Objetivo: identificar o local e os cuidados diretos recebidos por pessoas com úlceras da perna por doença falciforme nos serviços de atenção à saúde. Método: estudo transversal, realizado em 11 centros, no período de agosto de 2019 a abril de 2020. Fizeram parte do estudo 72 pessoas com úlcera da perna ativa. O estudo foi aprovado pelo Comitê de Ética em Pesquisa. Resultado: apresentavam anemia falciforme 91,7% dos participantes, com mediana de três anos de existência da úlcera; 77,8% eram redicivantes; 40,3% compravam os insumos; 66,7% trocavam o próprio curativo no domicílio; 52,8% realizavam uma ou mais trocas diárias; 45,8% dos tratamentos foram prescritos pelo médico; 37,5% eram pomada (colagenase ou antibiótico); 89% não utilizavam compressão para o manejo do edema. Conclusão: a maioria dos participantes não estava inserida na Rede de Atenção à Saúde para o tratamento da úlcera, e não recebia assistência sistematizada e nem insumos apropriados.


Objective: to identify the location and direct care received by people with leg ulcers due to sickle cell disease in health care services. Method: a cross-sectional study carried out in 11 centers from August 2019 to April 2020. The study included 72 people with active leg ulcers. The study was approved by the Research Ethics Committee. Results: a total of 91.7% of the participants had sickle cell anemia, with a median of three years of ulcer existence; 77.8% were recurrent; 40.3% bought the supplies; 66.7% changed their own dressings at home; 52.8% did one or more changes a day; 45.8% of the treatments were prescribed by physician; 37.5% were ointments (collagenase or antibiotics); and 89% did not use compression to manage edema. Conclusion: most of the participants were not included in the Health Care Network for ulcer treatment and did not receive systematized care or appropriate supplies.


Objetivo: identificar el lugar y los cuidados directos recibidos por personas con úlceras de pierna por enfermedad falciforme en los servicios de atención a la salud. Método: estudio transversal, realizado en 11 centros, en el período de agosto de 2019 a abril de 2020. Participaron 72 personas con úlcera de pierna activa. El estudio fue aprobado por el Comité de Ética en Investigación. Resultado: presentaban anemia falciforme 91,7% de los participantes, con una mediana de tres años de existencia de la úlcera; 77,8% eran recidivantes; 40,3% compraban los insumos; 66,7% cambiaban su propio vendaje en el domicilio; 52,8% realizaban uno o más cambios diarios; 45,8% de los tratamientos fueron prescritos por el médico; 37,5% eran pomada (colagenasa o antibiótico); y 89% no utilizaban compresión para el manejo del edema. Conclusión: la mayoría de los participantes no estaba integrada en la Red de Atención a la Salud para el tratamiento de la úlcera, y no recibía asistencia sistematizada ni insumos apropiados.

RESUMEN

Abstract Chronic ulcers significantly affect the quality of life of patients and impose a high cost on the healthcare system. The therapeutic management should be comprehensive, taking into consideration the etiological diagnosis of the wound and the characteristics of the wound bed when deciding on a therapeutic proposal appropriate to the healing phase, correcting factors that delay healing. During the epithelialization phase, repair techniques with grafts are recommended to shorten re-epithelialization time, improve the quality of scar tissue, and achieve adequate pain management. Currently, due to the reported benefits of skin appendages, the technique of follicular unit auto-grafting obtained with a scalp punch is among the chosen strategies for wound repair. This is a minimally invasive, outpatient practice, whose technique has advantages over the donor site, patients recovery and well-being.

Objetivo: Avaliar o risco de Lesão por Pressão em pessoas acamadas assistidas pela Estratégia Saúde da Família. Método: Estudo transversal, com abordagem quantitativa, realizado com 62 pacientes. A coleta de dados sucedeu-se por meio de questionário semiestruturado com dados sociodemográficos, clínicos e a avaliação do risco de Lesão por Pressão por meio da Escala de Braden. Os dados foram analisados pelo softwareestatístico Statistical Package for Social Science, versão 20.0. Resultados: Prevaleceram os pacientes do sexo feminino (61,3 %), cor branca (43,5%), viúvos (35,5%), aposentados (66,1%) e não alfabetizados (62,9%). O principal motivo de estar acamado foi devido a sequelas do Acidente Vascular Encefálico (35,5%). Evidenciou-se prevalência de risco muito alto em 59,7% das pessoas acamadas. Conclusão: O risco para Lesão por Pressão foi elevado, e a identificação dos fatores de risco é necessária e pode contribuir para estratégias preventivas ou redutoras deste agravo. (AU)


Objective: To assess the risk of pressure ulcers in bedridden individuals assisted by the Family Health Strategy. Method: A cross-sectional study employing a quantitative approach was conducted with 62 patients. Data collection was performed through a semi-structured questionnaire, encompassing sociodemographic and clinical data, as well as the assessment of pressure ulcer risk using the Braden Scale. Data were analyzed using the Statistical Package for the Social Sciences software, version 20.0. Results: Female patients (61.3%), Caucasians (43.5%), widows (35.5%), married individuals (66.1%), and those with no formal education (62.9%) predominated. The primary reason for being bedridden was sequelae from a stroke (35.5%). A prevalence of very high risk was observed in 59.7% of bedridden individuals. Conclusion: The risk of pressure ulcers was high, emphasizing the necessity of identifying risk factors to inform preventive or mitigating strategies for this condition. (AU)


Objetivo: Evaluar el riesgo de Úlceras por Presión en personas encamadas atendidas por la Estrategia Salud de la Familia. Método: Se realizó un estudio transversal con enfoque cuantitativo con 62 pacientes. La recolección de datos se realizó a través de un cuestionario semiestructurado con datos sociodemográficos y clínicos, así como la evaluación del riesgo de úlceras por presión utilizando la Escala de Braden. Los datos fueron analizados utilizando el softwareStatistical Package for Social Science, versión 20.0. Resultados: Predominaron pacientes de sexo femenino (61,3%), raza blanca (43,5%), viudas (35,5%), jubiladas (66,1%) y analfabetas (62,9%). El principal motivo de encamación fue por las secuelas de un accidente cerebrovascular (35,5%). Hubo una prevalencia de riesgo muy alta en el 59,7% de las personas encamadas. Conclusión: El riesgo de úlceras por presión fue alto, y la identificación de los factores de riesgo es necesaria y puede contribuir a estrategias preventivas o reductoras de esta condición. (AU)

Objetivo: Identificar la producción de conocimiento sobre el tratamiento de infecciones localizadas en heridas de difícil cicatrización. Método: Revisión integrativa de la literatura realizada en la Biblioteca Virtual en Salud; Base de datos de enfermería; Scientific Electronic Library; Web of Science; Biblioteca Cochrane; Catálogo de Tesis y Disertaciones de la Coordinación para el Perfeccionamiento del Personal de Educación Superior; y PubMed. Los artículos seleccionados no tienen límite de tiempo. Los estudios fueron exportados a la aplicación Rayyan y sometidos a evaluación doble ciego mediante la lectura del título y el resumen, según los criterios de inclusión y exclusión. La información fue analizada y sintetizada según el nivel de evidencia. Resultados: 19 estudios fueron incluidos para lectura completa. Se encontró como evidencia la higiene de la herida; la limpieza con ácido acético al 1%; la identificación y el tratamiento de biopelículas; el uso de cobertores y soluciones con acción antimicrobiana. Conclusión: La infección localizada de la herida ha sido objeto de varias investigaciones y las prácticas recomendadas se refieren a tratamientos tópicos. (AU)


Objective: To identify the knowledge production on the treatment of localized infections in hard-to-heal wounds. Method: An integrative literature review was conducted in the Virtual Health Library, Nursing Database, Scientific Electronic Library Online, Web of Science, Cochrane Library, Catalog of Theses and Dissertations of the Coordination for the Improvement of Higher Education Personnel, and PubMed. The selected articles had no time limit. The studies were exported to the Rayyan application and subjected to double-blind evaluation through title and abstract reading, based on inclusion and exclusion criteria. The information was analyzed and synthesized according to the level of evidence. Results: A total of 19 publications were fully analyzed. The evidence obtained on the topic includes wound hygiene, cleaning with 1% acetic acid, identification and treatment of biofilms, the use of dressings, and solutions with antimicrobial action. Conclusion: Localized wound infections have been the subject of various research studies, and the recommended practices refer to topical treatments. (AU)

Os estudos foram exportados para o aplicativo Rayyan e submetidos à avaliação duplo-cega por meio da leitura do título e do resumo, com base nos critérios de inclusão e exclusão. As informações foram analisadas e sintetizadas de acordo com o nível de evidência. Resultados: Foram analisadas 19 publicações em sua totalidade. Obteve-se como evidências acerca do tema a higienização da ferida, a limpeza com ácido acético 1%, a identificação e o tratamento de biofilmes, o uso de coberturas e as soluções com ação antimicrobiana. Conclusão: A infecção localizada de feridas tem sido objeto de várias pesquisas e as práticas recomendadas referem-se a tratamentos tópicos. (AU)

Introducción: las enfermedades del pie relacionadas a la diabetes mellitus representan una de las causas de mayor morbilidad e incapacidad en las personas con diabetes mellitus tipo 2, siendo la causa más frecuente de ingreso hospitalario en dicho grupo. Objetivo: describir las características clínicas de los pacientes con enfermedad del pie relacionadas a la diabetes mellitus del Hospital Nacional 2022-2023. Metodología: se seleccionaron 113 pacientes portadores de diabetes mellitus tipo 2 con pie diabético mayores de 18 años. Se evaluaron las variables demográficas, medidas antropométricas, características de la enfermedad, comorbilidades y características clínicas del pie. Resultados: de los 113 estudiados 42 pacientes (37 %) correspondieron al sexo femenino y 71 (63 %) al sexo masculino, promedio de edad fue de 65 años DE 12,191. 75 pacientes (66 %) presentaron pie diabético, con lesión Wagner grado 4. El 81 % (92) tenía hipertensión arterial, sedentarismo 65 % (84), en menor frecuencia pacientes con sobrepeso 38 % (43), obesidad 25 % (38), tabaquismo 23 % (26) y dislipidemia 18 % (20). Conclusión: las características clínicas de los pacientes con diabetes tipo 2 con lesión en el pie coinciden con otros trabajos obtenidos a nivel mundial. Es muy importante prestar atención a este grupo de riesgo, mediante medidas preventivas y realizar el tratamiento precoz para disminuir las complicaciones.


Introduction: foot diseases related to diabetes mellitus represent one of the causes of greatest morbidity and disability in people with type 2 diabetes mellitus, being the most frequent cause of hospital admission in said group. Objective: to describe the clinical characteristics of patients with foot disease related to diabetes mellitus at Hospital Nacional 2022-2023. Methodology: 113 patients with type 2 diabetes mellitus with diabetic foot over 18 years of age were selected. Demographic variables, anthropometric measurements, disease characteristics, comorbidities, and clinical characteristics of the foot were evaluated. Results: of the 113 studied, 42 patients (37 %) were female and 71 (63 %) were male, average age was 65 years SD 12,191. 75 patients (66 %) presented diabetic foot, with Wagner grade 4 lesion. 81 % (92) had high blood pressure, sedentary lifestyle 65 % (84), less frequently overweight patients 38 % (43), obesity 25% (38) ), smoking 23 % (26) and dyslipidemia 18 % (20). Conclusion: the clinical characteristics of patients with type 2 diabetes with foot injury coincide with other works obtained worldwide. It is very important to pay attention to this risk group, through preventive measures and carry out early treatment to reduce complications.

Objetivo: Avaliar os fatores preditores para ocorrência de lesão por pressão em pacientes oncológicos em cuidados paliativos. Métodos: Estudo epidemiológico observacional, analítico, com delineamento transversal e abordagem quantitativa. A coleta de dados foi realizada com 105 participantes, no período de maio a outubro de 2019, em uma Clínica de Cuidados Paliativos Oncológicos de um Centro de Alta Complexidade em Oncologia. Os foram inseridos no software Biostat 5.0, em que foi realizada primeiramente a análise de regressão logística univariada, e posteriormente foram selecionadas as variáveis para a regressão logística múltipla e assim definiram-se os fatores preditivos para lesão por pressão. Resultados: A prevalência identificada foi de 19,04% para lesão por pressão. A maioria da amostra eram mulheres (60%), com idade menor que 70 anos (70%). Dois terços apresentavam risco muito alto (15%), para lesão por pressão segundo a Escala de Braden, e possuíam como diagnóstico primário câncer de próstata (20%), seguido de colo uterino (15%). Conclusão: A presença de lesão medular e o uso de fralda descartável demonstrou forte correlação com o desenvolvimento de lesão por pressão, sendo estes os fatores preditivos identificados neste estudo. Conhecer o perfil desses pacientes auxilia na elaboração e sistematização das condutas de enfermagem, visando melhor qualidade e segurança no cuidado. (AU)


Objective: To evaluate the predictive factors for the occurrence of pressure injuries in cancer patients undergoing palliative care. Methods: Observational, analytical, cross-sectional epidemiological study with a quantitative approach. Data collection was carried out with 105 participants, from May to October 2019, in an Oncology Palliative Care Clinic of a High Complexity Oncology Center. The data were entered into the Biostat 5.0 software, in which the univariate logistic regression analysis was first performed, and then the variables for the multiple logistic regression were selected, thus defining the predictive factors for pressure injury. Results: The identified prevalence was 19.04% for pressure injuries. Most of the sample were women (60%), aged under 70 years (70%). Two-thirds were at very high risk (15%) for pressure injury according to the Braden Scale, and had prostate cancer as a primary diagnosis (20%), followed by cervix (15%). Conclusion: The presence of spinal cord injury and the use of a disposable diaper showed a strong correlation with the development of pressure injury, which are the predictive factors identified in this study. Knowing the profile of these patients helps in the elaboration and systematization of nursing procedures, aiming at better quality and safety in care. (AU)

La recolección de datos se realizó con 105 participantes, de mayo a octubre de 2019, en una Clínica de Cuidados Paliativos Oncológicos de un Centro Oncológico de Alta Complejidad. Los datos se ingresaron en el software Biostat 5.0, en el cual se realizó primero el análisis de regresión logística univariante, y luego se seleccionaron las variables para la regresión logística múltiple, definiendo así los factores predictivos de lesión por presión. Resultados: La prevalencia identificada fue del 19,04% para las lesiones por presión. La mayoría de la muestra fueron mujeres (60%), menores de 70 años (70%). Dos tercios tenían un riesgo muy alto (15%) de lesión por presión según la escala de Braden y tenían cáncer de próstata como diagnóstico primario (20%), seguido del cuello uterino (15%). Conclusión: La presencia de lesión medular y el uso de pañal desechable mostró una fuerte correlación con el desarrollo de lesión por presión, que son los factores predictivos identificados en este estudio. Conocer el perfil de estos pacientes ayuda en la elaboración y sistematización de los procedimientos de enfermería, buscando una mejor calidad y seguridad en la atención. (AU)

Objective. This article presents a literature review to explore and analyze the current situation of pressure ulcers or lesions or decubitus ulcers, pathophysiological, epidemiological aspects, and risk factors. The progress in evidence of the effectiveness of preventive repositioning in the appearance of these lesions in vulnerable hospitalized patients is also evaluated. Methods. Databases were reviewed in non-systematic manner, including the Cochrane Wounds Specialized Register; Medline, Scopus, PubMed, the Cochrane Central Register of Controlled Trials; MEDLINE (Ovid); EMBASE (Ovid), Web of Science, SciELO, and Lilacs. The general search terms included [pressure ulcers or pressure lesions or decubitus ulcers] and [prevention or preventive] and [repositioning or positioning or position changes or postural change] and [patient at risk or vulnerable] and [hospitalized or ICU or intensive care]. Systematic literature reviews, randomized clinical trials, observational studies, cost-effectiveness and qualitative studies in English or Spanish were included. Results. Although globally, the incidence, prevalence, and years of disability associated to these lesions has diminished between 1990 and 2019, the high impact on health persists. Evidence found on the effectiveness of repositioning in preventing pressure ulcers and health associated costs has been evaluated with certainty between low and very low, as a result of conducting research with serious methodological limitations that report results with high inaccuracy. Conclusion.The findings reported present that these lesions persist at hospital level and continue being a global social and health problem with high impact on health budgets. Likewise, there is a need to develop greater quality research on prevention strategies, such as repositioning, which validate their effectiveness, and justify their use.


Objetivo. Este artículo presenta una revisión de la literatura con el objetivo de explorar y analizar la situación actual de las úlceras o lesiones por presión o úlceras por decúbito, aspectos fisiopatológicos, epidemiológicos, y factores de riesgo. Se evalúa además el progreso en la evidencia de la eficacia del reposicionamiento preventivo en la aparición de estas lesiones en pacientes vulnerables hospitalizados. Métodos. Se revisaron bases de datos de forma no sistemática, incluyendo The Cochrane Wounds Specialised Register; Medline, Scopus, PubMed, the Cochrane Central Register of Controlled Trials; MEDLINE (Ovid); EMBASE (Ovid), Web of Science, Scielo, y Lilacs. Los términos de búsqueda generales incluyeron [úlceras por presión o lesiones por presión o úlceras por decúbito] y [prevención o preventivo] y [reposicionamiento o posicionamiento o cambios de posición o cambio postural] y [paciente en riesgo o vulnerable] y [hospitalizado o UCI o cuidados intensivos]. Se incluyeron revisiones sistemáticas de la literatura, ensayos clínicos aleatorizados, estudios observacionales, estudios de costo-efectividad y cualitativos en idioma inglés o español. Resultados. Aunque globalmente la incidencia, prevalencia y años de incapacidad asociado a estas lesiones ha disminuido entre 1990 y 2019, el impacto en salud persiste de forma elevada. La evidencia encontrada sobre la eficacia del reposicionamiento en prevención de úlceras por presión y costos asociados en salud ha sido evaluada con certeza entre baja y muy baja, como resultado de la realización de investigaciones con serias limitaciones metodológicas que reportan resultados con alta imprecisión. Conclusión.Los hallazgos reportados presentan que estas lesiones persisten a nivel hospitalario y continúan siendo un problema social y de salud mundial con alto impacto en los presupuestos en salud. Así mismo se presenta la necesidad de desarrollar mayor investigación de calidad en estrategias preventivas como el reposicionamiento, que validen su eficacia, y justifiquen su utilización.

Os termos gerais de pesquisa incluíram [úlceras de pressão ou lesões por pressão ou úlceras de pressão ou úlceras de decúbito] e [prevenção ou preventivo] e [reposicionamento ou posicionamento ou mudanças de posição ou mudança postural] e [paciente em risco ou vulnerável] e [hospitalizado ou UTI ou terapia intensiva]. Foram incluídas revisões sistemáticas da literatura, ensaios clínicos randomizados, estudos observacionais, estudos de custo-efetividade e qualitativos em inglês ou espanhol. Resultados. Embora, em geral, a incidência, a prevalência e os anos de incapacidade associados a essas lesões tenham diminuído entre 1990 e 2019, o impacto na saúde continua alto. As evidências encontradas sobre a eficácia do reposicionamento na prevenção de úlceras por pressão e os custos de saúde associados foram avaliadas com certeza baixa a muito baixa, como resultado de pesquisas com sérias limitações metodológicas que relataram resultados altamente imprecisos. Conclusão. Os resultados relatados mostram que essas lesões persistem em nível hospitalar e continuam a ser um problema social e de saúde global com alto impacto nos orçamentos de saúde. Também há necessidade de mais pesquisas de qualidade sobre estratégias preventivas, como o reposicionamento, para validar sua eficácia e justificar seu uso.

Antecedentes: La sífilis es una infección sexualmente transmisible sistémica crónica que afecta a docenas de millones de personas al año. A nivel anorrectal, su manifestación polimórfica obliga al diagnóstico diferencial con enfermedades anorrectales benignas y malignas. Objetivo: Describir las diferentes presentaciones de la sífilis anorrectal a propósito de 5 casos clínicos. Método: Estudio observacional, retrospectivo, descriptivo. Resultados: La mayoría de los pacientes fueron VIH positivos en edad sexual activa. Las manifestaciones registradas, al igual que las reportadas en la bibliografía fueron las fisuras, úlceras perianales y pseudotumores. Conclusiones: La sífilis es considerada "la gran simuladora". En la localización anorrectal se requiere una alta sospecha diagnóstica para diferenciarla de presentaciones similares de otras enfermedades anales benignas, la enfermedad inflamatoria intestinal y el cáncer anorrectal, con el fin de evitar el consiguiente riesgo de sobretratamiento. Background: Syphilis is a chronic systemic sexually transmitted infection that affects tens of millions of people annually. At the anorectal level, its polymorphic manifestation requires differential diagnosis with benign and malignant anorectal diseases. Objective: To review the presentation of anorectal syphilis from 5 clinical cases. Methods: Observational, retrospective, descriptive study. Results: Most of the patients were HIV positive in sexually active age. The manifestations recorded and reported in the literature were fissures, perianal ulcers, and pseudotumors. Conclusions: Syphilis is considered "the great pretender". In anorectal syphilis, a high diagnostic suspicion is needed to differentiate it from similar presentations due to other anal conditions, inflammatory bowel disease, and anorectal cancer, to avoid the consequent risk of overtreatment. Introducción: La úlcera por presión de talón es de difícil resolución por ser zona de apoyo especialmente cuando existe exposición de calcáneo. Una solución para esta patología es el colgajo fasciocutáneo en cono que es fácil, reproducible y segura. Material y Método: Se operó a 12 pacientes con este tipo de úlcera entre 2016 y 2022, siendo 10 hombres y 2 mujeres con edades entre 35 y 65 años portadores de paraplejia. En 6 de Ellos hubo osteomielitis de calcáneo que requirió de tratamiento antibiótico por 6 semanas. En todos ellos se practicó el colgajo en cono, descrito por el autor principal. Técnica quirúrgica: Primeramente, se realiza desbridamiento quirúrgico de la zona afectada. Posteriormente se practica el colgajo en cono que consiste en dos colgajos fasciocutáneos: uno de rotación, que cubre la lesión, el defecto dejado por éste es cubierto con un colgajo en avance en V-Y. Resultados: Todos los colgajos sobrevivieron sin signos de necrosis. No hubo recurrencia de las úlceras. La osteomielitis cedió con tratamiento antibiótico. Discusión: La exposición del calcáneo producto de una úlcera por presión de talón es de difícil tratamiento por ser una zona de apoyo constante. El tratamiento con el colgajo fasciocutáneo en cono ha probado ser de fácil resolución, reproducible y seguro para cubrir exposición de calcáneo. Conclusión: El tratamiento quirúrgico de la úlcera por presión de talón con el colgajo en cono es un procedimiento efectivo, reproducible, seguro y sin recidiva en nuestra casuística.


Introduction: The heel pressure sore is common in patients without sensivity such as paraplejic ones. When the calcaneous bone is exposed, with or without osteomielytis, a flap have to be used. In our experience the fasciocutaneous cone flap have been a good solution. Material and Method: Since 2016 and 2022 we have operated on 12 patients with heel pressure sore in paraplejic patients. In six of them we found osteomyelitis so the antibiotics were administred for six weeks without recurrence. There were 10 men and two women. Ages vary from 35 to 65 years old. Patients were para and tetraplejic ones. All were treated with de fasciocutaneous cone flap. Surgical technique. The area of pressure sore was treated with surgical debridment. After that, the fasciocutaneous cone flap was built. The cone flap consist in two flap: the first is a rotation and advance flap and the second is a V-Y advance flap that covers the defect made for the first one. Results: All the flaps survive with no necrosis, covering the complete defect. In case of osteomyelitis the antibiotics were administred for six weeks without recurrence. Discussion: Calcaneous exposure of the heel pressure sore needs coverage with a flap. The cone flap give good blood supply in patients with or whitout osteomyelitis with good results. This kind of flap is reliable, easy to perform and reproducible. Conclusión: The surgical technique of cone flap, for treatment of heel pressure sores, is reliable, sure, reproducible and whitout recurrence in our experience.

OBJETIVO: Analisar a dosimetria do laser de baixa intensidade no processo de cicatrização de úlcera venosa. MÉTODO: Trata-se de um protocolo de revisão sistemática registrado no International Prospective Register of Systematic Reviews (PROSPERO) sob código de registro CRD420211256286. Serão realizadas buscas por evidências científicas em 11 bases de dados, utilizando os idiomas português, inglês e espanhol. A exportação das publicações seguirá as etapas de identificação e seleção dos estudos, e extração dos dados. As divergências serão resolvidas por consenso dos dois revisores, e caso persistam, um terceiro revisor será consultado para decidir sobre a inclusão do material. A ferramenta Risk of Bias 2 (RoB 2) será utilizada para avaliar o risco de viés dos estudos dos ensaios clínicos randomizados, ao passo que a ferramenta Risk of Bias in Non-randomized Studies of Interventions (ROBINS-I) será utilizada para avaliar o risco de viés dos ensaios clínicos não randomizados. A análise crítica dos materiais selecionados quanto à dosimetria do laser de baixa intensidade para cicatrização de úlcera venosa resultará em uma síntese narrativa, sem metanálise.


OBJECTIVE: To analyze the dosimetry of low-level laser therapy in the healing process of venous ulcers. METHOD: This is a protocol for systematic review registered in the International Prospective Register of Systematic Reviews under registry code CRD420211256286. Articles will be searched in 11 databases using Portuguese, English, and Spanish languages. The export of publications will follow the steps of study identification, selection, and data extraction. Disagreements will be resolved by consensus among reviewers; if they persist, a third reviewer will be consulted to decide whether to include the material. The Risk of Bias 2 (RoB 2) tool will be used to assess the validity of randomized clinical trials, while the Risk of Bias in Non-randomized Studies of Interventions (ROBINS-I) tool will be used to assess the risk of bias in non-randomized clinical trials. The critical analysis of selected materials on dosimetry of low-level laser therapy for venous ulcer healing will result in a narrative synthesis without meta-analysis.

Introduction. Perforated peptic ulcer remains one of the critical abdominal conditions that requires early surgical intervention. Leakage after omental patch repair represents one of the devastating complications that increase morbidity and mortality. Our study aimed to assess risk factors and early predictors for incidence of leakage. Methods. Retrospective analysis of data of the patients who underwent omental patch repair for perforated peptic ulcer in the period between January 2019 and January 2022 in Mansoura University Hospital, Egypt. Pre, intra and postoperative variables were collected and statistically analyzed. Incriminated risk factors for leakage incidence were analyzed using univariate and multivariate analysis. Results. This study included 123 patients who met inclusion criteria. Leakage was detected in seven (5.7%) patients. Although associated comorbidities (p=0.01), postoperative intensive care unit admission (p=0.03), and postoperative hypotension (p=0.02) were significant risk factors in univariate analysis, septic shock (p=0.001), delayed intervention (p=0.04), preoperative hypoalbuminemia (p=0.017), and perforation size >5mm (p= 0.04) were found as independent risk factors for leakage upon multivariate analysis. Conclusion. Delayed presentation in septic shock, preoperative hypoalbuminemia, prolonged perforation, operation interval, and large perforation size > 5mm were detected as independent risk factors for leakage. Postoperative tachypnea and tachycardia with increased levels of C-reactive protein and total leucocytic count are alarming signs for incidence of leakage


Introducción. La úlcera péptica perforada es una de las afecciones abdominales críticas que requiere una intervención quirúrgica temprana. La fuga después de la reparación con parche de epiplón representa una de las complicaciones más devastadoras, que aumentan la morbilidad y la mortalidad. Nuestro estudio tuvo como objetivo evaluar los factores de riesgo y los predictores tempranos de fugas. Métodos. Análisis retrospectivo de los datos de los pacientes sometidos a reparación con parche de epiplón por úlcera péptica perforada, en el período comprendido entre enero de 2019 y enero de 2022, en el Hospital Universitario de Mansoura, Egipto. Se recogieron y analizaron estadísticamente variables pre, intra y postoperatorias. Los factores de riesgo asociados a la incidencia de fugas se analizaron mediante análisis univariado y multivariado. Resultados. Este estudio incluyó 123 pacientes que cumplieron con los criterios de inclusión. Se detectó fuga en siete (5,7 %) pacientes. Aunque las comorbilidades asociadas (p=0,01), el ingreso postoperatorio a la unidad de cuidados intensivos (p=0,03) y la hipotensión postoperatoria (p=0,02) fueron factores de riesgo en el análisis univariado, el shock séptico (p=0,001), el retraso en la intervención (p=0,04), la hipoalbuminemia preoperatoria (p=0,017) y el tamaño de la perforación mayor de 5 mm (p=0,04) se encontraron como factores de riesgo de fuga independientes en el análisis multivariado. Conclusión. Se detectaron como factores de riesgo independientes de fuga la presentación tardía en shock séptico, la hipoalbuminemia preoperatoria, la perforación prolongada, el intervalo operatorio y el tamaño de la perforación mayor de 5 mm. La taquipnea posoperatoria y la taquicardia con niveles elevados de proteína C reactiva y recuento leucocitario total son signos de alarma sobre la presencia de fuga.

This study aimed to determine the antiulcerogenic and antioxidant activities of Psyllium (Plantago ovata Forssk) seed ethanolic extract in rats. We assessed the antioxidant potential using free radical scavenging on DPPH, ß-carotene bleaching activity, ferric reducing power, and hydroxyl radical scavenging activity. In the antiulcerogenic study, pre-treatment with Plantago ovata seeds ethanolic extract (POE) (400 mg/kg b.wt) significantly protected against ethanol-induced gastric ulcer in rats by decreasing the ulcer index value and preserving the integrity of the gastric mucosa. The oxidative stress status in the stomach tissues showed a significant increase in the antioxidant enzyme levels of superoxide dismutase, catalase, and glutathione peroxidase with a significant decrease in lipid peroxidation during pre-treatment with POE. In conclusion, the POE protects against gastric ulcer due to its antioxidant potential and presence of bioactive molecules.


O presente estudo teve como objetivo determinar as atividades antiulcerogênica e antioxidante das sementes de Psyllium (Plantago ovata Forssk) em ratos. O potencial antioxidante foi avaliado utilizando o método do sequestro do radical livre DPPH, autooxidação do ß-caroteno, poder redutor de ferro e atividade de sequestro do radical hidroxila. No estudo antiulcerogênico, o pré-tratamento com o extrato etanólico das sementes de Plantago ovata (POE) (400 mg/Kg b.wt) reduziu a úlcera gástrica induzida pelo etanol em ratos, diminuindo o valor do índice de úlcera e preservando a integridade da mucosa gástrica. O estudo do estresse oxidativo nos tecidos estomacais mostrou um aumento significativo dos níveis das enzimas antioxidantes superóxido dismutase, catalase e glutationa peroxidase, com uma diminuição significativa da peroxidação lipídica enquanto pré-tratamento com POE. Em conclusão, o POE protege contra úlcera gástrica devido aos seus potenciais antioxidantes e à presença de moléculas bioativas.

Background: Diabetic foot ulcer (DFU) is a major complication of diabetes mellitus (DM) which is associated with high morbidity and mortality. There is high rate of bacteria colonization especially in those with tendencies for poor wound dressing. This is accompanied by high rate of inappropriate antibiotic usage. The aim of this study is to characterize microbial pathogens colonizing foot ulcers of diabetic patients in Enugu, Nigeria, and to determine the antibiotic susceptibility of these isolates. Methodology: This was a descriptive cross-sectional study of consecutively enrolled diabetic patients with foot ulcers in two tertiary healthcare facilities in Enugu, Nigeria, between May 2021 and February 2022. A structured questionnaire was used to obtain socio-demographic and clinical data of the patients. Pus samples and/or tissues were collected from the ulcer lesion of each patient for aerobic and anaerobic microbial cultures and biochemical identification using standard conventional techniques. Results: A total of 310 diabetic patients with foot ulcers were recruited into the study, with 62.3% (193/310) males and 37.7% (117/310) females, and mean age of 56.0±13.9 years. Bacteria and yeast were isolated from samples of 280 (90.3%) patients while samples of 30 (9.7%) patients had no microbial growth. Males had higher frequency of microbial isolates (90.7%, 175/193) than females (89.7%, 105/117), while the age group ≤ 40 years had higher frequency of microbial isolates (100%, 43/43) compared to other age groups, although the differences are not statistically significant (p>0.05). The distribution of the isolates showed that 15.7% (44/280) were monomicrobial while 84.3% (236/280) were polymicrobial. The highest single isolate was Bacteroides fragilis with 5.0% (14/280), followed by Staphylococcus aureus with 3.2% (9/280). Bacteroides fragilis and S. aureus occurred as the highest combined bacteria isolates with 5.7% (16/280). Most of the patients were colonized by combination of bacterial isolates. The susceptibility indicates that most of the anaerobic bacteria were sensitive to metronidazole while S. aureus isolates were resistant to ofloxacin at a rate of 65.0%. Conclusion: The findings in this study showed that there is high bacteria and fungi colonization of foot ulcers of diabetic patients in Enugu, Nigeria. Routine care of wounds especially frequent changes of dressing materials and the use of potent antiseptics, are recommended.

Background: Hepatitis C virus (HCV) infection is a global health problem and continues to be a major disease burden in the world, associated with serious health challenges including liver cirrhosis, cancer, lymphomas and death. This study was carried out to determine the prevalence of HCV infection among students of the University of Calabar. Methodology: In a cross-sectional study, 200 students were tested for the presence of anti-HCV antibodies using a rapid immunochromatographic (ICT) assay (CTK Biotech, Inc. USA). Seropositive samples were confirmed using reverse transcriptase-polymerase chain reaction (RT-PCR) assay for detection of HCV RNA. Structured questionnaires were used to collect subjects' socio-demographic data and risk factors of infection. Data were analyzed using SPSS version 16.0, with the level of significance set at p<0.05. Results: Of the 200 students screened, the seroprevalence of HCV was 15.0% (n=30) and 9.5% (n=19) was positive for HCV RNA by RT-PCR assay. The prevalence of anti-HCV antibody was significantly higher in females (18.8%, 12/64) than males (13.2%, 18/136) (x2=3.84, p=0.036). Alcohol consumption (OR=4.67, 95% CI=2.04-10.67, p=0.002), skin piercing (OR=32.99, 95% CI=5.95-72.37, p<0.0001), multiple sexual partners (OR=4.03, 95% CI=1.7-9.6, p=0.0018), and history of blood transfusion (OR=8.00, 95% CI=2.97-21.58, p<0.001) were risk factors significantly associated with HCV infection in the study participants. Conclusion: The findings of 15.0% and 9.5% prevalence of HCV infection by anti-HCV antibody and HCV RNA, respectively in this study, showed that there is relatively high prevalence of HCV infection among the students' population in University of Calabar, Nigeria. Hence, routine medical screening of students for HCV infection using rapid ICT and RT-PCR techniques is hereby recommended.


Contexte: L'infection par le virus de l'hépatite C (VHC) est un problème de santé mondial et continue de représenter un fardeau de morbidité majeur dans le monde, associé à de graves problèmes de santé, notamment la cirrhose du foie, le cancer, les lymphomes et la mort. Cette étude a été réalisée pour déterminer la prévalence de l'infection par le VHC parmi les étudiants de l'Université de Calabar. Méthodologie: Dans une étude transversale, 200 étudiants ont été testés pour la présence d'anticorps anti-VHC à l'aide d'un test immunochromatographique rapide (ICT) (CTK Biotech, Inc., USA). La prévalence des anticorps anti-VHC était significativement plus élevée chez les femmes (18,8%, 12/64) que chez les hommes (13,2%, 18/136) (x 2=3,84, p=0,036). Consommation d'alcool (OR=4,67, IC 95%=2,04-10,67, p=0,002), perçage cutané (OR=32,99, IC 95%=5,95- 72,37, p <0,001) Conclusion: Les résultats de 15,0 % et 9,5 % de prévalence de l'infection par le VHC par les anticorps anti-VHC et l'ARN du VHC, respectivement dans cette étude, ont montré qu'il existe une prévalence relativement élevée de l'infection par le VHC parmi la population étudiante de l'Université de Calabar, au Nigéria. Par conséquent, un dépistage médical de routine des étudiants pour l'infection par le VHC à l'aide de techniques rapides de TIC et de RT-PCR est recommandé.

Objective:To investigate the expression levels and clinical significance of serum microRNA (miR) -155 and miR-135b-5p in patients with peptic ulcer complicated with Helicobacter pylori ( Hp) infection. Methods:A prospective study was conducted, and 263 patients with peptic ulcer were selected consecutively from July 2021 to February 2023 at the Affiliated Hospital of Jining Medical College. Among them, 146 cases were confirmed as Hp infection ( Hp infection group) and 117 cases were not complicated with Hp infection (non Hp infection group) by 14C breath test; type Ⅰ Hp infection was in 110 cases, and type Ⅱ Hp infection was in 36 cases by immunoblotting method. The serum expression levels of miR-155 and miR-135b-5p were detected by real-time fluorescence quantitative polymerase chain reaction, serum gastrin level was detected by radioimmunoassay method, and the serum pepsinogen (PG) Ⅰ and PG Ⅱ were detected by enzyme linked immunosorbent assay. The clinical data were recorded. Multivariate Logistic regression analysis was used to analyze the independent risk factors of Hp infection in patients with peptic ulcer; receiver operating characteristic (ROC) curve was used to analyze the efficacy of serum miR-155 and miR-135b-5p in diagnosis the Hp infection in patients with peptic ulcer. Results:The gastrin, PG Ⅰ, PG Ⅱ, ulcer bleeding rate and recurrence rate in Hp infection group were significantly higher than those in non Hp infection group: (108.47 ± 15.35) ng/L vs. (79.63 ± 10.58) ng/L, (295.41 ± 37.26) pg/L vs. (236.75 ± 29.17) pg/L, (44.08 ± 8.52) pg/L vs. (39.29 ± 6.74) pg/L, 25.34% (37/146) vs. 15.38% (18/117) and 21.92% (32/146) vs. 11.97% (14/117), and there were statistical differences ( P<0.01 or <0.05). The serum miR-155 and miR-135b-5p in Hp infection group were significantly higher than those in non Hp infection group (1.94 ± 0.63 vs. 0.95 ± 0.29 and 1.86 ± 0.57 vs. 1.03 ± 0.31), and there were statistical differences ( P<0.01). The serum miR-155 and miR-135b-5p in patients with typeⅠ Hp infection were significantly higher than those in patients with type Ⅱ Hp infection (2.05 ± 0.66 vs. 1.60 ± 0.54 and 1.97 ± 0.61 vs. 1.52 ± 0.45), and there were statistical differences ( P<0.01). Multivariate Logistic regression analysis result showed that serum miR-155, miR-135b-5p, gastrin and PG Ⅰwere independent risk factors of Hp infection in patients with peptic ulcer ( OR = 1.443, 1.436, 1.452 and 1.438; 95% CI 1.165 to 1.787, 1.146 to 1.799, 1.187 to 1.777 and 1.150 to 1.798; P<0.01). ROC curve analysis result showed that the area under the curve of serum miR-155 combined with miR-135b-5p in the diagnosis of Hp infection in patients with peptic ulcer was significantly greater than that of serum miR-155 and miR-135b-5p alone (0.907 vs. 0.839 and 0.836, Z = 2.57 and 2.81, P = 0.010 and 0.005). Conclusions:The serum levels of miR-155 and miR-135b-5p are high in patients with peptic ulcer complicated with Hp infection, and the combination of the two has high diagnostic value for Hp infection in patients with peptic ulcer.

The prevalence of diabetes mellitus in China has recently been increasing year by year,and spontaneous skin ulcers in diabetic patients,as one of the most serious complications,often develop on the patient's extremities represented by foot ulcers.Due to the complexity and variety of its pathogenesis,it leads to poor clinical outcomes and difficulty in healing.Thus,pa-tients often face the risk of amputation and death.Therefore,the exploration of mechanisms of the vascular pathogenesis of diabetic delayed-healing wounds and targeted screening of therapeutic agents has become a current research hotspot.Herein,in this paper,we briefly review the role of impaired angiogenesis and vascular dysfunction in diabetic skin ulcers,and the research progress of classical hypoglycemic and natural compounds against vascular lesions is preliminarily summarized to provide a theoretical basis for effective clinical treatment.

Objective:To investigate the clinical effect of endoscopic injection of norepinephrine on cerebral infarction complicated by stress-induced gastrointestinal bleeding.Methods:A total of 150 patients with cerebral infarction complicated by stress-induced gastrointestinal bleeding who were admitted to the Intensive Care Unit of Lishui City People's Hospital from October 2020 to October 2021 were included in this study. These patients were randomly divided into a control group and an observation group using the random number table method, with 75 patients in each group. Patients in the control group received routine clinical treatment, while those in the observation group received endoscopic injection of norepinephrine in addition to routine clinical treatment. The hemostatic time, blood transfusion volume, and length of hospital stay were compared between the two groups. The stress index and inflammatory index were compared between the two groups before and after treatment. The hemostatic effect and adverse reactions were evaluated in each group.Results:The hemostatic time, blood transfusion volume, and length of hospital stay in the observation group were (16.16 ± 4.36) hours, (385.35 ± 41.28) mL, and (5.35 ± 1.28) days, respectively, which were significantly shorter or less than (27.27 ± 6.34) hours, (447.07 ± 32.07) mL, and (7.07 ± 2.07) days in the control group ( t = 12.50, 10.22, 6.12, all P < 0.001). After treatment, the levels of cortisol, norepinephrine, antidiuretic hormone, high-sensitivity C-reactive protein, interleukin-6, and tumor necrosis factor-α in the observation group were (288.33 ± 19.53) mmol/L, (29.17 ± 4.26) μg/L, (4.08 ± 1.08) mU/L, (38.27 ± 8.72) ng/L, (6.69 ± 1.35) μg/L, and (6.37 ± 1.51) mg/L, respectively, which were significantly lower than (327.22 ± 22.01) mmol/L, (39.32 ± 5.54) μg/L, (5.36 ± 1.22) mU/L, (51.24 ± 13.23) ng/L, (8.67 ± 2.29) μg/L, and (11.44 ± 3.13) mg/L in the control group ( t = 11.44, 12.57, 6.80, 7.08, 6.45, 12.63, all P < 0.001). The overall response rate of hemostasis in the observation group was 94.67% (71/75), which was significantly higher than 82.67% (62/75) in the control group ( χ2 = 5.37, P < 0.05). The incidence of adverse reactions in the observation group was 8.00% (6/75), which was slightly, but not significantly, lower than 14.67% (11/75) in the control group ( χ2 = 1.66, P > 0.05). Conclusion:Endoscopic injection of norepinephrine for the treatment of cerebral infarction complicated by stress-induced gastrointestinal bleeding can rapidly stop bleeding, effectively reduce inflammation,improve stress index, and be highly safe.

RESUMEN

Objective A model for studying oral ulcers induced by betel nut-extract was constructed in rats.Changes in the structure and diversity of oral flora were observed to explore the involvement of oral flora and local inflammatory factors in the pathogenesis of oral ulcers induced by betel nut-extract and to provide theoretical support for the prevention and treatment of oral ulcers in the clinic.Methods Thirty SD rats were randomly divided into normal,model and intervention groups(Guilin watermelon cream,8 mg/d for 7 days),with 10 rats/group.The oral mucosa of rats was subcutaneously injected with 10 g/mL of betel nut-extract to generate an oral ulcer model.The histomorphological changes were observed,and ulcer area and ulcer scores were assessed.Local oral tissue tumor necrosis factor-α(TNF-α),interleukin(IL)-2 and IL-8 levels were determined.Oral mucosal tissues were sampled for HE staining and analyzed for the structural distribution of oral flora and the diversity of microbial communities using high-throughput sequencing method.Results Compared with rats in the normal group,those in the model group had an increased ulcer area,significantly increased ulcer scores(P<0.01),and significantly increased levels of TNF-α,IL-2 and IL-8 in the oral mucosal tissues(P<0.01).The amount Streptococcus(P<0.05)and Veillonella(P<0.001)in the oral saliva of the model group rats was significantly reduced.The model group rats showed oral mucosal epithelial cell hyperplasia or focal necrosis,mucosal lamina propria edema,and hemorrhage accompanied by mass neutrophil and monocyte infiltration.Compared with the model group rats,the intervention group rats had significantly reduced ulcerated area(P<0.05,P<0.01)and ulcer scores(P<0.05).And oral mucosal tissue levels of TNF-α(P<0.01),IL-2(P<0.05)and IL-8(P<0.05),as well as significantly increased Streptococcus(P<0.001)and Veillonella(P<0.01)and significantly reduced Staphylococcus(P<0.01)in the oral saliva.The degree of lesions in the oral mucosal tissues was significantly improved in the intervention group.Conclusions Betel nut-extract can be used to successfully reproduce a rat model of oral ulcer,and it is speculated that the development of oral ulcers after exposure to betel nut-extract may be related to an imbalance in the oral flora and local tissue inflammatory mediators.
