RESUMEN

Objective:To evaluate the effect of dexmedetomidine on the blood concentrations of ropivacaine during sciatic nerve block in rabbits.Methods:Twelve New Zealand rabbits of both sexes, weighing 2-3 kg, were randomly divided into ropivacaine group (R group) and ropivacaine mixed with dexmedetomidine group (RD group). The right femoral vein was cannulated for blood sampling in both groups, 0.375% ropivacaine 3 ml was injected around the left sciatic nerve in group R, and 0.375% ropivacaine 3 ml containing 1.5 μg/kg dexmedetomidine was injected instead in group RD.Blood samples from the right femoral vein were collected before nerve block (T 0) and at 15, 30, 45, 60, 120 and 180 min after nerve block (T 1-6) for determination of plasma concentrations of ropivacaine using high-performance liquid chromatography after centrifugation, and concentration-time curves were plotted. Results:Compared with group R, the blood concentrations of ropivacaine were significantly decreased at T 1-3 ( P<0.05), no significant change was found in the blood concentrations of ropivacaine at T 4-6 ( P>0.05), the peak blood concentration of ropivacaine was significantly decreased ( P<0.01), and no significant change was found in the time to peak blood concentrations of ropivacaine or area under the concentration-time curves in group RD ( P>0.05). Conclusions:Dexmedetomidine can decrease the blood concentrations of ropivacaine during sciatic nerve block in rabbits.

PD-L1 (programmed cell death 1 ligand 1 ) is an immunosuppressive ligand which mainly expressed on tumor cells, inhibiting the activation of T lymphocytes through binding with PD-1 (programmed cell death protein 1), thus leading to immune escape.PD-1/PD-L1 immune checkpoint blockade therapy, which established based on the above mechanism, gained success in the clinical treatment of solid tumors.Various kinds of PD-L1 posttranslational modifications were identified following the in-depth studies of PD-L1, including glycosylation, phosphorylation, ubiquitination, palmitoylation, et al.Meanwhile, multiple studies indicated that posttranslational modifications governs PD-LI-mediated immune escape through regulating the protein stability and physiological functions of PD-L1, which makes posttranslational modifications of PD-L1 turns into a new "entry" point of PD-L1 studies.Drugs targeting posttranslational modifications of PD-L1 exhibited favorable applicational prospects in immunotherapy at the same time.Regulating PD-L1-mediated immune escape through intervening PD-L1 posttranslational modifications becomes a new strategy for improving the efficacy of immunotherapy.In this review, we summarized the posttranslational modifications of PD-L1 and its applicational prospects in immunotherapy, hoping to provide theoretical supports for future studies focused on PD-L1.

RESUMEN

Sjögren syndrome (SS) is a chronic autoimmune disease characterized by immune cell infiltration and progressive destruction of salivary and lacrimal glands. It not only affects the lacrimal and salivary glands, manifested as dry eyes and dry mouth, but also involves heart, lung,kidney,and central nervous system, seriously affecting human physical and mental health. Although western medicine has made extensive and in-depth research on the diagnosis and treatment of this disease in recent years,there is no effective treatment targeting the potential causes. Chinese medicine emphasizes the concept of holism,treatment and prescription formulation based on syndrome differentiation, and effect exertion via multiple targets,multiple levels,and multiple pathways,exhibiting great advantages in the treatment of SS. This paper reviews the mechanisms of Chinese medicine in treating SS from the perspectives of immunity regulation,aquaporin up-regulation, and anti-oxidative stress reported in the related literature,so as to provide more theoretical basis for the research and clinical treatment of SS.

Objective:To analyze the effect of first aid knowledge and skills training for women in poverty-stricken areas, and summarize the importance of first aid knowledge and skills for women in poverty-stricken families.Methods:By cluster sampling, 318 women from poverty-stricken families who participated in the vocational training class in Zhen’an County, Shaanxi Province were trained in first aid knowledge and skills. The mastery of first aid knowledge before and after the training was evaluated by questionnaire survey, and the mastery of first aid skills before and after the training was evaluated by on-site assessment.Results:After the theoretical training of women from poor families, the awareness rate ranked in the top five were fire, poisoning, burns, heatstroke and earthquake first aid measures, with a knowledge rate of 98.1%(312/318),96.5%(307/318), 95.3%(303/318), 93.4%(297/318), 91.8%(292/318), which were statistically different from the pre-training rate, 88.0%(280/318), 84.0%(267/318), 78.4%(249/318), 43.4%(138/318), 72.6%(231/318), ( χ2 value was 15.03-183.89, all P<0.01). The pass rate of post-training cardiopulmonary resuscitation, hemostatic, dressing, and carrying skills of poor family women were 69.8% (222/318) , 52.8% (168/318) , 50.0% (159/318) , 76.1% (242/318) , respectively, which were statistically different from pre-training, 1.3%(4/318), 2.2%(7/318), 1.9%(6/318), 6.9%(22/318), ( χ2 value was 191.57-326.19, all P<0.01). Conclusion:The effect of training on first aid knowledge and skills of women in poverty-stricken families is significant, and a long-term training mechanism can be established.

Objective To investigate the expression changes of the hydrogen sulfide synthases cystathionineγ-lyase (CSE), cystathionineβ-synthase (CBS), and 3-mercaptopyruvate sulfurtransferase (3-MST), after optic nerve crush (ONC) in rat the retina.Methods The rat model of ONC was established.Rats were divided into normal control, ONC, and sham control groups.Histopathologic changes in retina, the number of retinal ganglion cells (RGC) and retinal thickness of inner part (RTIP) were measured.The changes of CSE, CBS and 3-MST mRNA expression were detected with quantitative real-time PCR.Results The retinal histostructure was normal in normal controls and with minor changes in sham controls, respectively.Compared with sham group, significant retina damages were found in the ONC group:a time-dependent reduction of RGC number and RTIP.Expressions of CSE, CBS and 3-MST mRNA in rat retina were detected in normal control.Compared with normal controls, the expressions of CSE, CBS and 3-MST mRNA did not show any significant changes in the sham controls.Compared with sham controls, CBS mRNA expressions showed a time-dependent increase at 3 d, 7 d and 14 d after crush in the ONC group;CSE mRNA expressions increased to the peak at 3 d and then slightly reduced at 14 d after crush;3-MST mRNA expressions showed the trend of increase at 3 d and 7 d and then enhanced remarkably at 14 d after crush.Conclusion Hydrogen sulfide synthases CSE, CBS and3-MST expressions were up-regulated in rat retina following ONC, which may cause an increase in local endogenous hydrogen sulfide production in the retina and a compensatory protective effect.

<p><b>OBJECTIVE</b>To study the clinical and laboratory features of macrophage activation syndrome (MAS) at the early stage of diagnosis, and to explore a method for early identification of MAS.</p><p><b>METHODS</b>A retrospective analysis was performed for the demographic data, clinical and laboratory features, and treatment outcomes of 21 MAS patients.</p><p><b>RESULTS</b>Of the 21 MAS patients, 14 had systemic juvenile idiopathic arthritis, 5 had Kawasaki disease (KD), and 2 had connective tissue disease (CTD) as primary diseases. The median time of MAS onset was 19 days. The KD patients had the shortest time of MAS onset, while the CTD patients had the longest onset time (P=0.009). The top 10 clinical symptoms were fever (95%), rash (86%), lymph node enlargement (67%), hemophagocytic phenomenon in bone marrow (63%), pulmonary disease (62%), serous effusion (62%), hepatomegaly (52%), cerebrospinal fluid abnormalities (50%), central nervous system damage (43%), and splenomegaly (38%). The median of hemoglobin level was lower than the normal value. The medians of C-reactive protein level and erythrocyte sedimentation rate were higher than the normal values. There were significant increases in serum ferritin, glutamic-pyruvic transaminase, aspartate aminotransferase, lactate dehydrogenase, and triglyceride. The median of fibrinogen level was lower than the normal value. There were significant increases in D-dimer, interleukin-6 (IL-6), interleukin-10 (IL-10), and interferon-γ (IFN-γ). Of the 21 patients, 20 were improved and discharged.</p><p><b>CONCLUSIONS</b>If patients with rheumatic disease have persistent fever, hepatic dysfunction, coagulation disorders, multiple organ impairment, significantly increased IL-10 and IFN-γ, and a persistent increase in serum ferritin, the development of MAS should be considered.</p>

<p><b>OBJECTIVE</b>25-Hydroxyvitamin D3 [25(OH)D3] is the main product of vitamin D and can reflect the absolute concentration of active vitamin D in the body. This study examined serum 25(OH)D3 levels in children with juvenile idiopathic arthritis (JIA) in order to explore the association of vitamin D concentrations with the pathogenesis and disease activity of JIA.</p><p><b>METHODS</b>Serum samples were collected from 53 children confirmed as having JIA between January 2013 and March 2014, as well as 106 healthy children (control group) who underwent physical examination in the same period. Serum concentrations of 25(OH)D3 were measured using ELISA and compared between the cases and healthy controls. The association of serum 25(OH)D3 levels with JIA subtypes, ACR Pediatric 30 Score, peripheral blood C-reactive protein (CRP), and erythrocyte sedimentation rate (ESR) were analyzed in children with JIA.</p><p><b>RESULTS</b>Compared with the control group, the JIA group had significantly reduced serum 25(OH)D3 levels (median: 42.6 nmol/L vs 49.9 nmol/L; P<0.01). The percentage of subjects with severe deficiency of vitamin D in the JIA group was significantly higher than that in the control group (17.0% vs 6.6%; P<0.05). Serum 25(OH)D3 showed no significant correlations with JIA subtypes, ACR Pediatric 30 Score, CRP, and ESR in children with JIA.</p><p><b>CONCLUSIONS</b>Vitamin D concentrations are significantly decreased in children with JIA. Decreased vitamin D concentrations may be associated with the pathogenesis of JIA. However, vitamin D concentrations may have no correlations with JIA subtypes, disease severity, and disease activity.</p>

<p><b>OBJECTIVE</b>To investigate the risk factors for pleural lung disease (PLD) in children with juvenile idiopathic arthritis (JIA) and to provide a basis for the early diagnosis and timely treatment of this disease.</p><p><b>METHODS</b>A total of 360 children with a confirmed diagnosis of JIA were enrolled, and their clinical data were retrospectively analyzed. All patients underwent a chest X-ray. The patients with PLD were assigned to PLD group, while those without PLD were assigned to non-PLD group. The clinical, imaging, and laboratory results of JIA patients with PLD were analyzed.</p><p><b>RESULTS</b>Among the 360 JIA patients, 43 (11.9%) had PLD, and 9 (21%) of them had respiratory symptoms. Chest X-ray findings mainly included interstitial pneumonitis (53.5%) and pleurisy and/or pleural effusion (38.1%). In the 43 cases of JIA-PLD, 4 (9.3%) had normal chest X-ray findings but abnormal chest CT findings. The incidence of PLD was relatively high in patients aged under 3 years and those aged 12 years or above. Children with systemic JIA had a relatively high incidence of PLD. Compared with the non-PLD group, the PLD group had a significantly higher incidence of anemia, elevated white blood cell (WBC) count and IgG levels in peripheral blood, and positive rheumatoid factors or antinuclear antibodies (P<0.05).</p><p><b>CONCLUSIONS</b>Among children with JIA, PLD is mostly seen in patients with systemic JIA or aged <3 years or ≥ 12 years, especially those with anemia, elevated WBC count and IgG levels, and positive rheumatoid factors or antinuclear antibodies. For JIA patients with PLD, interstitial pneumonitis is usually seen on chest X-ray or CT, but respiratory symptoms are rarely observed. Routine use of high-resolution chest CT is recommended for early diagnosis and timely treatment of PLD in children with JIA.</p>

<p><b>OBJECTIVE</b>To study the changes in serum cytokines levels in children with newly diagnosed active systemic juvenile idiopathic arthritis (SJIA) and to explore the role of cytokines in the development and progression of SJIA.</p><p><b>METHODS</b>Seventy-four pediatric patients with active SJIA between January 2010 and December 2013 were included in the study. Serum levels of interleukin-2 (IL-2), interleukin-4 (IL-4), interleukin-6 (IL-6), interleukine-10 (IL-10), tumor necrosis factor (TNF), and interferon gamma (IFN-γ) were measured by flow cytometry in these patients. The levels of cytokines were also determined in 202 healthy children as the control group. Routine laboratory parameters including white blood cell (WBC) count, percentage of neutrophils, hemoglobin level, platelet count, hypersensitive C-reactive protein (hs-CRP), and erythrocyte sedimentation rate (ESR) were monitored in the patient group.</p><p><b>RESULTS</b>The WBC count, percentage of neutrophils, hs-CRP, and ESR in 74 cases of SJIA were significantly above the normal range, their platelet counts were within the normal range, whereas hemoglobin levels were below the normal range. Compared with the control group, the patient group showed a significantly increased level of IL-6 (P<0.01) and significantly reduced levels of IL-4, IL-10, and TNF (P<0.01). However, there were no significant changes in serum levels of IL-2 and IFN-γ in the patient group (P>0.05). In SJIA children, IL-6 level, which was significantly elevated, was negatively correlated with hemoglobin level, which was significantly reduced (r=-0.244, P<0.05).</p><p><b>CONCLUSIONS</b>Serum level of IL-6 is significantly increased in children with SJIA, and it has a negative correlation with anemia.</p>

ObjectiveTo evaluate the reliahility and validity of the chronic HBV-infections related stigma scale.MethodsThe initial items and construct of the scale were developed according to theoretical analysis and interviews of experts and patients.A total of 151 patients with chronic HBV-infection were administered by convenient sampling method in this pilot study. The reliability and the validity of the scale were then evaluated.ResultsThe response rate of the scale was 94.5%.The Cronbach α coefficients of all dimeusions ranged from 0.75-0.87.The results of correlation analysis showed that there were higher correlation coefficients ( r ranged from 0.62-0.86) between items and their hypothesized subscales than those with other subscales ( r ranged from 0.14-0.55).The scale distinguished between patients with low subscale scores ( the subscale scores were ( 1.89 ±0.30 ),( 1.86 ± 0.29 ),( 1.96 ± 0.23 ),( 2.29 ± 0.45 ),( 1.59 ± 0.42 ) independently) and those with high subscale scores(the subscalc scores were (3.62 ±0.44),(3.99 ±0.41 ),(3.79 ±0.37),(4.13 ±0.34),(3.10 ±0.53 ) independently) (P < 0.01 ).Confirmatory factor analysis showed that the main indices of goodness of fit CFI was 0.94,NNFI 0.92,RMSEA 0.087.ConclusionThe chronic HBV-infections related stigma has good psychometric properties regarding to reliability and validity.

RESUMEN

Objective To probe the value of ultrasound in detection of malignant foci among multiple thyroid nodules. Methods Ultrasonograms of 83 patients with pathologically confirmed thyroid cancer combined with benign nodules were analyzed.Results Among 83 patients of thyroid carcinoma, 89 malignant and 87 benign nodules were detected with ultrasound. Significant differences of two-dimensional ultrasounic signs including low echoes, border fuzzy, micro-calcification and rear echo attenuation were found bteween malignant and benign nodules (P<0.01). There were significant differences in the grade Ⅱ, Ⅲ blood flow between malignant and benign ones (P<0.01). Of 89 malignant nodules, small thyroid carcinoma with diameter ≤1 cm accounted for 52.81% (47/89), and ultrasound diagnostic rate of all cancer nodules was 67.42% (60/89).Conclusion Ultrasounic characteristics including low echoes, border fuzzy, micro-calcification, rear echo attenuation and internal rich blood flow contribute to differentiate the thyroid microcarcinoma from multinodules.In particular, the microcarcinoma should not be ignored.

<p><b>OBJECTIVE</b>To investigate the clinical features of recurrent Kawasaki disease (KD) and the relationship of recurrent KD with coronary artery lesions.</p><p><b>METHODS</b>The medical data of 20 children with recurrent KD who were admitted to the Children's Hospital from January 1998 to May 2007 were retrospectively studied. Their clinical features were compared with those of children with initial KD.</p><p><b>RESULTS</b>The incidence of recurrent KD was 1.34% (20/1489). KD relapsed 2 months to 4.6 years (average: 1.2 years) after the first episode in the 20 children. Compared with the initial KD group, the clinical symptoms in the recurrent KD group were incomplete, complicated and less severe. The period of fever, platelet count, C-reactive protein and ESR were remarkably reduced in the recurrent KD group, but the incidence of coronary artery lesions increased significantly compared with the initial KD group (40% vs 25%; P<0.05).</p><p><b>CONCLUSIONS</b>The clinical symptoms of recurrent KD were incomplete in children. Recurrent KD was associated with an increased incidence of coronary artery lesions.</p>

RESUMEN

A high activity isoflavone-glucosidase, which hydrolysis glycosides, was obtainde using liquid fermentation from Absidia sp. R strain. The isoflavone-glucosidase was purified 11 folds with yielding rate of 10.9% after ammonium sulfate precipitation and DEAE-Cellocuse (DE-52) ion exchange chromatography. SDS-PAGE results showed that the molecular weight is 53kD. And the optimum temperature, the optimum pH, Km and pI of the enzyme are 50 deegrees C, 5.0, 1.3 x 10(-2) mol/L and 3.2, respectively. The isoflavone-glucosidase is also rather stable under 60 degrees C and in pH range from 5.0 to 7.0. The enzyme can be activated by Co2+ and Ca2+, and be inhibited by Ag+ and Cu2+.

Using Vibrio fluvialis Ⅱ as host bacteria, 19 bacteriophages have been isolated from the 76 samples which were collected from Haliotis discus hannai ~growing seawater in Dalian marine culture company Dalian, liaoning province from May in 1996 to August I 1997. Ultrastructure of 19 bacteriophages were observed with electron to Bradley the results showed that of these bacteriophages belonged to Bradley A type, they have hexagonal heads of bacteriophages were identified with VP1,VP2,VP4,VP8 as representatives respectively. The phages remain stable at pH6. 0~10. 0, moreover VP2,VP4 and VP8 are rather stable at basic pHs. Although the characterization of heat inactivation course of VP4 is different from others, four phages are sensitive to heat and inactivated at 80℃ in 5 minutes. One step growth experiment showed that the eclipse period of VP1,VP2,VP4,VP8 are sensitive to heat and the eclipse period of VP1, VP2, VP4, VP8 are 42, 30, 46, 28 minutes. In this experiment we have isolated at least four different types phages, it suggest that in fact there is a population of phages in the seawater environment. The result of this study provided a way to find the potential value of phages as an indicator of pathogenic microorganisms Vibrio fluvilis Ⅱ in marine environment.
