Este artigo descreve o acesso e a percepção do acesso de usuários a serviços especializados no tratamento do vírus da imunodeficiência humana e da Síndrome da Imunodeficiência Humana (HIV/AIDS) em municípios baianos por meio de um recorte epidemiológico descritivo-analítico, com abordagem quantitativa de dados primários de estudo realizado em cinco serviços localizados na capital e oito no interior do estado da Bahia, Brasil. Participaram deste estudo 475 usuários, de 21 municípios baianos, predominantemente do sexo feminino, com faixa etária entre 25 e 44 anos, negros, cristãos, desempregados, com escolaridade compreendendo o ensino fundamental, renda de até três salários mínimos, heterossexual e cisgênero. O acesso à testagem para HIV e outras Infecções Sexualmente Transmissíveis (IST), profilaxias de pós-exposição e pré-exposição e outros insumos de prevenção não constituíram motivação de inserção no serviço. O tratamento específico e a assistência médica para HIV e outras IST foi motivação para que a maioria dos participantes se direcionassem ao serviço especializado. O deslocamento até o serviço de referência é realizado principalmente por transportes coletivos, e o trajeto casa-serviço tem duração de pelo menos uma hora. O horário de funcionamento da unidade atende às necessidades da maior parte dos participantes, mas parcela dos usuários mencionaram o desejo de funcionamento em horários alternativos. A marcação de consultas é realizada principalmente de forma presencial e por telefone, com possibilidade de agendamento diário, assim a maioria dos usuários avaliaram a organização do serviço para marcação de consultas como boa ou muito boa. O tempo de espera, entre marcação e consulta, mais comum é de uma semana a um mês.


This article describes user access and perception of access to specialized HIV/AIDS services in Bahia municipalities by applying a qualitative epidemiological, descriptive-analytical approach to primary data from research conducted in five services located in the capital and eight in the countryside. A total of 475 users from 21 municipalities in Bahia participated in this study. Most were female, between 25 and 44 years old, Black, Christian, unemployed, with complete primary education, income of up to 3 minimum wages, heterosexual and cisgender. Access to HIV testing and other STIs, post-exposure and pre-exposure prophylaxis and other prevention inputs did not constitute motivation for procuring the service. Specific treatment and medical care for HIV and other STIs motivated most participants to turn to specialized services. Commuting to the reference service is made mainly by public transport and the home-service journey takes at least one hour. The unit's opening hours meet the needs of most participants, but some mentioned the desire for alternative operating times. Appointments are booked mainly in person and over the phone, with the possibility of daily scheduling, so most users rated the service organization for scheduling appointments as good or very good. The most common waiting time between booking and consultation is one week to one month.


Este artículo describe el acceso y la percepción de acceso de los usuarios a servicios especializados en el tratamiento del virus de inmunodeficiencia humana y síndrome de inmunodeficiencia humana (VIH/sida) en municipios de Bahía (Brasil) mediante un enfoque epidemiológico, descriptivo-analítico, y un enfoque cuantitativo de datos primarios de un estudio realizado en cinco servicios ubicados en la capital y ocho en el interior del estado de Bahía. Participaron en este estudio 475 usuarios, de 21 municipios de Bahía, con mayor predominio femenino, con edades de entre 25 y 44 años, negros, cristianos, desempleados, con educación primaria, ingresos de hasta tres salarios mínimos, heterosexuales y cisgénero. El acceso a pruebas de VIH y otras infecciones de transmisión sexual (ITS), profilaxis posexposición y preexposición, y otros insumos de prevención no constituyeron una motivación para ingresar al servicio. El tratamiento específico y la asistencia médica para el VIH y otras ITS fueron la razón para que la mayoría de los participantes buscaran servicios especializados. El desplazamiento al servicio de referencia se realiza principalmente en transporte público, y el trayecto desde el domicilio al servicio tiene una duración mínima de una hora. El horario de atención de la unidad satisface las necesidades de la mayoría de los participantes, pero algunos usuarios mencionaron el deseo de que operase en horarios alternativos. Las citas se realizan principalmente de forma presencial y telefónica, con posibilidad de concertación diaria, por lo que la mayoría de los usuarios valoran como buena o muy buena la organización del servicio de citas. El tiempo de espera más habitual entre la reserva y la consulta es de una semana a un mes.

Introdução: A correção da relação radix dorso nasal é realizada há vários anos utilizando as mais variadas técnicas como enxerto de cartilagem auricular, septal ou costal, preenchimento com ácido hialurônico, uso de fáscia e enxertos e uso de cartilagem picada em cubo, silicone e ácido hialurônico. O uso de cartilagem fragmentada é descrito na literatura e tem ganhado adeptos nos últimos anos, pela facilidade em ser realizado. O objetivo neste estudo é descrever nossa experiência com a utilização do enxerto de cartilagem fragmentado no aumento do radix comparando a percepção visual e tátil do paciente e sua satisfação. Método: Estudo observacional em pacientes submetidos a rinoplastia no período de janeiro 2018 a junho de 2022, em cirurgias nas quais ocorreu o aumento de radix com o uso de enxerto de cartilagem picada. Resultados: Dos 47 pacientes, a maioria era do sexo feminino (35, 74,4%), com média de idade de 34,6 anos (18-44). Quanto ao tipo de pele, Fitzpatrick tipo 3 (n=28, 59,5%) foi o principal, sendo encontrado frequentemente pacientes com pele de média espessura (n=23, 48,9%). Complicações presentes foram infecção (1 caso), migração de cartilagem picada (3 casos), e reabsorção parcial (1 caso). Na avaliação da percepção tátil 42 pacientes (89,3%) percebiam, à palpação, as proeminências cartilaginosas e na visual apenas 2 (4%). A satisfação foi elevada em 45 (96%) pacientes. Conclusão: A cartilagem picada livre pode ser utilizada na região do radix com resultados satisfatórios.


Introduction: Correction of the nasal radix dorsum relationship has been carried out for several years using the most varied techniques such as auricular, septal, or costal cartilage grafts, filling with hyaluronic acid, use of fascia and grafts and the use of diced cartilage, silicone, and hyaluronic acid. The use of fragmented cartilage is described in the literature and has gained popularity in recent years, due to its ease of use. The objective of this study is to describe our experience with the use of fragmented cartilage graft in radix augmentation, comparing the patient's visual and tactile perception and satisfaction. Method: Observational study in patients undergoing rhinoplasty from January 2018 to June 2022, in surgeries in which the radix was increased with the use of minced cartilage graft. Results: Of the 47 patients, the majority were female (35, 74.4%), with a mean age of 34.6 years (18-44). As for skin type, Fitzpatrick type 3 (n=28, 59.5%) was the main one, with patients with medium-thickness skin being frequently found (n=23, 48.9%). Complications present were infection (1 case), migration of chopped cartilage (3 cases), and partial resorption (1 case). In the assessment of tactile perception, 42 patients (89.3%) perceived cartilaginous prominences on palpation and only 2 (4%) visually. Satisfaction was high in 45 (96%) patients. Conclusion: Free minced cartilage can be used in the radix region with satisfactory results.

RESUMO O objetivo deste estudo foi compreender as representações sociais de jovens que vivem com o HIV. Estudo qualitativo, ancorado na Teoria das Representações Sociais, realizado com 16 jovens em acompanhamento ambulatorial. Os dados foram coletados entre maio e outubro de 2022, por meio de entrevista semiestruturada. Para o tratamento dos dados, empregou-se a técnica de análise lexical, operacionalizada com o auxílio do software Iramuteq. A representação fortemente associada ao diagnóstico de HIV está relacionada ao desconhecimento, preconceito e aceitação, apresentando, também, elementos positivos como recomeço. Classes: Diagnóstico positivo para o HIV: o 'eu' de agora; HIV em face do desconhecimento e do estigma; e Relações interpessoais: diálogo, acolhimento e respeito necessários aos jovens. Compreendeu-se que as representações sociais dos jovens com HIV refletem a sua condição na sociedade. Os dados deste estudo mostram que existem formas distintas de vivenciar o HIV, muitas vezes representadas pela fragilidade, pela resignação, e outras, pelo fortalecimento. A fim de minimizar a vulnerabilidade do jovem à exposição ao HIV e aos riscos de conviver com o estigma e preconceito social, fazem-se necessárias a adoção de estratégias de conscientização da sociedade sobre a doença e a ampliação da divulgação de políticas públicas que abarcam essa temática.


ABSTRACT This study was aimed at understanding the social representations of young people living with HIV. This is a qualitative study, based on the Social Representations Theory, carried out with 16 young people in outpatient follow-up. Data were collected between May and October 2022, through semi-structured interviews. For data processing, the lexical analysis technique was used, operationalized with the aid of the IRAMUTEQ software. The representation strongly associated with the diagnosis of HIV is related to lack of knowledge, prejudice and acceptance, also presenting positive elements such as a new beginning. Classes: Positive diagnosis for HIV: the new 'me'; HIV in the face of ignorance and stigma; and Interpersonal relationships: dialogue, acceptance and respect necessary for young people. It was understood that the social representations of young people with HIV reflect their condition in society. The data show that there are different ways of experiencing HIV, often represented by fragility, resignation, and other times by strengthening. In order to minimize the vulnerability of young people to exposure to HIV and the risks of living with stigma and social prejudice, it is necessary to adopt strategies to raise awareness about the disease and to expand the dissemination of public policies that cover this topic.

Objetivo: desarrollar un puntaje predictivo de mortalidad para pacientes con covid-19. Materiales y méto-dos: estudio retrospectivo, analítico, observacional y transversal, realizado en dos fases. Se revisaron 620 historias clínicas con una cohorte de derivación de 320 pacientes y una de validación de 300 pacientes. Las variables se analizaron con test de Anova, chi cuadrado de Pearson y análisis multivariante con regresión binaria, que determinaron sensibilidad, especificidad y valor predictivo negativo y positivo. Los puntajes se compararon mediante curvas cor con los scoresnews y hews. Resultados: los dos puntajes obtenidos incluyeron valores de edad, conteo de linfocitos, SatO2/FiO2, leucocitos, plaquetas, ausencia de síntomas, hipertensión arterial, epid y dhl. El área bajo la curva (abc) fue de 0.838 para el puntaje con dhl, con una mortalidad del 100 % para 7.75 puntos o más, y un abc de 0.826 para el primer puntaje. En la cohorte de validación, el abc para el primer puntaje fue de 0.831 y para el score con dhl fue 0.855. El puntaje hewsobtuvo un abc de 0.451, y el news, un abc de 0.396. Conclusiones: se desarrollaron dos herramientas para predecir mortalidad en pacientes con covid-19, con alto poder de discriminación, superior a los puntajes británicos hews y news

Background@#Healthcare-acquired infections (HAIs) are adverse events brought about by non-compliance of the healthcare staff to set of infection prevention and control (IPC) standards. Consequently, additional medical costs, increased mortality and morbidity rates, and decreased quality of life among patients can happen. As valuable players in preventing and controlling HAIs, nurses must have good knowledge and strict compliance with infection control; however, recent evidence suggests that nurses may need more knowledge or better integration into practice. Nurses' degree of knowledge and practice in preventing and controlling HAIs and factors influencing them should be determined to provide solutions appropriately.@*Objectives@#The study specifically sought answers to the following questions: (1) What is the level of knowledge of nurses in the prevention and control of HAIs? (2) What is the degree of practice of nurses in the prevention and control of HAIs? (3) Is there a significant relationship between nurses' knowledge level and degree of practice in the prevention and control of HAIs? (4) What are the facilitating and hindering factors that affect nurses' practice in the prevention and control of HAIs?@*Methods@#The study utilized a quantitative descriptive correlational design. The study was conducted from May to June 2023 at a private tertiary hospital in Baguio City. The study included 128 nurses who fit the inclusion criteria. The respondents were asked to answer three questionnaires, and the data were statistically treated using mean, Spearman Rank correlation, frequency, percentage, and rank distribution.@*Results@#The study found that nurses possess good knowledge and a suboptimal degree of practice in preventing and controlling HAIs. The results showed that no significant relationship existed between nurses' knowledge level and degree of practice in preventing and controlling HAIs. Perceived personal benefits and organizational encouragement were seen to primarily facilitate the prevention and control practices of nurses. The primary hindrance identified was workload due to staff shortage, poor dissemination of guidelines, and personal discomfort associated with the use of PPE.@*Conclusions@#A good level of knowledge is a derivative of learnings obtained through various educational modalities and these strategies are considered effective means of knowledge formulation. However, created knowledge without actual application into practice results in overuse of unhelpful interventions. To avoid the unnecessary effects of ineffective knowledge translation, a multifactorial consideration is necessary to identify other factors that may influence the practices of nurses on HAI prevention and control because knowledge does not solely improve or worsen actual practices. Identified facilitating factors should be supported, and hindering factors should be addressed. Further recommendations based on the study results include strengthening existing programs and policies, and developing accessible materials to improve the present practices of nurses.

Résumé

Objective:To investigate the clinical characteristics, therapeutic efficacy and prognosis of patients with human immunodeficiency virus (HIV)-associated Hodgkin lymphoma.Methods:A retrospective case series study was conducted. The clinical data of 22 HIV-associated Hodgkin lymphoma patients in Chongqing University Cancer Hospital from December 2013 to June 2022 were retrospectively analyzed. Their clinical features, laboratory results, treatment, and prognosis were analyzed. Kaplan-Meier method was used to perform survival analysis.Results:The age [ M ( Q1, Q3)] of 22 patients was 44 years old (36 years old, 53 years old); 18 cases were male, 4 cases were female; clinical staging was stage Ⅲ in 5 patients and stage Ⅱ in 17 patients. All 22 patients were infected with HIV through sexual transmission, with 10 cases transmitted through man sex with man and 12 cases transmitted through heterosexual transmission. Nine patients were found to be infected with HIV at the time of diagnosis of lymphoma, and 13 patients presented with lymphoma at 22.2 months (12.3 months, 38.4 months) after diagnosis of HIV infection. Of the 22 patients, 3 abandoned treatment; 19 patients were treated with antiretroviral therapy combined with ABVD regimen chemotherapy, 9 patients had complete remission, and 10 patients had partial remission. After follow-up of 46.8 months (24.8 months, 64.5 months), the 5-year progression-free survival rate was 83.9%, and the 5-year overall survival rate was 89.5%. Conclusions:HIV-associated Hodgkin lymphoma exhibits an invasive process in clinical practice, and standardized antiretroviral therapy combined with ABVD regimen chemotherapy can lead to long-term survival for patients.

Résumé

Dry eye(DE)is a multifactorial ocular surface disorder arising from numerous pathologies. The pathogenesis of DE includes immune inflammation, oxidative stress, changes in tear film composition, corneal nerve abnormalities, and meibomian gland dysfunction. Among them, the immune inflammatory response is the most crucial in the pathogenesis of DE, which is regulated by both innate and acquired immune responses on the ocular surface. Multiple environmental stresses trigger the ocular surface innate immune response leading to corneal epithelial cell damage and inflammation and activate acquired immunity to participate in the ocular surface immune inflammatory response. Currently, multiple immune cells and inflammatory factors have been shown to be involved in the occurrence and development of DE. This article reviewed the immune progress and focused on the initiation and maintenance of acquired immunity in DE. Through the analysis of the latest viewpoints and research hot spots, we systematically introduced the immunomodulating mechanism underlying the mechanisms of the pathogenesis of DE and provided references for the prevention and treatment of DE.

@#Periodontal ligament stem cells (PDLSCs) have the potential for multidirectional differentiation and are the preferred seed cells for periodontal tissue regeneration. In recent years, a large number of studies have confirmed that PDLSCs also possess broad immunomodulatory properties. Therefore, in-depth exploration of their specific molecular mechanisms is of great significance for the treatment of periodontitis. The aim of this paper is to summarize the research progress on the regulation of PDLSCs on various immune cells and the effect of the inflammatory environment on the immune characteristics of PDLSCs to provide an important theoretical basis for the allotransplantation of PDLSCs and improve the therapeutic effect of periodontal tissue regeneration. Studies have shown that PDLSCs possess a certain degree of immunosuppressive effect on both innate and acquired immune cells, and inflammatory stimulation may lead to the impairment of the immunoregulatory properties of PDLSCs. However, current studies are mainly limited to in vitro cell tests and lack in-depth studies on the immunomodulatory effects of PDLSCs in vivo. In vivo studies based on cell lineage tracing and conditional gene knockout technology may become the main directions for future research.

Objective To analyze the seasonal and epidemiological characteristics of community-acquired pneumonia (CAP) in Chuzhou from 2010 to 2022. Methods The epidemiological data of CAP in Chuzhou from 2010 to 2022 were obtained from the center for diseases control and prevention of Chuzhou City, Anhui Province. After inclusion and exclusion, a total of 1 053 cases were enrolled. General data were collected. Then the characteristics of CAP patients in terms of gender, age, regional and seasonal distribution and pathogenic bacteria distribution were analyzed. Results A total of 316 patients with CAP, with a prevalence rate of 30.01%, including 152 males (48.10%) and 164 females (51.90%). Regarding age, 86 cases (27.22%) at 19-40 years, 106 cases (33.54%) at 41-60 years, and 124 cases (39.24%) at >60 years, suggesting a statistical difference in the CAP detection rate among different genders and ages (P<0.05). Seasonally, 118 cases (37.34%) were detected in spring, 13 cases (4.11%) in summer, 49 cases (15.51%) in autumn, and 136 cases (43.04%) in winter, with the highest CAP detection rate in winter, followed by spring. Pathogenic bacteria were positive in 198 of 316 patients, with a detection rate of 62.66%. A total of 125 strains of pathogenic bacteria were detected in sputum culture, of which 138 cases were Gram-negative, mainly Escherichia coli (24.24%, 48/198), and 60 cases were Gram-positive, mainly Streptococcus pneumoniae (14.14%, 28/198). Among 198 patients positive for pathogen detection, 41 cases were detected in spring, 37 cases in summer, 56 cases in autumn and 64 cases in winter. The drug sensitivity results showed that Escherichia coli had the highest resistance rate to ampicillin and cefazolin, and was sensitive to imipenem and other antibiotics; Streptococcus pneumoniae has the highest resistance rate to penicillin and erythromycin, and is sensitive to vancomycin. Conclusion CAP is quite common in elderly population in Chuzhou from 2010 to 2022, with a high prevalence rate in spring and winter, and the prevention work of high-risk groups should be strengthened.

ObjectiveTo explore the efficacy and safety of Lianhua Qingwen preparation in the treatment of community-acquired pneumonia (CAP). MethodThe PubMed,Embase,Cochrane Collaboration,CNKI,VIP,and Wanfang Medical Network database (CBM) were systematically searched for all the randomized controlled clinical trials (RCTS) of Lianhua Qingwen Preparation in the treatment of CAP from the establishment of the databases to February 2023. The inclusion criteria were established, and the search results were screened. The risk assessment tool (ROB) scale was used to evaluate the methodological quality of the final included studies, and the R software was used for data integration and meta-analysis. ResultA total of 30 pieces of literature were included,involving 2 800 patients. The combined use of Lianhua Qingwen preparation on the basis of antibiotics and other conventional treatments showed that Lianhua Qingwen preparation could improve the cure rate [relative risk(RR)=1.32,95% confidence interval(95% CI)[1.23,1.42],P<0.000 1)] and shorten the time of fever remission [Mean difference(MD)=-1.45,95% CI [-1.93,-0.97],P<0.000 1],and the duration of fever reduction was divided into general population and special population subgroups. The results showed that Lianhua Qingwan preparation could shorten the duration of fever reduction (general population MD=-1.51,95%CI [-2.07,-0.94],P<0.000 1, special population MD=-1.22,95% CI [-2.16,-0.29],P=0.010 6)and does not increase the incidence of adverse reactions(RR=0.85,95%CI [0.62,1.15],P<0.000 1). After nine pieces of virtual literature with negative results were supplemented by the shear compensation method,the cure rate of CAP by Lianhua Qingwan preparation was still improved (RR=1.20,95%CI [1.13,1.29],P<0.000 1). ConclusionThe application of Lianhua Qingwen preparation on the basis of antibiotics in the treatment of CAP can improve the cure rate and shorten the time of fever reduction.

【Objective】 To explore the effects of breastfeeding on the immune response of CD4+T lymphocytes in infants in non-inflammatory state, and to analyze the immunomodulatory significance of the whole composition of breast milk. 【Methods】 A retrospective cohort study was conducted. From January to September 2022, six-month-old infants who took physical examination in the Child Healthcare Department of Changzhou Children′s Hospital Affiliated to Nantong University, were selected based on inclusion criteria, and were divided into breastfeeding group (n=33) and formula feeding group (n=27) based on their feeding patterns. Flow cytometry was used to detect the percentage of CD4+ T cells, including helper T cell (Th) 1, Th2, Th17, regulatory T cell (Treg), and the levels of related cytokines interleukin (IL)-2, IL-4, IL-6, IL-10, tumor necrosis factor (TNF)-α, interferon (IFN)-γ, IL-17 in peripheral blood. The differences in these indicators between the two groups were compared. 【Results】 Compared with the formula feeding group, the breastfeeding group showed significantly higher percentages of Th1(t=3.038), Treg (t=2.088). The ratio of Th1 to Th2(Z=2.756), IL-10(Z=2.297) and IFN-γ (Z=2.076) in the peripheral blood of the breastfeeding group were also significantly higher. Conversely, the breastfeeding group had significantly lower percentage of Th17(Z=2.704) and IL-17A (t=2.187) (P<0.05). There was no significant difference the percentage of Th2, as well as in the levels of IL-2, IL-4, IL-6 and TNF-α between the two groups (P>0.05). 【Conclusions】 Breastfeeding has a regulatory effect on the immune response of infant CD4+ T lymphocytes. It promotes the development of Th1/Th2 towards Th1 and the immunomodulatory effect of Treg. Moreover, it inhibits the Th17 type immune response. These findings suggest that the complete composition of breast milk contributes to the development and maturation of infant immune system, enhancing immune defense and immune tolerance.

【Objective】 To analyze the effect of extensively hydrolyzed formula(eHF) in the treatment of feeding intolerance in preterm infants and the effect on hospital infection, in order to provide reference for the clinical treatment of feeding intolerance in preterm infants. 【Methods】 A total of 208 cases of preterm infants with feeding intolerance diagnosed and treated in Shandong Heze Municipal Hospital from April 2017 to February 2020 were selected into the clinical trial for eligibility assessment, then were randomly assigned into study group(n=100) and control group(n=100) after screening and exclusion. Children in the control group were fed with standard preterm formula, while children in the study group were fed with eHF. Feeding tolerance indicators, including daily milk intake, time to meconium evacuation, time to full gastrointestinal nutrition, total gastric residual counts(GRV1) in the 7-d period after resumption of breastfeeding, ratio of all-day gastric residual counts/all-day estimated milk intake after resumption of breastfeeding(GRV2) were compared between the two groups, and growth indicators(body weight growth rate, head dimension growth rate), complication incidence [necrotizing enterocolitis(NEC), pathological jaundice, positive fecal occult blood or blood in stool] and incidence of hospital-acquired infections. 【Results】 The daily milk intake(t=5.037) of the study group was higher than that of the control group, and the time of foetal excretion(t=9.217), the time to reach full gastrointestinal nutrition(t=15.833), GRV1(t=6.737), GRV2(t=9.956) were lower than those of the control group, and the differences were all statistically significant(P<0.05). The rate of weight gain(t=2.454) and head dimension growth(t=5.469) in the study group was significantly higher than those of the control group(P<0.05). The incidence of the three complications of NEC, pathological jaundice and positive fecal occult blood or blood in stool(χ2=4.310) and the incidence of hospital infections(χ2=4.688) were significantly lower in the study group than in the control group(P<0.05). 【Conclusions】 Compared with the standard formula milk for preterm infants, eHF can significantly improve the feeding intolerance of preterm infants, promote growth and development, and reduce the occurrence of hospital-acquired infections. Therefore, eHF can be widely used in clinic for preterm infants with feeding intolerance.

Community-acquired pneumonia (CAP) is a respiratory infection which takes a long time to fully recover after clinical symptoms are alleviated in the short term. It affects the physical health and quality of life of the patients in the long term and can occur repeatedly, which is related to inflammation, immunity, and the coagulation function. Lung Qi deficiency and collateral stasis are the key pathogenesis of CAP at the recovery stage. The development of CAP is accompanied by the decreased production and the increased consumption of Qi, which results in lung Qi deficiency. At the same time, heat pathogen forces the blood to move improperly, which depletes Qi and damages fluid, resulting in lung collateral stasis. Lung Qi deficiency and collateral stasis are causal and influence each other. The patients at the recovery stage of CAP generally present deficient lung Qi and healthy Qi, impaired immune function, and weakened defense function. However, pathogenic Qi, coagulation function changes, and thrombosis exist, and some coagulation factors are associated with the prognosis of CAP. The Chinese medicines for tonifying lung and supplementing Qi can help replenish healthy Qi, consolidate the body foundation, and regulate the inflammation. The Chinese medicines for activating blood and resolving stasis can dredge the lung collaterals, clear the pathogenic Qi, improve the microvascular circulation, and inhibit the inflammatory response. The Chinese medicines for supplementing Qi and activating blood can replenish healthy Qi and dispel pathogen to regulate immunity, inhibit inflammation, and alleviate the clinical symptoms, thus promoting the recovery from pneumonia. From lung Qi deficiency and collateral stasis, this paper summarizes the application and explains the scientific connotation of supplementing Qi and activating blood in preventing relapse after recovery of CAP, providing ideas for using this method to assist in preventing relapse after recovery of CAP.

ObjectiveTo understand the human immunodeficiency virus(HIV) infection status and related factors by using the HIV/AIDS sentinel surveillance data of men who have sex with men(MSM) in Jinhua City, Zhejiang Province. MethodsSnowball sampling method was used to recruit MSM receiving HIV sentinel surveillance from 2016 to 2021.The inclusion criteria were aged 18 years old and above, reported having anal sex with man in recent 6 months. Questionnaire-based interviews were conducted on a one-on-one basis. Data including the characteristics of demography and ethology were collected. Five milliliter of blood samples were taken after the questionnaire. HIV and syphilis was screened. The time trend was analyzed with χ2 test. Multivariate logistic regression model was used to analyze the factors associated with HIV infection status among MSM. ResultsThe positive rate of HIV was 8.0% (127/1 597), with an increasing trend (P=0.002), but the linear correlation was weak(r=0.075). The positive rate of syphilis was 5.2% (83/1 597), with no significant difference (P=0.661).The constituent ratios showed an increasing trend (P<0.05) in using condoms consistently, finding male sexual partners through Internet or dating software, having anal sex in the past week, using condoms every time during anal sex in the past 6 months, accepting prevention services of AIDS in the last year, and the overall awareness of AIDS related knowledge. Multivariate logistic regression analysis showed that who was from national minority (OR=2.27, 95%CI: 1.08‒4.73) and from other provinces (OR=1.68, 95%CI: 1.08‒2.62), who failed to consistently use condoms every time during anal sex in the past six months (OR=3.03, 95%CI: 2.02‒4.54), who never accepted prevention services of AIDS in the last year (OR=2.17,95%CI:1.44‒3.27), who don’t know the knowledge of AIDS (OR=1.86, 95%CI: 1.12‒3.07), and who was infected with syphilis (OR=2.35, 95%CI: 1.20‒4.61) were at higher risk for HIV infection among MSM. ConclusionThe positive rate of HIV remains at a certain level among MSM in Jinhua. High-risk groups such as the patients with syphilis infections and floating population from other provinces need to be paid close attention. It is suggested to further strengthen the promotion of the use of condoms, awareness of AIDS and syphilis, warning education and comprehensive intervention services.

Métodos Esta é uma revisão de escopo em que a seleção dos artigos foi realizada em abril de 2021 e atualizada em outubro de 2022 em dez fontes de dados; a revisão seguiu os pressupostos estabelecidos pelo Joanna Briggs Institute e o checklist dos Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews. Os resultados foram analisados descritivamente e sintetizados em um quadro. Resultados Foram selecionados 17 estudos com publicação predominante em 2017; Estados Unidos da América e Brasil foram os países com a maior quantidade de produções. A maioria dos estudos buscou avaliar o impacto e a eficácia das estratégias e desenvolver ou validar instrumentos de educação em saúde e atividades de prevenção. Em relação ao conteúdo abordado pelas estratégias, foram formadas cinco categorias: orientação inicial sobre HIV/AIDS, cuidados gerais, vida saudável, saúde sexual e suporte emocional. As estratégias educativas que se destacaram em relação à maior adesão dos pacientes ao tratamento estão relacionadas com o desenvolvimento de sistemas, programas e multimídia. As cartilhas promoveram empoderamento e autonomia de pessoas vivendo com HIV. Conclusão Foram mapeadas as principais estratégias educativas, com destaque para cartilhas, material impresso, recursos multimídia, sistemas, formulários e oficinas/workshops, abordando orientação inicial sobre HIV/AIDS, tratamento farmacológico, cuidados gerais, vida saudável, saúde sexual e suportes social e emocional.

Abstract Objective To map the scientific production on educational strategies and the content covered in the education of people living with HIV. Methods This is a scoping review in which the selection of articles was carried out in April 2021 and updated in October 2022 in ten data sources; the review followed the assumptions established by the Joanna Briggs Institute and the checklist of Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews. The results were descriptively analyzed and summarized in a chart. Results A total of 17 studies were selected, predominantly published in 2017; The USA and Brazil were the countries with the largest number of productions. Most studies sought to evaluate the impact and effectiveness of strategies and develop or validate health education instruments and prevention activities. Concerning the content covered by the strategies, five categories were formed: initial guidance on HIV/AIDS, general care, healthy living, sexual health, and emotional support. The educational strategies that stood out concerning greater patient adherence to treatment are related to the development of systems, programs, and multimedia. The booklets promoted empowerment and autonomy for people living with HIV. Conclusion The main educational strategies were mapped, with emphasis on booklets, printed material, multimedia resources, systems, forms, and workshops, covering initial guidance on HIV/AIDS, pharmacological treatment, general care, healthy living, sexual health, and social and emotional support. Objetivo: evidenciar as populações-chave ao vírus da imunodeficiência humana abordadas em estudos científicos de enfermagem. Método: revisão integrativa da literatura realizada nas bibliotecas National Library of Medicine, National Institutes of Health, Scientific Electronic Library Online e Biblioteca Virtual de Saúde; nas bases de dados Embase, Cummulative Index to Nursing and Allied Health Literature, Scopus, Web of Science e Literatura Latino-Americana e do Caribe em Ciências da Saúde. Resultados: identificados 1.059 artigos, dos quais 18 foram incluídos. Os achados foram agrupados conforme as dimensões da vulnerabilidade (individual, social e programática). Conclusão: o papel da enfermagem diante das dimensões individuais, sociais e programáticas da vulnerabilidade, foi importante para identificar as especificidades dessas populações e para a compreensão dos aspectos que tornam estes indivíduos vulnerabilizados ao HIV/aids. Verifica-se a intervenção direta da enfermagem por meio de ações de promoção e prevenção de modo a contribuir para as boas práticas de cuidado.


Objective: to highlight the key populations for the human immunodeficiency virus addressed in scientific nursing studies. Conclusion: the role of nursing in the face of the individual, social and programmatic dimensions of vulnerability was important to identify the specificities of these populations and to understand the aspects that make these individuals vulnerable to HIV/AIDS. There is direct nursing intervention through promotion and prevention actions in order to contribute to good care practices.

Conclusión: el papel de la enfermería frente a las dimensiones individual, social y programática de la vulnerabilidad fue importante para identificar las especificidades de estas poblaciones y comprender los aspectos que tornan a esos individuos vulnerables al VIH/SIDA. Existe intervención directa de enfermería a través de acciones de promoción y prevención para contribuir a las buenas prácticas de cuidado.

Resumo Objetivo Validar o conteúdo do roteiro de um recurso audiovisual para pessoas vivendo com HIV. Métodos Estudo metodológico com abordagem quantitativa, norteado pelo referencial teórico metodológico da psicometria. A validação de conteúdo foi realizada por juízes especialistas na área temática usando a técnica Delphi. A amostra foi composta por 22 juízes na primeira análise de validação e sete juízes na segunda análise. Resultados Todos domínios analisados apresentaram coeficiente de validade de conteúdo (CVC) >0,80, com CVC total (CVCt) de 0,96, consistência interna quase perfeita, Alfa de Cronbach de 0,988, ICC de 0,982 [IC95% 0,969-0,991] e p<0,005 significativo na primeira rodada de avaliação dos especialistas. Na segunda análise, os critérios usados para validação de conteúdo apresentaram CVCt de 0,97, com os critérios de objetividade, simplicidade, clareza, relevância, precisão, variedade, credibilidade e equilíbrio, atingindo 100% de aprovação. Conclusão O roteiro foi validado quanto ao conteúdo, mostrando ser um instrumento representativo e pertinente para construção do recurso audiovisual. Ele contribui para o avanço do conhecimento científico pois apresenta resultados com rigor metodológico, com inovações no campo da educação em saúde para pessoas vivendo com HIV.

La validación de contenido fue realizada por jueces especialistas en el área temática mediante el uso del método Delphi. La muestra estuvo compuesta por 22 jueces en el primer análisis de validación y siete jueces en el segundo análisis. Resultados Todos los dominios analizados presentaron coeficiente de validez de contenido (CVC) >0,80, con un CVC total (CVCt) de 0,96, consistencia interna casi perfecta, Alfa de Cronbach de 0,988, ICC de 0,982 [IC95 % 0,969-0,991] y p<0,005 significativo en la primera ronda de evaluación de los especialistas. En el segundo análisis, los criterios usados para la validación de contenido presentaron un CVCt de 0,97, con los criterios de objetividad, simplicidad, claridad, relevancia, precisión, variedad, credibilidad y equilibrio, con un 100 % de aprobación. Conclusión Se validó el guion en cuanto al contenido, lo que demuestra que es un instrumento representativo y pertinente para la elaboración del recurso audiovisual. Contribuye al avance del conocimiento científico ya que presenta resultados con rigor metodológico, con innovaciones en el campo de la educación para la salud para personas que viven con el VIH.


Abstract Objective The study aimed to validate the script content of an audiovisual resource for people living with HIV. Methods This methodological study had a quantitative approach and was guided by the methodological theoretical framework of psychometrics. Content validation was performed by expert judges in the thematic area using the Delphi technique. The sample consisted of 22 judges in the first validation analysis and seven judges in the second analysis. Results All domains analyzed had a content validity coefficient (CVC) >0.80, with a total CVC (CVCt) of 0.96, almost perfect internal consistency, Cronbach's alpha of 0.988, ICC of 0.982 [95%CI 0.969-0.991], and p<0.005 which was significant in the first round of expert evaluation. In the second analysis, the criteria used for content validation showed a CVCt of 0.97, with the criteria of objectivity, simplicity, clarity, relevance, accuracy, variety, credibility, and balance achieving 100% approval. Conclusion The script was validated in terms of content, showing to be a representative and relevant instrument for building the audiovisual resource. It contributes to the advancement of scientific knowledge as it presents results with methodological accuracy and innovations in the field of health education for people living with HIV.

ABSTRACT Objective: To assess differences in the sputum microbiota of community-acquired pneumonia (CAP) patients with either COPD or asthma, specifically focusing on a patient population in Turkey. Methods: This retrospective study included hospitalized patients > 18 years of age with a diagnosis of pneumonia between January of 2021 and January of 2023. Participants were recruited from two hospitals, and three patient groups were considered: CAP patients with asthma, CAP patients with COPD, and CAP patients without COPD or asthma. Results: A total of 246 patients with CAP were included in the study, 184 (74.8%) and 62 (25.2%) being males and females, with a mean age of 66 ± 14 years. Among the participants, 52.9% had COPD, 14.2% had asthma, and 32.9% had CAP but no COPD or asthma. Upon analysis of sputum cultures, positive sputum culture growth was observed in 52.9% of patients. The most commonly isolated microorganisms were Pseudomonas aeruginosa (n = 40), Acinetobacter baumannii (n = 20), Klebsiella pneumoniae (n = 16), and Moraxella catarrhalis (n = 8). CAP patients with COPD were more likely to have a positive sputum culture (p = 0.038), a history of antibiotic use within the past three months (p = 0.03), utilization of long-term home oxygen therapy (p < 0.001), and use of noninvasive ventilation (p = 0.001) when compared with the other patient groups. Additionally, CAP patients with COPD had a higher CURB-65 score when compared with CAP patients with asthma (p = 0.004). Conclusions: This study demonstrates that CAP patients with COPD tend to have more severe presentations, while CAP patients with asthma show varied microbial profiles, underscoring the need for patient-specific management strategies in CAP.

Introducción: los pacientes graves requieren ingreso en las unidades de cuidados intensivos por infecciones adquiridas en la comunidad. Objetivo: caracterizar, desde el punto de vista clínico y epidemiológico, a los pacientes graves con infecciones adquiridas en la comunidad durante la COVID-19. Métodos: estudio observacional, descriptivo, de serie de casos, en el periodo comprendido desde el 1ero de septiembre de 2020 hasta el 31 de enero de 2022. Se incluyeron a 277 pacientes. Se obtuvieron variables epidemiológicas y clínicas. El análisis estadístico se basó en medidas de resumen de la estadística descriptiva y de asociación. Resultados: el 52,7 % de los pacientes correspondieron al sexo femenino. La media de la edad fue 40,2 años (IC 95 %: 37,8-42,4). El Acute Physiology and Chronic Health Evaluation II (APACHE II) medio se estimó en 11, 8 (IC 95 %: 9,7-12,2). La escala Secuencial Organ Failure Assessment (SOFA) promedio fue de 1,8 (IC 95 %:1,4-2,2). El 31,7 % de los pacientes fueron hipertensos y 19,4 % diabéticos. La infección intraabdominal fue la localización principal (52,7 %), seguida de la neumonía (34,7 %). El 98,7 % tenía antimicrobianos al ingreso y el 26,3 % ventilación mecánica artificial. Conclusiones: los pacientes se caracterizan en su mayoría por pertenecer al sexo femenino, estar en la cuarta década de la vida, desarrollar alteraciones de sus sistemas fisiológicos y disfunción de órganos y tener hipertensión arterial y diabetes mellitus; infección intraabdominal o neumonía, así como requerir tratamiento con antimicrobianos, ventilación mecánica invasiva y drogas vasoactivas.


Introduction: critically ill patients require admission to intensive care units due to community-acquired infections. Objective: to characterize, from a clinical and epidemiological point of view, critical ill patients with community-acquired infections during COVID-19. Results: 52.7% of the patients were female. The mean age was 40.2 years (95% CI: 37.8-42.4). The mean Acute Physiology and Chronic Health Evaluation II (APACHE II) was estimated at 11.8 (95% CI: 9.7-12.2). The mean Sequential Organ Failure Assessment (SOFA) scale was 1.8 (95% CI: 1.4-2.2). 31.7% of the patients were hypertensive and 19.4% diabetic. Intra-abdominal infection was the main location (52.7%), followed by pneumonia (34.7%). 98.7% had antimicrobials on admission and 26.3% had artificial mechanical ventilation. Conclusions: the patients are mostly characterized by being female, being in the fourth decade of life, developing alterations in their physiological systems and organ dysfunction, and having high blood pressure and diabetes mellitus; intra-abdominal infection or pneumonia as well as requiring treatment with antimicrobials, invasive mechanical ventilation and vasoactive drugs.


Introdução: pacientes críticos necessitam de internação em unidades de terapia intensiva devido a infecções adquiridas na comunidade. Objetivo: caracterizar, do ponto de vista clínico e epidemiológico, pacientes graves com infecções comunitárias durante a COVID-19. Métodos: estudo observacional, descritivo, de série de casos, no período de 1º de setembro de 2020 a 31 de janeiro de 2022. Foram incluídos 277 pacientes. Foram obtidas variáveis epidemiológicas e clínicas. A análise estatística baseou-se em medidas-resumo de estatística descritiva e de associação. Resultados: 52,7% dos pacientes eram do sexo feminino. A média de idade foi de 40,2 anos (IC 95%: 37,8-42,4). A média do Acute Physiology and Chronic Health Evaluation II (APACHE II) foi estimada em 11,8 (IC 95%: 9,7-12,2). A média da escala Sequential Organ Failure Assessment (SOFA) foi de 1,8 (IC 95%: 1,4-2,2). 31,7% dos pacientes eram hipertensos e 19,4% diabéticos. A infecção intra-abdominal foi a principal localização (52,7%), seguida de pneumonia (34,7%). 98,7% tinham antimicrobianos na admissão e 26,3% tinham ventilação mecânica artificial. Conclusões: a maioria dos pacientes é do sexo feminino, na quarta década de vida, desenvolve alterações de seus sistemas fisiológicos e disfunção orgânica, além de hipertensão arterial e diabetes mellitus; infecção intra-abdominal ou pneumonia, além de necessitar de tratamento com antimicrobianos, ventilação mecânica invasiva e drogas vasoativas.

Résumé

ABSTRACT Chlamydia psittaci - related community-acquired pneumonia associated to acute myocarditis was diagnosed in a young man with no medical history, and a professional exposition to birds. The diagnosis was confirmed with positive specific polymerase chain reaction in bronchoalveolar lavage. The patient was treated with spiramycin for two weeks with anti-inflammatory treatment for myocarditis for three months. Clinical and biological improvement was rapidly observed followed by normalization of electrocardiogram and chest CT scan. No relapse was reported for over a two-year follow-up.
