ABSTRACT BACKGROUND: Loxosceles spp are arthropods found worldwide. Its bite may produce cutaneous loxoscelism (necrotic or edematous) or cutaneous-visceral loxoscelism. Depending on their severity and location, cutaneous forms are managed with local cold application and systemic administration of antihistamines, corticosteroids, antibiotics, polymorphonuclear inhibitors, and analgesics. OBJECTIVE: This study aimed to report a case of cutaneous loxoscelism and to identify the main dermatological manifestations associated with the Loxosceles spp bite. DESIGN AND SETTING: This case report and literature review was conducted in a Mexican university. METHODS: A detailed report on the medical management of a patient with cutaneous loxoscelism treated at the emergency department of a public hospital was published. Scopus, PubMed, Web of Science, and Google Scholar databases were searched to identify articles reporting cutaneous loxoscelism. The following keywords were used during the database search: "loxoscelism" OR "spider bite," OR "loxosceles" OR "loxosceles species" OR "loxosceles venom" OR "loxoscelism case report" AND "cutaneous" OR "dermonecrotic arachnidism." RESULTS: A 62-year-old female patient with cutaneous loxoscelism was treated with systemic dapsone and local heparin spray. Eighteen studies with 22 clinical cases were included in this systematic review. Of the 22 patients, 12 (54.5%) were men. L. rufescens was the predominant spider species. CONCLUSIONS: The administration of dapsone and heparin for the management of cutaneous loxoscelism demonstrated success in this case, with no sequelae observed. In general, the literature review indicated favorable outcomes in patients treated with antimicrobials and corticosteroids, with continuous healing of skin lesions. Objective: to describe the completeness of notifications of accidents involving venomous animals held on the Notifiable Health Conditions Information System (SINAN), in Brazil and its macro-regions, from 2007 to 2019. Methods: we analyzed essential and non-mandatory fields for snakebite, spider bite and scorpion sting notifications, considering the following completeness categories: Excellent (≤ 5.0% incompleteness), Good (5.0% to 10.0%), Regular (10.0% to 20.0%), Poor (20.0% ≤ 50.0%) and Very Poor (> 50.0%). Proportional change in completeness between 2007 and 2019 was estimated. Results: 1,871,462 notifications were investigated. The "localized manifestations", "systemic manifestations", "case classification", "case progression" and "zone of occurrence" fields had excellent or good completeness. Completeness was regular or poor for the "schooling" and "race/color" fields. The "occupation" field was predominantly poorly or very poorly filled in. There was a proportional worsening in completeness ((VP < 0) in most regions for the "zone of occurrence", "case progression" and "schooling" fields. Conclusion: completeness of most fields improved, although socioeconomic and occupational fields require more attention.
Objetivo: describir la completitud de las notificaciones de envenenamiento por serpientes, arañas y escorpiones en el Sistema de Información de Enfermedades de Declaración - Sinan, en Brasil y regiones, de 2007 a 2019. Métodos: se analizaron campos esenciales y no obligatorios para las notificaciones de ofidismo, aracneismo y escorpionismo, considerando las categorías de completitud: Excelente (≤ 5,0% de incompletitud), Buena (5,0% a 10,0%), Regular (10,0% a 20,0%), Mala (20,0% ≤ 50,0%) y Muy mala (> 50,0%). Se estimó el cambio proporcional en la completitud entre 2007-2019. Resultados: se investigaron 1.871.462 notificaciones. Los campos "manifestaciones locales y sistémicas", "zona de ocurrencia", "clasificación y evolución de casos" presentaron excelente o buena completitud. "Escolaridad" y "raza/color de piel" regular o mala completitud. "Ocupación" fue predominantemente mala/muy mala completitud. "Zona","evolución" y "escolaridad" mostraron empeoramiento proporcional en la completitud en la mayoría de las regiones ((VP < 0). Conclusión: hay mejoría y completitud en la mayoría de los campos, aunque los campos socioeconómico y ocupacional demandan atención.

Métodos: analisou-se a completude de campos de preenchimento essenciais e não obrigatórios das notificações de ofidismo, araneísmo e escorpionismo, utilizando-se a classificação de "Excelente" (≤ 5,0% de incompletude), "Bom" (5,0% a 10,0%), "Regular" (10,0% a 20,0%), "Ruim" (20,0% e ≤ 50,0%) e "Muito ruim" (> 50,0%); estimou- -se a variação proporcional (VP) da completude. Resultados: foram investigadas 1.871.462 notificações; os campos "manifestações locais", "sistêmicas", "classificação do caso", "evolução do caso" e "zona de ocorrência" apresentaram completude excelente ou boa; "escolaridade" e "raça/cor da pele", completude regular ou ruim; ocupação, completude ruim ou muito ruim; "zona", "evolução do caso" e "escolaridade" apresentaram piora proporcional na completude, na maioria das regiões ((VP < 0). Conclusão: melhorou a completude na maioria dos campos, exceto socioeconômicos e ocupacionais, que demandam maior atenção.

Objetivo: Analisar a tendência temporal dos acidentes causados por animais peçonhentos no Brasil, de 2007 a 2019. Métodos: Estudo transversal, realizado com dados do Sistema de Informação de Agravos de Notificação (Sinan). Para a análise temporal, utilizou-se a regressão linear de Prais-Winsten. Foram calculadas as incidências por sexo e faixa etária, risco relativo e letalidade. Resultados: No período de estudo, houve 2.102.657 casos de acidentes por animais peçonhentos. Com exceção do ofidismo, os demais acidentes apresentaram tendência temporal crescente na maioria das macrorregiões nacionais. Escorpionismo, ofidismo e araneísmo foram responsáveis por 86% dos acidentes, principalmente entre pessoas do sexo masculino em idade economicamente ativa. A de letalidade foi mais elevada para acidentes ofídicos (0,4%) e apílicos (0,3%). As crianças foram as principais vítimas de acidentes com abelhas, lagartas e "outros". Conclusão: Os acidentes com animais peçonhentos apresentaram tendência temporal crescente para a maioria dos agravos e diferentes perfis epidemiológicos.
Objetivo: Analizar la tendencia temporal de los accidentes con animales ponzoñosos en Brasil de 2007 a 2019. Métodos: Estudio transversal realizado con datos del Sistema de Información de Enfermedades de Declaración Obligatoria (Sinan). Para el análisis temporal se utilizó la Regresión Lineal de Prais-Wisten. Se calcularon las incidencias por sexo y grupo de edad, riesgo relativo y letalidad. Resultados: Durante el período del estudio hubo 2.102.657 casos de accidentes con animales ponzoñosos. Con excepción de la mordedura de serpiente, los demás accidentes mostraron una tendencia temporal creciente en la mayoría de las regiones del país. El escorpionismo, la mordedura de serpiente y la picadura de araña fueron responsables del 86% de los accidentes, afectando principalmente a personas del sexo masculino en edad laboral. Las tasas de letalidad más altas fueron por accidentes de mordedura de serpiente (0,4%) y picadura de araña (0,3%). Los niños fueron las principales vitimas de los acidentes con abejas, lagartas y "otros". Conclusión: Los accidentes com animales ponzoñosos mostraron una tendencia temporal creciente para la mayoría de las enfermedades y diferentes perfiles epidemiológicos.
Objective: to analyze the temporal trend of accidents involving venomous animals in Brazil from 2007 to 2019. Methods: this was a cross-sectional study carried out with data from the Notifiable Health Conditions Information System (SINAN). Prais-Winsten linear regression was used for the temporal analysis. We calculated incidence rates according to sex and age group, relative risk and case fatality ratio. Results: during the study period there were 2,102,657 cases of accidents involving venomous animals. With the exception of snakebite, the remaining accidents showed a rising temporal trend in most regions of the country. Scorpion stings, snake bites and spider bites were responsible for 86% of accidents, mainly affecting male people of working age. Accidents involving snakes (0.4%) and bees (0.3%) had the highest case fatality ratios. Children were the main victims of accidents involving bees, caterpillars and "others". Conclusion: accidents involving venomous animals showed a rising temporal trend for most conditions, as well as different epidemiological profiles.

Background: Loxoscelism is an important public health problem in Chile and South America, due to the higher rate of cutaneous-visceral involvement. The diagnosis of loxoscelism is mostly clinical without established diagnostic criteria. There is little evidence to support any treatment used in this condition. Aim: To characterize the clinical features and epidemiology of loxoscelism among patients consulting at the Emergency and Dermatology Services of a clinical hospital between 2013 and 2017. Material and Methods: Review of medical records of patients registered in the electronic clinical record system with a confirmed diagnosis of loxoscelism. Epidemiological, clinical, laboratory tests and treatment variables were analyzed. Results: We reviewed data from 200 patients. Ninety-four percent presented cutaneous loxoscelism and 5.5% cutaneous-visceral loxoscelism. Systemic symptoms were present in 73% of patients with cutaneous-visceral loxoscelism. Patients who developed systemic symptoms had an 18 times higher risk of developing cutaneous-visceral loxoscelism. Laboratory abnormalities were more common in patients with cutaneous-visceral loxoscelism. Not all patients with hematuria had cutaneous-visceral loxoscelism. Most patients required analgesia. Anti-loxosceles serum was not used in any patient. Conclusions: Many questions remain to be answered regarding the diagnosis and treatment of the disease. Studies are required to validate diagnostic criteria for loxoscelism, predictors for visceral involvement and response to treatment.

Despite the disrepute spiders have had for centuries, their bite is a rare occurrence. In the Mediterranean area, only two of the numerous known species are considered of medical significance: Latrodectus tredecimguttatus and Loxosceles rufescens. Spider bites have no pathognomonic signs or symptoms, therefore most diagnoses are presumptive; a spider bite can only be diagnosed when a spider (seen at the time of the bite) is collected and identified by an expert, since most physicians and patients are unable to recognize a certain spider species or distinguish spiders from other arthropods. Skin lesions of uncertain etiology are too often attributed to spider bites. In most cases, these are actually skin and soft-tissue infections, allergic reactions, dermatoses etc. Misdiagnosing a wound as a spider bite can lead to delays in appropriate care, cause adverse or even fatal outcomes and have medical-legal implications. Concerningly, misinformation on spider bites also affects the medical literature and it appears there is lack of awareness on current therapeutic indications for verified bites.(AU)

Abstract INTRODUCTION: Loxoscelism is a clinical condition involving spiders of the genus Loxosceles. One of the most severe complications is acute kidney injury (AKI). This study aimed to investigate AKI and other complications associated with loxoscelism. METHODS: We analyzed cases diagnosed with loxoscelism in an area where most accidents were caused by Loxosceles amazonica from January 2010 to December 2015. AKI was defined according to the KDIGO criteria. RESULTS: Forty-five patients were recorded: 95.6% presented characteristic necrotic skin lesions and 13.3% AKI. CONCLUSIONS: Loxoscelism could cause kidney involvement which is uncommon and could lead to the death of these patients.

La mordedura de araña es un accidente muy común e importante problema de salud pública en lugares con clima tropical y se presenta como un problema muy frecuente en algunas regiones de Latinoamérica. En Perú las especies de araña que presentan más casos de esta problemática son Loxosceles laeta, Latrodectus mactans and Phoneutria spp. El presente trabajo pretende explicar, clarificar y categorizar estas mordeduras de arañas, con el fin de dar a conocerlas para ayudar a que los profesionales de la salud tengan un mayor conocimiento sobre las mismas para un mejor manejo de los casos que se les presenten. Spider bites are a common and important public health issue in tropical climates, and become a frequent problem in certain Latin American regions. In Peru, the main species of spiders that represent this problematic are Loxosceles laeta, Latrodectus mactans and Phoneutria spp. The present article intends to explain, clarify and categorize these spider bites, in order to make them known by every health professional to its better management. Resumen Las arañas son artrópodos venenosos de distribución global. La mordedura de la mayoría de las arañas no tiene trascendencia clínica, debido a la dificultad que éstas tienen de inyectar veneno a los seres humanos por el pequeño tamaño de sus quelíceros. A pesar de esto, existen cientos de casos reportados con envenenamiento secundario a mordedura de arañas. El cuadro clínico es pleomórfico, aunque pueden distinguirse tres síndromes principales: latrodectismo, loxoscelismo y envenenamiento por la araña de "tela en embudo". El latrodectismo se debe a una neurotoxina llamada alfa-latrotoxina presente en el veneno de las "viudas negras" (latrodectus sp). El cuadro cursa con dolor intenso en el sitio de la mordedura, y la complicación sistémica más común es la hipertensión arterial. Las complicaciones neurológicas consisten en cefalea, letargia, irritabilidad, mialgias, temblor, fasciculaciones, ataxia o parestesias. Estas últimas se relacionan con liberación excesiva de neurotransmisores colinérgicos a nivel de la unión neuromuscular. El loxoscelismo es causado por arañas de la familia Sicariidae, géneros Loxosceles y Sicarius. Existen dos tipos de presentación, la cutánea (cambios limitados al área de piel afectada) y la víscerocutánea. En esta última, puede ocurrir coagulopatía severa, asociada o no a insuficiencia renal aguda, ictericia, fiebre, falla multiorgánica y shock. El veneno de las arañas "tela de embudo" tiene potentes neurotoxinas llamadas delta-atracotoxinas, las que simulan neurotransmisores liberados por vesículas pre-sinápticas, causando hiperestimulación de los receptores pos-sinápticos, lo que resulta en alteraciones sensitivas y parálisis muscular. También pueden desarrollarse alteraciones del sistema nervioso simpático y parasimpático, tales como diaforesis, sialorrea, hipotensión arterial y arritmias cardiacas.
Abstract Spiders are venomous arthropods with worldwide distribution. Most spider bites are clinically insignificant, as the size of their chelicerae is too small to inject enough amount of venom to human beings. Despite this, there are hundreds of reported cases of human envenoming related to spider bites. While clinical manifestations related to spider-bite envenoming are pleomorphic, three main clinical syndromes could be recognized: latrodectism, loxoscelism, and funnel-web spider envenoming. Latrodectism is caused by the potent neurotoxin (-latrotoxin present in the venom of widow spiders (Latrodectus sp.). Most cases present with severe pain at the site of the bite, with arterial hypertension being the most frequent systemic manifestation. Neurological complications mostly consist of headache, lethargy, irritability, generalyzed myalgia, tremor, fasciculations, ataxia, or paresthesias. The latter manifestations are related to an exhaustive release of cholinergic neurotransmitters at the neuromuscular junction. Loxoscelism is caused by envenoming by spiders of the family Sicariidae, genera Loxosceles and Sicarius. It presents in two different forms, cutaneous (with changes restricted to the affected skin) area and viscerocutaneous. In the latter, a severe coagulopathy may occur, associated or not with acute renal failure, jaundice, fever, multi-organ failure and shock. The venom of fennel-web spiders have potent neurotoxins, called (-atracotoxins, that stimulates neurotransmitter release from the presynaptic vesicle, causing hyperstimulation of post-synaptic receptors resulting in sensory disturbances and muscle paralysis. Also, sympathetic and parasympathetic manifestations, including diaphoresis, hypersalivation, arterial hypotension, and cardiac arrhythmias may occur.
RÉSUMÉ
El latrodectismo es un cuadro clínico causado por la mordedura de una araña del género Latrodectus spp., ampliamente distribuido en todo Chile continental. Generalmente las mordeduras se asocian a faenas agrícolas. El veneno del género Latrodectus contiene α-latrotoxina, una neurotoxina que actúa a nivel presináptico del sistema nervioso autónomo aumentando la liberación de acetilcolina. El cuadro clínico es inespecífico y hasta en un tercio de los casos ocurren síntomas sistémicos. El diagnóstico es clínico y depende del antecedente de la mordedura por una araña con un cuadro clínico compatible. El pronóstico es favorable: se reporta una letalidad entre 0 y 6 por ciento. El manejo es esencialmente sintomático, principalmente analgésico. Presentamos el caso de un hombre de 62 años, proveniente de Santa Cruz, VI región, trabajador en una plantación de trigo. Consultapor dolor torácico asociado a mialgias generalizadas y diaforesis. Por sospecha de latrodectismo, se administra neostigmina con buena respuesta clínica.(AU)
Latrodectism is a clinical entity caused by the bite of a spider of the genus Latrodectus spp. widely distributed throughout continental Chile. Generally, bites are associated with agricultural activities. The venom of spiders of the genus Latrodectus contains α-latrotoxin, a neurotoxin that acts at the presynaptic level of the autonomic nervous system, this way increases the release of acetylcholine. The clinical manifestations are non-specific and systemic symptoms occur in up to one-third of the cases. The diagnosis is clinical and depends on the history of being bitten by a spider with a compatible clinical presentation. The prognosis is favorable: a lethality between 0 and 6 percent has been reported. The clinical management is essentially symptomatic, mainly analgesic. We present the case of a 62-year-old man from Santa Cruz, VI region, working in a wheat farm. He attends the emergency service with chest pain associated with generalized myalgias and diaphoresis. On suspicion of latrodectism, neostigmine is administered with goodclinical response. (AU)

A total of 631 species of spiders have been reported in Korea. However, there are no spiders with noxious venom such as Latrodectus sp. Because of this, to date, no serious medical problems due to spider bites have been reported in Korea, and only two cases of spider bite were officially reported. However, as the number of earning spiders from other countries is increased, the number of cases of spider bite has also shown a recent increase. A 17-year-old man presented with numbness of both extremities after being bitten by a spider which he had as a pet. The spider is called an Indian ornamental tree spider (Poecilotheria regalis sp.), one kind of tarantula species. Herein, we report on a case of a spider bite by a tarantula.
