RÉSUMÉ

A Pancoast tumor is a rare form of lung cancer that occurs mainly in the apex of the lung as the main symptom of upper extremity pain. Oropharyngeal dysphagia is not a common symptom. This case report describes a 57-year-old male patient with a Pancoast tumor who presented with oropharyngeal dysphagia. The patient's symptoms included left shoulder and arm pain. The chest computed tomography revealed a mass in the apex of the left lung, invading the mediastinum and compressing the left brachial vein and brachial plexus. He was discharged after receiving palliative chemotherapy. The patient returned to the hospital with dyspnea and was diagnosed with aspiration pneumonia. The cranial nerve exam confirmed hoarseness and an absent gag reflex. In addition, the laryngeal elevation decreased, and the bedside water test was positive. A video fluoroscopic swallow study confirmed the presence of oropharyngeal dysphagia, which was attributed to left glossopharyngeal and vagus nerve damage associated with the Pancoast tumor. This case highlights the need to be aware that a Pancoast tumor can cause oropharyngeal dysphagia. If oropharyngeal dysphagia is suspected, VFSS should be performed to prevent complications leading to mortality from lung cancer.

BACKGROUND@#This study aimed to evaluate the effect of cervical cerclage on the recurrence risk for preterm birth in singleton pregnant women after a twin spontaneous preterm birth (sPTB).@*METHODS@#This multicenter retrospective cohort study included women who had a singleton pregnancy from January 2009 to December 2018 at 10 referral hospitals and a twin sPTB before the current pregnancy. We compared the cervical lengths during pregnancy and pregnancy outcomes, according to the placement of prophylactic or emergency cerclage. We evaluated the independent risk factors for sPTB (< 37 weeks of gestation) in a subsequent singleton pregnancy.@*RESULTS@#For the index singleton pregnancy, preterm birth occurred in seven (11.1%) of 63 women. There was no significant difference in the cervical lengths during pregnancy in women with and without cerclage. In a multivariate logistic regression analysis, the placement of emergency cerclage was an independent risk factor for subsequent singleton preterm birth (odds ratio [OR], 93.188; 95% confidence interval [CI], 1.633–5,316.628; P = 0.027); however, the placement of prophylactic cerclage (OR, 19.264; 95% CI, 0.915–405.786; P = 0.057) was not a factor. None of the women who received prophylactic cerclage delivered before 35 weeks' gestation in the index singleton pregnancy.@*CONCLUSION@#Cerclage did not lower the risk of preterm birth in a subsequent singleton pregnancy after a twin sPTB. However, emergency cerclage was an independent risk factor for preterm birth and there was no preterm birth before 35 weeks' gestation in the prophylactic cerclage group. Therefore, close monitoring of the cervical length and prophylactic cerclage might be considered in women who have experienced a twin sPTB at extreme gestation.

The National Health Information Standards Committee was established in 2004 in Korea. The practical subcommittee for laboratory test terminology was placed in charge of standardizing laboratory medicine terminology in Korean. We aimed to establish a standardized Korean laboratory terminology database, Korea-Logical Observation Identifier Names and Codes (K-LOINC) based on former products sponsored by this committee. The primary product was revised based on the opinions of specialists. Next, we mapped the electronic data interchange (EDI) codes that were revised in 2014, to the corresponding K-LOINC. We established a database of synonyms, including the laboratory codes of three reference laboratories and four tertiary hospitals in Korea. Furthermore, we supplemented the clinical microbiology section of K-LOINC using an alternative mapping strategy. We investigated other systems that utilize laboratory codes in order to investigate the compatibility of K-LOINC with statistical standards for a number of tests. A total of 48,990 laboratory codes were adopted (21,539 new and 16,330 revised). All of the LOINC synonyms were translated into Korean, and 39,347 Korean synonyms were added. Moreover, 21,773 synonyms were added from reference laboratories and tertiary hospitals. Alternative strategies were established for mapping within the microbiology domain. When we applied these to a smaller hospital, the mapping rate was successfully increased. Finally, we confirmed K-LOINC compatibility with other statistical standards, including a newly proposed EDI code system. This project successfully established an up-to-date standardized Korean laboratory terminology database, as well as an updated EDI mapping to facilitate the introduction of standard terminology into institutions.

The large volume of reactive oxygen species are generated during reperfusion after transient or post-procedural ischemia, which leads to cell injury. This ischemia-reperfusion (IR) injury may cause local and even systemic injuries. Thus, the need to reduce the IR injury has been highlighted and in this regard studies have demonstrated the ischemic preconditioning (IP) in which short ischemia and reperfusion are repeated before ischemia. Such IP is known to protect the tissues from IR injury by reducing inflammation response during ischemia. Thus, this study was based on IP known to protect the tissue with developing the mechanism of resistance to ischemia and reperfusion injury in cellular tissue. As the substance that plays an important role in the inflammatory response during IR injury is cytokines, this study was intended to review and discuss the methodologies of IP as well as to analyze the correlation of its effects on the expression of cytokines. Left common iliac artery in male mice of which weight was from 40 g to 45 g, was treated for ischemia. The animal groups consisted of ischemia (IC) group receiving 2-hour ischemic treatment alone; IP group receiving short 5-minute ischemia and reperfusion treatments repeated three times; and, ischemic preconditioning-ischemia (IP-IC) group receiving IP treatment followed by 2-hour ischemic treatment. Following these treatments in each group, reperfusion for intergroup comparisons was carried out at 30 minutes, and 1, 2 and 4 hours. The results of this study were as follows: First, the expression level of pro-inflammatory cytokines, IL-1β was the highest in IC group receiving 2-hour ischemic treatment alone (p<.001). Second, the expression level of anti-inflammatory cytokine, IL-4 was the highest in the IP group (p<.001). Third, the expression level of anti-inflammatory cytokine, IL-10 was the highest in the IP group (p<.001). In conclusion, even though the results had the degree of difference, the expression level of pro-inflammation cytokine, IL-1β in IC group was significantly lower than that in IP group, and the expression levels of anti-inflammatory cytokines, IL-4 and IL-10 in IP were significantly higher than those in IC group and IP-IC group.

Respiratory isolates of Klebsiella pneumoniae in Korea during 2002-2003 were studied to determine the prevalence and types of extended-spectrum beta-lactamases (ESBLs) and plasmid-mediated AmpC beta-lactamases (PABLs). ESBL-production was tested by double-disk synergy, and genotypes of beta-lactamases were determined by PCR and sequencing. ESBLs were detected in 28.4% of 373 isolates, and the most prevalent types were SHV-12 (63 isolates) and CTX-M-14 (9 isolates). Forty of 75 ESBL-producers (53.5%) also had PABLs: 21 isolates with CMY-2-like, 17 with DHA-1-like. Pulsed-field gel electrophoresis showed 19 types and 25 of 74 isolates had an identical pattern, indicating nosocomial spread. Dissemination of ESBL- and PABL-producing K. pneumoniae strains in Korea is a particular concern, as it limits the choice of antimicrobial agents for treatment of infections.
