ABSTRACT
Introducción: La tuberculosis (TB) extrapulmonar es la afectación de cualquier órgano, sin compromiso pulmonar demostrado, como consecuencia de la diseminación hematógena/linfática del bacilo de Koch. Presentación de caso: Paciente en puerperio inmediato cursando cuadro clínico de gonalgia que se estudió con resonancia magnética que mostró lesión endomedular en región distal del fémur izquierdo. Se estudió con tomografía de tórax, abdomen y pelvis que evidenciaron otras lesiones a nivel esplénico, sin compromiso hepático ni pulmonar. Se realizó punción diagnóstica femoral con evidencia de granulomas con necrosis central. Se interpretó tuberculosis extrapulmonar y se inició tratamiento antifímico con mejora sintomática. Discusión: La TB extrapulmonar puede impactar a nivel de pleura, ganglios linfáticos, vías urinarias, sistema osteoarticular, sistema nervioso central y abdomen. En el embarazo, la prevalencia de TB extrapulmonar es baja. Conclusión: La TB femoral y esplénica concomitante en pacientes embarazadas es un hallazgo infrecuente por lo que su análisis resulta de gran importancia. Arribar al diagnóstico requiere un elevado índice de sospecha. El retraso diagnóstico conlleva a un aumento de la morbimortalidad
Introduction: Extrapulmonary tuberculosis (TB) is the involvement of any organ, without demonstrated pulmonary involvement, as a consequence of the hematogenous/lymphatic dissemination of the Koch bacillus. Case presentation: Patient in the immediate postpartum period with clinical symptoms of gonalgia that was studied with magnetic resonance imaging showing intramedullary lesion in the distal region of the left femur. A CT scan of the chest, abdomen and pelvis showed other lesions at the splenic level, without liver or lung involvement. A femoral diagnostic puncture was performed with evidence of granulomas with central necrosis. Extrapulmonary tuberculosis was interpreted and antifimic treatment was started with symptomatic improvement. Discussion: Extrapulmonary TB can impact the pleura, lymph nodes, urinary tract, osteoarticular system, central nervous system and abdomen. During pregnancy, the prevalence of extrapulmonary TB is low. Conclusion: Concomitant femoral and splenic TB in pregnant patients is a rare finding, which is why its analysis is of great importance. Arriving at a diagnosis requires a high index of suspicion. Delayed diagnosis leads to an increase in morbidity and mortalit

La extensión completa de la rodilla es esencial para la marcha. Los pacientes con parálisis cerebral infantil con frecuencia pueden tener déficit de extensión de distinta magnitud, lo que compromete la marcha e incluso la bipedestación. El tratamiento de la contractura en flexión de rodilla parte por tratar la espasticidad de los músculos comprometidos y con fisioterapia. Cuando el flexo es estructurado, el tratamiento es quirúrgico mediante distintas técnicas, dependiendo de la magnitud de la contractura y de la edad del paciente. Las técnicas sobre partes blandas incluyen alargamientos funcionales de isquiotibiales y transferencias musculares. Cuando la contractura es capsular, es preferible realizar cirugía ósea, la cual extiende el fémur proximal, ya sea en forma progresiva, mediante fisiodesis anterior en pacientes pediátricos, o en forma aguda, mediante osteotomía extensora del fémur distal. Con frecuencia existe una patela alta, la cual hay que corregir en el mismo acto quirúrgico para mantener la eficiencia del aparato extensor
Full knee extension is essential for gait. Patients with cerebral palsy frequently have extension deficits of different magnitudes, which compromise walking and even standing up. The treatment of knee flexion contracture begins by addressing the spasticity of the involved muscles and includes physical therapy. For structured extension deficits, the treatment is surgical, using different techniques depending on the magnitude of the contracture and the patient's age. Soft tissue techniques include functional hamstring lengthening and muscle transfers. For capsular contracture, bone surgery is preferable and extends the proximal femur either progressively, through anterior physiodesis in pediatric patients, or acutely, by extensor distal femoral osteotomy. A high patella is common and requires correction during the same surgical procedure to maintain the efficiency of the extensor apparatus

Contexto: A rotura da inserção distal do tendão semimembranoso é uma lesão ocasionada no tendão de um dos músculos do grupo isquiotibiais, acarretando dor no joelho. Para o diagnóstico da lesão do tendão semimembranoso, a ressonância magnética é a principal modalidade de imagem. Descrição do caso: Homem de 63 anos acusa dor no joelho esquerdo há 1 semana. Apresenta melhora da dor com repouso e medicação. Ao exame físico, apresenta teste de Waldron positivo e testes de McMurray e Appley negativos, com edema articular, sem hematomas, apresentando limitação à movimentação devido à dor. A ressonância magnética apresenta a rotura do semimembranoso, e o tratamento foi realizado com medicação analgésica e anti-inflamatória. Discussão: Essa rotura é rara, e surge de uma força em valgo em uma perna hiperextendida externamente. O uso de esteroides anabolizantes pode aumentar o risco de rotura do tendão. Contudo, a ultrassonografia é um método de fácil acesso e é cada vez mais utilizada. O diagnóstico é realizado por meio de ressonância magnética. Seu tratamento pode ser realizado com anti-inflamatórios ou reparo cirúrgico. Conclusão: Relatamos um caso de rotura do tendão semimembranoso, composta por uma sintomatologia como dor no joelho, que pode ser tratada com anti-inflamatório ou reparo cirúrgico.

BACKGROUND@#To perform anatomical anterior cruciate ligament reconstruction (ACLR), tunnels should be placed relatively higher in the femoral anterior cruciate ligament (ACL) footprint based on the findings of direct and indirect femoral insertion. But the clinical results of higher femoral tunnels (HFT) in double-bundle ACLR (DB-ACLR) remain unclear. The purpose was to investigate the clinical results of HFT and lower femoral tunnels (LFT) in DB-ACLR.@*METHODS@#From September 2014 to February 2016, 83 patients who underwent DB-ACLR and met the inclusion and exclusion criteria were divided into HFT-ACLR (group 1, n = 37) and LFT-ACLR (group 2, n = 46) according to the position of femoral tunnels. Preoperatively and at the final follow-up, clinical scores were evaluated with International Knee Documentation Committee (IKDC), Tegner activity, and Lysholm score. The stability of the knee was evaluated with KT-2000, Lachman test, and pivot-shift test. Cartilage degeneration grades of the International Cartilage Repair Society (ICRS) were evaluated on magnetic resonance imaging (MRI). Graft tension, continuity, and synovialization were evaluated by second-look arthroscopy. Return-to-sports was assessed at the final follow-up.@*RESULTS@#Significantly better improvement were found for KT-2000, Lachman test, and pivot-shift test postoperatively in group 1 ( P >0.05). Posterolateral bundles (PL) showed significantly better results in second-look arthroscopy regarding graft tension, continuity, and synovialization ( P <0.05), but not in anteromedial bundles in group 1. At the final follow-up, cartilage worsening was observed in groups 1 and 2, but it did not reach a stastistically significant difference ( P >0.05). No statistically significant differences were found in IKDC subjective score, Tegner activity, and Lysholm score between the two groups. Higher return-to-sports rate was found in group 1 with 86.8% (32/37) vs. 65.2% (30/46) in group 2 ( P = 0.027).@*CONCLUSION@#The HFT-ACLR group showed better stability results, better PL, and higher return-to-sports rate compared to the LFT-ACLR group.

OBJECTIVE@#To evaluate the early effectiveness of local infiltration anesthesia (LIA) with compound betamethasone in total knee arthroplasty (TKA).@*METHODS@#The clinical data of 102 patients with knee osteoarthritis who were treated by TKA and met the selection criteria between May 2022 and March 2023 were retrospectively analyzed. They were divided into control group and study group according to whether LIA preparation was added with compound betamethasone, with 51 cases in each group. There was no significant difference of baseline data, such as age, gender, body mass index, operative side, preoperative range of motion (ROM), Knee Society Score (KSS), white blood cell (WBC), and hematocrit between the two groups ( P>0.05). The intraoperative total blood loss and hidden blood loss were recorded, and WBC was recorded on the 1st, 2nd, and 3rd days after operation. Pain was assessed by visual analogue scale (VAS) score on the 1st, 2nd, and 3rd days after operation and morphine intake milligrames equivalent within 48 hours after operation. Passive ROM, maximum extension and flexion angles of knee joint were measured on the 3rd day after operation; the early postoperative complications were recorded.@*RESULTS@#There was no significant difference in total blood loss and hidden blood loss between the two groups ( P>0.05). The postoperative pain levels in both groups were relatively mild, and there was no significant difference in VAS scores in the first 3 days after operation and in morphine intake milligrams equivalent within 48 hours after operation between the two groups ( P>0.05). The WBC in the first 3 days after operation was significantly improved in both groups ( P<0.05). The WBC in the study group was significantly higher than that in the control group on the 1st and 2nd days after operation ( P<0.05), but there was no significant difference between the two groups on the 3rd day after operation ( P>0.05). On the 3rd day after operation, the maximum extension angle of knee joint in the study group was smaller than that in the control group, while the maximum flexion angle and passive ROM of knee joint in the study group were larger than those in the control group, and the differences were significant ( P<0.05). There were 6 cases of fever and 17 cases of deep venous thrombosis in the control group, and 1 case and 14 cases in the study group, respectively. There was no poor wound healing and periprosthetic joint infection in the two groups, and there was no significant difference in the incidence of complications between the two groups ( P>0.05).@*CONCLUSION@#The application of compound betamethasone in LIA during TKA is a safe and optimal strategy to promote the early postoperative rehabilitation of patients.

OBJECTIVE@#To investigate the efficacy and clinical results of total internal protection technique in anterior cruciate ligament reconstruction.@*METHODS@#A total of 56 patients undergoing anterior cruciate ligament reconstruction treated from January 2018 to December 2019 were selected. According to the different surgical methods, they were divided into total internal reconstruction group and standard bone tunnel group. There were 21 patients in the total internal reconstruction group, including 15 males and 6 females, aged from 20 to 48 with an average of (35.6±6.7) years old, and 35 patients in the standard tibial tunnel group, including 26 males and 9 females, aged 22 to 51 years old with an average of (33.7±9.6) years old. Preoperative examination of Lachman test was positive, magnetic resonance indicated anterior cruciate ligament rupture. There were no significant differences between the two groups in age, sex, body mass index, time from injury to ACL reconstruction, combined meniscus injury and operation method, operation time, ligament diameter, ligament length and other general information. Postoperative evaluation included operation duration, length and diameter of transplanted tendon after braid. International Knee Documentation Committee (IKDC) score, Lysholm score, Tegner score and perioperative complications 2 years after surgery.@*RESULTS@#Both groups were followed up, ranging from 24 to 30 months with an average of (26.9±3.4) months. Postoperative incision healing was good, and no failure or joint infection occurred at the last follow-up. There was no statistically significant difference between the two groups in IKDC score, Lysholm score and Tegner score before, 1 year and 2 years after surgery. However, IKDC score, Lysholm score and Tegner score at 1 year and 2 years after surgery.@*CONCLUSION@#The same postoperative function and stability of knee joint can be obtained by both the residual whole technique and the standardized reconstruction technique. In the residual whole group, only the semitendinosus muscle is taken, and the femoral thin muscle is retained, with greater tibial bone mass preserved, which is safe and effective in clinical practice.

OBJECTIVE@#To explore effect of nerve growth factor (NGF) antibody on knee osteoarthritis (KOA) pain model was evaluated by in vitro model.@*METHODS@#Thirty male SPF rats aged 28-week-old were divided into blank group (10 rats with anesthesia only). The other 20 rats were with monoiodoacetate (MIA) on the right knee joint to establish pain model of OA, and were randomly divided into control group (injected intraperitoneal injection of normal saline) and treatment group (injected anti-NGF) intraperitoneal after successful modeling, and 10 rats in each group. All rats were received retrograde injection of fluorogold (FG) into the right knee joint. Gait was assessed using catwalk gait analysis system before treatment, 1 and 2 weeks after treatment. Three weeks after treatment, right dorsal root ganglia (DRG) were excised on L4-L6 level, immunostained for calcitonin gene-related peptide (CGRP), and the number of DRGS was counted.@*RESULTS@#In terms of gait analysis using cat track system, duty cycle, swing speed and print area ratio in control and treatment group were significantly reduced compared with blank group (P<0.05). Compared with control group, duty cycle and swing speed of treatment group were significantly improved (P<0.05), and there was no significant difference in print area ratio between treatment group and blank group (P>0.05). The number of FG-labeled DRG neurons in control group was significantly higher than that in treatment group and blank group (P<0.05). The expression of CGRP in control group was up-regulated, and differences were statistically significant compared with treatment group (P<0.05).@*CONCLUSION@#Intraperitoneal injection of anti-NGF antibody inhibited gait injury and upregulation of CGRP in DRG neurons. The results suggest that anti-nerve growth factor therapy may be of value in treating knee pain. NGF may be an important target for the treatment of knee OA pain.

The discoid meniscus is a common congenital meniscal malformation that is prevalent mainly in Asians and often occurs in the lateral discoid meniscus. Patients with asymptomatic discoid meniscus are usually treated by conservative methods such as observation and injury avoidance, while patients with symptoms and tears need to be treated surgically. Arthroscopic saucerization combined with partial meniscectomy and meniscus repair is the most common surgical approach., and early to mid-term reports are good. The prognostic factors are the patient's age at surgery、follow-up time and type of surgery. Some patients experience complications such as prolonged postoperative knee pain, early osteoarthritis, retears and Osteochondritis dissecans. The incidence of prolonged postoperative knee pain was higher and the incidence of Osteochondritis dissecans was the lowest. Retears of the lateral meniscus is the main reason for reoperation.

PURPOSE@#Many techniques have been described for the reconstruction of chronic lateral collateral ligament (LCL) rupture with different autograft options. The advantages of percutaneous LCL reconstruction include small incisions, minimal soft tissue disruption, less postoperative pain, and speedy rehabilitation and recovery. The aim of this study was to report the functional outcome of percutaneous LCL reconstruction and overall patient satisfaction in Africans.@*METHODS@#This prospective and interventional study involving 51 patients with chronic LCL rupture who had percutaneous LCL reconstruction using peroneus longus autograft was conducted between January 2021 and December 2022 in National Orthopaedic Hospital, Dala-Kano, Nigeria. The inclusion criteria were patients between the ages of 18 and 45 years with chronic isolated LCL and not more than 1 injury of knee ligament. Exclusion criteria were active infection, and multi-ligament knee injury requiring 2-staged surgery. The knee functions were assessed preoperatively, 3 months, 6 months, and 12 months postoperatively using the Lysholm scoring system. Patient satisfaction with the outcome of the treatment was assessed using a 5-point Likert scale. Relevant information was recorded into Microsoft Excel sheet and data was analyzed using SPSS version 23.0 for windows. The paired samples t-test was used to compare the clinical outcomes as continuous variables. Statistical significance was considered at p < 0.05.@*RESULTS@#The mean age of the patients was (30.10 ± 5.90) years. The median time from injury to surgery was 7 months (ranging from 3 to 28 months). The mean follow-up period was (14.07 ± 3.13) months. The mean preoperative and 1-year postoperative Lysholm scores were 44.33 ± 12.97 and 97.96 ± 1.23, respectively.@*CONCLUSION@#Percutaneous LCL reconstruction using peroneus longus autograft significantly improves patient knee function and results in excellent patient satisfaction.

Abstract Patella baja is an infrequent knee pathology, but it is limiting due to joint stiffness and localized pain in the anterior region of the knee. It may occur after trauma, prolonged immobilization or local surgical intervention. The striking pathological finding is the shortening and increase in thickness of the patellar ligament. Several surgical techniques have been described for its treatment, and there is no standardized treatment. We describe the case of a 73-year-old female patient who presented with knee stiffness, significant functional deficit, and patella baja after total knee arthroplasty. She underwent a recently described soft tissue surgical procedure, with excellent functional evolution, improving from a Lysholm Knee Score of 16 to 81 points, allowing early mobilization and return to daily life activities.
Resumo A patela baixa é uma patologia do joelho pouco frequente, mas limitante devido à rigidez articular e dor localizada na região anterior do joelho. Pode decorrer após trauma, imobilização prolongada ou intervenção cirúrgica local. O achado patológico marcante é o encurtamento e o aumento na espessura do ligamento patelar. Foram descritas várias técnicas cirúrgicas para o seu tratamento, não existindo nenhum tratamento padronizado. Descrevemos o caso de uma doente de 73 anos de idade que apresentava rigidez do joelho, deficit funcional importante e patela baixa após artroplastia total do joelho. Foi submetida a um procedimento cirúrgico de partes moles recentemente descrito, com excelente evolução funcional, melhorando de um Lysholm Knee Score de 16 para 81 pontos, permitindo a mobilização e regresso às atividades de vida diárias precocemente.

Abstract Osteoarticular tuberculosis of the knee is an uncommon disease presentation, especially in children under 1 year old. Diagnosis based on classic methods (such as culture and anatomopathological examination) is a challenge due to the paucibacillary characteristic of the infection. Risk factors include contact with individuals with bacilliferous tuberculosis, living in a region with high disease prevalence, and pediatric age group. We describe a case of chronic monoarthritis caused by Mycobacterium tuberculosis and intermittent inflammatory manifestations in a 10-month-old male patient with no extra-articular symptoms and no history of contact with bacilliferous tuberculosis. The culture was negative, and the anatomopathological examination was inconclusive for the etiologic agent. The detection of traces of M. tuberculosis DNA by a rapid molecular test (GeneXpert) based on the polymerase chain reaction technique established the diagnosis. The treatment consisted of antituberculosis drugs and led to complete resolution of the clinical-radiographic picture. This case emphasizes the importance of considering tuberculosis in the initial differential etiologic diagnoses of arthritis and, therefore, the need for an early, specific investigation, even when the clinical suspicion is not high.
Resumo A tuberculose osteoarticular do joelho é uma apresentação incomum da doença, especialmente em crianças com menos de 1 ano de idade. A característica paucibacilar da infecção torna o diagnóstico um desafio, levando em consideração métodos mais clássicos como a cultura e o anatomopatológico. Os fatores de risco são contato com indivíduos com tuberculose bacilífera, estar em uma região de alta prevalência, e populações pediátricas. O presente relato descreve um caso de monoartrite por Mycobacterium tuberculosis, de curso crônico e manifestações inflamatórias intermitentes em um paciente masculino de 10 meses, sem sintomatologia extra-articular e sem história de convívio ou contato prévio com tuberculose bacilífera. A cultura foi negativa e o exame anatomopatológico foi inconclusivo para o agente etiológico e o diagnóstico foi realizado pela detecção de traços de DNA de M. tuberculosis no teste rápido molecular (GeneXpert), utilizando a técnica da reação em cadeia da polimerase. O tratamento foi realizado com medicamentos antituberculose e houve resolução completa do quadro clínico-radiográfico. Este caso enfatiza a importância de considerar a tuberculose como parte dos diagnósticos etiológicos diferenciais iniciais das artrites e, portanto, a necessidade da investigação precoce específica a esta bactéria, ainda que a suspeição clínica não seja elevada.

Patellofemoral instability (PFI) is a common knee disorder in the skeletally immature patient. PFI presents challenges in this patient population that require an understanding of anatomy and biomechanics to formulate a treatment plan aimed at preventing redislocation and facilitating return to activity. In this review, the different presentations and therapeutic alternatives will be analyzed, providing a practical guide for the comprehensive management of this complex clinical condition. Abstract Objective To study the influence of various tunnel parameters and graft inclination angle (GIA) on the clinical and radiological outcome after anterior cruciate ligament reconstruction (ACLR) at long-term follow-up. Methods In this retrospective study, 80 patients with isolated anterior cruciate ligament (ACL) injury treated by single bundle ACLR using bone patellar tendon bone (BPTB) and hamstring (HT) autografts were evaluated clinically and radiologically at their long-term follow-up. The study population were divided into two groups based on ideal and nonideal tunnel parameters as well as ideal and nonideal GIA. The various tunnel parameters and GIA were interpreted with clinical and radiological outcome at long-term follow-up. Results Eighty patients, 36 (45%) using BPTB and 44 (55%) using HT autografts, were available to complete the study. Patients with ideal coronal tibial tunnel angle (CTTA) and coronal femoral tunnel angle (CFTA) show superior clinical outcome (pivot shift test) than nonideal CTTA and CFTA, which was found to be statistically significant (p-value < 0.038 and 0.024, respectively). Similarly, patients with ideal coronal tibial tunnel position (CTTP) show superior clinical outcome (International Knee Documentation Committee - IKDC objective) over nonideal CTTP (p-value < 0.017). All other tunnel parameters and GIA were not found to have influence on clinical outcome. None of the tunnel parameters have influenced osteoarthritis (OA) change. There was no progression of OA change in the study population at long-term follow-up after ACLR. Conclusion Ideal coronal tunnel parameters produced a better clinical outcome at long-term follow-up after ACLR. There was no progression of OA change at long-term follow-up after isolated ACLR.
Resumo Objetivo O objetivo deste estudo foi analisar a influência de vários parâmetros do túnel e do ângulo de inclinação do enxerto (GIA, do inglês graft inclination angle) nos desfechos clínicos e radiológicos da reconstrução do ligamento cruzado anterior (RLCA) no acompanhamento de longo prazo. Métodos Neste estudo retrospectivo, 80 pacientes com lesão isolada do ligamento cruzado anterior (LCA) submetidos à RLCA de feixe único com autoenxertos de tendão patelar ósseo (TPO) e isquiotibiais (IT) foram avaliados clínica e radiologicamente durante o acompanhamento em longo prazo. A população do estudo foi dividida em dois grupos com base nos parâmetros ideais ou não ideais do túnel, bem como no GIA ideal e não ideal. Os vários parâmetros do túnel e o GIA foram interpretados com os desfechos clínicos e radiológicos no acompanhamento em longo prazo. Resultados Oitenta pacientes, sendo 36 (45%) submetidos ao procedimento com autoenxertos de TPO e 44 (55%) com autoenxertos IT, puderam completar o estudo. Pacientes com ângulo do túnel tibial coronal (ATTC) e ângulo do túnel femoral coronal (ATFC) ideais apresentam resultados clínicos superiores (teste de pivot shift) do que aqueles com ATTC e ATFC não ideais, sendo a diferença estatisticamente significativa (valor de p < 0,038 e 0,024, respectivamente). Da mesma forma, pacientes com posição do túnel tibial coronal (PTTC) ideal apresentam resultado clínico superior (International Knee Documentation Committee [IKDC] objetivo) em relação àqueles com PTTC não ideal (valor de p < 0,017). Os demais parâmetros do túnel e o GIA não influenciaram o desfecho clínico. Nenhum dos parâmetros do túnel influenciou a alteração associada à osteoartrite (OA). Não houve progressão da alteração da OA na população do estudo no acompanhamento em longo prazo após a RLCA. Conclusão Os parâmetros ideais do túnel coronal produziram um melhor desfecho clínico no acompanhamento de longo prazo após a RLCA. Não houve progressão da alteração da OA no acompanhamento em longo prazo após a RLCA isolada.

Abstract Objective Lateral extra-articular tenodesis (LET) has been proposed to resolve rotatory instability following anterior cruciate ligament reconstruction (ACLR). The present meta-analysis aimed to compare the clinical outcomes of ACLR and ACLR with LET using the modified Lemaire technique. Materials and Methods We performed a meta-analysis following the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) staement. The literature search was performed on the PubMed, EBSCOHost, Scopus, ScienceDirect, and WileyOnline databases. The data extracted from the studies included were the study characteristics, the failure rate (graft or clinical failure) as the primary outcome, and the functional score as the secondary outcome. Comparisons were made between the patients who underwent isolated ACLR (ACLR group) and those submitted to ACLR and LET through the modified Lemaire technique (ACLR + LET group). Results A total of 5 studies including 797 patients were evaluated. The ACLR + LET group presented a lower risk of failure and lower rate of rerupture than the ACLR group (risk ratio [RR] = 0.44; 95% confidence interval [95%CI]: 0.26 to 0.75; I2 = 9%; p = 0.003). The ACLR + LET group presented higher scores on the Knee Injury and Osteoarthritis Outcome Score (KOOS) regarding the following outcomes: pain, activities of daily living (ADL), sports, and quality of life (QOL), with mean differences of 0.20 (95%CI: 0.10 to 0.30; I2 = 0%; p < 0.0001), -0.20 (95%CI: -0.26 to -0.13; I2 = 0%; p < 0.00001), 0.20 (95%CI: 0.02 to 0.38; I2 = 0%; p = 0.03), and 0.50 (95%CI: 0.29 to 0.71; I2 = 0%; p < 0.00001) respectively when compared with the ACLR group. Conclusion Adding LET through the modified Lemaire technique to ACLR may improve knee stability because of the lower rate of graft rerupture and the superiority in terms of clinical outcomes. Resumo Objetivo A tenodese extra-articular lateral (TEL) foi proposta para resolver a instabilidade rotatória após a reconstrução do ligamento cruzado anterior (RLCA). Esta metanálise teve como objetivo comparar os resultados clínicos da RLCA e da RLCA com TEL por meio da técnica de Lemaire modificada. Materiais e Métodos Esta metanálise foi feita de acordo com a declaração dos Itens Principais para Relatar Revisões Sistemáticas e Metanálises (Preferred Reporting Items for Systematic Reviews and Meta-Analysis, PRISMA, em inglês). A pesquisa bibliográfica foi realizada nos bancos de dados PubMed, EBSCOHost, Scopus, ScienceDirect e WileyOnline. Dos estudos incluídos foram extraídas informações sobre as características do estudo, a taxa de falha (falha clínica ou do enxerto) como resultado primário, e o escore funcional como resultado secundário. Foram feitas comparações entre os pacientes submetidos apenas à RLCA (grupo RLCA) e à RLCA e TEL pela técnica de Lemaire modificada (grupo RLCA + TEL). Resultados Foram avaliados 5 estudos que incluíam 797 pacientes. O grupo RLCA + TEL apresentou um risco menor de falha e menor taxa de rerruptura do que o grupo RLCA (razão de risco [RR] = 0,44; intervalo de confiança de 95% [IC95%] 0,26 a 0,75; I2 = 9%; p = 0,003). O grupo RLCA + TEL obteve pontuações maiores no Escore de Desfechos de Osteoartrite e Lesão no Joelho (Knee Injury and Osteoarthritis Outcome Score, KOOS, em inglês) com relação aos seguintes desfechos: dor, atividades cotidianas (AC), esportes, e qualidade de vida (QV), com diferenças médias de 0,20 (IC95%: 0,10 a 0,30; I2 = 0%; p < 0,0001), -0.20 (IC95%: -0,26 a-0,13; I2 =0%; p < 0,00001), 0,20 (IC95%: 0,02 a 0,38; I2 = 0%; p = 0,03) e 0,50 (IC95%: 0,29 a 0,71; I2 = 0%; p < 0,00001), respectivamente, quando comparado com o grupo RLCA. Abstract Objective: Our study investigated changes of knee laxities in athletes and non-athletes females and relationship between knee laxity and sex-steroid at menstrual cycle phases. Methods: Forty six healthy females, twenty four athletes and twenty two non-athletes not on hormone contraceptive pills, had no previous knee injuries and with regular menstrual cycles for 3 consecutive months, participated in the study. Medial and lateral knee laxities were determined by varus-valgus tests at follicular, ovulatory and luteal phases. Serum level of relaxin, estrogen, progesterone and testosterone were determined by ELISA and radioimmunoassay. Results: Knee laxities in athletes and non-athletes at 0° and 20° flexion were the highest in luteal phase with non-athletes possess greater laxity than athletes. Positive correlation between progesterone and relaxin levels with knee laxities were observed. Meanwhile, the levels of both hormones were highest in the luteal phase. Conclusion: Increased medial and lateral knee laxities in athletes and non-athletes associated with high serum progesterone and relaxin levels in luteal phase may contribute toward increased risk of non-contact knee injury. However, lower knee laxity in athletes than non-athletes suggest that exercise could be a protective factor.
Resumo Objetivo: Nosso estudo investigou alterações na lassidão do joelho em atletas e não atletas do sexo feminino e a relação entre a lassidão do joelho e esteroides sexuais nas fases do ciclo menstrual. Métodos: Quarenta e seis mulheres saudáveis, vinte e quatro atletas e vinte e duas não atletas, sem uso de pílulas anticoncepcionais hormonais, sem lesões anteriores no joelho e com ciclos menstruais regulares por 3 meses consecutivos, participaram do estudo. A lassidão medial e lateral do joelho foi determinada por testes de varo-valgo nas fases folicular, ovulatória e lútea. Os níveis séricos de relaxina, estrógeno, progesterona e testosterona foram determinados por ensaio imunoenzi mático (ELISA) e radioimunoensaio. Resultados: A lassidão do joelho em atletas e não atletas em 0° e 20° de flexão foi maior na fase lútea; as não atletas apresentavam maior lassidão do que as atletas. Houve uma correlação positiva entre os níveis de progesterona e relaxina e a lassidão do joelho. Além disso, os níveis desses dois hormônios foram maiores na fase lútea. Conclusão: O aumento da lassidão medial e lateral do joelho em atletas e não atletas, associado a altos níveis séricos de progesterona e relaxina na fase lútea, pode contribuir para o aumento do risco de lesão sem contato no joelho. No entanto, a menor lassidão do joelho em atletas do que em não atletas sugere que o exercício pode ser um fator protetor.

Introducción: la función del tejido meniscal es fundamental en la transmisión y distribución de cargas de la rodilla. En casos de lesión meniscal, la sutura es el tratamiento quirúrgico de elección, y pueden utilizarse diferentes materiales para realizarla. El objetivo del trabajo es analizar la aparición de quistes meniscales sintomáticos con el uso de sutura no reabsorbible del tipo monofilamento de polipropileno en un grupo de pacientes con lesiones en asa de balde y evaluar el material de sutura como causal de quiste.Materiales y métodos: se realizó un estudio retrospectivo de un grupo de pacientes con lesiones en asa de balde suturadas con material no reabsorbible del tipo monofilamento de polipropileno (MP) y sutura de alta resistencia de polietileno de ultra alto peso molecular UHMWPE (SAR) y se analizó la presencia de quiste meniscal sintomático como complicación. Los criterios de inclusión para este estudio fueron pacientes con lesiones en asa de balde a los cuales se les realizó sutura meniscal con técnicas combinadas con material de tipo no reabsorbible. Se excluyeron pacientes con lesiones que no fueran con patrón en asa de balde o a los cuales se les hizo sutura meniscal todo-adentro únicamente.Resultados: un total de veinticinco pacientes, quince de sexo masculino y diez de sexo femenino. La edad media del grupo evaluado fue de 27.8 años. El 72% fue suturado con sutura de alta resistencia (SAR) mientras que a un 28% se les realizó sutura con monofilamento de polipropileno (MP). El 92% de las lesiones se presentaron en el menisco interno. Se observaron tres pacientes con quistes meniscales sintomáticos asociados a la sutura MP, los cuales fueron tratados quirúrgicamente con buena evolución. Conclusión: identificamos en nuestra serie que la aparición de quistes sintomáticos se relaciona estrechamente con el uso de monofilamento de polipropileno (MP). Consideramos la utilización de hilos no reabsorbibles de alta resistencia (polietileno de ultra alto peso molecular UHMWPE) como la primera opción para realizar las suturas meniscales por la baja tasa de complicación y los buenos resultados. Introduction: The function of the meniscal tissue is fundamental in the transmission and distribution of knee loads. In cases of meniscal injury, suturing is the surgical treatment of choice, and different materials can be used to perform it. The aim of the work is to analyze the appearance of symptomatic meniscal cysts with the use of non-absorbable polypropylene monofilament suture in a group of patients with bucket handle injuries and to evaluate the suture material as a cause of the cyst. Materials and methods: a retrospective study was carried out on a group of patients with bucket-handle injuries sutured with non-absorbable polypropylene monofilament (MP) material and high-strength ultra-high molecular weight UHMWPE polyethylene (SAR) suture and the presence of a symptomatic meniscal cyst as a complication was analyzed. The inclusion criteria for this study were patients with bucket-handle injuries in whom meniscal suture was performed with combined techniques with non-resorbable material. Patients with injuries other than a bucket-handle pattern or who underwent all-in meniscal suturing only were excluded.Results: a total of twenty-five patients, fifteen males and ten females. The average age of the evaluated group was 27.8 years. 72% were sutured with high resistance suture (SAR) while 28% were sutured with polypropylene monofilament (MP). 92% of the injuries occurred in the medial meniscus. Three patients were observed with symptomatic meniscal cysts associated with the MP suture, which were treated surgically with good outcomes. Conclusion: we identified in our series that the appearance of symptomatic cysts is closely related to the use of polypropylene monofilament (MP). We consider the use of high-strength non-absorbable threads (ultra-high molecular weight polyethylene UHMWPE) as the first option to perform meniscal sutures due to the low complication rate and good results. Introducción: Los meniscos son estructuras semilunares formadas por fibrocartílagos, localizadas entre el fémur y la tibia. El menisco externo es más móvil, debido a sus inserciones a través de los ligamentos meniscotibiales y poplíteomeniscales. El menisco interno tiene un desplazamiento de 2-3 mm comparado con un desplazamiento de 9-10 mm del menisco externo. Se ha descripto en la literatura mundial que la hipermovilidad meniscal es secundaria a lesión de ligamentos poplíteomeniscales (principales estabilizadores), sin embargo, se realizó un estudio cadavérico donde se evidenció que los ligamentos poplíteomeniscales desempeñan un papel secundario. El objetivo de este estudio es demostrar que la lesión de los ligamentos meniscotibiales es la causante de la hipermovilidad meniscal externa.Materiales y métodos: se realizó un estudio cadavérico en 2022 en Arthrex, Naples, Florida, Estados Unidos. Previo a la valoración artroscópica se efectuó la sección de ligamentos meniscotibiales en el tercio posterior del menisco externo, manteniendo los ligamentos poplíteomeniscales y la inserción de la raíz posterior. Posteriormente, se efectuó la valoración artroscópica en la que se vio traslación anterior y superior del tercio posterior del menisco externo y se realizó la fijación meniscal.Resultados: mediante la fijación del tercio posterior del menisco lateral con técnica transósea, en una falla o insuficiencia de los ligamentos meniscotibiales, se logra estabilidad completa del menisco.Conclusión: la estabilidad principal del tercio posterior del menisco lateral está dada por la inserción periférica de los ligamentos meniscotibiales, por lo que la hipermovilidad meniscal externa no se debe a lesión de los ligamentos poplíteomeniscales. Introduction: The menisci are semilunar structures formed by fibrocartilage, located between the femur and the tibia. The lateral meniscus is more mobile due to its insertions through the tibial meniscus and popliteal meniscal ligaments. The medial meniscus has a displacement of 2-3 mm compared to a displacement of 9-10 mm for the external meniscus. It has been described in the world literature that meniscal hypermobility is secondary to injury to the popliteal meniscal ligaments (main stabilizers), however a cadaveric study was carried out where it was shown that the meniscal popliteal ligaments play a secondary role. The objective of this study is to demonstrate that injury to the meniscotibial ligaments is the cause of external meniscal hypermobility.Materials and methods: the cadaveric study was carried out in 2022 at Arthrex, Naples, Florida, United States. Prior to the arthroscopic evaluation, section of the meniscotibial ligaments was performed in the posterior third of the lateral meniscus, maintaining the popliteal meniscal ligaments and the posterior root insertion. Subsequently, the arthroscopic assessment is performed, showing anterior and superior translation of the posterior third of the external meniscus, and meniscal fixation is performed.Results: by fixing the posterior third of the lateral meniscus with a transosseous technique, in a failure or insufficiency of the meniscotibial ligaments, complete stability of the meniscus is achieved. Conclusion: the main stability of the posterior third of the lateral meniscus is given by the peripheral insertion of the meniscotibial ligaments, so external meniscal hypermobility is not due to injury to the popliteal meniscal ligaments. Introducción: La reconstrucción artroscópica del ligamento cruzado anterior (LCA) tiene un bajo índice de complicaciones postoperatorias. Cuando se presenta una infección en el postoperatorio inmediato, tanto el cirujano como el paciente se enfrentan a un escenario complejo y de difícil aceptación, que en algunos casos tiene consecuencias graves. En este contexto es importante conocer la incidencia reportada en la literatura para compararla con lo que ocurre en nuestras instituciones. El objetivo del presente trabajo es evaluar la incidencia de infección en una serie de casos consecutivos de reconstrucciones de LCA. Materiales y métodos: se realizó un estudio retrospectivo del período comprendido entre enero de 2015 y diciembre de 2017, donde se evaluó la incidencia de infección en una serie consecutiva de reconstrucciones de LCA. Fueron evaluados un total de setecientos catorce pacientes: seiscientos veintinueve masculinos y ochenta y cinco femeninos con un promedio de edad de veintiocho años (15-50).Resultados: de los setecientos catorce casos analizados, se encontraron seis infecciones postquirúrgicas (dos profundas y cuatro superficiales). Conclusiones: la incidencia de infección aguda postoperatoria luego de reconstrucción artroscópica del LCA fue del 0.84%, similar a lo reportado en la literatura. Se observó un franco predominio de infecciones en las reconstrucciones de LCA con injerto de isquiotibiales con respecto al resto de los injertos. Nivel de Evidencia: IV
Introduction: Complication rate of arthroscopic anterior cruciate ligament (ACL) reconstruction is low. Post-operative infection place both, the surgeon and patient in a difficult situation with an unknown outcome and possible serious sequalae. It is important to investigate the incidence of these complications in our institutions to compare them with existing literature. The aim of this study is to report the incidence of infection in a consecutive case series of ACL reconstruction. Materials and methods: retrospective study of consecutive ACL reconstructions performed between January 2015 to December 2017. The incidence of infection was evaluated in 714 cases, 629 males and 85 females, with average age of 28 years (15-50).Results: six post-operative infections were found (four superficial and two deep infections). Conclusions: the incidence of infection was 0.84% in this case series. Most of the infections were in patients with autologous gracilis-semitendinous grafts. Level of Evidence: IV

La rotura del tendón rotuliano es infrecuente y aún más lo es su re-rotura. La integridad de este tendón es fundamental para una adecuada movilidad de la rodilla y una marcha apropiada. El tratamiento se basa en diferentes técnicas quirúrgicas: reparación y aumentación tendinosa usando autoinjertos o aloinjertos. En el presente artículo se describe una alternativa en el posicionamiento de los autoinjertos del tendón de gracilis y tendón de semitendinoso, preservando su inserción en el caso de tener mala calidad de tejidos blandos. A los seis meses postoperatorios se logra un adecuado mecanismo extensor íntegro, con arcos de movilidad en el plano sagital de 0 a 90°, con un buen patrón de marcha sin dolor.
Rupture of the patellar tendon is infrequent and even more infrequent re-rupture; the integrity of this tendon is necessary for an adequate knee mobility and a proper gait. The treatment is based on different surgical techniques performing a tendon repair and augmentation using autografts or allografts. The present article describes an alternative in the positioning of autografts of gracilis tendon and semitendinosus tendon, achieving a complete extensor mechanism after a 6-month follow-up, with ranges of mobility in the sagittal plane from 0 to 90°, with an adequate gait pattern without pain

La rótula baja, también llamada patela ínfera, se describe como un acortamiento del tendón rotuliano. Al momento de medir la altura rotuliana existen diferentes métodos,1 uno de ellos es el índice de Insall-Salvati, que con un valor menor a 0.8 o una diferencia del 15% con la contralateral nos indica que estamos en presencia de una rótula baja. En nuestra experiencia, preferimos utilizar el índice de Caton-Deschamps, que define una rótula baja cuando el valor es menor a 0.6.En esta nota técnica, desarrollaremos un caso secundario o adquirido.
The patella baja, also called patella infera, is described as a shortening of the patellar tendon. When measuring patellar height, there are different methods,1 one of them is the Insall-Salvati index, which with a value less than 0.8 or a difference of 15% with the contralateral index indicates that we are in the presence of a low patella. In our experience, we prefer to use the Caton-Deschamps index, which defines a low patella when the value is less than 0.6.In this technical note, we will develop a secondary or acquired case.
