RÉSUMÉ

ABSTRACT A 22-year-old man complained of low visual acuity and pain in his left eye for five days. His ophthalmological examination revealed 2+ anterior chamber reaction and a white, poorly defined retinal lesion at the proximal portion of the inferotemporal vascular arcade. There were retinal hemorrhages in the inferotemporal region extending to the retinal periphery. In addition, venous dilation, increased tortuosity, and ischemic retinal whitening along the inferotemporal vascular arcade were also observed. A proper systemic work-up was performed, and the patient was diagnosed with ocular toxoplasmosis. He was treated with an anti-toxoplasma medication, and his condition slowly improved. Inferior macular inner and middle retinal atrophy could be observed on optical coherence tomography as a sequela of ischemic injury. To our knowledge, this is the first report of combined retinal branch vein and artery occlusion in toxoplasmosis resulting in a striking and unusual macular appearance.


RESUMO Um paciente do sexo masculino, com 22 anos de idade, queixou-se de redução da acuidade visual no olho esquerdo por 5 dias. O exame oftalmológico mostrou reação de câmara anterior 2+ e uma lesão retiniana esbranquiçada, pouco definida, na porção proximal da arcada vascular temporal inferior. Foram observadas hemorragias retinianas na região temporal inferior estendendo-se à periferia, assim como ingurgitamento venoso, aumento da tortuosidade e palidez isquêmica da retina no mesmo quadrante. Exames laboratoriais corroboraram o diagnóstico de toxoplasmose ocular. O paciente melhorou lentamente após tratamento apropriado. Foi evidenciada atrofia da retina macular inferior interna e média à tomografia de coerência óptica, como sequela da isquemia retiniana. Para nosso conhecimento, este é o primeiro relato de oclusão retiniana combinada de ramo arterial e venoso em toxoplasmose ocular, levando a um aspecto fundoscópico atípico e peculiar.

ABSTRACT Background: Systematization of characteristics and morphological changes in the retina, and understanding its role in occlusive diseases involving retinal vessels can contribute to research on the dynamic of the pathological process and to improved understanding of disease treatment. Objective: To research the basic morphometric and structural indicators of the macular region of the retina in patients with central retinal vein occlusion (CRVO). Settings and Design: Ufa Eye Research Institute, retrospective case series. Methods: The basic morphometric and structural indices of the macular region of the retina in 15 patients with CRVO were studied using the optical coherence tomography. Patients with CRVO (15 eyes) were included in Group I (study), and 10 patients with no retinal pathology (10 eyes) were included in Group II (control). Statistical analysis used: For the statistical analysis, the Statistica 6.1 program was used with a parametric method, the Student's test.To consider the probability of statistically significant differences, values beginning with up to p<0.05 were accepted. Results: In Group II, we determined the correct profile of the macula with fovea centralis and a range of retinal thickness from 221.4 ± 10.97 to 355.2 ± 12.17 µm, with an average value of - 299.01 ± 7.56 µm. In Group I mean retinal thickness at all points of the study area ranged from 247.86 ± 39.06 to 494.07 ± 40.22 µm with a gradual thickening of the periphery to the center, reaching a maximum at a distance of 893 µm from the center of the fovea. The average thickness of the retina as a result of edema increased by a multiple of 1.3 and amounted to - 386.97 ± 16.26 µm. Conclusions: The study indicated that the edema is the main morphological substrate, promoting structural changes in the central retina in retinal vein occlusion, and is cystic in nature. Increase in total retinal thickness (up to 386.97 ± 16.26 µm) in this pathology is largely due to the structural changes in zones, covering the îuter plexiform layer - the inner nuclear layer and the nerve fiber layer with the inner limiting membrane. Meanwhile a significant increase in the thickness of the outer plexiform with the inner nuclear layers (up to 94.94 ± 5.08 µm) and nerve fiber layers (up to 54.6 ± 3.26µm) was observed.


RESUMO Introdução: Sistematização característica e alterações morfológicas na retina, compreensão de sua arquitetura em doenças oclusivas de vasos retinianos para contribuir com a melhoria dos estudos sobre as regularidades da dinâmica do processo patológico, diagnóstico e tratamento da doença. Objetivo: Investigar a morfometria de base e indicadores estruturais da região macular da retina em pacientes com oclusão da veia central da retina (CRVO). Definições e design: Ufa Eye Research Institute, série de casos retrospectiva. Métodos: A análise morfométrica de base e índices estruturais da região macular da retina em quinze pacientes com CRVO. Foram estudados pacientes com CRVO (15 olhos), incluídos no Grupo I (em estudo), 10 pacientes com qualquer patologia da retina (10 olhos), incluídos no Grupo II (controle), com o uso da tomografia de coerência óptica. Para a análise estatística utilizou-se o programa Statistica 6.1 usando o método paramétrico - teste de Student. A probabilidade de diferenças estatisticamente significativas foram valores começando com até p <0,05. Resultados: No Grupo II, o perfil correcto da mácula com fovea centralis e uma gama de espessura da retina de 221,4 ± 10,97 a 355,2 ± 12,17µm, com um valor médio de - 299,01 ± 7,56 µm foram determinadas. No Grupo I, a espessura média da retina em todos os pontos da área de estudo variou de 247.86 ± 39.06 a 494,07 ± 40,22µm com um espessamento gradual da periferia para o centro, atingindo um máximo em um raio de 893µm a partir do centro da fóvea.A espessura média da retina, como resultado do edema aumentado em 1,3 vezes e elevou se a - 386,97 ± 16,26 µm. Conclusões: O estudo indicou que o edema é cístic por natureza e é o principal substrato morfológico responsável por mudanças estruturais que promovem a oclusão da veia central da retina. O aumento na espessura total da retina (de 386.97 ± 16.26 µm) nesta patologia deve-se às mudanças estruturais nas zonas que vão da camada plexiforme - nucleo interno, camada de fibras do nervo óptico e membrana limitante Foi observado ainda um aumento significativo da esessura do plexiforme externo com cas camadas nucleaes internas (de 94.94 ± 5.08 µm) e camadas de fibras do nervo óptico (de 54.6 ± 3.26µm).

This study investigated the baseline predictors of best corrected visual acuity (BCVA) and central retinal thickness (CRT) at 6 months in patients with treatment-naive branch retinal vein occlusion (BRVO) and central retinal vein occlusion (CRVO). This multicenter, interventional case series included 208 BRVO and 123 CRVO patients with follow-up period of 6 months or more. Outcome measures of BCVA (logMAR) included absolute change from baseline and a gain or loss of > or = 0.3 from baseline. Outcome measures of CRT included absolute change from baseline and a measurement of or = 400 microm at 6 months. Univariate and multiple regression analyses were done to find baseline predictors. For BRVO, younger age, worse baseline BCVA, and shorter duration of symptom were associated with more gain in BCVA. For CRVO, worse baseline BCVA was associated with more gain in BCVA. For CRT outcomes, higher baseline CRT predicted greater decrease at 6 months in both BRVO and CRVO. Younger age and better baseline BCVA were associated with an increased likelihood of measurement of a or = 400 microm at 6 months. In conclusion, several baseline factors including age, symptom duration, and baseline BCVA and CRT are associated with BCVA and CRT outcomes at 6 months, which may help to predict disease course for RVO patients.

PURPOSE: To determine safety, clinical and visual results, and potential complications of early radial optic neurotomy (RON) surgery in eyes with central retinal vein occlusion (CRVO), with relative afferent pupillary defect and visual acuity MATERIALS AND METHODS: This prospective, interventional case-series included 24 patients of CRVO who underwent RON within 2 months of disease onset. The preoperative examination included slitlamp biomicroscopy, fundus photography and fluorescein angiography. Foveal thickness was measured using optical coherence tomography (OCT) in the last 6 eyes only. In each case, RON was performed after informed consent. Two radial incisions were placed in the nasal quadrant of the optic disc, using a micro-vitreoretinal blade. The postoperative change in vision, clinical picture, fundus photographs, angiograms and foveal thickness by OCT were the main outcome variables studied. The Wilcoxan signed test was used to assess the results. RESULTS: Average symptom duration was 37.8 +/- 15.2 days (range 15-60 days, median: 34.5 days) and follow-up 7.7 +/- 2.1 months (range 1-12 months, median: 8 months). Visual outcome: 2 (8.33%) eyes each had fall and preservation of pre-RON visual acuity respectively. Twenty eyes (83.33%) showed increase in vision (of average 3 lines). Pre and postoperative vision ranged from 0.017-0.1 (average:0.061) and 0.017-0.667 (average: 0.17) respectively (P < 0.05). Clinical and angiographic outcome: decline in macular oedema, decreased or resolved intraretinal haemorrhages, resolution of venous dilatation and disc oedema could be appreciated in all cases. Foveal thickness: average pre and postoperative foveal thickness was 834.17 microm and 556.17 microm respectively (P < 0.05) in the 6 eyes where it was measured before and after RON. One eye developed retinal-detachment. CONCLUSION: Radial optic neurotomy is better than the natural course in eyes with CRVO, with vision < 6/60.

PURPOSE: There is currently no treatment proven effective for central retinal vein occlusion (CRVO). Radial optic neurotomy (RON) has recently surfaced as a new treatment for the disorder, however, and we compare here the visual acuity (VA) and arteriovenous transit time (AVTT) following RON and panretinal photocoagulation (PRP). METHODS: We conducted a retrospective, uncontrolled study of 27 patients. Of 27 eyes diagnosed with CRVO, 16 were treated with panretinal photocoagulation (PRP) and 11 with RON. VA and fluorescein angiography were used to monitor the evolution of CRVO, and for follow-up. RESULTS: All patients underwent PRP or RON with no major complications. The difference between pre- and post-operative VA was not statistically significant for either group (p=0.092 on PRP; p=.0081 on RON). The change in AVTT was also not statistically significant for either group (p=0.024 on PRP; p=0.027 on RON). Ultimately, we found no statistically-significant difference in comparing VA and AVTT for the two groups (p=0.074 on VA; p=0.510 on AVTT). CONCLUSIONS: No evidence supporting the effectiveness of RON could be found in this study, suggesting that surgical decompression by RON may not be effective for treating CRVO. Further studies regarding the efficacy of RON should be made.

Objetivo: descrever um método experimental de produçäo de oclusäo venosa retiniana, näo-invasivo, em coelhos, por princípio fotoquímico, utilizando-se rosa bengala intra-venosa e o laser Nd-YAG verde. Materiais e métodos: foram submetidos, à fototrombose, 74 olhos de 37 coelhos albinos, utilizando o rosa bengala como corante orgânico fotoestimulador e o laser Nd-YAG verde, de comprimento de onda 532 nm. Destes, 62 olhos foram submetidos à retinografia fluorescente para demostrar a trombose clinicamente. 12 olhos foram submetidos a estudo anatomo-patológico para evidenciar o trombo intraluminal. Resultados: a oclusäo venosa foi comprovada fluoresceinograficamente em 57 dos 62 olhos submetidos à fototrombose venosa por mecanismos fotoquímicos

RÉSUMÉ

Os autores apresentam o caso de uma paciente de 35 anos de idade com fenômenos trombóticos envolvendo artérias e veias de extremidades, além da artéria retiniana. Os eventos trombóticos ocorreram numa seqüência de alguns meses, envolvendo inicialmente as artérias femoral e ilíaca direitas, depois trombose venosa profunda e finalmente o envolvimento vascular retiniano. O anticorpo antifosfolipídio tipo IgG foi positivo. Ressalta-se a importância de pesquisa dos anticorpos antifosfolipídios em adultos jovens com fenômenos trombóticos.
