RESUMO

ObjectTo observe the clinical efficacy of Huangqin Beimutang on chronic rhinosinusitis in children. MethodA randomized controlled trial (RCT) was conducted on 70 children who met the criteria for chronic rhinosinusitis, with 35 cases in the Chinese medicine group and 35 cases in the western medicine group. In the western medicine group, children received mometasone furoate nasal spray, one spray per nostril, once a day for two weeks, and also received a small dose of azithromycin suspension at 4 mg·kg-1·d-1,once a day,3 days a week for 2 weeks. The children in the Chinese medicine group were treated with oral Huangqin Beimutang, one dose per day for 2 weeks. Before and after treatment, the scores of primary symptoms and signs of traditional Chinese medicine (TCM),visual analogue scale (VAS) symptom scores, sinus computed tomography (CT) efficacy scores, and clinical efficacy of TCM syndromes in the two groups were evaluated, and the occurrence of adverse events was recorded. ResultThe total effective rate of clinical efficacy of TCM syndrome in the Chinese medicine group was 88.57% (31/35), which was higher than 71.43% (25/35) in the western medicine group(χ2=8.458,P<0.05). The VAS scores, scores of TCM primary symptoms of nasal obstruction and runny nose, and physical sign scores in both groups were lower than those after treatment (P<0.01). The above indicators in the Chinese medicine group were superior to those in the western medicine group after treatment (P<0.05, P<0.01). Compared with the conditions before treatment, there was no significant improvement in headache in the western medicine group, while the headache score in the Chinese medicine group decreased after treatment (P<0.01). The CT scores of the two groups showed a downward trend, but the difference was not statistically significant. There were no adverse reactions during treatment in both groups. ConclusionHuangqin Beimutang can effectively improve the clinical symptoms of patients with chronic rhinosinusitis, and it is safe and effective.

RESUMO

Nitrate is the main form of inorganic nitrogen that crop absorbs, and nitrate transporter 2 (NRT2) is a high affinity transporter using nitrate as a specific substrate. When the available nitrate is limited, the high affinity transport systems are activated and play an important role in the process of nitrate absorption and transport. Most NRT2 cannot transport nitrates alone and require the assistance of a helper protein belonging to nitrate assimilation related family (NAR2) to complete the absorption or transport of nitrates. Crop nitrogen utilization efficiency is affected by environmental conditions, and there are differences between varieties, so it is of great significance to develop varieties with high nitrogen utilization efficiency. Sorghum bicolor has high stress tolerance and is more efficient in soil nitrogen uptake and utilization. The S. bicolor genome database was scanned to systematically analyze the gene structure, chromosomal localization, physicochemical properties, secondary structure and transmembrane domain, signal peptide and subcellular localization, promoter region cis-acting elements, phylogenetic evolution, single nucleotide polymorphism (SNP) recognition and annotation, and selection pressure of the gene family members. Through bioinformatics analysis, 5 NRT2 gene members (designated as SbNRT2-1a, SbNRT2-1b, SbNRT2-2, SbNRT2-3, and SbNRT2-4) and 2 NAR2 gene members (designated as SbNRT3-1 and SbNRT3-2) were identified, the number of which was less than that of foxtail millet. SbNRT2/3 were distributed on 3 chromosomes, and could be divided into four subfamilies. The genetic structure of the same subfamilies was highly similar. The average value of SbNRT2/3 hydrophilicity was positive, indicating that they were all hydrophobic proteins, whereas α-helix and random coil accounted for more than 70% of the total secondary structure. Subcellular localization occurred on plasma membrane, where SbNRT2 proteins did not contain signal peptides, but SbNRT3 proteins contained signal peptides. Further analysis revealed that the number of transmembrane domains of the SbNRT2s family members was greater than 10, while that of the SbNRT3s were 2. There was a close collinearity between NRT2/3s of S. bicolor and Zea mays. Protein domains analysis showed the presence of MFS_1 and NAR2 protein domains, which supported executing high affinity nitrate transport. Phylogenetic tree analysis showed that SbNRT2/3 were more closely related to those of Z. mays and Setaria italic. Analysis of gene promoter cis-acting elements indicated that the promoter region of SbNRT2/3 had several plant hormones and stress response elements, which might respond to growth and environmental cues. Gene expression heat map showed that SbNRT2-3 and SbNRT3-1 were induced by nitrate in the root and stem, respectively, and SbNRT2-4 and SbNRT2-3 were induced by low nitrogen in the root and stem. Non-synonymous SNP variants were found in SbNRT2-4 and SbNRT2-1a. Selection pressure analysis showed that the SbNRT2/3 were subject to purification and selection during evolution. The expression of SbNRT2/3 gene and the effect of aphid infection were consistent with the expression analysis results of genes in different tissues, and SbNRT2-1b and SbNRT3-1 were significantly expressed in the roots of aphid lines 5-27sug, and the expression levels of SbNRT2-3, SbNRT2-4 and SbNRT3-2 were significantly reduced in sorghum aphid infested leaves. Overall, genome-wide identification, expression and DNA variation analysis of NRT2/3 gene family of Sorghum bicolor provided a basis for elucidating the high efficiency of sorghum in nitrogen utilization.

RESUMO

Objective:To investigate the value of ultrasound combined with thyroglobulin (Tg) in preoperative N staging of thyroid carcinoma. Methods:The clinical data of 1 138 patients with thyroid carcinoma in the Cancer Hospital of the University of Chinese Academy of Sciences (Zhejiang Cancer Hospital) from August 2018 to October 2020, who confirmed by surgery and pathology were analyzed retrospectively. The 1 138 cases were divided into pN0, pN1a, and pN1b stages. Kappa consistency test was used to analyze the consistency of ultrasound evaluation of N staging and pathological N staging. Pathology result was taken as the gold standard to analyze the correlation between some preoperative serum markers and lymph node metastasis. The ROC curve was used to compare the diagnostic value of ultrasound, Tg, and ultrasound combined with Tg for lymph node metastasis. Results:The preoperative ultrasound assessment of N staging was moderately consistent with pathology(Kappa=0.459, P<0.01). Between pN0 and pN1(pN1a+ pN1b) stages, the differences in free triiodothyronine (fT3), anti-thyroid peroxidase antibody (TPOAb) and Tg were statistically significant (all P<0.05). Among the different indicators, only Tg had significant effect on lymph node metastasis ( P<0.01) .The area under the ROC curve (AUC) of Tg in predicting lymph node metastasis of thyroid cancer was 0.679, while the best cut-off value for Tg was 25.245 μg/L. The AUC of only ultrasound and ultrasound combined with Tg were 0.699 and 0.775, respectively. Therefore, combined diagnosis method was better than ultrasound only. Conclusions:Preoperative ultrasound and thyroglobulin has a specific value in evaluating the N staging of thyroid carcinoma. The combination of the two is more valuable in the diagnosis of lymph node metastasis than ultrasound only.

RESUMO

Objective:To analyze the social family factors influencing language delay in children with the age ranging from 18 to 42 months in Xiamen. Methods:A prospective cohort study was conducted to evaluate children with language delay (case group) and normal controls (control group) in Child Health Clinic and Developmental Behavior Clinic of the First Affiliated Hospital of Xiamen University between July 2017 and July 2019 via a self-made questionnaire and a language development scale, and the case-control ratio was 1∶4.The chi- square test, Logistic regression and generalized multifactor dimensionality reduction (GMDR) were adopted for statistical analysis, and the correction analysis was performed with Bonferroni correction. Results:A total of 126 children with language delay were collected in the case group, with the ratio of male to female being 2.05∶1.00. The control group was included 504 cases. There was no significant difference in gender and age between both groups. The chi- square test showed that there were statistical differences in maternal culture and screen time distribution between both groups ( P<0.05/13). Besides, the multivariate Logistic regression analysis suggested that significant risk factors for language delay in children included maternal culture, maternal-child interaction, and screen time. The GMDR analysis showed that screen time was the optimal single-mode for children at risk of language delay, while maternal culture and screen time constituted a statistically different two-factor model. Moreover, the marital-child interaction was included into the three-factor model. Conclusions:Screen time and maternal culture were the most important risk factors for language delay in children of Xiamen, and both factors would interact with maternal-child interaction, which could exert impacts on language delay in children.

RESUMO

The Alataw Pass, near the Ebinur Lake Wetland (northwest of China) and Taldykorgan (east of Kazakhstan), is a natural habitat for wild rodents. To date, little has been done on the surveillance of Bartonella spp. and Wolbachia spp. from fleas in the region. Here we molecularly detected Bartonella spp. and Wolbachia spp. in wild rodent fleas during January and October of 2016 along the Alataw Pass-Kazakhstan border. A total of 1,706 fleas belonging to 10 species were collected from 6 rodent species. Among the 10 flea species, 4 were found to be positive for Wolbachia, and 5 flea species were positive for Bartonella. Molecular analysis indicated that i) B. rochalimae was firstly identified in Xenopsylla gerbilli minax and X. conforms conforms, ii) B. grahamii was firstly identified in X. gerbilli minax, and iii) B. elizabethae was firstly detected in Coptopsylla lamellifer ardua, Paradoxopsyllus repandus, and Nosopsyllus laeviceps laeviceps. Additionally, 3 Wolbachia endosymbionts were firstly found in X. gerbilli minax, X. conforms conforms, P. repandus, and N. laeviceps laeviceps. BLASTn analysis indicated 3 Bartonella species showed genotypic variation. Phylogenetic analysis revealed 3 Wolbachia endosymbionts were clustered into the non-Siphonaptera Wolbachia group. These findings extend our knowledge of the geographical distribution and carriers of B. rochalimae, B. grahamii, B. elizabethae, and Wolbachia spp. In the future, there is a need for China-Kazakhstan cooperation to strengthen the surveillance of flea-borne pathogens in wildlife.

RESUMO

Cystic echinococcosis (CE) caused by E. granulosus is a serious helminthic zoonosis in humans, livestock and wildlife. Xinjiang is one of high endemic province for CE in China. A total of 55 sheep and cattle livers containing echinococcal cysts were collected from slaughterhouses in Changji and Yining City, northern region of Xinjiang. PCR was employed for cloning 2 gene fragments, 12S rRNA and CO1 for analysis of phylogenetic diversity of E. granulosus. The results showed that all the samples collected were identified as G1 genotype of E. granulosus. Interestingly, YL5 and CJ75 strains were the older branches compared to those strains from France, Argentina, Australia. CO1 gene fragment showed 20 new genotype haploids and 5 new genotype haplogroups (H1-H5) by the analysis of Network 5.0 software, and the YLY17 strain was identified as the most ancestral haplotype. The major haplotypes, such as CJ75 and YL5 strains, showed identical to the isolates from Middle East. The international and domestic trade of livestock might contribute to the dispersal of different haplotypes for E. granulosus evolution.

RESUMO

OBJECTIVE:To investigate the optimization scheme for variety and quantity of drugs equipped in emergency medical vehicle of clinical wards in order to meet the clinical first-aid drugs needs timely and accurately. METHODS:Used with large data analysis, by analyzing the data of drug use for large-scale and medium-scale first-aid in our hospital during May 2016-Mar. 2017 (before optimization equipment),the ratio of fully satisfied first-aid demand was calculated. The drug variety on the emergency medical vehicle of our hospital was determined according to the related first-aid drug catalogue. In addition, every drug was arranged respectively in ascending order according to the quantity used in firs-aid before optimization equipment, and then the number in 75% quantile of each variety was determined as the target quantity using quartile method. The drugs were arranged in descending order according to use frequency in ambulance, drug variety of each ward was determined according to 95% accumulative ratio. The ratio of drug variety outside ambulance used in first-aid was calculated after optimization (Apr.-Jul. 2017),and then compared with before optimization equipment. The rationality of optimization scheme was validated. RESULTS:Before optimization, there were 8 drug varieties equipped in emergency medical vehicle of our hospital, and 4 injections of each variety were prepared;the ratio of fully satisfied first-aid demand was 38%(776 times/2042 times). After optimization equipment,19 kinds of first-aid drugs which used mostly were determinated in the whole hospital, and the quantities of all kinds of drugs had been equipped scientifically;the ratio of fully satisfied first-aid demand was 78%(636 times/815 times). According to the characteristics of each ward,a personalized scheme (including specific varieties and corresponding quantities) was made. The ratio of drug variety outside emergency medical vehicle decreased from 46.42% to 25.51% in average. CONCLUSIONS:The optimization scheme formulated according to large data analysis can accurately meet clinical first-aid demand.

Two researchers independently screened studies according to the inclusion and exclusion criteria, extracted data and evaluated the methodological quality of included studies. Then Meta analysis was performed through the software RevMan 5.3.Results A total of 8 articles involving 550 patients were included. The results of Meta analysis showed effective rate ofYupingfeng Granules combined with western medicine for children with RRTI [OR=4.16, 95%CI (2.49, 6.95),P0.05). ConclusionYupingfeng Granules combined with western medicine can improve effective rate, antibody level and T-cell level in the treatment of children with RRTI. The overall clinical efficacy is better than single western medicine treatment. However, RCTs with large samples, multiple centers and high quality are needed for further verification.

RESUMO

Objective To retrospectively study the clinical and electrodiagnostic features in subacute combined degeneration of spinal cord (SCD)patients. Methods The clinical and electrodiagnostic recordings of all SCD patients conformed in our Neurology Department from Janu?ary 2013 to July 2015 were retrospectively reviewed. Totally 29 patients diagnosed as SCD,whom received no treatment before admitted in our hos?pital, were recruited for the study. Correlation analysis between serum level of vitamin B12(VB12),course of disease and degree of neural lesion was performed, and the electrodiagnostic features were summarized and analyzed. Results The course of disease had significant correlation with the degree of neural lesion(P=0.001),but there was no significant relevance between serum VB12 level and neural damage(P>0.05). Electrodi?agnostic examination revealed:The abnormal rate of motor nerve(15.20%)was lower than that of sensory nerve(42.75%,P<0.001)and the up?per limb nerve(27.1%)was lower than that of lower limb nerve(30.87%,P=0.578). There was no remarkable difference of abnormal rate in all kinds of nerves of motor and sensory nerve(P1=0.320,P2=0.755). In the analysis of the parameters of electromyograph, there was statistical signif?icance of the abnormal rates of compound muscle action potential(CAMP)between each motor nerve(P=0.005). There was statistical signifi?cance of the abnormal rates of CAMP and no function between each sensory nerve(all P<0.001). Conclusion The course of disease has signifi?cant correlation with the degree of neural lesion. Sensory nerve is tended to be betreffend than motor nerve. Phil.always nervous are more likely to be affected than others in motor nerve at CAMP. Tibial nerve is easier to be influenced at CAMP in sensory nerve ,and the lesion is more serious.

RESUMO

Objective To study the correlation between the electrophysiological features and mechanical ventilation and long?term outcome in Guil?lain?Barré syndrome(GBS)patients. Methods Electrophysiological and clinical data were retrospectively collected, and compared between venti?lated and not ventilated patients,as well as among each subtype of GBS. Results Totally 38 GBS patients were included in the study, among which 12(32%)were ventilated. The p/d CMAP ratio of the common peroneal nerve was significantly lower in ventilated group compared to not ventilated group(48.7±15.3 vs 80.8±24.0,P=0.005). AIDP was the most common subtype in ventilated patients compared with AMAN and undetermined (46%vs 0%and 9%,P=0.027). The long?term outcome score of AMAN was significantly lower than AIDP(4.3±1.3 vs 2.5±0.9,P=0.028). Conclusion Electrophysiological testing was predictive for mechanical ventilation and long?term outcome:low p/d CMAP ratio of the common pero?neal nerve was helpful for predicting mechanical ventilation,AIDP was prone to develop respiratory failure and had a worse outcome compared to AMAN.

RESUMO

OBJECTIVE@#To evaluate the prevalence and risk factors for the formation of aberrant artery collaterals in the uterus during uterine artery embolization (UAE).
@*METHODS@#The data of 144 women with scar in the uterus due to cesarean were retrospectively analyzed. They underwent UAE in the period of 2009-2014 and were divided into two groups according to a standard with or without the aberrant artery collaterals in the uterus. The risk factors were analyzed.
@*RESULTS@#Aberrant artery collaterals were found in thirty-four patients. According to multiple logistic regression analysis, the presence of placenta previa (RR=78.556, 95% CI: 2.869-2 150.651, P=0.010), pelvic inflammatory disease (RR=6.633, 95% CI: 1.595-27.592, P=0.009), pregnancy complications (RR=7.264, 95% CI: 1.622-32.531, P=0.010), abortions (RR=18.381, 95% CI: 1.683-200.752, P=0.017) and uterine fibroids or adenomyosis (RR=12.580, 95% CI: 1.004-157.550, P=0.050) were the factors for the presence of aberrant artery collaterals.
@*CONCLUSION@#Aberrant artery collaterals were more frequent in patients with pelvic inflammatory disease, pregnancy complications, abortions and uterine fibroids or adenomyosis.
