Résumé

【Objective】 To describe the distribution characteristics of knee and ankle joint range of motion and muscle atrophy related indexes in patients with Kashin-Beck disease (KBD) in Shaanxi Province so as to explore the correlation of knee and ankle joint range of motion (ROM) with muscle atrophy indexes and provide reference for clinical characteristics analysis of KBD patients. 【Methods】 To investigate the registered KBD patients from KBD areas in Shaanxi Province, we measured the general demographic data of the patients were collected and the ROM of the knee joint (flexion and extension), the ankle joint (dorsiflexion and plantar flexion), and the muscle atrophy related indexes such as the upper arm circumference, thigh circumference, calf circumference and grip strength. According to the population characteristics, i.e., gender, age, body mass index (BMI) and KBD grade, the median and quartile of joint ROM and muscle atrophy of KBD patients were reported, and then the differences in each index among different groups were analyzed. Partial correlation analysis was used to explore the correlation between indicators after controlling for variables such as gender, age and BMI. 【Results】 A total of 480 patients with KBD were investigated in this study, who consisted of 249 (51.9%) males and 231 (48.1%) females, with an average age of (63.10±7.32) years and an average BMI of (23.49±8.90) kg/m2. The knee flexion ROM, knee extension ROM, ankle dorsiflexion ROM and ankle plantar flexion ROM were [105.0 (95.0, 120.0)] °, [0.0 (-15.0, 0.0)] °, [5.0 (0.0, 15.0)] ° and [20.0 (15.0, 30.0)] °, respectively, in KBD patients in Shaanxi Province. The left thigh circumference, right thigh circumference, left calf circumference, right calf circumference, and upper arm circumference were [43.0 (40.0, 47.0)] cm, [43.0 (39.0, 47.0)] cm, [29.0 (27.0, 32.0)] cm, [29.5 (27.0, 32.0)] cm, [27.0 (25.0, 30.0)] cm, respectively. The left hand grip strength and right hand grip strength were [13.4 (9.5, 18.4)] kg and [13.9 (9.8, 18.2)] kg, respectively. With the increase of age, the extension range of the left and right knee joints of KBD patients showed a decreasing trend (H=31.499, 31.847; all P<0.001). The range of motion of bilateral knee flexion was higher in the normal BMI group than in the overweight or obese group, with statistically significant differences (H=7.753, 12.333; P=0.021, 0.002). The knee flexion, thigh circumference, and calf circumference of the left and right sides showed a decreasing trend under different KBD grades (H=14.345, 17.256, 8.000, 8.462, 8.558, 9.633; all P<0.05). Correlation analysis showed that knee flexion ROM was positively correlated with thigh circumference, calf circumference, and grip strength in patients with KBD (all P<0.05). There was a positive correlation between knee extension ROM and thigh circumference in patients with KBD (P<0.01). 【Conclusion】 The impaired joint ROM and muscle atrophy are serious in KBD patients in Shaanxi Province, and there is a correlation between joint motion and muscle atrophy.

Objective:To analyze the risk factors of postoperative bleeding after CT-guided percutaneous fly biopsy, and to construct a nomogram model for predicting postoperative bleeding.Methods:A total of 328 patients with CT-guided percutaneous fly biopsy who were admitted to the Affiliated Hospital of Ningbo University School of Medicine from June 2019 to March 2021 were selected, and the general clinical data of the patients were retrospectively analyzed. The patients were divided into observation group and control group according to postoperative bleeding. Statistical analysis was performed on clinical data of patients with statistical significance, receiver operating characteristic (ROC) curve analysis on continuous variables with statistical significance, and Logistic multiple regression analysis on all variables with statistical significance. The risk factors of bleeding complications in CT-guided percutaneous lung biopsy were obtained, and a Nomogram model for predicting bleeding after percutaneous lung biopsy was constructed using the R language software 4.0 "rms" package.Results:The results of the study showed that the size of the mass , the depth of the mass, the number of punctures, the number of cases with inflammation around the lesion , and the number of cases with chronic lung lesions in the observation group were greater than those in the control group: (5.20 ± 1.20) cm vs. (4.30 ± 0.90) cm, (4.30 ± 0.60) cm vs. (2.90 ± 0.30) cm, (2.70 ± 0.60) times vs. (2.30 ± 0.50) times, 38(14, 70) cases vs. 17(24, 60) cases, 102(39, 40) cases vs. 41(59, 40) cases; while the number of normal preoperative prothrombin time (PT) in the observation group was less than that in the control group: 4(5, 80) cases vs. 151(58, 30) cases ( P<0.05). ROC curve analysis was performed on the continuous variables with statistical significance in the control table of patients′general clinical data. The results showed that the area under the curve for swelling size, swelling depth, number of punctures, and number of tissue blocks cut were 0.563, 0.714, 0.680, and 0.559, respectively; the optimal cut-off values were 53.00 cm, 5.56 cm, 2.00 times, and 1.00 blocks ( P<0.05). The univariate indicators were included in the Logistic multiple regression model, and the results showed that tumor depth, puncture times, inflammation around the lesion, and abnormal preoperative PT were the risk factors for complications of percutaneous lung biopsy under CT ( P<0.05). The internal validation results showed that the Nomogram model predicted the risk of bleeding complicated by percutaneous lung biopsy under CT, with a C index of 0.687 (95% CI 0.241 - 1.988). The calibration curve showed good agreement between the observed and predicted values. The Nomogram model predicted percutaneous lung biopsy under CT with a bleeding risk threshold of >0.16, and the Nomogram model provided a clinical net benefit; in addition, the Nomogram model had a higher clinical net benefit than independent indicators. Conclusions:In conclusion, patients with poor coagulation function, inflammation around tumor lesions, deeper lesions, and more puncture times are more prone to bleeding. The Nomogram model constructed in this study has a high clinical application value for predicting the bleeding complications of CT-guided percutaneous lung biopsy.

ObjectiveTo analyze the quality of life and related factors in patients with depression, and to provide evidence for formulating the corresponding measures to improve the patients' quality of life. MethodsA total of 117 inpatients who met the International Classification of Diseases, tenth edition(ICD-10) for depression were selected in Central People's Hospital of Zhanjiang, Affiliated Hospital of Guangdong Medical University and Guangdong 999 Brain Hospital from November 19, 2018 to September 7, 2019. The values of patients' clinical objective indicators consisted of routine blood test, routine urine examination, blood biochemical tests and blood gas analysis were collected by reviewing their medical records, and all subjects were assessed using Quality of Life Instruments for Chronic Diseases-Depression V2.0 [QLICD-DE(V2.0)]. Simple correlation analysis was used to explore the correlation between the QLICD-DE (V2.0) scores and various clinical objective indicators, and multiple linear regression was adopted to further screen factors affecting quality of life of patients with depression. ResultsSimple correlation analysis suggested that QLICD-DE (V2.0) total score was positively correlated with total protein and platelet distribution width in patients with depression (r=0.198, 0.281, P<0.05 or 0.01), and negatively correlated with hematocrit (r=-0.300, P<0.01). Multiple linear regression analysis showed that albumin-to-globulin ratio and treatment compliance (B=-19.836, -3.711, P<0.05 or 0.01) were influential factors for physical function. Platelet distribution width (B=2.706, P<0.01) was an influential factor for psychological function. Bilirubin (B=-6.375, P<0.05) was an influential factor for social function. Platelet distribution width (B=2.101, P<0.05) was an influencing factor for specific module. The albumin-to-globulin ratio and platelet distribution width (B=-10.653, 2.114, P<0.05 or 0.01) were influencing factors for total score of QLICD-DE (V2.0). ConclusionTreatment compliance and clinical objective indicators platelet distribution width, bilirubin and albumin-to-globulin ratio may be the influencing factors of quality of life in patients with depression.

Objective To translate English version of sensory-motor profile awake ( SMP-a) into Chinese version (the Chinese Version of SMP-a),and analyze the reliability and validity of the scale before and after craniotomy under awakening anesthesia. Methods Eighty-one patients whose tumors were located near or already in sensory-motor functional area were included in this study. Before and after awake cranioto-my,the Chinese version of SMP-a was used to accurately assess the sensory-motor function of each patient. Finally, the reliability and validity of the scale were analyzed by SPSS statistical software. Results Cronbach's α coefficient in the Chinese version of SMP-a was 0. 971,and Cronbach's α coefficient in the four subscales of face,hand,leg and sensation was 0. 965,0. 989,0. 981 and 0. 970,respectively. The test-retest reliability of the Chinese version of sensorimotor assessment scale was 0. 910,0. 904,0. 884,0. 898 and 0. 695 (total,face,hands,legs and sensory score respectively). The raters' consistency reliability was above 0. 949,0. 960,0. 934,0. 887 and 0. 660,respectively. The Pearson correlation coefficients of sensorimotor function score with SF-36 physiological function factors and KPS score were 0. 868 and 0. 790,respectively. Conclusion Before or after operation,the Chinese version of SMP-a has preferable reliability,internal con-sistency reliability and structural validity. It is feasible in awakening anesthesia craniotomy,and the degree of damage can be determined by repeated measurement of the sensorimotor sites that may be impaired by the patient.

Résumé

Objective To evaluate the feasibility and efficacy of endoscopic push?radial dissection (EPRD)for benign esophageal stricture(BES). Methods Clinical data of 24 patients diagnosed as having BES who received EPRD were analyzed. The procedure and efficacy were evaluated. Results All 24 patients underwent EPRD successfully with mean operation time being 32 min(15?45 min).The mean esophageal stric?ture incision length was 3?? 2 cm (1?? 0?8?? 0 cm).No severe complications related to EPRD occurred, or trans?ferred for surgery. Patients were followed up for 1?5 months (mean 2?? 8 months). Dysphagia was relieved signif?icantly during the follow?up in 23 patients where endoscopy could go through smoothly. But dysphagia re?oc?curred in one patient 2 weeks after the operation, who underwent a second EPRD and stent implantation, then dysphagia was relieved. Conclusion EPRD is safe, feasible and effective for benign esophageal stricture.

Résumé

Objective To investigate the numbers, sizes and types distribution of diatoms in drowned and postmortem immersed rabbits’ lungs. Methods Sixty-two rabbits were randomly divided into drowning group (n=30), postmortem immersion group (n=30) and land death group (n=2), and the diatoms in each lung lobe were analyzed quantitatively and qualitatively by microwave digestion and scanning electron microscopy. Results In the drowning group, the diatoms were detected in each lung lobe with Cyclotella and Melosira in the majority. In the postmortem immersion group, Cyclotella was in the majority. And the diatoms weren’t detected in some lung lobes in postmortem immersion. There were significant dif-ferences in the detection rates of upper lobe of left lung, middle lobe and cardiac lobe of right lung in two groups (P<0.05). Conclusion Based on the microwave digestion and scanning electron microscopy, the numbers, sizes and types distribution of diatoms in drowned and postmortem immersed rabbits ’ lungs can be analyzed and used as references for testing theory.

The cyclic voltammetric behaviors of sodium selenite on the gold electrode modified by L-cysteine self-assembled monolayer (L-Syc,Au/SAMs) has been investigated in 0.05 mol/L H_2SO_4 solutions with sodium selenite. Sensitive anodic stripping current of selenium (Se) with the peak potential at 0.99 V (vs. SCE) has been observed. In comparison with the electrochemical properties of sodium selenite on the modified electrode with those on the naked gold electrode, it was found that selenite was reduced to selenium by the redox interaction with sulfur on the monolayer and the monolayer catalytically oxidizes Se as the electrode was positively scanned. According to the electrochemical properties of sodium selenite both on the naked and modified gold electrode, the Se deposition mechanism and catalytic mechanism as well as bionic membrane model were proposed for the bio-utilization of selenite through nano scale selenium.
