BACKGROUND:The distribution of horizontal pressure in adolescent idiopathic scoliosis can be used to evaluate the coronal imbalance.Currently,there are no reports on the characteristics of coronal pressure distribution and its correlation with coronal imbalance. OBJECTIVE:To explore the correlation between coronal pressure variation and coronal imbalance in adolescent idiopathic scoliosis patients. METHODS:A total of 39 adolescent idiopathic scoliosis patients who met the inclusion and exclusion criteria in Lianyungang First People's Hospital from March 2021 to June 2022 were selected as the adolescent idiopathic scoliosis group,and 30 matched healthy volunteers were recruited from the outpatient department as the control group.The horizontal position pressure,folding position pressure,coefficient of variation,and global and trunk pressure variation were measured by the TBED Postural Couch evaluation system.In the adolescent idiopathic scoliosis group,full-length spine radiographs were taken to measure Cobb angle,coronal balance,apical vertebral deviation and Nash-Moe rotation.The characteristics of coronal pressure variation and its correlation with coronal imbalance were analyzed. RESULTS AND CONCLUSION:(1)Compared with the control group,there was no statistically significant difference in the horizontal position pressure and folding position pressure in the adolescent idiopathic scoliosis group(P>0.05),but the global pressure variation and coefficient of variation were significantly increased(all P<0.05).(2)There were differences in both sides of the trunk of the adolescent idiopathic scoliosis group and the control group(P<0.05),and the convex side pressure variation in the adolescent idiopathic scoliosis group was higher than the concave side pressure variation in the left and right side pressure variation in the control group(all P<0.05).(3)The variation of the convex side pressure of the trunk in adolescent idiopathic scoliosis patients was positively correlated with coronal balance intensity(r=0.692,P<0.05),moderately positively correlated with Cobb angle and apical vertebral deviation(r=0.499,0.595,all P<0.05),and weakly correlated with Nash-Moe grade(r=0.377,P<0.05).The variation of the concave side pressure of the trunk was moderately positively correlated with coronal balance(r=0.410,P<0.05),and the rest was weakly correlated or not correlated(P>0.05).(4)These findings indicate that pressure variation may be used as an adjoint assessment tool in patients with mild to moderate scoliosis.

BACKGROUND:Distal tibial tuberosity-high tibial osteotomy is a surgical treatment for knee osteoarthritis,but there is still a lack of clinical studies on its effect on ankle joints. OBJECTIVE:To observe the effects of distal tibial tuberosity-high tibial osteotomy on ankle angle on coronal plane of the radiography of the full length of lower limb in weight loading. METHODS:Data of 40 patients(41 knees)with distal tibial tuberosity-high tibial osteotomy from March 2021 to March 2022 were retrospectively analyzed,including 31 females and 9 males,20 left knees and 21 right knees,aged 49-75 years,mean(63.44±6.57)years.The radiographic data of the full length of the lower limb in weight loading were collected before,week 2 and week 48 postoperatively.Hip-knee-ankle angle,talar tilt angle,tilt angle of the ankle,tibiocrural angle,and tibial articular surface angle were measured before and after surgery. RESULTS AND CONCLUSION:(1)Hip-knee-ankle angle improved from(-6.24±3.69)° before operation to(2.59±3.49)° week 2 postoperatively and(2.15±3.49)° week 48 postoperatively.The tilt angle of the ankle changed from(-7.90±3.11)° before operation to(-2.51±2.59)° week 2 postoperatively and(-2.46±2.42)° week 48 postoperatively,with statistically significant difference(P<0.001).(2)There was no significant difference in talar tilt angle,tibiocrural angle,and tibial articular surface angle before and week 2 postoperatively.(3)No significant difference in the angle changes was detected between week 2 and week 48 postoperatively.(4)It is indicated that distal tibial tuberosity-high tibial osteotomy can not only correct genu varus but also improve ankle angle.This result remains stable after 48 weeks of weight-bearing activities.

Résumé

La recesión gingival es considerada como una deformidad o condición mucogingival, la Academia Americana de Periodontología, define a la recesión gingival como el desplazamiento del margen del tejido blando apical a la unión cemento-esmalte con la exposición de la superficie radicular. El tratamiento de las recesiones gingivales es un motivo de consulta común debido a razones estéticas, hipersensibilidad dentinaria, molestias durante el cepillado e incluso temor a la pérdida dentaria. Es una situación clínica común, 60% de la población humana tiene algún tipo de recesión gingival. Al realizar el examen clínico a paciente masculino de 55 años, se observó una recesión gingival tipo 1 (RT1) sin pérdida de inserción interproximal de la clasificación de Cairo. Se realizó el colgajo posicionado coronalmente (CPC) utilizando una matriz dérmica acelular (MDA) de origen humano OrACELL®. Se obtuvo resultado favorable en el recubrimiento de recesiones gingivales múltiples; considerándolos como una buena alternativa frente a los injertos gingivales autógenos. Concluyendo que, el uso de la matriz dérmica acelular para el tratamiento de la recesión gingival tipo 1 es una adecuada opción para el recubrimiento radicular. Se recomiendan más estudios a largo plazo para ver la estabilidad de los resultados obtenidos con la MDA (AU)


Gingival recession, considered a deformity or mucogingival condition, the American Academy of Periodontology, defines gingival recession as the exposure of the root surface resulting from migration of the gingival margin apical to the cementoenamel junction (CEJ). The treatment of gingival recessions is a common reason for consultation due to aesthetic reasons, dentin hypersensitivity, discomfort during brushing and even fear of tooth loss. It is a common clinical situation, 60% of the human population has some kind of gingival recession. Clinical examination of a 55-year-old male patient showed a type 1 gingival recession (RT1) without loss of interproximal insertion of the Cairo classification. Coronally advanced flap (CAF) was performed using an acellular dermal matrix (ADM) of human origin OrACELL®. Favorable results were obtained in the coating of multiple gingival recessions; considering them as a good alternative to autogenous gingival grafts. Concluding that, the use of the acellular dermal matrix for the treatment of gingival recession type 1, is a suitable option for root lining. Further long-term studies are recommended to see the elasticity of MDA outcomes (AU)

Introdução: A Radiolucência intracoronal pré eruptiva (PECR) é um achado radiográfico, com aspecto de lesão radiolúcida na dentina coronária, adjacente a junção amelodentinária, em dentes não irrompidos. Objetivo: Relatar um caso de possível reabsorção intracoronal pré-eruptiva com destruição extensa de dentina e envolvimento pulpar de um segundo molar permanente recém-erupcionado. Método: Após definição do diagnóstico, optou-se por um tratamento conservador, com a realização de capeamento pulpar direto com a finalidade de manutenção da vitalidade pulpar e término de formação das raízes. Resultado: Após o tratamento, paciente permaneceu assintomática no controle de quinze dias, com teste de vitalidade normal e sem sinais de inflamação. Conclusão: A interpretação cuidadosa do exame radiográfico é crucial para a detecção precoce e manejo adequado dessas lesões.


Pre-eruptive intracoronal radiolucency (PECR) is a radiographic finding, with the appearance of a radiolucent lesion in the coronary dentin, adjacent to the dentinal-enamel junction, in unerupted teeth. Objective: To report a case of possible pre-eruptive intracoronal resorption with extensive dentin destruction and pulpal involvement of a newly erupted second permanent molar. Method: After defining the diagnosis, a conservative treatment was chosen, with direct pulp capping in order to maintain pulp vitality and end root formation. Result: After treatment, the patient remained asymptomatic in the fifteen-day control, with normal vitality test and no signs of inflammation. Conclusion: Careful interpretation of the radiographic examination is crucial for early detection and proper management of these lesions.

Résumé

Post-and-core crown is one of the most common forms of restoration of tooth after root canal therapy (RCT). Infection control is the core objective of RCT, which is usually well realized by endodontists. However, many prosthodontists often neglect the infection control of the tooth and the maintenance of the curative effect of RCT during the process of post-and-core crown, which may lead to the failure of the final restoration. The concept of integrated crown-root treatment advocated recently requires clinicians to consider the RCT and final restoration as a whole, rather than simply divided into two parts-endodontic treatment and restorative treatment. As the core content of integrated crown-root treatment, infection control should be taken seriously by clinicians and implemented throughout the whole treatment process, especially in the restorative treatment that is easily overlooked after RCT. Therefore, this article describes the infection control of post-and-core crown restoration, classifies the tooth requiring post-and-core crown restoration, and puts forward the measures of infection control before and during post-and-core crown restoration, in order to provide reference and guidance for clinical practice.

OBJECTIVE@#To retrospectively analyze the clinical efficacy of olecranon osteotomy approach in the treatment of Dubberley type Ⅲ coronal fractures of the distal humerus and summarize the treatment experience.@*METHODS@#From January 2016 to June 2020, 17 patients (5 males and 12 females) with Dubberley type Ⅲ coronal fractures of the distal humerus were treated by olecranon osteotomy approach. The age ranged from 37 to78 years old with an average of (58.5±12.9) years old. According to Dubberley classification, there were 5 cases of type Ⅲ A and 12 cases of type Ⅲ B. The curative effect was evaluated using the Borberg-Morrey elbow function score. The flexion, extension and rotation range of motion of the elbow joint, complications and postoperative imaging evaluation were recorded.@*RESULTS@#All the 17 patients got bony union. The follow-up time ranged from 12 to 33 months with an average of (15.6±5.6) months. There was 1 case of ischemic necrosis of capitulum humeri, 2 cases of traumatic arthritis and 1 case of heterotopic ossification, 1 case of malunion of fracture. The range of motion was (114.80±19.50) °. The Broberg-Morrey score was 85.3±8.2, excellent in 5 cases, good in 9 cases, fair in 3 cases and poor in 0 case.@*CONCLUSION@#Through olecranon osteotomy approach, the articular surface of distal humerus could be fully exposed, and the operation is convenient. Anatomical reduction and rigid fixation of the articular surface of distal humerus are the key factors for the succesful outcome.

Introduction: The coronal opening is essential, during endodontic treatment, for direct access to the root canals. Usually, in the anterior teeth, endodontic access is achieved on the palatal/lingual surface, more specifically in the cingulum region. However, the lack of observation for dental positioning can lead to some accidents and complications, such as buccal perforations, causing esthetic damage and compromising the treatment. Objective: To evaluate the applicability of a new approach for performing coronal opening in anterior teeth, regarding the wear caused on the tooth crown, and to evaluate the ease of performing the new technique to minimize the risk of perforation. Material and method: Ten students (n=10) from the Improvement in Endodontics program participated in the present study. Twenty artificial teeth were used, which were upper, central incisors. Each participant performed two coronal openings. First, with no guidance (Group A - Conventional Technique) and afterward, a new opening, but with guidance with the spherical diamond tip placed parallel to the long axis of the tooth (Group B - Modified Technique). Result: The results showed that 90% of the participants considered the Modified Technique as having lower risk of accidents, as well as being easier for locating the pulp chamber. There was a statistical difference both in the measurement of the total area of wear and in the width, while there was no statistical difference in the height. Conclusion: Preliminary data collected with the survey were satisfactory for the Modified Technique. Statistically, it showed a favorable difference in relation to the area of wear and the width. However, in height there was no statistical difference.


Introdução: A abertura coronária é fundamental durante o tratamento endodôntico para um acesso direto aos canais radiculares. Em geral, em dentes anteriores, o acesso endodôntico é realizado na face palatina/lingual, mais especificamente em região de cíngulo. Entretanto, a falta de observação do posicionamento dentário pode levar a alguns acidentes e complicações, como as perfurações por vestibular, ocasionando dano estético e comprometendo o tratamento. Objetivo: Avaliar a aplicabilidade de uma nova abordagem de realização da abertura coronária em dentes anteriores, quanto ao desgaste ocasionado na coroa dentária, e avaliar a facilidade de execução da nova técnica para minimizar os riscos de perfurações. Material e método: Dez alunos (n=10) do curso de Aperfeiçoamento em Endodontia participaram do estudo. Foram utilizados vinte dentes artificiais, sendo eles incisivos centrais superiores. Cada participante realizou duas aberturas coronárias. Primeiramente sem nenhuma orientação (Grupo A - Técnica Convencional) e após, uma nova abertura, porém com orientação, com a ponta diamantada esférica posicionada paralelamente ao longo eixo do dente (Grupo B - Técnica Modificada). Resultado: Os resultados demonstraram que 90% dos participantes consideraram a Técnica Modificada como a de menor risco de acidentes e, ainda, de mais fácil localização da câmara pulpar. Houve diferença estatística tanto para a medida da área total de desgaste quanto para a largura, enquanto na altura não houve diferença estatística. Conclusão: Os dados preliminares coletados pelo questionário se mostraram satisfatórios para a Técnica Modificada. Estatisticamente, apresentou diferença favorável em relação à área de desgaste e largura, já em altura não houve diferença estatística.

Objective:To explore the MRI features of fetal coronal cleft vertebrae, and to compare the efficacy of MRI and ultrasound in the diagnosis of fetal coronal cleft vertebrae, and to analyze the outcome of fetal coronal cleft vertebrae.Methods:From September 2019 to June 2021, 40 fetuses suspected of fetal vertebral deformities by ultrasound were retrospectively collected in Shandong Provincial Hospital Affiliated to Shandong University, who were diagnosed as coronal cleft vertebrae after MRI examination. Five cases of induced labor and 14 cases lost to follow-up were excluded, and 21 fetuses who underwent MRI after delivery were finally included. The gestational weeks were 25-34 (29.1±2.6) weeks, and there were 19 males and 2 females. Fetal spine MRI includes susceptibility weighted imaging (SWI) and T 2-true fast imaging with steady-state (True-FISP). The MRI features and outcome of fetal coronal cleft vertebrae were explored. The image quality scores of SWI, T 2-True-FISP and ultrasound were compared with Friedman test and Wilcoxon signed-rank test. The diagnostic accuracy of fetal coronal cleft vertebrae of SWI, T 2-True-FISP and ultrasound was calculated. Cochran test was used to compare the efficiency of 3 kinds of images, and the modified McNemar test was used for pairwise comparison between groups. Results:There were 10 cases of single and 11 cases of multiple fetal coronal cleft vertebrae, 16 cases of simple lumbar vertebrae, 2 cases of simple thoracic vertebrae, and 3 cases of thoracolumbar vertebrae. The common SWI features of 21 cases show longitudinal strip or dot high signal on the sagittal plane, and transverse fissure like high signal on axial plane. Anterior part of vertebral body was larger than posterior part in 19 cases of them. The image quality scores of SWI, T 2-True-FISP and ultrasound were 4 (3, 4), 2 (2, 2), 2 (2, 2), and the difference was statistically significant in general (χ2=34.24, P<0.001). Pairwise comparison showed that the image quality of SWI was better than those of T 2-True-FISP and ultrasound ( Z=-4.04, P<0.001; Z=-4.11, P<0.001), and there was no statistically significant difference between T 2-True-FISP and ultrasound ( Z=-0.58, P=0.388). The diagnostic accuracy of SWI, T 2-True-FISP and ultrasound was 100% (21/21), 66.7% (14/21), 47.6% (10/21). The diagnostic accuracy of SWI was better than those of T 2-True-FISP and ultrasound (χ2=5.14, P=0.008; χ2=9.09, P<0.001), and there was no statistically significant difference between T 2-True-FISP and ultrasound (χ2=0.75, P=0.194). MRI showed that coronal cleft vertebrae disappeared in all 21 fetuses after birth, including 1 case of syringomyelia and 1 case of fatty filum terminal. Conclusions:MRI, especially SWI, plays an important role in the diagnosis of fetal coronal cleft vertebrae. Fetal coronal cleft vertebrae disappeared in the follow-up after birth, which proved to be a normal physiological variation from the radiographic perspective.

Objective:To investigate the clinical manifestations and prognosis of severe coronal virus disease 2019(COVID-19)pneumonia combined with coronary artery disease(CAD)in the elderly.Methods:A total of 351 old patients(≥60 years)with severe COVID-19 pneumonia combined with CAD admitted to Tongji Hospital of Wuhan from February 2020 to March 2020 were enrolled in this retrospective study.The patients were divided into CAD group(n=52)and non-CAD group(n=299). The clinical data, the changes of blood tests and cardiovascular complications were observed.Results:Compared with non-CAD group, CAD group showed a longer time of stay in hospital[(12.3±2.6)d and(9.3±2.1)d, t=3.24, P=0.002], and higher incidence of cardiovascular complications, such as myocardial infarction(7.7% and 0.0%, χ2=21.6, P<0.001), heart failure(15.4% and 0.3%, χ2=34.7, P<0.001), arrhythmia(50.0% and 12.4%, χ2=24.45, P<0.001)and all-cause mortality(15.4% and 4.3%, χ2=7.94, P=0.005). The serum levels of interleukin-2 receptor, interleukin 6, CK-MB, cTnI, NT-proBNP, D-dimer and fibrinogen were higher in CAD group than in non-CAD group.The oxygenation index was significantly lower and the blood lactic acid level was higher in CAD group than in non-CAD group. Conclusions:The aged patients with severe COVID-19 pneumonia combined with CAD show obvious inflammatory reaction, high incidence of cardiovascular complications and high mortality.

ABSTRACT Objective: To conduct a comparative study of the results obtained in the treatment of adolescent idiopathic scoliosis (AIS) with different types of fixations (traditional, selective and multiple), and to evaluate the correction of angular deformity in the frontal plane by the Cobb and sacral clavicular angle (SCA) methods. Methods: A study of a group of 278 patients with AIS who underwent selective, traditional, and multiple fixation surgeries. Results: Significant corrections of both the Cobb angle and the SCA were observed. Conclusions: In the multiple fixation surgeries there was a 100% correction between the preoperative and postoperative SCA values and a 50% correction in the traditional and selective fixations, a difference considered significant. Regarding the Cobb angle, the three fixations presented corrections between preop and postop with significant differences. Level of evidence III; Retrospective Study.


RESUMO Objetivo: Realizar um estudo comparativo dos resultados obtidos no tratamento da escoliose idiopática do adolescente (EIA) com diferentes tipos de fixação tradicional, seletiva e múltipla e avaliar a correção da deformidade angular no plano frontal pelo método de Cobb e do â ngulo sacro clavicular (ASC). Métodos: Estudo de um grupo de 278 pacientes com EIA operados com fixações seletiva, tradicional e múltipla. Resultados: Observou-se correção significativa tanto do ângulo de Cobb quanto do ASC. Conclusões: Nas cirurgias com fixação múltipla constatou-se, entre pré-operatório (pré-op) e pós-operatório (pós-op), uma correção de 100% do ASC, e com as fixações tradicional e seletiva a correção foi de 50%, diferença que se considerou significativa. Com relação ao ângulo de Cobb as três fixações resultaram em correções entre o pré-op e o pós-op com diferença considerada significativa. Nível de evidência III; Estudo retrospectivo.

Objective:To explore the accuracy and clinical application value of a Multi-Agent Reinforcement Learning framework (MARL framework) in three-dimensional ultrasound to automatically locate the coronal plane of the uterus.Methods:A total of 144 female patients who underwent routine gynecological examinations in Luohu People′s Hospital during May 2020 were selected as the experimental subjects. The three-dimensional volume data of the uterus of all the experimental subjects were collected by using the Resona-8 high-end color Doppler ultrasound system. A sonographer with more than 5 years of clinical experience manually locate the coronal plane of the uterus in all collected data, and at the same time automatically locate the coronal plane of the uterus MARL framework. The coronal plane images of the uterus obtained by the two methods were saved, and the operation time of the two methods was recorded. The coronal plane uterine images obtained by the two methods were mixed together, and the images were scored 0-1 by two senior ultrasound experts in a double-blind manner. The average score greater than or equal to 0.6 points was considered qualified.Results:①In 144 volunteers, among the coronal planes of the uterus located by the two methods, 131 were qualified by the manual method, and 137 were qualified by the automatic method.There was no statistical difference between the manual and automatic coronal plane images of the uterus (χ 2=1.934, P=0.164) by the chi-square test. ②Using interquartile range analysis, the median and interquartile range of the image score of the automatic group was 0.80(0.75, 0.90), while the median and interquartile range of the image score of the manual group was 0.80(0.75, 0.90). The Wilcoxon signed rank test was used to analyze the quality of the coronal plane images obtained by manual and automatic methods, and the difference was not statistically significant ( Z=1.241, P=0.215). ③The paired t test was used to compare the time required to locate the coronal surface of the uterus, by manual method (63.65±10.182)s, by automatic method (3.25±0.294)s, the difference between the two methods was statistically significant ( t=19.52, P<0.001). Conclusions:The method based on MARL framework has a high correlation with the manual locating of the coronal plane of uterus in three-dimensional ultrasound, and greatly reduces the operation time. It can be effectively applied in clinical practice and lays a foundation for the automatic diagnosis of uterine related diseases.

Resumen Introducción: La extrusión rápida ortodóntica puede usarse como alternativa para evitar extraer una pieza fracturada que no podría ser restaurada por trauma, caries extensas, problemas periodontales (dos o tres paredes), dientes con reabsorción interna y dientes con perforaciones endodónticas. Objetivo: De este caso clínico es presentar a todos los odontólogos una solución sencilla de tratamiento integral de un paciente el cual sufrió un trauma facial, causándole una fractura vertical a nivel del tercio medio radicular del incisivo central superior derecho. Método: Mediante una extrusión ortodóntica, retratamiento endodóntico y abordaje periodontal, de manera que facilite la futura rehabilitación con corona completa. El resultado de este procedimiento se logró con mucho éxito gracias a que concluimos que se trató en forma multidisciplinaria, en donde cada especialidad actúa o interviene en un momento preciso. Conclusiones: La extrusión rápida es un procedimiento en el cual una pieza que ha sufrido una fractura radicular a nivel marginal puede ser tratada sin necesidad de realizar un alargamiento de corona comprometiendo así el nivel del hueso de la pieza.


Abstract Introduction: Rapid dental orthodontic extrusion may be used as an alternative to avoid the extraction of fractured teeth that might not be suitable for restauration because od trauma, extensive cavities, periodontal problems (two or three walls), teeth with internal resorption, or with endodontical perforations. Objective: Of this clinical case is to present to all dentists a simple solution of integral treatment of a patient who suffered trauma to his face causing a vertical fracture at the level of the middle third root of the right upper central incisor. Methods: Used in this case was through orthodontic extrusion, endodontic retreatment and periodontal approach, so as to facilitate future rehabilitation with a full crown. The result of this procedure was achieved with great success because we conclude that it was treated in a multidisciplinary way, where each specialty intervenes at a precise moment. Conclusions: Rapid extrusion is a procedure in which a tooth that shows a radicular fracture close to it´s margin may be treated without dental crown lengthening that would risk bone level.

Abstract: The aim of this study was to evaluate the influence of different coronal preflaring protocols (absent, conservative and conventional) on the accuracy of Root ZX II, Raypex 6, and RomiApex A-15 electronic foramen locators (EFLs). Twenty mandibular molars with Vertucci's type IV mesial roots were subjected to endodontic exploration and foraminal patency confirmation. Under 16x magnification, its real lengths (RL) were measured and registered (RL1). The canals were then irrigated with 2.5% sodium hypochlorite and electronically measured (EM1) employing the alginate model; all measurements were performed in triplicate by a blind operator using adjusted endodontic hand-files introduced until the apex foramen. Coronal preflaring procedures were sequentially performed with #25/.06 (conservative) and #25/.12 (conventional) instruments; new RLs extents were performed after each coronal preparation protocol (RL2/RL3), as same as electronic measurements (EM2/EM3). The devices error (mm) was evaluated considering the difference between RLs and EMs at each preparation stage; their precision was stablished adopting ±0.5 mm as tolerance margin. The EFLs error significantly reduced after conventional coronal preflaring protocol (p<0.05), which not occur after the conservative one. The best precisions values were noted after conventional preparation as 90% (Root ZX II), 97.5% (Raypex 6), and 92.5% (RomiApex A-15). No significant differences were found in EFLs comparisons, regardless of the coronal protocol tested (p>0.05). Under the conditions tested it can be concluded that the EFLs evaluated were precise. Moreover, the preflaring protocols influences its accuracy's, where the less conservative one produced the best results.


Resumo O objetivo deste estudo foi avaliar a influência de diferentes protocolos de pré-alargamento cervical (ausente, conservador e convencional) na precisão dos localizadores eletrônicos foraminais (LEFs) Root ZX II, Raypex 6 e RomiApex A-15. Vinte molares inferiores com raízes mesiais do tipo IV de Vertucci foram submetidos à exploração endodôntica e confirmação da patência foraminal. Sob ampliação de 16x, seus comprimentos reais (CR) foram medidos e registrados (CR1). Os canais foram então irrigados com hipoclorito de sódio a 2,5% e medidos eletronicamente (ME1) utilizando o modelo em alginato; todas as medidas foram realizadas em triplicata por um operador cego, utilizando limas endodônticas ajustadas introduzidas até o forame apical. Os procedimentos de pré-alargamento cervical foram realizados sequencialmente com os instrumentos #25/.06 (conservador) e #25/.12 (convencional); novas determinações de CRs foram realizadas após cada protocolo de preparação cervical (CR2/CR3), da mesma forma que as medidas eletrônicas (ME2/ME3). O erro dos dispositivos (mm) foi avaliado considerando a diferença entre CRs e MEs em cada estágio de preparação; sua precisão foi estabelecida adotando ± 0,5 mm como margem de tolerância. O erro dos LEFs reduziu significativamente após o protocolo convencional de alargamento cervical (p<0,05), o que não ocorreu após o conservador. Os melhores valores de precisão foram observados após a preparação convencional como 90% (Root ZX II), 97,5% (Raypex 6) e 92,5% (RomiApex A-15). Não foram encontradas diferenças significantes nas comparações entre os LEFs, independentemente do protocolo cervical testado (p>0,05). Sob as condições testadas, pode-se concluir que os LEFs avaliados foram precisos. Além disso, os protocolos de alargamento influenciam sua precisão, onde o menos conservador produziu os melhores resultados.

BACKGROUND: Pilon fractures usually happened during severe injury, associated with compression and comminution of metaphysis and soft tissue injury and primary articular cartilage damage in varying degrees The treatment is difficult and the prognosis is relatively poor. OBJECTIVE: To explore the clinical effect of application of meta-plate and mini-plate internal fixation for the treatment of anterior comminuted coronal plane Pilon fractures. METHODS: From January 2013 to June 2017, 17 patients of closed anterior comminuted coronal plane Pilon fractures were admitted in Department of Bone and Joint Surgery, Affiliated Hospital of Southwest Medical University. There were 7 males and 10 females, aged from 24 to 62 years, with an average age of 41.3 years. All patients signed the informed consent. This study was approved by the Hospital Ethics Committee. Preoperative CT scans and three-dimensional reconstruction confirmed that the main fracture line was in coronal plane and multiple fragments of distal tibial metaphysis were displaced obviously. According to Ruedi-Allgower classification, there were 3 cases of type and 14 cases of type III. Fracture fragments of articular surface were fixed with transverse metacarpophalangeal mini-plate and screws, and the distal tibial coronal fractures were fixed with anterior meta-plate. The ankle and hind foot score of the American Orthopaedic Foot and Ankle Society was used to evaluate the function after surgery. RESULTS AND CONCLUSION: (1) The 17 patients were followed up for 12 to 28 months. All incisions healed in the first stage without complications such as wound infection, skin necrosis, failure of internal fixation and neurovascular injury. Only one patient had redness on the edge of the incision after operation and healed after swelling reduction and dressing change. (2) Fractures healed within 3 to 6 months without delayed union or nonunion. (3) At the last follow-up, the American Orthopaedic Foot and Ankle Society ankle and hind foot score was excellent in 10 cases, good in 5 cases, and average in 2 cases. The excellent and good rate was 88%. (4) The application of transverse mini-plate and screws fixation can effectively support and fix the fracture fragments of articular surface. Combined with anterior meta-plate of distal tibia for the treatment of anterior comminuted coronal plane Pilon fractures can achieve good therapeutic effect, and strengthen biomechanical stability.

BACKGROUND: Currently, surgical methods for the treatment of degenerative scoliosis include simple decompression and decompression combined with internal fixation. However, there is still controversy over the choice of fixation and fusion segment in surgery, which requires more reliable evidence-based medical evidence for reference. OBJECTIVE: To evaluate the clinical efficacy and complication of short versus long segments of internal fixation for the treatment of degenerative scoliosis using meta-analysis. METHODS: CNKI, Wanfang Database, VIP, CBM, EMBase, PubMed, Web of Science, and Cochrane Library were searched to collect the randomized and non-randomized controlled studies which compared long fusion with short fusion in the treatment of degenerative scoliosis from inception to February 2019. Relevant conference papers and authoritative journals in the field were retrieved manually. The quality of the included studies was assessed by two evaluation members according to the Cochrane collaboration network standard or the Newcastle-Ottawa Scale. The included studies were analyzed by using RevMan 5.3 software. RESULTS AND CONCLUSION: (1) A total of 20 articles were included, involving a sample of 1 329 individuals. Long segment group consisted of 601 cases, and short segment group consisted of 728 cases. (2) The meta-analysis results showed that long-segment internal fixation surgery had better improvement of coronal Cobb angle (P=0.000 4), coronal balance (P=0.000 2), Oswestry disability index (P=0.003) and visual analogue scale score (P 0.05). (4) These findings verify that long-segment internal fixation is superior to short-segment internal fixation in the correction of scoliosis, but the recovery is slow and the incidence of some complications after operation is high. It should be considered comprehensively when selecting specific surgical procedures.

Coronal shear fractures of the distal humerusare rare, complex fractures that can be technically challenging even in the best of hands. Their peak incidence is seen in elderly females and is usually the result of low energy fall. The injury is usually part of complex elbow injury and in rare instances can be isolated entity. Complex injury pattern is characterized by shear fracture combined with fracture of radial head and collateral ligament injury. The preferred treatment for this complex fracture is Open reduction and internal fixation. The usual recommendation is a two-incision approach. We report a rare case in which fracture could not be classified using the traditional classification systems and in which entire fracture was managed via a single approach

SUMMARY OBJECT: To explore the treatment effect of the anterior medial neurovascular interval approach to coronal shear fractures of the distal humerus. METHODS: This prospective study included two female patients who were 30-64 years old, with a mean age of 47 years. Fractures were caused by falling from a bicycle. The time between the injury and operation was 1-2 days, with a mean time interval of 1.5 days. Two patients with coronal shear fracture of the distal humerus were treated with open reduction and internal fixation using anterior neurovascular interval approach. RESULTS: There were no intraoperative and postoperative neurological and vascular complications or infections, and the fracture was united. At 12 months after the surgery, the patient returned to work without pain, and with a normal range of motion for elbow and forearm rotation. The X-rays revealed excellent fracture union, no signs of heterotopic ossification, and no traumatic arthritis. According to Mayo's evaluation standards for elbow function, a score of 100 is excellent. CONCLUSIONS: The application of the anterior neurovascular interval approach of the elbow in the treatment of shear fracture of the articular surface of the distal humerus, particularly the trochlea of the humerus, can reduce the stripping of the soft tissue.


RESUMO OBJETIVO: Explorar o efeito do tratamento com uma abordagem anterior do intervalo neurovascular médio para fraturas de cisalhamento coronal da porção distal do úmero. METODOLOGIA: Este estudo prospectivo incluiu duas pacientes do sexo feminino de 30-64 anos de idade, com idade média de 47 anos. As fraturas foram causadas por quedas de bicicleta. O tempo entre a lesão e a operação foi de 1-2 dias, com um intervalo de tempo médio de 1,5 dias. Duas pacientes com cisalhamento coronal da porção distal do úmero foram tratadas com redução aberta e fixação interna utilizando a abordagem anterior do intervalo neurovascular. RESULTADOS: Não houve complicações neurológicas e vasculares intra e pós-operatórias, nem complicações ou infecções, e a fratura foi unida. Após 12 meses da cirurgia, as pacientes retornaram ao trabalho sem dor e com uma amplitude normal de movimento de rotação do antebraço e cotovelo. Os raios-X revelaram excelente união das fraturas, sem sinais de ossificação heterotópica e sem artrite traumática. De acordo com as diretrizes da clínica Mayo para avaliação da função do cotovelo, uma pontuação de 100 é considerada excelente. CONCLUSÃO: A aplicação da abordagem anterior do intervalo neurovascular do cotovelo no tratamento de uma fratura de cisalhamento da superfície articular da porção distal do úmero, especificamente da tróclea do úmero, pode reduzir o desgaste do tecido mole.

Background: Smear layer is always formed during the instrumentation process due to the action of endodontic instruments during the shaping process and it should be removed as it might decrease the overall success of endodontic therapy. Aim of the study: To compare the cleaning efficiency of different rotary Ni-Ti systems: ProTaper Next, Xpendo Shaper and WaveOne Gold by assessing their ability to remove the smear layer from root canals walls. Methods and materials: A total of 24 palatal roots of maxillary molars were used in this study and randomly assigned into 3 groups (n=8) as follows: Group 1: instrumentation with ProTaper Next system (Dentsply Maillefer, Switzerland), Group 2: instrumentation with Xp-endo Shaper system (FKG Dentaire, Switzerland) and Group 3: instrumentation with WaveOne Gold system (Dentsply Maillefer, Switzerland). The samples were irrigated with 5.25% NaOCl. All samples were then examined by scanning electron microscope (SEM) at the center of the coronal, middle and apical thirds. The data was statistically analyzed using Kruskal Wallis and Mann-Whitney U tests. Results: ProTaper Next files showed a lower average mean of smear layer when compared to WaveOne Gold and Xp-endo Shaper files at the coronal third. No significant differences between the average means of smear layer were found at the middle and apical thirds. Conclusion: None of the tested groups showed a completely smear layer free root canal walls. In general, ProTaper Next files showed the best performance at the coronal third. All the files showed comparable performance at the middle and apical thirds.

Post-endodontic restorations are both important and challenging for clinical success in endodontically treated posterior teeth. Several options have been proposed to restore endodontically treated molars.In pediatric dentistry, restoration using conventional single crowns, especially for partially erupted molars with insufficient retentive tooth structure, has proven to be difficult. However, the endocrown presents a conservative and esthetic restorative alternative to conventional crowns with post-and-core, as it acquires additional retention within the pulp chamber. The tooth preparation consists of a circular, equigingival, butt-joint margin and a central retention cavity in the pulp chamber that helps to construct both the crown and core as a single unit.This case report describes the esthetic and conservative endocrown restorations of erupting permanent first molars with extensive coronal destruction.

Résumé

Objective: To explore the risk factors of coronal imbalance after posterior long-level fixation and fusion for degenerative lumbar scoliosis. Methods: Retrospectivly analyzed the clinical records of 41 patients with degenerative lumbar scoliosis who had received posterior long-level fixation and fusion with selective transforaminal lumbar interbody fusion (TLIF) accompanied by Ponte osteotomy between August 2011 and July 2016. Patients were divided into imbalance group (group A, 11 cases) and balance group (group B, 30 cases) according to state of coronal imbalance measured at last follow-up. The radiographic parameters at preoperation and last follow-up were measured, and the variance of preoperative and last follow-up parameters were calculated. The radiographic parameters included coronal Cobb angle, coronal balance distance (CBD), apical vertebral translation (AVT), apical vertebral rotation (AVR), Cobb angle of lumbar sacral curve (LSC), and L 5 tilt angle (L 5TA). Univariate analysis was performed for the factors including gender, age, preoperative T value of bone mineral density, number of instrumented vertebra, upper and lower instrumented vertebra, segments of TLIF, decompression, and Ponte osteotomy, as well as the continuous variables of preoperative imaging parameters with significant difference were converted into two-category variables, obtained the influence factors of postoperative coronal imbalance. Multivariate logistic regression analysis was performed to verify the risk factors from the preliminary screened influence factors and the variance of imaging parameters with significant difference between the two groups. Results: The follow-up time of groups A and B was (3.76±1.02) years and (3.56±1.03) years respectively, there was no significant difference between the two groups ( t=0.547, P=0.587). The coronal Cobb angle, AVT, LSC Cobb angle, and L 5TA in group A were significantly higher than those in group B before operation ( P0.05), and there were significant differences between the two groups in parameters including the variance of CBD, L 5TA, and AVR ( P<0.05). Univariate analysis showed that preoperative L 5TA was the influencing factor of postoperative coronal imbalance ( P<0.05). Multivariate logistic regression analysis showed that preoperative L 5TA≥15° was an independent risk factor of postoperative coronal imbalance, and variance of pre- and post-operative AVR was a protective factor. Conclusion: Preoperative L 5TA≥15° is an independent risk factor for coronal imbalance in patients with degenerative lumbar scoliosis after posterior long-level fixation and fusion.
