Objective:To evaluate the clinical efficacy of heat-sensitive moxibustion combined with kneading and massage manipulation at pain points in treating patients with lumbar disc herniation (LDH) and chronic low back pain (LBP).Methods:Randomized controlled trial. A total of 91 patients with LDH and LBP who were admitted to Lu'an Hospital of Traditional Chinese Medicine from January 2020 to December 2022 were selected as the observation subjects. They were divided into the experimental group (46 cases) and the control group (45 cases) by random number table method. The control group was treated with conventional massage manipulation, while the experimental group was treated with heat-sensitive moxibustion combined with kneading and massage manipulation at pain points. Both groups received 4 weeks of continuous treatment. The degree of lumbar pain was evaluated with the Visual Analogue Scale (VAS), and the degree of lumbar dysfunction was evaluated with modified Oswestry Disability Index (ODI). Plasma viscosity, hematocrit, whole blood low shear viscosity and whole blood high shear viscosity were measured using a blood rheometer. Clinical efficacy was evaluated. Results:The total clinical effective rates in the experimental group and the control group were 93.48% (43/46) and 77.78% (35/45), with a statistical significance between groups ( χ2=4.58, P=0.032). After treatment, the VAS score [(3.81±0.74) vs. (4.29±0.85), t=2.88] and ODI score [(8.79±2.65) vs. (11.25±3.74), t=3.63] of the experimental group were lower than those of the control group ( P<0.01). Plasma viscosity [(1.35±0.06) mPa?s vs. (1.41±0.08) mPa?s, t=4.05], hematocrit [(37.46±2.38)% vs. (40.15±2.94)%, t=4.80], whole blood low shear viscosity [(7.41±1.53) mPa?s vs. (8.64±1.72) mPa?s, t=3.61] and whole blood high shear viscosity [(3.81±0.29) mPa?s vs. (4.07±0.31) mPa?s, t=4.13] were lower than those in the control group ( P<0.01). Conclusion:Heat-sensitive moxibustion combined with kneading and massage manipulation at pain points can effectively relieve LBP in patients with LDH and chronic LBP, improve lumbar dysfunction and hemodynamic status, and enhance clinical efficacy.

ObjectiveTo explore the effectiveness and safety of vibration-kneading abdominal tuina as an adjunct treatment for insomnia of phlegm-heat harassing the interior syndrome with "stomach disharmony" symptom. MethodsSeventy-six insomnia patients of phlegm-heat harassing the interior syndrome with "stomach disharmony" symptom were recruited, and randomly divided into the treatment group and the control group with 38 cases in each group. The control group received eszopiclone 2mg once a night and sleep hygiene guidance, while the treatment group received vibration-kneading abdominal tuina additionally, 5 times a week, 30 min each time. Both groups were treated for 4 weeks. The Pittsburgh Sleep Quality Index (PSQI) (including sleep quality, time to fall asleep, sleep duration, sleep efficiency, sleep disorders, daytime functioning scores, and total scores) and traditional Chinese medicine (TCM) symptom scores of "stomach disharmony" was assessed before and after 4-week treatment and at follow-up (12 weeks after treatment). Adverse events were also observed and recorded during the study. Pearson correlation analysis was used to assess the correlation between the pre-treatment PQSI total score and TCM symptom score of "stomach disharmony". ResultsThe PSQI scores of sleep quality, time to fall asleep, sleep duration, sleep efficiency, sleep disorders, daytime dysfunction scores, and total scores reduced in both groups decreased after treatment and at follow-up (P<0.05 or P<0.01), as well as the TCM symptom score of "stomach disharmony" (P<0.01). After treatment, the PSQI scores of sleep quality, sleep duration, and TCM symptom score of "stomach disharmony" of the treatment group were lower than those of the control group (P<0.05 or P<0.01). At follow-up, the PSQI scores of sleep quality, sleep duration, sleep efficiency, daytime dysfunction score, total score, and TCM symptom score of "stomach disharmony" of the treatment group were lower than those of the control group (P<0.05 or P<0.01). No adverse reactions occurred in any participants during the study. There was a positive correlation between the patients' pre-treatment PQSI total scores and TCM symptom score of "stomach disharmony" (r=0.88, P<0.01). ConclusionVibration-kneading abdominal tuina as an adjunct treatment could significantly improve the symptoms of stomach and epigastric symptoms in insomnia patients of phlegm-heat harassing the interior syndrome with "stomach disharmony" symptom, and improve the quality of sleep, with good immediate and long-term therapeutic effects, and sound safety.

ObjectiveTo explore the clinical effectiveness and safety of stick-point sinew-soothing and bone-setting manipulation for scapulohumeral periarthritis. MethodsUsing prospective randomised controlled trial method, 60 cases of patients with scapulohumeral periarthritis were collected and randomly divided into 30 cases each in control group and trial group. Both groups of patients were orally treated with celecoxib, on the basis of which the control group was treated with traditional bonesetting manipulation once every other day for 14 days, while the trial group was treated with stick-point sinew-soothing and bone-setting manipulation once every 3~5 days for 14 days. Both groups were treated for 2 courses. The main observation indexes were pain visual analogue scale (VAS) score and shoulder pain and dysfunction index (SPADI), which were evaluated once before treatment and after 2 and 4 weeks of treatment. The secondary effectiveness indicators included the university of California at Los Angeles shoulder rating scale (UCLA), traditional Chinese medicine syndrome score (including symptom scores as joint pain, pain in a fixed place, activity limitation, local stiffness), and serum interleukin (IL-6) and tumour necrosis factor α (TNF-α) levels before and after the treatment, in order to evaluate the clinical effectiveness, and to record the adverse reactions that occurred in the process of diagnosis and treatment. ResultsCompared with the groups before treatment, the pain VAS score, SPADI and scores of joint pain, pain with a fixed place, activity limitation and local stiffness were lower, UCLA score was higher, and serum IL-6 and TNF-α levels were lower at 4 weeks of treatment in the two groups (P<0.05). When comparing the two groups between the groups at 2 and 4 weeks of treatment, the pain VAS score, SPADI and TCM scores of each symptom in the study group were lower than those in the control group, the UCLA score was higher than those in the control group (P<0.01), and the serum IL-6 and TNF-α levels were lower than those in the control group at 4 weeks of treatment (P<0.01). The clinical effectiveness rate of the study group was 66.67%, which was significantly higher than that of the control group, which was 40.00% (P = 0.038). No adverse reactions were seen in both groups during the study. ConclusionCompared with the traditional massage manipulation, the treatment of scapulohumeral periarthritis with stick-point sinew-soothing and bone-setting manipulation has more advantages in relieving pain symptoms, reducing inflammatory reaction, and promoting the recovery of shoulder joint function.

Résumé

BACKGROUND:Among the pathogenic factors of cervical spondylosis, herniation of the intervertebral disc, dislocation of the facet joint and the stenosis of the intervertebral foramen are important factors leading to symptoms in patients. Moreover, inappropriate manipulation may aggravate the possibility of cervical disc rupture, leading to exacerbation of symptoms in patients. OBJECTIVE:To compare the effect between sagittal cervical manipulation and traditional cervical rotation manipulation on the area of the intervertebral disc, facet joint and intervertebral foramen at the operative segment by the finite element analysis. METHODS:The neck CT data of a male volunteer with a normal neck were selected and imported into Mimics 17.0 three-dimensional reconstruction software.Geo-magic Studio 12.0,Solidworks 2017 and Ansys Workbench 17.0 software were used for the construction of the finite element model of cervical vertebrae(C3-6)including intervertebral disc and articular cartilage. The lower end plate of the C5 vertebral body was fixed.A uniformly distributed vertical downward 50 N load was applied on the upper surface of the upper vertebral body(C3).The stress, deformation and deformation direction of the C4-5 intervertebral disc, joint capsule stress, the displacement of facet joints and the area of bilateral intervertebral foramen were compared between sagittal cervical manipulation and traditional rotation reduction. RESULTS AND CONCLUSION:(1)When using the rotation technique, the maximum normal equivalent stress(von Mises stress)of the C4-5 disc was 8.06 MPa;the total deformation was 1.05 mm,and the fiber ring expanded to the left and outside. When using the sagittal tip lifting technique, the maximum normal equivalent stress(von Mises stress)of the C4-5 disc was 2.60 MPa;the total deformation was 0.90 mm,and the fiber ring expanded to the left and back.Compared with the rotation technique, the pressure of the cervical manipulation technique on the disc was less(about 32.3%of the rotation technique),and the deformation degree of the disc was also light(about 85.7%of the rotation technique).(2)When the rotation technique was used, the maximum stresses of the left and right articular capsule ligaments were 0.37 MPa and 1.69 MPa,respectively. The overall displacement of the facet joint was 2.21 mm.The area of the right intervertebral foramen decreased by about 3.8%and the area of the left intervertebral foramen increased by about 0.9%.When the sagittal end lifting manipulation was performed, the maximum stresses of the left and right articular capsule ligaments were 0.27 MPa and 1.70 MPa,respectively;the overall displacement of the facet joint was 1.63 mm;the area of the right intervertebral foramen increased by about 2.6%,and the area of the left intervertebral foramen decreased by about 0.9%.Compared with rotation manipulation, sagittal end lifting manipulation had fewer changes in the displacement of facet joint, joint capsule stress and intervertebral foramen area,so it was safer to operate.(3)In conclusion, compared with cervical rotation manipulation, sagittal end lifting manipulation has fewer changes in facet joint displacement, intervertebral disc stress/deformation degree, joint capsule stress, and foraminal area.In clinical practice, more appropriate manipulation should be selected based on biomechanical results after an accurate assessment of patients'conditions.

Résumé

BACKGROUND:The evaluation and management of cervical vascular pathologies before orthopedic manual therapy have great significance for reducing risks, ensuring patients'interests and promoting optimal clinical decision-making.However,the research and exploration of this field in China are still in its infancy. In 2020,the International Federation of Orthopaedic Manipulative Physical Therapists(IFOMPT)released the International Framework for Examination of the Cervical Region for potential of vascular pathologies of the neck prior to Orthopaedic Manual Therapy(OMT)Intervention:International IFOMPT Cervical Framework(2020).This framework has important references and guiding values for the clinical practice of cervical manipulation in China. OBJECTIVE:To integrate and interpret the core content of the framework,to provide a reference for the clinical practice of cervical manual therapy in China. METHODS:On the basis of a full study of the framework, the authors interpret the core content of the framework from eight aspects including clinical reasoning, patient history, planning the physical examination, physical examination, risk and benefit.Meanwhile, seven databases including PubMed,EMbase,Cochrane Library,CNKI,WanFang,VIP and China Biomedical Literature Service System databases were searched by computer to screen clinical practice guidelines and expert consensus on neck pain.Through longitudinal comparison of multiple high-quality guidelines and consensus, the authors combine with the clinical practice in China to carry out discussion and analysis. RESULTS AND CONCLUSION:This framework expounds the management strategy and implementation path of cervical vascular disease evaluation before cervical manipulation from multiple dimensions, suggesting that we should improve clinicians'attention to cervical vascular disease evaluation before manipulation, and construct evaluation criteria and implementation path with Chinese clinical characteristics.Meanwhile,we should further carry out basic scientific research with multidisciplinary techniques and promote shared decision-making and teaching model innovation to achieve the best clinical decision.

Résumé

BACKGROUND:Lumbar fixed-point rotation operation needs collaborative operation of the doctor's hands, and outputs rotation and thumb thrust.Lumbar disc herniation can be treated through disc displacement and adjusting stress distribution. However, the mechanical effects of thumb thrust and the biomechanical effects of loading direction on manipulative effects remain unclear. OBJECTIVE:To compare the biomechanical difference of lumbar fixed-point rotation manipulation for treating lumbar disc herniation under different thrust directions. METHODS:The L3-5 normal three-dimensional finite element model was constructed and validity was verified.According to the intervertebral disc degeneration Pfirrmann grade, intervertebral disc degeneration was simulated by modifying the L4/5 intervertebral space height, the volume of the nucleus pulposus,as well as the material parameters of the annulus fibrosus, nucleus pulposus, and ligament. Finally, the pathological model of L4/5 moderate disc degeneration with left para-central herniation was constructed, and then the pathological models were used as research objects.Simulation technique:spinning to the right;taking the condition on changing the direction of the thumb thrust to establish three modes of operation(M1:thumb push to the left;M2:thumb push to the right;M3:no thrust push). The protrusion displacement and the disc stress, and the stress and strain of the facet joint cartilage were compared in the three operating modes. RESULTS AND CONCLUSION:(1)Maximum displacement value of L4/5 disc herniation:displacement was 2.672 3 mm for M1,1.156 1 mm for M2,1.826 4 mm for M3,M1>M3>M2.(2)The maximum Von Mises stress of L4/5 discs was 1.846 7 MPa for M1,0.419 0 MPa for M2,and 1.257 9 MPa for M3,M1>M3>M2.(3)L4/5 bilateral small cartilage produced different degrees of contact stress changes:It was 0.485 5 MPa for M1,0.026 7 MPa for M2,and 0.441 4 MPa for M3,M1>M3>M2.Right cartilage contact force was 0.000 5 MPa for M1,0.025 9 MPa for M2,and 0.001 3 MPa for M3,M2>M3>M1;the left greater than the right,M1 had the highest value;cartilage strain was consistent with contact stress changes.(4)Different operation modes will have some biomechanical influences on the diseased intervertebral disc and accessory structure. The M1 operation mode can maximize the displacement of protrusion, disc stress and left joint cartilage contact, which can better promote disc displacement, balance stress distribution and reduce facet joint disorder,so the operation is better.

Résumé

BACKGROUND:Cervical rotatory manipulation is widely used in the treatment of neck-related diseases with a clear curative effect, but it also has some risks in clinical practice. The previous study of our group found that cervical rotatory manipulation can reduce the tensile mechanical properties of the atherosclerotic carotid artery, but it is not clear about the effects of different cervical rotatory manipulations(positioning/non-positioning)and different degrees(mild/moderate/severe)of atherosclerosis on the tensile mechanical properties of the carotid artery. OBJECTIVE:To explore the effects of different cervical rotatory manipulations and different degrees of atherosclerosis on the tensile mechanical properties of the internal carotid artery. METHODS:The 120 male New Zealand rabbits were randomly divided into eight experimental groups with different degrees of atherosclerosis and different cervical rotatory manipulations:severe atherosclerosis + positioning/non-positioning cervical rotatory manipulation, moderate atherosclerosis + positioning/non-positioning cervical rotatory manipulation, mild atherosclerosis + positioning/non-positioning cervical rotatory manipulation, and normal rabbit + positioning/non-positioning cervical rotatory manipulation,as well as three model control groups:mild/moderate/severe atherosclerosis + non-cervical rotatory manipulation, and the blank control group.Two-factor analysis of variance was used to explore the main effects and interactive effects of different cervical rotatory manipulations and different degrees of atherosclerosis on the tensile mechanical properties of the internal carotid artery.One-way analysis of variance was applied to explore the influence of different cervical rotatory manipulations on the tensile mechanical properties of the internal carotid artery under the same degree of atherosclerosis. RESULTS AND CONCLUSION:(1)Both different cervical rotatory manipulations and different degrees of atherosclerosis were the main effect factors affecting the tensile mechanical properties of the internal carotid artery.(2)For both mild and severe atherosclerosis, both positioning and non-positioning cervical rotatory manipulations reduced the maximum stress of the internal carotid artery(P<0.05)and also increased the physiological elastic modulus of the internal carotid artery(P<0.05).(3)For moderate atherosclerosis, positioning and non-positioning cervical rotatory manipulations also increased the physiological elastic modulus of the internal carotid artery(P<0.05).Non-positioning cervical rotatory manipulation reduced the maximum strain of the internal carotid artery(P<0.05),and its maximum strain was also less than the internal carotid artery of the positioning cervical rotatory manipulations(P<0.05).(4)For the normal internal carotid artery,in addition to the maximum strain, both positioning and non-positioning cervical rotatory manipulations had no statistically significant effects on other tensile mechanical indicators of the internal carotid artery(P>0.05).(5)The results suggest that both positioning and non-positioning cervical rotatory manipulations may increase the stiffness of the atherosclerotic internal carotid artery, reducing its elasticity and brittleness. Therefore, both positioning and non-positioning cervical rotatory manipulations may increase the risk of cardiovascular events in mild/moderate/severe atherosclerotic internal carotid artery, and the more severe the atherosclerosis is,the greater the risk of positioning/non-positioning cervical rotatory manipulation treatment, but the risk of positioning cervical rotatory manipulation is not lower than that of non-positioning cervical rotatory manipulation.

Résumé

ABSTRACT Background: Functional constipation (FC) is a common global high prevalence issue in children. Objective: The purpose of the present study is to evaluate the effect of visceral manipulation (VM) on children with chronic interacble FC unresponsive to the standard treatment. Methods: This study was conducted as a randomized, single-blind controlled trial. Fifty-two children with refractory chronic functional constipation unresponsive to the standard medical treatment were randomly allocated to two groups of 26 control (standard medical care (SMC)) and 26 intervention (SMC with VM) for 4 weeks. Abdominal pain, painful defecation, stool consistency, defecation frequency, and the dose of oral laxative were evaluated before and after the treatment period using the Pain Rating Scale, Bristol stool form scale, and patient/parents report. Results: At the end of treatment, except for the dose of oral laxative in the control group, all of the results showed a significant difference in both groups (P<0.05). The dose of oral laxative in the intervention group decreased significantly (P<0.05), however, no significant change was observed in the control group (P>0.05). In the intervention group comparison, statistically significant differences were found in all variables except the Bristol stool form scale (P<0.05). The Bristol stool form scale after treatments was not different when the groups were compared (P=0.32), but the number of subjects who had normal stool consistency was significantly increased in the intervention group than in the control group (P<0.05). Conclusion: VM can be considered as a possible treatment without side effects besides SMC for the management of chronic FC. Further studies are needed to investigate the long-term effect of VM.

Security and human rights have a contradictory relationship. The crux of the problem lies in the tension that exists between the different approaches to security and the protection of human rights in different contexts and periods, in which fear plays a fundamental role. This review article describes how the public security, citizen security and human security perspectives can be manipulated so that they do not fulfil the objective of guaranteeing human rights and citizen freedoms and actually end up promoting the violation or limitation of these. In other words, these approaches exhibit the oppositional logic that states that what is gained in security is lost in rights, a perspective which ignores the dynamics that should accompany democratic regimes, where security is not an end in itself, but rather a means by which the state and its institutions can guarantee the effective enjoyment of human rights and civil liberties.

Objective: To analyze the relation between adherence levels to good practice recommendations for insulin therapy and glycemic control metrics in patients with diabetes mellitus. Methods: A descriptive, cross-sectional, quantitative study was conducted with 102 patients with diabetes mellitus. Data were collected by means of semi-structured interviews and complemented by information obtained from medical records. Participants answered four collection instruments: I) a sociodemographic and clinical characterization form, II) a reminder of insulin therapy guidelines, III) a capillary blood glucose self-monitoring record sheet and IV) a record form of glycemic control assessment metrics. Results: Results showed a statistically significant association between non-adherence level (100%) to insulin therapy recommendations with estimated time on target ≤ 70% and between non-adherence (80%) and standard deviation ≥ 50 mg/dl. Conclusion: These findings corroborate that non-adherence to good insulin therapy recommendations contribute to a lack of glycemic control

Identifying irregularities in restaurants can help minimize contamination risks. The objective of this study was to analyze the irregularities recorded in sanitary inspections of restaurants and similar establishments in Curitiba, Brazil, between 2005 and 2017. The description of the irregularities was categorized following the items and criteria outlined in the Good Practices (GP) legislation for food services of Brazil and the Five Keys to Safer Food, defined by the World Health Organization. Out of the total of 4,098 records, 8,327 irregularities were documented. Concerning GP, there was a prevalence of irregularities described in the years 2005, 2006 and 2013, mainly in the following categories: buildings, installations, equipment, furniture and utensils (42.80%); food preparation (11.54%); documentation and registration (9.92%) and raw materials, ingredients and packaging (9.06%). Regarding the Five Keys to Safer Food, only 3,209 (38.53%) corresponded to this theme, with particular emphasis on maintaining cleanliness (40.70%) and using safe water and raw materials (34.71 %). The study observed a declining trend in the description of irregularities over the study period. However, the successful implementation of GP in restaurants and similar establishments in Brazil remains a challenge, potentially favoring the occurrence of foodborne diseases and increasing the risk to public health.

Résumé

We describe the case of an 82-year-old man who had recently undergone cardiac surgery (quadruple coronary bypass), who consulted due to the appearance of a necrotic eschar on the thumb of the right index finger, together with paraesthesia and hypoaesthesia in the first 3 fingers of the same hand. An ultrasound scan of the right elbow was performed to rule out involvement of the median nerve and an anechoic, thick-walled mass was found, dependent on the wall of the proximal ulnar artery, compatible with a pseudoaneurysm of the same, compressing the nerve. Electromyography showed an acute lesion of the proximal median nerve and angio-CT confirmed the diagnosis of pseudoaneurysm of the proximal ulnar artery. Pseudoaneurysm is a dilatation by rupture of the arterial wall, which does not involve all three layers of the arterial wall and communicates with the vascular lumen. Its development after vascular manipulation is very rare, and it is uncommon for it to act by compressing a nerve structure. In our case, together with vascular surgery, treatment with intralesional thrombin was decided, with good evolution.

Objective:To evaluate the efficacy of Wechsler's bone setting manipulation in the treatment of lumbar disc herniation. Methods:A randomized controlled trial was carried out. 50 LDH patients from the Spinal Surgery Department of Ruikang Hospital Affiliated to Guangxi University of Chinese Medicine and the Ruikang Hospital Traditional Chinese Medicine Master Hall Affiliated to Guangxi University of Chinese Medicine were selected as observation subjects from January to November 2021. They were divided into two groups using a random number table method, with 25 patients in each group. The control group was treated with traditional bone setting techniques, while the observation group was treated with Wechsler's bone setting correction techniques. Both groups were treated for 3 weeks. VAS scale was used to evaluate the degree of lower back and leg pain, and the Japanese Orthopedic Association was used to evaluate treatment (JOA score) to evaluate lumbar function; the sacral tilt angle (SS), lumbar lordosis angle (LL), and coronal pelvic tilt angle values were measured on the lateral X-ray film of the entire spine; adverse reactions were recorded and clinical efficacy was evaluated. Results:The total effective rate of the observation group was 92.0% (23/25), and the control group was 68.0% (17/25), with statistical significance between the two groups ( χ2=4.50, P=0.034). The VAS score of the observation group after treatment was lower than that of the control group ( t=16.25, P<0.01); the JOA score was higher than that of the control group ( t=-29.39, P<0.01). The SS [(36.00 ± 1.87) ° vs. (34.16 ± 1.57) °, t=-10.64] and LL [(38.08 ± 2.60) ° vs. (34.96 ± 1.90) °, t=-10.04] in the observation group were higher than those in the control group ( P<0.01); the coronal pelvic tilt angle value [(0.47 ± 0.14) ° vs. (0.69 ± 0.14) °, t=42.57] was lower in the control group ( P<0.01). No adverse reactions occurred during the treatment period in both groups. Conclusion:Wechsler's bone setting manipulation can improve the pain symptoms of patients with LDH, and restore the spine pelvis biomechanical balance to a certain extent.

Résumé

For patients with femoral neck fractures who plan to undergo internal fixation, satisfied alignment of fracture ends is an important prerequisite for internal fixation stability and fracture healing. There are many reports on the reduction methods of displaced femoral neck fractures, which can be summarized into three categories:First, the solely longitudinal traction of lower limbs, supplemented by other manipulations such as rotation and compression; Second, the resultant force formed by the longitudinal traction of lower limbs and the lateral traction;the third is accomplished by vertical traction in the axis of femur with hip joint flexed. Each reduction method has its own advantages, but no single method can be applied to all fracture displacement. In this paper, some classical reduction techniques in the literatures are briefly reviewed. It is hoped that clinicians will not be limited to a certain reduction method, they should analyze the injury mechanism and fracture displacement process according to the morphology features and flexibly select targeted reduction methods to improve the success rate of closed reduction of femoral neck fracture.
