Introducción. La tuberculosis es una de las enfermedades infecciosas más antiguas y comunes, causada por una bacteria en forma de bastón, o bacilo, llamada Mycobacterium tuberculosis. Esta enfermedad es tratable con antibióticos, los cuales se prescriben durante meses debido a la lenta tasa de crecimiento de las bacterias. Entre los fármacos utilizados la Rifampicina se utiliza terapéuticamente para combatir esta enfermedad, sin embargo, algunos pacientes pueden presentar o desarrollar resistencia a este antibiótico, por lo que, es importante completar el tratamiento para evitar el desarrollo de bacterias farmacorresistentes y la re ocurrencia de la enfermedad. Objetivo. Caracterizar el Mycobacterium tuberculosis resistente a Rifampicina en la provincia de El Oro. Materiales y métodos. Se realizó un estudio con un enfoque cuantitativo, de tipo descriptivo y no experimental. La muestra se obtuvo de la base de datos del laboratorio clínico del área de micobacterias del Hospital Teófilo Dávila en el período 2019 - 2022, quienes luego de aplicar la prueba molecular rápida GeneXpert MTB/RIF o ULTRA determinaron que 48 pacientes presentaron resistencia a la Rifampicina en el tratamiento de la Mycobacterium tuberculosis. Resultados. El reporte del laboratorio evidenció que en el año 2022 se estableció el mayor número de casos de resistencia a Rifampicina para el tratamiento de Mycobacterium tuberculosis, alcanzando el 33,3%; el grupo etario de mayor afectación fue el adulto joven (20 a 49 años) con un 52,1%, y con una frecuencia elevada de 66,7% el sexo masculino. La comorbilidad con mayor predominio estuvo en los pacientes diagnosticados Diabetes Mellitus tipo 2 con un 27,1% y la condición de ingreso de pacientes con resistencia a Rifampicina, son de nuevos casos con 75%. Conclusiones. En la provincia de El Oro, entre el año 2019 - 2022 se presentaron 48 casos resistentes al antibiótico Rifampicina en el tratamiento de la TB, entre ellos el 75% corresponden a una resistencia inicial, es decir, pacientes que no fueron tratados contra la enfermedad, el otro 25% engloba a aquellos pacientes que recayeron en la enfermedad, fracaso o perdida de seguimiento por parte del laboratorio de vigilancia, área de micobacterias del Hospital Teófilo Dávila.


Introduction. Tuberculosis is one of the oldest and most common infectious diseases, caused by a rod-shaped bacterium, or bacillus, called Mycobacterium tuberculosis. This disease is treatable with antibiotics, which are prescribed for months due to the slow growth rate of the bacteria. Among the drugs used, Rifampicin is used therapeutically to combat this disease, however, some patients may present or develop resistance to this antibiotic, therefore, it is important to complete the treatment to avoid the development of drug-resistant bacteria and the reoccurrence of the disease. Objective. To characterize the Mycobacterium tuberculosis resistant to Rifampicin in the province of El Oro. Materials and methods. A quantitative, descriptive and non-experimental study was carried out. The sample was obtained from the database of the clinical laboratory of the mycobacteria area of the Teófilo Dávila Hospital in the period 2019 - 2022, who after applying the rapid molecular test GeneXpert MTB/RIF or ULTRA determined that 48 patients presented resistance to Rifampicin in the treatment of Mycobacterium tuberculosis. Results. The laboratory report showed that in the year 2022 the highest number of cases of resistance to Rifampicin for the treatment of Mycobacterium tuberculosis was established, reaching 33.3%; the age group most affected was young adults (20 to 49 years) with 52.1%, and with a high frequency of 66.7% in the male sex. The most prevalent comorbidity was in patients diagnosed with Diabetes Mellitus type 2 with 27.1% and the condition of admission of patients with resistance to Rifampicin, are new cases with 75%. Conclusions. In the province of El Oro, between 2019 - 2022 there were 48 cases resistant to the antibiotic Rifampicin in the treatment of TB, among them 75% correspond to an initial resistance, that is, patients who were not treated against the disease, the other 25% encompasses those patients who relapsed in the disease, failure or loss of follow-up by the surveillance laboratory, mycobacteria area of the Teófilo Dávila Hospital.


Introdução. A tuberculose é uma das doenças infecciosas mais antigas e mais comuns, causada por uma bactéria em forma de bastonete, ou bacilo, chamada Mycobacterium tuberculosis. A doença pode ser tratada com antibióticos, que são prescritos por meses devido à lenta taxa de crescimento da bactéria. Entre os medicamentos utilizados, a rifampicina é usada terapeuticamente para combater essa doença; no entanto, alguns pacientes podem desenvolver resistência a esse antibiótico, por isso é importante concluir o tratamento para evitar o desenvolvimento de bactérias resistentes aos medicamentos e a recorrência da doença. Objetivo. Caracterizar o Mycobacterium tuberculosis resistente à rifampicina na província de El Oro. Materiais e métodos. Foi realizado um estudo quantitativo, descritivo e não experimental. A amostra foi obtida do banco de dados do laboratório clínico da área de micobactérias do Hospital Teófilo Dávila no período de 2019 a 2022, que, após aplicar o teste molecular rápido GeneXpert MTB/RIF ou ULTRA, determinou que 48 pacientes apresentavam resistência à rifampicina no tratamento de Mycobacterium tuberculosis. Resultados. O laudo laboratorial demonstrou que o maior número de casos de resistência à Rifampicina para o tratamento do Mycobacterium tuberculosis se estabeleceu em 2022, atingindo 33,3%; a faixa etária mais afetada foi a de adultos jovens (20-49 anos) com 52,1%, e com alta frequência de 66,7% no sexo masculino. A comorbidade mais prevalente foi em pacientes diagnosticados com Diabetes Mellitus tipo 2 com 27,1% e a condição de admissão de pacientes com resistência à Rifampicina, são casos novos com 75%. Conclusões. Na província de El Oro, entre 2019 e 2022, houve 48 casos resistentes ao antibiótico Rifampicina no tratamento da TB, entre eles 75% correspondem a uma resistência inicial, ou seja, pacientes que não foram tratados contra a doença, os outros 25% incluem aqueles pacientes que recaíram na doença, falha ou perda de monitoramento pelo laboratório de vigilância, área de micobactérias do Hospital Teófilo Dávila.

El absceso hepático tiene baja incidencia, pero alta mortalidad. Su diagnóstico suele ser tardío dada la variabilidad de presentaciones clínicas. Presentamos tres casos en mujeres, de los cuales dos se manifestaron con síndrome febril con microbiología no precisada, mientras que el tercero con shock séptico secundario a colangitis, aislándose Escherichia coli. Todos fueron manejados con antibioticoterapia endovenosa empírica y drenaje quirúrgico, uno vía laparoscópica, dos percutáneos y uno asociado a una colangio-pancreatografía retrógrada endoscópica. La elección terapéutica se determina de acuerdo a la presencia de rotura, tamaño y/o loculaciones del absceso, combinando antibioticoterapia con métodos de drenaje quirúrgico mínimamente invasivos. Se contrasta y discute la bibliografía disponible, destacando la necesidad de investigaciones actualizadas en Chile.


Liver abscess has low incidence but high mortality. Its diagnosis is often delayed due to the variability of clinical presentations. We present three cases in women, two of which manifested with a febrile syndrome with unspecified microbiology, while the third presented with septic shock secondary to cholangitis, with Escherichia coli isolated. All cases were managed with empirical intravenous antibiotic therapy and surgical drainage, one through laparoscopy, two through percutaneous methods, and one associated with endoscopic retrograde cholangiopancreatography. The therapeutic approach is determined based on the presence of rupture, size, and/or loculations of the abscess, combining antibiotic therapy with minimally invasive surgical drainage methods. We discuss the available literature, emphasizing the need for updated research in Chile.

RESUMEN Introducción. El uso inapropiado de antimicrobianos en las unidades de cuidados intensivos (UCI) contribuye a la resistencia bacteriana. Objetivo. El propósito del presente estudio fue medir el uso y tipo de antimicrobianos en los pacientes hospitalizados en la Unidad de Cuidados Intensivos del Hospital Regional de Loreto (HRL). Métodos. Estudio de tipo descriptivo. Revisamos 120 historias clínicas en pacientes de UCI-HRL desde enero a junio 2023. Resultados. La mayoría de los pacientes usaron antimicrobianos (74,2%), indicados por shock séptico (42,7%), de 1 a 3 antimicrobianos (93,2%), con terapia de 1 a 3 días (45%). Del total de antimicrobianos usados, la mayor proporción fueron bactericidas 89 (90,4%), de la familia de cefalosporinas (33,1%) y carbapenémicos (23,5%); los fármacos más empleados fueron ceftriaxona (26,5%) y meropenem (21,1%) por vía intravenosa (90,4%). Conclusión. La mayor proporción de pacientes hospitalizados usan antimicrobianos de la familia de cefalosporinas y carbapenémicos.


ABSTRACT Introduction. Inappropriate use of antimicrobials in the Intensive Care Unit (ICU) contributes to bacterial resistance. Objective. The purpose of the present study was to measure the use and type of antimicrobials in patients hospitalized in the Intensive Care Unit of the Loreto Regional Hospital (HRL). Methods. Descriptive study. We reviewed 120 medical records in ICU-HRL patients from January to June 2023. Results. Most patients used antimicrobials (74.2%), indicated for septic shock (42.7%), 1 to 3 antimicrobials (93.2%), with therapy of 1 to 3 days (45%). Of the total number of antimicrobials used, the greatest proportion were bactericides 89 (90.4%), from the cephalosporin family (33.1%) and carbapenemics (23.5%); the most commonly used drugs were ceftriaxone (26.5%) and meropenem (21.1%) by the intravenous route (90.4%). Conclusion. The highest proportion of hospitalized patients use antimicrobials of the cephalosporin and carbapenemics family.

Introducción: el lupus eritematoso sistémico (LES) se conoce como una enfermedad autoinmune multisistémica, cuya causa es desconocida, se caracteriza por la presencia de complejos inmunes y autoanticuerpos. En series internacionales se describe una presentación de procesos infecciosos en estos pacientes hasta en un 75 % de los casos, las infecciones ocurren tanto al inicio de la enfermedad como en etapas tardías, y son causa directa de muerte en 30 % a 60 % de los casos y motivo de hospitalización hasta de 30 %. Objetivos: determinar los procesos infecciosos más frecuentes en pacientes con lupus eritematoso sistémico internados en el Servicio de Clínica Médica del Hospital Nacional en el periodo 2022-2023. Metodología: diseño observacional, descriptivo de corte transversal de pacientes con diagnóstico de lupus eritematoso sistémico según criterios de European League Against Rheumatism (EULAR)-2019 que se encuentran internados en el servicio de Clínica Médica del Hospital Nacional (Itaugua-Paraguay) en el periodo de 2022-2023. Resultados: la edad media de los pacientes fue de 34 años, con predominio del sexo femenino en el 88.18 %. Las infecciones del aparato respiratorio fue la más frecuentemente diagnosticada en 50 (45.45 %) pacientes, seguido por la infección de vías urinarias 47 (42.38 %) pacientes, el condicionante de riesgo predominante fue el uso de corticoides en un total 105 (96.40 %) pacientes, se estableció en un total de 97 (88.18 %) pacientes con antibioticoterapia, dentro del aspecto demográfico de la zona urbana 56 (50.90 %) pacientes y rural 54(49.1 %). Conclusión: la infección del aparato respiratorio fue la infección más frecuente, el condicionante de riesgo predominante es el uso de corticoides y recibieron antibioticoterapia la cual en monoterapia fue la más utilizada


Introduction: systemic lupus erythematosus (SLE) is known as a multisystem autoimmune disease, whose cause is unknown, and is characterized by the presence of immune complexes and autoantibodies. In international series, presentation of infectious processes is described in these patients in up to 75 % of cases. Infections occur both at the beginning of the disease and in late stages, and are a direct cause of death in 30 % to 60 % of patients. Cases and reason for hospitalization up to 30 %. Objectives: determine the most frequent infectious processes in patients with systemic lupus erythematosus admitted to the Medical Clinic Service of the Hospital Nacional in the period 2022-2023. Methodology: observational, descriptive cross-sectional design of patients with a diagnosis of systemic lupus erythematosus according to criteria of European League Against Rheumatism (EULAR)-2019 who are admitted to the Medical Clinic service of the Hospital Nacional (Itauguá-Paraguay) in the period of 2022-2023. Results: the average age of the patients was 34 years, with a predominance of the female sex in 88.18 %. Respiratory system infections were the most frequently diagnosed in 50 (45.45 %) patients, followed by urinary tract infection in 47 (42.38 %) patients. The predominant risk factor was the use of corticosteroids in a total of 105 (96.40 %) patients, it was established in a total of 97 (88.18 %) patients with antibiotic therapy, within the demographic aspect of the urban area 56 (50.90 %) patients and rural 54 (49.1 %). Conclusion: respiratory tract infection was the most frequent infection, the predominant risk factor is the use of corticosteroids and they received antibiotic therapy, which in monotherapy was the most used.

In advanced biotechnology, the utilization of enzymes to achieve new or modified compounds with antibacterial, fungicidal, and anti-cancer specifications is crucial. Mushroom lactases are a hopeful biocatalyst for the synthesis and modification of different compounds. They are an accessible and inexpensive enzyme for the preparation of reaction objects and have recently received attention. Laccase purification was performed from basidiomycete Lentinus strigosus (LS) in several stages: Stage 1. On ion-exchange chromatography on TEAE Servacell 23 (400 ml), two distinctly separated laccase activity peaks were observed, eluted from the carrier at 0.21 and 0.27 M NaCl. In order to reduce the loss of enzymes, all fractions with laccase activity were collected, concentrated, and desalted using an ultrafiltration cell (Amicon, United States) with a UM-10 membrane. Stage 2. The resulting preparation with laccase activity was applied to a Q-Sepharose column (60 ml). Two well-separated peaks with laccase activity were obtained during the elution: laccase I (0.12 M NaCl) and laccase II (0.2 M NaCl). Stage 3. In the course of further purification of both enzymes, carried out on anion-exchange carrier Resource Q (6 ml), a broken gradient was used: 0 - 10%, 10 - 20%, and 20 - 100% with 1M NaCl. Stage 4. Both laccase I and laccase II, obtained after Resource Q, were desalted, concentrated to 1 ml each, and applied to a Superdex 75 gel filtration column. As a result, two laccases were obtained in a homogeneous form.


Na biotecnologia moderna, o uso de enzimas para obter compostos novos ou modificados com propriedades antibacterianas, antifúngicas e anticancerígenas é crucial. Lactases de cogumelos são biocatalisadores promissores para síntese e modificação de diferentes compostos, por serem enzimas baratas e disponíveis para a preparação de componentes de reação, e vem recebendo a devida atenção recentemente. A purificação da lacase foi realizada a partir do basidiomiceto Lentinus strigosus em vários estágios: Etapa 1 - na cromatografia de troca iônica em TEAE Servacell 23 (400 ml), foram observados dois picos de atividade da lacase distintamente separados, com eluição do transportador a 0,21 e 0,27 M de NaCl. Para reduzir a perda de enzimas, todas as frações com atividade de lacase foram coletadas, concentradas e dessalinizadas em uma célula de ultrafiltração (Amicon, Estados Unidos) com membrana UM-10; Etapa 2 - a preparação resultante com atividade de lacase foi aplicada a uma coluna Q-Sepharose (60 ml). Durante a eluição, foram obtidos dois picos bem separados com atividade de lacase: lacase I (NaCl 0,12 M) e lacase II (NaCl 0,2 M); Etapa 3 - no decurso da purificação adicional de ambas as enzimas, realizada no Recurso Q de transportador de troca aniônica (6 ml), um gradiente quebrado foi usado: 0-10%, 10-20% e 20-100% com NaCl 1M; Etapa 4 - tanto a lacase I como a lacase II, obtidas após o Recurso Q, foram dessalinizadas e concentradas para 1 ml cada e aplicadas a uma coluna de filtração em gel Superdex 75. Como resultado, duas lacases foram obtidas de forma homogênea.

The present study was conducted to isolate and characterize bacteria from water and soil sample taken from the Lahore Canal at different sites i.e. Mall Road, Mohlanwal and Khera site. Isolated bacterial strains were identified on the basis of morphological and biochemical tests. Identification was confirmed by culturing bacteria on selective media. Antibiotic resistance test was also performed to observe the resistance of bacteria against different antibiotics. Blood agar test was performed for identification of different pathogenic bacteria. The result revealed that water and soil samples of Lahore Canal Lahore from different sites were contaminated with Escherichia coli, Salmonella sp., Vibrio sp., Bacillus spp., Enterococcus sp. and Staphylococcus spp. Due to presence of these pathogens, this water is not suitable for any domestic and irrigation use. Study also revealed that water of the Lahore Canal is harmful for human health as it is contaminated with bacteria that can cause severe disease e.g., Escherichia coli can cause gastroenteritis, Bacillus spp. can cause nausea and vomiting, Enterococcus may infect urinary tract, Salmonella sp. is responsible for Bacteremia, Staphylococcus spp. can cause mild fever and Vibrio sp. can be the reason of cholera. Thus it is rendered unfit for any kind of human use even other than drinking like swimming, bathing, washing etc., until and unless some remedial measures are employed to eradicate pathogenic microorganisms by WASA and LWMS according to standards of WHO. Similarly, it is quite harmful, when and where ever it is used for irrigation without proper treatment.


O presente estudo foi realizado para isolar e caracterizar bactérias de amostras de água e solo retiradas do Canal Lahore, em Lahore, em diferentes locais, ou seja, Mall Road, Mohlanwal e Khera. As cepas bacterianas isoladas foram identificadas com base em testes morfológicos e bioquímicos. A identificação foi confirmada por cultura de bactérias em testes de meios seletivos. O teste de resistência aos antibióticos também foi realizado para observar a resistência das bactérias a diferentes antibióticos. Foi realizado o teste de ágar sangue para identificar diferentes bactérias patogênicas. O resultado revelou que amostras de água e solo do Canal Lahore, Lahore, de diferentes localidades estavam contaminadas com Escherichia coli, Salmonella sp., Vibrio sp., Bacillus spp., Enterococcus sp. e Staphylococcus spp. Por causa da presença desses patógenos, essa água não é adequada para qualquer uso doméstico e de irrigação. O estudo revelou que a água do Canal Lahore é prejudicial à saúde humana, pois está contaminada com bactérias que podem causar doenças graves, por exemplo: Escherichia coli pode ocasionar gastroenterite; Bacillus spp. pode causar náuseas e vômitos; Enterococcus sp. pode infectar o trato urinário; Salmonella sp. é responsável pela bacteremia; Staphylococcus spp. pode causar febre leve; e Vibrio sp. pode ser a razão da cólera. Assim, torna-se imprópria para uso humano, como natação, banho, lavagem etc., até que algumas medidas corretivas sejam empregadas para erradicar microrganismos patogênicos por WASA e LWMS de acordo com os padrões da OMS. Da mesma forma, é bastante prejudicial, quando usada para irrigação sem tratamento adequado.

Resumo Na biotecnologia moderna, o uso de enzimas para obter compostos novos ou modificados com propriedades antibacterianas, antifúngicas e anticancerígenas é crucial. Lactases de cogumelos são biocatalisadores promissores para síntese e modificação de diferentes compostos, por serem enzimas baratas e disponíveis para a preparação de componentes de reação, e vem recebendo a devida atenção recentemente. A purificação da lacase foi realizada a partir do basidiomiceto Lentinus strigosus em vários estágios: Etapa 1 - na cromatografia de troca iônica em TEAE Servacell 23 (400 ml), foram observados dois picos de atividade da lacase distintamente separados, com eluição do transportador a 0,21 e 0,27 M de NaCl. Para reduzir a perda de enzimas, todas as frações com atividade de lacase foram coletadas, concentradas e dessalinizadas em uma célula de ultrafiltração (Amicon, Estados Unidos) com membrana UM-10; Etapa 2 - a preparação resultante com atividade de lacase foi aplicada a uma coluna Q-Sepharose (60 ml). Durante a eluição, foram obtidos dois picos bem separados com atividade de lacase: lacase I (NaCl 0,12 M) e lacase II (NaCl 0,2 M); Etapa 3 - no decurso da purificação adicional de ambas as enzimas, realizada no Recurso Q de transportador de troca aniônica (6 ml), um gradiente quebrado foi usado: 0-10%, 10-20% e 20-100% com NaCl 1M; Etapa 4 - tanto a lacase I como a lacase II, obtidas após o Recurso Q, foram dessalinizadas e concentradas para 1 ml cada e aplicadas a uma coluna de filtração em gel Superdex 75. Como resultado, duas lacases foram obtidas de forma homogênea.

Background: Resistance to antibiotics has increased dramatically over the past few years and has now reached a level that places future patients in real danger. Several factors contribute to this problem, some of them are health workers related and others are community related. This study aimed to assess the awareness of doctors in Elobeid hospital, North Kordofan state, Sudan about the problem, its causes, and its possible preventive measures. Methods: This descriptive, questionnaire-based study was conducted in Elobeid teaching hospital between 2020, February 27 and March 13. The study involved 50 doctors in the internal medicine department. Results: Out of the 50 doctors involved, 49 (98%) agreed that antibiotics resistance is a big progressive health threat in Sudan; 86% of the participants think that selfmedication is the leading cause of antibiotic resistance; and 98% believe that providing good quality culture and sensitivity service in hospitals and health centers is the most effective preventive measure. Regarding education about antibiotics as a part of academic activities in the unit during the last year, 36% of the participants received zero sessions, 68% received 1­5 sessions, and 12% received more than 5 sessions. The assessment of doctor's adherence to the Sudan Standard Treatment Guideline revealed that 28/50 participants (56%) did not see it or hear about it, 17/50 participants (34%) know it but don't comply, and only 5/50 participants (10%) know it and comply with it. Conclusion: Participants show a good awareness about the significance of antibiotics resistance as a health threat, a good awareness about the main causes and the possible preventive measures, a low to moderate frequency of education on antibiotics, and a very poor adherence to the Sudan Standard Treatment Guidelines.

La prescription des antibiotiques et la résistance liée à ces médicaments (considéré comme un motif d'échecs thérapeutiques) dans le domaine médical. L'objectif de cette étude est d'évaluer le niveau de connaissances des chirurgiens-dentistes des Centres Dentaires de l'Université Gamal Abdel Nasser de Conakry sur l'usage des antibiotiques et l'antibiorésistance. Méthodes : Il s'agissait d'une étude transversale de type descriptif d'une durée d'un mois allant 01 mai au 01 Juin 2023 dans les Centre Dentaires de l'Université Gamal Abdel Nasser de Conakry. Résultats : Des résultats de notre étude, il en ressort 66.67% de chirurgiens-dentistes au Centre Dentaire de l'Association France-Guinée (AFD) et 33.33% de chirurgiens-dentistes au Centre Dentaire de Mercy Ships. 70,83% des patients étaient des hommes. La tranche d'âge de 20-29 ans a été la plus représentée avec 62,5%. Le sexe masculin était plus représenté dans 70,83% des cas. La majorité des participants 83,33% avait une expérience professionnelle comprise entre 1-5 ans et 58,33% des répondants n'avaient bénéficié aucune formation continue en antibiothérapie. Avec un score attribué à chaque questionnaire, les chirurgiens-dentistes enquêtés avait un score globalement moyen selon leur niveau de connaissance et leurs attitude et pratique étaient jugées moyenne également. Conclusion : L'antibiothérapie est très fréquemment utilisée en milieu hospitalier en général et plus particulièrement par les chirurgiens-dentistes à cause de la prise en charge des pathologies infectieuses diverses dues à des germes bactériens variés. Ainsi, si cette antibiothérapie n'est pas pratiquée correctement par les chirurgiens-dentistes, ils feront face à une antibiorésistance qui conduira sans aucun doute à un échec de traitement.


The prescription of antibiotics and resistance linked to these drugs (considered a reason for therapeutic failures) in the medical field. The objective of this study is to assess the level of knowledge of dental surgeons Dental Centers of the Gamal Abdel Nasser University of Conakry on the use of antibiotics and antibiotic resistance. Methods: This was a descriptive cross-sectional study lasting one month from May 1 to June 1, 2023 in the Dental Centers of the Gamal Abdel Nasser University of Conakry. Results: From the results of our study, it emerges 66,67% of dental surgeons at the Dental Center of the France-Guinea Association (AFD) and 33,33% of dental surgeons at Mercy Ships Dental Center 70,83% of the patients were men. The age range of 20-29 years old was the most represented with 62,5%. The male gender was more represented in 70,83% of cases. The majority of participants 83,33%, had professional experience of between 1-5 years and 58,33% of respondents had not received any continuing training in antibiotic therapy. With a score assigned to each questionnaire, the dental surgeons surveyed had an overall average score according to their level of knowledge and their attitude and practice were also considered average. Conclusion: Antibiotic therapy is very frequently used in hospitals in general and more particularly by dental surgeons because of the management of various infectious pathologies due to various bacterial germs. Thus, if this antibiotic therapy is not practiced correctly by dentists, they will face antibiotic resistance which will undoubtedly lead to treatment failure.

RESUMEN

OBJECTIVE To establish a UPLC-MS/MS method for the determination of plasma concentration of three carbapenem antibiotics, i.e. ertapenem (ETP), imipenem (IPM) and meropenem (MEM). METHODS After protein precipitation with methanol, the plasma samples were separated by ACQUITY UPLC BEH C18 column (2.1 mm×50 mm, 1.7 μm) using stable isotopes of three antibiotics (ETP-D4, IPM-D4, MEM-D6) as the internal standard. The mobile phases were 98% acetonitrile +2% water +0.1% formic acid and 98% water +2% acetonitrile +0.1% formic acid, by gradient elution. The flow rate was 0.3 mL/min and the column temperature was 40 ℃. Scanning analysis was performed in the positive ion and multiple reaction monitoring mode. RESULTS The method had good specificity, good linearity (r2≥0.993) in the range of 0.2-200, 0.1-100 and 0.1-100 μg/mL of ETP, IPM and MEM, and good intra-batch and inter-batch precision and accuracy (all RE≤5.14%, all RSD≤11.15%), the matrix effect and extraction recovery were consistent (RSD≤12.99%). CONCLUSIONS This study establishes the UPLC-MS/MS method to simultaneously quantify the plasma concentration of ETP, IPM and MEM. The method has the advantages of simple pretreatment, short detection time and small sample quantity to meet clinical requirement.

Objective To design a medical order pre-evaluation tool for perioperative prophylactic application of antibiotics,and to improve the efficiency of antibiotics management in hospitals during the perioperative period.Methods Using the open-source software R as the platform,a web application was built with tidy verse and shiny package based on related documents and guidelines.The discharge records of Affiliated Hospital of North Sichuan Medical College from April 1,2021,to December 31,2022,were retrospectively reviewed using the constructed pre-evaluation tool and compared with previous manual evaluation results using McNemar's Chi-squared test.Results This medical order pre-evaluation tool can quickly complete perioperative antibiotics order sampling,batch pre-evaluation,result statistics,visualization,and result output,and flexibly adjust the evaluation rules according to actual needs.The pre-evaluation tool is more efficient,with a review speed of 13.46 ms per medical record.Among the 2 642 discharge medical records of manual review,there was no significant difference between systematic pre-evaluation and manual evaluation results(ratio of prophylactic use:76.85%vs.78.21%)in terms of the type of use(preventive or curative)(P= 0.078).Among the 1 857 discharge records judged to be prophylactic for both manual and systematic reviews,the difference in unreasonable detection rate(39.90%vs.30.32%)was statistically significant(P<0.001).Among the 63 typical ludicrous medical records confirmed by the review of clinical pharmacists with senior professional titles,60 were judged and limited by the pre-evaluation tool,and the detection rate of typical unreasonable was 95.24%.Conclusions The pre-evaluation tool based on R in this study can improve the efficiency of perioperative antibacterial drug evaluation.The evaluation conclusions and statistical results are reliable and are worthy of further development and application.

Objective To investigate the pathogenic characteristics and use of antibiotics in children with acute osteomyelitis,and to provide evidence for rational drug use in the clinic.Methods The clinical data of 266 children with acute osteomyelitis admitted to the Department of Orthopedics,Children's Hospital Affiliated to Zhengzhou University from January 2019 to December 2022 were retrospectively analyzed,and the basic information,pathogenic microorganisms,drug sensitivity test results,and antibacterial drug use of the children were statistically analyzed.Results In 266 cases of pediatric acute osteomyelitis,all underwent pathogen examination,and 148 cases were cultured for pathogens with a positive detection rate of 55.64%.A total of 154 strains of bacteria were detected.The top three pathogens were Staphylococcus aureus(104 strains,67.53%),Staphylococcus epidermidis(8 strains,5.19%),and Staphylococcus hominis(7 strains,4.55%).The detection rate of methicillin-resistant Staphylococcus aureus(MRSA)was 33.65%.Compared with methicillin-sensitive Staphylococcus aureus(MSSA),there was no significant difference in disease severity and prognosis of infection with MRSA.The utilization rate of antibiotics was 100.00%;227 cases of empirical antibiotics were used alone,accounting for 85.34%,primarily using ceftriaxone.Combination therapy was used in 39 cases,accounting for 14.66%,mainly ceftriaxone combined with vancomycin;the average course of intravenous drugs was 40.20 days.After receiving the drug sensitivity test results in 148 cases,76 cases(51.35%)continued the original treatment plan due to effective treatment.In 37 cases(25.00%),treatment was adjusted based on the drug sensitivity results due to poor treatment outcomes;the drug sensitivity results indicated sensitivity,but the clinical effect was not good in 25 cases accounting for 16.89%,which changed the drug treatment.In 10 cases(6.76%),clinical treatment was effective,leading to a switch to narrow-spectrum antibiotics or a change from combination therapy to monotherapy based on drug sensitivity results.Conclusion Empiric antibacterial therapy can cover common pathogens in children with acute osteomyelitis,and medication regimens can be adjusted according to clinical efficacy and drug sensitivity.However,the course of intravenous antibiotic treatment is too long,so it is necessary to further optimize the timing of transitioning from intravenous to oral administration.

RESUMEN

Fecal microbiota transplantation(FMT)is a therapeutic approach that targets intestinal microorganisms by transplanting fecal microorganisms from healthy individuals into the gastrointestinal tract of diseased individuals,thus restoring the recipient's disordered gastrointestinal microbiota by restructuring the intestinal flora.However,the mechanism of action and adverse effects of FMT in different diseases have not yet been clarified,thus limiting its wide clinical application.Its use still relies on in-depth preclinical studies;however,highly inconsistent or incomplete experimental details provided in current reports,coupled with a lack of authoritative standards and recommendations,seriously affect the interpretation of the study findings and replication of the experimental procedures,as well as hindering the clinical translation of the result.We therefore review and discuss the key steps of recipient selection and graft sample collection,storage,graft material preparation,and grafting route,with the aim of improving the utilization of experimental animals,consumables,and labor,and providing method ological recommendations and references to achieve replicability and standardization of preclinical FMT studies.

Augmented renal clearance(ARC)can be caused by the disease itself,inflammatory state,or therapeutic interventions,and is often associated with severe infections and trauma.Sepsis is one of the most important diseases with high morbidity and mortality in the world.Current studies suggest that ARC is significantly associated with inadequate antimicrobial therapy concentration in patients with sepsis,increasing the risk of clinical treatment failure.It is important to optimize the drug administration strategy for patients with ARC in sepsis,although the current clinical screening of ARC is not yet perfect.This review summarized the literature on changes in blood concentration of antibiotics and administration strategies in patients with sepsis with ARC,in order to provide appropriate medication and clinical guidance for ARC patients.

Objective To investigate the use and application trend of antibiotics in a cancer hospital in Tianjin,so as to provide reference for the rational clinical use of antibiotics.Methods The related data of antibiotics in the hospital from January 2018 to January 2021 were selected,and the utilization rate,defined daily doses(DDDs)of drugs,antibiotics use density(AUD),defined daily cost(DDC)and ranking ratio(B/A)of antibiotics were statistically analyzed.Results In 2018-2021,the annual use of antibiotics,utilization rate of antibiotics and AUD showed a downward trend.The mean annual utilization rate of antibiotics was 22.42%,and the mean annual AUD was 26.79 DDDs/(100 person·d).The top three antibacterial drugs in terms of annual consumption and DDDs were cephalosporins,carbapenems and penicillins.The main antibacterial drugs at all levels were unrestricted.There were 6 antibacterial drugs with DDC>100.Voriconazole had the highest DDC.The B/A of ciprofloxacin sodium chloride,moxifloxacin tablets,Latoxefin sodium,moxifloxacin sodium chloride was close to or equal t0 1,indicating that the synchronization of these antibacterial drugs was good.Conclusion In 2018-2021,the selection and use of antibiotics in the cancer hospital in Tianjin was reasonable,and the clinical application indicators met the national standards.

@#The treatment of patients with advanced lung cancer has been revolutionized with the advent of immunotherapy. However, not all patients can benefit equally from immunotherapy. In recent years, the relationship between intestinal flora and the efficacy of immunotherapy has gradually attracted scholars' attention. During the treatment of immune checkpoint inhibitors, the use of antibiotics, proton pump inhibitors and other drugs will affect the patient's intestinal flora, thus affecting the efficacy of immune checkpoint inhibitors, leading to poor prognosis of patients. This review will discuss that antibiotics and proton pump inhibitors reduce the efficacy of immunotherapy by affecting the diversity of intestinal flora, in order to facilitate the rational use of related drugs in clinical practice and improve the patient's outcomes.

Objective@#To report a case of a suture-related corneal abscess caused by Enterococcus casseliflavus on a phacoemulsification corneal incision. @*Method@#This is a case report.@*Results@#An elderly female underwent complicated phacoemulsification surgery on the left eye requiring sutures to seal the corneal incisions. Suture removal was incomplete at the side port, and a suture abscess developed after 9 weeks. Cultures revealed heavy growth of Enterococcus casseliflavus which was initially treated with an empiric broad-spectrum antibiotic (levofloxacin 1.5%) administered via intrastromal and subconjunctival injections, followed by culture-guided topical and oral medications. The corneal infection resolved but significant corneal thinning led to perforation, requiring a modified Gunderson flap procedure to repair the defect.@*Conclusion@#Intrastromal and subconjuctival injection of levofloxacin 1.5% may be a viable empiric treatment option for Enterococcus casseliflavus suture-related corneal abscess.

ObjectiveTo understand the usage of antibiotics in outpatients in a county of Zhejiang Province from 2015 to 2020, so as to provide a basis for further standardizing the clinical application of antibiotics in this region. MethodsOutpatient records and prescription records of 146 public medical institutions from 2015 to 2020 were extracted from the electronic medical record data sharing platform system of medical institutions in a county of Zhejiang Province. The utilization rates and the number of types of antibiotics used in outpatient patients were described. According to the drug anatomy, therapeutic and chemical classification system (ATC), clinical application classification and dosage form, the use status of different types of antibiotics was observed. The annual percentage change (APC) was used to analyze the time trend of antibiotic use. Among the antibiotic prescriptions in each year, the proportions of prescriptions aimed for different use reasons and patients were compared to analyze the changes in the structures of antibiotic prescriptions. ResultsDuring 2015‒2020, a total of 2 861 438 prescriptions were issued in146 primary public medical institutions in this county, including 314 642 prescriptions for antibiotics, and the overall utilization rate of antibiotics was 11%. The utilization rate of antibiotics showed a decreased trend across the years (from 14.23% in 2015 to 7.59% in 2020,APC=-11.51%, 95%CI=-0.01%‒-21.7%, t=-12.05, P<0.001) and an obvious seasonal variation character (higher in winter and spring, lower in summer and autumn). Cefoxitin sodium for injection was the most used antibiotic in 2015, and cefuroxime was the most used antibiotic from 2015 to 2020, respectively. The utilization rates of cephalosporins (APC=-11.06%, 95%CI=0‒-20.89%, t=-22.233, P<0.001), quinolones (APC=-9.74%, 95%CI=0‒-18.53%, t=-7.652, P=0.002), macrolides (APC=-17.52%, 95%CI=0‒-17.52%, t=-3.626, P=0.022) and lincoamides (APC=-49.01%, 95%CI=0‒-74%, t=-3.35, P=0.029) showed a decreasing trend across the years. There were differences in the distribution of drug use reasons (χ2=9 458.427, P<0.001), as well as the age (χ2=5 421.709, P<0.001) and sex (χ2=245.322, P<0.001) of patients. During the observation period, 50.69% of antibiotic prescriptions were used to treat respiratory diseases, with the highest proportion of 57.67% in 2015 and the lowest proportion of 34.93% in 2020. 58.37% of patients were aged 50‒70 years, with a decreasing proportion of patients under 50 years (17.83% in 2015, 6.64% in 2020) and an increasing proportion of patients over 70 years (24.1% in 2015, 38.41% in 2020). ConclusionThe utilization rate of antibiotics used in outpatients from primary public medical institutions in a county of Zhejiang Province shows a decreasing trend across the years, but higher than that of secondary and above hospitals in China, and also higher than that of primary medical institutions in economically-developed areas. The management and monitoring of the clinical use of antibiotics in the county should be actively strengthened.
