Résumé

This paper systematically analyzed the ancient monographs of acupuncture and moxibustion and comprehensive medical books from pre-Qin to 1911, and extracted the data according to the etiology and pathogenesis, treatment principles and methods, acupoint selection, needling and moxibustion, and taboos of needling and moxibustion. The pathogenesis of migraine in ancient books and documents is summarized as "the causes are diverse, and phlegm-dampness is the majority". For treatment, the features include "needling has a sequence, and the root and the branch should be treated separately" and "focusing on tonifying deficiency and drain excess". It is also obtained of the rich ideas of acupoints selection, extensive application records of moxibustion, unique application of bloodletting therapy and clear explanation of acupuncture and moxibustion taboos. All mentioned above is expected to enrich the ideas and methods of modern migraine treatment and improve the clinical effects.

The Korean Acupuncture and Moxibustion Medicine Society (KAMMS) celebrated its 50th anniversary with a commemoration ceremony and a Japan-Korea Symposium held in Seoul in November 2023. This report outlines the participation and highlights from the commemoration ceremony celebrating the 50th anniversary of KAMMS on November 11 (Saturday), along with insights shared during the Japan-Korea Symposium held as a special session within KAMMS' Fall Academic Conference 2023 on November 12 (Sunday). The summary includes presentations delivered by four speakers during the symposium. The Korean session focused on "Collaborative-Based Integrated Healthcare," discussing endeavors in integrated medicine involving Western medicine and Korean medicine. Meanwhile, the Japanese session titled "Introduction of Acupuncture Clinics in Japanese University Hospital" highlighted efforts in acupuncture and moxibustion education within the Tohoku University Hospital in Japan.

[Objective]To explore the inheritance status and academic characteristics of SHI's acupuncture and moxibustion school in northern Zhejiang,and to further enrich the connotation of Xiushui medical school.[Methods]Using the method of reviewing and researching the traditional medical history literature and modern literature,the origin,inheritance and academic characteristics of SHI's acupuncture and moxibustion were excavated,analyzed and sorted out.[Results]The culture of traditional Chinese medicine in northern Zhejiang is profound,there are many genres of acupuncture and moxibustion,and many famous doctors and scholars.The more influential schools of acupuncture and moxibustion are LING,SHI,YAN,SHENG and JIN,which have been passed down from generation to generation and innovated repeatedly.As one of the typical representatives,SHI's acupuncture has a long history,active inheritance and rich content.In clinical practice,great importance is attached to the spleen and stomach,and the treatment principle is to strengthen the spleen and benefit the stomach,warm and transport Yang Qi of middle-Jiao,and be good at applying warm acupuncture,so that Qi can be warmed and operated easier,and the acupuncture can be helped to regulate Qi.SHI's acupuncture also pays attention to the combination of acupuncture and moxibustion,and the combination of acupuncture and medicine to improve clinical efficacy.SHI's descendants are constantly innovating while inheriting and keeping the right,so as to enrich the disease spectrum,enrich the treatment methods and improve the clinical efficacy.[Conclusion]In the process of continuous inheritance and innovation,SHI's acupuncture has gradually formed a diagnosis and treatment system with acupuncture characteristics in northern Zhejiang,which can provide learning and reference for clinical practice.

By exploring the core ideas of the "Xingnao Kaiqiao" theory, analyzing the relationship between the "Xingnao Kaiqiao" theory and the TCM encephalopathy, and exploring the necessity of acupuncture for the treatment of TCM encephalopathy, in order to clarify how to diagnose and treat brain diseases in acupuncture and moxibustion. The "Xingnao Kaiqiao" is the core of the theory of the "Tiaoshen", the acupuncture of "Xingnao Kaiqiao" was based on the principle of "Xingnao","Xingshen" and "Tiaoshen", the idea of the "Tiaoshen" was inseparable from the diagnosis and treatment of the TCM encephalopathy. Based on the special features of acupuncture therapy and complexity and particularity of TCM encephalopathy, acupuncture and moxibustion treatment of encephalopathy was supposed to have their own unique system of syndrome differentiation and treatment. During the practice of acupuncture, the diagnosis and treatment of TCM encephalopathy should be based on "brain", "Xingnao Tiaoshen" as a general rule, adding and subtracting from the main points of the "Xingnao Kaiqiao", and the different brain diseases are matched with appropriate treatments.

Objective To analyze the selection rules and characteristics of acupuncture and moxibustion in the treatment of temporomandibular disorder(TMD)based on data mining techniques.Methods Computerized search of major databases such as CNKI,Wanfang,VIP,and CBM from January 2002 to June 2022.After screening and standardized treatment of acupuncture prescriptions,data analysis was performed to explore the selection and characteristics of core acupoints.Results A total of 92 papers were included,and 108 valid prescriptions and 36 acupuncture points were extracted with a total frequency of 555 times.Among them,11 high-frequency acupoints in the top 10 of the total frequency were used,including Xiaguan(ST7),Hegu(LI4),Jiache(ST6),Tinggong(SI19),Yifeng(SJ17),Ashi,Zusanli(ST36),Quanliao(SI18),Shangguan(GB3),Fengchi(GB20),Tinghui(GB2),with a total of 481 times,accounting for 85.79%of the total frequency.The association rules showed a very strong correlation and co-occurrence between Xiaguan(ST7),Hegu(LI4),Jiache(ST6),Tinggong(SI19),and the cluster analysis yielded 4 core clusters.Conclusion Acupuncture and moxibustion for treatment of this disease mainly follows the principle of"the main treatment should reach where the meridians pass",and the core combination of selected points is:Xiaguan(ST7)-Hegu(LI4)-Jiache(ST6)-Tinggong(SI19).

Objective To explore the effects of different acupoints,different target organs,and different interventions on acupoint efficacy based on ACU&MOX-DATA platform;To illustrate and visualize whether the above factors have the characteristics of"specific effect"or"common effect"of acupoint efficacy.Methods The multi-source heterogeneous data were integrated from the original omics data and public omics data.After standardization,differential gene analysis,disease pathology network analysis,and enrichment analysis were performed using Batch Search and Stimulation Mode modules in ACU&MOX-DATA platform under the conditions of different acupoints,different target organs,and different interventions.Results Under the same disease state and the same intervention,there were differences in effects among different acupoints;under the same disease state,the same acupoint and intervention,the responses produced by different target organs were not completely consistent;under the same disease state and acupoint,there were differences in effects among different intervention measures.Conclusion Based on the analysis of ACU&MOX-DATA platform,it is preliminary clear that acupoints,target organs,and interventions are the key factors affecting acupoint efficacy.Meanwhile,the above results have indicated that there are specific or common regulatory characteristics of acupoint efficacy.Applying ACU&MOX-DATA platform to analyze and visualize the critical scientific problems in the field of acupuncture and moxibustion can provide references for deepening acupoint cognition,guiding clinical acupoint selection,and improving clinical efficacy.

Childhood cerebral palsy is one of the main causes leading to disability in children,at present the specific treatments are lacking.As a traditional Chinese medicine treatment method,Acupuncture and moxibustion have been paid more and more attention in recent years because of its unique therapeutic effect.This paper reviews various methods of acupuncture and moxibustion treatment for cerebral palsy,and analyzes the commonalities and characteristics of these methods in depth.At the same time,this article discusses the influential factors and mechanism of acupuncture and moxibustion treatment of cerebral palsy,pointing out the questions and challenges in the field of acupuncture and moxibustion treatment of cerebral palsy.In order to better promote the study of acupuncture and moxibustion treatment of cerebral palsy,it is necessary to fur-ther strengthen the standardization of treatment methods and mechanism research in the future.

Résumé

The number of patients with novel coronavirus infections (COVID-19) continues to increase with Omicron now the predominant strain, even amidst the three-dense (close-in, dense, and close-out) avoidance and vaccination strategies. In collaboration with the local government, Tohoku University Hospital has provided a large-scale vaccination center, drive-through outpatient PCR testing clinic, outpatient assessment, support for management of residential care facilities for the mildly ill, support for elderly care facilities, and an antibody cocktail center, to minimize deaths due to COVID-19. In residential care facilities for the mildly ill and others in need of medical support, we established an information sharing and house call system. Furthermore, we developed DX in cooperation with a wide-area IT system and have so far continued to achieve zero fatalities in residential care facilities. In addition, the Japanese Society for Oriental Medicine has conducted clinical research using Kampo medicine for the prevention of disease onset, treatment in the acute stage, and treatment of prolonged symptoms, with results suggesting that Kampo medicine has some effect in reducing disease severity in the acute stage. The COVID-19 response is a long-term effort to update the knowledge and techniques in Western, traditional, and Future Medicine. The response and updating of combined knowledge and skills will continue over the longterm. It is also necessary to continue to disseminate and share information among various professions, and give back information in the field and make steady and continuous efforts among multiple professions.

The 2022 International Conference for Global Cooperation in Traditional Medicine was held in a hybrid meeting at the Novotel Ambassador Seoul Dongdaemun Hotel in Seoul on the 1st and 2nd of November 2022. This international conference was hosted by the Ministry of Health and Welfare of South Korea, organized by the National Institute for Korean Medicine Development, and supported by The Society of Korean Medicine. At this conference, a bilateral exchange of acupuncture and moxibustion therapies between Japan and Korea was held at the Republic of Korea-Japan Symposium I. The aims of the symposium were to combine conventional and traditional medical knowledge of frailty and sarcopenia and to share possible ideas to develop proper treatment for frailty and sarcopenia.

There is little common understanding among acupuncturists regarding the poor prognosis for peripheral facial paralysis, evaluation methods, precautions during treatment, including acupuncture and moxibustion treatment. Cooperation among relevant occupations is also a major issue. Currently, the treatment of paralysis is focused on how to minimize sequelae during the recovery process of paralysis in cases with poor prognosis. The goal is to prevent sequelae and improve patient quality of life (QOL). For that purpose, it is important to understand the guidelines for medical treatment of paralysis, and like other medical staff, the acupuncturist should provide appropriate medical examination, treatment, self-care guidance, etc., and cooperate with other medical specialists. We hope that this seminar will provide a common understanding of the pathophysiology and evaluation methods of paralysis, precautions for acupuncture and moxibustion treatment, and a consensus of clinical research and the future possibility of using acupuncture and moxibustion in the treatment of paralysis.

Résumé

This paper introduces the 2023 guidelines for the treatment of facial nerve palsy, which have been revised for the first time in 12 years, mainly related to acupuncture. The purpose of this study is to assist medical professionals in selecting appropriate evaluation and treatment for paralyzed patients. In this revision, the two clinical questions on acupuncture and moxibustion are recommended: (1) Is acupuncture effective for early recovery from paralysis (acute phase)? Both have been reorganized from "not recommended" to "weakly recommended". Regarding these results, several systematic reviews have been published in which acupuncture intervention can be expected to be more effective than conventional treatment (control) in recovering from paralysis in the acute phase and reducing sequelae such as contracture and stiffness in the chronic phase. Among them, it is suggested that prevention and reduction of aftereffects in particular contribute to the improvement of quality of life. Acupuncture and moxibustion treatment and guidance on self-care are also considered to be the role of the acupuncturist.

Résumé

Depressive disorder is manifested as emotional and physical abnormality. Theoretically, the governor vessel is distributed along the spine, related to the brain and communicated with five zang and six fu organs. It is the key meridian for understanding the various symptoms of depressive disorder. Depressive disorder is caused by dysfunction, stagnation or emptiness of the governor vessel, resulting in malnutrition of the brain. In clinical diagnosis and treatment, based on the theory of the governor vessel, the etiology and pathogenesis are analyzed in the patients with depressive disorder. In order to achieve harmonizing mutually the mental and physical conditions, acupuncture is delivered to adjust the spirit and physical state, moving cupping is to regulate the governor vessel, tuina manipulation is to promote meridians and collaterals and physical exercise is to coordinate the body and the spirit.

OBJECTIVE@#To assess the methodological quality, report quality and evidence quality of the Meta-analysis and systematic reviews of acupuncture and moxibustion for children with cerebral palsy, aiming to provide decision-making basis for clinical treatment.@*METHODS@#The systematic reviews and Meta-analysis of acupuncture and moxibustion for children with cerebral palsy were searched in CNKI, Wanfang, VIP, SinoMed, Cochrane Library, PubMed and EMbase. The retrieval time was from the database establishment to June 30th, 2022. AMSTAR 2 (a measurement tool to assess systematic reviews) was used to evaluate the methodological quality, and PRISMA (preferred reporting items for systematic reviews and Meta-analyses) was used to evaluate the report quality, and GRADE was used to evaluate the quality of evidence.@*RESULTS@#A total of 14 systematic reviews were included, including 37 primary outcome indexes. According to AMSTAR 2 evaluation results, there were 4 low quality studies, 10 very low quality studies, and low scores on items 2, 4, 7, 10 and 16. PRISMA scores ranged from 15 to 25, and the main reporting problems reflected in structured abstracts, program and registration, retrieval, and funding sources, etc. According to the GRADE classification results, there were 3 high quality evidences, 7 medium quality evidences, 10 low quality evidences and 17 very low quality evidences. The main downgrading factors were limitations, imprecision and publication bias.@*CONCLUSION@#Acupuncture and moxibustion has a certain effect for cerebral palsy in children, but the quality of methodology, reporting and evidence in the included literature is poor, and the comparison of curative effect between different acupuncture and moxibustion methods is unclear.

Résumé

By summarizing and exploring the theoretic connotation, key of functions and effect mechanism of acupoint compatibility, the effect of acupoint compatibility is concluded as the increase of "effect value" and the expansion of "effect domain". The increase of "effect value" is the concrete embodiment by the value of medical assessment scale, the value of objective index detection in clinical trial and the value of index detection in experiment research. The expansion of "effect domain" is the increase of effect target and the extension of effect scope. The paper interprets the scientific connotation of acupoint compatibility therapy from a new perspective, and emphasizes the innovative approaches to research while bringing forth new ideas on the research method. It is anticipated that a novel breakthrough can be achieved in the study of acupoint compatibility and the improvement of acupuncture-moxibustion efficacy.

Résumé

To enhance the clinical applicability of guidelines and provide more effective guidance for clinical practice, a clinical value assessment was conducted during the development of the World Federation of Acupuncture-Moxibustion Societies (WFAS) Clinical Practice Guideline of Acupuncture and Moxibustion for Migraine, which involved the evaluation of 59 acupuncture and moxibustion treatment protocols from randomized controlled trials (RCTs). This article introduced the methodology, content and results of the clinical value assessment of RCT-based acupuncture and moxibustion treatment protocols, which involved the integration of historical and contemporary medical evidence and expert consensus. It served as a methodological reference for the future development of acupuncture and moxibustion clinical practice guidelines.

It systematically examined the advantages of using the modified Delphi method in the formation of recommendations for acupuncture and moxibustion clinical practice guideline, with the aim of providing reference for the development of acupuncture and moxibustion guidelines in the same field.

Résumé

The latest guideline about ulcerative colitis (UC) clinical practice stresses that mucosal healing, rather than anti-inflammation, is the main target in UC clinical management. Current mucosal dysfunction mainly closely relates to the endoscopic intestinal wall (mechanical barrier) injury with the imbalance between intestinal epithelial cells (IECs) regeneration and death, as well as tight junction (TJ) dysfunction. It is suggested that biological barrier (gut microbiota), chemical barrier (mucus protein layer, MUC) and immune barrier (immune cells) all take part in the imbalance, leading to mechanical barrier injury. Lots of experimental studies reported that acupuncture and moxibustion on UC recovery by adjusting the gut microbiota, MUC and immune cells on multiple targets and pathways, which contributes to the balance of IEC regeneration and death, as well as TJ structure recovery in animals. Moreover, the validity and superiority of acupuncture and moxibustion were also demonstrated in clinic. This study aims to review the achievements of acupuncture and moxibustion on mucosal healing and analyse the underlying mechanisms.

Résumé

Immunotherapy targeting immune checkpoint molecules has emerged as a key approach in cancer treatment, representing the forefront of antitumor research. However, studies on immune checkpoint molecules have mainly focused on targeted therapies. Chinese medicine (CM) research as a complementary medicine has revealed that immune checkpoint molecules also undergo disease-specific changes in the context of autoimmune diseases. This review article presents a comprehensive analysis of CM studies on immune checkpoint molecules in the last 5 years, with a focus on their role in different diseases and treatment modalities. CM research predominantly utilizes oral administration of herbal plant extracts or acupuncture techniques, which stimulate the immune system by activating specific acupoints through temperature and needling. In this study, we analyzed the modulation and mechanisms of immune checkpoint molecules associated with different coinhibitory and costimulatory molecules, and reviewed the immune functions of related molecules and CM studies in treating autoimmune diseases and tumors. By summarizing the characteristics and research value of CM in regulating immune checkpoint molecules, this review aims to provide a useful reference for future studies in this field.

Résumé

The efficacy on chronic obstructive pulmonary disease (COPD) at stable stage treated with different methods of acupuncture and moxibustion was evaluated using network Meta-analysis method. The articles of the randomized controlled trial (RCT) on stable COPD treated with acupuncture and moxibustion were searched electronically in CNKI, Wanfang, VIP, SinoMed, PubMed, EMbase, Web of Science and Cochrane library. The search was conducted from the inception of the databases to March 20th, 2022. Data analysis was performed using R4.1.1, Stata16.0 and RevMan5.3 softwares. A total of 48 RCTs were included, involving 15 kinds of acupuncture and moxibustion interventions and a sample size of 3 900 cases. The results of network Meta-analysis showed that: ① For the forced expiratory volume in one second predicted (FEV1%), both the governor vessel moxibustion combined with conventional treatment (G+C therapy) and the yang-supplementing moxibustion combined with conventional treatment (Y+C therapy) obtained the better effect than that of the conventional treatment (P<0.05), and the G+C therapy was more effective compared with the thread-embedding therapy combined with conventional treatment (E+C therapy) and warm needling (P<0.05). ② Concerning to COPD assessment test (CAT) score, the results indicated that the Y+C therapy, and the mild moxibustion combined with conventional treatment (M+C therapy) were more effective when compared with the conventional treatment (P<0.05), and the effect of the Y+C therapy was better than that of the E+C therapy (P<0.05). ③ Regarding six-minute walking distance (6MWD), the effect of acupuncture combined with conventional treatment (A+C therapy) was better than that of either the E+C therapy or the conventional treatment (P<0.05). The effect of the G+C therapy was optimal for improving FEV1%, the Y+C therapy obtained the best effect for improving CAT score, and A+C therapy was the most effective for improving 6MWD. Due to the limitation of the quality and quantity of included studies, this conclusion needs to be further verified through high-quality RCT.
