ABSTRACT

Este artigo reflete sobre os modos como a cisnormatividade, conceito impulsionado pelos transfeminismos, tem auxiliado na composição da psicologia de maneira histórica. Ao elaborar uma crítica sobre como a violência de gênero está expressivamente presente no território brasileiro, discute-se como tem sido pensada a saúde mental, esfera que, uma vez inserida nesse contexto mais amplo, está sendo convocada a produzir saídas criativas em relação aos sujeitos que são alvo de discriminações transfóbicas. Na busca de deslocar o olhar do indivíduo para o social, foi realizado um estudo bibliográfico para investigar os diferentes impactos que a cisnormatividade opera em nossos currículos psicológicos, gerando efeitos na prática e na própria profissão. A aposta está em reconhecer outras epistemologias como projetos éticos e políticos a uma psicologia contemporânea, e a contribuição transfeminista a "outra" clínica. É nesse sentido que este trabalho se destina a pensar um modo de cuidado que esteja baseado na singularidade, mas que, ao mesmo tempo, seja capaz de dedicar alguma atenção ao paradigma normativo que nos guia como terapeutas.(AU)


This article reflects on the ways that cisnormativity, a concept boosted by transfeminisms, has played a historical role in the composition of psychology. Elaborating a criticism on how gender violence is expressively present in the Brazilian territory, we discuss how mental health is conceived, a sphere that, inserted in this wider context, is invited to create creative solutions related to the subjects who are the target of transphobic discrimination. Trying to shift the focus from the individual to the collective, a bibliographical study was conducted to recognize the different impacts that cisnormativity has in our psychological curriculums, having effects on the practice and on the profession itself. The goal is to recognize other epistemologies as ethical and political projects for contemporary psychology and the transfeminist contribution to "another" clinic. It is in this sense that this work aims to think about a form of care that is based on singularity, but that can also pay attention to the normative paradigm that guides us as therapists.(AU)


Este artículo reflexiona sobre las formas en que la cisnormatividad, un concepto impulsado por los transfeminismos, ha tenido un papel en la composición de la psicología de manera histórica. Al elaborar una crítica sobre como la violencia de género está expresamente presente en el territorio brasileño, se discute cómo se ha pensado la salud mental, dominio que, una vez insertado en este contexto más amplio, es convocado a producir soluciones creativas con relación a los sujetos que son objeto de discriminación transfóbica. Al desviar el enfoque del individuo hacia lo social, se realizó un estudio bibliográfico para investigar los diferentes impactos que tiene la cisnormatividad en nuestros planes de estudios psicológicos, generando efectos en la práctica y en la propia profesión. El foco está en reconocer otras epistemologías como proyectos éticos y políticos para la psicología contemporánea y la contribución transfeminista a una "otra" clínica. En este sentido, este trabajo pretende pensar en una forma de cuidado que se basa en la singularidad, al mismo tiempo que sea capaz de dedicar cierta atención al paradigma normativo que a nosotras nos guía como terapeutas.(AU)

ABSTRACT

Com os avanços tecnológicos e o aprimoramento da prática médica via ultrassonografia, já é possível detectar possíveis problemas no feto desde a gestação. O objetivo deste estudo foi analisar a prática do psicólogo no contexto de gestações que envolvem riscos fetais. Trata-se de um estudo qualitativo sob formato de relato de experiência como psicólogo residente no Serviço de Medicina Fetal da Maternidade Escola da Universidade Federal do Rio de Janeiro (UFRJ). Os registros, feitos por observação participante e diário de campo, foram analisados em dois eixos temáticos: 1) intervenções psicológicas no trabalho em equipe em consulta de pré-natal, exame de ultrassonografia e procedimento de amniocentese; e 2) intervenções psicológicas em casos de bebês incompatíveis com a vida. Os resultados indicaram que o psicólogo nesse serviço é essencial para atuar de forma multiprofissional na assistência pré-natal para gravidezes de alto risco fetal. Ademais, a preceptoria do residente é relevante para sua formação e treinamento para atuação profissional no campo da psicologia perinatal.(AU)


Face to the technological advances and the improvement of medical practice via ultrasound, it is already possible to detect possible problems in the fetus since pregnancy. The objective of this study was to analyze the psychologist's practice in the context of pregnancies which involve fetal risks. It is a qualitative study based on an experience report as a psychologist trainee at the Fetal Medicine Service of the Maternity School of UFRJ. The records, based on the participant observation and field diary, were analyzed in two thematic axes: 1) psychological interventions in the teamwork in the prenatal attendance, ultrasound examination and amniocentesis procedure; and 2) psychological interventions in cases of babies incompatible to the life. The results indicated that the psychologist in this service is essential to work in a multidisciplinary way at the prenatal care for high fetal risk pregnancies. Furthermore, the resident's preceptorship is relevant to their education and training for professional performance in the field of Perinatal Psychology.(AU)


Con los avances tecnológicos y la mejora de la práctica médica a través de la ecografía, ya se puede detectar posibles problemas en el feto desde el embarazo. El objetivo de este estudio fue analizar la práctica del psicólogo en el contexto de embarazos de riesgos fetal. Es un estudio cualitativo basado en un relato de experiencia como residente de psicología en el Servicio de Medicina Fetal de la Escuela de Maternidad de la Universidade Federal do Rio de Janeiro (UFRJ). Los registros, realizados en la observación participante y el diario de campo, se analizaron en dos ejes temáticos: 1) intervenciones psicológicas en el trabajo en equipo, en la consulta prenatal, ecografía y los procedimientos de amniocentesis; y 2) intervenciones psicológicas en casos de bebés incompatibles con la vida. Los resultados señalaron como fundamental la presencia del psicólogo en este servicio trabajando de forma multidisciplinar en la atención prenatal en el contexto de embarazos de alto riesgo fetal. Además, la tutela del residente es relevante para su educación y formación para el desempeño profesional en el campo de la Psicología Perinatal.(AU)

ABSTRACT Systemic lupus erythematosus is a multisystemic autoimmune disorder that predominantly affects women in reproductive years. Pregnancy in women with SLE is still considered a high-risk condition although several strategies may improve maternal and fetal outcomes. Preconception counseling is fundamental and should include identification of risk factors for adverse pregnancy outcomes, explanation of potential maternal and obstetric complications and timely planning of pregnancy. Risk stratification must consider end-organ damage, comorbidities, disease activity and autoantibodies profile in order to implement an individual-risk pregnancy monitoring plan by a multidisciplinary team. Hydroxychloroquine and low dose aspirin have shown to lower the risk of disease flares and preeclampsia with a good safety profile, so its use during pregnancy in all SLE patients is recommended. Lupus nephritis and preeclampsia share clinical and laboratory features hindering differentiation between both entities. Novel angiogenic markers and fetal ultrasound findings could be helpful in the differential diagnosis, especially after 20 weeks of gestation. Antiphospholipid antibodies, particularly lupus anticoagulant, are closely associated with obstetric complications. Therapy with low dose aspirin and heparin, according to risk profile, may improve live birth rates. Anti-Ro/La antibodies confer risk for neonatal lupus, and therefore preventive therapy and special fetal surveillance should be instituted.


RESUMEN El lupus eritematoso sistémico es un trastorno autoinmune multisistémico que afecta primordialmente a mujeres en edad reproductiva. El embarazo en mujeres con LES aún se considera una condición de alto riesgo, a pesar de que diversas estrategias pueden mejorar los desenlaces maternos y fetales. La asesoría preconcepción es fundamental, y debe incluir la identificación de factores de riesgo de desenlaces adversos del embarazo, una explicación de las posibles complicaciones maternas y obstétricas, así como la planificación oportuna del embarazo. La estratificación de riesgos debe considerar el daño orgánico terminal, las comorbilidades, la actividad de la enfermedad y el perfil de autoanticuerpos, a fin de llevar a cabo un plan de monitoreo de los riesgos individuales del embarazo por parte de un equipo multidisciplinario. La hidroxicloroquina y la aspirina a bajas dosis han demostrado reducir el riesgo de exacerbaciones de la enfermedad y de preeclampsia, con un buen perfil de seguridad, por lo cual se recomienda su uso en todas las pacientes con LES durante el embarazo. La nefritis lúpica y la preeclampsia comparten características clínicas y de laboratorio, obstaculizando la diferenciación entre las 2 entidades. Nuevos marcadores angiogénicos y hallazgos ecográficos fetales pudieran ser de utilidad para el diagnóstico diferencial, especialmente después de las 20 semanas de gestación. Los anticuerpos antifosfolípidos, en particular el anticoagulante lúpico, tiene una estrecha asociación con las complicaciones obstétricas. El tratamiento con aspirina a bajas dosis y heparina, según el perfil de riesgos, puede mejorar las tasas de nacimientos vivos. Los anticuerpos anti-Ro/La representan un riesgo de lupus neonatal, por lo cual debe instituirse tratamiento preventivo y vigilancia fetal especial.

INTRODUCCIÓN. En el paciente crítico ha existido un conglomerado de situaciones dadas por alteración de las hormonas acorde al comportamiento del eje hipotalámi-co-hipofisario- gonadal, entender su rol es fundamental. OBJETIVO. Describir las alteraciones de las hormonas sexuales en el paciente críticamente enfermo desde un enfoque fisiológico y clínico. MATERIALES Y MÉTODOS. Estudio observacional, de revisión bibliográfica y análisis sistemático de 84 artículos científicos y selección de muestra de 27 en MedLine, The Cochrane Library Plus, LILACS y Web of Science; en español e inglés y variables: hormonas esteroides gonadales, enfermedad crítica, endocrinología, estrés, gónadas y disfunción, periodo 1998-2017. CONCLUSIÓN. Las alteraciones detectadas fueron un mecanismo para la producción de hormonas esteroideas hacia la síntesis predominante de cortisol y soportar el alto estrés meta-bólico de los pacientes. Las citocinas pro inflamatorias fueron importantes en éstos cambios. La polifarmacia fue un factor adicional poco ponderado de la alteración endocrina sexual.


INTRODUCTION. In the critical patient there has been a conglomerate of situations given by alteration of the hormones according to the behavior of the hypothalamic-pi-tuitary-gonadal axis, understanding their role is fundamental. OBJECTIVE. Describe the alterations of sex hormones in the critically ill patient from a physiological and clinical approach.MATERIALS AND METHODS.Observational, literature review and systematic analysis of 84 scientific articles and sample selection of 27 in MedLine, The Cochrane Library Plus, LILACS and Web of Science; in Spanish and English and variables: gonadal steroid hormones, critical illness, endocrinology, stress, gonads and dysfunction, period 1998-2017. CONCLUSION. The alterations detected were a mechanism for the production of steroid hormones towards the predominant syn-thesis of cortisol and withstand the high metabolic stress of the patients. Pro inflam-matory cytokines were important in these changes. Polypharmacy was an additional unweighted factor of sexual endocrine disruption.

ABSTRACT

El embarazo y el parto constituyen hechos trascendentales en la vida de la mujer; son momentos de alta carga sentimental y emocional, que implican múltiples cambios biológicos en la madre. Estos constituyen procesos que pueden estar marcados por diferentes factores de riesgo que están presentes muchas veces en la etapa preconcepcional y que se exacerban entre las embarazadas adolescentes. Al respecto se presenta una revisión bibliográfica que recoge información teórica, epidemiológica y sobre algunas formas para prevenir los respectivos factores predisponentes.


Pregnancy and childbirth are transcendental events in the life of women. They are moments of high sentimental and emotional charge, which imply multiple biological changes in the mother. These ones constitute processes that can be marked by different risk factors that are present many times in the preconceptional stage and that are exacerbated among pregnant adolescents. In this regard, a bibliographic review is presented, which gathers theoretical and epidemiologi-cal information and some ways to prevent the respective predisposing factors.

ABSTRACT

Evidences shows that menopause affects women's health, but women's knowledge of proper care and maintenance is insufficient. To determine knowledge of hormone therapy [HT], reproductive physiology, and menopause in a population of 40-60 year old women. This cross-sectional study was conducted through a cluster sampling among 330 women in Yazd, Islamic Republic of Iran, in 2010. Data was collected using a questionnaire containing questions about reproductive physiology related to menopause and HT by interviewing. Inferential and descriptive statistics via SPSS.15 software were used for data analysis. Overall, 2.1% of women were current takers of HT, 13.4% had taken it in the past but had stopped and 84.5% had never taken hormone replacement therapy. Iranian women had low knowledge of HT, reproductive physiology, and menopause. Most of the women [85.5%] knew that hot flashes are common around menopause and only 77.2% knew decreasing estrogen production causes the menopause. They knew little about the effects of progestagens and the effects of HT on fertility. Logistic regression determined that age, educational level and BMI were the most important factors predicting use of HT after adjusting for other variables. Iranian women have a low HT usage rate and the majority of them are lacking of the knowledge about HT and menopause. Women need improved knowledge of the risks and benefits of HT as well as education about the reproductive system around menopause

Andropause is an emerging clinical concept that is gaining an increasing recognition; as the world becomes more aging. The clinical features though subtle are easy to identify and appropriate treatment of diagnosed cases will to a good extent alleviate a lot of age-related complaints and improve the general quality of life in the elderly men. Low level of clinical suspicion secondary to inadequate knowledge about this clinical entity remains the major obstacle to appropriate treatment. In this review; the literature has been perused and the definition; epidemiology; pathophysiology; clinical features; diagnosis and treatment are outlined

ABSTRACT

In Abedus ovatus Stal, the female reprodutive system consists of a pair of ovaries leading to oviduct and opens out by genital aperture. Each ovary is composed of five telotrophic ovarioles, with four different regions, terminal filament, germarium, vitellarium and pedicel. Germarium and vitellarium are the germ tubes in which the development of oocyte occurs. In the germarium, the differentiation of trophocytes into young oocytes was observed in three zones, Z I, Z II and Z III. Further development of oocytes in the vitellarium could be divided into seven stages, ST I-ST VII. The development eggs are lodged in the pedicel.

ABSTRACT

Este artículo es una revisión resumida de la bibliografía en torno a la depresión en el climaterio. El climaterio se considera como la fase de transición entre la etapa reproduciva y la no reproductiva de la mujer, la cual se presenta, aproximadamente, a partir de los 35 años de edad. El climaterio se caracteriza por síntomas vasomotores (bochornos) y por síntomas psicológicos (depresión), entre otros. Esta sintomatología se deriva de dos componentes principales: la disminución de estrógenos y los factores psico-socio-culturales. A pesar de que se han considerado otras hormonas como la TSH, la prolactina, la LH, la FSH y la GnRH, la hormona que tiene más peso como factor etiológico es el estrógeno. Este estudio está enfocado a los estrógenos y a su fisiología en la conducta, a su mecanismo de acción y a su relación con las beta-endorfinas. También trata brevemente de la progesterona, la FH, la LH, la GnRH, la TSH, la TRH y la prolactina. Se encontró que la fisiopatología de la depresión en la mujer climatérica se debe, principalmente a la deficiencia estrogénica, y que los estrógenos no sólo son la etiología de la depresión en el climaterio, sino que también participan en la depresión que ocurre en muchas otras etapas reproductivas y no reproductivas de la mujer (i.e menstruación, postparto, histerectomía con ooforectomía bilateral, etc.), lo que sugiere que el cuadro depresivo que se presenta durante el climaterio podría deberse a las fluctuaciones de los niveles séricos de los estrógenos
