RESUMEN

Objective:To explore the effect of NOD-like receptor thermal protein 3 ( NLRP3) knockout in γ-aminobutyric acid (GABA)-ergic neurons in the hippocampal CA1 area on improving cognitive dysfunction in mice after traumatic brain injury (TBI). Methods:Forty-eight healthy male NLRP3 flox/flox mice weighing 25-28 g were randomly divided into 4 groups ( n=12): sham-operated+control virus group (SV group), sham-operated+ NLRP3 specific knockout group (SG group), TBI+control virus group (TV group), TBI+ NLRP3 specific knockout group (TG group). TBI in the TV and TG groups was established by free-fall method, while surgical procedures such as scalp incision and cranial window opening without impact were given to the SV and SG groups. Adenovirus was injected into the hippocampal CA1 area of SG and TG groups 21 d before TBI to induce NLRP3 specific knockout in GABA-ergic neurons in the hippocampal CA1 area; empty virus was injected into the CA1 area of SV and TV groups. Cognitive function was evaluated using novel object recognition test 30 and 31 d after TBI, and learning and memory functions were assessed using Morris water maze test 32-36 d after TBI. Field potentials in the hippocampal CA1 area were recorded during novel object recognition 31 d after TBI. After behavioral tests, these mice were sacrificed. Immunofluorescent staining was used to detect the fluorescent intensity of microtubule-associated protein2 (MAP2), glutamic acid decarboxylase 67 (GAD67), and postsynaptic density protein 95 (PSD95) in the hippocampal CA1 area, as well as percentage of pyroptosis-associated inflammatory factor interleukin-18 (IL-18)/GAD67 double-positive neurons in total GAD67 positive neurons. Results:Compared with the SV and SG groups, the TV and TG groups had decreased novel object recognition index, decreased number of platform crossings during the experimental period, increased escape latency on day 3 and day 4 of the training period in Morris water maze test, decreased θ and γ oscillation power in the hippocampal CA1 area during novel object recognition, decreased fluorescent intensity of MAP2, GAD67, and PSD95 in the hippocampal CA1 area, increased percentage of IL-18/GAD67 double-positive neurons, with significant differences ( P<0.05). Compared with the TV group, the TG group had increased novel object recognition index, increased number of platform crossings in Morris water maze test, decreased escape latency during the training period, increased θ and γ oscillation power in the hippocampal CA1 area during novel object recognition, increased fluorescence intensity of MAP2, GAD67, and PSD95 in the hippocampal CA1 area, decreased percentage of IL-18/GAD67 double-positive neurons, with significant differences ( P<0.05). Conclusion:Specific inhibition of NLRP3 expression in GABA-ergic neurons in the hippocampal CA1 area can improve cognitive dysfunction in mice after TBI, whose mechanism may be related to inhibited GABA-ergic neuronal pyroptosis in the hippocampal CA1 area.

ObjectiveBy investigating the awareness situation of scientific research integrity among scientific researchers, their behavior and attitudes towards scientific research integrity, and the reasons for the occurrence of scientific research integrity, to identify the key influencing factors and risk points, and propose targeted management ideas for scientific research integrity construction. MethodsA web-based questionnaire was used to survey the awareness of scientific research integrity policies, behavior and attitude, and reasons for dishonest behavior in scientific research among scientific researchers at a tertiary A hospital. ResultsA total of 1,385 questionnaires were recovered, with 1,253 valid questionnaires. There were 246 males and 1,007 females, aged between 20 and 60 years old, with an average age of (37.98±8.97) years old. The average awareness rate of policies related to scientific research integrity was 39.44%. Scientific researchers believe that the top three risk points for dishonest behavior in scientific research were paper submission, entrusting third-party services, and the use of research funds. According to the different scientific research work involved by different survey subjects, the respondent population was set up with the attributes of the personnel in the basic information of the survey content on attitudes toward violating scientific research behavior norms, and was divided into four groups. The awareness situation of scientific research integrity policies was negatively correlated with the attitudes towards dishonest behavior in scientific research in the first and third groups. Publication of SCI papers and the awareness situation of scientific research integrity policies were negatively correlated with attitudes towards dishonest behavior in scientific research in the second group. Age and publication of SCI papers were related to attitudes towards dishonest behavior in scientific research in the fourth group, which were negatively correlated with the publication of SCI papers and positively correlated with age. ConclusionThe learning and training, and the rate of policy awareness of scientific research integrity were not high, and the awareness of scientific research integrity policies should be further improved. There are still risks and problems in the key links of scientific research activities, and it is necessary to strengthen the integrity management of the whole process of scientific research activities, establish the correct scientific values of scientific researchers, improve the scientific research evaluation system, and create a good scientific research environment.

RESUMEN

The NLRP3 inflammasome's core and most specific protein, NLRP3, has a variety of functions in inflammation-driven diseases. Costunolide (COS) is the major active ingredient of the traditional Chinese medicinal herb Saussurea lappa and has anti-inflammatory activity, but the principal mechanism and molecular target of COS remain unclear. Here, we show that COS covalently binds to cysteine 598 in NACHT domain of NLRP3, altering the ATPase activity and assembly of NLRP3 inflammasome. We declare COS's great anti-inflammasome efficacy in macrophages and disease models of gouty arthritis and ulcerative colitis via inhibiting NLRP3 inflammasome activation. We also reveal that the α-methylene-γ-butyrolactone motif in sesquiterpene lactone is the certain active group in inhibiting NLRP3 activation. Taken together, NLRP3 is identified as a direct target of COS for its anti-inflammasome activity. COS, especially the α-methylene-γ-butyrolactone motif in COS structure, might be used to design and produce novel NLRP3 inhibitors as a lead compound.

Objective:To investigate the effect of aucubin on behaviors and excessive activation of astrocytic in attention deficit/hyperactivity disorder (ADHD) model mice.Methods:Twelve wild-type C57BL/6 pregnant mice (female, clean grade) were intraperitoneally administered with esketamine (15 mg/kg) to establish an ADHD model in offspring mice. The offspring mice were divided into control+ saline group, control+ aucubin group, Ketamine+ saline group and Ketamine+ aucubin group according to the nest matching principle with 15 in each group.At 14 days after birth, mice in the control+ aucubin group and Ketamine+ aucubin group were administered with aucubin (5 mg/kg, once a day) by gavage for 5 days. Mice in control+ saline group and Ketamine+ saline group were administered with equal volume of 0.9% sodium chloride solution. The offspring mice were housed with their mothers in the same cage until 21 days after birth. Twenty-one days after birth, the offspring mice were evaluated by open field test and elevated plus maze tests. Immunofluorescence assay was used to detect the expression of glutamate decarboxylase 2 (GAD2), γ- aminobutyric acid (GABA) and glial fibrillary acidic protein (GFAP) in the amygdala. The morphological changes of astrocytes were quantitatively analyzed by Sholl analysis. GraphPad Prim 9.0.1 software was used for statistical analysis. The comparison of multiple groups was conducted by one-way ANOVA or Kruskal-Wallis test.Results:(1)The results of behavioral experiments showed that the total distance traveled in the open field test and the residence time in open arm of the elevated plus maze were statistically significant ( F=236.90, H=39.92, both P<0.001). The total distance ((7 044±249)mm, (22 891±2 175)mm, P<0.05) and the residence time in open arm(12.69(9.86, 17.24)s, 2.72(0.57, 3.87)s, P<0.05) of mice in Ketamine+ saline group were both higher than those in control+ saline group.The total distance((22 891±2 175)mm, (8 252±839)mm, P<0.05) and the the residence time in open arm(5.45(1.13, 10.99)s, 12.69(9.86, 17.24)s, P<0.05) of Ketamine+ aucubin group were both lower than those of Ketamine+ saline group.(2)The immunofluorescence results showed that the levels of GAD2, GABA and GFAP intensity in amygdala of mice in the four groups were statistically significant ( F=145.50, 50.08, 53.83, all P<0.05). Compared with control+ saline group, the fluorescence intensities of GAD2 ((100.00±9.60)%, (24.86±4.14)%, P<0.05) and GABA ((100.00±16.84))%, (25.48±5.70)%, P<0.05) of Ketamine+ saline group were down-regulated, and the GFAP((100.00±18.02)%, (223.80±25.85)%, P<0.05) was up-regulated. Compared with Ketamine+ saline group, the fluorescence intensities of GAD2 ((24.86±4.14)%, (56.08±6.55)%, P<0.05) and GABA((25.48±5.70)%, (52.59±15.74)%, P<0.05) in Ketamine+ aucubin group were up-regulated, but the fluorescence intensity of GFAP ((223.80±25.85)%, (157.10±22.10)%, P<0.05) was down-regulated.(3)Sholl analysis indicated that the number of the intersections between the astrocyte processes or the branches of astrocyte processes was statistically significant in the 4 groups ( F=12.47, P<0.05). Compared with control+ saline group, the number of the intersections in Ketamine+ saline group((2.07±0.48), (1.67±0.72), P<0.05) increased. While the number of the intersections in Ketamine+ aucubin group was lower than that of Ketamine+ saline group ((1.20±0.78), (2.07±0.48), P<0.05). Conclusion:Aucubin administration can alleviate ADHD-like behaviors in offspring mice, and the mechanism may be associated with the inhibition of excessive astrocytic activation.

Objective:To analyze the patent development status of 13 general hospitals in Beijing, for reference in promoting the management and translation of intellectual property rights of public hospitals.Methods:13 tertiary general hospitals in Beijing as one of the top 100 hospitals in China in terms of science and technology evaluaton metrics in 2020 were selected as the research subjects. The patent application and authorization information of 13 hospitals was retrieved from the IncoPat technology innovation intelligence platform, during the period from January 1, 2012 to December 31, 2021. The patent application and authorization status, international patent classification distribution, inventor information, patent validity, and registration of patent transfer were analyzed.Results:The numbers of patent applications and authorizations from 13 hospitals increased from 123 and 13 in 2012 to 2 053 and 2 368 in 2021, respectively. From 2012 to 2021, 13 hospital applied for 10 050 patents and authorized 6 896 patents, including 906 authorized invention patents, 5 863 utility model patents, and 127 appearance design patents. Valid patents amounted to 5 925, accounting for 85.92%, and valid authorized invention patents amounted to 796, accounting for 13.43%. 110 of these patents were transferred or licensed, accounting for 1.60% of the authorized patents.Conclusions:Aapplied and authorizated patents were rising year by year, but the ratios of invention patents and patient translation remained at a low level. The author suggestted that hospitals should highlight the value orientation of patents, strengthen-industry-academia-research cooperation, establish a professional management team and standardized management system for intellectual property, cultivate high-value patents, conduct pre patent application evaluation and screening, and improve patent conversion and technological innovation capabilities.

Objective:To investigate the influencing factors of the scientific research performance and analyze the differences between subjective and objective ones among the different characteristics of staffs in work and then propose tailored management strategies based on the research findings.Methods:A total number of 963 young researchers under the age of 45 (including doctors, medical technicians, scientific researchers) were surveyed and the impact on the scientific research performance of four subjective and objective factors including demographic characteristics and research motivation, self-efficacy, team support and the external environment were analyzed through self-developed questionnaire.Results:The academic degree, age, time spent overseas, administrative position, postgraduate mentor, professional title and levels, category of departments on the scores of scientific research performance had reached statistical significance ( P <0.05) by using Single factor analysis. The results showed that the impact on the scientific research performance followed a descending order of the category of postgraduate mentors, academic degree, professional title levels, the category of departments, self-efficacy, time spent overseas, and professional title series. Conclusions:According to the survey, this research focuses on the predictive factors which affecting scientific research performance as the important methods of scientific research promotion and talent training, and puts forward to the ideas of talent training, scheme of optimizing talent selection, carrying out accurate policy implementation and scientific assessment.

Objective To make good use of the incentive function of the reward policy of scientific research achievements in hospitals ,shorten the reward cycle of scientific and technological achievements ,reduce the management cost ,achieve the ul-timate goal of details management of scientific and technological achievements .Methods Through retrospective analysis of the incentive process to identify possible barriers ,reconstruct the rewarding procedures of scientific and technological achievements by bringing in informatization and publicity strategy ,compare the bonus distribution cycle with the traditional model .Results The results show that the reward model based on the scientific research management system reduces the bonus arrival waiting period by about 62% ,saves management costs and improves staff compliance .Conclusions According to the 4-year practice experiences ,the new management model has further released the positive impact of the hospital award policy ,solved the key problem of restricting the rewarding effect by technological means in the process of rewarding scientific research achievements .

Objective To provide authors with more quality services,so as to improve management efficiency,reduce management time and manpower cost,ultimately achieve the expected delicacy management of scientific papers.Methods Indicators of traditional management pattern and "double track parallel" management mode of the scientific papers management process were compared through retrospective study.Such indicators include authors' operation,data error rate,average working cycle,management cost,user experience,data barriers,self-management consciousness and others.Resuits Significant improvements were shown in the data accuracy,management cost savings,user satisfaction,and data sharing in the "double track parallel" management model.Conclusions 5 years of practice demonstrated that the "double track parallel" management model not only effectively connects hospital policy and technical method,but also solves the key problems of data barriers,disjointed management and low efficiency.Scientific paper management is expected to be more refined and effective.

RESUMEN

Objective:To evaluate the clinical application of vancomycin in a cancer hospital in order to promote the rational use of vancomycin. Methods:The records of 66 patients treated with vancomycin in 2015 were reviewed and analyzed in the respects of in-dications of medication, pathogenic examination, usage and dosage, course of treatment, drug combination, drug monitoring and drug utilization index(DUI). Results:Among the 66 cases, most drug use was rational, which accounted for 69. 70% with DUI of 0. 82. There were still several types of irrational drug use including no medication indications, irrational usage and dosage, too long or too short course of treatment, irrational combination and prophylactic use time. Conclusion:The application of vancomycin is basically ra-tional, while irrational phenomenon still exists. The management and supervision should be further strengthened in order to promote clinical rational drug use and ensure medication safety of patients.

Objective To exPlore the theraPeutic effect of cycloVirobuxine_D ( CVB_D) on heart failure by obserVing the influences of CVB_D on cardiac function,myocardial morPhology and structure in rats with heart failure. Methods The rat model of heart failure was established by ligating left anterior descending coronary artery,and then the rats were randomly diVided into 5 grouPs,Plus the sham grouP. EchocardiograPhy was used to assess cardiac function,including EF,LVSF,SV,HR,CO and so on. Myocardial tissue section was obtained. Change in the myocardial tissue was obserVed by HE staining. Myocardial ultra_structure was obserVed by using transmission electron microscoPe. Results As comPared with the model grouP,EF and LVSF were significantly increased in medium and high dose CVB_D grouPs (P<0. 05 or P<0. 01),SV was increased in low and medium dose CVB_D grouPs (P<0. 05),CO was increased in low,medium and high dose CVB_D grouPs (P<0. 05),LVIDs was decreased and LVPWs was increased in high dose CVB_D grouP (P<0. 05). Under light microscoPe,changes of myocardial structures were obserVed in different CVB_D dose grouPs: myocardial fiber gradually became tidy, striPes became clear, and contour became clear. The changes of myocardial ultra_structure were obserVed under transmission electron microscoPe: the myocardial fiber breakage was reduced and the number of mitochondria was increased,swelling degeneration was alleViated. Conclusion CVB_D significantly imProVed cardiac contraction and blood PumPing function of rats with heart failure, exerted mitigatiVe effect on damage of cardiac muscle fiber and mitochondria. It suggests that CVB_D may be effectiVe in the treatment of heart failure.

Objective To determine the median effective dose (ED50) of 0.375 % ropivacaine for ultrasound-guided subgluteus approach to the sciatic nerve block in the elderly patients.Methods Thirty ASA Ⅰ-Ⅲ patients of both sexes,aged 65-85 yr,scheduled for foot or ankle surgery under the sciatic nerve block,were enrolled in this study.The patients were placed in the Sim's position and received the sciatic nerve block using the posterior subgluteus approach guided by ultrasound.The initial dose of 0.375% ropivacaine was 14.5 ml in the first patient and the ratio between the 2 successive doses was 1.2.The ED50 and 95 % confidence interval of ropivacaine were calculated by up-and-down technique.Results The ED50 and 95% confidence interval of 0.375% ropivacaine resulting in complete block of the sciatic nerve by using the ultrasound-guided subgluteus approach were 13.4 and 11.5-16.2 ml,respectively.Conclusion The ED50 of 0.375 % ropivacaine for ultrasound-guided subgluteus approach to the sciatic nerve block is 13.4 ml in the elderly patients.

Objective To identify major factors which affect the completion quality of projects and build predictive models,and to propose strategies and suggestions for total process quality management of clinical research projects of the hospital.Methods Collect relevant literatures from home and abroad;consult completion reports of various research projects completed from 2003 to 2010 and sum up problems found in such projects.Carry out in-depth interviews with principal investigators,supervisors of scientific research of the hospital as well as administrators of sponsor institutions.A questionnaire survey was made on completed clinical research projects undertaken from 2003 to 2010.Results Six key factors affecting implementation of such projects were derived from factor analysis,which are research technology and external environment,project teamwork,research subject availability,project organization,manpower input,and research design.A multiple logistic regression analysis found project internal management and manpower input as two leading predictive factors for project completion outcomes,while the former has a greater impact than the latter.Conclusion Project management should focus on the 6 key factors affecting project quality.The two main predictive factors should attract greater attention and resources in both supervision and management.

In recent years,the state has a substantial increase in investment in Medical Research.The number of hospital-borne scientific research,funding amounts and types of projects is also increasing..Our hospital scientific management based oriented clinical needs,Construction Institute hospital as a work positioning,the whole process of quality management as the implementation of safeguards.Through a series of positive measures,gradually formed which are consistent with the management of the hospital research and development,and scientific research achievements into clinical practice.The research management changed from passive management model to proactively manage; from the emergency management to the whole process of managing; from the targeted management to guide the management of clinical needs.These measures effectively improve the level of scientific research in hospitals.

The scientific research fund is an important resource for carrying out the scientific re-search projects effectively. Whether it can be reasonable used not only relates to the fulfilled quality, bene-fit, using direction and quantity, but also makes the reformation of management methods inevitable with the increase of quantities of the projects and funds. By summarizing the previously practical experiences, this paper explored the network managements of scientific research funds based on workflow in the hospital. By means of computer and network, the concept of workflow was further introduced to the application of platform of scientific research funds management. It embodies the charactor of approval flow in workflow management system. Finally the data of scientific research funds can be shared and integrated into multi-departments, so it is beneficial to improve the fund's benefit and strengthens the supervision in the whole process manage-ment.

This paper reviewed and analyzed the 72 fulfilled projects in the discipline of Otorhinolar-yngology supported in division Ⅱ of basic research of clinical medicine by the National Natural Science Foun-dation of China (NSFC) from year 2002 to 2006, and gave a brief introduction to the part of research a-chievements. The science foundation promotes Otorhinolaryngology to a quick development, and produces some outstanding research achievements with international recognition. Thus, it plays an important role in el-evating the level of discipline and bringing up specialists. According to the retrospection and analysis of the fulfilled projects and their research achievements, we hope it can objective and overall reflect research pro-gress in the field of Otorhinolaryngology, and offer as a reference to the relevant researchers and scientific re-search managers.
