Clinical efficacy observation of filiform needle combined with fire needle for cervical radiculopathy due to wind-cold obstructing the meridians / 针灸推拿医学（英文版）

Objective:To observe the clinical efficacy of filiform needle combined with fire needles for cervical radiculopathy(CR)due to wind-cold obstructing the meridians. Methods:A total of 60 patients with CR due to wind-cold obstructing the meridians were randomized into an observation group and a control group,with 30 cases in each group.The control group was treated with filiform needle treatment,and the observation group was treated with additional fire needle point-pricking treatment.The visual analog scale(VAS)and 20-point scale of CR developed by Yasuhisa TANAKA(YT-20)were scored before treatment and after 1,2,3,and 4 weeks of treatments.The tenderness threshold was measured.The local skin temperature was measured by an infrared thermal imager.A safety evaluation was performed after treatment. Results:After treatment,the VAS score in the observation group at each time point was lower than that in the control group(P<0.05).The YT-20 score,tenderness threshold,and local skin temperature in the observation group were all higher than those in the control group(P<0.05).The VAS score in the observation group at each time point after treatment was lower than that before treatment(P<0.05),and the YT-20 score,tenderness threshold,and local skin temperature at each time point were higher than those before treatment(P<0.05).After 3 and 4 weeks of treatments,the VAS score in the control group was lower than that before treatment(P<0.05),and the YT-20 score,tenderness threshold,and local skin temperature were higher than those before treatment(P<0.05).The group factor effects of VAS and YT-20 scores,tenderness threshold,and local skin temperature between the two groups were statistically significant(P<0.05).There was no significant difference in the time effect and the interaction effect between time and group(P>0.05). Conclusion:Filiform needle combined with fire needle or applied alone both can relieve neck pain in patients with CR due to wind-cold obstructing the meridians,and improve the temperature of the neck.The combination of filiform needle and fire needle works more quickly and has better efficacy.

AP2α negatively regulates PDHA1 in cervical cancer cells to promote aggressive features and aerobic glycolysis in vitro and in vivo / 부인종양

Objective@#As a gate-keeper enzyme link, pyruvate dehydrogenase E1 subunit alpha (PDHA1) functions as a key regulator during glycolysis and the mitochondrial citric acid cycle, which has been reported in several tumors. Nevertheless, the effects of PDHA1 on biological behaviors and metabolism remain unclear in cervical cancer (CC) cells. The study aims to explore the PDHA1 effects on glucose metabolism in CC cells and its possible mechanism. @*Methods@#We first determined the expression levels of PDHA1 and activating protein 2 alpha (AP2α) as a PDHA1 potential transcription factor. The effects of PDHA1 in vivo were evaluated through a subcutaneous xenograft mouse model. Cell Counting Kit-8 assay, 5-ethynyl-2′-deoxyuridine (EdU) labeling assay, Transwell invasion assay, wound healing assay, Terminal deoxynucleotidyl transferase dUTP nick end labeling assay and flow cytometry were performed in CC cells. Oxygen consumption rate (OCR) levels were determined to reflect aerobic glycolysis level in gastric cancer cells. Reactive oxygen species (ROS) level was measured with 2′, 7′-dichlorofluorescein diacetate kit. The relationship between PDHA1 and AP2α was examined by conducting chromatin immunoprecipitation assay and electrophoretic mobility shift assay. @*Results@#In CC tissues and cell lines, PDHA1 was downregulated, while AP2α was upregulated. Overexpression of PDHA1 remarkedly inhibited the proliferation, invasion and migration of CC cells, and tumor growth in vivo, as well as promoted OCR, apoptosis and ROS production. Moreover, AP2α directly bound to PDHA1 within suppressor of cytokine signaling 3 promoter region to negatively regulate PDHA1 expression level. What is more, PDHA1 knockdown could effectively reversed the AP2α silencing-mediated suppressive effects on cell proliferation, invasion, migration, and the promotive effects of AP2α knockdown on OCR, apoptosis and ROS production. @*Conclusions@#Our findings demonstrate that AP2α negatively regulated PDHA1 via binding to PDHA1 gene promoter to promote malignant CC cell behaviors, which may provide a potential approach for CC therapeutics.

Evaluation of the treatment of functional dyspepsia of deficiency cold of spleen and stomach by He Style fire acupuncture / 国际中医中药杂志

Objective:To observe the curative effect of He Style fire acupuncture on functional dyspepsia of deficiency cold of spleen and stomach syndrome.Methods:Sixty patients who met the inclusion criteria from March 2017 to March 2019 in Beijing Hospital of Traditional Chinese Medicine Affiliated to Capital Medical University were divided into 2 groups according to the random number table method, with 30 in each group. The control group was given mosapride tablets, one tablet (5 mg) each time half an hour before meals. On the basis of the control group, the treatment group was given fire acupuncture on Zusanli(ST36), Zhongwan(CV12), Pishu(BL20), Weishu(BL21), Guanyuan(CV4) and Qihai (CV6) twice a week. The course of treatment for both groups of patients was 4 weeks. A follow-up was visited at 1 month after the treatment. The total Symptom Scores and Health-Related Quality of Life Scale (SF-36) scores were performed before and after treatment, and the total scores of syndromes of patients with Deficiency cold of spleen and stomach before and after treatment were used for clinical efficacy evaluation, and adverse events during treatment were recorded.Results:The total efficiency of the treatment group was 93.3% (28/30), and 76.7% (23/30) in the control group, and the 2 groups were statistically significant ( χ2=4.78, P<0.05). After treatment, the upper abdominal pain, upper abdominal burning, postprandial fullness and discomfort, early satiety, and total scores in the treatment group were significantly lower than those in the control group ( t values were 4.27, 5.16, 3.93, 4.69, 4.28, respectively, all Ps<0.05); during follow-up, the upper abdominal pain, upper abdominal burning, postprandial fullness and discomfort and the total scores in the treatment group were significantly lower than those in the control group ( t values were 3.63, 3.22, 4.03, 3.04, respectively, all Ps<0.05). In terms of SF-36, after treatment, the treatment group showed significantly higher scores in the control group of physical function, physiology, mental health, and health changes compared with the control group ( t values were 2.97, 4.05, 4.22, 3.05, respectively, all Ps<0.05). During follow-up, the treatment group with physiological function, physical function, physical pain, overall health, life vitality, mental health, health changes scores were significantly higher than those in the control group ( t values were 3.27, 4.23 3.85, 3.15, 3.25, 6.15, 3.85, respectively, all Ps<0.05). Conclusion:He Style fire acupuncture combined with western medicine treatment can improve the symptoms of upper abdominal pain and upper abdominal burning, and the quality of lifein the patients with functional dyspepsia.

A longitudinal study of posttraumatic growth and emotion regulation strategies in patients with moderate and severe burns in rehabilitation / 中国实用护理杂志

Objective To investigate the posttraumatic growth and emotional regulation strategies of patients with moderate and severe burns at different time points in the rehabilitation period, and to explore the correlation between posttraumatic growth and emotional regulation strategies, so as to provide a basis for targeted nursing intervention. Methods By convenience sampling method, 115 convalescent patients with moderate and severe burns in Linyi People′s Hospital burn plastic surgery center were investigated by General Information Questionnaire, Chinese Posttraumatic Growth Inventory (C-PTGI), Emotion Regulation Scale (ERQ) respectively on the day of discharge (T1), 3 months and 6 months after hospital discharge (T2, T3) from December 2017 to January 2019. Results The post-traumatic growth scores of patients T1, T2 and T3 were 58.18±9.51, 48.65±9.31 and 47.43±8.76, respectively. The post-traumatic growth scores showed a downward trend at three time points, and the difference was statistically significant (F=339.732,P<0.01). The scores of T1, T2 and T3 in the cognitive reassessment dimensions of patients′emotion regulation strategies were respectively 27.10±4.13, 26.95±3.76, 26.11±3.60. The scores are going down and the differences were statistically significant (F=27.798, P<0.01). The expression inhibition scores were 14.42±4.39, 15.20±3.30, 15.21±3.56 respectively. The scores are the rise and the differences were statistically significant (F=13.675, P < 0.01). There were positive correlations between cognitive reassessment scores and posttraumatic growth scores、scores of all dimensions (r=0.230-0.555, P<0.05). The expression inhibition scores were negatively correlated with the total post-traumatic growth scores, personal strength, and scores of new possibility dimensions (r=- 0.230-- 0.420, P<0.05). Conclusion Posttraumatic growth of patients with moderate to severe burns is not optimistic in the rehabilitation period. Therefore, it is necessary to pay attention to the guidance of emotional regulation strategies for patients with moderate to severe burns in the rehabilitation period, and carry out targeted interventions to promote the posttraumatic growth of patients.

Progress on practice and effect of transitional care in discharged patients with breast cancer / 中国实用护理杂志

Transitional care refers to carrying out related health care services to make discharged patients′nursing activities orderly, coordinated and continuous, so as to meet their various rehabilitation needs. In recent years, it has become a research focus of breast cancer discharged patients. This article reviews the concept of continuous care and its practice and effect in discharged patients with breast cancer, and provides evidence for further improvement of breast cancer patients′continuity of care in China.

Research progress of fire needling for chronic gastritis / 中国针灸

To explore the efficacy and mechanism of fire needling for chronic gastritis, "fire needling" and "chronic gastritis" were selected as keywords to collect literature in PubMed, CNKI database anddatabase. The clinical and experiment literature regarding fire needling for chronic gastritis was analyzed to make a comprehensive discussion on the efficacy and action mechanism of fire needling for chronic gastritis. The results indicated the efficacy of fire needling for chronic gastritis was superior to that of medication or filiform needle. The combination of fire needling and medication, acupoint injection and filiform needle could further improve the therapeutic effect for chronic gastritis. From the perspectives of TCM and western medicine, the action mechanism of fire needling for chronic gastritis was explored, and it was believed the fire-needle therapy has the effects of removing and nourishing, which conformed to the clinical syndromes of chronic gastritis. The fire needle could affect the prognosis by regulating the central nerve system, improving local blood circulation, regulating immune inflammation and regulating gastrointestinal hormones.

Clinical outcome analysis of mild-stimulation in poor ovarian responders / 中国医师杂志

Objective To explore the clinical outcomes of different mild-stimulation in poor ovarian responders in vitro culture/intracytoplasmic injection (IVF/ICSI) to provide the evidence for clinical treatment,and investigate clinical parameters including pre-ovulation rate,rate of oocytes retrieved,fertilization rate,good quality embryo rate,and pregnancy rate per cycle.Methods The clinical date of a total 180 IVF cycles of infertile patients undergone from Jan 2013 to May 2016 in medical reprodution center of Xiang Tan Central Hospital.According to the different methods of controlled ovarian hyperstimulation (COH),the cycles were divided into 3 groups:group A with 60 cycles of early five days using clomiphene,group B with 60 cycles of persistent using clomiphene,and group C with 60 cycles using artificial luteal-phase ovarian stimulation protocol.Results The basic situations in three groups were no significant difference (P ＞0.05).Compared to groups B and C,the pre-ovalution rate was more in group A (P ＜ O.05),and the rate of oocytes retrieved,fertilization rate,good quality embryo rate,and preganacy rate per cycle were higher in groups B and C (P ＜ 0.05).The rate of oocytes retrieved,fertilization rate,good quality embryo rate,and preganacy rate per cycle were no significant difference among gourps B and C.The good quality embryo rate and preganacy rate per cycle in groups B and C were higher than group A,and group B was higher than group A (P ＜ 0.05).Conclusions Persistent using clomiphene or persistent using clomiphene in COH could decrease the rate of pre-ovalution,and get more rate of oocytes retrieved,good quality embryo rate,and preganacy rate per cycle.

The study of the subthreshold depression and their quality of life for long-term survivors after breast cancer surgery / 中国实用护理杂志

Objective To investigate the subthreshold depression for long-term survivors after breast cancer surgery and its effect on their quality of life (QOL). Methods The self-made general questionnaire, Centre for Epidemiological Studies Depression Scale (CES- D), Hamihon Rating Scale for Depression and Functional Assessment of Cancer Therapy- Breast were used to investigate the subthreshold depression and its effect on their QOL for 210 long-term survivors after breast cancer surgery. The difference in general conditions and score of QOL between the subthreshold depression and non-subthreshold depression groups was also compared. Results The subthreshold depression with incidence rate of 46.67%(98/210) was observed for the log-term survivors after breast cancer surgery in this study. The results indicated that the age (≤50) (OR=5.627, 2.662,P<0.01), marriage (unmarried, divorced or widowed) (OR=5.087, P<0.01), employment (unemployed, sick leave or retired) (OR=2.077,P<0.05) and low income (<3000 RMB/month) (OR=3.375,P<0.01) were the key factors that led to the occurrence of subthreshold depression. The score of QOL for the subthreshold depression group were lower than those of the non-subthreshold depression group (t=-6.653--2.926,P<0.01), and had a significantly negative correlation with score of depression and QOL (β=-0.362--0.132). Conclusions A high incidence rate of subthreshold depression for long-term survivors after breast cancer surgery was observed. The incidence rate of subthreshold depression increased for the patients with younger age, low income,unmarried or unemployment. Due to their low QOL, attentions should be paid to the early detection and early prevention for the long-term survivors with subthreshold depression after breast cancer surgery.

Simulation and Observation of Vertical Cast-off Bloodstain Pattern / 法医学杂志

Objective To observe the characteristics of vertical cast-off bloodstain pattern by different hitting-tools. Methods The regular hitting tools, a kitchen knife, a dirk, a plane set-hammer and an iron pipe, were selected. At a distance of 30 cmaway fromthe wall, the hitting tool with 5 mL fresh chick-en blood made the cast-off bloodstain fromtop to bottom. Then the holistic distribution characteristics ( length , width and density ) of cast-off bloodstain and morphology characteristics ( length , width and contact angle) of first single cast-off bloodstain were analyzed. Results The distribution length of cast-off bloodstain formed by dirk was minimum( P<0 .05 ) . The distribution width of cast-off bloodstain formed by kitchen knife was minimum(P<0.05). Except the pair of kitchen knife and plane set-hammer, the distribution density between each two tools had statistical differences (P<0.05). The length of first single cast-off bloodstain formed by plane set-hammer was longest compared (P<0.05). The width of first single cast-off bloodstain had statistical differences between kitchen knife and plane set-hammer, and between dirk and plane set-hammer (P<0.05). Conclusion The type of hitting tool could be inferred by the specific characteristics of cast-off bloodstain pattern formed by every specific type of hitting tool in crime scene.

Effect of serum IGF-1, IGFBP-3 and HtSDS in rhGH-treated SGA children / 中国基层医药

Objective To investigate the clinical value of serum insulin-like growth factor-1 (IGF-1),insulin-like growth factor-binding protein-3 (IGFBP-3) and mean height standard deviation score(HtSDS) in recombinant human growth hormone (rhGH)-treated small for gestational age (SGA) children.Methods 32 cases of SGA failed of catch-up growth were subcutaneously injected with rhGH every night before bed at a dose of 0.15-0.20u· kg-1 · d-1.And the variation of serum IGF-1,IGFBP-3,HtSDS,body height and growth velocity (GV) were monitored at 3,6,9 and 12 months of the treatment and compared.Results The GV (cm/years) increased to (12.4 ± 3.2),(11.0 ± 2.3),(10.1 ± 3.5),(9.4 ± 1.8) versus (4.1 ± 0.5) before treatment,suggesting a significant catch-up growth after treatment (F =51.35,P ＜ 0.01).Accordingly,HtSDS increased to (-2.55 ± 0.73),(-2.39 ± 0.65),(-2.21 ± 0.58),(-2.09 ± 0.94) versus (-2.81 ± 0.64),suggesting that height gaps were reduced compared to the normal children of the same age and sex after rhGH treatment (F =4.99,P ＜0.01).The serum levels of IGF-1,IGFBP-3 were significantly increased compared to pre-treatment (F =34.52,14.04,all P ＜ 0.01),especially for serum IGF-1,which maintained at a high level after 3 months of treatment and up to peak after 6 months.However,the ratio of IGF-1/IGFBP-3 showed no increase after treatment (F =1.82,P ＞ 0.05).There was a significant positive correlation between the serum level of IGF-1 and △HtSDS after the rhGH treatment within six months (r3 =0.72,r6 =0.91),while there was no correlation between them after 9 months (r9 =0.26,r12 =0.33).There were no serious adverse events during the rhGH treatment.Conelusion The serum levels of IGF-1,IGFBP-3 were significantly increased after the rhGH treatment,especially for serum IGF-1.There was a significant positive correlation between the serum level of IGF-1 and △HtSDS within six months,but not after 9 months.The rhGH treatment could be a safe and effective strategy for SGA children.
