RESUMEN

BackgroundRehabilitation for schizophrenia typically relies on pharmacological interventions, yet their efficacy in improving social function and quality of life remains limited. In recent years, non-pharmacological approaches have shown promise in enhancing rehabilitation outcomes. However, research on the effectiveness of psychomotor therapy specifically for young and middle-aged schizophrenic inpatients is limited. ObjectiveTo explore the effects of psychomotor therapy on the rehabilitation of young and middle-aged schizophrenic inpatients, and to provide a reference for treatment strategies. MethodsA total of 104 young and middle-aged schizophrenic inpatients who met the International Classification of Diseases, tenth edition (ICD-10) diagnostic criteria and hospitalized in the Fourth People's Hospital of Wuhu from June 2021 to June 2022 were selected. Patients were randomly divided into two groups of 52 each using random number table method. Both groups received treatment with risperidone tablets (2~4 mg/d) , along with routine nursing care. Additionally, the research group received an extra 45~55 minutes of psychomotor therapy 2~3 times per week for 12 weeks. The control group received the same psychomotor therapy after the study. Positive and Negative Symptom Scale (PANSS), Scales of Social-skills for Psychiatric Inpatient (SSPI) and Insight and Treatment Attitude Questionnaire (ITAQ) were used to assess the patients before the intervention and at 4th, 8th and 12th week after the intervention. ResultsThe main effects of intervention at different time points for PANSS positive symptoms, negative symptoms and general psychopathology subscale scores, PANSS total score, SSPI score and ITAQ score were all statistically significant (F=33.989, 204.245, 82.817, 279.596, 26.144, 7.463, P<0.01). Furthermore, statistically significant between-group differences were observed in PANSS negative symptoms and general psychopathology subscale scores, PANSS total score, SSPI score and ITAQ score (F=30.053, 5.306, 33.417, 33.013, 18.608, P<0.05 or 0.01). Moreover, the interaction effect of time and group were statistically significant for PANSS positive symptoms, negative symptoms and general psychopathology subscale score, PANSS total score and SSPI score (F=3.472, 9.798, 3.843, 14.390, 20.661, P<0.05 or 0.01). After 12 weeks of intervention, the research group exhibited statistically significantly lower PANSS total score and subscale scores compared with baseline (P<0.01), while their SSPI total score was significantly higher than that of control group(P<0.01). Additionally, compared with the control group, the research group had statistically significantly lower PANSS total score and subscale scores , while their SSPI score was statistically significantly higher than those of control group(P<0.01). ConclusionPsychomotor therapy may contribute to the improvement of the psychiatric symptoms and social function in young and middle-aged inpatients with schizophrenia, enhancing their rehabilitation outcomes. RESUMEN

Objective@#To prospectively evaluate the outcomes of ultrasound (US)-guided radiofrequency ablation (RFA) in tertiary hyperparathyroidism (THPT). @*Materials and Methods@#Patients with THPT underwent RFA between September 2017 and January 2022. Laboratory parameters, including serum intact parathyroid hormone (iPTH) levels, were monitored for 48 months after RFA and compared with the levels at baseline. Complications related to RFA and changes in hyperparathyroidism-related clinical symptoms were recorded before and after RFA. @*Results@#A total of 42 patients with THPT were recruited for this study. Ultimately, 36 patients with renal failure and 2 patients who underwent successful renal transplantation (male:female, 17:21; median age, 54.5 years) were enrolled. The follow-up time was 21.5 ± 19.0 months in the 36 patients with renal failure. In these 36 patients, iPTH levels were significantly decreased to 261.1 pg/mL at 48 months compared with the baseline value of 1284.9 pg/mL (P = 0.012). Persistent hyperparathyroidism, defined as iPTH levels maintained at > 585.0 pg/mL for 6 months after treatment, occurred in 4.0% of patients (1/25). Recurrent hyperparathyroidism, defined as iPTH levels > 585.0 pg/mL after 6 months, were 4.0% (1/25) and 0.0% (0/9) at 6 months and 4 years after treatment, respectively. In two patients with THPT after successful renal transplantation, iPTH decreased from the baseline value of 242.5 and 115.9 pg/mL to 171.0 and 62.0 pg/mL at 6 months after treatment. All complications resolved within 6 months of ablation without medical intervention, except in 10.5% (4/38) patients with permanent hypocalcemia. The overall symptom recovery rate was 58.8% (10/17). The severity scores for bone pain, arthralgia, and itchy skin associated with hyperparathyroidism improved after treatment (P < 0.05). @*Conclusion@#US-guided RFA is an effective and safe alternative to surgery in the treatment of patients with TPTH and improves hyperparathyroidism-related clinical symptoms. Keywords: Ultrasound; Radiofrequency ablation; Te

RESUMEN

OBJECTIVE To provide reference for safe drug use in clinic by mining the adverse drug events (ADE) of 3 kinds of anti-influenza A virus drugs (oseltamivir, zanamivir, baloxavir marboxil). METHODS The ADE data of oseltamivir, zanamivir and baloxavir marboxil were collected from the FDA adverse event reporting system (FAERS) between the first quarter in 2004 and the third quarter in 2022, and mined by using reporting odds ratio (ROR) method. The designated medical events (DME) were estimated. The system organ class (SOC) in the Medical Dictionary for Regulatory Activities (MedDRA, version 25.0) was used for the classification and statistics of drug ADE terminology. RESULTS A total of 12 636, 1 749 and 1 283 ADE reports were retrieved for oseltamivir, zanamivir and baloxavir marboxil, involving 26, 16 and 17 SOCs, respectively. Oseltamivir was strongly associated with sleep terror, abnormal behavior, hallucination and delirium. Zanamivir was implicated in abnormal behavior, delirium, incoherence, and altered state of consciousness with prominent signal intensity. Baloxavir marboxil was strongly associated with ischemic colitis, hemorrhagic cystitis, erythema multiforme and melaena. Erythema multiform was detected in the DME of three drugs with strong signals. CONCLUSIONS When clinically administering the three drugs, it is crucial to pay close attention to both common adverse reactions and those ADEs that are not explicitly mentioned in the drug instructions. For oseltamivir, clinicians should exercise caution due to the potential risk of acute kidney injury and fulminant hepatitis, necessitating regular monitoring of the patient’s liver and kidney function. When prescribing zanamivir, caution should be exercised due to ADEs related to the respiratory system, including acute respiratory distress syndrome and respiratory failure, necessitating close monitoring of the patient’s respiratory status. Similarly, for baloxavir marboxil, clinicians should be vigilant for potential ADEs such as erythema multiforme and rhabdomyolysis.

RESUMEN

ObjectiveTo investigate whether the effects of paeonol (Pae) on angiotensin Ⅱ (AngⅡ)-induced senescence in vascular smooth muscle cells (VSMCs) were related to angiotensinogen of silencing regulatory information factor 6 (SIRT6)/adenosine diphosphate ribose polymerase 1 (PARP1) signaling pathway in VSMCs. MethodThe model of VSMC-stress aging induced by AngⅡ (100 nmol·L-1) was established. The rats were divided into normal group, model group, low, medium, and high-concentration Pae groups (30, 60, 120 μmol·L-1). The positive rate of cell senescence was detected by SA-β-Gal staining, the ability of cell proliferation was detected by the cell counting kit-8 (CCK-8) method, the expression of SIRT6, PARP1, p16, p21, p53, proliferating cell nuclear antigen (PCNA), deoxyribonucleic acid (DNA)-damaged protein γ-H2AX was detected by Western blot, and VSMC proliferation was detected by EdU staining. The silenced VSMCs were prepared by siRNA-SIRT6 transfection, and the protein expressions of SIRT6, PARP1, p16, and γ-H2AX in VSMCs silenced by SIRT6 were observed. ResultThe results of SA-β-Gal staining showed that the senescence positive rate of SA-β-Gal staining in the model group was higher than that in the normal group (P<0.01), and the positive rate of SA-β-Gal staining in the Pae group was significantly lower than that in the model group (P<0.05, P<0.01). The results of Western blot showed that as compared with the normal group, the expression of PCNA, SIRT6, and PARP1 in the model group was down-regulated, and the expression of aging-related proteins p16, p21, p53, and γ-H2AX was up-regulated in the model group (P<0.05, P<0.01). Compared with the model group, Pae promoted the protein expression of PCNA, SIRT6, and PARP1 and inhibited the protein expression of p16, p21, p53, and γ-H2AX in a dose-dependent manner (P<0.05, P<0.01). The results of EdU staining showed that the number of EdU positive cells in the model group was lower than that in the normal group (P<0.01), and the number of EdU positive cells in Pae groups was significantly higher than that in the model group (P<0.05, P<0.01). After SIRT6 silencing, the effects of Pae on promoting SIRT6 and PARP1 and inhibiting P16 were reversed (P<0.05, P<0.01). In addition, the addition of SIRT6 inhibitor (IN-1) promoted the occurrence of cell senescence induced by AngⅡ (P<0.05, P<0.01). ConclusionPae can effectively inhibit the aging of VSMCs, and its mechanism may be related to the regulation of SIRT6/PARP1 signal pathway.

Objective:To investigate the clinical features and prognosis of patients with choroidal tuberculoma. Methods:A retrospective and observational study. From 2011 to 2022, 15 patients (16 eyes) with choroidal tuberculoma diagnosed and treated in Department of Ophthalmology, Shanghai Eye & ENT Hospital of Fudan University were included. The patients underwent examinations including best corrected visual acuity (BCVA), fundus color photography, optical coherence tomography (OCT), fluorescein fundus angiography (FFA), and indocyanine green angiography (ICGA). Chest CT examination, purified protein derivative test and interferon-γ release test were also performed. BCVA was performed using the Snellen visual acuity chart, which was converted to Logarithm of the minimum angle of resolution (logMAR) visual acuity when recorded. All the patients received antitubercular therapy and the duration was 12-15 months. Thirteen patients were combined with oral corticosteroids. The average follow-up time was 36 months. Rank sum test was used to compare logMAR BCVA before and after treatment. Results:Among the 15 patients, 5 were male and 10 were female; 1 case was bilateral involvement; 7 patients had a strong positive tuberculin skin test; 8 patients had a positive interferon-γ release assay. Six patients had pulmonary tuberculosis. One patient had peritoneal tuberculosis. Lesions were located in the posterior pole in 12 eyes, 1 eye with papillary tuberculoma. Peripheral lesions were found in 4 eyes. There were 9 eyes each with anterior segment inflammation or vitreous inflammation; 7 eyes were with exudative retinal detachment. OCT examination revealed a hyporeflective thickening of the choroidal stroma, the corresponding elevation of the retina, and often accompanied by subretinal fluid. FFA revealed hyperfluorescence of the mass and fluorescence accumulation with subretinal fluid at the late stage. ICGA revealed hypofuorescence of the lesion. The lesions subsided after treatment, and there was no recurrence during follow-up period. Cataract surgery was performed in 4 eyes due to complicated cataract. One eye undergone vitrectomy due to secondary rhegmatogenous retinal detachment. Anti-vascular endothelial growth factor therapy was performed in 1 eye due to secondary choroidal neovascularization. Ocular wall perforation occurred in 1 eye, and the condition was stable after treatment. Before treatment, the average logMAR BCVA was 1.02±0.57, which improved to 0.31±0.35 after treatment. The difference between the mean logMAR BCVA before and after treatment was statistically significant ( P<0.05). Conclusions:Combination of medical history, clinical manifestations, laboratory tests, and imaging can assist in the diagnosis of choroidal tuberculoma. The main manifestations are subretinal yellowish-white lesions in posterior pole. Standardized anti-tuberculosis therapy can effectively improve the prognosis of vision.

RESUMEN

Liver sinusoidal endothelial cells (LSECs) locate on the surface of hepatic sinusoids. As the first line of defense between the liver and blood, LSECs are the most abundant non-parenchymal cells in the liver. Under physiological conditions, LSECs may induce liver immune tolerance through participating in substance transport and metabolic waste removal, thereby maintaining liver homeostasis, and under pathological conditions, LSECs may promote liver immune response via antigen presentation. LSECs have been found to play a crucial regulatory role in maintaining the balance between liver regeneration and liver fibrosis. This article reviews the progress of researches on LSECs functions, LSECs changes in liver injury, signal pathways associated with regulation of LSECs functions, and the interaction between LSECs and other types of cells in the liver, aiming to elucidate the function of LSECs and their roles in liver diseases.

RESUMEN

OBJECTIVE To provide a reference for safe drug use in clinic. METHODS ADE reports related to nilotinib from the first quarter of 2007 to the fourth quarter of 2022 were collected from the US FDA adverse event reporting system database. The reporting odds ratio (ROR) and proportional reporting ratio (PRR) of disproportionality measures were used to mine potential ADE signals, which were compared with drug instruction and related case report, and were screened and analyzed according to the designated medical events (DME) list formulated by the European Medicines Agency. RESULTS Totally 23 332 cases of ADE with nilotinib as the primary suspected drug were reported. A total of 359 positive signals were obtained, involving 24 system organ classes (SOC),mainly concentrated in various examinations, heart organ diseases, vascular and lymphatic diseases, all kinds of nervous system diseases, etc. Among them,ADEs such as vertebral artery stenosis, coronary artery stenosis, arterial disease, liver infection and the second primary malignant tumor were not mentioned in the instructions. Seven DMEs were detected,of which bone marrow failure, pulmonary hypertension and deafness were not mentioned in the drug instruction. CONCLUSIONS The common ADE signals of nilotinib excavated in this study are consistent with the instructions. In clinical use, special attention should be paid to DME not mentioned in the instructions such as bone marrow failure, pulmonary hypertension and deafness; cardiac function, blood glucose and blood lipid indexes should be monitored closely.

RESUMEN

Parkinson's disease (PD), known as one of the most universal neurodegenerative diseases, is a serious threat to the health of the elderly. The current treatment has been demonstrated to relieve symptoms, and the discovery of new small-molecule compounds has been regarded as a promising strategy. Of note, the homeostasis of the autolysosome pathway (ALP) is closely associated with PD, and impaired autophagy may cause the death of neurons and thereby accelerating the progress of PD. Thus, pharmacological targeting autophagy with small-molecule compounds has been drawn a rising attention so far. In this review, we focus on summarizing several autophagy-associated targets, such as AMPK, mTORC1, ULK1, IMPase, LRRK2, beclin-1, TFEB, GCase, ERR

Objective:To study the incidence of glucose-6-phosphate dehydrogenase (G6PD) deficiency and the gene carrying status of newborns in Guangxi Zhuang Autonomous Region (Guangxi for short), so as to provide theoretical basis for clinical genetic counseling and accurate diagnosis. Methods:A total of 63 606 newborns who underwent G6PD screening in Guangxi Neonatal Disease Screening Center from January 2017 to December 2018 were selected as study subjects; heel blood was collected to prepare dry blood spots. Fluorescence quantitative analysis was used in the preliminary screening, and the newborns with positive preliminary screening were recalled by telephone; further diagnosis was carried out via the G6PD/6-phosphogluconate dehydrogenase (6PGD) ratio method and genetic testing, the diagnosis rate of the two methods of newborns with positive preliminary screening were compared and analyzed, and genetic mutation testing was conducted. Results:Among 63 606 newborns who underwent G6PD preliminary screening, 4 267 newborns with G6PD positive were detected, and the positive rate of preliminary screening was 6.7%. Among them, the positive rates of preliminary screening of males and females were 10.3% (3 508/33 988) and 2.6% (759/29 618), respectively. The positive rate of preliminary screening of males was significantly higher than that of females ( P < 0.01). A comparative analysis of 777 newborns (519 males and 258 females) that underwent G6PD/6PGD ratio method and genetic testing at the same time as the recall showed that the diagnosis rate of the two methods for male newborns was the same, both of which were 95.6% (496/519). Among female newborns, 168 and 236 confirmed cases were detected by G6PD/6PGD ratio method and genetic testing, respectively, and the diagnosis rates were 65.1% (168/258) and 91.5% (236/258), respectively. The results of genetic mutation testing showed that the five common genotypes in Guangxi were c.1388 G>A, c.1376 G>T, c.95 A>G, c.871 G>A, and c.1024 C>T, respectively. Conclusions:The positive rate of G6PD preliminary screening of newborns in Guangxi is relatively high. It is recommended that G6PD/6PGD ratio method and genetic testing should be performed at the same time for diagnosis of female newborns with positive preliminary screening to avoid missed diagnosis and misdiagnosis.
