RÉSUMÉ
Introducción: El vértigo se define como la distorsión en la sensación de movimiento propio cuando se realiza un movimiento normal de la cabeza, para su estudio y categorización por el médico general, se recomienda el enfoque TiTrATE, que determina el momento de inicio, duración, evolución y los desencadenantes del vértigo. Objetivo: creación de una aplicación web progresiva (PWA) llamada TiTrATEapp y aplicarla en los pacientes que acudan a la emergencia en el mes de febrero 2023. Método: estudio tipo prospectivo, experimental, observacional, de análisis comparativo. Resultado: se creó una aplicación web progresiva (PWA) llamada TiTrATEapp, basada en el algoritmo TiTrATE, comparándose el tiempo de cálculo de la aplicación del algoritmo de forma manual y digital, obteniendo como resultado que la aplicación digital es más rápida y eficaz. Conclusión: Las aplicaciones móviles son una herramienta objetiva que ayuda los profesionales de la salud a diagnosticar y a tomar decisiones clínicas de forma práctica basadas en la evidencia científica, la digitalización del TiTrATE permite a los médicos realizar un diagnóstico acertado de forma rápida y orientan a la referencia del paciente a la especialidad correcta y la eficiencia de recursos. Introduction: Vertigo is defined as the distortion in the sensation of self-movement when a normal movement of the head is made, for its study the TiTrATEapproach is recommended for the general practitioner, which determines the moment of onset, duration, evolution and triggers of vertigo for its study and categorization accurately. Objective: to create a progressive web application (PWA) called TiTrATEapp, Method: Prospective, observational, longitudinal study. Result: creation of a progressive web application (PWA) called TiTrATEapp, based on the TiTrATE algorithm, comparing the calculation time of the application of the algorithm manually and digitally, resulting in the digital application being faster and more efficient. Discussion: Vertigo is a difficult symptom to decipher in the emergency room, due to the subjective description of patients and the wide variety of probable causes, the digitization ofTiTrATE allows to rule out potentially dangerous causes, quickly, accurately. At present there are no studies that measure the effectiveness of mobile applications for the study of vertigo. Conclusion: Mobile applications are an objective tool that helps health professionals diagnose and make clinical decisions in a practical way based on scientific evidence, the digitalization of TiTrATE allows resident doctors to make an accurate diagnosis quickly and guide the patient's referral to the correct specialty and resource efficiency. Introduction: Due to its precision and simplicity, videonystagmography is the most used technique for evaluating eye movements in patients with vestibular complaints. However, its high cost limits its widespread use. In response, the development of low-cost goggles emerged as an alternative to traditional videonystagmography equipment. Objetives: This study aimed to compare the performance between low-cost goggles and videonystagmography in assessing spontaneous nystagmus. Materials and Methods: We conducted a cross-sectional study at the Department of Otolaryngology at the Red UC Christus. Patients referred for vestibular evaluation were assessed using both videonystagmography and a prototype of low-cost goggles. Two blinded clinicians with varying levels of expertise reviewed recordings from both tests to identify spontaneous nystagmus. Results: Of 104 participants, 56.7% exhibited spontaneous nystagmus detected by videonystagmography. The low-cost goggles demonstrated a concordance of k= 0.60 for nystagmus detection and k= 0.47 for classification when used by the less experienced clinician. The more experienced clinician achieved a concordance of k= 0.82 for detection and k= 0.73 for classification. Conclusion: While clinical assessments can be conducted without specialized equipment, videonystagmography offers a comprehensive and detailed evaluation, making it the preferred and widely utilized method. Nevertheless, low-cost goggles enable the detection of spontaneous nystagmus, demonstrating satisfactory agreement with videonystagmography.
Introducción: Debido a su precisión y simplicidad, la videonistagmografía es la técnica más utilizada para la evaluación de movimientos oculares en pacientes con queja vestibular. Sin embargo, su alto costo limita su utilización. Frente a esto, el desarrollo de gafas de bajo costo se posiciona como una alternativa al equipo tradicional de videonistagmografía. Objetivo: El objetivo de este estudio fue comparar el rendimiento entre gafas de bajo costo y videonistagmografía en la evaluación del nistagmo espontáneo. Material y Métodos: Realizamos un estudio transversal en el Servicio de Otorrinolaringología de la Red UC Christus. Los pacientes derivados para evaluación vestibular fueron evaluados con video-nistagmografía y con un prototipo de gafas de bajo costo. Dos clínicos cegados con distinta experiencia revisaron las grabaciones de ambas pruebas en busca de nistagmo espontáneo. Resultados: De 104 participantes, el 56,7% presentó nistagmo espontáneo detectado por videonistagmografía. Las gafas de bajo costo mostraron una concordancia de k= 0,60 para la detección del nistagmo y k= 0,47 para la clasificación cuando fueron utilizadas por el clínico menos experimentado. El clínico más experimentado logró una concordancia de k= 0,82 para la detección y k= 0,73 para la clasificación. Conclusión: Aunque las evaluaciones clínicas pueden realizarse sin equipo especializado, la videonistagmografía proporciona una evaluación integral y detallada, convirtiéndola en el método preferido y ampliamente utilizado. Sin embargo, las gafas económicas permiten la detección de nistagmo espontáneo, mostrando un acuerdo satisfactorio con la videonistagmografía.

Introducción: El traumatismo craneoencefálico (TCE) puede generar vértigo, mareo e inestabilidad. Posibles causas otorrinolaringológicas son el vértigo postural paroxístico benigno (VPPB) que constituye el diagnóstico más frecuente, y la hipofunción vestibular. Objetivo: Describir la prevalencia de hipofunción vestibular en un grupo de pacientes con VPPB asociado a TCE. Material y Método: Estudio retrospectivo de pacientes con VPPB asociado a TCE que requirieron maniobra de reposición (MRP) entre los años 2017 y 2021. La información clínica, características clínico-demográficas, hallazgos en pruebas de función vestibular y número de MRP fueron evaluados. Resultados: Se incluyeron 48 pacientes con una edad promedio de 60,8 ± 16,5 años, siendo un 52% mujeres. La prevalencia de pacientes con paresia vestibular concomitante correspondió al 35,4%. Al comparar al grupo con y sin paresia se observó: (1) en el grupo con paresia fue, significativamente, más frecuente presentar contusión cerebral asociada, 47,1% vs 12,9%; (2) el sexo masculino fue, significativamente, más frecuente en el grupo con paresia, 70,59% vs 35,5%; (3) en ambos grupos, la mediana de MRP fue 1. Conclusión: La presencia de paresia vestibular en pacientes con VPPB secundario a TCE, no es un hallazgo infrecuente, en nuestro estudio, correspondió a un 35,4%, siendo este más frecuente en hombres. Adicionalmente, la contusión cerebral asociada es más frecuente en el grupo con paresia.
Introduction: Head trauma can generate vertigo, dizziness and instability. Possible otorhinolaryngologic causes are benign paroxysmal postural vertigo (BPPV), which is the most frequent diagnosis, and vestibular hypofunction. Aim: To describe the prevalence of vestibular hypofunction in a group of patients with BPPV associated with head trauma. We studied the clinical characteristics, vestibular function test findings and the number of (PRM). Material and Method: Retrospective study of patients with BPPV associated with head trauma who underwent particle repositioning maneuvers (PRM) during the years 2017 to 2021. Clinical characteristics, vestibular function test findings and the number of PRM were evaluated. Results: 48 patents were included. The mean age was 60.8 ± 16.5 years old, 52% were women. The prevalence of patients with concomitant vestibular paresis was 35.4%. When comparing the groups with and without paresis the following was observed: (1) associated brain contusions were significatively more frequent in the paresis group, 47.1% vs 12.9%; (2) male sex was significatively more frequent in the paresis group, 70.59% vs 35.5%; (3) in both groups, the median of needed PRM was 1. Conclusion: The presence of vestibular paresis in patients with BPPV secondary to head trauma is not an infrequent finding. In our study, its prevalence was 35.4%, being significatively more frequent in men. Also, associated brain contusions were significatively more frequent in the paresis group.

El vértigo posicional paroxístico benigno (VPPB) es un síndrome vestibular episódico (SVE) que es reconocido por ser el trastorno más frecuente observado en la clínica, siendo de buena y pronta resolución en la gran mayoría de los casos. Sin embargo, pueden presentarse variantes muy poco habituales o atípicas, donde el canalith jam es una de las formas más resistentes al tratamiento mediante maniobras de reposición, y por lo mismo, el reconocimiento adecuado de este cuadro es esencial para su correcto abordaje. Se presentan dos casos de VPPB con canalith jam en el CSC horizontal y se proponen cinco criterios diagnósticos para su identificación.
Benign paroxysmal positional vertigo (BPPV) is an episodic vestibular syndrome (EVS) that is recognized for being the most frequent disorder observed in the clinic, with good and prompt resolution in the vast majority of cases. However, very unusual or atypical variants can occur, where the canalith jam is one of the forms most resistant to treatment by means of repositioning maneuvers, and for the same reason, the adequate recognition of this condition is essential for its correct approach. Two cases of BPPV with canalith jam in the horizontal semicircular canal and five diagnostic criteria for its identification are presented.

ntrodução: A reabilitação vestibular é um tratamento para tontura crônica que utiliza exercícios personalizados visando restaurar o controle postural e reduzir a tontura. Pouco se discute na literatura sobre os benefícios em longo prazo desta intervenção. Objetivos: Descrever o perfil dos pacientes atendidos no Ambulatório de Reabilitação Vestibular e verificar a melhora do equilíbrio corporal após a alta fonoaudiológica. Métodos: Foram colhidas informações acerca dos dados sociodemográficos, diagnóstico, tratamento anterior e queixas existentes. As informações foram obtidas por contato telefônico e acesso aos prontuários. Os dados foram analisados estatisticamente utilizando nível de significância de 5%. Resultados: Participaram 26 indivíduos, sendo 21 (80,8%) do gênero feminino, com média de idade de 67 anos. A queixa principal foi tontura não rotatória. O resultado do teste vestibular mais comum foi hipofunção vestibular unilateral. Dentre os entrevistados, 25 (96,2%) relataram melhora dos sintomas com o tratamento, com redução da pontuação obtida no Dizziness Handicap Inventory. Sete participantes (26,9%) permaneceram assintomáticos desde o término da reabilitação. Aqueles que relataram ainda sentirem tontura, descreveram que esta possui menor intensidade que no período anterior à intervenção.Conclusão: Houve prevalência de indivíduos do gênero feminino, idosos, com ensino fundamental incompleto, sem diagnóstico otoneurológico estabelecido, com queixa de tontura não rotatória e resultado do teste vestibular de hipofunção vestibular unilateral.A reabilitação vestibular foi eficaz para redução dos sintomas apresentados. A exposição sucessiva aos exercícios após o tratamento auxilia na manutenção do equilíbrio. Contudo, a adesão à realização dos exercícios após a alta ainda é baixa. (AU)
Introduction: Vestibular rehabilitation is a treatment for chronic dizziness that uses personalized exercises aimed at restoring postural control and reducing dizziness. There is little discussion in the literature about the long-term benefits of this intervention. Objectives: To describe the profile of patients seen at the Vestibular Rehabilitation Outpatient Clinic and verify body balance improvement after speech-language-hearing therapy discharge. Methods: Sociodemographic data, diagnosis, previous treatment, and existing complaints were collected. The information was obtained via phone calls and medical records. The data were statistically analyzed using a significance level of 5%. Results: 26 individuals participated, of whom 21 (80.8%) were female, with a mean age of 67 years. The main complaint was non-rotational dizziness. The most common vestibular test result was unilateral vestibular hypofunction. Among the interviewees, 25 (96.2%) reported improved symptoms after the treatment, with reduced Dizziness Handicap Inventory scores. Seven participants (26.9%) remained asymptomatic since the end of rehabilitation. Those who still reported dizziness described it as less intense than before the intervention. Conclusion: There was a prevalence of females, older adults with incomplete middle school, no established otoneurological diagnosis, complaint of non-rotational dizziness, and vestibular test results of unilateral vestibular hypofunction. Vestibular rehabilitation effectively reduced the symptoms. Successive exposure to exercises after treatment helps maintain balance. However, adherence to exercise after discharge is still low. (AU)

Hay poco debate en la literatura sobre los beneficios a largo plazo de esta intervención. Objetivos: Describir el perfil de los pacientes atendidos en el Ambulatorio de Rehabilitación Vestibular y verificar la mejora del equilibrio corporal después del alta fonoaudiológica. Métodos: Se recopilaron información sobre datos sociodemográficos, diagnóstico, tratamiento previo y quejas que aún persistían. La información se obtuvo por contacto telefónico y acceso a los registros médicos. Los datos se analizaron estadísticamente utilizando un nivel de significación del 5%. Resultados: Participaron 26 individuos, siendo 21 (80,8%) del género femenino, con una edad promedio de 67 años. La queja principal fue vértigo no rotatorio. El resultado del examen vestibular más común fue hipofunción vestibular unilateral. Entre los entrevistados, 25 (96,2%) informaron una mejora en los síntomas con el tratamiento, con una reducción en la puntuación obtenida en el Dizziness Handicap Inventory. Siete participantes (26,9%) permanecieron asintomáticos desde el final de la rehabilitación. Aquellos que informaron que todavía experimentaban vértigo describieron que este tenía una intensidad menor que en el período anterior a la intervención. La migraña vestibular es una de las etiologías más frecuentes del síndrome vestibular episódico a nivel mundial. Presenta varias hipótesis de patofisiología, principalmente a nivel de sistema vestibular central y genético. Su diagnóstico es fundamentalmente clínico, pero se han observado alteraciones a nivel de la función vestibular y de las pruebas oculomotoras. Los hallazgos clínicos no solo están presentes en el momento de la crisis, sino también se han observado en intervalos asintomáticos. La paresia unilateral en la prueba calórica suele ser más frecuente que una ganancia baja del reflejo vestíbulo-ocular del canal lateral en la videonistagmografía, sin embargo, existe una tasa elevada de discordancia entre ambas pruebas. Respecto a la prueba de estudio del movimiento ocular, se han observado alteraciones en el seguimiento pendular, movimiento sacádico, nistagmo optocinético, nistagmo evocado por la mirada, nistagmo espontáneo y nistagmo posicional. Es frecuente observar que el nistagmo provocado por cambios posicionales presenta características centrales durante la crisis de migraña vestibular, pero también se pueden presentar en periodos libres de síntomas en este grupo de pacientes.
Vestibular migraine is one of the most frequent etiologies of episodic vestibular syndrome worldwide. It presents several pathophysiology hypotheses, mainly at the central vestibular system and genetic level. Its diagnosis is fundamentally clinical, but changes in vestibular function and oculomotor tests have been observed. Clinical findings are present not only during the crisis, but also have been seen in the symptom-free interval. Unilateral paresis on caloric testing is usually more common than low gain of the vestibulo-ocular reflex in the lateral canal on videonystagmography, however, there is a high rate of discrepancy between these tests. Regarding the eye movement study test, alterations have been seen in pendulum tracking, saccadic movement, optokinetic nystagmus, gaze-evoked nystagmus, spontaneous nystagmus and positional nystagmus. It is common to observe that nystagmus caused by positional changes has central features during vestibular migraine attacks, but it can also be seen in the symptom-free interval in this group of patients.

Dizziness or vertigo is a common clinical symptom, and its underlying etiology is complex. Many clinicians are confused about its diagnosis and treatment. This article presents a case about chronic vestibular syndrome. And case appreciation and academic discussion are conducted by well-known domestic neurologists and otologists, so as to provide a good thinking model and basic ideas for the diagnosis and treatment of dizziness or vertigo, hoping to further improve the diagnosis and treatment level among clinicians.

Persistent postural-perceptual dizziness(PPPD) is the most common chronic vestibular disease, the clinical manifestation is dizziness, unstable and non-rotational dizziness for three months or more. And the symptom is exacerbated by upright posture, active or passive movement, and complex visual stimuli. In addition, PPPD is a functional disease, so routine vestibular function tests and imaging tests are often negative. According to the diagnostic criteria established by the Barany Association, the diagnosis of PPPD often relies on history. This article provides a review of PPPD-related questionnaires.

Isolated otolith dysfunction(iOD) involves a group of unexplained vestibular syndromes that manifest clinically as a sense of translation, tilting or floating, and blurred vision with head movement, with normal semicircular canal function but abnormal otolith function on laboratory vestibular testing. As vestibular medicine has gained widespread popularity in recent years, increasing attention has also been paid to iOD and case reports, clinical studies and diagnostic criteria have been published. However, there is no consensus document to guide the diagnosis of this disease in China. In this context, the Special Committee on Vertigo of China Medical Education Association organized a group of domestic experts in vestibular medicine and formulated this diagnostic consensus after thorough discussion based on the latest evidence in China and abroad, in order to promote the best clinical practice for iOD.

Objective:To investigate the classification of head shaking nystagmus(HSN) and its clinical value in vestibular peripheral diseases. Methods:Clinical data of 198 patients with peripheral vestibular disorders presenting with HSN were retrospectively analyzed. Video Nystagmograph(VNG) was applied to detect spontaneous nystagmus(SN), HSN, and Caloric Test(CT). The intensity and direction of SN and HSN as well as the unilateral weakness(UW) and direction preponderance(DP) values in caloric test was analyzed in patients. Results:Among the 198 patients with vestibular peripheral disease, there were 105 males and 93 females, with an average age of(49.1±14.4) years (range: 14-87 years). One hundred and thirty seven patients were diagnosed as Vestibular Neuritis(VN), 12 as Meniere's Disease(MD), 41 as sudden deafness(SD) and 8 as Hunt's syndrome accompanied by vertigo. Among them, there were 116 patients in the acute phase, including 68 cases(58.6%) with decreased HSN, 4 cases(3.4%) with increased HSN, 5 cases(4.3%) with biphasic HSN, 38 cases(32.8%) with unchanged HSN, and 1 case(0.9%) with perverted HSN. There were 82 cases in the non-acute phase, 51 cases(62.2%) with decreased HSN, 3 cases(3.6%) with increased HSN, 9 cases(11.0%) with biphasic HSN, and 19 cases(23.2%) with unchanged HSN. In biphasic HSN, the intensity of phase I nystagmus was usually greater than that of phase II, and the difference was statistically significant(P<0.01). There was no correlation between HSN type and course of disease or DP value. The intensity of HSN was negatively correlated with the course of disease(r=-0.320, P<0.001) and positively correlated with DP value(r=0.364, P<0.001), respectively. The intensity of unchanged nystagmus and spontaneous nystagmus were(8.0±5.7) °/s and(8.5±6.4)°/s, respectively. There was no statistically significant difference in the intensity of nystagmus before and after shaking the head. Conclusion:HSN can be classified into five types and could be regarded as a potential SN within a specific frequency range (mid-frequency). Similarly, SN could also be considered as a common sign of unilateral vestibular impairment at different frequencies. HSN intensity can reflect the dynamic process of vestibular compensation, and is valuable for assessing the frequency of damage in peripheral vestibular diseases and monitoring the progress of vestibular rehabilitation.

Objective:To study the frequency of SLC26A4 gene mutation sites in children with enlarged vestibular aqueduct deafness in Yunnan, report the new mutation sites of SLC26A4 gene, further clarify the mutation spectrum of SLC26A4gene, and explore the association between biallelic and monoallelic mutations of SLC26A4 gene and CT phenotype of inner ear, so as to provide basis for clinical and genetic diagnosis of deafness. Methods:Review the results of temporal bone CT examination of 390 children after cochlear implantation in the Department of Otolaryngology, Kunming Children's Hospital from August 2016 to September 2021. Sanger sequencing of SLC26A4 gene was performed in 59 children with enlarged vestibular aqueduct. According to the genetic test results, the children who underwent temporal bone CT examination were divided into two groups: SLC26A4 biallelic mutation group(homozygous mutation and compound heterozygous mutation), monoallelic mutation group, and the association with inner ear CT phenotype was analyzed, and the new sites were summarized and analyzed. Results:The c.919-2a>g mutation was the most common mutation in children with enlarged vestibular aqueduct with SLC26A4 gene mutation. Three new variants of SLC26A4 gene were found; CT examination combined with genetic testing found that a part of children with enlarged vestibular aqueduct was associated with SLC26A4 monoallelic mutation or no SLC26A4 gene mutation was detected. Further research is needed to investigate the involvement of other pathogenic factors in the pathogenesis of EVA.

La terapia de rehabilitación vestibular es el tratamiento con mayor evidencia en la recuperación para la mayoría de los trastornos de equilibrio. En los casos que presentan una alteración estable del procesamiento central del equilibrio, o mixta, es decir, acompañada de una alteración a nivel del sistema nervioso periférico, la terapia de rehabilitación vestibular no se excluye como tratamiento. No obstante, los progresos suelen ser limitados y requieren de una mayor cantidad de sesiones. En este trabajo analizaremos un caso mixto, un paciente con síndrome de núcleo fastigial y el vértigo posicional paroxístico benigno (VPPB), desde la pesquisa y evaluación hasta el tratamiento y alta, en el Hospital Clínico Universidad de Chile.
Vestibular rehabilitation therapy is the treatment with the greatest evidence of recovery for most balance disorders. In the cases that have a loss of central balance processing, or mixed, that is, stable accompanied by a disorder of the peripheral nervous system the vestibular rehabilitation therapy is not excluded as a treatment; however, progress is usually limited and requires a greater number of sessions. In this work we will analyse a mixed case, a patient with nucleus fastigial syndrome and a benign paroxysmal positional vertigo, from the investigation and evaluation to the treatment and discharge, at the Hospital Clínico Universidad de Chile.

RESUMO Objetivo verificar a relação entre o risco de queda, o nível de confiança nas atividades que envolvem equilíbrio e a idade com os valores de ganho e simetria dos canais semicirculares (CSCs), por meio do Vídeo Teste do Impulso Cefálico (vHIT), em pacientes com disfunção vestibular periférica. Método estudo transversal, composto por 12 indivíduos submetidos à avaliação funcional por meio da Activities-specific Balance Confidance Scale (ABC Scale) e do Dynamic Gait Index (DGI) e avaliação vestibular com o vHIT. Os resultados foram comparados por meio do Coeficiente de Correlação de Spearman. Resultados observou-se predomínio do gênero feminino (75%), com média de idade de 59 anos e 8 meses. A média de escore no DGI foi de 19,70 pontos e 43,65% na ABC Scale, característico de baixo nível de confiança. Observou-se correlação de grau moderado entre os parâmetros idade com o DGI, DGI e ABC Scale com os valores de ganho e simetria dos CSCs do vHIT. Conclusão Observaram-se relações entre o maior risco de queda em pacientes com hipofunção vestibular e idade avançada e entre o baixo nível de confiança para realizar atividades diárias diante da assimetria de ganho dos CSCs.
ABSTRACT Purpose to verify the relationship between the risk of falling, the level of confidence in activities involving balance, and age with the values of gain and symmetry of the semicircular canals (SSCs), using the Head Video Impulse Test (vHIT) in patients with peripheral vestibular dysfunction. Methods Cross-sectional study in 12 individuals submitted to functional evaluation using the Activities-specific Balance Confidence Scale (ABC Scale) and the Dynamic Gait Index (DGI), and vestibular evaluation with vHIT. The results were compared using Spearman's Correlation Coefficient. Results There was a predominance of females (75%), with a mean age of 59 years and 8 months. The mean score in DGI was 19.70 points and on the ABC Scale, 43.65%, which is characteristic of a low confidence level. A moderate correlation was observed between age and DGI and between DGI and ABC Scale and SCC gain and symmetry values with vHIT. Conclusion A relationship was observed between a higher risk of falling in patients with vestibular hypofunction and advanced age and between a low level of confidence to perform daily activities due to asymmetrical SCC gain.

RESUMO Objetivo Avaliar a efetividade da reabilitação vestibular na funcionalidade, ganho dos canais semicirculares ao Video Head Impulse Test (v-HIT), nas respostas ao Potencial Evocado Miogênico Vestibular (VEMP), sintomas depressivos, equilíbrio e tontura em idosos. Métodos Estudo longitudinal, quase experimental e analítico, realizado entre dezembro de 2019 e julho de 2022, com 50 idosos com disfunção vestibular, submetidos à reabilitação vestibular e à aplicação dos exames VEMP, v-HIT, dos questionários e escalas Escala Visual Analógica, Dizziness Handicap Inventory e Escala de Equilíbrio de Berg, além do Questionário de Atividades Funcionais de Pfeffer e Escala de Depressão Geriátrica. Todos os exames e questionários foram aplicados antes e após oito sessões semanais de reabilitação vestibular. A análise estatística foi realizada pelo programa SPSS, por meio do teste Wilcoxon (p<0,05). Resultados Observou-se melhora do ganho do canal semicircular anterior direito (0,71/0,78), redução do incômodo da tontura (7/5), do impacto da tontura na qualidade de vida (35/15), melhora do equilíbrio (45/51), além de melhora da funcionalidade (2/1) e redução dos sintomas depressivos (5/3). Conclusão Idosos com disfunção vestibular submetidos à reabilitação vestibular apresentaram melhora do ganho do reflexo vestíbulo-ocular, do equilíbrio, da funcionalidade, redução do impacto da tontura na qualidade de vida e dos sintomas depressivos.
ABSTRACT Purpose To assess the effectiveness of vestibular rehabilitation on functioning, gains in semicircular canals in the Video Head Impulse Test (v-HIT), on responses to the vestibular evoked myogenic potentials (VEMP), depressive symptoms, balance, and dizziness in older adults. Methods Longitudinal, quasi-experimental, analytical study conducted between December 2019 and July 2022, in 50 older adults with vestibular disorders, submitted to vestibular rehabilitation and examinations: VEMP, v-HIT, the questionnaires, visual analog scale, Dizziness Handicap Inventory, Berg Balance Scale, Pfeffer's Functional Activities Questionnaire, and Geriatric Depression Scale. All examinations and questionnaires were applied before and after eight weekly vestibular rehabilitation sessions. Statistical analysis was performed in SPSS, with the Wilcoxon test (p<0.05). Results There was a gain in the anterior right semicircular canal (0.71/0.78), reduction in dizziness discomfort (7/5) and in the impact of dizziness on the quality of life (35/15), improvement in balance (45/51) and functioning (2/1), and reduction in depressive symptoms (5/3). Conclusion Older adults with vestibular disorder submitted to vestibular rehabilitation improved their gain in vestibulo-ocular reflex, balance, and functional activities and reduced their depressive symptoms and the impact of dizziness on their quality of life.

Estratégia de pesquisa: trata-se de uma revisão integrativa de literatura realizada entre julho e outubro de 2020, os artigos foram selecionados por meio das principais bases de dados da saúde MEDLINE via PubMed, LILACS via Portal Regional da BVS; Cochrane, Scopus, Web of Science, e CINAHL acesso via Portal CAPES, utilizando os descritores "Cognition" OR "Cognitive Dysfunction" AND "Vestibular Function Tests" AND "Vertigo" AND "Vestibular Rehabilitation" e seus correlatos em português e espanhol. Critérios de seleção: Foram incluídos artigos publicados até 2020 que investigaram a cognição com avaliação e reabilitação vestibular (tradicional e/ou com tecnologias) em indivíduos acima de 18 anos. Foram excluídos artigos que não possuíam texto completo disponível ou que utilizaram outra forma de tratamento. Resultados: dos 6965 artigos resultantes da busca inicial, 16 foram incluídos na presente revisão por satisfazerem os critérios de inclusão. Introduction: The literature reports an association between imbalance and cognitive impairment. However, it is not yet clear which cognitive skills are involved with the vestibular system. Objective: To evaluate which cognitive skills are involved in vestibular assessment and rehabilitation in young and older adults. Research strategy: This is an integrative review of the literature, conducted between July and October 2020. The articles were selected through search in the main health databases - MEDLINE via PubMed, LILACS via Regional Portal of VHL, Cochrane, Scopus, Web of Science, and CINAHL, accessed via Portal CAPES, using the following descriptors "Cognition" OR "Cognitive Dysfunction" AND "Vestibular Function Tests" AND "Vertigo" AND "Vestibular Rehabilitation", and their equivalent terms in Portuguese and Spanish. Selection criteria: Articles published until 2020, investigating cognition with vestibular assessment or traditional and/or technology rehabilitation in subjects aged 18 years or older were included. Articles not available in full text or that used other types of treatment were excluded. Results: 16 out of the 6,965 articles initially retrieved met the inclusion criteria and were included in this review; 12 of them are cross-sectional, and four longitudinal studies. Conclusion: There was a relationship between uni- and bilateral vestibular dysfunction and working memory, executive functions, spatial navigation, and attention. The studies that conducted vestibular rehabilitation found improved overall cognitive skills, visuospatial capacity, attention, executive functions, spatial working memory, increased vestibulo-ocular reflex, postural control gains, and diminished psychological suffering.

Conclusión: hubo relación entre la disfunción vestibular uni y bilateral con la memoria de trabajo, funciones ejecutivas, navegación espacial y atención. Abstract Introduction: Bilateral vestibulopathy is a rare chronic condition with multiple etiologies. Bilateral vestibulopathy is characterized mainly by unsteadiness when walking or standing, which worsens in darkness, as well as oscillopsia. The degree of handicap caused by bilateral vestibulopathy is variable and remains controversial. Objectives: To determine the value of the video Head Impulse Test in quantifying vestibular deficit and to establish its impact on the quality of life. Methods: Twenty patients (mean age, 41.9 years; range 14-80 years) fulfilling the recent Barany criteria of bilateral vestibulopathy, responded to the Situational Vertigo Questionnaire and underwent vestibular examination including fixation, positional tests, oculomotor test battery and video head impulse test. Results: The relation between each of the video head impulse test parameters and the scores from the questionnaire were statistically analyzed. We observed that patients with covert saccades on the video head impulse test were more likely to have a better quality of life than those with both covert and overt saccades, regardless of the vestibulo-ocular reflex gain in each semicircular canal. The presence of covert saccades was found to be associated with an improved quality of life regardless of the severity of vestibule ocular reflex-deficit. Our conclusion was that vestibule ocular reflex gain, measured by video head impulse test, does not quantify the severity of affection of quality of life in patients with bilateral vestibulopathy. Conclusion: Covert saccades are strategies aiming at minimizing the blurring of vision during head movement, that is an adaptive mechanism that improves quality of life. Therefore, we recommend that video head impulse test should be a part of the routine diagnostic workup of bilateral vestibulopathy.
Resumo Introdução: A vestibulopatia bilateral é uma condição crônica rara, com múltiplas etiologias. É caracterizada principalmente por instabilidade ao caminhar ou ficar de pé, que piora na escuridão, e oscilopsia. O grau de deficiência causado pela vestibulopatia bilateral é variável e permanece controverso. Objetivos: Determinar o valor do teste do impulso cefálico na quantificação do déficit vestibular e estabelecer seu impacto na qualidade de vida. Método: Vinte pacientes (média de 41,9 anos; variação de 14 a 80) que atendiam aos critérios recentes da Bárány Society de vestibulopatia bilateral responderam ao Situational Vertigo Questionnaire e foram submetidos a exame vestibular, inclusive fixação, testes posicionais, bateria de testes oculomotores e teste do impulso cefálico com vídeo. Resultados: A relação entre cada um dos parâmetros do teste do impulso cefálico com vídeo e os escores do questionário foram analisados estatisticamente. Observamos que pacientes com sacadas corretivas cobertas do tipo covert no teste de impulso cefálico com vídeo tinham maior probabilidade de ter melhor qualidade de vida do que aqueles com ambas sacadas corretivas cobertas e sacadas corretivas abertas do tipo overt, independentemente do ganho no reflexo vestíbulo-ocular em cada canal semicircular. Verificou-se que a presença de sacadas corretivas do tipo covert está associada a uma melhor qualidade de vida, independentemente da gravidade do déficit no reflexo vestibulo-ocular. Concluímos que o ganho no reflexo vestíbulo-ocular, medido pelo teste do impulso cefálico com vídeo, não quantifica a gravidade do comprometimento da qualidade de vida em pacientes com vestibulopatia bilateral. Conclusão: As sacadas corretivas do tipo covert são estratégias que visam minimizar o embaçamento da visão durante o movimento da cabeça, ou seja, um mecanismo adaptativo que melhora a qualidade de vida. Portanto, recomendamos que o teste do impulso cefálico com vídeo faça parte da rotina de diagnóstico da vestibulopatia bilateral.

RESUMO Esta pesquisa teve como objetivo caracterizar o efeito da reabilitação vestibular (RV) sobre o ganho do reflexo vestíbulo-ocular (RVO), a ocorrência das sacadas compensatórias, bem como sobre o equilíbrio corporal e a qualidade de vida, em três pacientes com hipofunção vestibular periférica. Trata-se de um estudo descritivo. Participaram da pesquisa três pacientes do gênero feminino, duas com 55 anos e uma com 67 anos, com diagnóstico médico de disfunção vestibular periférica. As participantes foram submetidas à anamnese, questionário Dizziness Handicap Inventory (DHI), avaliação clínica do equilíbrio corporal e ao Vídeo Teste do Impulso Cefálico (vHIT), pré e após RV. A RV foi aplicada de forma personalizada, baseada no protocolo de Cawthorne e Cooksey, associada a estímulos de realidade virtual. Após a RV, observou-se a redução da média do escore total do DHI, sugestivo da diminuição na restrição de participação. Na avaliação clínica do equilíbrio obtiveram-se resultados dentro da normalidade para as provas alteradas, pré RV. Quanto ao vHIT, constatou-se aumento do ganho do RVO para os canais semicirculares anteriormente afetados, condizente com padrões de normalidade, e extinção ou diminuição de ocorrência das sacadas compensatórias, nos três casos avaliados. O aumento do ganho do RVO e a extinção ou redução das sacadas compensatórias, após a RV, evidenciam sinais sugestivos de compensação vestibular. Esses resultados mostraram-se compatíveis com o aumento da estabilidade postural e menor restrição da qualidade de vida. Os achados, em conjunto, demonstram o benefício proporcionado pela RV nos três casos avaliados.
ABSTRACT The purpose of this research is to characterize the vestibular rehabilitation (VR), vestibulo-ocular reflex (VOR) gain, the occurrence of compensatory saccades, the static and dynamic balance, and the impact on quality of life in three patients with peripheral vestibular hypofunction. This is a descriptive study, approved by the ethics in research committee, under number 4,462.519. Three female patients participated in the study, two aged 55 and one aged 67, with a medical diagnosis of peripheral vestibular dysfunction. The participants underwent anamnesis, Dizziness Handicap Inventory (DHI) questionnaire, clinical assessment of postural balance and Video Head Impulse Test (vHIT), pre and post VR. The VR was applied in a personalized manner, based on the Cawthorne and Cooksey protocol, associated with virtual reality stimuli. After VR, a reduction in the total score average of DHI was observed, suggesting a decrease in participation restriction. The clinical balance assessment results were within the normal range for the altered tests, pre VR. In the three evaluated cases, vHIT showed increased RVO gain for the previously affected semicircular ducts (SCDs), compatible with normality standards, and reduction or extinction in the occurrence of compensatory saccades. The increase in VOR gain and the reduction or suppression of compensatory saccades after VR are suggestive signs of vestibular compensation. These results were compatible with increased postural stability and less restricted quality of life. These findings demonstrate the benefit provided by VR in the three evaluated cases.

RESUMO Objetivo avaliar se a posturografia, exame que avalia a habilidade de manter o equilíbrio em condições sensoriais conflitantes, pode identificar risco de queda em idosos com tontura. Métodos comparar os resultados posturográficos de idosos com e sem história de quedas, pareados por gênero, idade e diagnóstico etiológico da tontura. Resultados dezoito idosos com quedas - 4 com único episódio e 14 com história de 2 ou mais quedas no último ano - foram comparados com 18 idosos sem quedas, pareados por gênero, idade e diagnóstico etiológico. Pacientes com quedas apresentaram resultados piores para as análises de dependência visual (p=0,04, p=0,01, p=0,03). Pacientes com quedas recorrentes (2 ou mais episódios) apresentaram piores resultados em diversas condições sensoriais: somatossensorial, vestibular, dependências visuais e índice de equilíbrio composto. Conclusão a posturografia mostrou-se útil na identificação de idosos com quedas, principalmente em indivíduos com quadros recorrentes.
ABSTRACT Purpose the aim of this study is to determine whether posturography, an exam used to investigate the ability to maintain balance under conflicting sensory conditions, can identify the risk of falls in eldery patients with dizziness. Methods to compare the posturographic results of elderly people with falls vs elderly people with no falls, paired by sex, age and dizziness etiology. Results 18 fallers, and, of these, 14 with two or more falls in the last year were compared with 18 elderly people without falls. Comparing subjects without falls vs subjects with at least one fall in the last year, fallers obtain worse scores in conditions of visual dependence. Comparing non fallers with subjects with two or more falls, people with recurrent falls obtain worse score in several conditions: somatosensorial, vestibular, visual conflict, and in the main measure, the composite score. Conclusion posturography appears to be a useful tool to identify those at high risk of recurrent falls.

Introducción: El vértigo, los mareos y el desequilibrio se encuentran entre las quejas más comunes de los adultos mayores pudiendo ocasionar repercusiones biopsicosociales como aislamiento, depresión y disminución de la autonomía. Objetivo: El objetivo del presente estudio fue determinar el impacto de la autopercepción de la discapacidad vestibular y de la sintomatología asociada sobre la calidad de vida de adultos mayores autovalentes, residentes en la comunidad y sin patología vestibular diagnosticada. Material y Método: Se realizó un estudio observacional, de alcance analítico y de corte transversal. Participaron 30 sujetos adultos mayores sanos, residentes en zonas urbanas y rurales de la Región Metropolitana. Se utilizó el dizziness handicap inventory para medir la autopercepción de dificultad vestibular y el cuestionario WHOQOL-BREF para medir calidad de vida. Resultados: Ajustando por edad, escolaridad, zona de residencia y desempeño cognitivo, el puntaje del dizziness handicap inventory predijo de manera significativa (p < 0,05) la dimensión física, psicológica y el puntaje total del WHO-QOL-BREF, explicando hasta un 35%, 9% y 16% de los puntajes, respectivamente. Conclusión: La autopercepción de la dificultad vestibular afecta significativamente la dimensión de salud física y la salud psicológica de la calidad de vida, independiente de la edad, la escolaridad, la zona de residencia y el desempeño cognitivo.
Introduction: Vertigo, dizziness and imbalance are among the most common complaints of the elderly, and can cause biopsychosocial impacts such as isolation, depression and decreased autonomy. Aim: The aim of the present study was to determine the impact of self-perception of vestibular disability and associated symptoms on the quality of life of self-reliant, community residents and without diagnosed vestibular disorders older adults. Material and Method: An observational, analytical, and cross-sectional study was carried out. Thirty healthy elderly subjects participated, residing in urban and rural areas of the Metropolitan Region. The dizziness handicap inventory was used to measure self-perception of vestibular disability and the WHOQOL-BREF questionnaire to measure quality of life. Results: Adjusting for age, schooling, area of residence and cognitive performance, the dizziness handicap inventory score significantly predicted (p < 0.05) the physical and psychological dimension and the total score of the WHOQOL-BREF, explaining up to 35%, 9% and 16% of the scores, respectively. Conclusion: The self-perception of vestibular difficulty significantly affects the dimension of physical and psychological health of quality of life, independent of age, schooling, area of residence and cognitive performance.

Abstract Introduction: Caloric testing is the most frequently used test to assess peripheral vestibular function since the beginning of the 20th century. However, the video head impulse test, vHIT, has gained prominence in the field of neurotology, as it is a faster examination, easier to perform and less uncomfortable for the patient. Objective: To compare, through systematic review and meta-analysis, the proportion of altered cases between vHIT tests and caloric testing in patients with chronic dizziness, in addition to assessing the sensitivity and specificity of vHIT, with caloric testing as the gold standard. Methods: The literature search was carried out in the PubMed, Scopus, BVS-Bireme, Web of Science, Embase, Cochrane and ProQuest indexed databases, with no restrictions regarding the publication period. All articles that contained the results of the two tests were included in the evaluation of patients with dizziness. Two researchers independently conducted data selection and extraction from the studies, strictly following the inclusion and exclusion criteria defined in the research protocol. In case of disagreement during the selection, a discussion was carried out with a third evaluator. Results: Eleven of the 1293 initial articles met the eligibility criteria and were analyzed. 2670 patients were evaluated, of which 1112 (41.6%) were males and 1558 (58.4%) females, with a mean age of 51.6 years. The proportion of altered results in the vHIT was 21% (95%CI 9% --33%), and 55% in the caloric testing (95%CI 43% --67%). Conclusion: The vHIT does not substitute for caloric testing. The tests are complementary in assessing the patient with dizziness, as they describe the tonotopy of the ampullary crest at different frequency ranges of stimulation. In chronic cases, the vHIT has a low sensitivity and high diagnostic specificity in comparison to caloric testing.
Resumo Introdução: A prova calórica é o exame mais usado para avaliação da função vestibular periférica desde os primórdios do século XX. Porém, o vídeo teste de impulso cefálico, vHIT, tem ganhado destaque no campo da otoneurologia por ser um exame mais rápido, de fácil execução e menos desconfortável para o paciente. Objetivo: Comparar, através de revisão sistemática e metanálise, a proporção de casos alterados entre os exames vHIT e prova calórica nos pacientes com tontura crônica, além de avaliar a sensibilidade e especificidade do vHIT, tendo a prova calórica como padrão-ouro. Método: A busca na literatura foi feita nas bases de dados indexadas PubMed, Scopus, BVS-Bireme, Web of Science, Embase, Cochrane e ProQuest, sem restrições quanto ao período da publicação. Foram incluídos todos os artigos que tivessem os resultados dos dois exames na avaliação de pacientes com tontura. Dois pesquisadores conduziram de forma independente a seleção e extração de dados dos estudos, obedeceram rigorosamente os critérios de inclusão e exclusão definidos no protocolo de pesquisa. Em caso de discordância na seleção, fez-se discussão com um terceiro avaliador. Resultados: Onze dos 1.293 artigos iniciais preencheram os critérios de elegibilidade e foram analisados. Foram avaliados 2.670 pacientes, 1.112 (41,6%) do sexo masculino e 1.558 (58,4%) do feminino, com média de 51,6 anos. A proporção de exames alterados no vHIT foi de 21% (95% IC 9%-33%) e na prova calórica foi de 55% (95% IC 43%-67%). Conclusão: O vHIT não substitui a prova calórica. Ambos os testes são complementares na avaliação do paciente com tontura, pois descrevem a tonotopia da crista ampular em diferentes faixas de frequência de estimulação. Nos quadros crônicos, o vHIT tem baixa sensibilidade e alta especificidade diagnóstica em relação à prova calórica.
