RESUMEN

Purpose@#With Korea’s recent rapid change into a multicultural society, cultural competence is being emphasized as a core nursing competency. This study investigated the effects of a cultural nursing course that aimed to enhance the cultural competence of nursing students in Korea. @*Methods@#This was a single-group pre- and post-comparison study. The subjects were 69 nursing students at Dongyang University who attended a cultural nursing course in 2015, of whom 62 students responded to the survey. The 13-week cultural nursing course was held for 2 hours a week. The methods of the course included small group activities, discussions and presentations, experiential learning, reflective activities, and lectures. Nursing students’ cultural competence was measured pre- and post-course with the Cultural Competence Scale for Korean Nurses, which contains 33 items scored on a 7-point Likert scale. @*Results@#After completing the cultural nursing course, students’ total cultural competence scores increased, as did their scores in each category (cultural awareness, cultural knowledge, cultural sensitivity, and cultural skills) (P<0.001). There was no significant difference in cultural competence by gender (P<0.001). @*Conclusion@#This cultural nursing course was found to be effective in enhancing the cultural competence of nursing students. Therefore, the educational program developed in this study can be extended to other university-level nursing programs in Korea.

BACKGROUND: Health illiteracy is a problem often unrecognized by health care providers. It influences medical costs and the health status of adults. The purposes of this study were to determine the level of health literacy in community-dwelling adults and to identify the factors influencing it. METHODS: A cross-sectional survey was conducted in Seoul, Gyeonggi and Chungcheong province. A total of 420 adults aged 18 or older were interviewed by trained nursing students between November 1 to December 30, 2011. Health literacy was measured using the Short Test of Functional Health Literacy in Adults. Data were analyzed using the PASW 18.0 program. RESULTS: The mean score of health literacy was 50.64+/-19.18. In the multiple linear regression analysis, health literacy was significantly associated with education (beta=0.17, P=0.001), alcohol use (beta=-0.12, P=0.010), and perceived health status (beta=0.11, P=0.029). These factors accounted for about 7% of health literacy. CONCLUSIONS: Health literacy is a very important public health issue. Our findings showed that educational level, alcohol use and perceived health status should be considered when assessing this issue in patients. Furthermore, the development of a standardized Korean assessment tool for health literacy and specified interventions for enhancing health literacy are needed to improve health outcomes.

RESUMEN

PURPOSE: This study was conducted to develop a respiratory nursing program using recorder playing for patients with asthma and to determine the effects of the program. METHOD: Participants were 26 patients with asthma (experimental group: 13, control group: 13). The experimental group participated in the respiratory nursing program for six months from April, to October, 2007. Content included playing a recorder, group education about asthma, and educational messages including cellular phone messages. The control group received usual care during the six months. RESULTS: In this research the respiratory nursing program using recorders was developed in 3 stages: initial stage, advanced stage, and wrap up stage. Compared to the control group, fatigue status in the experimental group improved significantly as well as level of forced expiratory volume in 1 second. However significant differences were found in sleep status, mood, or quality of life. CONCLUSIONS: These results suggest that this respiratory nursing program for symptom management of patients with asthma led to improvements in self care activity. Nursing interventions are needed to maintain and further enhance the quality of life of these patients and the interventions should be implemented over a transition period.

PURPOSE: This study was to investigate the reference accuracy in major nursing journals in Korea. METHODS: The references in articles from eight nursing journals from 2006 were compared with PubMed for authors, year, title, journal, volume, and page accuracy. Four hundred sixty-six references were reviewed. Errors were classified as major or minor and categorized by bibliographic headings (author, title, journal, year, volume and page). RESULTS: Of the 466 references, 223(47.9%) had citation errors. The reference error rates ranged from 28.6% to 58.7%. Most errors occurred in the author element (37.9%), followed by title (20.9%), journal (19.0%), page (13.9%), volume (5.9%), and year (2.4%). CONCLUSION: This study identified a considerable error rate in the references of nursing journals. Inaccuracy of references is a reflection on scholarly work of authors and journals. Authors and Editorial committees are responsible for the accuracy of references.

PURPOSE: The purpose of this study was to identify the need for advanced practice nurse and explain the relationship between self-efficacy and professional self-concept among Korean nurses. METHOD: The participants were 511 Korean nurses who were recruited in hospitals or public health centers in Korea. The instruments used in this study were Sherer et al's self-efficacy instrument, Arthur's professional self-concept instrument, and a structured questionnaire on the need for advanced nurse practitioners. Cronbach alpha, mean, SD, X2, t-tests, ANOVA, Pearson correlation coefficient were used to analyze the data using SPSS PC+ ver 12.0 for Windows. RESULTS: Of the nurses, 57.9% of wanted to be advanced practice nurses. The mean of professional self-concept and self-efficacy scores for the nurses were at the medium level. Professional self-concept and self-efficacy of nurses showed significant differences according to age, working location, marital status, level of education, and length of clinical experience. A significant positive correlation was found between professional self-concept and self-efficacy. CONCLUSION: Before planning to develop the role of Advanced practice nurse, there is a need to measure self-efficacy and professional self-concept among nurses. Politically, strategies to increase self-efficacy and professional self-concept among Korean nurses will be necessary in order to prepare good advanced practice nurse.

RESUMEN

PURPOSE: This study was to determine the current trend of nursing research as exploring both quantitative and qualitative methodologies, and to provide the explicit direction to improve the quality of published papers. METHODS: Total of 366 articles published between 2004 and 2006 was reviewed using the criteria of analysis. RESULTS: There was more number of quantitative studies than qualitative studies. More studies were conducted with subjects who had health problems, and studies that targeted women and elderly population have been significantly increased. In quantitative methodology, utilization of experimental and quasi-experimental designs has been increased, however descriptive study was dominant as yet. In qualitative methodology, studies using grounded theory and phenomenology were frequently published. It was noted that theoretical framework and rational for sample size were rarely presented in quantitative study. Philosophical position and the process of preparation for study, which guided the research, were not clearly described in qualitative study. CONCLUSION: The findings of this review suggest that published studies have been improved and diversified, however, detailed and clear evaluation tool that assesses study process and method should be developed as a way to further improve the quality of published papers.

PURPOSE: This study was performed to identify current characteristics of the Journal of Korean Academy of Nursing and to explore a way to elevate it to an international level and to critique the overall review process so as to delineate the advanced, objective paper appraisal in this journal. METHODS: Data was collected using self administered questionnaires to 75 journal reviewers belonging to the Korean academy of nursing and its division academy of nursing from August 15th to September 30th, 2006. RESULTS: The majority of reviewers pointed out a lack of discrimination between the Journal of Korean Academy of Nursing and other journals. Among the main answers of reviewers, Creativeness(52.3%) and excellence of nursing(38%) will be critical factors to develop in order to elevate to an the journal to an international level. In specific evaluation areas, reviewers preferred a subjective critique method(60%), and the condition of the decision making process regarding paper acceptance as a combination of checklist and subjective evaluation(84%). Subjective evaluation opinions with major categories will occur in the next revised evaluation format. 76% of reviewers agreed with the current objective evaluation form. CONCLUSIONS: The journal review process should be evaluated on a regular basis to elevate the journal level and a mutual agreement of the journal's scope, range, and purpose will be necessary. As a recommendation, an attempt at various approaches in journal reviews and reviewer training should be made.

RESUMEN

PURPOSE: This study was done to provide information about issues of clinical competency from the perspective of new graduate nurses and to make suggestions for improving undergraduate education programmes. It was also done to clarify learning experiences between fundamentals of nursing and the other major areas of nursing. METHOD: For this study, 7 new graduates and 8 experienced nurses participated. The data were collected through in-depth interviews and analysed in terms of Strauss and Corbin's grounded theory methodology. RESULT: Core category and main categories, were delineated. Clinical nursing competency was found to be a dynamic process with each participant actively engaged in acquiring problem solving ability in diverse clinical settings. These findings have value in understanding the embedded meaning of clinical nursing competency. CONCLUSION: Therefore, the educational programs reflecting the experience of new nursing staff should be developed.

RESUMEN

PURPOSE: The purpose of this study was to identify nurses' perceptions of the importance of concrete items as a basis for nursing practice. A total of 179 concrete items for basic nursing, which clinical nurses must know in order to practice appropriately, were identified. METHOD: The participants in this study were 225 nurses who had worked for two to five years in university hospitals. The nurses were asked to rate the importance of the 179 items (in 39 middle range categories) on a 5 point scale. RESULT: The mean age of the nurses was 26 and about 54% were university graduates. Of the 39 middle range categories those rated as the top ten were transfusion (4.682), IV injection (4.492), po medication (4.476), surgery (4.469), infection control (4.438), IM injection (4.413), safety (4.388), oxygenation (4.376), diagnostic test (4.366), and fluid & electrolyte balance (4.359). The categories that had the lowest scores were sexuality (3.449), conceptual nursing (3.465), spirituality (3.527) and personal hygiene (3.548). CONCLUSION: The results of this study show that most nurses evaluate items which they use frequently in their practice as important.

RESUMEN

PURPOSE: This study was designed to investigate the survey of analyzing nursing interventions and nursing outcomes of the patients with liver transplantation. METHOD: The subjects of this study consisted of 83 patients and fifteen nurses who took care of them. The methods of this study were retrospective and descriptive survey. RESULT: The mean age of patients was 42.7 years and the subjects were 62.7% males. 56.7% of these patients were diagnosed with liver cirrhosis of B type. The mean duration of hospitalization was 48.6 days. Ninety-five nursing interventions were performed at least daily. The most frequent used interventions were "environmental management: comfort", "medication administration: parenteral", "cough enhancement", and " oxygen therapy". Even though SGOT level to identify nursing outcomes was decreased, SGOT was not within normal limits. Therefore, even after discharge of patients, There is a need to take care of them carefully. CONCLUSION: Conclusion: These findings revealed the significance and need of nurse practitioners who performed professional nursing intervention for the patients with liver transplantation. Especially, it is necessary needs to develop the nursing intervention programs for comfort.

PURPOSE: The purpose of this study is to compare spiritual well-being and perceived health status between Korean and Korean-American and to provide basic data that might develop comprehensive health care program including spiritual dimension. METHOD: The subject of the study were 411 adults chosen from religious organization located in Gyeongsang Province, Korea and Chicago, U.S.A.. The instruments used in the study were Spiritual Well-being Scale by Paloutizian and Ellison and Health Self Rating Scale by North Illinois University. Analysis of data was done by using descriptive statistics, Pearson correlation coefficient, ANCOVA, ANOVA and Duncan test with SPSS program. RESULT: 1) The mean score of spiritual well-being of Korean was 3.17 and Korean-American was 2.63, there was significant difference between two groups. 2) Perceived health status of Korean was 2.37 and perceived health status of Korean-American was 2.54, there was significant difference between two groups. CONCLUSION: According to this study, it is important to take into considerations spiritual aspects and cultural and environmental elements in developing the comprehensive health care program.

PURPOSE: The purpose of this study was to assess social support, mood, and commitment to a plan for exercise of Korean adults with chronic diseases. Further, the relationships between social support, mood, and commitment to a plan for exercise were assessed. METHOD: The participants in this study were 230 Korean adults over 18 years of age with chronic diseases who were recruited in hospitals or health centers. The instruments used in this study were Exercise Social Support Scale, POMS (Profile of Mood States), and Commitment to a Plan for Exercise Scale. RESULT: The results of descriptive analysis showed that Korean adults with chronic diseases were perceived with relatively low commitment to a plan for exercise while they were perceived with moderate social support and mood states. Pearson's correlation coefficient showed significant relationship between these three variables. Stepwise regression indicated that 37% of the variance in commitment to a plan for exercise was explained by social support and vigor factor of the POMS. CONCLUSION: Health professionals can use this information for planning a targeted strategy to increase regular exercise among Korean adults with chronic diseases in order to improve their quality of life and control disease-related symptoms.

PURPOSE: This study was to identify and clarify the relationship between perceived health, self-esteem, and self-care agencies for promoting self care among Korean adolescents. METHOD: Data were collected from 817 adolescents in schools located in Seoul, Kyungki-do, and Chuncheon from Sept, 16th to Sept, 28th, 1999, and from Mar 10th to Mar 25th, 2000. The instruments used for this study were the Health Self-Rating Scale, Self-Care Agency Assessment Questionnaire (Denyes, 1981), the Self-Esteem Questionnaire (Rosenberg, 1971). RESULT: 1) The mean perceived health status among Korean adolescents was 8.75 (SD=1.72) 2) The mean self-esteem was 27.27 (SD=4.64). 3) The mean self-care agency was 99.64 (SD=21.02) and the average self-care agency score was 3.99 (SD=0.84). In the subcategories, the highest degree was feelings towards health (4.15), followed by ego strength (4.06), attending to health (3.87), general health knowledge (3.56), and the lowest degree was specific health knowledge (3.20) 4) There was statistical significant differences between demographic factors and self-care agencies, expecially, gender (t=28.65), grade (F=3.79), pocket money (t=5.72), and height (F=9.82) 5) The statistical relationship between perceived health status, self-esteem, and self-care agencies were found to have a positive correlation. 6) Self-care agencies among adolescents was the highest factor predicting self-esteem (15%). CONCLUSION: The relationship between perceived health status, self-esteem, and self-care agencies revealed a significant positive correlation among adolescents. Therefore, nursing intervention for adolescents needs to develop self-esteem programs to increase self-care agencies.

In Korea, the number of older persons has been increasing annually. The proportion of older person aged 65 and over in Korea was 6.6% in 1998 and will reach 12% in 2020. Also, the older persons who are at greates risk for chronic illness with functional dependency is growing the fastest. One of the main diseases of elderly is dementia. Caring for the elderly, in particular elderly mentally ill patients has become one of the biggest social concerns in Korean society today. The caregiver who looks after a disabled or demented elderly person is usually a woman who is either a spouse of daughter in law in Korea. Caregivef s stress leads to dysphoria and in vulnerable individuals, to depression. Hence, family caregivers of people with dementia frequently experience significant emotional, physical and financial stress. So, there is a need to develop, for the Korean family caregiver, home-based nursing interventions using computer-assisted instructions made available either on CD ROM or over the internet. The purpose of this study was to develop and to examine the effect of a caring program for the family caregivers among the Korean demented elderly. The family caregiver nursing intervention program was developed using a team approach involving nursing professors and computer professionals. The application is available on CD ROM as well as on the World Wide Web. The CD ROM program is made up of five sections: 1) definition and symptoms of dementia to support basic medical and nursing knowledge including psychological problems of the demented elderly, 2) methods for coping with the demented elderly including video techniques, 3) physical care and ADL management for the demented elderly including vido techniques, 4) stress management methods for the family caregiver including music therapy, exercise therapy, oriental acupucture technique on the hand, foot massage and paper handicraft, 5) social referral information service. To examine the effect of a CD ROM based caring program, the study was implemented by nonequivalent control group pretest-posttest design. The subjects of this study were 52 family caregivers who were composed of 22 caregivers for the experimental group and of 30 caregivers for the control group. In this study, the measuring instruments were family caregivers nursing care need assessment tool, fatigue tool for family caregiver' s physical health state, OARS for family caregiver' s psychiatric heath state and family burden scale for family caregiver' s psychological health state. The results of the analysis were as follows: 1. There was no significant difference between experimental group and control group on pretest. So two group had homogeneity. 2. There was significant difference between experimental group and control group on posttest measuring nursing care need state(t=8.55, p<0.01)and family caregivef s psychological health state(t=2.58, p<0.05). Therefore, this CD ROM based caring program was revealed the positive effect for family caregivers among the Korean demented elderly. For further research, with distributing the CD ROM, it suggests that the effective caring program to decrease family caregivef s fatigue and psychological health state as like anxiety, stress, strain will be developed.

The ratio of the elderly population is rapidly growing due to socio-economic development and better medical services. As the average life-span of the Korean elderly becomes longer, the proportion of the elderly is increasing. Therefore, the elderly need to sustain a high quality of life. However, all these factors, such as sensory loss, decreased endurance, mobility limitation, and a loss of independence, act to diminish the quality of life among the elderly. It is necessary for elders to adapt themselves to aging, to maintain their physical functions and especially, it is important to keep their independence in activity of daily living and self care. Therefore, this study was designed to identify the degree of activity in daily living and exercise of a self-care agency and to investigate the relationship between activity of daily living and self-care agency. Also, this research aimed to contribute to nursing interventions that maintain and promote quality of life of the elderly and to enable themselves to live independently. The subject of this study were 160 elders, over age 65, who were sampled in C city. Data were collected by observation and questionnaires from May,1999 to December, 1999. The measurement tools were Barthel Index(1979), Lawton & Brody(1969) for activity of daily living and Kearney & Fleischer(1979) for self-care agency. Data were analyzed by SAS PC+ package and included descriptive analysis, t-test, ANOVA, Pearson correlation coefficient, and Cronbach's alpha. The conclusions of this study are summarized as follows; 1. The mean of ADL and IADL was 96.09 and 16.38, respectively. The score described activity of daily living among the elders in C province as moderate. Particularly, in the items of IADL, the item of medication was the highest score. 2. The mean for exercise of self-care agency was 76.48, a moderate degree. 3. The degree of ADL and IADL according to demographic characteristics was significantly different statistically in pocket money, the presence of disease, perceived health status and in occupation, pocket money, family type, the presence of disease, and perceived health status, respectively. 4. The dagree of selt-care agency according to demographic characteristics was significantly different statistically in gender, education level, economic status, and pocket money. 5. There was a significant positive correlation between activity in daily living and self-care agency. Based on these results, further research is suggested to discover the variables influencing the activity of daily living and self-care agency to develop nursing strategies for improving self-care and activity of daily living among the elderly.

The purpose of this study was to assess the present status of the course content of Fundamental Nursing in accordance with the study objectives which were developed for the National Qualifying Examination by the first Faculty-Workshop in January, 1999 and the study objectives as related to other Nursing courses. The study sample included all schools of nursing and the members of the faculty for Fundamental Nursing of seventy schools(66.7%), out of the target population of 105, responded to a questionnaire which was developed by the research team. The collected data were analyzed by frequencies, percentages, means, and standard deviation using the SAS(Statistical Analysis System). The results are as follows: 1) According to this survey the course content of Fundamental Nursing as described in the study objectives for the National Examination, 223(86.77%) study objectives were included in the course of Fundamental Nursing by more than 70% of the schools of nursing. But twenty-three of the study objectives are not included by 30~50% of the schools of nursing. They consider this content to be taught in other courses. The study objectives, which less than 50% of the schools reported as being included in their course in Fundamental Nursing were, all study objectives for "nursing assessment(communication)", "nursing assessment (physical examination)", "record and report", "sex", and 'use of computer program for nursing diagnosis' and 'use of computerized nursing planning program'. 2) Items that were not included in the study objectives for Fundamental Nursing but were included in course content for over 50% of the schools included, 'health of individual, family, community', 'change of nursing practice', 'professionality of nursing' and all of the study objectives belonging to "loss and grief" and "nursing during all stages of surgical operations". We hope that these results will be helpful in modifying the study objectives for the National Examination as developed by the first Faculty-Workshop and recommend the need for continuous survey research to produce content that is in accordance with study objectives and a National Examination that reflects the content of Fundamental Nursing.

RESUMEN

Massage therapy is a traditional, alternative and nonphamacological means of promoting rest and relaxation. However, nursing intervention by massage for middle-aged women is rarely practiced by nurses. The purpose of this research was to examine the effects of the hand and arm massage as an independent nursing intervention tool for middle- aged women. The data used in this research were collected from forty-nine subjects using a nonequivalent control group non- synchronized design. Twenty-four persons for the experimental group and Twenty-five persons for the control group were selected from D city and C city from July 1997 to September 2000. Subjects' ages were between forty and fifty-six years old with mean the age of 45.6. Hand and arm massage developed by Cayce and Reilly was applied to the experimental group for a session of 15 minutes two or three times a week for four weeks. The instruments used for the measurement of the subjects' stress, anxiety, depression and the middle-life crisis were Langners's 22-item Screening Score, Spielberger's State Anxiety Inventory, Zung's Self-rating Depression Scale, and Kim's Middle Life Crisis Scale(1988). These psychological factors were measured before and after the implementation of hand and arm massage. The data were analyzed with mean+/-s.d, percent, t-test, and a paired t-test. The results were summarized as follows; 1. Before the treatment, there were no significant differences between the two groups. 2. After the treatment, there were significant differences in the stress and the occurrence of mid life crisis between the two groups. The findings suggest that the use of the hand and arm massage for middle-aged women made significant changes in the level of stress and middle life crisis. Therefore, it is recommended that hand and arm massage be used as an independent nursing intervention tool for middle-aged women. For further research, is needed replication of this concept of research with different subjects in a larger population. Also, it is recommended to investigate the effects of massage with aroma therapy for the berefit of decreasing womens' stress level further.

The purpose of this study was to assess the perceived exercise self-efficacy and exercise benefits/barriers of Korean adults with chronic diseases, and the relationship between the two variables. For the study, 249 Korean adults with chronic diseases with ages ranging from 18 to 79 years were recruited from hospitals or health centers in five Korean cities and surrounding rural areas. The research instruments were the scales that researchers psychometrically verified the Exercise Self-Efficacy Scale, developed by Bandura (1997), and the Exercise Benefits/ Barriers Scale, developed by Sechrist, Walker, and Pender(1987). Results of descriptive analysis showed that Korean adults with chronic diseases perceived relatively low exercise self-efficacy and relatively high exercise benefits/ barriers. Exercise self-efficacy was significantly correlated with gender, education, regular exercise, and exercise benefits/barriers was significantly correlated with gender, regular exercise. Pearson correlation coefficient showed the significant relationship between the two variables. Further researches, which are a study to evaluate a causal structure for Pender's Health Promotion Model and an intervention study to increase physical activity of chronic patients, are recommended.

The disease patterns among the Korean was shifted from acute and infectious diseases to chronic diseases. According to the these disease patterns trends, people have concerned about the health promotion and health behaviors. Pender's(1996) revised health promotion model(HPM) is consist of three categories; Individual characteristics and experiences, Behavior-specific cognitions and affect, behavioral outcome. Of these categories, individual characteristics and experiences, this category of variables is considered to be of biological, psychological and socio-cultural personal factors, especially, individual fatigue. Futhermore, these variables constitute a critical core for nursing intervention, as they are subject to modification through nursing actions. But there is no few the research of the relationship between the fatigue and health promotion. Therefore, the purpose of this study is to investigate the correlation between the fatigue and health promoting life style among a rural college students. Additionally, this descriptive correlational study identified the relation of demographic factors and fatigue, health promoting life style. From June 20 to 26, 1998, a convenience sample of 270 college students completed the questionnaire of the fatigue and health promoting life style profile which were developed by the Yoshitake(1978) and Walker, et al.(1987), respectively. The descriptive correlational statistics, mean, t-test, ANONA, Pearson correlation coefficient were used to analyze the data gathered with SAS pc+ program. The results were as it follows: 1. The average fatigue score of the subjects was 64.93+/-12.89. Fatigue scores by subcategory were physical symptoms(23.5+/-4.87), psychological symptoms(22.11+/-4.66) and neuro-sensory symptoms(19.32+/-5.14). With the respect to the demographic characteristics of the subjects, there were statistically significant differences between the demographic factors and fatigue, especially, sex(t==3.69 p0.05). But there was negative significant relationship between health promoting life style and psychological symptom as a fatigue subcategory(r=-0.15721, p<0.05). The self-actualization showed negative significant correlation with all fatigue subcategory. The health responsibility showed significant relationship with total fatigue(r=0.13050, p<0.05). For further research, it suggests to replicate the correlational and causal study between the fatigue and the health promoting life style using the another fatigue scale which is able to measure the subjective and objective fatigue degree. And it needs to develop the nursing intervention program for maintaining and promoting the health behavior as well as for decreasing the college students's fatigue.
