RÉSUMÉ

Objective To discuss nicardipin′s influence on post operation cognitive dysfunction (POCD) in senior patient after hip joint replacement operation. Methods 180 senior patient, who received selective unilateral hip joint replacement operation between October 2015 and October 2016 under the condition of combined spinal-epidural anesthesia (CSEA) were randomly divided into Group A (with nicardipine) and Group B (without nicardipine).Nicardipine was only appropriately pumped into vein in time in Group A.MAP was observed and recorded 30 minutes after starting the operation and at the end of the operation.Mini-mental state examination (MMSE) was applied to score the patient one day before, one day, three days and five days after operation, and the number of POCD was recorded. Results Compared with Group B, Group A was obviously lower in MAP level (P<0.05) after 30 minutes.MMSE score of Group A was obviously higher (P<0.01) one day after operation. The number of POCD in Group A was 8 (8.89%) significantly lower than than that of Group B (19, 21.11%).Conclusion Nicardipine could maintain hemodynamic stability of senior patients receiving selective unilateral hip joint replacement operation under the CSEA and prevent POCD to a certain extent.

RÉSUMÉ

Objective To explore the gray matter changes in aggressive patients with schizophrenia, and the relationship between the gray matter and aggression in patients. Methods Eighteen aggressive patients with schizophrenia (SZ1),18 age-and gender-matched un-aggressive patients with schizophrenia (SZ2) and 18 normal controls (NC) were enrolled in the study. Then a 3.0 T magnetic resonance imaging (MRI) scan was conducted for each participant. The voxel-based morphometry (VBM) approach and the Chinese version of Buss & Perry aggression questionnaire (B&P) were used to explore imaging data and to assess the aggression, respectively. Results Compared with NC,patients with schizophrenia showed changes in gray matter volume (GMV) in the frontal, temporal and the occipital lobes (P<0.05,AlphaSim corrected).Compared with SZ2,SZ1 showed increased GMV in the right supramarginal gyrus, right postcentral gyms, bilateral insula and orbito-frontal gyri (P<0.05,AlphaSim corrected). The GMV of the right insula, right postcentral gyms and right supramarginal grus were positively associated with B&P scores in patients with schizophrenia (P<0.01,AlphaSim corrected),respectively. Conclusions These preliminary findings support that the aggression in schizophrenia is associated with GMV changes of brain regions in patients with schizophrenia. The right postcentral gyrus, the right insula and the right supramarginal gyrus may be involved in the neural mechanism of aggression in schizophrenia.

Objective To analyze the dose of radiation to medical staff during 125I seeds loading Methods The radiation dose at different distances was measured by using thermoluminescence dosimeters (TLD),and the safe range for medical person was calculated. The doses and annual accumualive doses to the medical staff were estimated. Results The dose attenuation rates were 77.61%,98.04%,98.79%,99.30%,99.71% and 100% at distance of 10,20,30,40,50 and 100 cm from seeds tank, respectively. The dose to fingertips, chest, lens and thyroid of doctor were 51.08,35.50,34.73 and 33.78 μGy,and the annual dose reached 12.77,8.88,8.68 and 8.45 mGy when the number of annual operations was 250.The attenuation rate was 79.60% and 28.36% of inside and outside lead glass, respectively. Conclusions The radiation is undetectable at 100 cm away from seeds tank in the process of loading of 125I seeds, and lead glass is necessary for radiation protection.

@#Objective To study the effects of carotid sympathectomy on symptom or intercurrent damage of refractory cerebral palsy. Methods 204 cases with cerebral palsy of mixed type and with intercurrent damage were treated with bilateral carotid sympathectomy. Results 187 cases(91.7%) were followed up successfully, which showed 74.4% of torsion or tension spasm,42% of athetosis,63.6% of cognition language disturbance,81.6% of sialorrhoea,27.6% of strabismus and 88% of deglutition handicap improved. The scores of activity of daily living improved 43.8%.The total incidents effective rate was 82.7%.Conclusion Carotid sympathectomy is an effective and safe microsurgery method for the symptom or intercurrent damage of the refractory cerebral palsy.
