RESUMO

Objective To investigate the possible mechanism of Xuebijing injection in regulating abnormal degradation of pulmonary vascular endothelial calyx to improve acute lung injury induced by severe heatstroke.Methods Forty-eight Wistar rats were randomly divided into control group,heatstroke group and Xuebijing group.Before heatstroke induction,rats in Xuebijing group were administrated with Xuebijing injection(2 ml/kg,2 times/d)for 3 days.All rats were exposed to an environment with temperature of(40±2)℃and humidity of 65%±5%for 60 minutes to induce heatstroke.Two hours later,the lung wet/dry weight ratio was recorded;the concentration of proteins in BALF was measured;the pulmonary vascular permeability was measured by Evans blue(EB);HE staining was used to observe the pathological changes of lung tissue;the changes of hyaluronic acid(HA)on the surface of pulmonary vessels were observed by immunofluorescence;Western blotting was used to detect the expression of Syndecan-1,Glypican-1,VE-Cadherin,Occludin,VCAM-1 and E-selectin in lung tissues;Enzyme-linked immunosorbent assay(ELISA)was utilized to quantify the concentration of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)in serum and heparanase(HPA)in lung tissue.Results Xuebijing could decrease the lung wet/dry weight ratio,reduce protein exudation and improve pulmonary vascular permeability(P<0.01);reduce the histological injury(P<0.01);reduce the degradation of HA,Syndecan-1 and Glypican-1 on the surface of pulmonary vessels(P<0.01);increase the expression of VE-Cadherin and Occludin(P<0.01);regulate the overexpression of VCAM-1 and E-selectin(P<0.01);down-regulate the expression of TNF-α,IL-6 and HPA(P<0.01).Conclusion Xuebijing injection decrease the expression of HPA,improve the disintegration of pulmonary vascular endothelial calyx,repair the integrity of pulmonary vessels,reduce the damage of cell connections,down-regulate the expression of adhesion molecules,inhibit the inflammatory reaction,relieving acute lung injury caused by severe heat stroke.

Objective: To explore the association between healthy lifestyle and risk of rehospitalization in male or female patients with chronic heart failure (CHF). Methods: Discharged patients with CHF of Henan Provincial People's Hospital Collaboration Hospital were recruited in our study from January 1,2017 to December 31, 2018. The basic information of patients were collected through the electronic medical record system,the questionnaires were used to collect the related influencing factors. Healthy lifestyle includes 4 items, namely non-smoking, moderate exercise, healthy body mass index (BMI) and reasonable diet.Multivariate logistic regression was used to analyze the association between healthy lifestyle and the risk of rehospitalization of CHF patients of different genders. Results: A total of 2 697 patients with CHF were enrolled in this study, including 1 308 male patients(621 rehospitalizations,687 controls)and 1 389 female patients(684 rehospitalizations,705 controls).Among male patients, there was no significant difference in age, residence, marital status, education level, average monthly income, and medical insurance between the rehospitalization group and the control group (all P>0.05). Among female patients, there was no significant difference in age, residence, marital status, education level, average monthly income, and medical insurance between the rehospitalization group and the control group (all P>0.05). Whether in male or female patients with CHF, we found that patients with 4 healthy lifestyles were associated with reduced risk of rehospitalization: male patients OR=0.34, 95%CI 0.11-0.99, P=0.002,female patients OR=0.27, 95%CI 0.13-0.79, P=0.012. A combination of non-smoking and any other 2 healthy lifestyles was associated with reduced risk of rehospitalization: male patients with no smoking, moderate exercise, healthy BMI, OR=0.32, 95%CI 0.11-0.99, P=0.043; female patients OR=0.28, 95%CI 0.12-0.93, P=0.032;male patients with no smoking, moderate exercise, reasonable diet OR=0.42, 95%CI 0.24-0.98, P=0.044,female patients OR=0.40, 95%CI 0.12-0.94, P=0.031;male patients with no smoking, healthy BMI, reasonable diet OR=0.31, 95%CI 0.21-0.92, P=0.039,female patients OR=0.27,95%CI 0.11-0.87, P=0.014. In female patients with CHF, the combination of non-smoking and moderate sports was associated with reduced risk of hospitalization (OR=0.23, 95%CI 0.19-0.97, P=0.038), while no similar results were seen in male patients (OR=0.65, 95%CI 0.33-1.84, P=0.315). Conclusion: Healthy lifestyle is associated with reduced risk of rehospitalization in patients with CHF, and the related healthy lifestyle and its combination may differ between male and female patients.

Objective To investigate the association between serum uric acid and brachial-ankle pulse wave velocity (baPWV) levels and to examine any possible effect modifiers among hypertensive patients in rural areas of Eastern China. Methods This cross-sectional study was conducted in Lianyungang and Anqing Cities from July to September 2013. Hypertensive patients with serum uric acid and baPWV measurements were included in this study. Multivariate linear regression was used to assess the relationship between serum uric acid levels and baPWV. Results Finally, a total of 4 278 participants were included in this study. The mean age was 65.2 (standard deviation(SD):7.4) years and the mean value of baPWV levels were 1835.1 (SD: 383.4) cm/s. Regression analysis showed that there is a positive correlation between serum uric acid and baPWV levels (per SD increment, β=20.5 cm/s, 95% CI: 8.5-32.5, P=0.001. When uric acid was categorized in tertiles, a significantly higher baPWV level was found in participants in tertile 3 (≥349μmol/L)(β=51.2 cm/s, 95% CI: 23.8-78.6, P=0.001), compared with those in tertile 1(<277 μmol/L). Furthermore, the serum uric acid-baPWV association was significantly stronger in those systolic blood pressure (SBP)≥140 mm Hg (vs.<140 mm Hg; p-interaction=0.001). Conclusion There is a positive relationship between uric acid levels and baPWV among hypertensive patients, especially in those with higher SBP levels.

Objective To explore the relationship between fasting blood glucose and branchial-ankle pulse wave velocity (baPWV) levels among hypertensive population in rural areas of Eastern China. Methods A cross-sectional analysis were conducted among hypertensive population in Lianyungang and Anqing city from July to September 2013. Multivariable linear regression models were used to analyze the association of serum glucose and baPWV. Results A total of 3 894 hypertensive participants were included in the present study. The prevalence of normal fasting glucose (NFG), impaired fasting glucose (IFG) and diagnosed diabetes (DM) were 44.5%, 43.0%, and 12.5%, respectively. Each 1-mmol/L increase in fasting blood glucose levels was significantly associated with a 26.2 cm/s increase in baPWV levels (95% CI: 19.6-32.8, P<0.001). Consistently, significantly higher baPWV levels were observed in participants with IFG (β=50.4 cm/s, 95% CI: 28.4-72.3, P<0.001) and DM (β=113.4 cm/s, 95% CI: 80.2-146.5, P<0.001) when compared to those with NFG. Furthermore, a stronger positive association between fasting blood glucose and baPWV was observed in participants with systolic blood pressure (SBP) ≥140 mmHg (β=34.3 cm/s, 95% CI: 24.5-44.2, P<0.001; vs.<140 mmHg; β=18.1 cm/s, 95% CI: 9.1-27.2, P<0.001; Pinteraction=0.027). Conclusion There was a positive association between fasting blood glucose and baPWV levels among hypertensive population in rural areas of Eastern China.

@#The rehabilitation exercise has been found to be an important part of the treatment of the senior coronary heart disease in the recent researches. The senior patients with coronary heart disease are always complicated, so the exercises should start with low intensity and increase gradually. It is important to pay attention to the warm up and relaxation before and after the exercises. There are few patients accepted rehabilitation, which need pay more attention on it to help the senior patients improve their quality of life.
