [(0.205 +/- 0.275) vs (2.11 +/- 3.00) vs (1.575 +/- 0.828) mumol, P < 0.05].</p><p><b>CONCLUSIONS</b>Gly16 homozygous beta 2AR was associated with asthma susceptibility in Chinese patients with Han ethnic nearby Beijing region, and Gly16 homozygous beta 2AR was associated significantly with nocturnal asthma. Glu27 homozygous beta 2AR was related to hyper-bronchial reactivity of asthmatics.</p>
