To summarize Professor AI Rudi's experience in the staged external treatment of eczema by traditional Chinese medicine (TCM). It is believed that dampness is the key pathological factor of eczema, and the treatment for dampness should be carried out throughout the whole process, with the leading idea of “three-stage treatment, dampness as the root, and treatment according to the symptoms”. The preparation and formulas of the external treatment can be applied according to the pathogenesis and lesion characteristics of each stage. In acute stage with wind-dampness and heat in the skin as the main mechanism, the treatment is to clear heat and astringe, resolve toxins and relieve itching, and the preparation is mostly solution and lotion, and the formula could be Yangzheng Xi Formula (痒症洗方), Jingfang Kushen Decoction (荆防苦参汤), Kuding Erhuang Decoction (苦丁二黄汤), and Xianglian Jinhuang Powder (香连金黄散). In subacute stage with phlegm-dampness and heat as the main mechanism, the treatment is to remove dampness and turbidity, clean up the residual heat; the preparation is mostly ointment, and the formula include Shehuang Ointment (蛇黄软膏), Huanglian Ointment (黄连膏), and Zihuan Diding Ointment (紫花地丁软膏). In chronic stage with phlegm-dampness stagnation as the main mechanism, the treatment is to remove dampness and eliminate phlegm, remove blood stasis and stop itching, and the formula could be Zhiyang soft Ointment (止痒软膏), Runji Ointment (润肌膏), Yufu Ointment (愈肤膏); jojoba oil and olive oil are often used as external moisturiser for daily care.

Hyperplasia of mammary glands is a benign breast disease with disordered breast structure. Nowadays, the incidence rate of breast hyperplasia in women is increasing year by year, and the etiology is related to the imbalance of estrogen and progesterone in the body. The symptoms include breast pain, breast nodules, or nipple discharge, which can develop into breast cancer in the context of psychological pressure. Therefore, it is timely and effectively necessary for people to treat the symptoms. At present, traditional Chinese medicine(TCM) often treats breast hyperplasia by oral drug, external application, acupuncture, moxibustion, and massage, while western medicine often uses hormone therapy or surgery. TCM can regulate hormone levels to treat breast hyperplasia. Acupuncture, moxibustion, and other methods can stimulate acupoints to reduce breast lumps. However, since TCM is easy to produce hepatorenal toxicity after long-term use and simple external treatment is slow to take effect, rapid and effective treatment is difficult to be achieved. Although western medicine can inhibit the disease, it is easy to produce toxic and side effects if taken for a long time. In addition, surgery can only remove the focus and the recurrence rate is high. Some studies have found that the combination of oral and external use of TCM compounds has a significant effect, with mild toxic and side effects, few adverse reactions, and a low recurrence rate. Based on the relevant literature in recent years, this article reviewed the combination of oral and external treatment of TCM in the treatment of hyperplasia of mammary glands, discussed the effectiveness, clinical evaluation indexes, and mechanism, and pointed out the existing shortcomings to explore a comprehensive therapy worthy of clinical application.

Objective To investigate the molecular mechanism of YingZhongXiao Ointment(YZXO)in the treatment of goiter based on network pharmacology and experimental validation.Methods TCMSP,TCMID,BATMAN-TCM,TCM-MESH,were used to screen the effective active ingredients and targets of YZXO.GeneCards,OMIM,Pharm GKB and Drug Bank databases were used to obtain goiter disease targets.After mapping the drug targets to the disease targets,the intersecting targets were screened,and then the intersecting targets were used to construct a PPI network and screen the core targets,which were subjected to GO and KEGG enrichment analysis.Results In this study,227 active ingredients,314 targets and 847 goiter targets were screened,and 78 potential targets for goiter treatment were obtained after mapping.449 biological processes were obtained from GO enrichment analysis,and 118 pathways were identified from KEGG enrichment analysis,including PI3K-Akt,mitogen-activated protein kinase(MAPK),tumor necrosis factor(TNF)and other signalling pathways.The results of animal experiments showed that compared with the model group,the gene expression of PI3K/Akt/m TOR signalling pathway in thyroid tissue of rats in each dose group and LT4 group was decreased(P<0.01 or P<0.05),p-PI3K/PI3K、p-Akt/Akt was decreased(P<0.01),the protein content of VEGF and PCNA was decreased(P<0.01 or P<0.05),and the protein content of Fas and FasL was increased(P<0.05 or P<0.01).Conclusion This study suggests that the mechanism of YZXO treated goiter by Resolving the Fire may be related to downregulation of PI3K/Akt/m TOR signalling pathway,inhibition of abnormal thyroid cell proliferation and promotion of thyroid cell apoptosis.

Normal human blood pressure has a unique "dipper type" rhythm, but various pathological factors will lead to abnormal nighttime blood pressure decline, seriously damage the target organs such as the heart, brain and kidneys, and significantly increase the incidence of cardiovascular events. Therefore, it is necessary to pay sufficient attention to rhythm disorder of blood pressure and intervene early. Modern Traditional Chinese Medicine (TCM) mostly understands the changes of rhythm of blood pressure from the view of the unity of nature and man, yin-yang theory and the theory of meridian flow, and study the syndrome distribution and TCM constitution law of rhythm disorder of blood pressure by combining TCM syndrome differentiation method and TCM constitution theory. TCM treatment can restore blood pressure rhythm and improve clinical symptoms and sleep quality of patients.

OBJECTIVE@#To explore the clinical effect of Qufu Shengji ointment(QFSJO) in promoting the wound healing after trauma.@*METHODS@#From January 2014 to June 2018, 60 patients with soft tissue injury, skin defect and wound infection caused by violent trauma were admitted, including 32 males and 28 females, aged from 18 to 65 years, with an average age of 41.3 years. Among them, 30 patients were treated with QFSJO (QFSJO group) and 30 patients were treated with normal saline iodophor (control group). The reduction rate of wound area, the days of decayed flesh, the time of new epithelium and the recovery rate of 28 days after dressing change were compared between the two groups.@*RESULTS@#In the QFSJO group, after using large dose of QFSJO, the pus of the wound increased, the granulation grew, and the new epithelium appeared on the edge of the wound, showing a rapid healing phenomenon. The wound healing rate of QFSJO group was higher than that of the control group at all time points, and the time of decaying flesh and new epithelium appeared in QFSJO group was earlier than that of the control group. The recovery rate of QFSJO group was significantly higher than that of the control group(<0.05). All the patients were followed up, and the duration ranged form 6 to 12 months, with an average of 9.4 months. The exposed areas of bone and teadon were covered well. The vital signs of the two groups were stable and no adverse reactions occurred.@*CONCLUSIONS@#QFSJO can promote the growth of granulation tissue, promote the production of new skin, and accelerate the healing of infectious wound after trauma.

Siddha system of medicine, one of the ancient, traditional Indian systems of medicine has unique diagnostic methods, therapeutics and treatment procedures. It has a vast range of external modalities of treatment for health management. This external management is classified into 32 types. They include minor surgical procedures and treatment procedures. These therapies are used both as mainstream and supportive therapies � both curative and prophylactic. Most of the therapies are aimed to maintain the equilibrium of the three humours (Vatham, Pitham, Kabam). Among them Nasiyam (Nasal instillation) is one of the external therapies which is a non-invasive procedure. Nasiyam is the process by which the drug is administered through nostrils. This is the treatment procedure to balance the Thirithodam in its normal level. This paper documents the efficacy of Nasiyam, methods of application, shelf life, effect of Nasiyam in treating various diseases, the list of single drug and compound drug formulations that can be used as Nasiyam, Indications and contraindications are discussed in detail. Nasiyam therapy is used to treat the diseases of vitiated Kabam such as migraine, sinusitis, bronchial asthma, nasal polyp etc.

Introduction: Patola is an important plant drug used in many Ayurvedic medicinal preparations. The source plant of Patola is Trichosanthes cucumerina var. cucumerina, belonging to Cucurbitaceae family, found distributed (growing wild) throughout India especially in Bengal, Gujarat, Konkan, Deccan and Kerala. It is an extensively climbing annual herb with slightly downy or pubescent slender five angular 杅urrowed stems. It is found climbing over small trees, hedges and suitable supports. The present work is taken up with an objective to standardize Patola with detailed physical and preliminary phytochemical standards. Methods: Samples of Trichosanthes cucumerina var. cucumerina (Patola) was collected from Thellakom, Kottayam district. The plant was properly identified and authenticated by Dept of Botany, University of Kerala. The sample was shade dried and stored in an air tight container. Physicochemical, phytochemical, chromatographic and heavy metal analysis was carried out as per ICMR procedures. Result: On physicochemical evaluation water soluble extractive value was 9.07+ 0.012 and alcohol soluble extractive value was 18.72+ 0.027. Heavy metals copper, cadmium, iron and lead levels were within normal limits. HPTLC was carried out with Toluene: Ethyl acetate: Formic acid as solvent system and 3 spots were obtained. Conclusion: Patola, Trichosanthes cucumerina var. cucumerina, Phytochemicals, HPTLC, AAS.

Pancreatic pseudocyst (PPC) is one of the common peri-pancreatic complications of severe acute pancreatitis (SAP), and it belongs to the category of Zhengjiajiju in traditional Chinese medicine (TCM). Clinically, skilled use of surgical minimally invasive means combined with TCM treatment has remarkable advantages in promoting the rate of cystic absorption, reducing the surgical intervention rate, lowering the incidence of complications, etc. TCM syndrome differentiation treatment for SAP complicated with PPC can start from Shaoyang meridian lesions, and the treatment of Shaoyang liver and gallbladder visceral disease can be taken as an example. The clinical therapeutic result is significant by using soothing liver and normalizing gallbladder function, removing stagnancy and obstruction of the fu-organs and invigorating blood circulation and softening hard mass as the basic TCM rules to dialectically use the PPC basic prescription, locally accompanying with external therapy. The discussion of this article focuses on the experience of using TCM syndrome differentiation for treatment of patients with PPC induced by SAP.

Objective To observe the efficacy of Tongmai Acupoint Plaster combined with Alprostadil injection for treatment of lower extremity arteriosclerosis obliterans (ASO). Methods Totally 180 cases of ASO Ⅱ(intermittent claudication period) patients were randomly divided into acupoint sticking group, combination group, Western medicine group, with 60 cases in each group. All groups quit smoking, had diet control, proper exercise, control of blood sugar, stop hypolipidemic and anti-coagulation medicine. The acupoint sticking group was treated with Tongmai Acupoint Plaster, each selected with the efficacy of treatment of lower limbs paralysis acupoint 10-12, alternating dressing; dressing was changed 1 time a day, 40 day as a treatment course. Western medicine group was given Alprostadil injection 10 μg, with 10 mL of normal saline, intravenous infusion, once a day, 15 d as a treatment course, for two courses, with 10 d as interval. Combination group was treated with Tongmai Acupoint Plaster combined with Alprostadil injection, the same as the application method above. The clinical efficacy, the scores of symptoms and signs, the ankle brachial index, blood lipid, the peak value of blood flow in the tibial anterior,posterior tibial, and dorsalis pedis arteries were observed and compared between the 2 groups. Results The total effective rate was 78.33% (47/60) in the acupoint sticking group, 80.00% (48/60) in the Western medicine group, and 93.33%(56/60) in the combination group, with the combination group better than the acupoint sticking group and Western medicine group (P0.05). There was statistical significance in scores of symptoms and signs, ankle brachial index before and after treatment in the three groups (P0.05). There was statistical significance in blood lipids before and after treatment in acupoint sticking group and combination group (P<0.05). There was statistical significance in tibial anterior, posterior tibial, and dorsalis pedis artery peak before and after treatment in the three groups (P<0.05, P<0.01). Conclusion Tongmai Acupoint Plaster has good efficacy in the treatment of ASO, equivalent with the efficacy of Alprostadil injection, which can regulate blood lipids and improve arterial blood flow. Ther combination of them has better efficacy.

Objective To compare the therapeutic effect of conventional treatment and conventional treatment plus external application of Shuangbai Powder for patients with wounded limb injured by venomous snake. Methods One hundred patients bitten by venomous snake were randomized into treatment group and control group, 50 cases in each group. The control group was given conventional treatment including repeatedly washing the wound with hydrogen peroxide, debriding the wound, letting blood and draining toxicity, local blocking with chymotrypsin, wet packing the wounded limb with magnesium sulfate, and injection with anti-venomous serum, tetanus antitoxin, antibiotics, furosemide and energy mixture. The treatment group was given external application of Shuangbai Powder on the basis of the treatment for the control group. Both groups were given the comprehensive nursing of psychological nursing, wound nursing, dietary nursing, defecation nursing and functional nursing. The swelling-subsiding time for the wounded limb and pain scores of visual analogue scale (VAS) in the two groups were compared. Results(1) After treatment, swelling-subsiding time for the wounded limb of the treatment group was shorter than that of the control group, the difference being statistically significant (P < 0.05). (2) After treatment for 4 days and at the end of the treatment, VAS scores of the two groups were decreased compared with those before treatment(P < 0.05), and the decrease of VAS scores in the treatment group after treatment for 4 days was superior to that in the control group (P < 0.05). Conclusion The conventional treatment plus external application of Shuangbai Powder is an effective therapy for patients with wounded limb injured by venomous snake by shortening swelling-subsiding time and relieving pain for the wounded limb .

As a case, the effect is better. Most literaturewasclinical research and the most commonly used for external therapywasacupuncture.ConclusionThe research and clinical work of essential hypertension treatment with external therapy of traditional Chinese medicinewerepaid more close attention in recent years, but therewerestill some problems that need to be solvedto form a viable, effective treatment system.

Objective To observe the therapeutic effect of external application of herbal medicine for ankylosing spondylitis(AS).Methods Two hundred and seventy-nine AS patients were randomized into group A(N=175)and group B(N=104).The two groups received oral use of sulfasalazine,and group A additionally received external application of herbal medical ointment(mainly composed of Pheretima,Squama Manis,Rhizoma Dioscoreae Nipponicae,Radix Rehmanniae Preparata,Rhizoma Cibotii,Cortex Eucommiae,Radix Dipsaci)on 3~4 pairs of bilateral acupoints of Jiaji,dressing change once every three days,for continuous 6 months.After treatment,the therapeutic effect was evaluated.The primary outcome measures of health general score(BAS-G),disease activity index(BASDAI),function index(BASFI),Likert four-grade scores of general pain and rachialgia,duration of morning stiffness,general assessment by physician were observed before and after treatment.The secondary outcome measures of occiput to wall distance,finger to floor distance,chest expansion,Schober's test,spinal movement,"4"shape test were compared.The changes of erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),complements 3 and 4(C3 and C4)as well as the adverse actions were also observed.Results After treatment,46 patients were clinically relieved,65 markedly effective,52 effective,12 ineffective,and the total effective rate was 93.14% in group A;11 patients were clinically relieved,27 markedly effective,48 effective,18 ineffective,and the total effective rate was 82.61% in group B.The difference of the therapeutic effect was significant between the two groups(P
