Introducción: Los niños con quemaduras tienen un manejo complejo por lo cual el uso de la membrana amniótica como una cobertura temporal es un método válido para su implementación. Objetivos: Demostrar el uso de la membrana amniótica como cobertura temporal en pacientes pediátricos con quemaduras de segundo grado superficiales y profundas internados en el Centro Nacional de Quemaduras y Cirugías Reconstructivas (CENQUER), en julio y diciembre del 2021. Materiales y métodos: Estudio descriptivo de tipo retrospectivo mediante la revisión de casos clínicos-quirúrgicos de los pacientes pediátricos que ingresaron por diagnóstico de quemadura de segundo grado superficial y profundo en el CENQUER, durante el período comprendido entre julio y diciembre del 2021, donde se aplicó́ membrana amniótica humana. Resultados: Población de 14 pacientes, 11 provenían del departamento central y 3 del interior del país, con un promedio de 13% (R: 3%-25,5%) de superficie corporal quemada (SCQ) todos con quemaduras en promedio 7% tipo AB y 6% tipo ABB, 7 ingresaron con una gravedad moderada, 4 graves y 3 leves; 7 del sexo masculino y 7 del femenino , con un rango de edades entre 8 meses y 11 años cuya media fue de 4 años y una mediana de 7 años, con respecto al peso un promedio de 18 kg al ingreso % (R: 9-35 kg), las causas de las lesiones fueron 12 por escaldadura por agua caliente (85 %) y 2 por escaldadura con comida; 10 pacientes presentaron internación en sala y en la Unidad de terapia intensiva Pediátrica (UTIP) 4 pacientes, el promedio de tiempo transcurrido desde la quemadura a la consulta fueron de 7 hs (R: 1-48 hs), las regiones anatómicas que se cubrieron fueron: 4 pacientes en miembros superiores y tórax anterior (28 %), 2 exclusivamente en tórax anterior (16 %), 4 solo en miembros superiores (28 %), 4 en miembros inferiores (28 %), a los que se les colocó amnios por única ocasión en 10 pacientes a las 24 hs de la internación y en 4 pacientes a las 48 hs de internación, se utilizó antobioticoterapia parenteral en 3 pacientes y 11 pacientes no necesitaron antibioticoterapia parenteral, no se presentaron complicaciones en ninguno de los pacientes y tuvieron un promedio de internación de 8 días (R: 3- 16 días). Conclusión: El uso de membrana amniótica humana en pacientes quemados son favorables para los pacientes pediátricos; como demuestra en el estudio el menor uso de antibioticoterapia parenteral por la cobertura rápida de las heridas causadas por las quemaduras y el menor número de días de internación.


Introduction: Children with burns have a complex management for which the use of the amniotic membrane as a temporary cover is a valid method for its implementation. Objectives: Demonstrate the use of the amniotic membrane as temporary coverage in pediatric patients with superficial and deep second-degree burns admitted to the National Center for Burns and Reconstructive Surgery (CENQUER), in July and December 2021. Materials and methods: Descriptive retrospective study through the review of clinical-surgical cases of pediatric patients who were admitted due to a diagnosis of superficial and deep second-degree burns in the CENQUER, during the period between July and December 2021, where human amniotic membrane was applied. Results: Population of 14 patients, 11 came from the central department and 3 from the interior of the country, with an average of 13% (R: 3%-25.5%) of burned body surface area (SCQ), all with average 7% AB type burns. and 6% type ABB, 7 admitted with moderate severity, 4 severe and 3 mild; 7 males and 7 females, with an age range between 8 months and 11 years, whose average was 4 years and a median of 7 years, with respect to weight an average of 18 kg at admission % (R: 9- 35 kg), the causes of the injuries were 12 due to scalding due to hot water (85%) and 2 due to scalding with food; 10 patients were hospitalized in the ward and in the Pediatric Intensive Care Unit (PICU) 4 patients, the average time elapsed from the burn to the consultation was 7 hours (R: 1-48 hours), the anatomical regions that were covered were: 4 patients in the upper limbs and anterior thorax (28%), 2 exclusively in the anterior thorax (16%), 4 only in the upper limbs (28%), 4 in the lower limbs (28%), who received amnios for a single occasion in 10 patients at 24 hours of hospitalization and in 4 patients at 48 hours of hospitalization, parenteral antibiotic therapy was used in 3 patients and 11 patients did not need parenteral antibiotic therapy, there were no complications in any of the patients and they had an average hospital stay of 8 days (R: 3-16 days). Conclusion: The use of human amniotic membrane in burn patients is favorable for pediatric patients; As shown in the study, the lower use of parenteral antibiotic therapy due to the rapid coverage of wounds caused by burns and the lower number of days of hospitalization.

The aim of our study was to investigate the differential effects of dexamethasone (DXM) and hydrocortisone (HCS) on somatic growth and postnatal lung development in a rat model of bronchopulmonary dysplasia (BPD). A rat model of BPD was induced by administering intra-amniotic lipopolysaccharide (LPS) and postnatal hyperoxia. The rats were treated with a 6-day (D1-D6) tapering course of DXM (starting dose 0.5 mg/kg/day), HCS (starting dose 2 mg/kg/day), or an equivalent volume of normal saline. DXM treatment in a rat model of BPD induced by LPS and hyperoxia was also associated with a more profound weight loss compared to control and LPS + O2 groups not exposed to corticosteroid, whereas HCS treatment affected body weight only slightly. Examination of lung morphology showed worse mean cord length in both LPS + O2 + DXM and LPS + O2 + HCS groups as compared to the LPS + O2 alone group, and the LPS + O2 + DXM group had thicker alveolar walls than the LPS + O2 group at day 14. The HCS treatment was not significantly associated with aberrant alveolar wall thickening and retarded somatic growth. The use of postnatal DXM or HCS in a rat model of BPD induced by intra-amniotic LPS and postnatal hyperoxia appeared detrimental to lung growth, but there was less effect in the case of HCS. These findings suggest that effect of HCS on somatic growth and pulmonary outcome may be better tolerated in neonates for preventing and/or treating BPD.

BACKGROUND: Skin disease leading to extensive blistering and loss of skin is associated with a characteristic smell. Odor can cause physiologic disturbances such as increase in heart rate and respiratory rate. It can also cause nausea and vomiting and is disturbing to bystanders. AIMS: To test odor reducing capability of activated charcoal. METHODS: In this blinded experimental study we used putrefied amniotic membrane to produce odor and studied the effectiveness of activated charcoal and soda-bi-carbonate to reduce odor. RESULTS: Statistical analysis with Kruskal Wall's Chi Square Test and Man Whitney U test showed significant reduction of odor using activated charcoal by itself or along with soda-bi-carbonate. CONCLUSION: We recommend the usage of activated charcoal with/without soda bicarbonate as an inexpensive practical measure to reduce foul odor associated with extensive skin loss.

OBJETIVO: Comparar, por microscopia eletrônica, a integridade anatômica e a presença de fatores de crescimento e citocinas da membrana amniótica preservada com glicerol/MEM (1:1) e dimetilsulfóxido puro. MÉTODOS: As membranas amnióticas preservadas em glicerol/MEM (1:1) ou dimetilsulfóxido puro foram processadas para microscopia eletrônica de transmissão e varredura. Como controle, membrana amniótica fresca foi imediatamente fixada após coleta e processada para microscopia eletrônica. As citocinas e os fatores de crescimento avaliados foram: TGF-beta- fator transformador de crescimento beta; TGF-beta ativ- fator transformador de crescimento beta ativado; EGF- fator recombinante de crescimento epitelial humano; FGF-4- fator de crescimento fibroblástico 4; FGF-beta- fator de crescimento fibroblástico básico; IL-4- interleucina 4; PGE2- prostaglandina E2; IL-10- interleucina 10; KGF- fator de crescimento de queratinócito; HGF- fator de crescimento de hepatócito. RESULTADOS: As membranas amnióticas do grupo controle apresentavam epitélio íntegro, com microvilos na superfície e complexos juncionais entre as células e a membrana basal. As membranas amnióticas preservadas em glicerol/MEM tinham aspecto semelhante às do controle, com maior altura das células epiteliais. Já as membranas amnióticas preservadas em dimetilsulfóxido mostraram redução das junções intercelulares e destacamento do epitélio da membrana basal. As citocinas e fatores de crescimento não apresentaram diferenças entre os grupos, exceto FGF-4, FGF-beta, PGE2 e KGF. CONCLUSÕES: A membrana amniótica preservada em meio glicerol/MEM apresentou melhor integridade tecidual, com menor desprendimento do epitélio da membrana basal, em comparação com a preservada no dimetilsulfóxido puro. Os fatores de crescimento e citocinas estavam, em sua maior parte, preservados com as duas técnicas de preservação.


PURPOSE: To compare the anatomical structure and the presence of growth factors and cytokines of amniotic membrane preserved in glycerol/MEM (1:1) or undiluted dimethyl sulfoxide through electron microscopy. METHODS: Amniotic membrane preserved in glycerol/MEM (1:1) or undiluted dimethyl sulfoxide were processed for transmission and scaning electron microscopy. As control, freshly collected amniotic membrane was fixed and processed for electron microscopy. The cytokines and growth factors assessed were: TGF-beta (transforming growth factor beta); TGF-b activ (activated transforming growth factor beta); EGF (epidermal growth factor); FGF-4 (fibroblast growth factor 4); bFGF (basic fibroblast growth factor); IL-4 (interleukin 4); PGE2 (prostaglandin E2); IL-10 (interleukin 10); KGF (keratinocyte growth factor); HGF (hepatocyte growth factor). RESULTS: Amniotic membrane from the control group showed intact epithelium, with surface microvilli and junctional complexes between the cells and the basal membrane. Glycerol/MEM preserved amniotic membrane had similar aspect to the control, with higher epithelial cells. Those amniotic membranes preserved in dimethyl sulfoxide disclosed less intercellular junction and detachment of the epithelium from the basal membrane. The cytokines and growth factors did not disclose significant differences, except for FGF-4, bFGF, PGE2 and KGF. CONCLUSIONS: Amniotic membrane preserved in glycerol/MEM showed a better tissue structure, with less detachment of the epithelium from the basal membrane, in comparison to undiluted dimethyl sulfoxide. The majority of the growth factors and cytokines were kept with both techniques of preservation.

OBJETIVO: Avaliar as características morfológicas da membrana amniótica desepitelizada por diferentes técnicas. MÉTODOS: A membrana amniótica humana foi coletada no momento do parto, fixada em concentrações crescentes de glicerol (0-50 por cento em DMEM) e preservada a 80°C até a hora de ser usada. O estudo consistiu de 4 grupos: epitélio intacto (controle) e membranas desepitelizadas pela tripsina (2 mg/mL a 1:250), dispase (1,2 U/mL em solução salina balanceada de Hank livre de Mg2+ e Ca2+) e ácido etilenodiaminotetra-acético (EDTA), 0,02 por cento). As amostras foram submetidas à análise por microscopia eletrônica (de varredura e de transmissão). RESULTADOS: A microscopia eletrônica de varredura mostrou epitélio intacto no grupo controle e sua ausência nas membranas amnióticas desepitelizadas pela tripsina e pela dispase. Naquelas tratadas com o ácido etilenodiaminotetra-acético, havia áreas com e sem epitélio. Quando avaliadas pela microscopia eletrônica de transmissão, o epitélio estava intacto e firmemente aderido à membrana basal através de hemidesmossomos nos grupos controle e em parte do ácido etilenodiaminotetra-acético. Havia apenas fibras colágenas nas membranas tratadas com dispase e tripsina. CONCLUSÕES: O tratamento da membrana amniótica com tripsina e dispase pode causar completa retirada do epitélio e da membrana basal, ao passo que o ácido etileno- diaminotetra-acético pode preservar áreas com epitélio intacto e parcialmente destruir a membrana basal em outras.


PURPOSE: To evaluate the morphological features of the amniotic membrane denuded by different techniques. METHODS: Human amniotic membrane was collected at the time of delivery, fixed in increasing concentrations of glycerol (0-50 percent in DMEM) and preserved at -80°C until the time of use. The study consisted of 4 groups: intact epithelium (control) and denuded by trypsin (2 mg/mL at 1:250), dispase (1.2 U/mL in Mg2+ and Ca2+ free Hank's balanced salt solution) or ethylenediaminetetraacetic acid (EDTA), 0.02 percent. Specimens were submitted to electron (scanning and transmission) microscopy analysis. RESULTS: Scanning electron microscopy disclosed intact epithelium in the control group and its absence in the amniotic membranes denuded by trypsin and dispase. In those denuded by ethylenediaminetetraacetic acid there were areas with and without epithelium. When assessed by transmission electron microscopy, the epithelium was intact and firmly adhered to the basement membrane by hemidesmossomes in controls and in parts of ethylenediaminetetraacetic acid group. There were only collagen fibers in the dispase- and trypsin-treated groups. CONCLUSIONS: Trypsin and dispase treatment of the amniotic membrane may cause complete denuding of the epithelium and basement membrane whereas ethylenediaminetetraacetic acid may leave some intact epithelium-areas and partially destroy the basement membrane in others.

RÉSUMÉ

OBJECTIVE: To characterize the effect of IL-1 beta, on connective tissue metabolism in a human chorioamniotic membrane tissue culture (CAM). TYPE OF STUDY: Experimental, in an in vitro model. MATERIAL AND METHODS: CAM explants obtained from cesarean sections were cultured. The presence of local infection was excluded by microbiological methods. An XTT viability essay of the explants was carried out. Explants were stimulated with different doses of IL-1 beta within a 0-10 ng/mL range. After the stimulation, protein content was measured, MMP-9 production was determined by zymography, and each explant was divided in two parts: one was used for collagen measurement and the other analyzed by electronic microscopy. RESULTS: CAMs kept adequate viability and functionality. IL-1 beta stimulation produced an increase in the amount of MMP-9 expressed, as determined by the zymography method with a maximum effect 36 hours after stimulation. Collagen content decreased in a progressive manner after IL-1 beta stimulation and reached its minimum after 36 hours. The characteristic pattern of collagen fibers gradually lost its organization, and could not be observed any more after 36 hours. CONCLUSIONS: The information presented here allows us to conclude that IL-1 beta is capable of inducing an enzymatic expression affecting connective tissue, thus confirming its participation in membrane degradation processes under inflammatory conditions.
