ABSTRACT

Objective To compare the clinical efficacy of DSA-guided pyloric balloon dilation with that of conservative medical treatment for postsurgical gastroparesis syndrome(PGS).Methods The clinical data of 41 patients with PGS,who were admitted to the Department of Interventional Radiology,Affiliated Third Hospital of Suzhou University of China to receive DSA-guided pyloric balloon dilation(study group,n=21)or conservative medical treatment(control group,n=20)between January 2012 and February 2021,were retrospectively analyzed.The median follow-up period in the control group and the study group was 626 days and 588 days respectively.The average length of stay(ALOS)was compared between the two groups.Results In the study group,all the 21 patients returned to normal food intake within 2 days after successful DSA-guided pyloric balloon dilation treatment,and they were all discharged within one week after treatment.The ALOS in the study group was(10.2±1.3)days,which was strikingly shorter than(34.5±14.2)days in the control group,the difference between the two groups was statistically significant(P<0.001).Conclusion For the treatment of PGS,DSA-guided pyloric balloon dilation carries better clinical efficacy when compared with conservative medical treatment,therefore,it can be used as the preferred treatment for patients with PGS.Further studies are need to be conducted before its therapeutic mechanism can be clarified.

ABSTRACT

Objective To conduct an epidemiological survey of common congenital heart disease(CHD)among children in ethnic minority areas in southeastern Guizhou and to explore the influencing factors of delayed medical treatment.Methods From January 2019 to July 2022,18 850 children aged 3 months to 14 years in Qiandongnan Miao and Dong Autonomous Prefecture were selected;105 children with CHD were included in the training set,and they were divided into delayed group(80 cases)and non-delayed group(25 cases)according to whether or not to delay medical treatment.In addition,children with CHD(35 cases)from July 2022 to December 2022 were included in the validation set.The general data of the subjects in the two groups were compared and ana-lyzed.Multivariate logistic regression was performed and risk scoring model was constructed.Results The preva-lence of CHD in 18 850 children was 5.57‰(105/18 850),with the highest prevalence in Liping County,and the lowest in Huangping County.The proportion of children with secondary atrial septal defect was the highest,and that of the aortic valve malformation was the lowest.Among the complex cases of CHD,the proportion of children with single type was the highest,and that of children with three or more types were the lowest.Among children with CHD,the rate of delayed medical treatment was 76.19% (80/105).The median delay in medical treatment was 12 months,with an average of(18.78±4.77)months.Multifactor logistic regression analysis showed that heart murmur(level 2~3),less-educated(primary and secondary school)guardian,family per capita income<2 000 yuan,and frequent drinking of the guardian were independent risk factors for delayed medical treatment(P<0.05),and commercial settlement of medical expenses was independent protective factor(P<0.05).Risk scoring model divided the children into three groups:low risk(≤80 points),medium risk(>80 points and≤134 points)and high(>134 points)risk group.The evaluation of the model show that it was accurate,effective,safe,and reliable.Conclusion The highest prevalence is observed in Liping County.The proportion of children with secondary atrial septal defect and the proportion of children with single type are the highest.Delayed medical treat-ment is found in most of the children with CHD.Cardiac murmur,education background of the guardian,per capita family income,guardian alcohol consumption,and medical expense settlement method are all independent influencing factors for delayed medical treatment.

ABSTRACT

Objective To investigate the changing trend of the current situation of Internet-based oncology outpatient treatment and provide support for the development and management of Internet hospitals.Methods The ARIMA and GM(1,1)models were constructed based on the Internet-based outpatient data of a cancer hospital from January 2021 to June 2023,and the fitting effect was evaluated by mean absolute error(MAE)and root mean square error(RMSE).Based on the model,the pro-portion of Internet-based outpatient visits and the offline outpatient visits were predicted from July to December 2023.Results ARIMA(1,1,2)and GM(1,1)models were used to predict the proportion of Internet-based outpatient visits.The average abso-lute errors were 2.06%and 2.41%,and the root-mean-square errors were 3.01%and 3.17%,respectively.The ARIMA(0,1,1)and GM(1,1)models were used to predict the proportion of Internet-based outpatient visits to the offline outpatient visits,with the rate of the average absolute errors of 0.58%and 1.08%,respectively,and the rate of the root mean square errors 0.75%and 1.31%,respectively.The figures indicated that the ARIMA model had a better prediction effect.The forecast results showed that the predicted value of Internet outpatient service in December 2023 was 90.35%,and the predicted value of Internet-based outpatient service accounted for 16.46%of the offline outpatient service.Conclusion In 2021-2023,the proportion of Inter-net-based outpatient visits in the cancer hospital showed a steady trend,and the proportion of Internet outpatient visits in the off-line outpatient visits showed a rising trend.Therefore,hospitals need to establish a continuous monitoring mechanism,constantly adjust management strategies and measures to meet the needs of patients and continue to promote the high-quality development of Internet-based medical services.

[Objective]To explore the specificity and effectiveness of Medical Records Integration of Palace in Qing Dynasty for the treatment of diseases,and to provide a reference for the modern clinical external therapy.[Methods]Taking 117 fumigating and washing decoctions in Medical Records Integration of Palace in Qing Dynasty as the research object,using Excel software,the formulas,dosage,medicinal properties,and efficacy of Qinggong fumigating and washing decoctions were organized and counted.Combining with the relevant medical cases and the commentaries in the book,the use of the fumigating and washing decoctions in the Qing Palace was systematically organized.[Results]The 117 fumigating and washing decoctions in Medical Records Integration of Palace in Qing Dynasty show many features such as there are many kinds of formula,the quality of the formula is refined;the effect is strength and special focus,formula with modification according to symptoms,flexible usage of medicine,treatment first,independent use of the amount of medicine,good use of the wind-extinguishing medicinal,filling in the poisonous features when needed.It also has other advantages such as a wide range of audiences,a unique approach,sophisticated instruments,and a meticulous process of preparation of the liquid.[Conclusion]The use of the fumigating and washing decoctions in Medical Records Integration of Palace in Qing Dynasty has palace characteristics and advantages,exploring its use can gain unique insights and revelations,which helps to carry forward the characteristics of court medication and promotes the development of external therapeutic methods of traditional Chinese medicine as high research value.

ABSTRACT

Moyamoya angiopathy is a chronic progressive occlusive intracranial vasculopathy, and CT angiography, MRI, digital subtraction angiography are the auxiliary examinations. Headache is a common symptom in Moyamoya angiopathy (MMA ) patients, and the phenotypes of headache attributed to MMA mainly include migraine-like headache and tension type-like headache; mechanism involves in dilatation of intracranial and extracranial arteries and leptomeningeal collaterals, cerebral hypoperfusion, vascular endothelial damage, genetic susceptibility, and mental stress. Strategies such as surgical revascularization and medical treatment are given. This article focuses on clinical manifestations, pathogenesis, diagnoses, treatments and prognoses of headache attributed to MMA, in order to deepen the understanding of clinical workers on this symptom.

This paper briefly described the communication context of hospice care, the communication subject and the current situation of hospice care, analyzed the problems and challenges encountered by professional health caregivers, which mainly including medical staff, in the communication context of hospice care diagnosis and hospice care, and made the prospects of the development of effective communication measures.

The development of the"integration of physical education,medical care,and elderly care"model is an important part of building a higher quality elderly care service system during the 14th Five Year Plan period.Based on literature review and logical analysis,it analyzes the driving forces and difficulties of the development of the"integration of sports,medicine,and nutrition"model during the 14th Five Year Plan period,and proposes optimization paths.It is believed that the core driving forces for the development of the"integration of physical education,medical care,and elderly care"model are the aging population and increasing demand for health,the continuous improvement of medical and health services,and the gradual increase in national policy support.The development of the"integration of physical education,medical care,and nursing"model faces practical difficulties such as incomplete laws and regulations,shortage of professional talents,low social awareness,imbalanced resource allocation,and unsmooth industrial collaboration.Based on this,combined with the national health development demand during the 14th Five Year Plan period,it is proposed to strengthen the construction of laws and regulations and formulate unified standards and norms;innovate talent training channels and establish a diversified training system;increase publicity and education efforts to raise public awareness and understanding;integrate and optimize resource allocation to improve resource utilization efficiency;build an industrial integration complex to promote industrial synergy and linkage.

ABSTRACT

Objective To identify and analyze the digital supervision issues of cross-province medical treatment in China,and propose countermeasures and suggestions.Methods Using literature research,expert interviews,and questionnaire surveys,from January 29,to February 132023,managers,staff from remote departments of medi-cal insurance bureau and scholar in the field of medical insurance in H province were selected.Principa l component analysis was used to extract questions,and based on the extraction results,a multiple logistic regression model to explore that have a significant impact on the digital supervision of cross-province direct settlement of medical treat-ment.Results The problems of digital supervision of direct settlement for medical treatment across provinces and re-gions in China can be divided into 7 categories,with a cumulative variance contribution rate of 63.17%.The connec-tion of digital regulatory systems,audit rules and early warning issues,as well as the lack of legal systems and defi-nitions have a significant impact on regulatory effectiveness(P<0.05).Conclusion Incomplete construction of the digital supervision system between the place of insurance participation,and inadequate legal and institutional sateguards for the digital supervision of the direct settlement of cross-provincial cross-district medical consultations.Therefore,strengthening the basic construction of the digital supervision system for cross provincial and cross regional medical direct settlement,and improving the legal system for cross provincial and cross regional medical direct settle-ment digital supervision,have important guiding significance for the digital supervision of cross provincial and cross regional medical direct settlement in China.

ABSTRACT

Objective To investigate the effect of anterograde lavage via ileal double-loop stoma on preoperative colonoscopy and postoperative bowel function.Methods A total of 191 patients who underwent laparoscopic anterior rectal resection plus prophylactic double-loop ileal stomy in Quanzhou First Hospital Affiliated to Fujian Medical University from January 2019 to December 2021 were selected and divided into anterograde group(n=97)and retrograde group(n=94)according to different lavage methods.Patients in anterograde group underwent temporary double-loop ileal stomy with distal lavage.Patients in retrograde group underwent anal lavage.The qualified rate of colonoscopy,external damage rate of intestinal mucosa,exhaust time,defecation time,fluid intake time,total hospitalization time,total hospitalization cost and satisfaction of two groups of patients were compared.Results The qualified rate of preoperative colonoscopy in anterograde group was significantly higher than that in retrograde group,and the external damage rate of intestinal mucosa was significantly lower than that in retrograde group(P<0.05).The postoperative exhaust time,fluid intake time and total hospitalization time in anterograde group were significantly shorter than those in retrograde group(P<0.05).The total hospitalization cost in anterograde group was significantly lower than that in retrograde group(P<0.05).The satisfaction of patients in anterograde group was significantly higher than that in retrograde group(89.69%vs.52.13%,Z=-7.165,P<0.001).Conclusion Anterograde lavage via ileal double-loop stoma can improve the qualified rate of preoperative colonoscopy,is conducive to postoperative intestinal function recovery,reduce patients'pain,and improve patients'satisfaction and comfort,which is worthy of clinical application.

@#Objective To explore the application effect of"Internet + Medical treatment"model in patients with pulmonary rehabilitation after lung cancer surgery.Methods A total of 87 patients with lung cancer who received elective surgery in our hospital from June 2022 to January 2023 were selected and divided into control group(44 cases)and observation group(43 cases)according to random number table method.The control group received routine intervention,and the observation group received intervention based on"Internet + medical treatment"mode.The pulmonary function indexes[forced vital capacity(FVC),first second forced vital capacity(FEV1),respiratory flow(PEF)],cardiorespiratory endurance level[6min walking test(6MWT)],respiratory fatigue and quality of life(WHOQOL-BREF)of the two groups were compared.Results After 6 months of intervention,the scores of FVC,FEV1,PEF,6MWT distance and WHOQOL-BREE in the study group were higher than those in the control group,and the scores of respiratory fatigue were lower than those in the control group,with statistical significance(P<0.05).Conclusion"Internet + Medical treatment"intervention can improve lung function,increase cardiopulmonary endurance,reduce respiratory fatigue and improve quality of life in patients recovering from lung cancer surgery.

Introducción: El embarazo ectópico intersticial es una forma de presentación poco frecuente, con una incidencia del 2-4% de los embarazos ectópicos; sin embargo, a pesar de su baja incidencia la mortalidad es cinco veces mayor, impactando en las cifras de mortalidad materna y representando en torno al 10-15% de los casos. Objetivo: Presentar un caso de embarazo ectópico intersticial, cuya ocurrencia es poco frecuente, así como el abordaje satisfactorio del manejo médico con mifepristona y metotrexato. Caso clínico: Mujer de 28 años con antecedente de resección tubárica por quiste paraovárico derecho, quien acudió a urgencias por hallazgo en ecografía obstétrica de sospecha de embarazo intersticial izquierdo y se le administró manejo farmacológico con dosis de metotrexato y mifepristona, con éxito. Conclusiones: El manejo médico con metotrexato y mifepristona para el embarazo ectópico intersticial parece ser una elección eficaz en los casos con estabilidad hemodinámica y deseo de conservación de la fertilidad.


Background: Interstitial ectopic pregnancy represents a rare form of presentation, with an incidence of 2-4% of all ectopic pregnancies. However, despite its low incidence, it is associated with a five-fold increase in mortality, significantly impacting maternal mortality rates, accounting for approximately 10-15% of cases. Objective: To present a case of interstitial ectopic pregnancy, which is a rare occurrence, as well as the successful medical management approach with mifepristone and methotrexate. Case report: A 28-year-old women with a history of right paraovarian cyst tubal resection presented to the emergency department due to suspected left interstitial pregnancy identified on obstetric ultrasound. The patient was successfully managed with pharmacological treatment using doses of methotrexate and mifepristone. Conclusions: Medical management with methotrexate and mifepristone for interstitial ectopic pregnancy appears to be an effective choice in cases with hemodynamic stability and a desire for fertility preservation.

@#Authoritative guidelines and consensus domestic and overseas suggest that patients with diabetes get influenza vaccine and pneumococcal vaccine,which can significantly reduce the incidence and hospitalization rate of influenza and pneumococcal diseases in diabetic patients,and can also reduce the risk of cardiovascular events and mortality.The protective effect of simultaneous administration is more significant.At present,the vaccination rate of influenza and pneumococcal vaccines for diabetic patients is still low in China.Effective measures should be taken to promote the integration of medical treatment and prevention,strengthen knowledge popularization and guide publicity,and improve the vaccination rate,so as to effectively improve the survival status and prognosis of diabetic patients.This paper compre-hensively reviews the research progress on the effectiveness of influenza and pneumococcal vaccines in diabetic patients at home and abroad in recent years.

ABSTRACT

Objective To understand the current situation and explore the influencing factors of delay in seeking medical treatment for common symptoms of residents in the rural areas of Sichuan province. Methods In July 2019,multi-stage random sampling was carried out in Zigong city,Sichuan province,and the data were collected by face-to-face questionnaire interview.The residents who had lived at hometown for more than half a year in the past year and had seen a doctor in the most recent month were surveyed.Logistic regression was adopted to predict the influencing factors of delay in seeking medical treatment. Results A total of 342 subjects were enrolled,and the incidence of delay in seeking medical treatment was 13.45%(46/342).Compared with the young and middle-aged(<65 years)people,the elderly(≥65 years)people were more likely to have delay in seeking medical treatment (OR=2.187,95%CI=1.074-4.457,P=0.031).The rural residents who gave higher score of the overall quality of township health centers were less likely to have delay in seeking medical treatment (OR=0.854,95%CI=0.735-0.992,P=0.039). Conclusions The occurrence of delay in seeking medical treatment for common symptoms of rural residents in Sichuan province is low.Age and the overall quality evaluation of township health centers affect the occurrence of delay in medical treatment among the rural residents in Sichuan province.Efforts should be made to improve the awareness of disease prevention among the elderly in rural areas.The investment in health resources in township health centers should be increased to strengthen the introduction and training of talents.These measures can improve the health services in township health centers,guide residents to make timely use of health resources,and reduce the occurrence of delay in seeking medical treatment.

ABSTRACT

Objective:To explore the causes and feelings of delayed experience of seeking medical treatment in patients with advanced lung cancer, and to provide new insights for more targeted health education and medical care services.Methods:A semi-structured in depth interview based on the theory of planned behavior was conducted among 30 patients with advanced lung cancer who experienced medical delay from November to December in 2021 admitted to First Affiliated Hospital of Guangxi Medical University. The interview content was analyzed and abstracted by using Colaizzi phenomenological analysis method and Nvivo11.0 software.Results:The delay duration of 30 patients with advanced lung cancer ranged from 90 to 213 days. Four subject groups were extracted by generic analysis: the cause of delay, the cause to seek medical help, the worry about the disease, and solutions.Conclusions:The delay behavior of patients with advanced lung cancer is affected by external situational factors such as symptom severity, family economic capacity, social support, accessibility of health services, prevalence of novel coronavirus, and subjective psychological factors such as sense of stigma and burden of disease, it is necessary to reduce the occurrence of medical delay in patients with advanced lung cancer through the comprehensive management strategy of multiple channels.

Objective:To explore the reasons for delayed medical treatment in diabetic foot patients using the health ecology model as a framework.Methods:From November of 2021 to March of 2022, descriptive phenomenological research methods was used in 20 patients with diabetic foot who delayed medical treatment were interviewed face-to-face semi-structured in depth, and the data analysis was carried out using the seven-step method of Colaizzi phenomenology to refine the theme.Results:According to the five levels of the health ecology model, the reasons for delayed medical treatment were summarized into 11 themes, and the reasons for the personal trait layer included foot ulcers are not detected promptly, and the perceptual decline of advanced patients. Behavioral characteristics included lack of knowledge of diabetic foot, negative medical psychology. Reasons for the interpersonal network included insufficient family support and lack of social support; reasons for living and working conditions included high economic pressure, low electronic health literacy, and inconvenient medical treatment; the reasons for the policy environment included uneven medical insurance reimbursement ratios and limited medical conditions.Conclusions:The reason for the delay in seeking medical treatment for diabetic foot patients is multi-layered and systematic, and medical staff need to improve the self-management ability of diabetic patients′ feet from the individual level, and also need to guide patients from the system level on how to obtain the best medical resources.

Ocular fundus diseases is a kind of ophthalmic diseases that occur in the vitreous, retina, choroid and optic nerve, including a series of pathophysiological changes such as inflammation, exudation and proliferation. Because of high morbidity and high blindness rate, ocular fundus diseases has been paid more and more attention from medical community. With the continuous deepening of research on its etiology, anatomy and pathological mechanism in recent years, clinicians have obtained more abundant treatment methods than in the past, and the medical treatment of ocular fundus diseases have made many phased progress. However, due to its wide spectrum of diseases and complex pathological mechanism, clinicians still need to further explore more effective treatment methods, and improve the effect of diagnosis and treatment to ocular fundus diseases.

The tiered medical treatment system stands as a cornerstone in the deepening reforms of China′s medical and health sectors, playing a crucial role in building a healthy China. Exploring the harmonious coexistence mechanism of multiple entities on the supply and demand sides of the tiered diagnosis and treatment system, and promoting the formation of a scientific and reasonable tiered diagnosis and treatment order, has become an urgent public proposition that needs to be answered and has significant social impact. Addressing the challenges in China′s tiered medical treatment system, particularly its need for a more systematic, comprehensive, and collaborative approach, this study is informed by a thorough literature review. Based on the harmonious management and symbiosis theories, the authors proposed a theoretical concept and future research path for the formation of a harmonious symbiotic mechanism in the tiered diagnosis and treatment system, in order to pave the way for exploring the harmonious symbiotic mechanism in the tiered diagnosis and treatment system.
