ObjectiveTo investigate the efficacy and safety of cinobufagin tablets combined with thalidomide/dexamethasone (TD) regimen in the treatment of newly diagnosed multiple myeloma (NDMM) with phlegm and stasis obstruction. MethodsThe clinical data of 50 patients with NDMM of phlegm and stasis obstruction who were hospitalized at the Jiangsu Province Hospital of Chinese Medicine from June 1st, 2015 to July 31th, 2019 were retrospectively analyzed, and they were divided into a control group (bortezomib/dexamethasone-containing regimen, 27 cases) and an observation group (cinobufagin tablets combined with TD regimen, 23 cases). The clinical efficacy and safety were compared between the two groups after two or three courses of treatment. The primary outcomes were clinical remission rate including overall response rate and deep remission rate, one-year and two-year overall survival rate, and adverse effects. The secondary outcomes were the proportion of plasma cells in bone marrow, hemoglobin, β2-microglobulin, lactate dehydrogenase, serum creatinine, blood urea nitrogen, bone pain score, and KPS functional status score (KPS score) before and after treatment. ResultsIn terms of clinical efficacy, there was no statistically significant difference (P>0.05) in the overall response rate [the observation group 69.57%(16/23) vs the control group 70.37% (19/27)] and deep remission rate [the observation group 56.52% (13/23) vs the control group 55.56% (15/27)] between groups after the treatment. The one-year overall survival rates of the observation group and the control group were 90.9% and 92.4%, and the two-year overall survival rates were 81.8% and 80.9% respectively, with no statistically significant differences between groups (P>0.05). During the treatment, no renal function injury occurred in both groups. The incidence of peripheral nerve injury in the observation group was 8.70%, which was lower than 48.15% in the control group (P<0.01). After the treatment, the proportion of myeloma plasma cells, β2-microglobulin, serum creatinine level, and bone pain score decreased, while the hemoglobin level and KPS score increased in both groups (P<0.05 or P<0.01). Compared between groups after treatment, the bone pain score of the observation group was lower than that of the control group, while the KPS score was higher than that of the control group (P<0.05). ConclusionThe clinical efficacy of cinobufagin tablets combined with TD in the treatment of NDMM is equivalent to bortezomib/dexamethasone-containing regimen, but the former is more helpful in relieving the pain and improving the quality of life, and has better safety.

Objective:To prepare specific molecular probe 18F-AlF-1, 4, 7-triazacylononane-1, 4, 7-triacetic acid-(polyethylene glycol) 4-ZD2 ( 18F-AlF-NOTA-PEG 4-ZD2) for targeting extradomain-B fibronectin (EDB-FN), and evaluate its properties in vitro and in vivo. Methods:18F-AlF-NOTA-PEG 4-ZD2 was prepared by one-step chelation labeling with Al 18F. The radiochemical purity and in vitro stability were determined by high performance liquid chromatography (HPLC). The partition coefficient (logP) of 18F-AlF-NOTA-PEG 4-ZD2 was evaluated, and the cell uptake experiment was carried out (triple-negative breast cancer (MDA-MB-231) cells (1×10 6/tube) were divided into 3 groups ( n=3 per group); positive group, inhibition group, control group). MicroPET imaging was performed on MDA-MB-231 bearing nude mice ( n=3) after 18F-AlF-NOTA-PEG 4-ZD2 injection (30, 60, 90, 120 min) and compared with blocking group ( n=3, NOTA-PEG 4-ZQ 2 was preinjected at 0.5 h before 18F-AIF-NOTA-PEG a-ZD2 injection). Independent-sample t test was used to analyze the data. Results:18F-AlF-NOTA-PEG 4-ZD2 was successfully prepared. The optimized radiochemical yield was (33.8±2.1)% (undecay corrected, n=8) and the radiochemical purity was >96%. After incubating 120 min at 37 ℃, the radiochemical purity of 18F-AlF-NOTA-PEG 4-ZD2 in human serum and PBS was >93%, indicating its good stability in vitro. The specific activity was (11.1±3.2) GBq/μmol, and logP was -1.43±0.05. The uptake value of tumor cells was (1.77±0.28) percentage applied activity (%AR)/10 6 cells at 120 min post-injection in positive group, and the total uptake value of the inhibition group was (0.76±0.07) %AR/10 6 cells ( t=4.30, P=0.032). MicroPET imaging in tumor bearing nude mice showed that 18F-AlF-NOTA-PEG 4-ZD2 was mainly metabolized by the liver and kidneys. The tumor uptake value was (1.94±0.21) percentage activity of injection dose per gram of tissue (%ID/g) at 60 min post-injection and the tumor/muscle ratio was 3.80±0.25 at 90 min post-injection in the experimental group, while the tumor uptake value of tumor bearing nude mice in the blocking group was (0.43±0.09) %ID/g at 60 min post-injection ( t=3.18, P=0.006). Conclusions:18F-AlF-NOTA-PEG 4-ZD2 can be prepared simply with high labeling rate and good stability in vitro, with high tumor uptake and tumor/muscle ratio in microPET imaging, and good specificity and long tumor residence time. The probe has good application prospect in breast cancer with high expression of fibronectin subtype EDB-FN.

We constructed the CS1-targeted second- and third-generation CAR-T cells with genetic engineered 4-1BB or/and ICOS as a costimulatory signaling molecule by use of lentiviral platform. The CS1-targeted second-generation CAR-T cells with ICOS or 4-1BB had similar anti-neoplastic activity. When effector/target ratio was 1:1, the CAR-T cells with ICOS showed better killing effect on IM9-lucgfp cells than those with 4-1BB. However, The CS1-targeted third-generation CAR-T cells exihibited lower cytolytic capacity against IM9-lucgfp cells than the CS1-targeted second-generation CAR-T cells when the ratio of effector/target was 1:1, 2:1 or 5:1. When the ratio of effector/target was 10:1, the killing efficacy of both the second- and third-generation CAR-T cells against IM9-lucgfp cells was more than 85%, significantly higher than that of the control T cells. Taken together, both the CS1-targeted second- and third-generation CAR-T cells with ICOS or/and 4-1BB could efficiently kill CS1-positive multiple myeloma cells, but the CS1-targeted second-generation CAR-T cells had more potent killing effect on CS1-positive multiple myeloma cells than the CS1-targeted third-generation CAR-T cells.

Objective@#To explore the anatomical characteristics of the mesopancreas, to define the range of the total mesopancreas excision and to evaluate the feasibility, safety and effectiveness in the treatment of pancreatic cancer.@*Methods@#A regional anatomical and pathological study was performed on 14 cadavers with large slices and paraffin sections. The clinical and pathological data of 58 consecutive patients underwent total mesopancreas excision for pancreatic head carcinoma from January 2013 to December 2015 were prospectively collected and analysed. The perioperative morbidity, mortality and clinical outcomes of patients underwent total mesopancreas excision were compared with the patients underwent conventional pancreaticoduodenectomy from January 2010 to December 2012.@*Results@#The mesopancreas located in the retropancreatic area, extending from the head, neck, and uncinated process of pancreas to the aorto-caval groove, in which there were loose areolar tissue, adipose tissue, nerve plexus, lymphatic and capillaries. Although no fibrous sheath or fascia like mesocolorectum was found around the structures, a relatively fixed extent could be defined according to its embryologic and anatomic characters. In clinical practice, total mesopancreas excision was classified into two levels according to the extent of resection in this series: level Ⅰ was a"standard total mesopancreas excision" or"total mesopancreas excision in a narrow sense" , which was similar to the extent of standard resection from consensus statement of ISGPS. Level Ⅱ was defined as any procedure extending the range of level Ⅰ, called the"extended total mesopancreas excision" or"total mesopancreas excision in a broad sense". In TMpE group, the intraoperative blood loss( (461.4±184.5)ml vs. (532.2±319.8)ml, P=0.301), operation time( (368.6±92.5)minutes vs. (397.1±112.7)minutes, P=0.559), total complication rate (39.7% vs. 51.2%, P=0.250), fistula mortality (25.9% vs. 30.2%, P=0.628) were all reduced. There were significantly higher R0 rate (91.4% vs.76.7%, P=0.041) and more harvested lymph nodes (16.2 vs. 11.4, P=0.000) and lower total and local recurrence: rate (half-year local recurrence rate: 7.8% vs. 23.7%, P=0.036; one-year local recurrence rate: 18.2% vs. 39.5%, P=0.018) and longer disease-free survival (16.9 months vs. 13.4 months, P=0.044) and overall survival(22.5 months vs. 19.9 months, P>0.05) were also found in the study group.@*Conclusions@#Mesopancreas is different from mesorectum since it has no fascial envelop, which should be regarded as a surgical concept, rather than an anatomical structure. Total mesopancreas excision is safe and feasible for pancreatic head cancer and probably helps to increase the R0 resection rate and improve the clinical outcomes.

Interventions: An analysis of the target/non-target [T/N] value of the focus was performed
Main outcome measure: T/N value of the nasopharyngeal focus and neck
Results: The focus hypoxic examination of 32 of 38 patients was positive [84%]. T/N values for hypoxic status in the 32 patients with a positive nasopharyngeal focus and normal tissue were 1.89 +/- 0.95 and 1.18 +/- 0.36, respectively. The mean T/N values for NPC hypoxic focus before and after radiotherapy were significantly different among the 32 patients. The hypoxic status of the nasopharyngeal focus positively correlated with its response to radiotherapy. The correlation coefficient was 0.641
Conclusions: 99mTc-HL91 hypoxic imaging can reveal the hypoxic status of the NPC focus. The hypoxic status of NPC was closely correlated with the curative effect of 3-D CRT

BACKGROUND:Currently, it is stil controversial about the border, surrounding fascia, space of pelvic cavity, distribution of nerves and lymph nodes of the mesorectum, and the development of new technologies makes a progress in related anatomic research. OBJECTIVE:To summarize the previous studies so as to describe clearly the progress of mesorectal anatomy and to discuss its clinical value. METHODS: Using “rectum; mesentery; fascia; space; nerve; lymph node; total mesorectal excision (TME); clinical anatomy” as key words, a computer-based search of PubMed was done for articles related to the mesorectum and surrounding fasciae, space of pelvic cavity, distribution of nerves and lymph nodes. RESULTS AND CONCLUSION:Fresh or frozen specimens are often used for studying the mesenterium, fascia, nerves and lymph nodes by using traditional pelvic and perineum anatomical methods. Computer-assisted anatomical dissection can combine immunostaining with computer imaging. A three-dimensional model can wel reflect the relationship among the different anatomical structures, as wel as nerve traveling and spatial location. Mesorectum is located behind the denonviliers and in the front of the sacral fascia of the rectum. Pelvic splanchnic nerve of the mesorectum is derived from the anterior sacral nerve root, runs through the presacral fascia, and enters into the neuro-fascial layervia the pesacral space, which is divided into the upper and lower parts according to the peritoneum. There are more folds in the rear of lymph nodes within the mesorectum within and near the peritoneum. There are stil a lot of controversies about anatomical relationship between the mesorectum and surrounding structures, and to elaborate these issues can provide an objective basis for guiding clinical work.

Objective To study the optimum condition of preparing ofloxaxcin gelatin microspheres. Methods Oflox?axcin gelatin microspheres were manufactured using the emulsion chemical-cross linking method and gelatin was employed as carrier, liquid paraffin as oil phase, Span80 as emulsifier, . The loading capability and entrapment efficiency of the ofloxax?cin gelatin microspheres were determined by UV Spectrophotometry. The effect of gelatin concentration, oil/water volume ra?tio, gelatin/ofloxaxcin mass ratio and volume fraction of span80 on drug loading capability and entrapment efficiency were in?vestigated. The optimum proportions of each component was obtained by L9 (34) orthogonal test, based on the above 4 factors, using sum of drug loading capability and entrapment efficiency as evaluation index. Results The optimum condition for manufacturing high quality ofloxaxcin gelatin microspheres used 20%of gelatin concentration,water/oil volume ratio at 3.5∶1, gelatin/ofloxaxcin mass ratio at 1∶1, the span 80 volume fraction of 2.5%. Conclusion Drug loading capability and entrap?ment efficiency of the ofloxaxcin gelatin microspheres reached 80%using this manufacture technology therefore the prepara?tion was stable and feasible.

BACKGROUND:The pelvic nerves innervate the pelvic viscera as wel as bring sensory information to the central nerve system, including splanchnic nerves and spinal nerves. Each of them comprises both motor fibers and sensitive fibers. Mostly, the key part of splanchnic plexus is pelvic plexus. Total mesorectal excision proposed by Heald in 1982 has been the “gold standard” for diagnosis and treatment of colorectal cancer. However, it carries a high risk of nerve damage during surgery, which results in urinary retention, sexual dysfunction and other complications. OBJECTIVE:To summarize the former researches so as to get a precise understanding of the pelvic fascia and nerve structure. METHODS:Using “splanchnic nerves, superior hypogastric plexus, pelvic plexus, pelvic splanchnic nerve, total mesorectal excision (TME), clinical anatomy” as key words, a computer-based search of PubMed was done for articles related to the pelvic nerves, including its pathway, consistent, ganglia, and reflection in pelvic viscera, published from 2000 to 2015. RESULTS AND CONCLUSION:The main splanchnic plexus in the pelvic cavity includes superior hypogastric plexus (it is located in the triangle formed by left and right common iliac artery and the sacral promontory), and pelvic plexus (hypogastric nerve, pelvic splanchnic nerve and sacral splanchnic nerve converge at the bottom of rectum, formed pelvic plexus, also known as the inferior hypogastric plexus). It is flattened against the lateral aspect of the rectum, the dorso-lateral bladder wal and the seminal vesicles. Nerves come from the plexus contain the sympathetic nerve, parasympathetic nerve and sensory nerve. They are in charge of the motions and sensations of the pelvic organs. The definite knowledge on the anatomy of pelvic fascia and nerve structures can avoid nerve damage during operation, which can help to improve the life quality of patients.

Objective To evaluate the desflurane-induced sympathetic activation with finger tip perfusion index (FTPl). Methods Forty-eight ASA ⅠorⅡ patients aged 25-60 yr undergoing elective surgery under general anesthesia were randomly divided into 3 groups (n=16 each): groupⅠsevoflurane; group Ⅱ desflurane and groupⅢ desthrane + propofol. Anesthesia was induced with midazolam 0.03 mg/kg, propofol 2 mg/kg and fentanyl 2 μg/kg. Tracheal intubatian was facilitated with vecuronium 0.1 mg/kg. The patients were mechanically ventilated (VT 8-10 ml/kg, RR 12 bpm). PETCO2 was maintained at 35-40 mm Hg. In group ⅠandⅡanesthesia was maintained with sevoflurane and desflurane respectively. The end-tidal concentration of sevoflurane and desflurane were rapidly increased to 0.5 MAC, 1.0 MAC and 1.5 MAC and maintained at each level for 5 rain. In group Ⅲanesthesia was maintained with TCI of propofol and destlurane. The target plasma prepofol concentration was set at 1 μg/ml and the end-tidal concentration was rapidly increased to 0.5 MAC and 1.0 MAC and maintained at each level for 5 mitt. MAP, HR, FTPI and BIS were continuously monitored and recorded at 5 rain after midazolam (T0), 3 rain after induction of anesthesia (T1), immediately after tracheal intubation (T2), as soon as the end-tidal concentration of sevoflurane and desflurane reached 0.5 MAC(T3), 1.0 MAC(T5) and 1.5 MAC(T7) and after being maintained at 0.5 MAC (T4), 1.0 MAC (T6) and 1.5 MAC (T8) for5 rain. The plasma renin activity and the levels of angintonin Ⅱwere determined at T0, T1, T2, T5 and T7 by using radioimmuno-ssay. Results FTPI was significantly lower at T5 than at T4 in group Ⅱ HR and MAP were significantly lower at T4-6 and T8 and FTPI were significantly lower at T3~6 and T0 than at T7 in group Ⅱ. HR and MAP were significantlyhigher while FTPI was lower at T7 in groupⅡthan in group Ⅰ The changes in MAP and FTPI between T4 and T5 were significantly less in group Ⅲthan in group Ⅱ. The time when the highest value of FTPI was maintain was significantly shorter than the time when the highest values of HR and MAP were maintained at T5 and T7 in group ⅡΔFTPI was negatively correlated with ΔHR and ΔMAP (r=-0.593, P<0.05;r=-0.591, P<0.05). Conclusion FTPI can reflect the desflurane-induced sympathetic activation promptly and sensitively.

Apoptosis and necrosis are two forms of liver diseases at cell level. Research on hepatocyte apoptosis will become the important topic for treatment of liver disease and understanding of regeneration after liver transplantation. In this study, we analyzed the correlation between endoplasmic reticulum stress and cell apoptosis from effect and function of endoplasmic reticulum stress, influence of endoplasmic reticulum stress on cell apoptosis and association between hepatocyte apoptosis and related disease. Endoplasmic reticulum stress is a self-protection mechanism for cells, but too much endoplasmic reticulum stress will harm cells even cause apoptosis. It is still unclear about the relationship of the endoplasmic reticulum stress and hepatocyte apoptosis, so we need further researches. The progress of studies about the relations between hepatocyte diseases and endoplasmic reticulum stress will improve treatments of liver diseases.

Portal hypertension (PHT), as a disease with high incidence all over the world, badly affects the patients' health. The research on hemodynamics plays an important role in exploring the pathogenesis of PHT, assessing the risk of hemorrhage induced by the complications, selecting the scheme and time of surgical operations as well as evaluating the curative effects of medication. In this article, the main factors in the pathogeny of PHT are reviewed and the surgical treatments are discussed from the hemodynamic viewpoint. Then some recent results in the hemodynamic research of PHT are summarized. In addition, main ideas are put forward as to establishing a new global biomechanical model of PHT.

<p><b>OBJECTIVES</b>To investigate the clinical value of combined dye-isotope technique in detecting sentinel lymph node (SLN) and to examine whether the characteristics of SLN accurately predict cervical lymph node metastasis in lingual carcinoma.</p><p><b>METHODS</b>Thirty patients with lingual carcinoma without lymph metastasis were injected with a dose of about 18.5 MBq of (99m)Tc-SC (sulfur colloid), around the tumor tissues before surgery, and lymphoscintigraphy was performed 5, 10, 30, 60 minutes, and 6 hours after injection. In the following day, all patients were injected with isosulfan blue dye around the primary tumor during surgery to trace SLN and underwent standard cervical lymph node dissection after SLN dissection. The pathological results of SLN were compared with standard lymph node dissection for their ability to accurately predict the final pathological status of the cervical lymph nodes.</p><p><b>RESULTS</b>SLN was successfully identified in 100% of the patients. Both positive and negative predictive values of SLN were 100%. The accuracy rate was 100%, and there were no false negatives.</p><p><b>CONCLUSIONS</b>The detection of SLN using combined dye-isotope technique could accurately predict cervical lymph node metastasis in lingual carcinoma.</p>

Objective To observe the change of renal morphological structure and apoptosis of renal cells. Methods Healthy adult male Wistar rats were randomly divided into control group(n=5),2 weeks CCl_4 group(n=5) and 6 weeks CCl_4 group(n=5).The control group were injected oily solution with 0.12ml/100g rat weight,the CCl_4 group were subcutaneously injected 60% carbon tetrachloride oily solution with 0.3ml/100g rat weight.At the first day of 3 weeks and 7 weeks,2 weeks CCl_4 group and 6 weeks CCl_4 group rats were respectively killed.The morphological structure in renal central section was observed after HE staining.The expressions of Caspase-3 and Bax were detected by immunohistochemistry.The apoptosis of rat renal cells was detected by TUNEL staining. Results The structures of rat renal tubules and corpuscle was normal in the control group.We detected the histological change in proximal tubules in the 6 weeks CCl_4 group,tiny tomentums of epithelial cells fell off,cells was vacuolization.The structure of renal corpuscle was normal.The pathological changes of 2 weeks CCl_4 group were similar,but relative little and limited.There was no Caspase-3 and Bax positive cell in control group and 2 weeks CCl_4 group,but lots of Caspase-3 and Bax positive cells were in 6 weeks CCl_4 group,mainly fastened on epithelial cells of proximal tubules near the medulla,and brown granules were mostly located in the cytoplasm.There was no positive cell in control group by TUNEL staining.There were a lot of apoptosis epithelial cells of proximal tubules,renal glomerular also had a few of TUNEL positive cells in the 2 weeks CCl_4 group.Apoptosis cells mainly located in the epithelial cells of distal tubules in the 6 weeks CCl_4 group,a few of apoptosis epithelial cells were in proximal tubules.Conclusion In carbon tetrachloride induced renal injuried rats,apoptosis cells mainly happened in epithelial cells of proximal tubules in 2 weeks.The apoptosis of epithelial cells of distal tubules presented 6 weeks later.

Objective To explore changes of three-dimensional organization of collagen fibers framework in hepatic fibrosis rats. Methods Healthy adult male Wistar rats(n=30) were randomly divided into control group(n=10) and hepatic fibrosis group.Hepatic fibrosis rats were induced by subcutaneous injections of 60% carbon tetrachloride oily solution.Rats were killed at the end of 10 week and 12 week as 10week(n=10) and 12week(n=10) hepatic fibrosis group.Distribution of collagen was detected by sirius red and polarized microscope,and the architecture was observed under the scanning electron microscope in hepatic fibrosis rats. Results Under the scanning electron microscope,with aggravation of hepatic fibrosis,kinds of collagen fibers and interlaced reticulation compartment were observed,some round or olivary !honeycomb"-like frameworks were formed,which disparted and wrapped up normal liver lobule,and the collagen fiber sheaths formed the deposition of collagen fibers surrounded portal areas or vessels.Pseudolobule vesicles formed in 12week hepatic fibrosis rats were more delicate and intact than that of in 10week hepatic fibrosis rats. Conclusion The microframework of the collagen fibers was changed in hepatic fibrosis.This structure was mainly composed of thick and slight hyperplastic collagen fibers.;

Objective To research the expression of glucose regulating protein 78(GRP78) in livers of carbon tetrachloride(CCl-4) induced hepatocirrhosis rats.Methods Healthy adult male Wistar rats were randomly divided into control group(n=5)and hepatocirrhosis group(n=5).Oily solution with 0.12ml per 100g rat weight was subcutaneous injected in control group,and 60% carbon tetrachloride oily solution with 0.3ml per 100g rat weight were subcutaneous injected in hepatocirrhosis group for 20 weeks.At the first day of 21 weeks,all rats were killed.Each blood was collected from right ventricle and some hepatic function was measured.The morphological structure in hepatic middle lobule were observed through the staining of H E and Sirius red.The expressions of GRP78 was detected by immnunohistochemistry,Western blotting and RT-PCR.Results The serum ALT in 20 weeks hepatocirrhosis group(278.2?88.42) was evidently higher than that in control group(154.8?9.94,t=3.10,P

Objective To study the effect of transplanted with human mesenchymal stem cells(MSCs) on the rat liver fibrosis in carbon tetrachloride(CCl-4)-induced hepatocirrhosis rats. Methods Wistar rats were divided into control(n=8) and hepatocirrhosis groups,and 24 hepatocirrhosis rats were made by subcutaneous injection with 60% CCl-4-peanut oil for 7 weeks,named 7-week hepatocirrhosis group(n=8).Other 7 weeks hepatocirrhosis rats were subdivided into MSC control group(n=8) and MSC transplanted group(n=8).After identification to MSCs with FACS calibur flow cytometry,MSCs were injected into the superior mesenteric vein for 3 weeks in MSC transplanted group.All of rats were killed at the beginning of 11-week.The clinical blood biochemical parameters from the inferior vena cava were examined.The survival of human MSCs in liver was observed by immunohistochemistry reaction of anti-hEGF-R.The degree of liver cirrhosis was evaluated by stains of Sirius red. Results Plasma ALP,ALB and CHO were abnormal to varying degree in 7-week hepatocirrhosis,MSC control and MSC transplanted groups.In 7 weeks hepatocirrhosis rats,a large amount of hyperplastic collagenous fiber was presented in liver tissue and the frame of the pseudolobuli was formed.These were similar to MSC control group.Only in MSC transplanted group,a few human EGF-R(+) cells were observed in livers.The amount of hyperplastic collagenous fiber in MSC transplanted group was significantly lower than those of the 7-week hepatocirrhosis and MSC control gruops.Conclusion Liver fibrosis could be improved obviously in hepatocirrhosis rats transplanted with human MSCs.These experimental results could probably provide an experimental evidence of treatment of liver cirrhosis.
