RESUMO

OBJECTIVE To investigate the risk factors for hypokalemia caused by amphotericin B liposome, and to provide reference for clinical use of drugs. METHODS A retrospective analysis was used to collect the information of patients who used amphotericin B liposome during the hospitalization in First Affiliated Hospital of Hainan Medical College from January 2012 to December 2021. The details of use information about amphotericin B liposome and the potassium supplementation were collected. The patients were divided into hypokalemia group and normal group according to the occurrence of hypokalemia. Univariate and multi-variate Logistic regression analyses were used to analyze the risk factors for hypokalemia induced by amphotericin B liposome. RESULTS Of the 121 patients included in this analysis, 60 patients were in hypokalemia group, 61 patients were in normal group. The following parameters of the hypokalemic group were significantly higher or longer than those of the normal group, such as the maintenance dose, cumulative dose and maximum daily dose (in patients with severe hypokalemia) of amphotericin B liposome, treatment days, the maintained days of hypokalemia, daily dose of potassium supplement (in patients with moderate or severe hypokalemia), the duration of potassium supplement (in patients with moderate hypokalemia). Results of single factor analysis showed that the cumulative dose of amphotericin B liposome ≥200 mg and the duration of treatment ≥5 days were independent risk factors of hypokalemia caused by this drug (P<0.05). Multi-variate analysis results showed that the presence of basic hypokalemia, body weight <50 kg, cumulative dose of amphotericin B liposome ≥200 mg and the duration of treatment ≥5 days were the independent risk factors for hypokalemia caused by amphotericin B liposome (P<0.05). CONCLUSIONS The incidence of hypokalemia caused by amphotericin B liposome is high, the independent risk factors for hypokalemia include cumulative dose ≥200 mg, treatment days ≥5 days, the presence of basic hypokalemia and body weight < 50 kg. It is suggested that serum potassium should be elevated to normal level before amphotericin B liposome treatment, and the level of serum potassium should be monitored during medication to reduce the occurrence of hypokalemia.

RESUMO

Objective:To observe the effect of combining scalp acupuncture with feedback training on muscle tone, motor ability, plantar pressure distribution and joint range of motion of stroke survivors with lower limb spastic paralysis. Methods:A total of 212 stroke survivors were randomly divided into a combination group ( n=70), an exercise group ( n=70) and a scalp acupuncture group ( n=72) according to a random number table. All of the patients received routine medication and 120 minutes of rehabilitation training six days a week for 8 consecutive weeks. The rehabilitation training included guidance on good posture, posture change training, balance training and gait training. The scalp acupuncture and exercise groups were additionally provided with 40 minutes of scalp acupuncture and lower limb intelligent feedback training, while the combination group was given both. Muscle tension, balance and lower extremity movement were quantified before and after the training, and a gait evaluation system was used to detect the plantar pressure distribution on the affected side and the range of motion range of the lower limb joints during walking. Results:After the treatment, significant differences were observed in the average modified Ashworth scores, modified Berg scale scores, Fugl-Meyer lower extremity ratings and modified Barthel Index scores compared with before the treatment. The average scores of the combination group were then significantly better than those of the other two groups. The average load bearing ratios of the affected toes, metatarsal bones Ⅰ-V, medial arch, external arch, medial heel and external heel had improved significantly in all of the groups, with the improvement of the combination group again significantly greater than those of the other groups. The same pattern of improvement was observed in the groups′ average pelvic rotation angles, hip flexion angles, knee flexion angles and ankle dorsiflexion angles. Conclusion:Combining scalp acupuncture with intelligent feedback can significantly improve lower extremity functioning after a stroke. That should improve performance in the activities of daily living of stroke survivors.

RESUMO

OBJECTIVE To study the dynamic changes of drug resistance and infectious distribution of Pseudomonas aeruginosa and guide drug treatment in clinic.METHODS The susceptibility to 12 antibiotics were detected in 758 P.aeruginosa strains.RESULTS The resistance of P.aeruginosa to 12 antibiotics was increased in the past four years. The resistant rate to imipenem and amikacin was below 25%.The infection caused by P.aeruginosa happened frequently in Department for Cadre,Intensive Care Units(ICU) and Department of Respiration.Among 758 strains of P.aeruginosa,87 strains were identified multi-drug resistant and found more frequently in Department for Cadre,ICU,Department of Respiration and Department of Burn.CONCLUSIONS The drug resistance of P.aeruginosa has become a more serious problem than before. The monitor of drug-resistant P.aeruginosa could guide treatment and provided epidemiologic and dynamic changes data for clinic. The infection of multi-drug resistant P.aeruginosa meets most frequently in patients who received antibiotic treatment for a long time and had a long stay in the hospital.
