ABSTRACT Milestones throughout history marked the path to reach peritoneal dialysis (PD) as a treatment for advanced chronic kidney disease (CKD). The Egyptian embalmers, Galen (2nd century) and Vesalius (16th century) provided knowledge about the anatomy of the peritoneum. It was not until 1628 that Asellius Gaselli described the abdominal lymphatic capillaries. The 19th century was rich in advances: the cell was identified as the unit of living beings and the phenomenon of osmosis (Dutrochet, 1828), crystalloids and colloids and their passage or not through a membrane (Graham T, 1850), the flow of solutes and particles through the peritoneal membrane (Recklinghausen, 1863), the absorption of hypotonic substances and the increase in effluent with hypertonic ones (Wegner G, 1877), and animal experiments confirmed that fluid removal and other substances occurred primarily through blood vessels (Starling & Tubby, 1894). But it was not until the 20th century that PD was applied as treatment. The first attempt to use the peritoneum to treat uremia was made by Georg Ganter in 1923, first in animals with ureteral ligation and then in two patients. It was not until 1937 that the first case that survived a peritoneal "lavage" was published (Wear et al), but it was Fine, Frank and Seligman who initially in nephrectomized dogs and later in patients with acute kidney injury (ARI) demonstrated that the method was not only viable, but also succesful. Then progress continued, especially for patients with ARI, but also in some cases with advanced CKD: the double hanging bottle (Maxwell M, 1959), chronic intrahospital dialysis with a cycler (Tenckoff et al, 1965), plastic bags for PD, until 1975 when Moncrief et al launched continuous ambulatory PD, and in 1981 automated PD was introduced. The 1990s saw the expansion of PD, to date installed as one of treatment alternatives for advanced CKD.

RESUMEN

Adrenocortical carcinoma (ACC) with inferior vena cava thrombosis is rare and has a poor prognosis, and the current literature overwhelmingly supports aggressive surgical intervention. This article summarizes the management of a patient with ACC with inferior vena cava thrombosis, and discusses the feasibility of detailed preoperative imaging data and intraoperative ultrasound to assess the superior and inferior boundaries of ACC with inferior vena cava thrombosis, while describing the intraoperative ultrasound-guided surgical planning and procedure for ACC with retrohepatic inferior vena cava tumor thrombus. Furthermore, it also demonstrates that it is feasible to accurately assess the superior and inferior boundaries of ACC with inferior vena cava thrombosis by preoperative multimodal imaging and intraoperative ultrasound, determine the mode of flow blockage during the operation, and obtain radical resection of the tumor.

RESUMEN

Preeclampsia and intrauterine growth restriction (IUGR) of the fetus are the two most common pregnancy complications worldwide, affecting 5%-10% of pregnant women. Preeclampsia is associated with significantly increased maternal and fetal morbidity and mortality. Hypoxia-induced uteroplacental dysfunction is now recognized as a key pathological factor in preeclampsia and IUGR. Reduced oxygen supply (hypoxia) disrupts mitochondrial and endoplasmic reticulum (ER) function. Hypoxia has been shown to alter mitochondrial reactive oxygen species (ROS) homeostasis and induce ER stress. Hypoxia during pregnancy is associated with excessive production of ROS in the placenta, leading to oxidative stress. Oxidative stress occurs in a number of human diseases, including high blood pressure during pregnancy. Studies have shown that uterine placental tissue/cells in preeclampsia and IUGR show high levels of oxidative stress, which plays an important role in the pathogenesis of both the complications. This review summarizes the role of hypoxia-induced mitochondrial oxidative stress and ER stress in the pathogenesis of preeclampsia/IUGR and discusses the potential therapeutic strategies targeting oxidative stress to treat both the pregnancy complications.

RESUMEN

@#
Telemedicine has been an emerging method for delivering healthcare services due to the challenges brought by the COVID-19 pandemic. This descriptive quantitative correlational study is aimed at assessing the knowledge, attitudes and perceptions (KAP), and their relationship among adults aged 18-34 without prior experience in the use of telemedicine in Manila, Philippines during the COVID-19 pandemic. The study had 322 eligible respondents who answered an online survey questionnaire of three parts that asked for their socio-demographic profile, KAP on telemedicine. Descriptive statistics, profile analysis and Spearman's rho were utilized as statistical tools. The respondents' knowledge was on an average to low level, and attitude and perception towards telemedicine were both neutral. Results showed no significant difference between the KAP of the respondents when analyzed according to socio-demographic variables. Knowledge and attitude have a weak positive linear relationship while knowledge and perception have a moderate positive linear relationship. A strong positive linear relationship was indicated between attitude and perception. A positive attitude can be attributed to a positive perception towards telemedicine but both do not consequently come from high knowledge levels of it.

RESUMEN

We investigated the effect of supplementing fertilization medium and/or culture medium with astaxanthin (AST) on the two phases of in vitro fertilization: gamete fertilization and embryo development. Methods: Mouse cumulus-oocyte complexes were divided into four groups with 5 µM AST added to the fertilization medium (group 3, n=300), culture medium (group 2, n=300), or both media (group 4, n=290). No AST was added to the control group (group 1, n=300). Results: The fertilization rate was significantly higher (p<0.001) in the groups using AST supplemented fertilization medium (group 3, 79.0%; group 4, 81.4%) than those without AST (group 1, 56.3%; group 2, 52.3%). The blastocyst rate calculated from the two-cell stage was significantly lower (p<0.001) in the groups using AST-supplemented embryo culture medium (group 2, 58.0%; group 4, 62.3%) than in those without AST (group 1, 82.8%; group 3, 79.8%). The blastocyst rate calculated from the number of inseminated oocytes was highest in group 3 (189/300, 63.0%) and lowest in group 2 (91/300, 30.3%) with statistical significance compared to other groups (p<0.001). There were significantly higher numbers of cells in the inner cell mass and trophectoderm, as well as significantly higher total blastocyst cell counts, in group 3 than in the control group. Conclusion: An increased blastocyst formation rate and high-quality blastocysts were found only in the fertilization medium that had been supplemented with AST. In contrast, AST supplementation of the embryo culture medium was found to impair embryo development.

RESUMEN

Objective To investigate the estimation of early and mid-term wound age by a combination of four mRNAs, the DNA polymerase delta-interacting protein 3 (POLDIP3) mRNA, regulator of chromosome condensation 1 like (RCC1L) mRNA, proline-rich 5 (PRR5) mRNA, and ribonucleic acid export 1 (RAE1) mRNA in rats skeletal muscles. Methods The model of rat skeletal muscle contusion was established, and then contusion area muscle tissue was extracted 4, 8, 12, 16, 20, 24, 28, 32, 36, 40, 44 and 48 h after injury. Histomorphological changes during the repair process after rat skeletal muscle contusion were observed. The relative expressions of Poldip3, Rcc1l, Prr5 and Rae1 mRNAs were detected by reverse transcription real-time quantitative polymerase chain reaction (RT-qPCR). Different stages of wound age were classified by using the expression patterns of four genes at various time points after injury. The accuracy of the results was verified by Fisher discriminant analysis. Results Histomorphological results showed that the repair process after skeletal muscle contusion occurred with the prolonging of time. Through combination of the expression trends of the four kinds of mRNAs, the 48 h after injury could be divided into three periods, 4-12 h, 16-28 h and 32-48 h. The Fisher discrimination method showed that the classification accuracy rates of the three periods were 83.3%, 75.0% and 73.3%, respectively. Conclusion The classification discrimination based on the relative expression of every gene has a higher accuracy, and the accuracy of wound age estimation with combination of mRNA relative expressions is higher than that with a single indicator. By combining with Fisher discrimination method, this method can be used for early and mid-term wound age estimation.

RESUMEN

@#Interleukin-33(IL-33) is a new member of the interleukin-1 (IL-1) cytokine superfamily. It can activate mast cells, lymphocytes and macrophages to produce Th2 cytokines and plays a very important role in inflammation, infection, and autoimmune disease. The classical signal pathway of IL-33 includes the isotrimer of ST2 and interleukin-1 receptor accessory protein (IL-1 RAcP), which transduces signals into cells. The IL-33/ST2 signaling pathway affects bone metabolism by activating T and B lymphocytes. This article reviews the role of the IL-33/ST2 signaling pathway in bone metabolism. The results of a literature review showed that at present, scholars at home and abroad still dispute the role of IL-33 in bone metabolism. Some scholars believe that IL-33 can inhibit osteoclast formation, and IL-33 has been recently implicated in physiological bone remodeling. However, other scholars believe that IL-33 can promote osteoclast formation and differentiation, which leads to bone absorption. IL-33 and its signaling pathway are involved in bone metabolism of alveolar bone in periodontitis and periapical periodontitis. The specific mechanism remains unclear, and further studies are warranted.

RESUMEN

PURPOSE: This study examined the relationship between caffeine intake and metabolic syndrome in Korean adults using the 2013 ~ 2016 Korea National Health and Nutrition Examination Survey data (KNHANES). METHODS: The caffeine database (DB) developed by Food and Drug Safety Assessment Agency in 2014 was used to estimate the caffeine consumption. The food and beverage consumption of the 24 hr recall data of 2013 ~ 2016 KNHANES were matched to items in the caffeine DB and the daily caffeine intakes of the individuals were calculated. The sample was limited to non-pregnant healthy adults aged 19 years and older, who were not taking any medication for disease treatment. RESULTS: The average daily caffeine intake was 41.97 mg, and the daily intake of caffeine of 97% of the participants was from coffee, teas, soft drinks, and other beverages. Multivariate analysis showed that the caffeine intake did not affect metabolic syndrome, hypertension, low HDL-cholesterol, and abdominal obesity. Diabetes and hypertriglyceridemia, however, were 0.76 (95% CI: 0.63 ~ 0.93), and 0.87 (95% CI: 0.77 ~ 0.98) in third quintile (Q3), and 0.66 (95% CI: 0.53 ~ 0.82) and 0.83 (95% CI: 0.73 ~ 0.94) in fourth quintile (Q4) compared to Q1, respectively. Therefore, caffeine intake of 3.66 ~ 45.81 mg per day is related to a lower risk of diabetes and hypertriglyceridemia. CONCLUSION: The study showed that adequate caffeine intake (approximately 45 mg) was associated with a lower prevalence of diabetes and hypertriglyceridemia. Therefore, it can be used as a guideline for the adequate level of caffeine intake for maintaining health.

RESUMEN

OBJECTIVES@#To explore the homogeneity level of four different functional mRNA (PUM2, TAB2, Cx45 and CHRNA1) expressions in rats with skeletal muscle contusion.@*METHODS@#The relative expressions of PUM2, TAB2, Cx45 and CHRNA1 mRNAs were detected by quantitative real-time reverse transcription-polymerase chain reaction (RT-qPCR). The coefficient of variation (CV) of the relative expressions for different individuals in each injury group was calculated. The extreme value of CV, cumulative variability, and CVCV were compared.@*RESULTS@#A high CV of PUM2 and TAB2 mRNAs appeared on several different time points. However, the CV of Cx45 and CHRNA1 mRNAs was relatively low. The cumulative variability from high to low was PUM2, CHRNA1, TAB2 and Cx45 mRNAs. The relative expression of PUM2 mRNA was significantly higher than that of TAB2, Cx45 and CHRNA1 mRNAs ( P<0.05). There was no statistical significance (P>0.05) in the CVCV of the relative expression of TAB2, CHRNA1 and Cx45 mRNAs.@*CONCLUSIONS@#As the mRNAs involving in biological process regulation, PUM2 and CHRNA1 mRNAs show a lowest individual homogeneity of the relative expression followed by TAB2 mRNA. As the mRNAs participating in the composition of cellular structure, Cx45 and CHRNA1 mRNAs show a high individual homogeneity of the relative expressions. The functional classification should be considered for the screening of the mRNA indicators used for wound age estimation.

Except that PIP5K9 and PIP5K11 were unstable proteins, the rest were stable proteins, and the corresponding proteins of PIP5Ks have no transmembrane domain, and all were non-transmembrane proteins. Subcellular localization results showed that the proteins PIP5K1, PIP5K4, PIP5K6, PIP5K9, and PIP5K11 were less likely to have a lead peptide, and there was no corresponding amino acid cleavage site, ie, localized in other organelles or possibly cytoplasmic proteins; Protein PIP5K2 was localized in chloroplasts, the chloroplast transit peptide has a higher value, and the protein PIP5K8 was located in the secretory pathway. From the perspective of the relative expression of PIP5Ks in the differential expression, except that no fluorescence was detected in PIP5K4, the other PIP5Ks had a certain differential expression. The changes of relative expression of PIP5Ks showed similar trends, the levels of PIP5K1, PIP5K2 relative expression were high, followed by the relative expression of PIP5K8, PIP5K9, and PIP5K6 and PIP5K11 only have fewer expressions. Conclusion It is concluded that PIP5Ks may have a certain effect on the inflorescence axis traits in ramie. The differential expression of PIP5K on the inflorescence axis has the certain regularity with the development of inflorescence.
