SUMMARY: Endometriosis, defined as the presence of endometrial glands and stroma outside the uterine cavity, is a chronic inflammatory condition that affects between 8 % and 44 % of women of reproductive age. Occasionally it presents as a sensitive mass in the abdominal wall, in relation to a surgical scar. On the other hand, in the most severe stage of endometriosis, intestinal involvement is common, and endometriotic disease of the appendix may be present even in appendices with macroscopically normal appearance. Simultaneous affectation of both locations is very rare. Nevertheless, treatment of choice is the excision of both lesions with safety margins. The aim of this manuscript was to report a case of simultaneous deep endometriosis affecting the total abdominal wall and vermiform appendix, resected in a single surgical procedure, which subsequently required abdominal wall repair with mesh plasty. Since desmoid tumors and endometriosis share similar clinical signs and unspecific imaging exams, both options should be considered in case of abdominal wall mass in female patients of childbearing age, especially if they have a history of uterine-related surgery.


La endometriosis se define como la presencia de glándulas endometriales y estroma fuera del útero. Es una afección crónica que afecta entre el 8 % y el 44 % de las mujeres en edad reproductiva. Ocasionalmente se presenta como una masa sensible en la pared abdominal, en relación con una cicatriz quirúrgica. Por otro lado, en su estadio más grave de la endometriosis, la afectación intestinal es común y puede afectar al apéndice, pudiendo estar presente incluso en apéndices de apariencia macroscópicamente normal. La afectación de ambas localizaciones simultáneamente es muy infrecuente. Sin embargo, el tratamiento de ambas lesiones es su exéresis quirúrgica con márgenes de seguridad. El objetivo de este manuscrito fue reportar un caso de endometriosis profunda simultánea que afectaba la pared abdominal total y el apéndice vermiforme, las que fueron resecadas en un solo tiempo quirúrgico, incluyendo posteriormente reparación de la pared abdominal con uso de malla. Dado que los tumores desmoides y la endometriosis comparten signos clínicos similares y exámenes de imágenes inespecíficos, se deben considerar ambas opciones en caso de masas en la pared abdominal de mujeres en edad fértil, especialmente si tienen antecedentes de cirugía relacionada con el útero.

Objetivo: Estudiar la relación que existe entre algunos factores predisponente a desarrollar endometriosis y síntomas más frecuentes que manifiestan las pacientes. Métodos: Estudio observacional longitudinal, retrospectivo y descriptivo, mediante la revisión de 455 historias tomadas 560 pacientes que acudieron a la consulta de endometriosis y realizando diagnóstico definitivo en el Hospital Universitario "Dr. Ángel Larralde" y consulta privada, en Venezuela, estado Carabobo, en el periodo de enero 2006 a mayo 2023. Resultados: El diagnóstico se realizó en un 47,1 % de las pacientes con edades entre 26 a 35 años. La menarquia se presentó entre los 8 y los 12 años (el 69 %). El 63 % no tenía embarazos. La dismenorrea primaria fue más frecuente en el 63,07 %. El dolor pélvico y dispareunia en el 48 %, los síntomas digestivos en el 21 %. La localización más frecuente fue en ovario (78,7 %). Conclusión: La endometriosis, enfermedad inflamatoria benigna, estrógeno dependiente, se caracteriza por presencia de glándulas y estroma endometrial ectópico, usualmente acompañados de fibrosis. Se manifiesta en el periodo reproductivo, su principal síntoma es el dolor. Se localiza con mayor frecuencia en la pelvis y de esta en los ovarios, en intestino, vejiga, hígado, pulmones, cerebro, pared abdominal y periné. No hay tratamiento para su cura, pero si para los síntomas y retardar su progreso. Su diagnóstico por su desarrollo multifactorial y manifestaciones clínicas en muchas oportunidades se realiza en forma tardía(AU)


Objective: To study the relationship that exists between some predisposing factors to develop endometriosis and the most frequent symptoms manifested by patients. Methods: Longitudinal, retrospective and descriptive observational study, through the review of 455 records taken from 560 patients who attended the endometriosis consultation and made a definitive diagnosis at the "Dr. Ángel Larralde" and private consultation, in Venezuela, Carabobo state, in the period from January 2006 to May 2023. Results: The diagnosis was made in 47.1% of the patients aged between 26 and 35 years. Menarche occurred between 8 and 12 years of age (69%). 63% did not have pregnancies. Primary dysmenorrhea was more frequent in 63.07%. Pelvic pain and dyspareunia in 48%, digestive symptoms in 21%. The most frequent location was in the ovary (78.7%). Conclusion: Endometriosis, a benign, estrogen-dependent inflammatory disease, is characterized by the presence of ectopic endometrial glands and stroma, usually accompanied by fibrosis. It manifests itself in the reproductive period, its main symptom is pain. It is located most frequently in the pelvis and from this in the ovaries, intestine, bladder, liver, lungs, brain, abdominal wall and perineum. There is no treatment to cure it, but there is treatment for the symptoms and slowing its progress. Its diagnosis due to its multifactorial development and clinical manifestations is often made late(AU)

INTRODUCTION: Pulp stones (PS) are calcifications commonly found in the pulp tissue that may be associated with systemic diseases. OBJECTIVE: To evaluate the association between PS and systemic diseases. METHODS: A case-control study with the inclusion of individuals from 18 to 65 years of age, of both sexes. Analysis was made of 1047 digital panoramic radiographs. The controls could not have any teeth with PS; the cases were the contrary. A questionnaire comprising demographic, habit, and general health (diabetes, problems with blood vessels, altered cholesterol level, heart attack, kidney or gallbladder stone, arthritis, or autoimmune disease, and for women, endometriosis, and ovarian cyst). Data were submitted to the Student's t-test to identify differences between groups about sex and age. The Chi-square test was applied to the cross-tabulation. The analyses were performed using SPSS®, version 25.0, with a 5% significance level. RESULTS: 490 patients participated (242 cases and 248 controls). There was no difference between groups for the sex (p=0.966) and age (p=0.186). Only "kidney stone" was associated with the case group (p=0.001), being almost three times higher when compared to the control group. No significant differences were found in females about the presence or absence of PS (p>0.05). CONCLUSÃO: In this research, it is suggested the existence of an association between kidney stones and the presence of pulp stones.


INTRODUÇÃO: Nódulos pulpares (NP) são calcificações comumente encontradas no tecido pulpar que podem estar associadas a doenças sistêmicas. OBJETIVO: Avaliar a associação entre NP e doenças sistêmicas. MÉTODOS: Estudo caso-controle com inclusão de indivíduos de 18 a 65 anos de idade, de ambos os sexos. Foram analisadas 1047 radiografias panorâmicas digitais. Os controles não poderiam ter dentes com NP; os casos foram o contrário. Foi aplicado um questionário aos participantes, contendo variáveis demográficas, de hábitos e de saúde geral (diabetes, problemas com vasos sanguíneos, nível de colesterol alterado, ataque cardíaco, cálculo renal ou biliar, artrite ou doença autoimune, e para as mulheres, endometrioses e cisto no ovário). Os dados foram submetidos ao teste t de Student para identificar diferenças entre os grupos em relação ao sexo e à idade. O teste Qui-quadrado foi aplicado para a tabulação cruzada. As análises foram realizadas no SPSS®, versão 25.0, com nível de significância de 5%. RESULTADOS: Participaram 490 pacientes (242 casos e 248 controles). Não houve diferença entre os grupos para sexo (p=0,966) e idade (p=0,186). Apenas "cálculo renal" associou-se ao grupo caso (p=0,001), sendo quase três vezes maior quando comparado ao grupo controle. Não foram encontradas diferenças significativas no sexo feminino em relação à presença ou ausência de PS (p>0,05). CONCLUSÃO: Nesta pesquisa, sugere-se a existência de uma associação entre cálculos renais e presença de Nódulos pulpares.

Antecedentes: En las mujeres con endometriosis, son frecuentes las disfunciones sexuales, particularmente el dolor/dispareunia. Objetivo: Evaluar y caracterizar la prevalencia de disfunciones sexuales en las mujeres con endometriosis en Armenia (Quindío), Colombia. Método: Estudio de corte transversal, en 137 mujeres mayores de 18 años con diagnóstico de endometriosis confirmada histológicamente, atendidas en tres clínicas de alta complejidad entre 2017 y 2022. Se utilizó como instrumento el Índice de Función Sexual Femenina (IFSF). Resultados: La edad promedio de las participantes fue de 34,68 ± 7,32 años. La prevalencia de disfunciones sexuales fue del 75,91%, caracterizando en primer lugar el dolor/dispareunia (75,91%), seguido de disminución del deseo sexual (56,93%) y en tercer lugar problemas de lubricación (54,01%). El promedio en la puntuación del IFSF fue de 20,82 ± 7,22 y la proporción de mujeres con puntuación ≥ 26,55 fue del 24,08%. La mediana en el número de disfunciones sexuales fue 3 (rango: 1 y 6). Conclusiones: En las mujeres con endometriosis se evidencia una alta prevalencia de disfunciones sexuales, equivalente a tres cuartas partes de las afectadas. Al momento de atender una mujer con este padecimiento se requiere evaluar sus efectos en la función sexual para poder implementar intervenciones terapéuticas oportunas.


Background: Sexual dysfunctions, particularly pain/dyspareunia, are common in women with endometriosis. Objective: To evaluate and characterize the prevalence of sexual dysfunctions in women with endometriosis in Armenia (Quindío), Colombia. Method: Cross-sectional study in 137 sexually active women older than 18 years with a diagnosis of histologically confirmed endometriosis, attended in three highly complex clinics between 2017 and 2022. The Female Sexual Function Index (FSFI) was used as an instrument. Results: The average age of the participants was 34.68 ± 7.32 years. The prevalence of sexual dysfunctions reported 75.91%, characterizing pain/dyspareunia in the first place (75.91%), followed by decreased sexual desire (56.93%) and thirdly problems of lubrication (54.01%). The average score of the FSFI was 20.82 ± 7.22; the proportion of women with a score ≥ 26.55 was 24.08%. The median in the number of sexual dysfunctions reached 3 (range: 1 and 6). Conclusions: In women with endometriosis there is evidence of a high prevalence of sexual dysfunctions, equivalent to three-quarters of the affected women. When caring for a woman with this condition, it is necessary to evaluate its effects on sexual function in order to implement timely and effective therapeutic interventions.

Aim: this study aimed to evaluate the effects of surgical treatment for endometriosis on the metabolic profile of women diagnosed with deep endometriosis. Methods: we conducted a prospective observational study with a sample of 30 women in the menacme diagnosed with deep endometriosis who underwent videolaparoscopic surgery in a reference center in Brazil between October 2020 and December 2021. A total of 30 women performed clinical and laboratory tests regarding their metabolic profile on two occasions, during preoperative tests and six months after video-laparoscopy. Results: patients had lower average levels of Total Cholesterol (TC), Low-Density Cholesterol (LDL-c), Triglycerides (TGC), and Fasting Glycemia (FG) after the surgical procedure. The average TC level was 8.2% lower after surgery, LDL-c was 12.8% lower, TGC was 10.9% lower, and FG was 7.3% lower. The results showed a statistically significant difference for all these parameters (p < 0.001). Conclusions: video-laparoscopy was associated with a favorable lipid profile compared to the preoperative lipid profile, with a significant improvement in the average levels of LDL-c, HDL-c, TC, TGC, and FG. Long-term follow-up studies are needed to determine whether surgical treatment for endometriosis can improve the metabolic parameters of women with endometriosis and favor a lower predisposition to atherogenesis.


Objetivo: Aeste estudo teve como objetivo avaliar os efeitos do tratamento cirúrgico da endometriose no perfil metabólico de mulheres com diagnóstico de endometriose profunda. Métodos: foi realizado um estudo observacional prospectivo com uma amostra de 30 mulheres na menacme, com diagnóstico de endometriose profunda, que foram submetidas à videolaparoscopia em um centro de referência no Brasil, entre outubro de 2020 e dezembro de 2021. As mulheres realizaram exames clínicos e laboratoriais quanto ao seu perfil metabólico em duas ocasiões, durante exames pré-operatórios e seis meses após a videolaparoscopia. Resultados: as pacientes apresentaram níveis médios mais baixos de Colesterol Total (CT), Colesterol de Baixa Densidade (LDL-c), Triglicerídeos (TGC) e Glicemia de Jejum (GJ) após o procedimento cirúrgico. O nível médio de CT foi 8,2% menor após a cirurgia, o LDL-c foi 12,8% menor, o TGC foi 10,9% menor e a GJ foi 7,3% menor. Os resultados mostraram diferença estatisticamente significativa para todos esses parâmetros (p < 0,001). Conclusões: a videolaparoscopia foi associada a um perfil lipídico favorável em comparação ao perfil lipídico pré-operatório, com melhora significativa nos níveis médios de LDL-c, HDL-c, CT, TGC e GJ. Estudos de acompanhamento a longo prazo são necessários para determinar se o tratamento cirúrgico da endometriose pode melhorar os parâmetros metabólicos de mulheres com endometriose e favorecer uma menor predisposição à aterogênese.

Background@#Current medical management of endometriosis leads to suppression of ovulation and will not be helpful for women with endometriosis who are desirous of pregnancy. Thus, drugs that can both treat endometriosis and its associated infertility are highly warranted.@*Objective@#Anti-angiogenic agents are potential drugs for patients with endometriosis and infertility. Among these drugs, dopamine agonist (DA) is promising since it does not interfere with ovulation, is safe, and not teratogenic. The aim of the study is to determine the efficacy and safety of DA for improving reproductive outcomes in women with endometriosis and infertility.@*Methods@#A qualitative narrative review was done from inception to July 31, 2022 using the appropriate MeSH terms in PubMed, Cochrane Database of Systematic Reviews, the Cochrane Central Register of Controlled Trials, ClinicalTrial.gov, and World Health Organization International Clinical Trials Registry Platform. Date analysis was through qualitative analysis and synthesis of researches and their outcome measures.@*Results@#No studies used the core outcomes for trials evaluating treatments for infertility associated with endometriosis. All the included articles in the review supported the possible anti-angiogenic effects of DA on the vascular endothelial growth factor [VEGF] /VEGF receptor system. The use of DA does not have an effect on ovulation and menstrual cyclicity. Studies on safety profile of DA were consistent with existing data.@*Conclusion@#Most of studies reviewed demonstrated that DA were effective in reducing endometriotic lesions. However, further research is required to establish whether this anti-angiogenic effect can improve reproductive outcomes in women with endometriosis-associated infertility.

@#Detection of an adnexal mass during pregnancy has increased during the last decade due to liberal use of sonographic examination during the first trimester. While most are benign or physiologic, the probability of a malignant tumor should always be considered. Deciduosis, a phenomenon related to pregnancy, refers to the presence of decidua outside the uterine cavity, most commonly seen within the pelvis. It is a benign condition, which usually regresses during the postpartum period. However, during pregnancy, this ectopic decidua may enlarge and mimic malignancy.

Introduction.L'endométriose est une pathologie mal connue et sous explorée en Afrique en particulier en Côte d'Ivoire. L'objectif de notre étude était d'étudierles caractéristiques épidémio-cliniques et à l'imagerie par résonnance magnétique(IRM)de l'endométriose pelvienne à Abidjan.Méthodologie. Il s'agissait d'une étude prospective et descriptive qui s'est déroulée à Abidjan sur une durée 15 mois. Les examens ont été réalisés sur une IRM 1,5 T avec les séquences conventionnelles. Les patientes retenues ont réalisé une IRM du pelvis pour suspicion d'endométriose pendant la période. N'ont pas été retenues les patientes qui ont réalisés l'examen pour d'autres affections gynécologiques. L'ensemble des données ont été recueillies à partir des comptes rendus d'IRM des patientes. Les paramètres épidémio-cliniques; les paramètres IRM des lésions endométriosiques ont été étudiés. Nous avons utilisé le test de khi carré pour vérifier le lien entre certains facteurs.Résultats.Nous avons enregistré 68 patientes dont l'âge moyen était de 38,61 ans. L'adénomyose représentait la localisation la plus fréquente (67,65%) suivi de l'atteinte ovarienne (35,29%). Dans l'adénomyose, la zone jonctionnelle était inférieure à 20 mm dans 44,19%. L'endométriose ovarienne a été objectivée chez 24 patientes, soit 35,29% des cas. Une endométriose sous péritonéale a été objectivée dans 19,12% des cas. L'atteinte tubaire était de 10,29%. L'association endométriose et fibrome a été observé chez 44,12% des patientes. Le risque d'adénomyose était élevé après 40 ans p < 0,005.Conclusion.L'IRM apparait comme l'examen d'imagerie de référence dans le diagnostic et le bilan d'extension de l'endométriose pelvienne. A Abidjan, le diagnostic d'endométriose se fait à un âge avancé.


Introduction.Endometriosis is a poorly understood and under-explored condition in Africa, particularly in Ivory Coast. The aim of our study was to investigate the epidemiological and clinical characteristics, as well as magnetic resonance imaging (MRI) features of pelvic endometriosis in Abidjan. Methodology.This was a prospective and descriptive study conducted in Abidjan over a period of 15 months. The examinations were performed on a 1.5 T MRI machine using conventional sequences. Patients who underwent pelvic MRI for suspected endometriosis during the study period were included, while those who underwent the examination for other gynecological conditions were excluded. All data were collected from the MRI reports of the patients. Epidemiological and clinical parameters, as well as MRI parameters of endometriotic lesions, were analyzed. The chi-square test was used to verify the association between certain factors. Results.We included 68 patients with a mean age of 38.61 years. Adenomyosis was the most common localization (67.65%), followed by ovarian involvement (35.29%). In adenomyosis, the junctional zone was less than 20 mm in 44.19% of cases. Ovarian endometriosis was documented in 24 patients, accounting for 35.29% of cases. Subperitoneal endometriosis was observed in 19.12% of cases. Tubal involvement was seen in 10.29% of cases. The co-occurrence of endometriosis and fibroids was observed in 44.12% of patients. The risk of adenomyosis was higher after the age of 40 (p < 0.005). Conclusion.MRI appears to be the imaging modality of choice for diagnosing and assessing the extent of pelvic endometriosis. In Abidjan, endometriosis is diagnosed at an older age.

Abstract Objective To evaluate the effects of surgical treatment of deep endometriosis on the metabolic profile, quality of life and psychological aspects. Methods Prospective observational study, carried out with women of reproductive age diagnosed with deep endometriosis, treated in a specialized outpatient clinic, from October/2020 to September/2022, at a University Hospital in Fortaleza - Brazil. Standardized questionnaires were applied to collect data on quality of life and mental health, in addition to laboratory tests to evaluate dyslipidemia and dysglycemia, at two moments, preoperatively and six months after surgery. The results were presented using tables, averages and percentages. Results Thirty women with an average age of 38.5 years were evaluated. Seven quality of life domains showed improved scores: pain, control and impotence, well-being, social support, self-image, work life and sexual relations after surgery (ES ≥ 0.80). There was an improvement in mental health status with a significant reduction in anxiety and depression postoperatively. With the metabolic profile, all average levels were lower after surgery: total cholesterol 8.2% lower, LDL 12.8% lower, triglycerides 10.9% lower, and fasting blood glucose 7.3% lower (p < 0.001). Conclusion Surgical treatment of deep endometriosis improved the quality of life and psychological aspects of patients. The lipid profile of patients after laparoscopy was favorable when compared to the preoperative lipid profile.

Abstract Objective: To evaluate and compare the sexual function and pelvic floor muscles (PFM) function of women with endometriosis and chronic pelvic pain (CPP) with and without Myofascial Pelvic Pain Syndrome (MPPS). Methods: Cross-sectional study conducted between January 2018 and December 2020. Women with deep endometriosis underwent assessments for trigger points (TP) and PFM function using the PERFECT scale. Electromyographic activity (EMG) and sexual function through Female Sexual Function Index (FSFI) were assessed. Statistical analyses included chi-square and Mann-Whitney tests. Results: There were 46 women. 47% had increased muscle tone and 67% related TP in levator ani muscle (LAM). Weakness in PFM, with P≤2 was noted in 82% and P≥3 in only 17%. Incomplete relaxation of PFM presented in 30%. EMG results were resting 6.0, maximal voluntary isometric contraction (MVIC) 61.9 and Endurance 14.2; FSFI mean total score 24.7. We observed an association between increased muscle tone (P<.001), difficulty in relaxation (P=.019), and lower Endurance on EMG (P=.04) in women with TP in LAM. Participants with TP presented lower total FSFI score (P=.02). TP in the right OIM presented increased muscle tone (P=.01). TP in the left OIM presented lower values to function of PFM by PERFECT (P=.005), and in MVIC (P=.03) on EMG. Conclusion: Trigger points (TP) in pelvic floor muscles (PFM) and obturator internus muscle (OIM) correlates with poorer PFM and sexual function, particularly in left OIM TP cases. Endometriosis and chronic pelvic pain raise muscle tone, weaken muscles, hinder relaxation, elevate resting electrical activity, lower maximum voluntary isometric contraction, and reduce PFM endurance.

ABSTRACT Objective: To verify whether there are pathological dysfunctions in the cornea of patients with endometriosis. Methods: Case-control research with a quantitative approach that compared topographic and tomographic examinations of the cornea of patients with a laparoscopic diagnosis of endometriosis, without the use of hormonal medications, to the control group. Results: We analyzed 78 eyes, 34 from the endometriosis group and 44 from the control group. The loss of orthogonality between the axes of the corneal curvatures was more frequent in the group with endometriosis (p = 0.0744). The difference between the mean keratometric measurements of the two eyes was significantly greater in the control group (p = 0.0204). In the tomographic findings, the group with endometriosis presented higher means of posterior elevation compared to the control group (p = 0.0060). Conclusion: The results do not allow us to conclude that women with endometriosis have a higher risk of developing corneal ectasia, although the posterior elevation map demonstrated a greater posterior curvature of the cornea in this group, with a statistically significant difference. However, an isolated increase in the posterior elevation map does not have good diagnostic accuracy.


RESUMO Objetivo: Verificar se há disfunções patológicas na córnea de pacientes portadoras de endometriose. Métodos: Pesquisa do tipo caso-controle de abordagem quantitativa, que comparou exames topográficos e tomográficos da córnea de pacientes com diagnóstico laparoscópico de endometriose, sem uso de medicações hormonais, ao grupo controle. Resultados: Foram analisados 78 olhos, 34 do grupo com endometriose e 44 do grupo controle. A perda da ortogonalidade entre os eixos das curvaturas corneanas foi mais frequente no grupo com endometriose (p = 0,0744). A diferença entre as médias das medidas ceratométricas dos dois olhos foi significativamente maior no grupo controle (p = 0,0204). Nos achados tomográficos, o grupo com endometriose apresentou maiores médias de elevação posterior em relação ao controle (p = 0,0060). Conclusão: Os resultados não permitem concluir que portadoras de endometriose têm maior risco de desenvolver ectasia corneana, embora o mapa de elevação posterior tenha demonstrado maior curvatura posterior da córnea nesse grupo, com diferença estatisticamente significativa. Contudo, um aumento isolado no mapa de elevação posterior não possui boa acurácia diagnóstica.

Este trabalho tem o objetivo de analisar as concepções de maternidade para mulheres inférteis de diferentes níveis socioeconômicos que estão em tratamento de reprodução assistida. Trata-se de um estudo qualitativo, descritivo, que utilizou como instrumento uma entrevista semiestruturada e contemplou temas como o significado de família, desejo/expectativas sobre filho e gestação e expectativas sobre a maternidade. Participaram da pesquisa 48 mulheres inférteis acima de 35 anos que usam tecnologias de reprodução assistida de alta complexidade em instituições privada e pública. Os dados foram tratados pela análise de conteúdo em que emergiram os temas: representações sociais da família; representações sociais da maternidade; expectativas com a gestação e os modelos maternos; e o filho imaginado. As participantes representaram a família de forma positiva, como um sistema de suporte, de fundação e origem de amor, configurando-a como um laço social. Por outro lado, as concepções de família com base na consanguinidade também estiveram presentes, representando a família pela perpetuação da espécie e pela importância do laço biológico. A maternidade foi marcada por significativa idealização, sendo vista como um papel gratificante e de realização da feminilidade. O peso da cobrança social para procriar também foi sentido como um dever a cumprir e que, na impossibilidade de se realizar, gera sentimentos de inferioridade, menos-valia, impotência e inadequação perante a sociedade, o que reforça o estigma da infertilidade. Tais resultados apontam a importância de reflexões sobre o papel da mulher na nossa cultura, visto que a maternidade é ainda utilizada como medida para o sucesso ou fracasso feminino. Faz-se necessário também refletir sobre a possibilidade da maior inserção do trabalho psicológico na reprodução assistida, visto a carga emocional e social envolvidas nesse processo.(AU)


This study aimed to analyze the conceptions of motherhood for infertile women from different socioeconomic levels who are undergoing assisted reproduction treatment. This is a qualitative and descriptive study that used a semi-structured interview as an instrument and included topics such as the meaning of family and desires/expectations about the child, pregnancy, and motherhood. A total of 48 infertile women over 35 years of ages using high-complexity assisted reproductive technologies in private and public institutions participated in this research. The data were treated by content analysis in which the following themes emerged: family social representations; social representations of motherhood; expectations with pregnancy and maternal models; and the imagined son. Participants represented the family in a positive way as a support system and the foundation and origin of love, embracing the family as a social bond. On the other hand, the family concepts based on inbreeding were also present, representing the family by perpetuation of the species and the importance of biological bonds. Motherhood was marked by significant idealization, being seen as a gratifying role and the fulfillment of femininity. The weight of the social demand to procreate was also felt as a duty to be fulfilled that, in the impossibility of carrying it out, generates feelings of inferiority, worthlessness, impotence, and inadequacy toward society, which reinforce the stigma of infertility. Results point to the necessary reflections on the role of women and our culture since Motherhood is still used as a measure of female success or failure. They also point to a reflection on the possibility of greater inclusion of psychological work in assisted reproduction given the emotional and social burden involved in this process.(AU)


Este estudio tuvo como objetivo analizar las concepciones de maternidad de mujeres infértiles, de diferentes niveles socioeconómicos, que se encuentran en tratamiento de reproducción asistida. Se trata de un estudio cualitativo, descriptivo, que utilizó como instrumento una entrevista semiestructurada e incluyó temas como el sentido de la familia, deseos/expectativas sobre el hijo y el embarazo y expectativas sobre la maternidad. Participaron en la investigación un total de 48 mujeres infértiles, mayores de 35 años, usuarias de tecnologías de reproducción asistida de alta complejidad en instituciones públicas y privadas. Los datos se sometieron a análisis de contenido del cual surgieron los temas: representaciones sociales familiares; representaciones sociales de la maternidad; expectativas con el embarazo y modelos maternos; hijo imaginado. Las participantes representaron a la familia de manera positiva, como sistema de apoyo, fundamento y origen del amor, configurándola como vínculo social. Por otro lado, también estuvieron presentes las concepciones familiares basadas en la consanguinidad, representando a la familia para la perpetuación de la especie y la importancia del vínculo biológico. La maternidad estuvo marcada por una importante idealización, vista como un rol gratificante y de realización de la feminidad. También se sintió el peso de la demanda social de procrear como un deber que cumplir y que, ante la imposibilidad de realizarlo, genera sentimientos de inferioridad, desvalorización, impotencia e inadecuación en la sociedad, lo que refuerza el estigma de la infertilidad. Por tanto, son necesarias reflexiones sobre el papel de la mujer en nuestra cultura, ya que la maternidad se sigue utilizando como medida del éxito o fracaso femenino. También se reflexiona sobre la posibilidad de una mayor inclusión del trabajo psicológico en la reproducción asistida dada la carga emocional y social que implica este proceso.(AU)

Abstract Objective: To evaluate the association between clinical and imaging with surgical and pathological findings in patients with suspected neuroendocrine tumor of appendix and/or appendix endometriosis. Methods: Retrospective descriptive study conducted at the Teaching and Research Institute of Hospital Israelita Albert Einstein, in which medical records and databases of patients with suspected neuroendocrine tumor of appendix and/or endometriosis of appendix were analyzed by imaging. Results: Twenty-eight patients were included, all of which had some type of appendix alteration on the ultrasound examination. The pathological outcome of the appendix found 25 (89.3%) lesions compatible with endometriosis and three (10.7%) neuroendocrine tumors. The clinical findings of imaging and surgery were compared with the result of pathological anatomy by means of relative frequency. Conclusion: It was possible to observe a higher prevalence of appendix endometriosis when the patient presented more intense pain symptoms. The image observed on ultrasound obtained a high positive predictive value for appendicular endometriosis.

Abstract Endometriosis is a complex disease that affects 10-15% of women of reproductive age. Familial studies show that relatives of affected patients have a higher risk of developing the disease, implicating a genetic role for this disorder. Little is known about the impact of germline genomic copy number variant (CNV) polymorphisms on the heredity of the disease. In this study, we describe a rare CNV identified in two sisters with familial endometriosis, which contain genes that may increase the susceptibility and progression of this disease. We investigated the presence of CNVs from the endometrium and blood of the sisters with endometriosis and normal endometrium of five women as controls without the disease using array-CGH through the Agilent 2x400K platform. We excluded common CNVs that were present in the database of genomic variation. We identified, in both sisters, a rare CNV gain affecting 113kb at band 3q12.2 involving two candidate genes: ADGRG7 and TFG. The CNV gain was validated by qPCR. ADGRG7 is located at 3q12.2 and encodes a G protein-coupled receptor influencing the NF-kappaβ pathway. TFG participates in chromosomal translocations associated with hematologic tumor and soft tissue sarcomas, and is also involved in the NF-kappa B pathway. The CNV gain in this family provides a new candidate genetic marker for future familial endometriosis studies. Additional longitudinal studies of affected families must confirm any associations between this rare CNV gain and genes involved in the NF-kappaβ pathway in predisposition to endometriosis.

Background: Hemolacria or the presence of blood in tears is a rare condition, and there are only a few cases reported in the literature. Hemolacria is associated with multiple underlying diseases, including vicarious menstruation due to extragenital endometriosis. Case report: We present a 26-year-old woman with hemolacria and abdominal pain related to her menstrual cycle. The patient was diagnosed with bilateral ovarian endometriomas. After ruling out other possible causes of hemolacria, a progestin-only treatment was applied, with improvement of the abdominal pain and complete remission of hemolacria. Conclusions: When faced with hemolacria, a thorough anamnesis and physical examination must be performed, sometimes involving more than one specialist to reach a diagnosis. Considering hemolacria is a sign of a subjacent pathology, its treatment should be specific one for the disease in each case.


Antecedentes: La hemolacria o presencia de sangre en las lágrimas es una afección poco frecuente y sólo hay unos pocos casos descritos en la literatura. La hemolacria se asocia a múltiples enfermedades subyacentes, incluida la menstruación vicaria debida a endometriosis extragenital. Caso clínico: Presentamos a una mujer de 26 años con hemolacria y dolor abdominal relacionado con su ciclo menstrual. La paciente fue diagnosticada de endometriomas ováricos bilaterales. Tras descartar otras posibles causas de hemolacria, se aplicó un tratamiento sólo con progestágenos, con mejoría del dolor abdominal y remisión completa de la hemolacria. Conclusiones: Ante una hemolacria se debe realizar una anamnesis y exploración física minuciosa, en la que a veces interviene más de un especialista para llegar al diagnóstico. Teniendo en cuenta que la hemolacria es signo de una patología subyacente, su tratamiento debe ser el específico para la enfermedad en cada caso.

RESUMEN

El síndrome de hemivagina obstruida y anomalía renal ipsilateral (OHVIRA) es producido por una alteración en el desarrollo de los conductos de Müller y Wolff en la vida fetal. El síndrome es poco frecuente, se reporta una prevalencia de 1/2.000 a 1/28.000 casos. La endometriosis se presenta en un 19% de los casos complicando esta patología. El tratamiento del síndrome OHVIRA consiste en resecar el tabique vaginal drenando el hematocolpos. Hasta el momento no existe un consenso en recomendar la realización de una laparoscopia diagnóstica. El objetivo de este estudio es reportar la eventual importancia de la laparoscopia diagnóstica/terapéutica como parte del manejo del síndrome OHVIRA.


Obstructed hemivagina and ipsilateral renal anomaly (OHVIRA) syndrome is caused by a defect in the development of Müllerian and Wolffian ducts at fetal life. The syndrome is uncommon, with a reported prevalence of 1/2,000 to 1/28,000 cases. Endometriosis is present in 19% of cases complicating this pathology. Treatment of OHVIRA syndrome consists in resecting the vaginal septum and draining the hematocolpos. Until now there isnt an agreement on recommending diagnostic laparoscopy as part of the treatment. The aim of this study is to report the importance of diagnostic/therapeutic laparoscopy in the management of OHVIRA syndrome.

Introdução:A endometriose consiste em uma patologia ginecológica bastante prevalente emmulheres de múltiplas faixas etárias, consistindo em um desafio constante para a fertilidade, sexualidade e demais aspectos da qualidade de vida.Objetivo:Avaliar o perfil epidemiológico, formas de tratamento e os aspectos biopsicossociais associados à saúde sexual das mulheres com endometriose. Metodologia:Esta é uma revisão integrativa da literatura cujapergunta norteadorafoi "Como a endometriose influencia nos aspectos biopsicossociais inerentes ao comportamento sexual feminino?". Foi aplicada a estratégia de busca "Endometriose AND Dispareunia AND Qualidade de Vida" na Biblioteca Virtual em Saúde (BVS) e sua versão em inglês "EndometriosisAND DyspareuniaAND Quality of Life" na base de dados PubMed, com ofiltro"últimos10anos"aplicadoemambasasplataformas.Critérios de inclusão: estudos que abordassem mulheres cisgênero, em idade fértil,com vida sexual ativa, diagnóstico prévio de endometriose pélvica ou profunda e distúrbios ou queixas sexuais. Critério de exclusão: artigos que contemplassem mulheres em uso de psicofármacos. Resultados:Inicialmente,foram obtidos 227 artigos. Apósanálise primária, 189 estudos foram excluídos, seguindo para a etapa seguinte apenas 38. Destes, somente 15 atenderam aos critérios e foram considerados válidos para compor o presente estudo.No âmbito sexual, a endometriose pode acarretar aredução da frequência das relações, sangramentos durante ou após o coito, desinteresse em preliminares, desconforto em certas posições, términos de relacionamentos, conflitos conjugais, e, sobretudo, dispareunia.Conclusões:Diante dos prejuízos em diversos âmbitos da vida e bem-estar feminino causados pela endometriose, faz-se indispensável maior qualificação dos serviços de saúde para o diagnóstico precoce e intervenções efetivas, bem como apoio, acolhimento e acompanhamento multiprofissional contínuo. Além disso, a adaptação, compreensão e solicitude dos parceiros são fundamentais para que as mulheres sejam capazes de melhor gerenciar tais desafios (AU).


Introduction:Endometriosis is a gynecological pathology that is quite prevalent among women of multiple age groups, representing a constant challenge to fertility, sexuality, and other aspects of quality of life.Objective:To evaluate the epidemiological profile, forms of treatment and biopsychosocial aspects associated with the sexual health of women with endometriosis.Methodology:This is an integrative review of the literature whose guiding question was "How does endometriosis influence the biopsychosocial aspects inherent to female sexual behavior?". The search strategy "Endometriosis AND Dyspareunia AND Quality of Life" was applied in the Biblioteca Virtual em Saúde (BVS) and in the PubMed database, with the "last 10 years" filter applied on both platforms. Inclusion criteria: studies that addressed cisgender women, of childbearing age, with active sexual life, previous diagnosis of pelvic or deep endometriosis and sexual disorders or complaints. Exclusion criteria: articles that included women using psychotropic drugs. Results:Initially, 227 articles were obtained. After primary analysis, 189 studies were excluded, only 38 going on to the next stage. Of these, only 15 met the criteria and were considered valid to be part of the present study. In the sexual sphere, endometriosis can lead to a reduction in the frequency of intercourse, bleeding during or after coitus, lack of interest in foreplay, discomfort in certain positions, relationship endings, marital conflicts, and, above all, dyspareunia.Conclusions:Given the damage to various areas of life and female well-being caused by endometriosis, it is essential to improve the quality of health services for early diagnosis and effective interventions, as well as support, reception, and continuous multidisciplinary monitoring. Furthermore, adaptation, understanding and concern from partners are fundamental for women to be able to better manage such challenges (AU).

Criterios de inclusión: estudios dirigidos a mujeres cisgénero en edad fértil, con vida sexual activa, diagnóstico previo de endometriosis pélvica o profunda, y trastornos o quejas sexuales. Criteriode exclusión: artículos que incluían mujeres usuarias de psicofármacos.Resultados:Inicialmente se obtuvieron 227 artículos. Después del análisis primario, se excluyeron 189 estudios y solo 38 pasaran a la siguiente etapa. De estos, solo 15 cumplieron con los criterios y seconsideraron válidos para el presente estudio. En el ámbito sexual, la endometriosis puede provocar reducción de la frecuencia de relaciones sexuales, sangrado durante o después de las relaciones sexuales, falta de interés por los juegos previos, molestias en determinadas posiciones, rupturas, conflictos matrimoniales y, sobre todo, dispareunia.Conclusiones:Ante los daños causados por la endometriosis en diversos ámbitos de la vida y el bienestar de las mujeres, es indispensable mejorar la calidad de losservicios de salud para el diagnóstico precoz y las intervenciones efectivas, así como apoyo, acogida y seguimiento multidisciplinar continuo. Además, la adaptación, comprensión y solicitud de las parejas son fundamentales para que las mujeres puedan gestionar mejor estos desafíos (AU).

Abstract Objective To correlate the morphological aspects with pelvic pain in women with deep infiltrating endometriosis. Methods A retrospective study with 67 women with deep endometriosis who underwent surgical treatment in a tertiary hospital from 2007 to 2017. The following variables were considered: age, parity, body mass index, site of involvement, hormonal treatment before surgery, pelvic pain, and morphometric analysis. The histological slides of the surgical specimens were revised and, using the ImageJ software for morphometric study, the percentages of stromal/glandular tissues were calculated in the histological sections. Results The mean age of the women was 38.9 ± 6.5 years. The mean pain score was 8.8 ± 1.9 and the mean time of symptomatology was 4.7 ± 3.5 years, with 87% of the patients undergoing hormone treatment prior to surgery. The average expression of CD10, CK7, and S100 markers was 19.5 ± 11.8%, 9.4 ± 5.9%, and 7.9 ± 5.8% respectively. It was found that the greater the expression of CD10, the greater the level of pain (p = 0.02). No correlation was observed between the expression of CD10, CK7, and S100 markers and age and duration of symptoms. Conclusion Women with deep infiltrating endometriosis have a positive association between the level of pain and the fibrosis component in the endometrial tissue's histological composition.


Resumo Objetivo Correlacionar os aspectos morfológicos com a dor pélvica em mulheres com endometriose profunda. Métodos Estudo retrospectivo com 67 mulheres com endometriose profunda submetidas a tratamento cirúrgico em hospital terciário de 2007 a 2017. As seguintes variáveis foram consideradas: idade, paridade, índice de massa corporal, local do acometimento, tratamento hormonal antes da cirurgia, dor pélvica e análise morfométrica. As lâminas histológicas das peças cirúrgicas foram revisadas e, por meio do software ImageJ para estudo morfométrico, foram calculadas as porcentagens de tecidos estromais/glandulares nos cortes histológicos. Resultados A média etária das mulheres foi de 38,9 ± 6,5 anos. O escore de dor médio foi de 8,8 ± 1,9 e o tempo médio de sintomatologia foi de 4,7 ± 3,5 anos, sendo que 87% das pacientes realizavam tratamento hormonal antes da cirurgia. A expressão média dos marcadores CD10, CK7 e S100 foi de 19,5 ± 11,8%, 9,4 ± 5,9% e 7,9 ± 5,8%, respectivamente. Verificou-se que quanto maior a expressão de CD10, maior o nível de dor (p = 0,02). Não foi observada correlação entre a expressão dos marcadores CD10, CK7 e S100 com a idade e duração dos sintomas. Conclusão Mulheres com endometriose profunda apresentam associação positiva entre o nível de dor e o componente de fibrose na composição histológica do tecido endometrial.

Introducción. La intususcepción del apéndice corresponde a su invaginación en el ciego. Existen varias causas, pero la endometriosis ha sido informada pocas veces. Aunque el diagnóstico se debe sospechar clínicamente, por lo general su causa solo se determina en el intraoperatorio, donde se deben tener en cuenta causas oncológicas que requieran una resección amplia. Caso clínico. Mujer de 21 años que consultó por dolor abdominal agudo generalizado. Se practicó una tomografía computarizada de abdomen, observando una intususcepción del apéndice en el ciego, estriación de la grasa pericecal y adenomegalias. Se realizó laparoscopia diagnóstica encontrando intususcepción casi completa del apéndice cecal, de aspecto neoplásico. Se convirtió a laparotomía para proceder a hemicolectomía derecha, con vaciamiento ganglionar y anastomosis del íleon al colon transverso. Discusión. La sospecha clínica de intususcepción debe corroborarse mediante ecografía, tomografía o estudios baritados. El tratamiento siempre es quirúrgico, como en el caso de nuestra paciente, quien evolucionó de forma adecuada y continuó asintomática después de un año de seguimiento. Conclusión. El diagnóstico temprano de la intususcepción permite realizar tratamientos quirúrgicos menos agresivos y disminuye el riesgo de filtración de la anastomosis. Se debe tener en cuenta el diagnóstico de endometriosis como posible causa. Se debe realizar el manejo complementario por parte de ginecología.


Introduction. The intussusception of the appendix corresponds to its invagination in the cecum. There are several causes, endometriosis being rarely reported. Although the diagnosis must be suspected clinically, its cause is generally only determined intraoperatively, where oncological causes that require extensive resection must be taken into account. Clinical case. A 21-year-old woman who consulted due to acute generalized abdominal pain, an abdominal tomography was performed, finding an intussusception of the appendix in the cecum, striation of pericecal fat, and lymph nodes. A diagnostic laparoscopy was performed, finding almost complete intussusception of the appendix, with a neoplastic appearance. She was converted to laparotomy to perform a right hemicolectomy, with lymph node dissection and ileal to transverse anastomosis. Discussion. Clinical suspicion of intussusception should be confirmed by ultrasound, abdominal tomography, or barium studies. Treatment is always surgical, as in the case of our patient, who evolved adequately and remained asymptomatic after one year of follow-up. Conclusion. Early diagnosis of intussusception allows for less aggressive surgical treatment and decreases the risk of anastomosis leakage. The diagnosis of endometriosis should be taken into account as a possible cause. Complementary management by gynecologists should be performed.

Abstract Objective Endometriosis causes a decrease in oocyte quality. However, this mechanism is not fully understood. The present study aimed to analyze the effect of endometriosis on cumulus cell adenosine triphosphate ATP level, the number of mitochondria, and the oocyte maturity level. Methods A true experimental study with a post-test only control group design on experimental animals. Thirty-two mice were divided into control and endometriosis groups. Cumulus oocyte complex (COC) was obtained from all groups. Adenosine triphosphate level on cumulus cells was examined using the Elisa technique, the number of mitochondria was evaluated with a confocal laser scanning microscope and the oocyte maturity level was evaluated with an inverted microscope. Results The ATP level of cumulus cells and the number of mitochondria in the endometriosis group increased significantly (p < 0.05; p < 0.05) while the oocyte maturity level was significantly lower (p < 0.05). There was a significant relationship between ATP level of cumulus cells and the number of mitochondrial oocyte (p < 0.01). There was no significant relationship between cumulus cell ATP level and the number of mitochondrial oocytes with oocyte maturity level (p > 0.01; p > 0.01). The ROC curve showed that the number of mitochondrial oocytes (AUC = 0.672) tended to be more accurate than cumulus cell ATP level (AUC = 0.656) in determining the oocyte maturity level. Conclusion In endometriosis model mice, the ATP level of cumulus cells and the number of mitochondrial oocytes increased while the oocyte maturity level decreased. There was a correlation between the increase in ATP level of cumulus cells and an increase in the number of mitochondrial oocytes.
