RESUMO

BACKGROUND:Olfactory dysfunction is an early biological marker of various diseases. However, the neuroimaging mechanism by which olfactory dysfunction occurs following cerebral small vessel disease is unclear. OBJECTIVE:To explore the different neuroimaging mechanisms of olfactory function regulation in patients with cerebral small vessel disease and Parkinson's disease, and explore the potential application value of olfactory function assessment in patients with cerebral small vessel disease. METHODS:Neuropsychological and olfactory tests,high-resolution structural magnetic resonance and resting-state functional magnetic resonance data were collected in 80 patients with cerebral small vessel disease,44 healthy controls and 29 patients with Parkinson's disease.DPABI,SPM12 and SPSS were used to analyze and compare the amplitude of low frequency fluctuation, regional homogeneity and functional connectivity values between the cerebral small vessel disease, control and Parkinson's disease groups.Correlations between the significantly altered resting-state functional magnetic resonance imaging measures and olfactory and cognitive scores were evaluated. RESULTS AND CONCLUSION:Compared with the control group,low-frequency fluctuation amplitude of the right dorsolateral superior frontal gyrus and the regional homogeneity of the left wedge leaf were significantly reduced in the cerebral small vessel disease and Parkinson's disease groups. The right dorsolateral superior frontal gyrus and the left cuneiform lobe are the seed points.Compared with the Parkinson's disease group, the functional connectivity values of the right anterior cunei, inferior temporal gyrus, anterior central gyrus and dorsolateral superior frontal gyrus, left posterior central gyrus and inferior temporal gyrus were significantly enhanced in the control and cerebral small vessel disease groups. The left cuneiform lobe was the seed point.Compared with the control group, the functional connectivity of the left lingual gyrus was significantly weakened in the cerebral small vessel disease and Parkinson's disease groups. The functional connectivity values of the left middle temporal gyrus and the right posterior central gyrus were enhanced in the control group compared with the cerebral small vessel disease and Parkinson's disease group, and that was enhanced in the cerebral small vessel disease group compared with the Parkinson's disease group.Correlation analysis showed that the olfactory score and cognitive score were positively correlated in the cerebral small vessel disease group, and the regional homogeneity of the left wedge lobe was negatively correlated with the Montreal Cognitive Assessment Scale score, while the functional connectivity of left wedge lobe-left middle temporal gyrus in the Parkinson's disease group was positively correlated with the olfactory recognition score, and the functional connectivity values of the left wedge lobe-left posterior central gyrus and left wedge lobe-left lingual gyrus were positively correlated with the olfactory identification score and the total olfactory score, respectively. The regulation of olfactory function in patients with cerebral small vessel disease has a different neuroimaging mechanism from that of olfactory dysfunction in patients with Parkinson's disease. The olfactory function of patients with cerebral small vessel disease is related to cognitive function. It is speculated that the olfactory function following cerebral small vessel disease is a secondary change of brain dysfunction, while olfactory dysfunction following Parkinson's disease is directly caused by abnormal function of olfactory-related brain areas.Olfactory function assessment in patients with cerebral small vessel disease has potential application in predicting cognitive function.

RESUMO

Objective:To establish and validate a nomogram model for early prediction of the risk of acute pancreatitis (AP) progressing to severe acute pancreatitis (SAP).Methods:CT signs and clinical laboratory parameters of 361 AP patients admitted to our Hospital from January 2016 to July 2022 were retrospectively collected. There were 221 males (61.2%) and 140 females (38.8%). According to the Atlantic score, all patients were divided into the SAP group (64 cases) and the non-SAP (NSAP) group (297 cases). Univariate analysis was used to screen out variables with statistically significant differences. Multivariate Logistic regression analysis was used to screen out the independent risk factors of SAP, and finally a nomogram prediction model was established. Receiver operating characteristic (ROC) curve, calibration curve and decision curve (DCA) were used to evaluate the predictive efficacy, accuracy and clinical practicability of the model, and Bootstrap method was used to verify the model internally. Results:Univariate analysis and multivariate Logistic regression analysis showed that pleural effusion ( OR=7.353, 95% CI: 3.344-16.170), posterior pararenal space (PPS) involvement ( OR=3.149, 95% CI: 1.314-7.527), serum creatinine concentration (Cr) ( OR=1.027, 95% CI: 1.017-1.038) and serum calcium concentration (Ca 2+) ( OR=0.038, 95% CI: 0.009-0.166) were independent risk factors for SAP ( P<0.05). A Nomogram model was established based on these four factors. The area under the ROC curve (AUC) of this model was 0.905 (95% CI: 0.869-0.933), indicating high predictive efficiency. Internal verification showed that the model had good accuracy in predicting SAP, and C-index was 0.90. DCA analysis showed that the model had high clinical practicability. Conclusions:The Nomogram model combining pleural effusion, PPS involvement, Cr and Ca 2+ had a good effect on early prediction of SAP, which could provide a new reference tool for clinical diagnosis and treatment.

RESUMO

Objective:To prepare a fluorescent probe Cetuximab-IRDye800CW targeting epidermal growth factor receptor (EGFR) and investigate its application value in surgical navigation of glioblastoma (GBM).Methods:The fluorescence properties of Cetuximab-IRDye800CW were determined by fluorescence spectrophotometer. The specificity of Cetuximab-IRDye800CW bound to GBM cells was verified by Western blot. The competitive binding method of enzyme-linked immunosorbent assay (ELISA) was used to prove whether the probe could achieve tumor targeting by binding to EGFR. Subcutaneous models of 6 nude mice of GBM were divided into experimental group ( n=3; injected with Cetuximab-IRDye800CW) and control group ( n=3; injected with IRDye800CW), and images were obtained at 5 min, 24 h, 48 h and 72 h after injection. Differences of mean fluorescence intensity (MFI) and tumor to background ratio (TBR) between experimental group and control group were compared. In situ models of GBM nude mice were established ( n=6), and MRI and intraoperative navigation were conducted, which were compared with pathological distribution. Independent-sample t test was used to analyze the data. Results:The maximum emission wavelength of Cetuximab-IRDye800CW was 820 nm, which could be received by near infrared fluorescence imaging equipment. Western blot showed that Cetuximab-IRDye800CW was only bound to GBM cells. The competitive binding of ELISA showed that Cetuximab-IRdye800CW could achieve tumor targeting by binding with EGFR. At 5 min, 24 h, 48 h and 72 h after injection of fluorescent materials, the MFI values of experimental group were 109.00±3.81, 73.36±9.93, 55.24±8.82, 37.71±6.11, which were higher than those of control group (91.32±4.17, 42.91±5.39, 25.08±6.05, 8.33±1.00; t values: 4.36-9.40, P values: 0.011-0.049). The TBR of experimental group was higher than that of control group at 24 h and 48 h after injection (24 h: 2.40±0.28 vs 1.57±0.07, t=4.94, P=0.039; 48 h: 2.07±0.12 vs 1.22±0.08, t=9.85, P=0.010). GBM in situ model was successfully constructed and verified by MRI, and the tumor was visualized under the fluorescence device navigation. Pathological distribution of the tumor with HE staining was consistent with fluorescence imaging. Conclusion:Cetuximab-IRDye800CW has fluorescence imaging capability and can identify tumor boundaries in intraoperative navigation of GBM, which has potential clinical application value.

RESUMO

ObjectiveTo evaluate the effect of Astragali Radix (AR)-Angelicae Sinensis Radix (ASR) drug pair on supplementing Qi and activating blood circulation in rats with Qi deficiency and blood stasis and provide a theoretical basis for clinical rational medication and identification and quality control of compound pharmacodynamic substances from the three aspects of characteristic map, identification of pharmacodynamic substances, and comparison of blood components. MethodHigh-performance liquid chromatography (HPLC) was employed to establish the fingerprint of AR∶ASR (3∶1), and ultra-high performance liquid chromatography-Q-Exactive Orbitrap-mass spectrometry (UPLC-Q-Exactive Orbitrap-MS) was employed to analyze the ingredients of the decoction. Adult male Wistar rats with SPF grades were selected and randomly divided into a blank group, a model group, a 3∶1 group, and a 5∶1 group. The rat model of Qi deficiency and blood stasis syndrome was prepared by controlling food intake and swimming in cold water every day. In parallel, each group was given medicine (or water) once a day. The dose of drug groups was 10.2 g∙kg-1, and the model group and blank group were given the same amount of distilled water for 15 d. Animal behavior, body weight, whole blood and plasma viscosity, thymus index, spleen index, the levels of adenosine triphosphate (ATP), adenosine diphosphate(ADP), von willebrand factor (vWF), and ATP/ADP value in serum of rats were recorded. The morphology of vascular endothelium was observed by hematoxylin-eosin (HE) staining and scanning electron microscopy. UPLC-Q-Exactive Orbitrap-MS was used to analyze prototype and metabolic components in serum. ResultThe fingerprint of AR-ASR drug pair (AR-ASR 3∶1) was established. UPLC-Q-Exactive Orbitrap MS identified 49 chemical components in vitro and preliminarily identified 11 prototype components absorbed into blood in vivo. As compared with the blank group, the body mass decreased significantly (P<0.01), the whole blood (high shear, middle shear, and low shear) viscosity and plasma viscosity were significantly increased (P<0.05, P<0.01), the thymus index and spleen index decreased significantly (P<0.05, P<0.01), serum ATP content decreased significantly (P<0.01), ADP content increased significantly (P<0.01), ATP/ADP value decreased significantly (P<0.01), and vWF content increased significantly (P<0.01). The results of HE staining and scanning electron microscopy showed that the vessels were partially damaged, showing the structural disorder of the intima, the bulge, defect, and roughness of the endothelium, and the obvious cell adhesion and migration in the model group. As compared with the model group, the body mass also increased significantly (P<0.01). The results of whole blood and plasma viscosity showed that the whole blood low shear viscosity was significantly decreased in the 3∶1 group (P<0.05). The results of thymus index and spleen index showed that 5∶1 group significantly increased the thymus index of rats (P<0.05). The results of serum ATP and ADP levels showed that the 5∶1 group had more significant effects on ATP and ADP levels (P<0.05), and both groups significantly reduced ATP/ADP values (P<0.01). The results of serum vWF level showed that the vWF content in the 3∶1 group decreased significantly (P<0.05). The results of HE staining and scanning electronic microscopy showed that the damage of vascular endothelium was improved in the treatment group and the structure of intima was neat. ConclusionAR-ASR drug pair can improve the macro and micro indexes of rats with qi deficiency and blood stasis in the 3∶1 and 5∶1 groups. Overall, the 5∶1 ratio has a better effect on supplementing Qi but 3∶1 ratio has a better effect on promoting blood circulation.

RESUMO

Jiawei Xiaoyao San (JWXYS) has shown excellent clinical efficacy in anxiety disorder, but has not yet attracted widespread attention. The animal experiments, clinical trials and mechanism studies of JWXYS were reviewed in this article, which may provide a reference for developing new anxiolytic drugs based on this prescription. The literature was searched in PubMed and CNKI and the documents written in English or with English abstracts were selected. JWXYS could reduce the anxiety symptoms of patients alone and reduce the adverse reactions when it is used in combination with other drugs in the clinic. In preclinical studies, JWXYS also showed therapeutic effects in reducing anxiety-like behavior. The mechanisms may include improving the hypothalamic-pituitaryadrenal (HPA) axis and hormone disorders, increasing neurotransmitter content, neurogenesis, and regulating the synthesis of related enzymes. This article shows that JWXYS could effectively treat anxiety disorders by regulating the central nervous system. In the future, with the participation of more researchers, it is expected to develop innovative drugs for the treatment of anxiety disorders based on JWXYS.

RESUMO

Background@#Due to the unavailability of an effective vaccine or antiviral drug against the African swine fever virus (ASFV), rapid diagnosis methods are needed to prevent highly contagious African swine fever. @*Objectives@#The objective of this study was to establish the ladder-shape melting temperature isothermal amplification (LMTIA) assay for the detection of ASFV. @*Methods@#LMTIA primers were designed with the p72 gene of ASFV as the target, and plasmid pUC57 was used to clone the gene. The LMTIA reaction system was optimized with the plasmid as the positive control, and the performance of the LMTIA assay was compared with that of the commercial real-time polymerase chain reaction (PCR) kit in terms of sensitivity and detection rate using 200 serum samples. @*Results@#Our results showed that the LMTIA assay could detect the 10 4 dilution of DNA extracted from the positive reference serum sample, which was the same as that of the commercial real-time PCR kit. The coincidence rate between the two assays was 100%. @*Conclusions@#The LMTIA assay had high sensitivity, good detection, and simple operation. Thus, it is suitable for facilitating preliminary and cost-effective surveillance for the prevention and control of ASFV.

RESUMO

Na?ve CD4 + T cells differentiate into a variety of T helper (Th) subsets that secrete various cytokines to exert biological effects. Th22 cells, a novel identified CD4 + T cell subset, are distinct from Th1, Th2 and Th17 cell subsets. Th22 cells express chemokine receptors CCR4, CCR6 and CCR10, and secrete multiple cytokines such as IL-22, IL-13 and TNF-α, but not IL-17, IL-4 IFN-γ; and IL-22 is considered as major effector cytokine of Th22. The understanding on functions and differentiation mechanisms of Th22 cells have been constantly improved, and Th22 cells play important roles in human common viral infections. The article reviews the current advances about the characteristics, function, differentiation of Th22 cells, the roles of Th22 cells and the key molecules in several human common viral infections, which would provide novel immune strategies for the prevention and treatment of human viral infection.

RESUMO

Red blood cell (RBC) transfusion is a clinically effective therapy in anemia, for example in patients with malignancies (Shander et al., 2020), bleeding (Odutayo et al., 2017), and preoperative anemia (Padmanabhan et al., 2019). The past few decades have witnessed a shortage of blood for transfusion due to limited health insurance coverage for blood use and the rapid expansion of hospitals (Chen et al., 2011; Shi et al., 2014). Blood donation levels may easily be affected by general changes in the environment, policy, major events such as disasters, and public sentiment (Hu et al., 2019). Meanwhile, the transfusion of allogeneic RBC is a double-edged sword, increasing the possibility of infectious and immunological complications, and also leading to higher morbidity and mortality after transfusion (Frank et al., 2012). Considering that the continual shortfall has been increasingly prominent, identifying the factors associated with RBC transfusion could help blood transfusion departments to improve their supply of blood products as well as their inventory management (O'Donnell et al., 2018).

RESUMO

Objective:To evaluate the multi-leaf collimator (MLC) performance of TrueBeam accelerator using trajectory log files. Methods:All tests were performed 5 times under different gantry-collimator angle combination. The 1 mm picket fences were constructed by static or dynamic MLC. The control ability for small-field accuracy of accelerator was evaluated. Repeatability was assessed by MLC repeat motion. The movement performance of difference velocities along one direction and the opposite direction were evaluated via a 1 cm picket fences which slipped from -7 cm to 7 cm with a uniform velocity and stopped or immediately back at 7 cm. The MLC performance in a complex program was evaluated by a cross movement test. Results:Both the static and the dynamic picket fences yielded high accuracy. The deviation spectrums of MLC in different gantry angle were consistent, however, an absolute difference of 0.001 1 mm was found. For uniform velocity movement tests with 0°gantry, the RMSE of MLC was increased from 0.015 0 mm to 0.059 8 mm when the speed was accelerated from 5 mm/s to 25 mm/s. Similar results were obtained in non-zero gantry angle. The "overspeed" effect caused by the direction change movement of MLC was less obvious than that caused by speed changed from zero to a uniform velocity movement state. There was no significant change in speed before and after the MLC crossing. The MLC speed fluctuated around the set value, which was independent of the gantry angle. Conclusion:A method for evaluating the performance of MLC using trajectory log files is established, which can evaluate the MLC performance of TrueBeam accelerator and be used for MLC rapid quality control in clinical practice.

Objective To evaluate the feasibility of total marrow and lymphatic irradiation (TMLI)with helical tomotherapy as a conditioning regimen before hematopoietic stem cell transplantation (HSCT).Methods Seven children with acute lymphoblastic leukemia and aplastic anemia were recruited as study subjects. The median age was 7 years old.The prescribed dose was 12 Gy/6 fractions twice daily. The exposure dose of the target and the organs at risk between helical helical tomotherapy-based TMLI regimen and total body irradiation (TBI) regimen were statistically compared, and acute toxicity grading was performed for all patients. Results Compared with the TBI regimen, the average exposure dose reduction for organs at risk after the TMLI regimen was ranged from 4.2% to 40.6%.The average exposure dose reduction for the kidney was the largest among all organs. The acute toxicities experienced by all patients were graded and recorded including 2 cases of nausea,5 cases of vomiting,1 case of anorexia,1 case of eryhema,3 cases of diarrhea, and 1 case of oral mucositis.Only grade 1-2 toxicities were observed, and no grade 3-4 toxicities occurred. Conclusions The findings in this study confirm the feasibility of helical helical tomotherapy-based TMLI regimen.Compared with the TBI regimen, the mean duration of treatment for the TMLI regimen with an equivalent dose is not increased. The exposure dose experienced by organs at risk is reduced and the predicted incidence rate is decreased when the TMLI regimen is employed, which provides a myeloablative pretreatment strategy. However, the long-term toxicity of TMLI regime remains to be evaluated by clinical trials.

RESUMO

Objective To investigate the influence of Zafirlukast on the inflammatory factors and immune function of patients with seasonal allergic asthma. Methods 182 patients with seasonal allergic asthma from the department of respiratory in our hospital were selected and divided into two groups, 91 cases in each group. The control group treated with budesonide and formoterol fumarate powder for inhalation, the experiment group received Zafirlukast on the basis of the control group, serum levels of inflammatory cytokines, peripheral blood T lymphocyte subsets, serum immunoglobulin levels, lung function, and the clinical effect were compared after the treatment. Results Compared with the control group, serum levels of TNF-,IL-4,IL-6 were lower, level of IL-10 was higher in the experiment group after treatment, CD8 +T lymphocyte levels was lower, CD3 +, CD4 +T lymphocyte and CD4 +/CD8 +ratio levels were higher after treatment, serum levels of IgE, IgG, IgA were lower after treatment, levels of PEF,FEV1 ,FEV1/FVC were higher after treatment, the difference was statistically significant (P<0.05).The effective rate in the experiment group 94.51% was higher than the control group 81.32% (P<0.05).Conclusion Zafirlukast can significantly reduce the levels of serum inflammatory factors in patients with seasonal allergic asthma, improve the cellular immune function, reduce the level of immunoglobulin, effectively improve lung function, shorten the length of hospital stay, has better clinical curative effect.

RESUMO

Objective The attitude towards death and hospice care was surveyed among 170 medical staff in geriatrics department and ICU of Renji Hospital and Tangqiao Community Health Service Center with the Revised Attitude towards Death Scale ( in Chinese ) and a self-designed questionnaire , and 158 valid questionnaires were received .The average score of negative attitude to death was 37.0 ±8.4 and the influencing factor was positions (P=0.02).The average score of positive attitude to death was 28.8 ± 4.3, and the influencing factors were positions (P=0.01)and religious belief (P=0.02).The mean score of attitude toward hospice care was 35.4 ±3.9 and the marital status(P=0.00)was the independent factor . It is necessary for medical staff to participate in professional training for death education , which would make them to take better care for terminal stage patients with rationality and sensibility .
