BACKGROUND: The urea breath test (UBT-13C) is a non-invasive technique that allows the diagnosis and confirmation of eradication of Helicobacter pylori infection. Aim: To evaluate H. pylori positivity and values of UBT-13C among infected Chilean children and adults, and to analyze its variation in relation to sex, nutritional status, and age of the patients. Material and Methods: Retrospective study of 1141 patients aged 6 to 94 years, with an indication for a UBT-13C either for diagnosis or for confirmation of eradication of H. pylori infection. 13C enrichment was measured using an infrared spectrometer calculating the delta 13C values before and after the ingestion of 13C marked urea. The clinical data of the patients were obtained at the time of the examination. Results: We included 241 children and 900 adults. Infected children obtained lower UBT-13C delta values than infected adults (16.1 ± 8.7 and 37 ± 52.9, respectively). The rates of infection were higher in males who were recruited for diagnosis. Significant differences were obtained between positivity for H. pylori in overweight and obese children but not adults. UBT-13C titers were significantly associated with the body mass index (BMI) only in adults. Conclusions: H. pylori infection rates are similar between sexes and are higher in children probably because of selection bias. In children, H. pylori positivity is associated with higher BMI and excess malnutrition although with similar UBT-13C values. In adults, H. pylori infection is not related with BMI, but a higher BMI impacts UBT-13C titers.


ANTECEDENTES: La prueba de aliento con urea (UBT-13C) es una técnica no invasiva que permite el diagnóstico y confirmación de erradicación de la infección por Helicobacter pylori. Objetivo: Evaluar los valores de UBT- 13C en niños y adultos chilenos infectados y analizar su variación en relación al sexo, diagnóstico nutricional y edad de los pacientes. Material y Métodos: Estudio retrospectivo de 1.141 pacientes de 6 a 94 años. El enriquecimiento de13C se midió usando un espectrómetro de infrarrojos, calculando el delta 13C antes y después de la ingesta de urea marcada con 13C. Los datos clínicos de los pacientes se obtuvieron al momento del examen. Resultados: Incluimos 241 niños y 900 adultos con valores delta de UBT-13C de 16,1 ± 8,7 frente a 37 ± 52,9, respectivamente. Las tasas de infección fueron mayores en los hombres reclutados para el diagnóstico. Se obtuvieron diferencias significativas entre la positividad para H. pylori en niños con sobrepeso y obesidad, pero no en adultos. Los títulos de UBT-13C se asociaron significativamente con el índice de masa corporal (IMC) solo en adultos. Conclusiones: Las tasas de infección por H. pylori son similares entre los sexos y aumentan en los niños probablemente debido al sesgo de selección. En niños, la positividad para H. pylori se asocia con un IMC más alto y malnutrición por exceso, aunque con valores similares de UBT-13C. En los adultos, la infección por H. pylori no se relaciona con el IMC ni con la obesidad, pero el aumento del IMC afecta los títulos de UBT-13C.

RÉSUMÉ

¹³C metabolic flux analysis (¹³C-MFA) enables the precise quantification of intracellular metabolic reaction rates by analyzing the distribution of mass isotopomers of proteinogenic amino acids or intracellular metabolites through ¹³C labeling experiments. ¹³C-MFA has received much attention as it can help systematically understand cellular metabolic characteristics, guide metabolic engineering design and gain mechanistic insights into pathophysiology. This article reviews the advances of ¹³C-MFA in the past 30 years and discusses its potential and future perspective, with a focus on its application in industrial biotechnology and biomedicine.

Este estudo teve como objetivo avaliar méis comercializados em Pernambuco, por meio da análise isotópica do carbono, para detecção de adulteração. Ao total, 55 amostras foram submetidas à avaliação isotópica do mel e de sua proteína. Os resultados revelaram que 31 (56,36%) méis estavam adulterados com açúcar comercial. A variação isotópica das amostras com adulteração apresentou valores entre -1,270/00 e -12,690/00, o que ratificou o comércio de méis adulterados. É preciso intervenção efetiva e incisiva das autoridades fiscalizadoras competentes frente à fraude, haja vista o risco da população consumidora em adquirir produtos com baixo valor nutricional e / ou depreciação sanitária.

Abstract The aim of this study was to analyze the isotopic composition in muscle of striped weakfish Cynoscion guatucupa from Southwest Atlantic Ocean in order to evaluate a possible variation in δ13C and δ15N in response to dietary shifts that occur as animals grow. We also explored for isotopic evidence of differences between sample locations. The results showed an agreement between isotope analysis and previous conventional studies. Differences in the isotope composition between sampling location were not observed. A positive relation exists between isotope values and total body length of the animals. The Cluster analysis defined three groups of size classes, validated by the MDS. Differences in the relative consumption of prey species in each size class were also observed performing isotope mixing models (SIAR). Variation in δ15N among size classes would be associated with the consumption of a different type of prey as animals grow. Small striped weakfish feed on small crustaceans and progressively increase their consumption of fish (anchovy, Engraulis anchoita), increasing by this way their isotope values. On the other hand, differences in δ13C values seemed to be related to age-class specific spatial distribution patterns. Therefore, large and small striped weakfish remain specialized but feeding on different prey at different trophic levels. These results contribute to the study of the diet of striped weakfish, improve the isotopic ecology models and highlight on the importance of accounting for variation in the isotopic composition in response to dietary shifts with the size of one of the most important fishery resources in the region.


Resumo O objetivo deste estudo foi analisar a composição isotópica no músculo de Cynoscion guatucupa no Oceano Atlântico Sul Ocidental, a fim de avaliar uma possível variação de δ13C e δ15N como resposta às mudanças ontogenéticas na dieta. Foram também exploradas evidências isotópicas de diferenças entre os locais de amostragem. Os resultados mostraram uma concordância entre a análise de isótopos e os estudos convencionais prévios. Diferenças na composição isotópica entre locais de amostragem não foram observadas. Uma relação positiva existe entre valores isotópicos e comprimento total dos animais. A análise de cluster definiu três grupos de classes de tamanho, validados pelo MDS. Foram também observadas, através de modelos isotópicos mistos (SIAR), diferenças no consumo relativo de espécies de presas para cada classe de tamanho. Variações em δ15N entre as classes de tamanho estariam associadas ao consumo de diferentes tipos de presas ao longo do crescimento dos animais. Indivíduos pequenos alimentam-se de pequenos crustáceos e progressivamente aumentam seu consumo de peixes (Engraulis anchoita), aumentando assim os seus valores isotópicos. Por outro lado, as diferenças nos valores de δ13C parecem estar relacionadas com variações no padrão de distribuição espacial específico de cada classe idade. Desta maneira, tanto C. guatucupa grandes como pequenos permanecem especializados, porém alimentando-se de presas diferentes, em diferentes níveis tróficos e em diferentes áreas. Estes resultados contribuem para o estudo da dieta de C. guatucupa, melhoram os modelos de ecologia isotópica e ressaltam a importância de se considerar a variação na composição isotópica na resposta às mudanças de dieta relacionadas ao tamanho de um dos principais recursos pesqueiros da região.

OBJECTIVE: The 13C-urea breath test is the main non-invasive test for the diagnosis of Helicobacter pylori infection. The availability of this test throughout the country is limited, mainly due to the difficulty in obtaining the labeled isotope from abroad. Recently, researchers from the Nuclear Energy Center in Agriculture at the University of São Paulo (CENA/USP) succeeded in synthesizing 13C-enriched urea for Helicobacter pylori diagnosis. The aim of the study was to compare the performance of the 13C-urea breath test using 13C-urea acquired abroad with that of a test using 13C-urea synthesized in Brazil. METHOD: Sixty-four dyspeptic patients participated in the study (24 men and 40 women). Initially, the patients performed the 13C-urea breath test using the imported substrate (Euriso-Top, France). Seven to fourteen days later, all the patients repeated the test using the Brazilian substrate. The samples from both examinations were processed in an infrared isotope analyzer (IRIS, Wagner Analisen Technik, Germany), and all delta over baseline (DOB) [%] values above four were considered positive results. RESULTS: Twenty-seven patients (42%) exhibited negative results for Helicobacter pylori infection, and thirty-seven patients (58%) exhibited positive results when tested using the foreign substrate (gold standard). There was a 100% concordance regarding the presence or absence of infection when the gold standard results were compared with those obtained using the Brazilian substrate. CONCLUSIONS: Similar performance in the diagnosis of Helicobacter pylori infection was demonstrated when using the 13C-urea breath test with the Brazilian 13C-urea substrate and the test with the substrate produced abroad. This validation represents an important step toward increasing the availability of the 13C-urea breath test throughout the country, which will have a positive influence on the management of Helicobacter pylori infection.

Abstract Mangrove communities were selected in the state of Santa Catarina, Brazil, near their southernmost limit of distribution, to study mineral nutrient relation in soils and plants. Communities included three true mangrove species, Rhizophora mangle, Laguncularia racemosa and Avicennia germinans, and two associated species, the fern Acrostichum danaeifolium, and the grass Spartina densiflora. The sites included communities in the lower Río Tavares near Florianopolis city, Sonho beach near Palhoça city, and the Santo Antonio lagoon. These sites included a full range of mangroves under humid climate where winter temperatures, instead of salinity, may be the main factor regulating their productive capacity and species composition. Soil salinity was determined by the concentration of soluble Na, and soil C and N were linearly correlated indicating their association in organic matter. Tavares site showed higher specific conductivity, and concentrations of Na and Mg in the soil layer below 40 cm depth, indicating larger influence of marine water. Isotopic signature of C increased with soil depth suggesting that microorganisms decomposing organic matter are releasing 13C depleted CO2. Nitrogen isotopic signature decreased with soil depth, indicating enrichment in 15N possibly as a result of denitrification in the upper soil layers. Mineral elements in leaf tissues showed A. schaueriana with higher concentrations of N, P, Na, K, Cu, Zn, and Na/Ca ratio. Spartina densiflora was characterized by the lowest N and K concentrations, and the highest concentrations of Al and Fe. Rhizophora mangle and L. racemosa had the highest Ca concentrations. Carbon isotopic signatures identified S. densiflora as a C4 plant, and A. schaueriana as the mangrove species occupying comparatively more water stressed microsites than the rest. Leaf nitrogen isotopic signatures were positive, in correspondence with the soil values. The results support the hypothesis that sites sampled were comparatively fertile, because sediment transport through fresh water run-off is predominant in humid coasts, and therefore plants were not limited by nutrient supply, nor particularly stressed by soil salinity.


Resumo Foram selecionadas florestas de mangue próximas ao limite sul de distribuição dos manguezais, no estado de Santa Catarina, Brasil, para o estudo do conteúdo de nutrientes no solo e em tecido vegetal. As comunidades estudadas eram compostas por três espécies típicas de mangue: Rhizophora mangle, Laguncularia racemosa e Avicennia germinans, e duas espécies associadas: a samambaia-do-brejo, Acrostichum danaeifolium, e a herbácea Spartina densiflora. Os locais de estudo incluiram comunidades no curso inferior do Rio Tavares na cidade de Florianópolis; Praia do Sonho, próxima à cidade de Palhoça e a Lagoa de Santo Antonio. Esses locais incluiram manguezais submetidos a clima úmido, onde as temperaturas do inverno, mais do que a salinidade, são o principal fator regulador da capacidade produtiva do sistema e da composição de espécies. A salinidade do solo foi determinada pela concentração de Na solúvel. Os conteúdos de Carbono e Nitrogênio no solo apresentaram correlação linear, indicando sua associação na matéria orgânica. A área do Rio Tavares apresentou maior influência marinha evidenciada pelas altas condutividade específica e concentrações de Na e Mg nas camadas do solo abaixo de 40 cm de profundidade. A assinatura isotópica do C aumentou com a profundidade no solo, o que sugere que microorganismos decompositores da matéria orgânica estão liberando CO2 pobre em 13C. A assinatura isotópica do Nitrogênio diminui com a profundidade do solo, indicando enriquecimento em 15N, possivelmente como resultado de processo de denitrificação nos níveis superiores do solo. Os tecidos foliares de A. schaueriana apresentaram as maiores concentrações de N, P, Na, K, Cu, Zn, e da razão Na/Ca. Spartina densiflora apresentou as menores concentrações de N e K e as maiores concentrações de Al e Fe. Rhizophora mangle e L. racemosa apresentaram as maiores concentrações de Ca. Os resultados da assinatura isotópica de Carbono identificam S. densiflora como uma planta C4 e A. schaueriana como a espécie de mangue que ocupa os locais com maior estresse hidrico. As assinaturas isotópicas do Nitrogênio foram positivas, estando de acordo com os valores observados para o solo. Os resultados sustentam a hipótese de que os locais estudados são fertéis, pois o escoamento superficial de água doce é predominante em costas úmidas e, portanto, as plantas não estão sujeitas à limitação por aporte de nutrientes e não estão submetidas a estresse salino.

Objective Adapt the 6 minutes walking test (6MWT) to artificial gait in complete spinal cord injured (SCI) patients aided by neuromuscular electrical stimulation. Method Nine male individuals with paraplegia (AIS A) participated in this study. Lesion levels varied between T4 and T12 and time post injured from 4 to 13 years. Patients performed 6MWT 1 and 6MWT 2. They used neuromuscular electrical stimulation, and were aided by a walker. The differences between two 6MWT were assessed by using a paired t test. Multiple r-squared was also calculated. Results The 6MWT 1 and 6MWT 2 were not statistically different for heart rate, distance, mean speed and blood pressure. Multiple r-squared (r2 = 0.96) explained 96% of the variation in the distance walked. Conclusion The use of 6MWT in artificial gait towards assessing exercise walking capacity is reproducible and easy to apply. It can be used to assess SCI artificial gait clinical performance. .


Objetivo Adaptar o teste de caminhada dos 6 minutos (TC6) para marcha artificial de pacientes com lesão medular completa associado a eletroestimulação neuromuscular. Método Nove participantes do sexo masculino com paraplegia (AIS A) participaram do estudo. O nível de lesão variou entre T4 e T12 , tempo de lesão variou entre 4 e 13 anos. Os pacientes realizaram dois TC6 (TC6-1 e TC6-2). Os participantes usaram eletroestimulação neuromuscular e foram auxiliados por andador. As diferenças entre os dois TC6 foram avaliadas pelo teste t pareado e calculado o r2. Resultados Não foi encontrada diferença estatística entre TC6-1 e TC6-2 para frequência cardíaca, distância, velocidade média e pressão arterial. O r2 = 0,96 explica 96% da variação na distância caminhada. Conclusão O uso do TC6 em marcha artificial para avaliação da capacidade de exercício de caminhada é reprodutível e fácil de aplicar. Esse teste pode ser utilizado para avaliar o desempenho clínico da marcha artificial de indivíduos com lesão medular. .

Most main oilfields in China have already entered a "double high" development stage (high water cut, high recovery degree). To further enhance oil recovery in reservoirs after polymer flooding (RAPFs), an efficient activator formulation for promoting metabolism of endogenous microorganism was studied by aerogenic experiments, physical simulation experiments, electron microscopy scanning and pyrophosphate sequencing. Results show that the activator could activate the endogenous microorganisms in the injected water and make the pressurized gas reach 2 MPa after 60 d static culture of the activator in a high pressure vessel. The oil recovery efficiency of natural core physical simulation flooding can be improved by more than 3.0% (OOIP) in RAPFs when injected 0.35 PV activator with 1.8% mass concentration, and a lot of growth and reproduction of activated endogenous microorganism in the core was observed by electron microscopy scanning. Field trial with 1 injector and 4 producers was carried out in the east of south II block of Sa Nan in December 2011. By monitoring four effective production wells, changes of carbon isotope δ13C (PDB) content of methane and carbon dioxide were -45 per thousand to -54 per thousand and 7 per thousand to 12 per thousand. Compared with east II of Sa Nan block, the oil amount increased by 35.9%, water cut stabled at 94%. The incremental oil was 5 957 t during the three and a half years, which provides an alternative approach for further improving oil recovery in similar reservoirs.

A la mayoría de las personas a las que se les menciona una prueba de aliento inmediatamente les viene a la mente la visión de un policía de tránsito en medio de un retén, comprobando con un instrumento si el conductor de un vehículo ha consumido bebidas alcohólicas. El conductor sospechoso debe soplar en un pequeño dispositivo manual que, al cabo de unos segundos, indica la cantidad de etanol que está circulando en su sangre. Esta visión es real pero sólo es una de las múltiples aplicaciones de las pruebas de aliento. El aliento, como objeto de análisis, similar a la sangre o la orina, no es nuevo: desde los tiempos de Hipócrates, los médicos han sabido que el aroma de la respiración humana puede proporcionar pistas para el diagnóstico de múltiples enfermedades. El buen clínico conoce el olor dulce y afrutado de la acetona en el aliento de pacientes con diabetes mellitus no controlada, el olor a humedad y a pescado en pacientes con enfermedad hepática avanzada, el olor a orina fermentada característico de las enfermedades renalesterminales y, el hedor pútrido de un absceso pulmonar. La diferencia de aquellos médicos y los de hoy es el instrumento que tienen para leer el aliento: el olfato, finamente entrenado, en los primeros y los instrumentos, de alta tecnología y excelente desempeño analítico, en los segundos...

O estudo objetivou avaliar o turnover do 13C no sangue e plasma de codornas japonesas utilizando a técnica de isótopos estáveis, para a obtenção do patamar de equilíbrio isotópico que servirá de fundamento para estudos de rastreabilidade. Foram utilizadas 300 aves durante o período experimental de 1-42 e 49-97 dias de idade. Os tratamentos da primeira fase foram constituídos de dietas à base de arroz (C3), contendo ou não farinha de carne e ossos bovinos e um com dieta à base de milho (C4). Nessa primeira fase foi analisado o turnover do sinal isotópico do matrizeiro à base de dietas C4 para dietas à base de C3, como também as diferenças isotópicas das dietas contendo ou não farinha de origem animal. Na segunda fase houve uma substituição de dietas, ou seja, as aves no tratamento C4 da primeira fase passaram a consumir dieta C3, e o tratamento que antes consumia dieta C3 passou para dieta C4. Para determinar a taxa de turnover e o percentual estimado de participação da farinha na composição do material coletado, foi empregado o modelo de diluição isotópica utilizando valores do δ13C. A comparação entre as meias-vidas do sangue e plasma da primeira fase revelou o enriquecimento do δ13C na dieta; já na segunda fase foi possível observar as velocidades de incorporação após a troca das dietas...


The study aimed to evaluate the turnover of 13C in the blood and plasma of Japanese quail using the technique of stable isotopes to obtain the level of isotopic equilibrium that will be the foundation for studies of traceability. A total of 300 birds during the experimental period of 1-42 and 49-97 days of age. Stable isotope composition of marine top predator’s tissues provides insight information of its trophic ecology and migratory behavior. Previous reports have shown that dermal tissues could record longer patterns of hunting and movement. Based on this, the aim of this study was to describe the feeding and migratory habits of the white shark from Isla Guadalupe, using stable isotopic analysis of dermis. We considered a small subset of many possible prey taxa that the sharks could have eaten throughout their migration: pinnipeds, squid and tuna. We grouped the data in five focal areas: Gulf of California, Coast of California, Isla Guadalupe, SOFA and Hawaii. We performed a Bayesian mixing model to study the trophic ecology of this top predator. Average isotopic values for dermis tissue of white shark were δ13C (-14.5‰) and δ15N (19.1‰). Corrected white shark dermal mean values to resemble muscle were δ13C (-16.6‰) and δ15N (21.2‰). Mixing model data from dermis showed predation in offshore areas such the SOFA and a main importance of pinnipeds as prey of the white shark in Isla Guadalupe. Rev. Biol. Trop. 62 (2): 637-647. La composición isotópica de los tejidos de los depredadores tope en el ambiente marino provee información sobre su ecología trófica y su comportamiento migratorio. Estudios previos han mostrado que el tejido dérmico puede registrar patrones largos de movimiento y caza. El objetivo de este estudio fue describir los hábitos tróficos y migratorios de los tiburones blancos de Isla Guadalupe realizando un análisis de isótopos estables de su tejido dérmico. Consideramos un pequeño grupo de muchos posibles taxa que los tiburones pudieron haber comido a lo largo de su migración: pinípedos, calamares y atunes. Estas presas fueron agrupadas en cinco áreas focales: Golfo de California, Isla Guadalupe, Costa de California, SOFA y Hawái. Realizamos un modelo de mezcla bayesiano para estudiar la ecología trófica de este depredador tope. Los promedios del valor isotópico de la dermis del tiburón blanco fueron δ13C (-14.5‰) y δ15N (19.1‰). Los promedios del valor isotópico de la dermis transformada para semejar músculo fueron δ13C (-16.6 ‰) y δ15N (21.2‰). El modelo de mezcla mostró una probable depredación de los tiburones en áreas oceánicas como el SOFA y confirmó la importancia de los pinnípedos como presa principal del tiburón blanco en Isla Guadalupe.

The aim of this study is to develop a synthetic medium suitable for 13C metabolic flux analysis (13C-MFA) of Streptomyces rimosus. The cell growth rate and oxytetracycline production by S. rimosus M4018 were compared when M4018 cells were growth on the optimized chemically defined media with organic nitrogen sources or inorganic nitrogen sources. First, a synthetic medium contained KNO3 as the main nitrogen source was screened, then optimized by a response surface method. Using this new medium, the oxytetracycline yield was increased from 75.2 to 145.6 mg/L. Furthermore, based on the 13C-MFA, we identified that Entner-Doudoroff pathway does not exist in S. rimosus cells cultured in a chemically defined medium with feed of 100% 1-13C labeled glucose. This study is helpful for subsequent 13C-MFA application of S. rimosus.

Using stable nitrogen and carbon isotope signatures, we investigated the trophic ecology and identified potential prey fish groups supporting the giant Arapaima within floodplain lakes of the Essequibo River basin in southwestern Guyana. Morphological descriptions of feeding structures and digestive tract are presented together with preliminary data on Arapaima diets. Stable isotope results suggest that algivorous/detritivorous and omnivorous fishes contributed most to Arapaima biomass, and generally, that was consistent with what is known about Arapaima diets. Stable nitrogen isotope ratios for piscivorous fishes in these lakes were higher than nitrogen isotope ratios for Arapaima, indicating that piscivorous fishes are unlikely to constitute a major source of energy for Arapaima. This population of Arapaima has an intestine averaging 1.45 times total body length, relatively small teeth, and numerous, closely-spaced gill rakers. These morphological features, together with isotope data, support our inference that Arapaima are secondary consumers and may be better characterized as omnivores and not top predators.


Utilizando firmas de isotopos estables de nitrógeno y carbón, investigamos la ecología trófica e identificamos los grupos de peces que potencialmente mantienen a la Arapaima en los lagos inundables de la cuenca del río Essequibo, al suroeste de Guyana. Presentamos descripciones morfológicas de las estructuras alimentarias y tracto digestivo de la Arapaima, conjuntamente a datos preliminares de sus dietas. Los isotopos estables sugieren que peces algívoros/detritívoros y peces omnívoros son los principales contribuyentes de la biomasa de la Arapaima, y estos resultados son compatibles con lo que se conoce actualmente de la dieta de la Arapaima. A diferencia, las proporciones del isotopo estable de nitrógeno para peces piscívoros en estos lagos resultaron más altas que los valores obtenidos para el isotopo estable de nitrógeno en la Arapaima. Esto indica que es improbable que sean peces piscívoros los que constituyan la fuente energética principal de la Arapaima. La población de Arapaima estudiada presenta un intestino que promedia 1,45 veces la longitud total del cuerpo, dientes relativamente pequeños, y agallas con branquiespinas numerosas y cercanamente espaciadas. Estas características morfológicas, conjuntamente a los datos obtenidos a través del uso de isotopos estables apoyan nuestra inferencia que la Arapaima es un consumidor secundario y que puede ser caracterizada como un pez omnívoro y no como un depredador mayor.

OBJECTIVE: Helicobacter pylori (H pylori) is a common bacterial infection that is associated with significant morbidity and mortality worldwide. This bacterium causes a chronic infection that is causally related to illnesses ranging from gastritis, peptic ulcer disease to gastric cancer. It is generally considered that it is acquired in childhood but the prevalence varies considerably between countries and communities. There are few data on the prevalence of H pylori in the Caribbean and none on the prevalence of H pylori in children in the Bahamas. The aim of this pilot study was to determine the prevalence of H pylori infection in a cohort of school children in the Bahamas. METHODS: One hundred and sixty-one children attending a public primary school in the Bahamas were invited to participate in this study. Consent was obtained for 107 children and each participant completed a brief questionnaire. Valid data were available for 96 of these children. Active H pylori infection was determined using the 13C urea breath test (UBT). RESULTS: Fifty-two children tested positive for H pylori, yielding a prevalence of 54.2%. The median age in the study was nine years with 46.9% male and 53.1% female. No significant relationship was found between gender, breastfeeding, pets and H pylori status. CONCLUSION: The prevalence reported in this study is the highest reported in asymptomatic children in the Caribbean. Further studies are required to determine risk factors for acquisition of H pylori infection in this population.


OBJETIVO: Helicobacter pylori (H pylori) es la causa de una infección bacteriana común a nivel mundial, asociada con una morbosidad y mortalidad significativas. Esta bacteria causa una infección crónica que se haya causalmente relacionada con un número de enfermedades que van desde la gastritis y la úlcera péptica hasta el cáncer gástrico. Generalmente se considera que es adquirida en la niñez, pero la prevalencia varía considerablemente entre los países y comunidades. Hay pocos datos con relación a la prevalencia de H pylori en el Caribe, y no existe ningún dato sobre la prevalencia de H pylori en los niños de Bahamas. El objetivo de este estudio piloto fue determinar la prevalencia de la infección por H. pylori en una cohorte de niños escolares en la Bahamas. MÉTODOS: Ciento sesenta y un niños que asistían a una escuela primaria pública en Bahamas, fueron invitados a participar en este estudio. Se obtuvo consentimiento para 107 niños, y cada participante respondió a una breve encuesta. Había datos válidos disponibles para 96 de estos niños. La infección activa por H pylori fue determinada usando la prueba de aliento con urea-13C (UBT). RESULTADOS: Cincuenta y dos niños resultaron positivos a la prueba de H pylori, para una prevalencia de 54.2%. La edad promedio de la población en estudio fue de nueve años, con un 46.9% de varones y un 53.1% de hembras. No se halló ninguna relación significativa entre el género, la lactancia materna, las mascotas, y la condición del H pylori. CONCLUSIÓN: La prevalencia reportada en este estudio es la más alta que se haya reportado en niños asintomáticos en el Caribe. Se requieren estudios ulteriores a fin de determinar los factores de riesgo que conducen a la infección por H pylori en esta población.

Gene expression is regulated by different transcriptional regulators. The transcriptional regulator isocitrate lyase regulator (IclR) of Escherichia coli represses the expression of the aceBAK operon that codes for the glyoxylate pathway enzymes. In this study, physiological and metabolic responses of the deletion of the ic1R gene in E. coli BW25113 were investigated based on the quantification and analysis of intracellular metabolic fluxes. The knockout of the iclR gene resulted in a decrease in the growth rate, glucose uptake rate and the acetate secretion rate, but a slight increase in biomass yield. The latter could be attributed to the lowered metabolic fluxes through several CO2 generating pathways, including the redirection of 33% of isocitrate directly to succinate and malate without CO2 production as well as the reduced flux through the pentose phosphate pathway. Furthermore, although the glyoxylate shunt was activated in the iclR mutant, the flux through phosphoenolpyruvate (PEP) carboxykinase kept almost unchanged, implying an inactive PEP-glyoxylate cycle and no extra loss of carbon atoms in the mutant strain. Both the reduced glucose uptake rate and the active glyoxylate shunt were responsible for the minor decrease in acetate secretion in the ic1R knockout strain compared to that in the wild-type E. coli strain.

CONTEXT: The standard doses of 13C-urea in 13C-urea breath test is 75 mg. OBJECTIVE: To assess the diagnostic accuracy of 13C-urea breath test containing 25 mg of 13C-urea comparing with the standard doses of 75 mg in the diagnosis of Helicobacter pylori infection. METHODS: Two hundred seventy adult patients (96 males, 174 females, median age 41 years) performed the standard 13C-urea breath test (75 mg 13C-urea) and repeated the 13C-urea breath test using only 25 mg of 13C-urea within a 2 week interval. The test was performed using an infrared isotope analyzer. Patients were considered positive if delta over baseline was >4.0‰ at the gold standard test. RESULTS: One hundred sixty-one (59.6 percent) patients were H. pylori negative and 109 (40.4 percent) were positive by the gold standard test. Using receiver operating characteristic analysis we established a cut-off value of 3.4 percent as the best value of 25 mg 13C-urea breath test to discriminate positive and negative patients, considering the H. pylori prevalence (95 percent CI: 23.9-37.3) at our setting. Therefore, we obtained to 25 mg 13C-urea breath test a diagnostic accuracy of 92.9 percent (95 percent CI: 88.1-97.9), sensitivity 83.5 percent (95 percent CI: 75.4-89.3), specificity 99.4 percent (95 percent CI: 96.6-99.9), positive predictive value 98.3 percent (95 percent CI: 92.4-99.4), and negative predictive value 93.0 percent (95 percent CI: 88.6-96.1). CONCLUSIONS: Low-dose 13C-urea breath test (25 mg 13C-urea) does not reach accuracy sufficient to be recommended in clinical setting where a 30 percent prevalence of H. pylori infection is observed. Further studies should be done to determine the diagnostic accuracy of low doses of 13C-urea in the urea breath test.


CONTEXTO: A dose convencional de 13C-ureia para a realização do teste respiratório com 13C-ureia é 75 mg. OBJETIVO: Determinar a precisão diagnóstica do teste respiratório contendo 25 mg de 13C-ureia comparada com a dose convencional de 75 mg para o diagnóstico de infecção por H. pylori. MÉTODOS: Duzentos e setenta pacientes adultos (96 homens, 174 mulheres, idade mediana de 41 anos) realizaram o teste respiratório convencional (75 mg 13C-ureia) e repetiram o teste respiratório usando apenas 25 mg de 13C-ureia dentro de 2 semanas de intervalo. O teste respiratório foi realizado empregando-se analisador de isótopos por infravermelho. Os pacientes foram considerados positivos quando apresentavam valor delta acima da linha de base >4.0 no teste respiratório convencional (padrão-ouro). RESULTADOS: Cento e sessenta e um pacientes (59,6 por cento) eram H. pylori negativos e 109 (40,4 por cento) eram positivos aos testes respiratórios convencionais. Para definição do melhor ponto de corte discriminatório entre positivos e negativos pelo teste respiratório com 25 mg, foi utilizado a curva ROC, obtendo-se o valor de 3,4 por cento, considerando-se a prevalência de 30,2 por cento (IC 95 por cento: 23.9-37.3) da infecção por H. pylori no laboratório, onde se realizou este estudo. Desta forma, para o teste respiratório com 25 mg foi obtida uma precisão diagnóstica de 92,9 por cento (IC 95 por cento: 88,1-97,9), sensibilidade de 83,5 por cento (IC 95 por cento: 75,4-89,3) e especificidade de 99,4 por cento (IC 95 por cento: 96,6-99,9). CONCLUSÕES: Teste respiratório com dose baixa (25 mg) de 13C-ureia não proporciona precisão suficiente para ser recomendado em ambientes clínicos, onde a prevalência de H. pylori está situada em torno de 30 por cento.

BACKGROUND/AIMS: Revaprazan (Revanex(R)) is a novel proton pump inhibitor (PPI) that has a somewhat different effect on proton pump compared with the other PPI's, also (called as 'acid pump antagonist'). We aimed to examine the false negative rate of 13C-urea breath test (UBT) in the patients with Helicobacter pylori (H. pylori) associated peptic ulcer disease who were treated with revaprazan and evaluate the anti-urease activity of revaprazan. METHODS: Total 55 patients were enrolled in this study. They received EGD examination between January 2009 and December 2009 and diagnosed histologically as H. pylori associated peptic ulcer disease. All patients took revaprazan only. Three patients were excluded because of underlying chronic disease and inappropriate breath sampling. The remaining 52 patients had UBT at 0, 2 and 4 weeks of revaprazan use. After 2 weeks of the cessation of revaprazan, they had the fourth UBT. RESULTS: At 2 and 4 weeks, the false negative rates of UBT were 5.8% and 23.1%, respectively (p=0.05). After 2 weeks of the cessation, the cases of the false negative result were five. Four out of five patients had prolonged negative results on two or three successive tests, and baseline 13C difference value did not predict the false negative results. CONCLUSIONS: False negative results of UBT were common and increased with prolonged use of acid pump antagonist. As PPI, it had also anti-urease activity and most patients (47/52, 90.4%) reverted to positive results by 2 weeks after the cessation of taking the medication.

In the structural determination of natural glycosides, nuclear magnetic resonance (NMR) is an important approach in determining the configuration of glycoside bond. The test of coupling constant of the anomeric proton and chemical shift of the anomeric carbon are two common methods, but these methods are not suitable for some sugars. For those sugars, detailed 13C NMR analysis is an alternative choice. This paper summarizes the characteristics of 1H and 13C NMR data of the common monosaccharides published in the literatures, in order to search an approach to determine the configuration of glycoside bond.
