Objective:To investigate the differences in the expression of microRNA (miR)-216a and its target gene SerpinB5 at the tissue level, and the effects of miR-216a on the proliferation of different liver cancer cells by regulating the expression of SerpinB5.Methods:Through bioinformatics prediction and selection of miR-216a that regulated SerpinB5. the expressions in liver cancer and normal tissues were detected by real time polymerase chain reaction (PCR). The miR-216a simulacrum and inhibitor, si-Serpinb5 and pcdna3.1-Serpinb5 to HepG2 and MHCC97H (97H) were transfected with liposomes, respectively. Real time PCR and Wester-Blot were used to detect the expression of miR-216a and SerpinB5 before and after transfection, and CCK8 was used to detect the influence of both on the proliferation of liver cancer cells.Results:The expression of miR-216a in human liver cancer tissues was higher than that in adjacent tissues, and the difference was statistically significant ( P < 0.01). The expression of SerpinB5 in human liver cancer tissues was lower than that adjacent tissues, and the difference was statistically significant ( P < 0.01). In HepG2 and 97H, miR-216a inhibitor and SerpinB5 overexpression group showed down-regulated miR-216a expression, which was statistically different from the control group ( P < 0.01). The proliferation of miR-216a inhibitor and pcdna3.1-serpinb5 group was lower than the control group, with statistically significant differences ( P < 0.01). Conclusions:The high expression of SerpinB5 can inhibit the proliferation of liver cancer cells, suggesting that SerpinB5 may have an anti-oncogene effect. MiR-216a may negatively regulate the expression of SerpinB5 and affect the proliferation of HCC cells.

Objective To analysis the expression of thyroid transcription factor 1 (TTF-1) in patients with lung adenocarcinoma, and discuss the relationship between TTF-1 expression and prognosis of patients with advanced lung adenocarcinoma. Methods A total of 786 cases of lung adenocarcinoma who were admitted to Xi'an Chest Hospital from January 2012 to June 2016 were selected. The expression of TTF-1 was detected by immunohistochemistry. The relationship between TTF-1 expression and patients ' clinicopathological features, treatment and survival were analyzed. Cox proportional hazard model was used to analyze the relationship between TTF-1 and prognosis of patients with advanced lung adenocarcinoma. Results Among 786 patients, 559 patients (71.12%) had positive TTF-1 expression, 227 patients (28.88%) had negative TTF-1 expression. The expression rates of TTF-1 in patients with well-differentiated, early stage, no lymph node metastasis, and no distant metastasis [77.26% (197/255), 78.89% (269/341), 78.95% (225/285), and 75.61%(372/492)] were higher than those in patients with poorly differentiated, late stage, lymph node metastasis and distant metastasis 68.17% (362/531), 65.17% (290/445), 66.67% (334/501), 63.60% (187/294), the differenceswere statistically significant (χ 2 values were 6.917, 25.261, 13.339, and 12.911, all P < 0.05). The expressions of TTF-1 in primary lesions and metastatic lesions were consistent (κ = 0.894, P < 0.01). Among 385 patients with advanced lung adenocarcinoma who were unable to perform the operation, the proportion of epidermal growth factor receptor (EGFR) mutation in TTF-1 positive expression patients (45.60%, 109/239) was significantly higher than that in TTF-1 negative expression patients (26.02%, 38/146), and the difference was statistically significant (χ 2 = 14.721, P < 0.01). The median progression free survival (PFS) time of TTF-1 positive expression patients was significantly longer than that of TTF -1 negative expression patients (6.00 months vs. 4.20 months, P < 0.01), whether EGFR was mutation or not, the median PFS time of TTF-1 positive expression patients was significantly longer than that of TTF-1 negative expression patients (P =0.003; P < 0.01). TTF-1 expression (HR = 0.793, 95% CI 0.639-0.984, P = 0.011) and EGFR gene status (HR =0.694, 95% CI 0.432-0.864, P = 0.004) were independent influencing factors affecting the PFS of patients with advanced lung adenocarcinoma. Conclusions TTF-1 is widely expressed in lung adenocarcinoma and is associated with tumor differentiation, staging, lymph metastasis and distant metastasis. TTF-1 is a prognostic influencing factor in patients with advanced lung adenocarcinoma and can be used as a predictor of treatment for patients with advanced lung adenocarcinoma.

Objective To explore the correlation between serum hepatitis C virus (HCV)-RNA level with cholinesterase (CHE), albumin (ALB) and prealbumin (PA) in patients with hepatitis C, and provide the references for the early diagnosis and the prognosis monitoring of hepatitis C. Methods Four hundred and fifty-five patients with hepatitis C were selected. The serum level of HCV-RNA was determined by quantitative real-time polymerase chain reaction (real-time PCR), and serum levels of CHE, ALB and PA were detected using the automatic biochemistry analyzer. The patients were divided into 6 group according to the result of HCV-RNA level:HCV-RNA<103 kU/L group (group A, 52 cases), 103 kU/L≤HCV-RNA<104 kU/L group (group B, 77 cases), 104 kU/L≤HCV-RNA<105 kU/L group (group C, 81 cases), 105 kU/L≤HCV-RNA<106 kU/L group (group D, 92 cases), 106 kU/L≤HCV-RNA<107 kU/L group (group E, 87 cases) and HCV-RNA≥107 kU/L group (group F, 66 cases). Moreover, the patients were divided into 3 groups according to the result of serum CHE: CHE normal group (> 5000 U/L, 321 cases), CHE mild abnormal group (4000- 5000 U/L, 56 cases) and CHE abnormal group (<4000 U/L, 78 cases). Results With the rising level of serum HCV-RNA from group A to group F, the serum levels of CHE, ALB and PA were all gradually decreased in hepatitis C patients, CHE: (7288 ± 2817), (6316 ± 2341), (6103 ± 2596), (5208 ± 2222), (4282 ± 2173) and (3905 ± 1378) U/L; ALB: (46.3 ± 9.9), (44.0 ± 8.4), (43.1 ± 7.6), (42.6 ± 7.1), (41.1 ± 5.4) and (39.3 ±5.1) g/L;PA:(212.1 ± 67.8), (179.9 ± 72.8), (163.4 ± 57.5), (137.4 ± 60.3), (120.6 ± 45.0) and (112.5 ± 42.0) mg/L, and there were statistical differences (F=21.08, 6.08 and 27.54;P<0.01). With the decreasing level of serum CHE, the serum levels of ALB and PA were all gradually decreased, ALB:(45.4 ± 10.1), (33.1 ± 4.2) and (31.5 ± 8.8) g/L;PA:(209.3 ± 56.4), (108.4 ± 44.1) and (81.5 ± 49.6) mg/L, and there were statistical differences (F = 70.23 and 152.57, P<0.01). The bivariate Spearman correlation analysis result showed that the serum HCV-RNA level was negatively correlated with serum CHE, ALB and PA (r =-0.357, -0.326 and-0.471; P<0.05), and the serum CHE was positively correlated with serum ALB and PA (r=0.726 and 0.807, P<0.05). Conclusions The serum HCV-RNA level is closely related to liver function indices. Performing simultaneous detection of serum HCV-RNA level and serum PA is helpful in the early diagnosis and treatment of Hepatitis C.

Objective: To study the effects of potentill on all systems of experimental animals to provide experimental evidence for the comprehensive safety evaluation of potentill.Methods: Mice and rats were given potentill orally to determine the influence on all systems, and the general pharmacological activities of potentill were recorded.Results: The general behavior of mice was normal after the adminisration of potentill , the test samples showed no synergetic hypnotic effect with nembutal at subthreshold dose, and had no influence on the spontaneous activity and body coordination ability of mice (P>0.05).The test samples showed no obvious influence on the cardiovascular system and respiratory system of rats (P>0.05).Conclusion: Potentill meets the general requirements of pharmacology at the prescribed dose.

Regarding radioactive iodine-refractory failure or advanced differentiated thyroid cancer,multiple multikinase inhibitors including sorafenib and lenvatinib,which target platelet derived growth factor receptor,vascular endothelial growth factor pathway,and rearranged during transfection (RET) pathway were proved to have obvious antitumor activity.Moreover,selective BRAF inhibitor,promoting drug uptake of radioactive iodine also showed a certain therapeutic effect.These molecular targets which are relevant with differentiated thyroid cancer occurrence,development,invasion and metastasis have become of its moment,and,selective inhibitors and re-differentiation agents were shown to be promising.In the future,individual genetic testing would provide more specific information in directing individualized molecular-targeted therapy.

Objective To explore the role of the SerpinB5 and β-catenin in occurrence and development of the primary hepatocellular carcinoma (HCC). Methods The expressions of SerpinB5 and β-catenin protein and mRNA in carcinoma tissues and paracancerous tissues of 60 patients with primary HCC were detected by immumohistochemistry and real-time quantitative reverse transcriptional polymerase chain reaction (RT-PCR) methods. Results The positive expression rate of SerpinB5 protein and SerpinB5 mRNA in carcinoma tissues were significantly lower than those in paracancerous tissues:25.0%(15/60) vs. 63.3%(38/60) and 1.12 ± 0.43 vs. 5.19 ± 0.39, and there were statistical differences (P<0.01). The positive expression rate of β-catenin protein and β-catenin mRNA in carcinoma tissues were significantly higher than those that in paracancerous tissues: 65.0%(39/60) vs. 31.7%(19/60) and 4.23 ± 0.25 vs. 1.19 ± 0.17, and there was statistical difference (P<0.01). Decreased SerpinB5 expression was associated with higher serumα-fetoprotein level, larger tumor size, poor differentiation, advanced TNM stage, capsule invasion and tumor thrombosis (P < 0.01 or 0.05). Increased β-catenin expression was associated with poor differentiation, advanced TNM stage, capsule invasion and tumor thrombosis (P < 0.01 or < 0.05). The correlation analysis result showed that SerpinB5 had negative correlation withβ-catenin (carcinoma issues:r=-0.346, P=0.001;paracancerous tissues:r=-0.258, P = 0.024). Conclusions The abnormal expression of SerpinB5 and β-catenin may contribute to the progression and biologically malignant behavior of primary HCC, and SerpinB5 and β-catenin exists synergistic effect in the occurrence and development of primary HCC.

Objective To explore the effect of diabetic liaison nurses on controlling blood sugar levels in patients with hyperglycemia in department other than endocrinology. Methods Four hundred diabetic patients with high blood sugar were selected from January to December, 2014 in department other than endocrinology. They were divided randomly into 2 groups equally:the control group and the observation group. The control group received traditional nursing care, while blood sugar management was carried out by diabetic liaison nurse in the observation group. Result Pre-discharge sugar metabolism in the observation group was significantly better than that in the control group (P<0.05). Conclusion The diabetic liaison nurses in other departments than the endocrinology department can help control blood sugar levels in patients with hyperglycemiain.

Objective To observe insulin resistance in first-degree relatives of patients with Graves disease. Methods All subjects in control group and experiment group including first-degree relatives of GD patients underwent oral glucose tolerance tests (OGTT) and insulin releasing tests then the degree of insulin resistance was analyzed. Results Blood glucose at each point of OGTT, insulin level and insulin resistance index 1 (HOMA-IR) of experiment group were higher than those in control group, while insulin activity index (IAI) and HOMA-βwere significantly lower than those in control group. Conclusion Patients insulin resistance could be found among first-degree relatives of GD patients.

ObjectiveTo investigate the application of pericardial devascularization and omental packing combined with gastric noose cerclage in the treatment of portal hypertension. MethodsThe clinical data of 59 patients who were treated in our hospital and received pericardial devascularization and omental packing combined with gastric noose cerclage from June 2007 to June 2013 were analyzed retrospectively, and the indices such as changes in portal venous pressure, portal vein diameter, and liver function after surgery, postoperative rebleeding rate, and survival rate were observed. Analysis of variance was applied for comparison between multiple groups, and paired t-test was applied for comparison between two groups. ResultsAll the 59 patients received a successful surgery, and the portal venous pressure was significantly decreased after the surgery; after the emergency operation, upper gastrointestinal bleeding was stopped, with no complications such as gastroparesis and pancreatic fistula; there was no death. The postoperative follow-up was performed for 2~5 years, and 1 case of rebleeding and 1 death occurred. ConclusionPericardial devascularization and omental packing combined with gastric noose cerclage have the advantages of both devascularization and shunt, and can achieve complete devascularization of the vessels at the lower segment of the esophagus, the cardia, and the gastric fundus, and the submucosal vessels in the gastric wall, with exact hemostasis, few postoperative complications, low rebleeding rate, and simplicity in operation, which is a reasonable, reliable, safe, and effective operation method.

Objective To evaluate the efficacy of composite skin grafts consisting of allogeneic acellular dermal matrix and autologous scalp in the repair of defects after excision of giant pigmented nevi in children.Methods Eighteen children with giant pigmented nevi were included in this study.All the patients received excision of giant pigmented nevi.The defects were repaired using composite skin grafts consisting of allogeneic acellular dermal matrix and autologous razor-thin scalp grafts in 10 children from June 2009 to October 2012 (test group),and using autologous thin or intermediate-thickness skin grafts in 8 children from March 2006 to May 2009 (control group).Donor site healing time,skin graft survival rate,and the degree of scar proliferation were compared between the two groups.Results Significant differences were observed at donor sites between the test group and control group in healing time ((5.31 ± 1.45) vs.(11.63 ± 1.69) days,P < 0.05) and scar score (1.62 ± 0.38 vs.6.38 ± 0.58,P < 0.05).At recipient sites,the survival rate of skin grafts was similar between the test group and control group ((94.44 ± 2.56)% vs.(95.13 ± 3.13)%,P > 0.05),while scar score was significantly different (5.38 ±0.62 vs.8.40 ± 0.41,P < 0.05).Conclusion Composite skin grafts consisting of allogeneic acellular dermal matrix and autologous scalp appear to be a good option for the repair of defects after excision of giant pigmented nevi in children,with minor donor-site injuries and satisfying cosmetic and functional outcomes at recipient sites.

Objective: To discuss the application value with two-dimensional ultrasound speckle tracking technique in the evaluation of left ventricular function in patients with OSAS. Methods: Selected 110 cases of hospital patients with OSAS in recent years, used STI technical analysis of left ventricular myocardium using different directions overall resilience, Left ventricular torsion and systolic mitral annular midpoint of the maximum displacement (MAD) and left ventricular ejection fraction (LVEF) relationship, performed using SPSS 17.0 statistical correlation analysis. Results: MAD had a significant correlation with LVEF(r=0.81, P<0.01); GLS(r=-0.74, P<0.01),GRS GRS(r=0.57, P<0.01),GCS (r=-0.55, P<0.01) and LVtw(r=0.65, P<0.01)were significantly associated with LVEF;MAD and GLs(r=-0.69, P<0.01),GRs (r=0.49, P<0.01), LVtw (r=0.45, P<0.01)were significantly associated;MAD and GCs (r=-0.39, P<0.05)with a negative correlation. Conclusion:STI resilience through the myocardium, ventricular torsion and systolic mitral annular maximum displacement at the midpoint of the analysis, can accurately evaluate left ventricular longitudinal, radial and torsional motion synchronization, highly correlated with left ventricular ejection fraction, good consistency, with a very broad application prospects.

Surgical resection and liver transplantation are still the preferred treatments for patients with hepatocellular carcinoma (HCC).For the patients with advanced HCC which are not suitable for surgical resection,traditional chemotherapy can not improve the prognosis.Molecular targeted therapy is a new method and tendency in the treatment of HCC.Multiple targets inhibitors,vascular endothelial growth factor inhibitors and monoclonal antibodies for HCC are widely researched and applied.

The application of methyl in drug design is reviewed in this paper. Methyl can modulate the physicochemical, pharmacodynamic, and pharmacokinetic properties by ortho effect, inductive effect, and conformational effect. It also improves the metabolic stability as a soft metabolic point. In addition, introducing methyl into drug molecules can also be applied as a strategy in new uses of old drugs and generate me-too drugs.

Objective To explore exenatide in the treatment of metformin(MET)alone,sulfonylurea (SU)alone or MET + SU combination therapy with poor glycemic control in type 2 diabetic pa-tients and to find effective nursing measures.Methods 24 patients were randomly divided into the con-trol group and the exenatide group with 12 patients in each group.In the exenatide group,exenatide 5μg twice a day for 4weeks,then 10μg twice a day for 12 weeks.Changes of HbAlc,body weight,BMI,FBG,P2hBG,and rate of adverse reaction were compared between two groups.Results Glycosylated hemoglobin (HbAlc),body weight,BMI,FBG,P2hBG in the control group before and after treatment showed no significant difference,while the exenatide group showed better results compared with those before treatment and the control group.Nursing intervention played evident effect on reducing adverse effect such as nausea,vomiting,diarrhea,low blood sugar,headache.Conclusions For patients with type 2 diabetes,using MET,SU alone or MET + SU combination therapy showed poor results of blood sugar control,addition of exenatide therapy can effectively control blood sugar,nursing intervention can significantly alleviate the adverse effects of patients.

Objective To evaluate the application value of the one-stepdiabetes education management system in clinic. Methods The “one-step” diabetes education management system was construct.80 newly diagnosed patients with type 2 diabetes mellitus were selected and randomly divided into the control group and the experimental group with 40 patients in each group.The patients in the experimental group received “one-step”diabetes education management system by diabetes specialist nurses,while patients in the con-trol group received routine diabetic education.The fasting blood glucose (FBG),HbAlc,medical compliance behaviors and other indices were compared between two groups. Results FBG,HbAlc and medical compliance were better,and the risk score of complications was higher in the experimental group than those of the control group,there were significant differences between them. Conclusions Application of “one-step”diabetes education management system in treatment and nursing of patients shows maneuverability and practicality.It has clinical application value and deserves to be spread.

Objective To study seasonal variation of blood pressure (BP) in patients with essential hypertension (EH) and its association with meteorological elements. Methods In total, 1800 patients with essential hypertension were recruited by cluster sampling from three townships and towns in Qingzhou,Shandong province, 1155 men and 645 women, with an average age of 61.3 years (ranging from 29 to 85years). BP was measured for all of them in sitting position at 8:00 in the morning every seven to 14 days during July 2008 to June 2009. Local meteorological data were collected accordingly such as temperature,humidity and atmospheric pressure, and multivariate regression analysis was performed to show their association. Results In general, BP in patients with EH presented a decreasing trend during the first half year of observation, with the lowest in the summer ( May to August). From September to October, their BP began to increase gradually, and reached the peak in the winter ( November to February nest year). There was significant difference in systolic BP (SBP) and diastolic BP (DBP) between various seasons (Pspring-summer = 0.002 and 0.000, Pwinter-spring = 0.001 and 0.000, Psummer-autumn = 0.045 and 0.000,Psummer-winter =0. 000 and 0. 000, Pautumn-winter =0. 000 and 0. 000, respectively), except for those between the spring and autumn. Both SBP and DBP reversely associated with outdoor air temperature and room temperature(beta= -0.08, t = -2.39, P <0.05 and beta = -0.24, t = -6.21, P <0.01,respectively), (beta = -0.08, t = -2.39, P <0.05 and beta = -0.24, t = -6.21, P <0.01,respectively). Conclusions BP in patients with EH reveals seasonal variation, with the lowest in the summer and the highest in the winter and evident fluctuation in the spring and autumn, especially in SBP,which associates with room temperature and their blood vessel elasticity.

BACKGROUND: Degeneration and regeneration after peripheral nerve injury is a very complex pathophysiological process. It still needs to investigate the promotive effects of electric stimulation in neuranagenesis.OBJECTIVE: To compare the effect and result of percutaneous nerve-muscle electric stimulation therapy in peripheral neuranagenesis of different position.DESIGN: A case analysis based on patients.SETTING: Laboratory of neuroelectrophysiology and department of orthopedics of a medical university affiliated hospital.PARTICIPANTS: Totally 78 patients with peripheral neurologic impairment proved by clinical diagnosis and electromyogram(EMG) were selected from the patients with peripheral nerve injury visited or hospitalized by the Department of Neurology and Orthopedics, Affiliated Hospital of Ningxia Medical University between October 1999 and June 2003.METHODS: Cantana EMG was used. Totally 78 cases received percutaneous nerve-muscle electric stimulation (PNMES) therapy for the observation of the restoration of peripheral nerve function. Therapeutic effectiveness of different course of disease was compared and analyzed by comparing and analyzing the EMG before and after therapy.RESULTS: In 78 patients with brachiplex nerve and sci atic nerve impairment, 53 cases had completely restored motor and sensory functions, 18 cases had significantly improved motor and sensory functions with the effective rate of 91% (71/78) after 1 to 10 courses of therapy. Post-therapeutic denervated potential, motor nerve conduction velocity(MNCV) and sensory nerve conduction velocity(SNCV) were significantly improved compared with that of before therapy.CONCLUSION: PNMES therapy can promote peripheral neuranagenesis,improve the local blood circulation of the impaired nerve and elevate the excitability of nerve and muscle.
