RÉSUMÉ

Los lipomas del colon ocupan el tercer lugar en frecuencia de aparición de tumores benignos. Estos tumores están formados por tejido adiposo bien diferenciado con un estroma fibroso. La gran mayoría de estos lipomas es asintomática, algunos en raras ocasiones presentan complicaciones de urgencia. El fin de esta publicación es presentar un caso de obstrucción de colon por lipoma. Benign colonic lesions are infrequent and account for a low percentage of all colonic tumors. Among the benign tumors, lipomas are third in frequency. They are composed of mature adipose tissue with fibrous stroma. Most of them are asymptomatic but in rare instances, they may present as surgical emergencies. We present one case of colonic obstruction caused by lipomas. (AU)

RÉSUMÉ

El lipoma gástrico (LG), es un tumor benigno raro que representa el 5 % de los lipomas gastrointestinales y menos del 1 % de todos los tumores gástricos. Se localizan habitualmente a nivel de la submucosa y generalmente en la región antral. La endoscopia y la tomografía computarizada son las herramientas diagnósticas más utilizadas en el proceso diagnóstico. Dada su relativa rareza, quedan a menudo fuera del diagnóstico diferencial de las masas submucosas gastrointestinales superiores. El objetivo de este manuscrito, es reportar dos casos tratados de forma consecutiva y revisar la literatura existente en esta materia. Se trata de dos pacientes de sexo masculino, de 49 y 69 años de edad, a quienes se diagnosticó una lesión submucosa gástrica por endoscopia y fueron tratados quirúrgicamente y cuyos especímenes fueron estudiados histológicamente, concluyéndose en ambos casos la existencia de un LG. Ambos pacientes evolucionaron de forma correcta y permanecen en controles clínicos y endoscópicos hasta la actualidad, sin inconvenientes. El LG es una entidad poco común, que puede simular una enfermedad maligna. Presentamos dos casos tratados quirúrgicamente con resultados satisfactorios.


Gastric lipoma (GL), is a rare benign tumor that represents 5 % of gastrointestinal lipomas and less than 1 % of all gastric tumors. They are usually located at the level of the submucosa and usually in the antral region. Endoscopy and computed tomography are the diagnostic tools most used in the diagnostic process. Given their relative rarity, they are often left out of the differential diagnosis of the upper gastrointestinal submucosal masses. The aim of this manuscript was to report two cases treated consecutively and review the existing literature on this subject. Two male patients, 49 and 69 years of age, were diagnosed with a gastric submucosal lesion by endoscopy and were treated surgically. The specimens were studied histologically; concluding the existence of a GL in both cases. The two patients evolved adequately and have remained in clinical and endoscopic controls until now, without problems. GL is a rare entity, which can simulate a malignant disease. We present two cases treated surgically with satisfactory results.

Los lipomas de laringe son tumores benignos de crecimiento lento, pero pueden producir síntomas graves como disnea inspiratoria, apneas del sueño y disfagia. Se describe el caso clínico de un paciente con un lipoma extenso de laringe e hipofaringe que consultó por disfagia y disnea inspiratoria intermitente, y que fue tratado con éxito mediante un abordaje transoral con microscopio asistido con cauterio y láser de CO2. Laryngeal lipomas are benign, slow-growing tumors, but they can produce severe symptoms such as inspiratory dyspnea, sleep apnea, and dysphagia. We describe the clinical case of a patient with an extensive lipoma of the larynx and hypopharynx who consulted for dysphagia and intermittent inspiratory dyspnea, and who was successfully treated by a transoral approach with microscope and assisted with cautery and CO2 laser. RESUMOLipoma arborescens (LA) é definido como uma proliferação vilosa lipomatosa da membrana sinovial com substituição difusa do tecido sinovial por células gordurosas maduras. A lesão é muito rara, envolve a articulação do joelho (região suprapatelar) e se associa a doenças articulares degenerativas. Descrevemos o caso de um paciente de sexo masculino, portador de espondilite anquilosante com quadro de monoartrite de joelho direito e aumento de volume importante na região suprapatelar. O diagnóstico de lipoma arborescens foi confirmado a partir da ressonância magnética do joelho direito.


ABSTRACTLipoma arborescens (LA) is defined as a villous lipomatous proliferation of the synovial membrane with diffuse substitution of the synovial tissue by mature fat cells. The injury is very rare, involving the knee joint (suprapatellar region) and is associated with degenerative joint disease. We describe the case of a male patient, suffering from ankylosing spondylitis with monoarthritis of the right knee and swelling in suprapatellar region important. The diagnosis of lipoma arborescens was confirmed from the MRI of the right knee.

RÉSUMÉ

Here, we describe a popliteal mass that was initially misdiagnosed as a simple popliteal cyst, which finally turned out to be osteochondrolipoma. A 63-year-old housewife presented with sustained knee pain in association with a palpable mass on the popliteal fossa. The mass was in the posteromedial area and soft, non-tender, non-movable in the posteromedial area. Using plain radiography, the mass appeared as a round, soft tissue density lesion containing bony fragments. We performed an ultrasound-guided needle biopsy in conjunction with magnetic resonance imaging, followed by an open excisional biopsy. Microscopically, histological sections showed a lipoma with cartilaginous and osseous differentiation, finally diagnosed as osteochondrolipoma. In conclusion, popliteal masses are not always simple cysts, and the evaluation of masses in the popliteal fossa is always necessary.

A síndrome do impacto é definida pela colisão dos tendões dos músculos que formam o manguito rotador contra o arco coracoacromial. Vários fatores concorrem para essa doença e são classificados como estruturais ou funcionais. Os primeiros são alterações do arco coracoacromial, úmero, bursa e manguito rotador; os fatores funcionais estão relacionados com o mecanismo de elevação do membro superior, por meio da atividade sincronizada e equilibrada entre o manguito rotador e os músculos da cintura escapular. Os autores relatam aqui, o caso de um lipoma parosteal do úmero proximal, situado entre os ventres musculares do deltoide, redondo menor e infraespinhal ocasionando os sinais clínicos da síndrome do impacto. Trata-se de uma ocorrência rara, caracterizada como uma causa estrutural para o desencadeamento dessa sintomatologia.


The impingement syndrome is defined by the compression of the rotator cuff tendons against the coracoacromial arch. Several factors contribute to this condition and they are classified as structural or functional factors. The former are changes in the coracoacromial arch, proximal humerus, bursa and rotator cuff, and the latter are related to the mechanism of the upper limb by means of synchronized activity and balanced between the rotator cuff and scapular girdle muscles. The authors report here a case of parosteal lipoma of the proximal humerus, located between the muscles deltoid, teres minor and infraspinatus causing clinical signs of impingement. It is a rare occurrence, characterized as a structural cause for the onset of this symptom.

Colonic lipomas have very low frequency, are usually asymptomatic and diagnosis is made incidentally. Seventy-five per cent of lipomas larger than 4 cm are symptomatic, causing abdominal pain, rectal bleeding, obstruction and exceptionally invagination. The resection of invaginated segment is mandatory in cases with invagination and can be performed by laparoscopy when colonic dilation is moderate. We present a 73-year-old man who entered the emergency department complaining of intermitent abdominal pain, rectal bleeding, absence of bowel movements and flatulence, during four days. A CT scan showed a generalized colonic dilation until left lower quadrant. A colo-colonic invagination secondary to an endoluminal lipoma was observed in sigmoid colon. A laparoscopic sigmoidectomy was performed with extracorporeal termino-terminal anastomosis. The postoperative period was uneventful and the patient was discharged from the hospital five days later. A sumbmucous colonic lipoma was diagnosed in the pathological study.

Los lipomas están dentro del grupo de las neoplasias benignas no epiteliales de baja frecuencia y es la segunda neoplasia más frecuente de colon, son tumores bien diferenciados originándose de tejido conectivo intestinal, con incidencia del 0.035% a 4.4%, presentándose entre los 50 y 70 años de edad, predomina en raza blanca, 50-70% están confinados al colon ascendente, el 90% son de localización en tejido submucoso y el resto subseroso, normalmente cursan de forma asintomática y presentan manifestaciones clínicas cuando tienen un tamaño mayor a 2cm, interfiriendo con el funcionamiento mecánico normal del colon. Benign Gastric tumors are rare and generally account for less than 10% of all stomach tumors. Gastric lipoma is a rare tumor that constitutes approximately 3% of all benign tumors of the stomach and mainly is seen as a submucosal mass. Most gastric lipoma are asymptomatic and are found accidentally. Occasionally they can cause symptoms such as gastrointestinal bleeding, obstruction, abdominal pain and intussuception. CT scan and endoscopy are helpful in diagnosis. The main modality of treatment is surgery. Diagnosis is confirmed by histology. In this article, a patient with weakness, fatigue, and melena complaints is presented. On gastric endoscopy a yellowish submucosal mass was seen in antrum. Abdominal CT scan disclosed a mass with fat density resembling lipoma. The patient underwent surgery and diagnosis of lipoma was confirmed by histology. Although gastric lipoma is rare, it should be considered in the assessment and differential diagnosis of hemorrhagic submucosal masses in the stomach

RÉSUMÉ

OBJECTIVE: To observe whether microsurgical removal of medullary lipomas and untethering of the medulla is a safe and efficient procedure. METHOD: A retrospective study was carried out on 38 patients with medullary lipomas associated with spinal dysraphism who underwent operations between January 1986 and January 2008, at the Neurosurgery Department of the Federal Hospital for State Public Servants, in Rio de Janeiro. RESULTS: No deaths occurred in this series, and there was no worsening of motor or bladder function among the patients. Seven individuals presented improvements in their motor deficit. Nine patients presented improvements in bladder function. Three individuals with trophic lesions achieved wound healing. CONCLUSION: Microsurgical removal of medullary lipomas associated with spinal dysraphism proved to be a safe procedure without deaths and with a low morbidity rate, and several patients achieved improvements in their neurological symptoms.


OBJETIVO: Observar se a remoção microcirúrgica dos lipomas medulares e a liberação da medula da tração exercida pelo lipoma é um procedimento seguro e eficaz. MÉTODO: Realizamos estudo retrospectivo de 38 pacientes com lipomas medulares associados ao disrafismo espinhal operados entre janeiro de 1986 a dezembro de 2009 no Serviço de Neurocirurgia do Hospital Federal dos Servidores do Estado do Rio de Janeiro. RESULTADOS: Nessa série não ocorreu nenhum óbito, ou piora da função motora ou vesical em nenhum paciente. Observamos melhora do défice motor em 7 pacientes. Nove pacientes apresentaram melhora da função vesical. Três indivíduos com lesões tróficas apresentaram cicatrização das suas feridas. CONCLUSÃO: A remoção microcirúrgica dos lipomas medulares associados ao disrafismo espinhal se mostrou segura, sem nenhum óbito, com baixa morbidade e com melhora dos sintomas neurológicos em vários pacientes.

RÉSUMÉ

We report a rare case of giant mesenteric lipoma presenting with colicky abdominal pain. A 29-yr-old woman underwent laparoscopic resection for a giant mesenteric lipoma causing compression of the ileal loop. The resected ileal segment was encased by a giant fatty tissue, and normal mucosal fold patterns of the resected ileum were effaced by the mass. Microscopically, the mass was characterized by homogenous mature adipose tissue without cellular atypia, which was compatible with the diagnosis of a mesenteric lipoma. Despite the benign nature of this tumor, total excision with or without the affected intestinal loop should be considered if intestinal symptoms such as abdominal pain are present.
